BISPECIFIC ANTIBODIES AND USES THEREOFTechnical Filed

[0001] The present disclosure relates to a bispecific antibody and uses thereof, and more particularly to a bispecific antibody specifically binds to CD16A expressed on a natural killer cell and a B-cell marker such as CD19 or CD20 expressed on a B cell. The present disclosure further relates to the polypeptides, polynucleotides, vectors, cells, and method of treatment involved in the preparation and uses of the bispecific antibody.Background

[0002] Autoimmune diseases are induced by dysregulated inflammation against autoantigens then affect distinctive organs or system with a spectrum of symptoms and clinical conditions. Based on the specific target tissue and clinical manifestations, rheumatoid arthritis (RA) , multiple sclerosis (MS) , and inflammatory bowel disease (IBD) , systemic lupus erythematosus (SLE) , etc. are well characterized. The accumulatively prevalence of all these autoimmune diseases is about 5%–10%worldwide.

[0003] Treatment for autoimmune diseases aims to alleviate symptoms, control disease activity, and mitigate damage to affected tissues. Conventional treatments for autoimmune diseases suppress the general immune system to modulate the inflammation, such as the immunosuppressive / anti-inflammatory drugs. However, these therapeutic approaches are not always successful based on poor clinical efficacy due to the heterogeneous of different diseases, or the side effect, particularly increased infection risk due to the systematic immune suppression. Then target therapies appear to overcome the disadvantage of the conventional methods, leading to more selectively inflammatory inhibition and minimal disruption to homeostatic immune functions, such as Janus kinase (JAK) inhibitors, sphingosine 1-phosphate receptor (S1PR) modulators, anti-TNF-α antibodies, anti-IL-23 antibodies and B cell depletion therapies (BCDT) .

[0004] B cells are pivotal in autoimmune diseases, participant in multiple pathological mechanisms during the development of the diseases. Firstly, B cells directly contribute to the generation of autoantibody, which targets the autoantigen, inducing an antibody mediated immune attack on healthy tissues, followed by inflammation and damage. Secondly, B-cells can secrete various cytokines like TNF-α, IL-6, GM-CSF and IL-10, contributing to tissue inflammation and damage. Thirdly, B cells act as antigen-presenting cells (APCs) , capturing and presenting autoantigens to T cells, activating T cells to against healthy cells expressing the autoantigen, further amplifying the cell-mediated immune response. In addition, dysregulation of B-regs and tertiary lymphoid tissue (TLT) sustentation by B cells are also involved in autoimmune diseases.

[0005] BCDT was first considered for autoimmune diseases two decades ago, and it shows convincing clinical responses and acceptable safety profiles. Till now five monoclonal antibodies have been approved to treat autoimmune diseases, including RA, MS and SLE. B cell surface markers such as CD20 and CD19 are well-known targets for the antibody’s development in clinic. Therapies targeting CD20, exemplified by Rituximab, can deplete B cells effectively through antibody-dependent cell-mediated cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC) , showed efficacy in several autoimmune diseases. Inebilizumab, an anti-CD19 ADCC enhancing afucosylated IgG1 antibody, also demonstrates clinical improvement in specific autoimmune disease. However, since the diverse spectrum of autoimmune diseases, monoclonal antibodies are not always responsive in all the autoimmune diseases. The role of B cells in pathogenesis, degree of B cell depletion, the heterogenicity of human B cell subsets, the variability of autoantibody effects, and the role of long-lived plasma cells, all contribute to different clinical efficacies crossing the various autoimmune diseases. CAR-T cell therapies and bispecific antibodies as emerging BCDT, are developed to enhance the B cell depletion and expand the populations.

[0006] Currently, FDA-approved CAR-T cell therapies in oncology are targeting CD19 and BCMA for B cell and plasma cell depletion. Similar CAR-T cell therapies now are re-purposed in the autoimmune area and show remarkable efficacy in the small number of patients. Car-T targeting CD19, such as Relma-Cel in the investigator-initiated trial (IIT) , 4 / 12 patients show 100%SRI-4 response rate followed up for at least six months, and 11 / 12 patients stop using traditional corticosteroids and immunosuppressants. Both MB-CART19.1 (autologous CD19 Car-T) and BRL-301A (universal CD19 Car-T) also demonstrate preliminary efficacy in idiopathic inflammatory myopathy (IIM) , systemic sclerosis (SSc) and SLE in the IITs. Meanwhile, BCMA targeting Car-T, like Eque-cel in phase1 (NCT04561557) , demonstrates durable pathogenic antibody clearance, and potential clinical efficacy in neuromyelitis optical spectrum disorder (NMOSD) . However, the promising efficacies of CAR T cells for B cell depletion in both oncology and autoimmune associate with significant toxicities. The major toxicities are cytokine release syndrome (CRS) , immune effector cell associated neurotoxicity syndrome (ICANS) and leukopenia, which limited the application of Car-T in the non-oncology area.

[0007] Like Car-T, nowadays bispecific antibodies which was developed in blood malignancies are also evaluated in autoimmune diseases. Mosunetuzumab is an FDA approved T cell engaging CD20 / CD3 bispecific antibody, that can redirect T cells to eliminate the malignant B cells. The study to explore the safety and pharmacodynamics of Mosunetuzumab to SLE patients is on-going (NCT05155345) . Another CD20 / CD3 bispecific antibody imvotamab is also trying to prove its early clinical efficacy in IIM (NCT06524687) , SLE (NCT06041568) and RA (NCT06087406) . In addition, with CD20 T cell engager, publications of Teclistamab, a BMCA / CD3 bispecific antibody approved in multiple myeloma are also reported the preliminary efficacy in SLE, IIM, SSc, RA and   syndrome. Side effects are also similar to the published clinic safety profiles of most of the CD3 bispecific T cell engager, such as CRS and lymphopenia.

[0008] Compared with Car-T and T cell engager, natural killer (NK) cell engager showed better safety profiles in the blood malignancy studies. CD16 / CD30 bispecific antibody AFM13, demonstrates no CRS / ICANS occurred in the Hodgkin's lymphoma patients without compromising the clinical efficacy (NCT02321592) ; and CD16 / IL-15 / CD133 trispecific antibody GTB-3550, either doesn’ t show CRS during the trail with acute myeloid leukemia patients (NCT03214666) . Till now, no application of NK cell engaging antibody in autoimmune disease was reported.

[0009] Natural killer (NK) cells are lymphocytes of innate immune system. Human NK cells are usually classified on the basis of two surface receptors CD56 and CD16 expression. CD56brightCD16- NK cells produce more cytokines, chemokines and growth factors but exhibit lower cytotoxicity. However, CD56dimCD16+ NK cells are highly toxic and also can produce cytokines and chemokines. There are two types of CD16 (FCγRIII) , encoded by two genes, A and B. One type, CD16A, is a transmembrane protein that co-localizes with CD3ζ and Fc-εRI-γ on NK cells. Upon ligation, it induces a potent series of signals resulting in cytokine production and cytotoxic effector activity via ADCC. The second type, CD16B, is found on neutrophils and is not involved in tumor cell killing. Although the extracellular domains are highly homologous, glycosylphosphatidylinositol linkage differentiates CD16B from CD16A. An essential feature of NK cells is their ADCC effector activity, a major mechanism pursued by majority of monoclonal antibody therapies, especially preventing cancer. The different polymorphism of CD16a, either 158F (lower affinity) or 158V (higher affinity) which affect its affinity for Fc portion, low affinity between Fc domain and CD16a make NK mediated ADCC efficacy is very limited, causing lots of tumors not respond to mAb therapy or relapse in the advanced time.

[0010] Various strategies have been used to improve NK ADCC anti-tumor potential. These include glycoengineering the Fc region of cytotoxic antibodies to improve their binding to CD16a, as well as amino acid substitutions. Another approach to augment NK killer function is the design of bi-or tri-specific antibody with one arm on tumor associated antigen, and another one on CD16 or other NK specific receptors, commonly called NK engagers structures (NKCE) . These constructs could recruit NK cells with high affinity to tumors, and then kill them directly. AFM13, a bispecific NKCE targeting CD30 and CD16a, has been evaluated in clinic for CD30 positive relapsed or refractory lymphomas. Other bispecific targeting CD16a and tumor antigen EGFR, CD123, CD33 and BCMA had been evaluated efficacy in clinical or preclinical studies. However, NKCE format was rarely used in the field of inflammatory or autoimmune disease till now. Artiva currently uses AlloNK, an allogeneic NK cells in combination with Rituximab for treatment of SLE in patients with active lupus nephritis in clinical trials. Activated T cells can drive cell death after engagement of their highly specific T cell receptors, and this mean T cells can easily produce storm of cytokines and generate system toxicity if off-target appeared. Because NK cells integrate information of different receptors, they are far less likely to kill normal cells expressing a target antigen. Hence, NK cells may have safer therapeutic index than T cells which means TCE or CAR-T cells.Summary

[0011] One aspect of the present disclosure provides a bispecific antibody comprising (a) an IgG antibody comprising two heavy chains (HCs) and two light chains (LCs) , each of the two heavy chains comprising a heavy chain variable region (VH) and a heavy chain constant region (CH) , each of the two light chains comprising a light chain variable region (VL) and a light chain constant region (CL) , wherein the IgG antibody binds specifically to a B-cell marker; and (b) an anti-CD16A VHH, covalently linked to C-terminus of each of two heavy chains of the IgG antibody.

[0184] Humanization of Anti-CD19 VH / VL was performed in 3 steps. First, FMC63 VH and VL from NCBI database accession PDB: 7URV_D were used to search against IMGT-aligned human germline IGHV and IGKV gene sequences compiled from IMGT database. The closest human gene allele IGHV4-30-4*01 and IGKV1-39*01 were selected based on sequence identity. CDR residues according to Kabat scheme were inserted at the corresponding positions in the germline sequences. Second, the 3D structure of CDR grafted sequences was predicted by AlphaFold, subset of vernier zone residues were selected and back mutated to parental residues if suspected by visualization to be incompatible with CDR. Lastly, humanness score T20 of resulting sequences were calculated based on a report by Sean et. al., BMC Biotechnology, 13: 55, 2013. The humanization process ultimately generated humanized anti-CD19 antibody comprising VH and VL having amino acid sequences shown in SEQ ID NO: 6 and 7, respectively. The humanized anti-CD19 antibody further comprises a human IgG1 heavy chain constant region having an amino acid sequence shown in SEQ ID NO: 14, and a human IgG1 light chain constant region having an amino acid sequence shown in SEQ ID NO: 15.

[0186] This example demonstrates the construction of two bispecific antibodies with one domain binding to CD19 or CD20 antigen, and the other domain binding to CD16A. CD16-VH-RM72, a fully human CD16A single domain antibody was chosen as a building block for constructions. CD16-VH-RM72 has an amino acid sequence shown in SEQ ID NO: 3. IgG1 type anti-CD20 mAb Ofatumumab or Obinutuzumab (BM366-09, Heavy chain: SEQ ID NO: 148; Light chain: SEQ ID NO: 146) was fused to N-terminal of CD16-VH-RM72 chain with a flexible (G4S) 3 linker in between to generate BM366-01 (Heavy chain: SEQ ID NO: 4; Light chain: SEQ ID NO: 5) and BM366-05 (with YTE mutation of BM366-01, Heavy chain: SEQ ID NO: 149; Light chain: SEQ ID NO: 5) or BM366-18 (Heavy chain: SEQ ID NO: 147; Light chain: SEQ ID NO: 146) and BM366-18AF (Afucosylation version of BM366-18, i.e., the core fucose from the glycan at N297 (EU numbering) position was removed; Heavy chain: SEQ ID NO: 147; Light chain: SEQ ID NO: 146) . Humanized anti-CD19 IgG antibody as mentioned in Example 1 was fused to N-terminal of CD16-VH-RM72 chain with a flexible (G4S) 3 linker in between to generate another bispecific antibody, BM366-02 (Heavy chain: SEQ ID NO: 8; Light chain: SEQ ID NO: 9) . These bispecific constructs were shown in Figure 1.

[0187] Example 3. CHO cell production and binding activity

[0188] DNA encoding these constructs BM366-01 / BM366-02 / BM366-05 / BM366-18 were transiently transfected into Expi-CHO cells to produce recombinant bispecific antibodies, the resultant conditioned medium was harvested and BM366-01 / BM366-02 / BM366-05 / BM366-18 bispecific antibodies were purified using Protein A column. Second step of purification was performed using POROS XS. BM366-18AF was produced by transiently transfect plasmids into FucoFree TurboCHOTM cells, in which two fucosyltransferase (Fut8) alleles in the genome of CHO-K1 cells were knocked out. Fut8 can catalyze the formation of α-1, 6 fucosidic bonds, therefore, the antibodies produced by FucoFree TurboCHOTM cells are no fucosylation. More than 98%purity was achieved for BM366-01 / BM366-02 / BM366-05 / BM366-18 / BM366-18AF after these 2 steps purification process. Expression titer for these five bispecific is about 66 mg / liter for BM366-01, 75 mg / liter for BM366-02, 750 mg / liter for BM366-05, 127 mg / liter for BM366-18, and 200 mg / liter for BM366-18AF. For cell-based assays described below, endotoxins level was measured using a standard LAL-assay.

[0189] The purified antibodies were used for binding affinity measurement. Binding to CD16A was determined using biotin-CD16A 158V antigen immobilized on streptavidin probe by gator technology. Equilibrium dissociation constants (KD) for binding were determined by fitting 1: 1 Langmuir model to the data. BM366-01 and BM366-02 bind to CD16A 158V with KD value at 6.37E-011 M and 7.33E-011 M respectively (Figure 2) .

[0190] Further, we used the surface plasmon resonance (SPR) method to detect the affinity of BM366-05 / BM366-09 / BM366-18 / BM366-18AF for hCD16A 158F, hCD16A 158V, and Cyno CD16A 158F. Specifically, 0.5 μg / mL of his-tagged CD16A antigen was injected into the experimental channel at a flow rate of 10 μL / min and captured by an Anti-his mAb (10 μg / mL) immobilized on the CM5 chip. Subsequently, BM366 antibody (start from 25 nM, 2-fold dilution, 5 gradients) was injected into the experimental and reference channels at a flow rate of 30 μL / min. After 200 s association, dissociation is carried out for about 300 s. The KD value is obtained by fitting in a 1: 1 binding. As shown in Figure 3, the KD values of BM366-05 bind to hCD16A 158F and hCD16A 158V are 4.813E-10M and 8.707E-10M, respectively. The KD values of BM366-09 bind to hCD16A 158F and hCD16A 158V were 4.059E-8M and 1.392E-8M, respectively. Encouragingly, the KD values of BM366-18 for hCD16A 158F and hCD16A 158V were 3.189E-10M and 4.543E-10M, respectively, which were 30-fold and 127-fold higher than those of BM366-09. The affinity of BM366-18AF (without fucose modification) for hCD16A 158F (KD: 1.710E-10M) and hCD16A 158V (KD: 1.686E-10M) was 1.86-fold and 2.39-fold higher than that of BM366-18, respectively. These results indicate that both the bispecific antibody containing anti-CD16A VH and the absence of fucose modification can further improve the affinity of the bispecific antibody to the human CD16A antigen. In order to evaluate the efficacy in cynomolgus monkeys, we measured the affinity of BM366 antibody for Cyno CD16A (158F) . As shown in Figure 4, the KD value of BM366-09 bind to Cyno CD16A (158F) is 1.626E-8M, which is close to that of human CD16A. Although BM366-18AF bind Cyno CD16A (158F) with KD values of 2.461E-9M, which are about 7-fold higher than that of parent antibody BM366-09, it is much lower than the affinity for binding to human CD16A. BM366-18AF has an increased affinity for the CD16A 158F genotype compared to obinutuzumab, and a significant proportion (approximately 80-85%) of the population carries at least one 158F allele.

[0193] Jurkat CD16A (158F / V) -NFAT-luc reporter cells were Jurkat cell line stably transfected exogenous luciferase gene under the control of NFAT responsive element and human FCGR3A gene bearing 158F or 158V mutation respectively. When Jurkat CD16A (158F / V) -NFAT-luc reporter is engaged by bispecific antibody in the presence of target cell, nuclear factor of activated T cells (NFAT) was activated and promoted downstream luciferase expression. The amount of luciferase produced is indicative of degree of cell activation triggered by BM366-01 or BM366-02.

[0194] Ramos cells were seeded at a density of 1×10^4 cells per well at a ratio of 1: 1 together with Jurkat-NFAT-Luc2-CD16A-158F or 158 V cell. Then, a serially diluted of BM366-01 or BM366-02 antibody were added. Rituximab and Ofatumumab were positive control for BM366-01 and Inebilizumab for BM366-02. Human IgG1 and BMP01-16, an unrelated bispecific antibody was used as negative controls. After incubation for 6 hrs, luciferase expression was detected using ultra luciferase detection kit. BM366-01 showed better induction ADCC efficacy in a dose and target cell dependent manner compared with its parent antibody Ofatumumab and Rituximab with an EC50 is 30 pM for 158 F and 7 pM for 158 V genotype. Detailed information was shown in Figure 6.

[0197] Example 5. Antibody-dependent cellular cytotoxicity (ADCC)

[0198] The efficacies of BM366-01 and BM366-02 were further characterized by their ability to enhance NK cell killing in vitro. The killing was evaluated by detecting lactate dehydrogenase (LDH) released from dying cells in assay’s supernatants using PBMC as effector and CD19 / CD20 antigen expressing cell as target cell.

[0199] Firstly, Multiple PBMC were screened and one PBMC donor Lot#2407110217 was selected as suitable effector cell for the following assay. Raji cell was used as target cell to evaluate ADCC efficacy of BM366-01, with anti-CD20 Rituximab and anti-CD20 Ofatumumab as references. Nalm-6 and Ramos cells were used to evaluate ADCC efficacy of BM366-02, with anti-CD19 Inebilizumab as reference.

[0205] Next, the efficacy of BM366-01 and BM366-05 were further characterized by their B cell depletion potency, in which PBMCs from three different patients with systemic lupus erythematosus (SLE) . The result was evaluated by detecting B cell counts in each well by FACS after 24h of BM366 antibodies treatment.

[0206] The main experimental procedure is briefly described as follows: after resuscitation, PBMCs were added to a 96-well round bottom plate at 1.5x10^5 / well, followed by different concentrations of antibodies and mixed well (starting at 25 nM, 4-fold dilution, 9 concentration gradients) . After 24 h of incubation at 37℃, the supernatant was collected and frozen in a -80℃ for subsequent detection of cytokine release. The cell pellet was washed once with a FACS buffer, then resuspended and incubated for 30 min at 4℃ with eF450 labeled anti-human CD45 and Brilliant Violet 786 labeled anti-human CD19. The number of B cells per well was counted by FACS.

[0210] Similar to BM366-05, BM366-18 / BM366-18AF bispecific antibody had better B cell depletion activity than its parent antibody. As shown in Figure 14, BM366-18 has a 2-fold higher depletion activity of B cells derived from PBMCs in SLE patients compared with Obinutuzumab. The activity of BM366-18AF, the fucose free form of BM366-18, was further improved, 6.4-fold higher than that of Obinutuzumab. Moreover, BM366-18AF was more active than BM366-01 and BM366-05, although only 1.2~2.4-fold better.

[0211] Example 7. In vivo pharmacodynamics study of BM366 bispecific antibodies in hCD20 / hCD16A KI C57 mice (158VV)

[0212] B-hCD16A / hCD20 mice was obtained by cross-breeding B-hCD16A mice and B-hCD20 mice and was used in this pharmacodynamics study at 6-8 weeks of age. Mice were grouped according to their body weight and number of peripheral blood B cells, including Obinutuzumab group (1mpk) , Rituximab group (1mpk) , BM366-05 group (1.2mpk) , BM366-18AF group (1.2mpk) , and control group (n=3) . Antibodies are administered by intravenous injection. Peripheral blood was collected before administration (D0) and on the first and third days after administration, and the changes of B cells (mCD45 mCD3-mCD19) and NK cells (mCD45 mCD3-mCD19-NK1.1) in peripheral blood were detected by flow cytometry. Mice were euthanized on the third day, and the spleen, lymph nodes (from groin, armpit and neck) , and tibia bone marrow of the hind limbs were collected for flow cytometry to detect changes in B cells.

[0213] As shown in Figure 15, a significant decrease in B cells in peripheral blood was induced in both dosing groups on the first day after administration. BM366-18AF has the best clearance effect, better than Rituximab. On the third day, BM366-18AF was able to clear 99.9%of peripheral blood B cells compared to baseline (D0) , and Rituximab had clearance efficiencies of 97.1%. The clearance efficiency of BM366-18AF against B cells in lymph nodes was 92%, and Rituximab cleared B cells with efficiencies of 81% (Figure 16) . In addition, the group treated with bispecific antibodies BM366-18AF had more NK cells. In the spleen, BM366-18AF cleared 95%of B cells compared to the control group (PBS) , which was better than Rituximab (64%) (Figure 17) .

[0214] Example 8. In vivo pharmacodynamics study of BM366 bispecific antibodies in cynomolgus monkey

[0215] In vivo pharmacodynamics study was conducted by using adult Cynomolgus. Before starting the PD study, ADA-negative animals were selected for enrollment. The main experimental steps were as follows: BM366 antibody was coated with an ELISA plate at a concentration of 1μg / mL, and a 1: 100 dilution of cynomolgus monkey serum was added after 1h of blockade at room temperature. After an additional 1 h of incubation, the EZ-LinkTM NHS-biotin-labeled BM366 antibody was added. Three washes were performed after 1 h of incubation and SA-HRP was added. After adding TMB, the OD450nm value was measured. Serum with OD450nm values less than 2-fold the blank serum were ADA negative.

[0216] All antibodies were injected i. v. as a single dose. Before the start of administration (D0) , blood was collected from each cynomolgus monkey and the counts of different B cell subsets and NK cells were detected by flow cytometry as baseline. Peripheral blood was collected on D1, D3 and D5 after administration, and the number of B cell subsets and NK cells was also detected by flow cytometry. On the fifth day, one inguinal lymph node was collected from each cynomolgus monkey and flow cytometry was performed to detect the B cell subset and NK cells. Peripheral blood B cells from each cynomolgus monkey were analyzed by flow cytometry on the first, third, and fifth days following administration of 10 mpk or 12 mpk antibodies. As shown in Figure 18A, both Rituximab and BM366-18AF achieved more than 99%depletion of CD3-CD19+ B cells.

[0217] Similar depletion was observed for memory B cells and  B cells in peripheral blood (Figures 18B and 18C) . Furthermore, we evaluated the effects of these antibodies on the deep clearance of B cells in lymph nodes. As illustrated in Figure 19A, BM366-18AF exhibited excellent depletion of B cells in lymph nodes, with a 93.4%reduction in the proportion of CD3-CD19+ B cells compare to the vehicle group, which was superior to the effect of Rituximab (78.2%) . Likewise, a similar depletion trend was noted for memory B cells and  B cells in lymph nodes (Figures 19B and 19C) The data was analyzed by GraphPad Prism 8.
