[0004] The present invention relates to compositions and methods for treating unscheduled bleeding, improving health-related quality of life and alleviating menstrual symptoms in adolescent female subjects. Specifically, the invention relates to compositions comprising estetrol (E4) and drospirenone (DRSP), and associated methods that improve the bleeding profile, health-related quality of life and menstrual symptoms.

[0005] BACKGROUND OF THE INVENTION

[0006] Unscheduled bleeding is a prevalent and distressing issue among adolescent females, also among those using hormonal contraceptives. Associated with unscheduled bleeding are physical discomfort, psychological stress, and non-compliance with contraceptive regimens. Currently, treatment options for adolescent females are poorly understood, as most data come from studies involving adult populations. Recently, data from studies conducted specifically in the adolescent population became available for progestin-only pills based on drospirenone (e.g. SLYND® (fda.gov)). Nevertheless, despite their advantages, progestin-only pills have the drawback of an unfavourable bleeding pattern and are hence not always preferred by premenopausal subjects. This suboptimal efficacy was even commented on and emphasized by the Food and Dmg Administration (FDA) in a public letter to the manufacturer (FDA, Letter to Exeltis re ND A 211367 SLYND® (drospirenone) tablets, for oral use, 2023).

[0007] Interestingly, there are notable discrepancies in the occurrence of unscheduled bleeding between adult and adolescent patients. While the percentage of adult females experiencing unscheduled bleeding / spotting with a progestin-only pill based on drospirenone was described to decrease over time, the percentage of adolescent females with unscheduled bleeding or spotting was maintained at a relatively consistent level during the study (53.0% in Cycle 1 versus 52.2% in Cycle 13, LABEL (fda.gov)).

[0008] Further, adolescents frequently report menstrual-related symptoms that impact daily functioning and wellbeing. Data on the impact of combined oral contraceptives (COCs) on adolescent menstrual-related symptoms and health-related quality of life (HRQoL) however remain sparse.

[0009] As adolescent users do not behave similarly to adults, there is an urgent need for novel therapeutic approaches tailored specifically to adolescents experiencing unscheduled bleeding, and / or menstrual-related symptoms, and / or impacted health-related quality of life. Addressing this unmet need, the present invention aims to provide a more effective, safe, and user-friendly solution for adolescent females seeking treatment for bleeding irregularities, and / or menstmal related symptoms, and / or impacted health-related quality of life. SUMMARY OF THE INVENTION

[0010] Based on extensive research, the present inventors have shown that a composition comprising an estetrol component and drospirenone can be beneficial to adolescent subjects suffering from unscheduled bleeding and / or spotting, and / or menstrual related symptoms, and / or impacted health-related quality of life.

[0011] A phase III study was conducted to assess safety, compliance and pharmacokinetics associated with the use of a COC comprising an estetrol component and drospirenone in postmenarchal adolescents. The COC was well- tolerated and no safety concerns were raised during the study. Bleeding data in adolescents showed good cycle control and acceptable bleeding pattern based on the gradual reduction in the incidence of unscheduled bleeding. The adolescents also experienced a reduction in the number of unscheduled bleeding and / or spotting days per cycle from 9 to 6 days. In addition, improvements in respectively well-being and cycle-related symptoms were found, the latter being more pronounced than in adult patients. This is particularly unexpected, as adolescents generally have more symptoms at baseline than adults.

This rapid decrease is unexpected, since it was reported for SLYND® that the percentage of adolescent females with unscheduled bleeding or spotting was maintained at a relatively consistent level during treatment while the percentage of adult females experiencing unscheduled bleeding / spotting with SLYND® decreased over time.

[0014] A further unexpected effect of the composition comprising an estetrol component and drospirenone was that despite the fact that having a stable bleeding pattern was not an inclusion requirement for the present study an excellent control of scheduled bleeding was observed, exceeding the scheduled bleeding control observed in adolescents upon use of SLYND®, notwithstanding that the study using SLYND® did require a stable bleeding pattern over a least four menstrual cycles prior to screening for enrolment eligibility.

Mean age was 15.2 years, ranging from 12 to 17 years. Most subjects (78.3%) were aged 15 to 17 years. Mean BMI was 21.2 kg / m2, 2.8% of subjects were in the <5th BMI percentile (underweight) and 4.7% were in the > 95th BMI percentile (obese). Most of the subjects (91.5%) were starters, defined as not having used hormonal contraceptives for 2 months before screening, and 19.8% of subjects had previously used contraceptive methods such as COC’s, progestin-only pills or intrauterine devices. The study included 6 visits: Visit 1 (Screening Visit), Visit 2 (Subject Enrollment Visit), three on-treatment visits (Visit 3, Visit 4, Visit 5 / Early Termination (ET) Visit) and one post-treatment visit (Visit 6). The site also completed an Eligibility Confirmation phone call to the subject after Visit 1 and a follow-up call after Visit 2, within 7 days following the first Investigational Product intake. Bleeding and spotting data was summarized by the methods suggested by Mishell et al (Mishell D.R., Jr, Guillebaud J., Westhoff C., Nelson A.L., Kaunitz A.M., Trussell J., Davis A.J. Cycle 1 depicts a higher percentage of subjects reporting bleeding and / or spotting. The pattern of bleeding and / or spotting in Cycle 2 and beyond shows fewer subjects reporting bleeding and / or spotting.

Figures 6 and 7 highlight the symptoms that showed the most pronounced changes within the domains exhibiting the greatest shifts in T-scores. This analysis enabled item-level insight into symptom evolution over time, in line with the established methodology.

[0286] Of the 112 enrolled subjects, 105 received at least one dose of the study drug and 89 completed all six treatment cycles.

Smaller changes were seen in the remaining subscales.

Significant reductions in T-scores were observed from baseline to EoT for menstrual Pain (-6.2), premenstrual and menstrual Water Retention (-3.6 and -3.7 respectively) and menstrual Negative Affect (-7.3).

[0294] Fourteen of the 26 symptoms were reported as severe or strong by at least 10% of participants at baseline and showed an intensity reduction of more than 40% (Table 10). These symptoms were headache, cramps, backache, fatigue, general aches and pain (domain Pain), painful or tender breasts, swelling (breast abdomen) (domain Water Retention), loneliness, anxiety, mood swings, crying, irritability, tension, and feeling sad or blue (domain Negative Affect). Notable improvements (>20% difference between participants improving versus worsening) were observed for menstrual cramps, backache, and general aches and pain (Pain domain); premenstrual and menstrual breast tenderness (Water Retention domain); and menstrual mood swings, crying, tension, and feeling sad or blue (Negative Affect domain) (Table 7). In the physical and emotional domains, Pain and Negative Affect showed the largest T-score reductions during the menstrual phase (-6.2 and -7.3, respectively; Table 6), with 52.9% of participants experiencing improvement in menstrual cramps and 45.1% in menstrual mood swings (Table 7). Shift analyses for these symptoms are shown in Figure 6 and Figure 7, respectively, with additional figures for other symptoms provided as Figures 8 to 19. Overall, shifts were more pronounced (skipping two or more intensity levels) for those reporting improvement compared to those reporting worsening.

[0296] In the European phase 3 trial evaluating E4 / DRSP in adults, MDQ scores for Pain, Water Retention, and Negative Affect showed statistically significant reductions, with mean T-score decreases ranging from -1.4 to -3.5 points (Bitzer J, Bouchard C, Zatik J, Weyers S, Piltonen T, Suturina L, et al. Effects of E4 / DRSP on selfreported physical and emotional premenstrual and menstrual symptoms: data from the phase 3 clinical trial in Europe and Russia. In the current study, improvements in these domains were even more pronounced, with T-score reductions ranging from -3.6 to - 7.3 points, suggesting a similar or even greater benefit in alleviating physical and emotional premenstrual and menstrual symptoms in adolescents.

[0299] While there are no prior published studies using the KIDSCREEN-27 to assess the impact of combined oral contraceptives on quality of life in adolescents, the observed improvements in Physical Well-being (+8.3%), Autonomy & Parents (+11.4%), and School Environment (+8.4%) are noteworthy (Table 11). These changes exceed the standard threshold of 0.5 SD typically regarded as clinically meaningful in QoL research. The improvements in T-scores in our study suggest a possible normalizing or even enhancing effect of E4 / DRSP in a healthy, treatment-seeking population. Further, the European KIDSCREEN norm study found average scores of around 50 (SD 10) in the general adolescent population. In our study, improvements not only exceed these average values but may reflect enhanced daily structure, predictability, and reduced cycle-related disruptions due to contraceptive use.

[0300] Both of the instruments used in this study were developed through rigorous methodology to ensure psychometric validity and international applicability. The KID SCREEN project, funded by the European Commission, involved researchers from 13 countries who employed literature reviews, Delphi consensus, focus groups with children and parents, and cross-cultural validation using both classical and modem test theory. It has been validated in multiple populations and languages and is widely used in clinical and research contexts to measure cyclical physical and emotional symptoms across the menstrual cycle. The short form used in this study (MDQ-SF) retains the most clinically relevant domains, making it appropriate for adolescent use in clinical trials (Moos RH. [0303] Together, the findings suggest that initiation of E4 / DRSP not only alleviates bothersome menstrual symptoms but may also enhance daily functioning, self-perceived well-being, and broader psychosocial outcomes in adolescents starting contraception. The use of age-appropriate, validated instruments strengthens the robustness of this conclusion. Importantly, the shift analysis offers clinically relevant insights beyond what is captured by changes in mean T-scores. It reveals how individual symptoms evolve in severity, providing a more nuanced understanding of treatment response that can guide personalized care decisions.

[0305] E4 / DRSP significantly improved HRQoL and reduced menstrual distress in adolescent users over six cycles. These results support the use of natural estrogen-containing COCs for both contraceptive and noncontraceptive benefits in this population. The KIDSCREEN-27 and MDQ-SF proved effective in capturing relevant outcomes.

Claims

46CLAIMS1. A composition comprising an estetrol component and drospirenone for use in treating unscheduled bleeding and / or spotting in an adolescent female subject aged below 16 years old and postmenarchal, preferably for decreasing unscheduled bleeding events and / or spotting events.

2. The composition for use according to claim 1, wherein the adolescent female subject is aged between 10 and 15.5 years old and postmenarchal, preferably wherein the adolescent female subject is aged between 12 and 15 years old and postmenarchal.

3. The composition for use according to claim 1 or 2, wherein a bleeding event is any evidence of vaginal blood loss that requires the use of sanitary protection with a tampon, menstrual cup, pad or pantyliner and wherein a spotting event is any evidence of minimal vaginal blood loss that does not require new use of sanitary protection, including panty liners.

The composition for use according to any one of claims 1 to 3, wherein an unscheduled bleeding event or spotting event is a bleeding or spotting event that is not considered a scheduled bleeding event or spotting event, preferably wherein a scheduled bleeding event or spotting event has:- a start date and stop date in the scheduled bleeding interval (day 25 - day 3 next cycle); or- a start date before day 25 and stop date before day 3 (early withdrawal bleeding / spotting); or- a start date in the scheduled bleeding interval (day 25 - day 3 next cycle) and continuing into next cycle (continued withdrawal bleeding / spotting).

5. The composition for use according to any one of claims 1 to 4, wherein the unscheduled bleeding events and / or spotting events are decreased compared to the unscheduled bleeding events and / or spotting events prior to the use of said composition.

The composition for use according to any one of claims 1 to 5, wherein the decrease in the unscheduled bleeding events and / or spotting events prior to the use of the composition is a decrease in the unscheduled bleeding and / or spotting compared to the unscheduled bleeding events and / or spotting events upon using an oral contraceptive, preferably upon using a progestin-only pill preferably comprising drospirenone, preferably upon using a combined oral contraceptive comprising as estrogenic component a component that is not an estetrol component, preferably wherein said combined oral contraceptive comprising as estrogenic component a component that is not an estetrol component is a 2nd generation combined oral contraceptive.

7. The composition for use according to any one of claims 1 to 6, wherein the unscheduled bleeding events and / or spotting events are decreased in occurrence and / or intensity by at least about 25%, preferably at least about 50%, more preferably by at least about 75%.

8. The composition for use according to any one of claims 1 to 7, wherein the number of days said unscheduled bleeding events and / or spotting events occur is reduced to 8 days or less, preferably to 6 days or less, more preferably 5 days or less within one cycle.

479. The composition for use according to any one of claims 1 to 8, wherein the decrease in unscheduled bleeding events and / or spotting events is a decrease in the intensity of unscheduled bleeding events and / or spotting events, preferably over a period of at least one month.

10. The composition for use according to any one of claims 1 to 9, wherein the estetrol component is administered at a daily dose of from about 1 mg to about 40 mg or wherein the composition comprises from about 1 mg to about 40 mg of an estetrol component.

The composition for use according to any one of claims 1 to 10, wherein the estetrol component is administered at a daily dose of from about 5 mg to about 25 mg, preferably at a daily dose of from about 10 mg to about 20 mg, more preferably at a daily dose of about 15 mg or wherein the composition comprises from about 5 mg to about 25 mg of an estetrol component, preferably from about 10 mg to about 20 mg of an estetrol component, more preferably about 15 mg of an estetrol component.

The composition for use according to any one of claims 1 to 11, wherein the drospirenone is administered at a daily dose of from about 0.5 mg to about 10 mg or wherein the composition comprises from about 0.5 mg to about 10 mg of drospirenone, preferably wherein the drospirenone is administered at a daily dose of from about 1 mg to about 4 mg or wherein the composition comprises from about 1 mg to about 4 mg of drospirenone.

The composition for use according to any one of claims 1 to 12, wherein the estetrol component is estretrol, preferably wherein the estetrol component is estetrol monohydrate, more preferably wherein the estetrol component is administered at a daily dose of about 15 mg estetrol monohydrate or wherein the composition comprises about 15 mg of estetrol monohydrate.

14. The composition for use according to any one of claims 1 to 13, wherein the drospirenone is administered at a daily dose of about 3 mg or wherein the composition comprises about 3 mg of drospirenone.

15. The composition for use according to any one of claims 1 to 14, wherein the estetrol component and drospirenone are formulated together in a single oral dosage unit (i.e. a Combined Oral Contraceptive - COC).
