[0003] The invention relates to a compound of formula (I), for use in treating GM1 gangliosidosis. Specifically, the invention relates to a compound of formula (I) for use in reducing the accumulation of GM1 ganglioside in the brain and / or spinal cord of a subject, increasing neuronal survival in a subject, and / or preventing neuronal degeneration in a subject.
[0004] BACKGROUND OF THE INVENTION
[0005] GM1 gangliosidosis is an inherited lysosomal storage disorder that progressively destroys neurons in the brain and spinal cord. GM1 gangliosidosis is caused by genetic changes in the GLB1 gene and is inherited in an autosomal recessive manner. The GLB1 gene provides instructions for making p- galactosidase. This enzyme is found in lysosomes, p-galactosidase helps break down several molecules, including GM1 ganglioside. GM1 ganglioside is important for normal functioning of neurons in the brain.
[0006] The GLB1 gene variants produce versions of p-galactosidase that are not as effective at breaking down GM1 ganglioside as the normal version of the enzyme. Without enough functional p- galactosidase, GM1 ganglioside cannot be broken down when it is no longer needed. This substance builds up to toxic levels in many tissues and organs, particularly in the brain. Damage caused by the buildup of GM1 ganglioside leads to the destruction of neurons, causing many of the signs and symptoms of GM1 gangliosidosis.
[0007] The severity of symptoms and the time at which they first present can vary greatly in GM1 gangliosidosis. In general, an earlier manifestation of symptoms, as occurs in infantile or type I GM1 gangliosidosis, is often part of a more severe and rapidly progressive disease. Signs and symptoms children with GM1 gangliosidosis may experience include distinct facial features (coarse facial features), poor muscle tone (hypotonia), enlargement of the liver and spleen (hepatosplenomegaly), exaggerated startle reaction, developmental regression, skeletal abnormalities, dysmorphia, organomegaly, cardiomyopathy, interstitial lung disease, seizures, ataxia, dysarthria and visual impairment. No treatment for GM1 gangliosidosis currently exists. Thus, a need exists for a therapy for GM1 gangliosidosis.
[0008] WO2015 / 147639 A1 describes a wide range of derivatives of deoxynojirimycin, including a compound of a formula (referred to as compound of formula (I) herein):
[0009] Compound of formula (I) is a potent dual inhibitor of glucosylceramide synthase and non-lysosomal glucosylceramidase (GBA2, UniProt code: Q9HCG7). This compound is known under the commercial name “nizubaglustat”.

[0011] SUMMARY OF THE INVENTION
[0012] The inventors have tested the compound of formula (I) (i.e. nizubaglustat) in the human induced pluripotent stem cell model of GM1 gangliosidosis and found that this compound is able to reduce GM1 ganglioside accumulation in GLB1 defective cerebral organoids in a dose-dependent fashion and concomitantly increased neuronal survival. The inventors have further tested the compound of formula (I) (i.e. nizubaglustat) in animal (mouse) model of GM1 gangliosidosis (strain: B6.129P2(FVB)-Glb1,m1Adz / J) and found that the compound is suitable for the treating GM1 gangliosidosis and / or the symptoms associated with GM1 gangliosidosis. The compound also shows no toxicity for the human brain cells.

Human iPCs cells G22 were transduced using a 4D-Nucleofector with a plasmid expressing CRISPR / Cas9 and a custom-designed guide RNA targeting exon 2 in the GLB1 locus. Single clones were generated and screened for the presence of homozygotic frameshift in GLB1 coding sequence that would result in knockout, confirmed by absence of p-Galactosidase activity. The clone Ex2-3 was selected, expanded, and used to generate brain organoids containing microglia. [0137] As visualized in Figure 1 , human iPCs cells G22 (Shaker et al. 2022) were transduced using a 4D-Nucleofector (Lonza) with a plasmid expressing CRISPR / Cas9 and a custom-designed guide RNA targeting exon 2 in the GLB1 locus (#62988, Addgene) as described in (Shaker et al. 2021 ). Single clones were generated and screened for the presence of homozygotic frameshift in GLB1 coding sequence that would result in knockout, confirmed by absence of p-Galactosidase activity. The clone Ex2-3 was selected, expanded, and used to generate brain organoids containing microglia as described in (Aguado et al., 2023). After 45 days of maturation, WT and KO brain organoids were exposed to vehicle, 5nM or 500nM of nizubaglustat for 14, 18, 20 and 24 weeks and processed for immunostaining. GM1 accumulation was measured using a cholera toxin subunit-B FITC conjugate (C1655, Sigma)

[0139] To evaluate the efficacy of nizubaglustat, a novel and potent dual NLGase (GBA2) and GCS inhibitor currently in clinical development, and to create a tool for mechanistic studies, a human brain organoid model of GM1 gangliosidosis has been developed. Using CRISPR-Cas9, a bi- allelic 1 base pair (bp) frameshift deletion in Exon 2 of the GLB1 gene in healthy hESC was created to result in a non-functional 3-galactosidase enzyme (Figure 2A). Due to the 1 bp deletion in clone 2.3, the elastin binding protein (EBP) will also be out of frame from amino acid 25 which would predict that this protein is either non-functional and / or unstable at the mRNA or protein level.
[0140] As visualized in Figure 2B, B-galactosidase (3-gal.) staining of the clonally derived Ex 2-3 GLB1 null line confirmed the absence of 3-gal. enzyme activity. The 3-gal. enzyme converts an exogenous substrate (chromogen) to a blue color when the enzyme is present. In Figure 2B, top left, the blue 3-gal. staining is present in the organoid tissue slices, confirming the activity of the enzyme. In contrast, the lack of substrate conversion correlates to a less blue color, or 3-gal enzyme present. The low levels of blue seen in the Ex 2-3 GLB1 null-line represent non-specific, background levels (Figure 2B, top right). Figure 2B bottom figures show the same effect, but for exemplary whole organoids (bottom, left, G22 control organoids and bottom, right, the EX 2-3 GLB1 null line).
[0141] Figure 2C illustrates the neuronal phenotype of the cells (Neun) as well as the presence of microglia (Iba1 ) and astrocytes (GFAP). Neun is associated with neuronal differentiation and a common marker used to indicate a neuronal phenotype that is present throughout the whole cell life. Iba1 (Ionized calcium-binding adaptor molecule 1 ) is a pan microglial marker and it’s expression increases with microglial activation. Glial fibrillary acidic protein (GFAP), is expressed by numerous cell types, including astrocytes. [0158] In GM1 gangliosidosis, GM1 accumulates in the lysosome. This is due to a primary deficiency of the degrading enzyme 3-GAL, encoded by the GLB1 gene.
[0159] The production of GM1 depends on the upstream formation of GlcCer within the Golgi. AZ-3102 (also termed nizubaglustat) is a potent and selective inhibitor of both GCS and GbA2.
[0160] Nizubaglustat’s beneficial effects are agnostic to the aetiology of gangliosidosis, as demonstrated in a mouse model of GM1 gangliosidosis.
[0161] Nizubaglustat was tested in a mouse model of GM1 gangliosidosis to determine its effect on extending the survival of diseased animals. The study was performed under the guidance of Dr. Jagdeep Walia, MD, MBBS, FRCCP, FCCMG, at the Kingston Health Sciences Centre and Queen’s University Kingston, ON, Canada. Briefly, 3-galactosidase deficient mice (fi-gal- / -), created via targeted disruption of the murine fi-gal locus by homologous recombination (Hahn, del Pilar Martin et al. 1997), were administered vehicle (acidified water) or 3 mg / kg / day of nizubaglustat. Mice were randomly allocated to treatment groups and each cohort had male and female animals in equal proportions (N=6 mice). With the exception of the technician responsible for making formulations, all other staff, were blinded to the treatments. Drug and / or vehicle were administered by oral gavage once a day, between 8.00 - 12.00 am. Animals were dosed at 10 mL / kg from post-natal day 30 until they reached their humane endpoints (N=6 / dose group). Animals were preventively euthanized for reducing distress upon occurrence of immobility, tremors or seizures, poor body condition, inability to perform righting reflex, 15% weight loss, and moribundity.
[0162] Upon observing the final death in one treatment group, the study was unblinded to the PI and Sponsor to preserve blinding of ongoing cohorts for behavioral, PK, and further dose explorations. At the time of this submission, all animals in the nizubaglustat treatment group (Figure 6) presented are alive and will not be prematurely sacrificed. [0166] Nizubaglustat’s beneficial effects in a mouse model of GM1 gangliosidosis were tested as in Example 2, but at doses 0.2, 0.5, 3, 6 mg / kg once per day from 4-weeks of age until mice reached their humane endpoints. All dosing groups were blinded to animal care staff and the Pls of the study. Mice were randomly allocated to treatment groups and each cohort had male and female animals in equal proportions (N=7-14 mice per group). Behavioural testing included Rotarod and Open Field tests.
[0167] All animals in the nizubaglustat treatment group (Figure 8) had a significant increase in survival versus vehicle treated GM1 mice. Vehicle treated animals reached their human endpoint with a median survival of 170 days versus 263 days for nizubaglustat treated animals (p<0.0001 ). Doses as low as 0.2 mg / kg / day also conferred a significant increase in survival (p=0.0328), Log-rank [Mantel- Cox] test). Statistical evaluation was performed with GraphPad Prism (10.2.2). Doses at 3 and 6 mg / kg / day did not result in statistically different probability of survival. Rotarod data shows significant, time-associated differences in nizubaglustat treated mice as compared to controls indicating improved neuromotor function (Figure 10). Figure 9 illustrates results of Open Field test (OFT).

[0170] Blood sampling:
[0171] At the designated time points, animals had a terminal blood draw via the descending abdominal vena cava including baseline. Plasma was collected and assayed for baseline measurements, nizubaglustat PK and PD levels and biomarkers at week 24. Samples were collected at the following time points, 5 min and 1 , 2, 4, 8, and 24 h post-dose. Blood was collected by cardiac puncture with a 23-gauge needle. The needle was removed, and the blood was transferred to the sample tube (MiniCollect® K2EDTA (potassium ethylenediaminetetraacetic acid). The tube was inverted thoroughly to facilitate homogeneous distribution of the EDTA and prevent clotting. The blood samples were centrifuged at 3000 x g for 10 minutes at room temperature (22°C). 50 pL plasma aliquots were transferred to pre-labeled 1 .5ml LoBind Eppendorf tubes, frozen on dry ice and stored at -80°C.
[0172] Brain Sampling and Tissue preparation
[0173] After perfusion with normal saline, the skull was opened, and the brain was removed carefully and hemisected on a cooled surface. The left hemisphere was weighed, snap frozen on dry ice and stored at -80 °C. Mouse right hemi-brains were fixed by immersion in freshly prepared 4% paraformaldehyde in PB (pH 7.4) for two hours at room temperature. Rostral (RB), mid (MB) and caudal (CB) brain sections were obtained. Afterwards, hemispheres were transferred to 15 % sucrose / PBS and stored at 4 °C to ensure cryoprotection. The CB sections were analyzed for PK and PD.
[0174] Bioanalysis of Formulations, Nizubaglustat and Biomarkers:
[0175] Formulations, plasma, brain tissues were analyzed for the concentration of nizubaglustat using a reversed-phase LC-MS / MS method. Mouse plasma and brain tissue were used as species matrix. The LLOQ for nizubaglustat was 0.5 ng / mL plasma, 0.5 ng / g brain tissue. The LLOQ for GlcCer C16:0 and C18:0 was 10 pmol and 25 pmol / g tissue, respectively.

[0177] The survival benefit was assessed by Kaplan-Meier survival analysis followed by log-rank (Mantle- Cox) test. Behavioral test used ordinary one-way ANOVA with Dunnet’s multiple comparison test (GraphPad Prism (version 10.4.1 )). Group comparison performed using three-way ANOVA Tukey’s comparison test (GraphPad Prism).

[0179] Motor functions were evaluated using established methods in the mouse: accelerated rotarod performance test for sensorimotor coordination (RR). Testing occurred after daily gavage in a separate testing room. The animals were brought to the testing room 30-60 minutes prior to the testing procedures to allow for acclimatization. RR assessments at predefined intervals until the humane endpoints were reached. Each mouse was tested a minimum of 3 times in the RR. In the RR, all animals were habituated to the device for 5 min using a continuous rotation speed of 4 - 40 rotations per minute (rpm) for the first day. For the subsequent experiments, no prior habituation was conducted. Time was measured until the animals fell from the rod. Each mouse was allowed three trials on the RR with at least 10 minutes for rest in-between assessments. The highest number achieved was recorded for each of the following parameters: latency to fall, end rotations per minute and distance travelled.

[0181] Figure 11 shows the mean nizubaglustat plasma and brain concentrations (at 24 weeks) vs time. Nizubaglustat shows sustained concentrations in brain and increased BBB permeability in diseased animals versus normal (Het) animals.

CLAIMS1. A compound of formula (I),for use in treating GM1 gangliosidosis in a subject.
2. A compound of formula (I),for use in reducing the accumulation of GM1 ganglioside in the brain and / or spinal cord of a subject, preferably wherein the subject suffers from GM1 gangliosidosis.
3. A compound of formula (I),for use in increasing neuronal survival in a subject, preferably wherein the subject suffers from GM1 gangliosidosis.
4. A compound of formula (I),for use in preventing neuronal degeneration in a subject, preferably wherein the subject suffers fromGM1 gangliosidosis.for use in increasing survival of a subject, preferably wherein the subject suffers from GM1 gangliosidosis.
6. The compound for use of any one of claims 1 -5, wherein the compound is in a crystalline form.
7. The compound for use of claim 6, wherein the crystalline form is a crystalline free base.
8. The compound for use of claim 6 or claim 7, wherein the crystalline form displays a reflection, stated as a 20 value, at 17.8 ± 0.2°, in an X-ray powder diffraction pattern, optionally wherein the reflection at 17.8 ± 0.2° is one of the four strongest reflections in the X-ray powder diffraction pattern.
9. The compound for use of claim 8, further displaying one or more reflections, stated as a 20 value, at one or more of 4.1 ± 0.2°, 8.3 ± 0.2°, 12.4 ± 0.2°, 13.6 ± 0.2°, 14.5 ± 0.2°, 14.9 ± 0.2°, 15.2 ± 0.2°, 17.2 ± 0.2°, 19.3 ± 0.2°, 21 .2 ± 0.2°, 22.4 ± 0.2°, 22.9 ± 0.2° and 23.3 ± 0.2°, in an X-ray powder diffraction pattern.
10. The compound for use of any one of claims 1 -9, wherein the compound is administered to the subject at a dosage ranging from about 0.1 mg total / day to about 15 mg total / day.11 . The compound for use of any one of claims 1 -10, wherein the compound is administered to the subject daily.
12. The compound for use of any one of claims 1 -11 , wherein the subject:(a) exhibits one or more symptoms of GM1 gangliosidosis;(b) does not exhibit any symptoms of GM1 gangliosidosis;(c) has been diagnosed with GM1 gangliosidosis; or(d) has not been diagnosed with GM1 gangliosidosis.
13. The compound for use of any one of claims 1 -12, wherein the subject is human, preferably the subject is between 28 days and 30 years of age.
14. The compound for use of any one of claims 1 -13, wherein the compound is administered to the subject in a pharmaceutical composition which has a pH of between 4.0 and 7.5.
15. The compound for use of any one of claims 1 -14, wherein the compound exhibits a substantial lack of brain toxicity in the subject.
16. The compound for use of claim 6 or claim 7, wherein the crystalline form displays a reflection, stated as a 20 value, at 17.2 ± 0.2°, 17.8 ± 0.2°, 21 .2 ± 0.2° and / or 22.4 ± 0.2°, in an X-ray powder diffraction pattern.
17. The compound for use of any one of claims 1 -16, wherein the compound is administered to the subject at a dosage of between about 3 mg total / day and about 9 mg total / day.
18. The compound for use of any one of claims 1 -17, wherein the compound is administered to the subject at a dosage of 9 mg total / day.
