Description

Application of Jiawei Fengfang Pills in the preparation of medicines for treating lymphadenitis

[0001] This application claims priority to the Chinese patent application filed with the China Patent Office on July 20, 2023, with application number CN202310900502.9 and invention name “Application of Jiawei Fengfang Pills in the Preparation of Drugs for the Treatment of Lymphadenitis”, the entire contents of which are incorporated by reference into this application. Technical Field

[0002] The present application relates to the technical field of traditional Chinese medicine preparations, and in particular to the use of modified Fengfang pills in the preparation of medicines for treating lymphadenitis. Background Art

[0003] Lymphadenitis is relatively common in clinical practice and is divided into acute and chronic lymphadenitis. There are currently no large-scale statistical figures on the incidence rate for reference. According to the number of people visiting the department, it accounts for about 10%. The cause and pathogenesis are still difficult problems in the medical community. Some scholars believe that it is related to viral infection and autoimmunity.

[0004] Lymphadenitis is a self-limiting disease with a generally good prognosis. However, chronic lymphadenitis often recurs repeatedly and, if left untreated, can easily develop into histiocytic necrotizing lymphadenitis. Surgeons typically use antibiotics or surgery to treat acute lymphadenitis. However, antibiotic treatment can easily lead to drug resistance, which can affect the effectiveness of treatment, while surgical treatment can cause damage to the patient's body. Furthermore, most patients with chronic lymphadenitis do not seek medical attention or are unaware of available treatment options, preferring to wait for spontaneous recovery. A small number of patients experience repeated attacks or develop histiocytic necrotizing lymphadenitis, making treatment more difficult.

[0005] Traditional Chinese medicine preparations are currently an ideal treatment method due to their advantages of minimal toxic side effects, lack of drug resistance, and ability to avoid damage to the patient's body caused by surgery. However, there are currently no traditional Chinese medicine preparations that have good therapeutic effects on both acute and chronic lymphadenitis.

[0056] The present application provides the use of a traditional Chinese medicine composition in the preparation of a drug for treating lymphadenitis. The traditional Chinese medicine composition comprises the following components in parts by weight: 10 to 30 parts of nectarine nest, 5 to 27 parts of Sophora flavescens, 5 to 27 parts of Scutellaria barbata, 5 to 27 parts of Trillium, 5 to 15 parts of antler frost, 5 to 27 parts of Pulsatilla scabra, 5 to 15 parts of bitter liquorice, 5 to 15 parts of liquorice, 5 to 15 parts of bupleurum, 5 to 12 parts of green peel and 0.378 to 2 parts of musk.

[0057] Unless otherwise specified, this application has no special requirements on the sources of the components of the traditional Chinese medicine composition, and commercially available products familiar to those skilled in the art can be used.

[0058] The Chinese medicine composition described in the present application includes 10 to 30 parts by weight, preferably 20 to 30 parts, and more preferably 30 parts. The honeycomb described in the present application has a slightly sweet and slightly salty taste, is neutral in nature, and enters the liver and stomach meridians. It is an excellent medicine for attacking toxins, treating sores, relieving swelling and relieving pain. The stomach meridian passes through the neck and can treat some diseases of the neck, such as lymphadenitis, lymph node tuberculosis, sore throat, etc.

[0059] Based on the mass of the honeycomb, the traditional Chinese medicine composition described in the present application includes 5 to 15 parts of Bupleurum, preferably 10 to 15 parts, and more preferably 15 parts.

[0060] Based on the mass of the honeycomb, the traditional Chinese medicine composition described in the present application includes 5 to 12 parts of green peel, preferably 10 to 15 parts, and more preferably 12 parts.

[0061] One of the common dialectical types of lymphadenitis is liver and stomach fire poison, which is caused by liver qi stagnation, which turns into fire over time, and the liquid becomes phlegm. The phlegm and fire rise and gather in the neck. Therefore, the treatment must be effective with products that can enter the liver to manage qi and relieve depression, and can also eliminate blood stasis, disperse knots, remove dampness and raise clearness. Bupleurum and Citrus aurantium both have the function of soothing the liver and regulating qi. Bupleurum has a rich aroma, which rises lightly and clearly, promotes the flow of qi, regulates liver qi, and is good at soothing the depression in the upper burner. Citrus aurantium tastes pungent and warm, and its qi is strong, sinks downward, breaks up the qi stagnation in the liver meridian, and is good at soothing the depression in the lower burner. The two are used together, and they rise and fall appropriately, and can communicate up and down. Qi stagnation can be relieved, qi stagnation can be relieved, and qi stagnation can be dispersed. The medicinal power is stronger than that of ordinary liver-soothing and qi-regulating agents. Bupleurum and Citrus aurantium are both ministers.

[0062] Based on the mass percentage of the honeycomb, the Chinese medicine composition described in the present application includes 5 to 27 parts of Trillium gracile, preferably 18 to 27 parts, and more preferably 27 parts. Acute lymphadenitis often presents with redness, swelling, heat, and pain, and can be treated with heat-clearing and detoxifying drugs. Trillium gracile is sweet, slightly pungent, cold, and slightly toxic. It has the effects of clearing heat and detoxifying, reducing swelling and dispersing nodules, and can be used as an adjuvant for carbuncle, furuncle, and lymph node tuberculosis.

[0063] Based on the mass of the honeycomb, the traditional Chinese medicine composition of the present application includes 5 to 27 parts of Sophora flavescens, preferably 18 to 27 parts, and more preferably 27 parts. Sophora flavescens has the effects of clearing away heat and detoxifying, reducing swelling and relieving sore throat, and is mainly used to treat symptoms such as fire toxicity, sore throat, sore throat, sore gums, and mouth sores. Its effect is similar to that of Trillium gracile and is an adjuvant.

[0064] Based on the mass of the honeycomb, the Chinese medicine composition described in the present application includes 5 to 27 parts of Scutellaria barbata, preferably 18 to 27 parts, and more preferably 27 parts. Traditional Chinese medicine believes that blood stasis is caused by water, and water-dampness and blood stasis often affect each other. Therefore, proper blood circulation is also required in treatment. Scutellaria barbata is pungent, bitter, and cold, and enters the lung, liver, and kidney meridians. Scutellaria barbata can clear away heat and toxins, disperse blood stasis and stop bleeding, and also promote diuresis and reduce swelling, making it an adjuvant.

[0065] Based on the mass of the honeycomb, the Chinese medicine composition described in this application includes 5 to 15 parts of antler frost, preferably 10 to 15 parts, and more preferably 15 parts. Water and blood stasis, though different in form but the same in origin, are both yin evils caused by sluggish yang qi. During treatment, attention should always be paid to the body's vital energy and its circulation. Deer antler frost is a yang-tonifying herb that can warm the kidneys and boost yang, increasing the body's vital energy. It has a salty and warm nature. Salt can soften and disperse lumps, while warmth can dredge the meridians and reduce swelling, making it an adjuvant.

[0066] Based on the mass parts of the honeycomb, the traditional Chinese medicine composition of the present application includes 5 to 27 parts of Pulsatilla chinensis, preferably 18 to 27 parts, and more preferably 27 parts. For the condition of liver qi stagnation, which turns into fire over time and turns liquid into phlegm, it is necessary to use drugs that can resolve phlegm and dissipate stagnation. Pulsatilla chinensis has the effects of clearing away heat and detoxifying, resolving phlegm and dissipating stagnation, and eliminating carbuncle and dissipating stagnation. It is mainly used to treat sores, carbuncles, swellings and toxins, and can be used as an adjuvant in the acute attack period of lymphadenitis.

[0067] Based on the mass of the honeycomb, the traditional Chinese medicine composition of the present application includes 0.378 to 2 parts of musk, preferably 0.378 to 1 part, and more preferably 0.378 parts. Musk is an excellent product for promoting blood circulation, relieving pain, and treating various symptoms of blood stasis, whether new or long-standing. A small amount of musk is used to promote blood circulation and relieving pain, and is used as an adjuvant.

[0068] Based on the mass of the honeycomb, the Chinese medicine composition of the present application includes 5 to 15 parts of bitter liquorice, preferably 10 to 15 parts, and more preferably 15 parts. Bitter liquorice is bitter in taste and cold in nature. It has the effects of clearing away heat and detoxifying, drying dampness and killing insects. It is used externally for sores, boils, swelling and pain as a guiding drug.

[0069] Based on the mass of the honeycomb, the Chinese medicine composition of the present application includes 5 to 15 parts of licorice, preferably 10 to 15 parts, and more preferably 15 parts. Licorice harmonizes various medicines and serves as an adjuvant.

[0070] The acute stage of lymphadenitis is often diagnosed as wind-heat phlegm toxin syndrome and heat toxicity syndrome, and the chronic stage is often diagnosed as liver depression phlegm fire syndrome and yang deficiency phlegm coagulation syndrome. The treatment principle for cervical lymphadenitis in both acute and chronic stages is to give priority to "elimination method". This application uses beehive as the main medicine, bupleurum and green peel as the ministerial medicines, trillium, sophora flavescens, whiteheada, bark lotus, musk and antler powder as the adjuvant medicines, and liquorice and bitter liquorice as the guiding medicines. The combination of these medicines has a unique clinical effect. It has been used for more than 20 years and can cure chronic recurrent lymphadenitis once and for all. It is safe and has no toxic side effects. The total clinical effectiveness rate can reach 75% and the cure rate can reach 50%.

[0071] In the present application, the dosage form of the drug preferably includes pills, capsules, oral liquids, tablets or powders. [0113] The diagnostic criteria for lymphadenitis in traditional Chinese and Western medicine are:

[0114] 1. Traditional Chinese Medicine Diagnostic Criteria

[0115] Wind-heat evil toxin type: submandibular and cervical lymph nodes are swollen, with obvious tenderness and can be moved when pushed. The skin color does not change. There is fever, headache, soreness in the limbs, pain in the throat, etc. The tongue coating is thin and yellow, and the pulse is floating and rapid.

[0116] Phlegm-fire internal knot type: The lymph nodes are obviously enlarged, hard in texture, and painful when pressed; then the lumps become fixed and cannot be moved, and there is a sense of fluctuation when pressed, accompanied by high fever, thirst, poor appetite, constipation, red tongue, yellow and greasy tongue coating, and rapid and strong pulse.

[0117] 2. Western medicine diagnostic standards

[0118] Acute lymphadenitis: It is mostly caused by sudden bacterial infection, resulting in local redness and swelling of the human body. It is characterized by an acute condition. Most patients experience swelling and pain in the affected area, accompanied by decreased appetite, fatigue, headache, etc. A small number of patients also have fever, nausea and vomiting and other discomfort reactions, and there is also swelling in the lymph area, which causes pain when pressed.

[0119] Chronic lymphadenitis: caused by chronic inflammation of adjacent organs, symptoms such as repeated pain and swelling of lymph nodes may occur. The disease period is longer, and the symptoms are not as obvious as acute lymphadenitis. However, the swollen lymph nodes are harder and the pain is not obvious when pressed. Subsequently, the lymph nodes gradually become smaller or even disappear.

[0120] Nonspecific lymphadenitis: Symptoms are usually mild, with only mild lymphadenopathy and pain. Severe cases may include localized redness, warmth, and tenderness. In severe cases, the lymph nodes may continue to enlarge, even fusing into clumps. Abscesses may form, fluctuating, and even rupture and discharge pus. Nonspecific lymphadenitis is localized lymphadenopathy caused by common bacterial or viral infections.

[0121] Specific lymphadenitis: The patient may not experience significant pain, but the lymph nodes continue to enlarge, often accompanied by fever, fatigue, mild local pain, and even rupture and pus discharge from the lymph nodes. Specific lymphadenitis is often associated with fungal infections, tuberculosis infections, and other conditions.

[0122] Diagnostic classification:

[0123] Traditional Chinese Medicine classification: (1) Wind-heat evil toxicity type, (2) Phlegm-fire internal accumulation type.

[0124] Western medicine classification is based on four diagnostic criteria and a comprehensive judgment of the classification. For example, if acute lymphadenitis is met, it is also necessary to determine whether it is acute specific or acute nonspecific based on the specific and nonspecific diagnostic criteria, and then finally determine the diagnostic classification. The specific classifications are as follows: (1) acute specific lymphadenitis, (2) acute nonspecific lymphadenitis, (3) chronic specific lymphadenitis, and (4) chronic nonspecific lymphadenitis.

[0125] Efficacy evaluation criteria: (1) Cured: lymph node swelling and pain and systemic symptoms disappear, and blood routine is normal; (2) Effective: lymph node swelling and pain improve, systemic symptoms gradually disappear, and blood routine is normal; (3) Ineffective: local and systemic symptoms worsen, even abscesses form, and white blood cell count in blood routine increases.

[0126] Directions:

[0127] Take two pills twice a day, in the morning and evening, two pills each time. One course of treatment is 30 days.

[0128] The total number of cases taking the modified Fengfang Pills prepared in different embodiments and the results of efficacy evaluation after taking them for 1 to 6 courses of treatment are shown in Table 1, wherein, effective rate = number of effective cases / total number of cases × 100%; cure rate = number of cured cases / total number of cases × 100%; ineffective rate = number of ineffective cases / total number of cases × 100%; total effective rate = (number of effective cases + number of cured cases) / total number of cases × 100%.

[0129] Table 1 Clinical effects of patients with lymphadenitis after taking the Chinese medicine composition of the present application

[0130] Note: The total number of cases in Table 1 is the sum of the number of cases of wind-heat evil toxin type and the number of cases of phlegm-fire internal knot type, or the sum of the number of cases of acute nonspecific lymphadenitis, chronic nonspecific lymphadenitis, acute specific lymphadenitis and chronic specific lymphadenitis. It is the total number of cases of two different types of classifications in traditional Chinese medicine and Western medicine.

[0131] As can be seen from Table 1, the modified Fengfang pills prepared in the present application have a good therapeutic effect on lymphadenitis.

[0132] Typical Cases

[0133] The following typical cases all use Jiawei Fengfang Pills in the following ways: take them twice a day, once in the morning and once in the evening, two pills at a time, and 30 days constitutes a course of treatment.

[0134] Case 1: Zeng, a 28-year-old male, was first admitted to the hospital on May 12, 2009, with fever and weight loss for over a month. A month prior, the patient had persistent fever without a clear cause, accompanied by multiple swollen and fused lymph nodes in the neck, each about the size of a soybean and markedly tender. His temperature rose daily in the afternoon, peaking at 40.3°C. He also experienced significant fatigue, decreased appetite, and weight loss. Lymphoma, tuberculosis, and other common infections were ruled out. A cervical lymph node biopsy revealed pathology suggesting chronic nonspecific lymphadenitis. This condition falls within the treatment scope of this application. After two courses of Jiawei Fengfang Pills prepared in Example 4, the patient's temperature returned to a range of 36.4-37.2°C, and the size of the swollen lymph nodes decreased to approximately one-quarter of their original size. At a follow-up visit on September 23, 2009, he continued taking the pills for two more courses. Six months of follow-up revealed that the patient's lymphadenopathy had completely resolved, with no significant discomfort.

[0135] Case 2: Wei, male, 26 years old, was first diagnosed on December 11, 2010. He was admitted to the hospital for fever, fatigue, and swollen cervical lymph nodes for more than half a month. The patient had a fever half a month ago, with the highest temperature reaching 39.6°C. The cervical lymph nodes were swollen as the size of an egg. The pathology of the external hospital considered tuberculosis and was to be given anti-tuberculosis treatment. The patient came to the Affiliated Hospital of Inner Mongolia Medical University for a further lymph node biopsy and was diagnosed with chronic specific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 4 of the present application for two courses of treatment, the cervical lymph nodes were shrunk to the size of soybeans, the fever was alleviated, and the body temperature was 37.0°C. After continuing to take the medicine for 2 months, the swelling disappeared completely, and no fever symptoms reappeared. There was no recurrence during the 1-year follow-up.

[0136] Case 3: Xi, a 20-year-old female, presented to the doctor on March 5, 2011, with cervical lymphadenopathy for over six months. She was diagnosed with chronic lymphadenitis. Initially, she was treated with Western anti-tuberculosis drugs and antibiotics for over four months without success. She then continued with a combination of traditional Chinese medicine for softening, dispersing, and eliminating scrofula for over two months, still without success. A careful examination revealed 18 cervical lymphadenopathy, ranging from small, bean-sized to large, apricot-sized. These 18 nodes were not red or painful. Her tongue was coated with a white coating, and her pulse was deep and stringy. Based on her pulse and symptoms, she concluded that a deep pulse indicates qi stagnation; a stringy pulse indicates liver pulse; and a tuberculosis with unchanged skin color indicates phlegm nodules. After one course of treatment with the modified Fengfang Pills prepared in Example 5 of this application, her cervical lymphadenopathy improved compared to before. After one month of treatment, the swelling had significantly shrunk, and the pain had eased. After one month of consolidation medication, the swelling and pain had disappeared.

[0137] Case 4: Xu, a 52-year-old male, was first diagnosed on March 23, 2011. He had a low-grade fever for 10 days, peaking at approximately 38.8°C. He also had a mild cough, no sputum, and chills, but no night sweats, headaches, or sore throat. He experienced mild lower back pain during fever and a palpable 1x2 cm lymph node on the right side of his neck. He visited a local clinic, but no examination was performed. Oral cold medication and cephalosporins were ineffective, so he sought treatment at the Affiliated Hospital of Inner Mongolia Medical University. Physical examination revealed several tender, palpable lymph nodes on the right side of his neck. Ultrasound scan of the lateral neck revealed multiple, enlarged lymph nodes with thickened cortex and presence of lymphatic hilum structures. A lymph node biopsy revealed chronic specific lymphadenitis. The doctor prescribed Jiawei Fengfang Pills, prepared in Example 5 of this application. After taking the pills for one month, his symptoms were significantly alleviated, and an examination revealed a significant reduction in the enlargement. After another course of treatment, his symptoms resolved, with no significant discomfort. Follow-up for six months revealed no recurrence.

[0138] Case 5: Fu, a 36-year-old female, was admitted to the hospital on March 6, 2011, due to chills and fever for half a month. Physical examination showed a temperature of 38.0°C. Groups of enlarged, soybean-sized lymph nodes were palpable in the right submandibular area and bilaterally in the neck. These nodes were firm, tender, and movable. The surface skin was not red and warm. There were no abnormalities in the heart, lungs, and abdomen, and no redness or swelling in the joints of the limbs. Renal function, biochemistry, and blood lipids were normal. Chest anteroposterior and lateral radiographs and cervical and lumbar spine radiographs were normal. Chest CT scans showed minimal bilateral pleural effusions. Ultrasound of the liver, gallbladder, and pancreas showed no abnormalities. The spleen was approximately 36 cm thick and 114 cm long, and enlarged. The electrocardiogram was normal. Color Doppler ultrasound of both necks revealed multiple enlarged lymph nodes. On the fourth day of hospitalization, a biopsy of the right cervical lymph nodes was performed, and pathological examination revealed lymphadenitis. The patient was clinically diagnosed with acute nonspecific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 6 of the present application for one course of treatment, the lymphadenopathy symptoms were alleviated. The patient continued to take the medicine for one course of treatment and showed no obvious discomfort during the six-month follow-up.

[0139] Case 6: Zhang, female, 11 years old, first diagnosed on May 4, 2011. The patient found a tumor in the left submandibular area for one week. Physical examination: The maxillofacial area was basically symmetrical on both sides, with no restriction in opening the mouth, and the opening range was 92.5px. A mid-substance tumor with a diameter of about 100px, a round shape, and mild tenderness could be felt in the left submandibular area. No tumor was felt inside the mouth. Imaging findings: an oval-shaped low-density mass on the surface and subcutaneous surface of the left submandibular gland, with uneven density, mostly low density, thickening of the adjacent skin and subcutaneous fascia, slight enlargement of the submandibular gland, annular enhancement after enhancement, uneven thickness of the annular wall, and an uneven inner wall. Multiple adjacent submandibular lymph nodes were enlarged and enhanced. No obvious damage was found in the adjacent bone. Pathological results: acute specific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 3 of the present application for one course of treatment, the tumor and symptoms disappeared.

[0140] Case 7: Ye, male, 44 years old, first visited the hospital on June 12, 2011, with the main complaint of "intermittent hematuria for 2 years, lower abdominal soreness and pain, fatigue, edema below the knees, and frequent urination at night for 3 months". Physical examination showed a temperature of 36.5°C, 6 to 7 lumps with a diameter of about 1.5 to 2.0 cm were palpable under the chin and in the sternocleidomastoid process of the neck, i.e., the outer edge of the muscle and the supraclavicular fossa. The masses were hard and medium in texture, well-moved, and non-tender. Renal function was normal. A cervical lymph node biopsy confirmed chronic specific lymphadenitis. After taking the modified Fengfang pills prepared in Example 6 of the present application for 3 months, the patient no longer had soreness and pain, his urine output was normal, and he felt that his cervical lymph nodes had shrunk significantly, with no local compression symptoms. After a follow-up of 1 year, the patient's swelling disappeared and he had no obvious discomfort.

[0141] Case 8: Zhang, female, 55 years old, presented to the doctor on July 8, 2011 with the main complaint of "chronic lymphadenopathy for 5 years". She had no fever, neck pain, and obvious pain when turning her neck. Physical examination showed a temperature of 36.3°C, pulse of 76 beats / min, and blood pressure of 96 / 60 mmHg. 5-6 lumps with a diameter of about 1.5-2.0 cm were palpable behind the ears and on the sternocleidomastoid process of the neck, i.e., the outer edge of the muscle and the supraclavicular fossa. These lumps were firm, well-moved, and non-tender. Auscultation of the heart and lungs was normal, and her abdomen was soft. Other abnormalities were normal. She was diagnosed with chronic specific lymphadenitis. In view of the above experience, after taking the modified Fengfang Pills prepared in Example 6 of the present application for one course of treatment, her neck pain improved significantly. After taking the pills for another two weeks, she came to the doctor for examination. Physical examination showed that the cervical lymph nodes were palpable, 1.2-1.8 cm in size, and had significantly decreased in size. After two more months of treatment, the cervical lymph node masses completely disappeared. There was no recurrence during the three-month follow-up.

[0142] Case 9: Zhi, a 26-year-old male, presented to the Affiliated Hospital of Inner Mongolia Medical University on August 14, 2011. He had a history of skin infection and bilateral mandibular lymphadenopathy and pain. His condition improved after treatment. He subsequently developed recurrent, irregular mandibular lymphadenopathy. Examination revealed palpation of bilateral mandibular lymphadenopathy, measuring 5×2 cm. 2 The patient was diagnosed with acute nonspecific lymphadenitis. The patient was treated with the modified Fengfang Pills prepared in Example 6 of the present application for one course of treatment. The mandibular lymph node enlargement was significantly reduced, the texture became softer, and the pain was relieved. To consolidate the therapeutic effect, the patient was continued to take the medicine for one course of treatment. No recurrence was observed during the one-year follow-up.

[0143] Case 10: Ms. Li, a 12-year-old female student, came to our department for treatment on August 22, 2011. She had a history of chronic rhinitis and recurrent bilateral mandibular lymphadenopathy. Examination: Palpation revealed bilateral mandibular lymphadenopathy, with the right side being more affected. The lymphadenopathy varied in size, approximately 1 × 1 cm. 2 The patient was of similar size, firm texture, and tender. No abnormal changes were observed in the mouth. A cervical lymph node biopsy confirmed the diagnosis of chronic nonspecific lymphadenitis. After one course of treatment with the modified Fengfang Pills prepared in Example 4 of the present application, the swelling and pain disappeared and the disease returned to normal. No recurrence was observed during the one-year follow-up.

[0144] Case 11: Guo, a 22-year-old female, presented to the hospital on October 28, 2011. She reported suffering from cervical lymphadenopathy for four years. She had not received systematic examination or treatment since the onset of her illness. She experienced pain and swelling in the enlarged lymph nodes with emotional distress or fatigue, often accompanied by intermittent low-grade fever. She sought Traditional Chinese Medicine treatment at the Affiliated Hospital of Inner Mongolia Medical University. During the consultation, she presented with cervical lymphadenopathy that radiated to the shoulders, fatigue, weight loss, loss of appetite, restlessness, insomnia, a low-grade fever, and occasional fever in the palms and soles. Her tongue was pale with a thin white coating, and her pulse was deep and thready. Examination revealed two soybean-sized enlarged lymph nodes palpable in front of the sternocleidomastoid muscle on the left side of the neck; one broad bean-sized enlarged lymph node on the right side of the neck; and one date-pit-sized enlarged lymph node palpable in the right anterior hip fossa. All nodes were firm, tender, movable, and not adherent to the skin. She was diagnosed with chronic specific lymphadenitis. After taking the modified Fengfang pills prepared in Example 6 of the present application for 2 months, the patient's spirits improved, all the subjective symptoms were eliminated, all the swollen lymph nodes disappeared, and there was no recurrence during the 1-year follow-up.

[0145] Case 12: Yang, male, 36 years old, came to the clinic on January 24, 2012, after discovering a lump on the left side of his neck one month ago. The lump was initially the size of a broad bean, but gradually increased to the size of a ping-pong ball. He experienced occasional pain in the submandibular area, paroxysmal pain in the lower left posterior teeth, pain when biting, and a low-grade fever and pain in the lower left posterior teeth. Ultrasound examination showed acute nonspecific lymphadenitis in the left submandibular area. After taking the modified Fengfang Pills prepared in Example 6 of the present application for one course of treatment, the patient complained of significant improvement in pain and reduction in the lump. After another course of treatment, the lump and pain symptoms disappeared.

[0146] Case 13: Wang, male, 19 years old, first visit on February 4, 2012, with I degree enlargement of bilateral tonsils and a jujube-sized mass under the mandible, which was movable, hard, with clear boundaries and obvious tenderness. Three days later, the submandibular mass increased to the size of an egg. He had used erythromycin and sulfamethoxazole for anti-inflammatory treatment, but the effect was not obvious. A cervical lymph node biopsy was performed: it was diagnosed as acute specific lymphadenitis. After taking the modified Fengfang pills prepared by Example 6 of the present application for one course of treatment, the swelling and pain disappeared completely. After taking it for half a month, B-ultrasound showed that there was no abnormality in the cervical lymph nodes. The patient was advised to maintain a calm state of mind, exercise in moderation, eat a low-fat diet, and undergo regular checkups. A follow-up visit was conducted for half a year without recurrence.

[0147] Case 14: Mr. He, male, 33 years old, was admitted to the Department of Respiratory Medicine on February 4, 2021, due to "fever for 1 week". The patient reported that he had a fever after catching a cold a week ago, with the highest body temperature reaching 39°C. He had persistent fever in the morning and at night, aversion to cold, a little cough and sputum, coughing up white sputum, accompanied by neck pain, body aches, and fatigue. He had been treated in another hospital on January 30, 2012, and no obvious abnormalities were found in CRP, blood routine, and chest CT. He was given oral treatment of "oseltamivir 75mg bid and Lianhua Qingwen Capsules 4 tablets tid", but the symptoms did not improve significantly. On February 4, 2012, for further treatment, he visited the Department of Respiratory Medicine of the Affiliated Hospital of Inner Mongolia Medical University and was admitted to the hospital on the outpatient side with "fever to be investigated". Physical examination: The patient's body temperature was 38.2°C when he was admitted to the hospital. He was conscious but in poor spirits. There was no congestion in the pharynx, no enlarged tonsils, and no purulent secretions on the surface of the tonsils. A few enlarged lymph nodes the size of soybeans were touched on the left side of the neck. They were tender, soft, and had normal mobility. Lymph node color Doppler ultrasound and related examinations were performed, and it was considered to be acute nonspecific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 5 of the present application for one course of treatment, the enlarged lymph nodes shrank and the fever disappeared. After taking another course of treatment, the swelling completely disappeared and no fever symptoms reappeared.

[0148] Case 15: Fan, female, 14 years old, was admitted to the Affiliated Hospital of Inner Mongolia Medical University on June 29, 2012, mainly due to "a neck swelling found 8 days ago and a fever for 7 days". Before admission, the child's blood test showed no obvious abnormalities. The child was suspected of "lymphadenitis" and was treated with Chinese herbal decoctions and Chinese patent medicines. Fever appeared at night that day, with the highest temperature reaching 38.2°C. The fever gradually rose, with a fever peak every 4 hours. The cervical lymph nodes gradually swelled, with obvious tenderness, accompanied by itchy skin, but no rash or joint swelling and pain. On the day of admission, a follow-up neck color ultrasound showed "diffuse enlargement of the bilateral submandibular glands and multiple enlarged lymph nodes in the bilateral neck and submandibular areas". Considering the possibility of acute specific lymphadenitis, the patient's family was communicated with and, after the family agreed, a lymph node biopsy was performed on July 8 to confirm the diagnosis. After taking the modified Fengfang Pills prepared in Example 6 of the present application for one course of treatment, the swelling and pain symptoms improved and the fever disappeared.

[0149] Case 16: Mr. Liu, a 7-year-old male, first visited the hospital on July 5, 2012. He was 125 cm tall and weighed 31 kg. He presented to the Affiliated Hospital of Inner Mongolia Medical University with recurrent fever for a year, worsening for two days. In 2011, the child developed an unexplained fever, with body temperature fluctuating between 38 and 39°C. He took cephalosporins (the specific dosage is unknown) without significant effect. After transfer, he was diagnosed with Kawasaki disease and treated with dexamethasone. He was discharged after his fever resolved. Approximately two and a half months later, he developed a fever again and was given lansoprazole capsules 15 mg once daily. He was discharged after his fever resolved. He developed a fever again in January and April 2012, and was discharged after medical treatment. Two days prior (July 3, 2012), he developed a fever, accompanied by palpable lymphadenopathy. He sought treatment with traditional Chinese medicine and was admitted to the Affiliated Hospital of Inner Mongolia Medical University. Lymph node biopsy showed chronic specific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 6 of the present application for two courses of treatment, the fever was significantly improved. The patient continued to take the medicine for two courses of treatment at the follow-up visit. After one year of follow-up, the child's fever symptoms disappeared and there was no obvious discomfort.

[0150] Case 17: Zang, a 22-year-old male, was admitted to the Affiliated Hospital of Inner Mongolia Medical University on May 13, 2013, due to intermittent lymphadenopathy for three years and fever for 11 days. He reported a fever that had reached 40°C three years prior. He was admitted to the Pediatric Hematology Department of the First Affiliated Hospital of Harbin Medical University and presented with splenomegaly and multiple cervical, axillary, and intra-abdominal lymphadenopathy (specific details unknown). His specific diagnosis was unknown. During his hospitalization, he was treated with vitamin C, calcium supplements, and methylprednisolone (specific dosage and method unknown) for approximately two weeks. His condition improved significantly, his white blood cell count returned to normal, and his lymph nodes and spleen shrank to normal limits. He was discharged from the hospital with no discomfort and no follow-up examinations. Around April 2, 2013, after eating spicy food, the patient discovered that the right cervical lymph nodes were swollen. The lymph nodes were about the size of a quail egg, tender, and had poor mobility. The patient was treated at Shenzhen Traditional Chinese Medicine Hospital and given Chinese medicine treatment (specific details are unknown). After taking oral Chinese medicine for more than one month, the condition did not improve. For further treatment, the patient came to the Affiliated Hospital of Inner Mongolia Medical University. On May 16, the pathology report of the right cervical tumor was: it was considered to be chronic nonspecific lymphadenitis, and immunohistochemistry confirmed the diagnosis. After two courses of oral administration of the modified Fengfang Pills prepared in Example 6 of the present application, the swelling and pain were relieved, and a lymph node biopsy was normal. After two more courses of consolidation, the swelling and pain disappeared, and the patient was cured.

[0151] Case 18: Chen, a 48-year-old female, was admitted to the hospital on May 4, 2014, with a fever and neck swelling for one month, which worsened for five days. A month earlier, the patient had developed a fever without any cause, with a maximum temperature of 38.5°C, accompanied by chills, fatigue, sore throat, and muscle aches. A painless swelling was palpable on the left side of the neck. She sought medical attention at a local clinic for suspected infection and received intravenous acyclovir and amoxicillin-clavulanate potassium and symptomatic treatment for five days, with her temperature gradually returning to normal. Five days earlier, she developed a fever again, with a temperature as high as 39.0°C, accompanied by fatigue, chills, and night sweats, dizziness, headache, joint and muscle aches, and tongue ulcers. Simultaneously, the swollen lymph nodes on the left and right sides of the neck rapidly enlarged, leading to her being admitted to the Affiliated Hospital of Inner Mongolia Medical University. The patient was conscious. Multiple enlarged lymph nodes were found in both neck areas. The nodes were irregular in shape and had no ulceration on the surface. The largest node was about 3 cm × 3 cm in diameter. They were hard, with poor mobility and tenderness on palpation. Multiple enlarged lymph nodes were palpable in the axilla and groin areas. The nodes were hard, with poor mobility and tenderness. The pathological results of the left cervical lymph nodes showed acute nonspecific lymphadenitis. After one course of treatment with the modified Fengfang pills prepared in Example 5 of the present application, the symptoms were significantly improved. After two months of treatment, the fever disappeared and the swelling was reduced. After one month of consolidation medication, there was no obvious discomfort.

[0152] Case 19: Ms. Li, a 19-year-old unmarried female, was admitted to our department on May 18, 2016, with a 10-day history of cough, sputum, and fever. Ten days prior, the patient developed a low-grade fever after catching a cold. She had a fever approximately once daily, never exceeding 38.0°C. She received oseltamivir, an antiviral, and cefdinir, an antibacterial treatment, for three days, but the fever persisted, rising to 40.0°C. She subsequently sought treatment at the university hospital and received oral antiviral therapy (ribavirin) and Tylenol. While the temperature decreased to no more than 38.0°C, it continued to recur daily, often in the afternoon. On May 11, the patient was admitted to the Affiliated Hospital of Inner Mongolia Medical University. After admission, the patient underwent physical examination: body temperature: 37.8°C, pulse: 114 beats / min, respiration: 20 times / min, blood pressure: 110 / 70 mmHg, blood oxygen: 100%, pharyngeal congestion, tonsil II degree enlargement, a small amount of white exudate attached to the surface, no suppuration, soft neck, and multiple enlarged lymph nodes palpable in the left neck. The texture was soft, the range of motion was good, and there was mild tenderness. On May 29, the patient underwent cervical lymph node biopsy, and the pathological examination suggested acute specific lymphadenitis. After taking the modified Fengfang Pills prepared in Example 6 of this application for one course of treatment, the swelling and pain were relieved, and the body temperature returned to normal. After continuing to take it for one course of treatment, the fever symptoms did not recur, and the swelling and pain disappeared. After that, the patient was advised to undergo consolidation treatment for one month, and there was no recurrence after one year of follow-up.

[0153] Case 20: Mr. Lv, a 36-year-old male, accidentally discovered a 2-cm diameter, nontender, movable, round mass in his neck during an upper respiratory tract infection three months ago and sought treatment at a local hospital. Suspected of lymphadenopathy caused by the upper respiratory tract infection, he received anti-inflammatory and symptomatic treatment for five days. The upper respiratory tract infection resolved, but the neck mass persisted. Two months prior, the patient developed a feeling of pressure in his neck and difficulty breathing. Seeking further treatment, he was admitted to the Affiliated Hospital of Inner Mongolia Medical University on October 10, 2017. Two soybean-sized lymph nodes were palpable in his left submandibular area; they were nontender and well-mobile. He also had chronic pharyngeal congestion, with no bilateral tonsil enlargement. Scattered, hyperplastic lymphoid follicles were observed on the posterior pharyngeal wall. The neck was symmetrical, with no redness, swelling or ulceration of the skin. A mass with a diameter of 2 cm could be felt below the hyoid bone in the anterior midline of the neck. It had clear boundaries, a smooth surface, a cystic feel, no tenderness, and could move up and down with swallowing. No cord-like protrusions were felt connected to its upper end. A lymph node biopsy was performed, and the patient was diagnosed with chronic nonspecific lymphadenitis caused by chronic pharyngitis. The symptoms fell within the treatment scope of this application, so the symptoms were significantly improved after taking the modified Fengfang Pills prepared in Example 4 of this application for two courses of treatment. The patient continued to take the pills for two courses of treatment at the follow-up visit, and the patient had no obvious discomfort during a six-month follow-up.

[0154] Comparative Application Examples

[0155] This application is based on clinical case collection and tracking, formula optimization and clinical efficacy verification of the embodiments, combined with the establishment of a rat lymphadenitis model, comparison of rat cervical lymph node B-ultrasound, and comparison of rat lymph node tissue sections. The unique efficacy of the modified Fengfang pills provided in the application for treating lymphadenitis is fully demonstrated. However, it should be noted that the acute lymphadenitis, chronic lymphadenitis, specific lymphadenitis, and nonspecific lymphadenitis classification mentioned in this application belong to the category of Western medicine, and the two wind-heat evil toxin type and phlegm-fire internal knot type mentioned in Table 1 of Application Example 1 are the two most common types summarized based on the dialectical classification of Chinese medicine clinical and modified Fengfang pills for the clinical treatment of lymphadenitis. They are listed separately for explanation and calculation of efficiency. However, regardless of Western medicine classification or Chinese medicine classification, the final effect of taking the modified Fengfang pills provided in the application for treatment is judged by the disappearance of apparent redness, swelling, heat, and pain, the disappearance of pain, and the reduction or disappearance of lymph node enlargement, which are also the main observation indicators of animal pharmacodynamics.

[0156] The rat modeling method in this application theoretically belongs to the type of acute nonspecific lymphadenitis. However, the effect observation combined with the comprehensive judgment of traditional Chinese and Western medicine shows that nonspecific lymphadenitis is mainly caused by Staphylococcus aureus or hemolytic streptococci. Western medicine should use anti-infection treatment. The Jiawei Fengfang Pills provided in this application have antibacterial and anti-inflammatory effects and good therapeutic effects. Specific lymph nodes are caused by some special bacteria, including tuberculosis, parasites, etc., which cause lymphadenopathy and local redness, swelling, heat and pain. Judging from the symptoms of redness, swelling, heat and pain, they are consistent with the characteristics of traditional Chinese medicine surgery. Taking the Jiawei Fengfang Pills provided in this application can achieve the effects of clearing heat and detoxification, anti-inflammatory and analgesic, and has an excellent therapeutic effect.

[0157] The following are the pharmacodynamic animal experimental methods and some experimental pictures:

[0158] 1. Establishment of an Animal Model of Lymphadenitis and Verification of Model Success

[0159] (I) Materials and methods

[0160] 1. Experimental Animals

[0161] Thirty rats of either sex were randomly divided into two groups. (1) Model group: Fresh egg yolk was mixed with 10% saline solution and emulsified in a homogenizer to form egg yolk emulsion. 0.5 ml of egg yolk emulsion was injected into the hind leg (muscle) and flank (subcutaneous) of one side. (2) Blank group: No treatment was given.

[0162] 2. MRI examination

[0163] A 1.5T superconducting MRI machine was used, using a head coil and conventional SE sequence with a scanning field of 150 mm × 150 mm. Axial and coronal scans were primarily performed, with sagittal scans sometimes added. T1WI: TR 510 ms, TE 14 ms; T2WI: TR 1800 ms, TE 70 ms; slice thickness 3 mm, acquired twice. (1) MRI examinations were performed 7 days after the first egg yolk latex injection in the model group; (2) MRI examinations were performed at any time in the blank group.

[0164] 3. Pathological examination

[0165] After the MRI scan, the animals were sacrificed, the superior popliteal and internal iliac lymph nodes were exposed, the lymph nodes were removed and the size of the superior popliteal lymph nodes was measured, then the lymph nodes were immersed in 10% neutral formalin, embedded in paraffin, stained with HE and observed under a light microscope.

[0166] 4. Evaluation method

[0167] The size (short diameter) of the popliteal lymph nodes on T1WI was measured and the average value was calculated. The results are shown in Table 2. The signal intensity of the popliteal lymph nodes and surrounding muscles on T1WI and T2WI was measured using the region of interest method. The muscle signal intensity was used as a reference, and the lymph node signal intensity was divided by the muscle signal intensity to obtain the normalized signal intensity. The results are shown in Table 3 and Figures 1 to 4.

[0168] 5. Statistical methods

[0169] SPSS10.0 statistical software package was used to analyze variance of the completely randomized design to evaluate the differences in the size and signal intensity of the popliteal lymph nodes between the two groups. The SNK-q test was used for multiple comparisons of the differences in the means between the two groups.

[0170] (2) Results

[0171] 1. Animal performance after modeling

[0172] Model group: The tolerance to egg yolk latex was good, with no local reaction or systemic damage, and enlarged popliteal lymph nodes could be palpated within 6-7 days;

[0173] Blank group: no significant changes.

[0174] MRI manifestations

[0175] Lymph node size:

[0176] (1) Model group: Multiple groups of lymph nodes were enlarged, and MRI could show the popliteal, internal iliac, and superficial inguinal lymph nodes. Both the anterior and posterior groups of internal iliac lymph nodes were shown at the bifurcation of the abdominal aorta, with a total of 3 to 4 lymph nodes in this area. (2) Blank group: Popliteal lymph nodes were smaller, but clearly shown on both T1WI and T2WI. Only 3 mice had internal iliac lymph nodes clearly shown on T1WI, but they were difficult to identify on T2WI. As can be seen from Table 2, the popliteal lymph nodes in the model group were larger than those in the blank group.

[0177] Table 2 Popliteal lymph node size in each group (unit: mm; )

[0178] Lymph node signal characteristics:

[0179] (1) Model group: clear boundaries, uniform signal intensity, on T1WI, the signal intensity was lower than that of fat tissue and close to that of muscle tissue (see Figures 1 and 2); on T2WI, the signal intensity was significantly higher than that of muscle tissue (see Figure 3); (2) Blank group: clear boundaries, uniform signal intensity, on T1WI, the signal intensity was lower than that of fat tissue and close to that of muscle tissue (see Figure 4); on T2WI, the signal intensity was higher than that of muscle tissue. As can be seen from Table 3, there was no significant difference in the signal intensity of the popliteal lymph nodes between the two groups on both T1WI and T2WI.

[0180] Table 3 Standardized signal intensity of popliteal lymph nodes in each group (unit: sl; )

[0181] 2. Pharmacodynamic Verification in Model Group Rats

[0182] (1) Method of administration

[0183] The dosage of each group of rats was calculated based on the equivalent dose ratio of 0.018 based on the body surface area of human rats. The model group with successful modeling was given a medium dose of 2.520 g·kg of drug aqueous solution per kg of rat body weight. -1 ·d -1 At the same time, a control group was set up, and rats were given 1.250 g·kg of 0.9% saline per kg. -1 ·d -1 The drugs were prepared according to Examples 1-6 and Comparative Examples 1-5; the drug concentration in the aqueous solution was 0.241 g / ml; the drugs were administered by gavage once daily for 14 consecutive days. Using a LOGIQE9 ultrasound diagnostic instrument and an L8-18i-D ultrasound probe, lymph nodes of rats administered with different drugs and saline were examined. Pathological sections of lymph node tissue were also taken from rats administered with the drug prepared in Example 4 and with saline. The results are shown in Figures 5 to 15.

[0184] As shown in Figures 5 to 15, the size of the lymph nodes of rats given normal saline for two weeks is 40*78mm (Figure 5), the size of the lymph nodes of rats given the drug prepared in Example 4 for two weeks is 0*0mm (Figure 6), the size of the lymph nodes of rats given the drug prepared in Example 5 for two weeks is 20*15mm (Figure 7), and the size of the lymph nodes of rats given the drug prepared in Example 6 for two weeks is 15*10mm (Figure 8). The number of lobules of the lymph node tissue of rats given normal saline for two weeks increased, the number of acinar in the lobules increased, the number of glandular epithelial cells increased, the number of sloughed epithelial cells and secretions were visible in the lumen, the ducts were dilated, and the swelling was obvious (Figure 9). The number of lobules of the lymph node tissue of rats given the drug prepared in Example 4 for two weeks was small, mainly because the ducts and acinar in the lobules did not expand, the secretions decreased, and the swelling was obvious. The symptoms were significantly improved (Figure 10). The size of the lymph nodes of the rats given the medicine prepared in Comparative Example 1 for two weeks was 30*20mm (Figure 11), the size of the lymph nodes of the rats given the medicine prepared in Comparative Example 2 for two weeks was 40*35mm (Figure 12), the size of the lymph nodes of the rats given the medicine prepared in Comparative Example 3 for two weeks was 35*20mm (Figure 13), the size of the lymph nodes of the rats given the medicine prepared in Comparative Example 4 for two weeks was 20*15mm (Figure 14), and the size of the lymph nodes of the rats given the medicine prepared in Comparative Example 5 for two weeks was 40*20mm (Figure 15). It can be seen that the modified Fengfang pills prepared in the present application have a significant therapeutic effect on lymphadenitis compared with the normal saline treatment group and the medicines prepared in Comparative Examples 1 to 5. The enlarged lymph nodes disappeared or were significantly reduced, and the redness, swelling, heat and pain on the surface disappeared.

[0185] Among them, the formulation and ratio of Comparative Example 1 are exactly the same as those of Example 4, except that the dosage form is changed to a capsule; the formulation and ratio of Comparative Example 2 are exactly the same as those of Example 4, but the extraction process is changed, and the dosage form is a granule; the formulation of Comparative Example 3 lacks the honeycomb, and the other medicinal materials and ratios are exactly the same as those of Example 4, and the dosage form is a large honey pill; the formulation of Comparative Example 4 lacks the Radix Sophorae Tonkinensis, but retains the honeycomb and Trillium gracile medicinal pair, and the other medicinal materials and ratios are exactly the same as those of Example 4, and the dosage form is a large honey pill; the formulation of Comparative Example 5 lacks Trillium gracile, and the other medicinal materials and ratios are exactly the same as those of Example 4, and the dosage form is a large honey pill; as shown in Figures 6, 11 and 12, the therapeutic effects of Comparative Examples 1 and 2 after the improved dosage form and process are poor, and the therapeutic effect of Comparative Example 2 after the medicinal materials are extracted and changed to granules is the worst. The pills in Example 4 are 6 g per pill, and the dosage is two pills at a time, twice a day. The capsules can only be filled with 0.3 g per pill according to the pharmacopoeia standard. Patients take capsules and pills of the same medicine. The capsules need to take 40 pills per meal to achieve the effective rate of the pills. Such a large amount of capsules is not conventionally used. Therefore, according to the conventional dosage, the pills are significantly better than the capsules.

[0186] As shown in Figures 13 and 15 , the therapeutic effects of Comparative Example 3, which lacked Nymphaeaceae, and Comparative Example 5, which lacked Trillium gracile, were significantly worse. The drug prepared in Comparative Example 4, in which Nymphaeaceae and Trillium gracile were present as a drug pair and combined with Licorice root, showed improved therapeutic effects ( Figure 14 ), but the therapeutic effect was still inferior to that of the drug prepared in Example 4.

[0187] In summary, the Chinese medicine composition provided in this application (i.e., Chinese medicine composition) has a good therapeutic effect on lymphadenitis, is safe and has no toxic side effects, and can cure chronic recurrent cases in one go.

[0188] Although the above embodiment provides a detailed description of the present application, it is only a part of the embodiments of the present application, not all of the embodiments. People can also obtain other embodiments based on this embodiment without creativity, and these embodiments all fall within the scope of protection of the present application.
