Abstract
Description
Method for providing information for predicting lung cancer treatment prognosis and composition for predicting lung cancer treatment prognosis
[0001] The present invention relates to a method for providing information for predicting the prognosis of lung cancer treatment and a composition for predicting the prognosis of lung cancer treatment.
[0002]
[0003] Normally, cells divide, grow, and die to maintain a balance in cell number through cellular regulatory functions. However, if a change occurs in the genes of a cell for various reasons, the cell changes abnormally, matures incompletely, and proliferates excessively. This is called cancer.
[0004] Meanwhile, lung cancer is divided into two types: non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC). NSCLC, for which surgery is the best treatment option in the early stages, accounts for approximately 85% of all lung cancers. SCLC is a highly aggressive form, accounting for approximately 15% of all lung cancers. Compared to NSCLC, SCLC has smaller cancer cells and is composed of neuroendocrine cells. It is known for its rapid growth and rapid metastasis even in the early stages, making it a poor prognosis, with a five-year survival rate of between 5% and 10%. While remarkable advances in life sciences over the past several decades have significantly improved the five-year survival rates of most cancers compared to 30-40 years ago, the survival rate for SCLC has remained relatively stagnant. This is because, unlike NSCLC, effective treatments for SCLC have not yet been developed.
[0005] The ideal treatment for lung cancer is early detection and complete surgical removal. However, more than half of lung cancer patients are diagnosed at an advanced stage that makes surgery inoperable, making early treatment practically impossible. Furthermore, post-diagnosis lung cancer, especially small cell lung cancer, has a very low chance of being cured through surgery upon detection. Current clinical diagnostic methods cannot accurately predict the prognosis of early-stage small cell lung cancer with sufficient accuracy to direct more aggressive treatment regimens to patients at high risk of recurrence. Therefore, a technology for predicting the prognosis of lung cancer patients is urgently needed.

[0038]
[0039] The present invention provides a method for providing information for predicting the prognosis of lung cancer treatment, comprising the steps of measuring lactate dehydrogenase levels, creatinine concentrations, and platelet counts in a sample isolated from an individual; and calculating an EASIX score using the measured values to predict the prognosis of lung cancer treatment of the individual.
[0040] The above-mentioned individual may be an animal undergoing lung cancer treatment or having a history of lung cancer treatment, an animal suspected of needing lung cancer treatment, or an animal seeking information for predicting the prognosis of lung cancer treatment. The above-mentioned animal may be a mammal, including a human.
[0041] The above sample is separated from an individual and may be tissue, whole blood, plasma, serum, blood, bone, bone marrow or cerebrospinal fluid (CSF), and is preferably tissue, whole blood, plasma, serum or blood, but is not limited thereto.
[0042] Regarding the lactate dehydrogenase levels mentioned above, high serum lactate dehydrogenase levels indicate enzyme release due to inflammation and tissue turnover, which may reflect high tumor burden and metabolic activity. High serum lactate dehydrogenase levels may be associated with lower treatment response rates and worse progression-free survival and overall survival in lung cancer patients treated with first-line platinum-based chemotherapy. Thrombocytopenia confirmed by bone marrow biopsy indicates bone marrow invasion of lung cancer, which may negatively impact prognosis.
[0043] The substance for the above measurement is not limited in type as long as it can detect and measure lactate dehydrogenase, creatinine, and platelets by interacting with them, such as by binding to or reacting with them. Specifically, the substance may be an antibody, aptamer, protein, peptide, nucleic acid, polymer, compound, or reagent.
[0044] This may be a method for providing information for predicting the prognosis of lung cancer treatment, which further includes a step of determining that the higher the EASIX score, the worse the prognosis of lung cancer treatment, but is not limited thereto.
[0045] The above EASIX score is a method for providing information for predicting lung cancer treatment prognosis, calculated according to the following mathematical formula 1:
[0046] [Mathematical Formula 1]
[0047] (EASIX score) = (Lactate dehydrogenase level (U / L)) Х (Creatinine concentration (mg / dL)) / (Platelet count (10 9 / L))
[0048] It may include, but is not limited to.
[0049] The method may further include a step of determining that the prognosis for lung cancer treatment is poor when the EASIX score is 0.5 to 3, 0.5 to 2.75, 0.5 to 2.5, 0.5 to 2.25, 0.5 to 2, 0.5 to 1.75, 0.5 to 1.5, 0.5 to 1.25, 0.5 to 1, 0.5 to 0.75, 0.75 to 2, 1 to 2, 1.25 to 2, or 1.25 to 1.5.
[0050] The lung cancer may be selected from the group consisting of small cell lung cancer, non-small cell lung cancer, adenocarcinoma, squamous cell carcinoma, and large cell carcinoma. Specifically, the lung cancer may be small cell lung cancer. [0051] The above small cell lung cancer includes both limited and extensive stage cancers depending on the stage, and may be small cell carcinoma or mixed small cell carcinoma, but is not limited thereto.
[0052] The present invention may further include a step of determining that the prognosis for lung cancer treatment is poor if the EASIX score is 0.69 or higher, 0.71 or higher, 0.73 or higher, 0.75 or higher, 0.77 or higher, or 0.79 or higher. Preferably, the present invention may further include a step of determining that the prognosis for limited-stage small cell lung cancer treatment is poor if the EASIX score is 0.73 or higher.
[0053] The present invention may further include a step of determining that the prognosis for lung cancer treatment is poor if the EASIX score is 1.23 or higher, 1.25 or higher, 1.27 or higher, 1.29 or higher, or 1.31 or higher. Preferably, the present invention may further include a step of determining that the prognosis for extensive-stage small cell lung cancer treatment is poor if the EASIX score is 1.27 or higher.
[0054] The above treatment refers to an approach aimed at achieving beneficial or desirable clinical outcomes. For the purposes of the present invention, beneficial or desirable clinical outcomes include, but are not limited to, alleviation of symptoms, reduction in the extent of the disease, stabilization (i.e., non-worsening) of the disease state, delay or reduction in the rate of disease progression, improvement or palliation and alleviation (partial or total) of the disease state, whether detectable or undetectable. Treatment may also mean increasing survival compared to the expected survival rate in the absence of treatment. Treatment refers to both therapeutic treatment and preventative or prophylactic measures. The above treatments include not only the treatment required for the disorder being prevented, but also the treatment required for the disorder that has already occurred.
[0055] Palliating means that the extent of the disease state and / or undesirable clinical signs are reduced and / or the time course of progression is slowed or prolonged compared to when no treatment is administered.
[0056] The above prognosis refers to the course of the disease, including the likelihood of death or progression due to lung cancer, including the onset, recurrence, metastatic spread, survival rate, disease-free survival rate, drug resistance or sensitivity, and whether or not a patient is cured. Specifically, the prognosis may refer to the survival prognosis following lung cancer treatment, including surgery, chemotherapy, chemoimmunotherapy, chemoradiotherapy, or a combination of these therapies, in lung cancer patients. Furthermore, the prognosis of lung cancer treatment may refer to the patient's responsiveness to therapeutic agents for lung cancer treatment. Specifically, the prognosis refers to the risk of developing small cell lung cancer and the survival prognosis after onset, and more specifically, it refers to the prognosis of patients treated with chemotherapy for small cell lung cancer.
[0057] The above anticancer chemotherapy is a cancer treatment method that treats a tumor by administering to the subject an effective amount of a substance useful for treating cancer that has an anticancer effect. The substance useful for treating cancer includes an anticancer composition, an anticancer agent, an antineoplastic composition, etc. The anticancer agent is not limited thereto, but includes chemotherapeutic agents; growth inhibitors; cytotoxic agents; agents used in radiotherapy; antiangiogenic agents; antilymphangiogenic agents; apoptotic agents; anti-tubulin agents; And other cancer therapeutic agents, such as anti-HER-2 antibodies, anti-CD20 antibodies, epidermal growth factor receptor (EGFR) antagonists (e.g., tyrosine kinase inhibitors), HER1 / EGFR inhibitors (e.g., erlotinib (Tarceva™), platelet-derived growth factor inhibitors (e.g., Gleevec™ (imatinib mesylate)), COX-2 inhibitors (e.g., celecoxib), interferons, cytokines, antagonists (e.g., neutralizing antibodies) that bind to one or more of the targets ErbB2, ErbB3, ErbB4, PDGFR-beta, BlyS, APRIL, BCMA VEGF and VEGF receptor(s), TRAIL / Apo2, and other bioactive and organic chemical agents, etc. In addition, as an anticancer agent for treating small cell lung cancer of the present invention, more preferably One or more of Etoposide, Adriamycin, Vincristine, Cyclophosphamide, and Ifosphamide may be selected and used, but is not limited thereto.
[0058] The above prognosis refers to the preliminary assessment of a patient's likelihood of survival based on a favorable or unfavorable response to treatments such as chemotherapy, chemoradiotherapy, or other lung cancer treatments. Predicting survival prognosis can help determine the most appropriate treatment option for a patient, determine whether the patient will respond favorably to that treatment, or predict long-term survival after receiving that treatment.

[0075]
[0076] The present invention is described in more detail below with reference to the following examples. These examples are intended to illustrate the present invention and are not intended to limit its scope.
[0077]
[0078] Experimental example
[0079] 1. Method
[0080] 1.1. Method of selecting objects
[0081] We retrospectively reviewed the electronic medical records of 264 consecutive patients with histologically diagnosed small-cell lung cancer (SCLC) at a single institution from July 2006 to August 2021. Eligibility criteria were as follows: (1) receiving concurrent chemoradiotherapy (CCRT) or palliative chemotherapy with etoposide or irinotecan plus platinum-based regimens as first-line treatment; (2) documented baseline serum lactate dehydrogenase (LDH), creatinine, and platelet counts; and (3) no evidence of a secondary primary cancer. Due to the retrospective nature of this study, which involved electronic medical record review without investigator intervention, informed consent was not required. This study was approved by the Institutional Review Board of Gyeongsang National University Hospital.
[0082] 1.2. Evaluation
[0083] Endothelial Activation and Stress IndeX (EASIX) was calculated from serum lactose dehydrogenase (LDH) (IU / L), serum creatinine level (mg / dL), and platelet count (x10) obtained within 7 days before the start of the first cycle of chemotherapy. 3 / μL). There was no restriction on each serum marker for EASIX calculation. The cutoff value of EASIX was determined through time-dependent receiver operating characteristics (ROC) curve analysis, and the Youden index (sum of sensitivity and specificity) was maximized to predict the median overall survival (OS) in limited-stage disease (LD) (18 months) and extensive-stage disease (ED) (10 months).
[0084] Using these cutoff values, patients were divided into high and low EASIX groups. Performance status was assessed using the Eastern Cooperative Oncology Group Performance Status (ECOG PS) scale. Response to treatment was assessed using the Response Evaluation Criteria in Solid Tumors version 1.1. The objective response rate (ORR) was defined as the sum of complete response (CR) and partial response (PR). Treatment-related adverse events (AEs) were assessed using the National Cancer Institute Common Toxicity Criteria for Adverse Events version 5.0. Treatment-related death was defined as death from any cause unrelated to disease progression during chemotherapy, including deaths occurring within 30 days of the last cycle of first-line chemotherapy. Premature discontinuation of treatment was defined as premature discontinuation of planned first-line chemotherapy or concurrent chemoradiotherapy (CCRT) due to treatment-related adverse reactions, patient preference, or unknown causes, and not due to treatment resistance.
[0085] 1.3. Statistical Analysis
[0086] Correlations between variables were tested using the Mann-Whitney U test and the chi-squared test for continuous and categorical variables, respectively. Time-to-progression (TTP) was calculated as the time from the start of treatment to progression or the last follow-up date. Progression-free survival (PFS) was defined as the same as TTP, but death from any cause was considered an event. Overall survival (OS) was defined as the time from the first day of treatment to death or the last follow-up date. The Kaplan-Meier method, the log-rank test, and Cox regression analysis were used to estimate survival rates. All variables with an F value less than 0.2 in the univariate analysis were included in the multivariate Cox regression analysis. Statistical significance was defined as a two-sided P value of < 0.05. All statistical analyses were performed using STATA version 16.1 (College Station, TX, USA) and R (version 4.1.1, Foundation for Statistical Computing, Vienna, Austria).
[0087]
[0088] 2. Results
[0089] 2.1. Patient characteristics
[0090] A total of 264 patients were included in the analysis. The cutoff values for EASIX were 0.73 (area under the curve [AUC] 0.672, sensitivity 63.0%, specificity 71.1%) and 1.27 (AUC 0.597, sensitivity 40.0%, specificity 81.0%) in the LD and ED groups, respectively. The number of patients with high and low EASIX scores was 41 and 48 in the LD group, and 51 and 124 in the ED group, respectively (Figures 1 and 2).
[0091] Patient characteristics were compared between the high-EASIX and low-EASIX groups (Table 1). In the LD group, the high-EASIX group had a higher proportion of elderly patients (median age, 69 vs. 65 years) and male patients (100% vs. 72.9%) than the low-EASIX group. Prophylactic cranial irradiation was performed less frequently in the high-EASIX group than in the low-EASIX group (46.3% vs. 68.8%), possibly because of a lower treatment response rate in the high-EASIX group. In the ED group, male patients (96.1% vs. 85.5%) and poor performance status (35.3% vs. 18.6%) were more common in the high-EASIX group than in the low-EASIX group. In both stages (LD and ED), the gender difference between the high-EASIX and low-EASIX groups may be due to the intrinsic difference in serum creatinine levels by gender (mean±SD serum creatinine levels, 0.83±0.35 mg / dL in men vs. 0.57±0.16 mg / dL in women, P<0.001). Most patients were treated with etoposide and platinum, regardless of tumor stage and EASIX level. Three and 19 patients were treated with immune checkpoint inhibitors (ICIs) plus etoposide and platinum in the LD and ED, respectively.
[0092]
[0093] 2.2. Treatment response
[0094] The treatment response rates according to EASIX are shown in Table 2. In the LD group, complete response (CR) rates were 19.5% and 33.3% in the high-EASIX and low-EASIX groups, respectively (P = 0.050). The objective response rate (ORR) was significantly lower in the high-EASIX group than in the low-EASIX group (85.4% vs. 97.9%, P = 0.028). In the ED group, the treatment response rate tended to be lower in the high-EASIX group; however, statistical significance was not found. A complete response (CR) was achieved in only one patient in the low-EASIX group. The ORR was 68.6% and 79.8% in the high-EASIX and low-EASIX groups, respectively (P = 0.112). Treatment regimens did not affect response rates (ORR, etoposide-based vs. irinotecan-based, 92.1% vs. 100%, P > 0.998 [LD] and 76.1% vs. 83.3%, P = 0.734 [ED]; platinum-based alone vs. ICI-combined, 91.9% vs. 100%, P > 0.998 [LD] and 76.3% vs. 79.0%, P > 0.998 [ED], respectively).
[0095]
[0096] 2.3 Treatment-related adverse reactions
[0097] There was no difference in hematologic toxicity between the two groups (Table 3). The median treatment cycles were 4 and 5.5 in the high-EASIX and low-EASIX groups, respectively, in both stages (LD and ED). Because a total of 4 cycles of chemotherapy (with an optional 2 cycles in the ED) is currently recommended, it is difficult to determine whether the high-EASIX group was undertreated. The frequency of premature discontinuation and treatment-related mortality were not statistically different between the two groups (EASIX and low-EASIX groups), regardless of stage (LD and ED).
[0098]
[0099] 2.4. Survival period
[0100] The median follow-up period was 27 months and 18 months in the LD and ED groups, respectively. In LD, the median TTP was 13.1 months and 21.4 months in the high-EASIX and low-EASIX groups, respectively (P = 0.010, Fig. 3). The median PFS was 6.8 months and 15.5 months in the high-EASIX and low-EASIX groups, respectively (P = 0.004, Fig. 4). The difference in survival was more evident with regard to overall survival (OS) (median OS in the high-EASIX and low-EASIX groups, 9.8 months and 40.5 months, respectively; P < 0.001; Fig. 5). Similar results were observed in ED patients. The median TTP was 5.4 months and 6.4 months in the high-EASIX and low-EASIX groups, respectively (P = 0.007, Fig. 6). The median PFS was 4.5 months and 6.2 months in the high-EASIX group and the low-EASIX group, respectively (P = 0.004, Fig. 7). The median OS was 7.2 months in the high-EASIX group and 11.9 months in the low-EASIX group, respectively (P < 0.001, Fig. 8).
[0101] Multivariate analysis was performed to determine PFS and OS (Table 4). Joint independent prognostic factors and dependent variables at all stages were ECOG PS and EASIX. In LD, high EASIX was an independent prognostic factor for worsening PFS (hazard ratio [HR] 1.967, 95% confidence interval [CI] 1.176–3.290, P = 0.010) and OS (HR: 1.460, 95% CI: 1.069–1.994, P = 0.017). In the ED, high EASIX was associated with worse PFS (HR: 1.562, 95% CI: 1.107-2.204, P = 0.011) and OS (HR: 1.667, 95% CI: 1.334-2.448, P = 0.009).
