[0001] Cross-reference to related applications
[0002] This application claims priority to Swedish Patent Application No. 2251441-8, filed on December 9, 2022, the disclosure of which is incorporated herein by reference in its entirety. Technical field
[0003] The present invention relates to apical sodium-dependent bile acid transporter (ASBT) inhibitors for the treatment of kidney diseases and disorders (such as cholemic nephropathy). Such treatments include reducing serum bile acid concentration, increasing urinary bile acids, and improving liver and kidney parameters. Background art
[0004] Cholemic nephropathy is a state of kidney injury / failure in patients with obstructive jaundice. Also known as biliary cast nephropathy, bile acid nephropathy, icteric nephrosis / nephropathy, or jaundice-associated nephropathy, cholemic nephropathy represents an underestimated but important cause of renal dysfunction in cholestasis or advanced liver disease with jaundice. Cholemic nephropathy is a common complication in patients with liver diseases (such as cirrhosis, alcoholic steatohepatitis, drug-induced cholestatic liver injury, and fulminant hepatitis) and is associated with high morbidity and mortality. Cholemic nephropathy is characterized by hemodynamic alterations in the liver, kidney, and systemic circulation, intratubular cast formation, and tubular epithelial cell injury, but its underlying pathophysiological mechanisms are not well understood.
[0005] Toxic bile acids have been considered to play a role in the development of cholestatic renal injury (Fickert et al., Hepatology 2013, vol. 58, p. 2056-2069; Krones et al., Dig. Dis. 2015, vol. 33, p. 367-375; Tinti et al., Life 2021, vol. 11, 1200). The less toxic bile acid norursodeoxycholic acid has been shown to improve renal injury and has been proposed as a medical treatment for cholemic nephropathy (Krones et al., J Hepatol. 2017, vol. 67, p. 110-119).
[0006] Despite the increasing interest in cholemic nephropathy and the growing understanding of the mechanisms leading to this disease, there is currently no specific treatment available for this condition. Description of the drawings

[0033] The apical sodium - dependent bile acid transporter (ASBT, also known as ileal bile acid transporter (IBAT), ISBT, ABAT or NTCP2; gene symbol SLC10A2) is expressed in the apical membrane of ileal enterocytes, renal proximal tubular epithelial cells, bile duct epithelial cells, large bile duct cells and gallbladder epithelial cells. In the ileum, where ASBT is mainly expressed, it mediates the reabsorption of conjugated bile acids for recycling back to the liver. Inhibition of ASBT disrupts the enterohepatic circulation and leads to fecal elimination of bile acids similar to surgical interruption of the enterohepatic circulation. Removal of bile acids from the enterohepatic circulation results in decreased bile acid levels in serum and liver. Thus, ASBT inhibitors have been developed for the treatment of liver diseases associated with elevated bile acid levels.
[0034] ASBT is also expressed in the proximal tubular epithelial cells of the kidney. Thus, systemically available ASBT inhibitors can also inhibit bile acid reuptake in the kidney. It is believed that this leads to increased bile acid levels in urine and increased removal of bile acids from the body via urine. Thus, targeting renal ASBT may be an additional means of increasing bile acid excretion, thereby further reducing the bile acid load in serum and liver.
[0035] Many agents that are not cleared by the diseased liver ultimately enter renal tissue, where they may cause kidney disease. It is not currently known through which mediators the diseased liver causes kidney injury, but candidate mediators are bile acids, bilirubin, and inflammatory mediators such as cytokines. It has now been found that ASBT inhibitors may also play a key role in mediating the toxic effects of bile acids in the kidney. The inventors have observed that ASBT is strongly downregulated after bile duct ligation (BDL) in mice (see Figures 2 and 3), and inhibition of renal ASBT greatly improves cholestatic nephropathy in mice. Specifically, it has been found that the proximal tubular epithelial cells of the kidney are the cause of kidney disease because these cells accumulate bile acids through the transporter ASBT. It has been found that inhibition of ASBT almost completely inhibits the uptake of bile acids into the proximal tubular epithelial cells, thereby preventing bile casts and kidney disease. This is unexpected because ASBT is mainly expressed in the proximal tubules, while bile casts form in the distal tubules. Given that many mediators may be involved in the etiology of kidney disease, it is surprising that ASBT inhibitors have such a profound effect. The similarity of the histological key features and ASBT expression in the kidneys of patients with cholestatic nephropathy suggests a therapeutic prospect in humans. Thus, inhibition of ASBT may have a protective effect on the kidney, especially in cases where the patient also has advanced liver disease.
[0036] Accordingly, in a first aspect, the present invention relates to an ASBT inhibitor (such as any ASBT inhibitor described herein) or a pharmaceutically acceptable salt thereof for use in the treatment of a kidney disease or disorder. [0037] In some embodiments, the kidney disease or disorder is a bile acid-dependent kidney disease or disorder, e.g., a kidney disease or disorder that may benefit from partial or complete inhibition of renal ASBT. Non-limiting examples of bile acid-dependent kidney diseases or disorders include cholestatic nephropathy, chronic kidney disease, hyperbilirubinemia, renal dysfunction in obstructive jaundice, age-induced impairment of renal mitochondrial function, renal inflammation, acute kidney injury (AKI), renal ischemia / reperfusion injury (IRI), chronic kidney disease (CKD), chronic renal insufficiency, end-stage renal disease (ESRD), renal proximal tubular injury, type 1 hepatorenal syndrome, type 2 hepatorenal syndrome, and acute-on-chronic liver disease.

[0314] In some embodiments, the ASBT inhibitor is administered orally. Since ASBT is mainly expressed in the ileum (where it is commonly referred to as IBAT), an ASBT inhibitor does not need to be systemically available. In fact, the systemic absorption of the vast majority of known ASBT inhibitors is low, e.g., less than 10%. However, since ASBT is also expressed in the proximal tubular cells of the kidney, systemically available ASBT inhibitors can also inhibit the reuptake of bile acids in the kidney. It is believed that this may lead to an increase in the level of bile acids in the urine and an increase in the removal of bile acids from the body through the urine. Thus, compared to non-systemically available ASBT inhibitors that act only in the ileum, systemically available ASBT inhibitors that act not only in the ileum but also in the kidney are expected to result in a greater reduction in bile acid levels. Therefore, targeting renal ASBT may be an alternative or additional means of increasing bile acid excretion in serum, liver, and kidney and reducing bile acid load in serum, liver, and kidney. Thus, in some embodiments, the ASBT inhibitor is systemically available. In some embodiments, the systemic absorption of the ASBT inhibitor is between about 10% and about 100%, e.g., between about 10% and about 75%, between about 10% and about 50%, between about 10% and about 25%, between about 25% and about 100%, between about 25% and about 75%, between about 25% and about 50%, between about 50% and about 100%, between about 50% and about 75%, or between about 75% and about 100%. In some embodiments, the systemic absorption of the ASBT inhibitor is greater than about 10%, e.g., greater than about 15%, e.g., greater than about 20%, e.g., greater than about 25%, or e.g., greater than about 30%. In some embodiments, the systemic absorption of the ASBT inhibitor is about 10%, about 15%, about 20%, about 25%, about 30%, about 40%, about 50%, or higher.
[0315] In some embodiments, the ASBT inhibitor is administered subcutaneously. It has been found that subcutaneous administration of an ASBT inhibitor can result in high bioavailability and a constant exposure lasting more than 24 hours. Thus, compared to oral administration of an ASBT inhibitor, subcutaneous administration of an ASBT inhibitor can provide a different and potentially more sustained bile acid modulating effect. This effect can be used to treat diseases in which stronger inhibition of bile acid cycling is required or when oral administration is unlikely to provide a benefit (i.e., when bile flow is blocked).
[0316] In some embodiments, oral administration of an ASBT inhibitor is combined with subcutaneous administration of an ASBT inhibitor. Such combination therapy may have additive or synergistic effects and may result in even greater excretion of bile acids. Examples of ASBT inhibitors that are not systemically available include, but are not limited to: elobixibat, obeticholic acid, maralixibat, volixibat, and linerixibat. After oral administration of these ASBT inhibitors, systemic absorption is less than 10%. Further examples of suitable ASBT inhibitors are disclosed, for example, in WO 2019 / 234077, WO 2020 / 161216, WO 2020 / 161217, WO 2021 / 110884, WO 2021 / 110885, WO 2021 / 110886, WO 2021 / 110887, and WO 2022 / 029101.
[0317] In some embodiments, a patient does not respond to treatment with an orally administered, non-systemically available ASBT inhibitor. Since subcutaneous administration of an ASBT inhibitor results in the regulation of renal ASBT, it is believed that subcutaneous administration of an ASBT inhibitor may result in stronger regulation of ASBT than oral administration of the compound.
[0318] In some embodiments, a patient cannot tolerate treatment with an orally administered, non-systemically available ASBT inhibitor, for example when the patient experiences severe side effects (such as severe diarrhea). Since subcutaneous administration of an ASBT inhibitor also results in the regulation of renal ASBT, bile acids are excreted not only in the feces but also in the urine. This is expected to result in a reduced incidence of diarrhea.
[0319] Also provided herein are methods of treating a kidney disease or disorder as defined herein in a subject, the methods comprising administering to the subject a therapeutically effective amount of an ASBT inhibitor (e.g., any of the ASBT inhibitors described herein) or a pharmaceutically acceptable salt thereof. [0357] ASBT inhibitor compounds 1 to 5 were used in the experiments described herein. The potency (hIBATIC 50 ), permeability, and bioavailability of these compounds are shown in Table 1 below. The values were determined using assays such as those described in WO 2020 / 161217.
[0358] Table 1.
[0359]
[0360] Example 1
[0361] Biliary nephropathy mouse model - bile duct ligation (BDL) in mice
[0362] Time-resolved events in the liver and kidneys after extrahepatic common bile duct ligation were evaluated.
[0363] Male mice (C57BL / 6n) were used. After completion of the quarantine and acclimation periods, animals were randomly assigned to experimental groups (n = 5 mice per group and time point) based on body weight. On day 0, animals underwent bile duct ligation or sham surgery. Animals were euthanized after 1 or 3 days or after 1, 3, 6, 9, or 12 weeks, and samples (bile, blood, urine, liver, and kidney tissues) were collected and analyzed.
[0364] BDL led to excessive accumulation of bile in the gallbladder and caused an increase in serum ALT and AST, which peaked on day 1 and then decreased as the liver adapted to cholestasis but remained higher than control values ( Figure 1A and 1B ). Alkaline phosphatase (ALP) enzyme activity showed a time-dependent increase after BDL ( Figure 1C ).
[0365] Analysis of the expression of bile acid transporters showed that ASBT was strongly downregulated after BDL. Figure 2A -D shows the compensatory changes in bile acid transporter expression during 12 weeks after BDL or sham surgery. Figure 2A : Sinusoidal uptake transporter NTCP; Figure 2B : Sinusoidal uptake transporter Cyp7a1; Figure 2C : Apical transporter Bsep; Figure 2D : Sinusoidal efflux transporter MRP4. Figure 3A -E shows the expression of apical uptake transporters ASBT ( Figure 3A ) and OATP1a1 ( Figure 3B ), apical efflux transporter MRP4 ( Figure 3C ), and basolateral efflux transporters MRP3 ( Figure 3D ) and OSTα ( Figure 3E ) during 12 weeks after BDL or sham surgery.
[0366] Similar studies were conducted using female mice.
[0367] Example 2
[0368] Effect of ASBT inhibition in BDL mice
[0369] The short-term effect of oral administration of compound 1 on the urinary excretion of bile acids in BDL mice was evaluated.
[0370] Female mice (C57BL / 6n) were used. After completion of the quarantine and acclimation periods, the animals were randomly divided into experimental groups (n = 4 - 7 mice per group) based on body weight. On day 0, the animals underwent bile duct ligation. Starting on day 7, the animals were treated with vehicle or different doses of compound 1 for 5 days, as shown in Table 2. Compound 1 was administered orally by gavage, twice daily. Spontaneous urine samples were collected by bladder catheterization before BDL and on days 7, 8, 9, 10, 11, and 12 after BDL, and the concentration of bile acids in the urine was determined.
[0371] Table 2.
[0372]
[0373] The dose - dependent study showed that during the 5 - day analysis period, urinary bile acids increased strongly due to ASBT inhibition. After treatment with compound 1, the urinary excretion of bile acids increased sharply, with bile acid concentrations in all treatment groups at least about 20 - fold higher. The results are shown in Figure 4 (total concentration of all bile acids) and Figure 5A -C (concentrations of the individual bile acids tauro - α / β - muricholate, taurocholate, and taurocholic acid sulfate, respectively).
[0374] Example 3
[0375] Effect of ASBT inhibition on biliary nephropathy in bile duct - ligated mice
[0376] The long - term effect of compound 1 on kidney injury in BDL mice was investigated by histology, clinical chemistry, and in vivo imaging.
[0377] Female mice (C57BL / 6n) were used. After completion of the quarantine and acclimation periods, the animals were randomly divided into experimental groups (n = 15 mice per group) based on body weight. On day 0, the animals underwent bile duct ligation or sham surgery. The animals were treated with vehicle or with compound 1 at a dose of 60 mg / kg, as shown in Table 3. Compound 1 was administered orally by gavage, twice daily. Spontaneous urine samples were collected by bladder catheterization before BDL and on days 1 and 1, 2, 3, 4, 5, and 6 weeks after BDL. At the end of the experiment (week 6), blood, urine, bile, and tissue samples were collected.
[0378] Table 3.
[0379]
[0380] Severe fibrosis and cystic dilation of the renal tubules were observed in the BDL vehicle control, and Compound 1 almost antagonized it to the level of control animals without BDL. The urinary kidney injury markers neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule-1 (KIM-1) were strongly reduced by Compound 1 (see Figure 13 and Figure 14 respectively). In addition, serum BA was reduced by approximately 90% (total bile acids see Figure 6 , individual bile acids see Figures 7 - 10 ), and urinary BA increased significantly. In vivo imaging with Evans blue showed severe vascular leakage in the kidneys after BDL, which was completely reversed by Compound 1.
[0381] The levels of liver enzymes (ALT, AST, and ALP), total bilirubin, and blood urea nitrogen (BUN) after 6 weeks of treatment are shown in Figure 15. The expression of different bile acid transporters after 6 weeks of treatment is shown in Figures 16 and 17.
[0382] In the BDL-vehicle group, approximately 50% of the mice died during the 6-week period, while no deaths occurred in the group of BDL mice treated with Compound 1 (see Figure 12 ). In addition, the weight loss caused by BDL was strongly improved by ASBT inhibition (see Figure 11 ). A similarly designed study using male mice (6 per group) confirmed the therapeutic efficacy of Compound 1 observed in the female cohort.
[0383] Example 4
[0384] Study of the treatment time window
[0385] The effect of the time period before starting treatment with Compound 1 on renal injury in BDL mice was evaluated.
[0386] Male mice (C57BL / 6n) were administered. After completion of the quarantine and acclimation periods, the animals were randomly divided into experimental groups (n = 7 mice per group) based on body weight. On day 0, the animals underwent bile duct ligation or sham surgery. Starting on day 3, 21, 42, or 63, the animals were treated with vehicle or with Compound 1 at a dose of 60 mg / kg for 28 days, as shown in Table 4 below. Compound 1 was administered p.o. twice a day. The animals were weighed daily. Blood, urine, bile, and tissue samples were collected at the end of the experiment.
[0387] Table 4.
[0388]
[0389] Graphs of the survival rate (%) of the four groups are shown in Figure 18. Graphs of the weight change (%) of the four groups after sham surgery or BDL are shown in Figure 19.
[0390] When the administration started on day 3, 4-week treatment with Compound 1 completely prevented kidney injury and reversed weight loss. When the administration started on day 21, Compound 1 almost completely reversed kidney injury and reversed weight loss. When the administration started on day 42, Compound 1 partially reversed kidney injury and reversed weight loss. When the administration started on day 63, Compound 1 had a minimal effect.
[0391] Example 5
[0392] Comparison of different ASBT inhibitors
[0393] Male mice (C57BL / 6n) were used. After completion of the quarantine and acclimation periods, the animals were randomly divided into experimental groups based on body weight (n = 12 mice per bile duct ligation group and n = 5 mice for sham operation control). On day 0, the animals underwent bile duct ligation or sham operation. Starting on day 3, the animals were treated with vehicle or an ASBT inhibitor for 19 days as shown in Table 5 below. The ASBT inhibitor was administered p.o. once daily. The animals were weighed daily. On day 21, blood samples were collected 2 hours and 6 hours after the last dose. On day 24, the animals were sacrificed and samples (urine and blood; liver, kidney, heart, and spleen tissues) were collected and analyzed.
[0394] Table 5.
[0395]
[0396] After 19 days of treatment, it was observed that Compounds 1 and 2 decreased serum bile acids and increased urinary bile acids. Figure 20A and 20B show the concentrations of total bile acids in blood and urine on day 21 (i.e., the 19th day of treatment), respectively. After 18 days of treatment, Compounds 1 and 2 also decreased urinary NGAL; see Figure 21 .
[0397] Example 6
[0398] Comparison of different ASBT inhibitors
[0399] Male mice (C57BL / 6n) were used. After completion of the quarantine and acclimation periods, the animals were randomly divided into experimental groups (n = 10 mice per bile duct ligation group) based on body weight. On day 1, the animals underwent bile duct ligation. Starting from day 4, the animals were treated with either vehicle or an ASBT inhibitor for 5 days as shown in Table 6 below. The ASBT inhibitor was administered p.o. once daily. The animals were weighed daily. On day 6, the animals were moved to individual cages for cumulative overnight fecal collection (days 6 to 7). On day 8, blood samples were collected 2 and 6 hours after the last dose. The animals were then sacrificed and samples (urine and blood; liver, kidney, and ileum tissues) were collected and analyzed.
[0400] Table 6.
[0401]
[0402] After 5 days of treatment, all compounds were observed to reduce serum bile acids and increase urinary bile acids. The concentrations of total bile acids in blood and urine on day 8 (i.e., day 5 of treatment) are shown in Figure 22A and 22B respectively. After 5 days of treatment, compound X and compound 5 significantly reduced urinary NGAL; see Figure 23 .
[0403] The concentrations of compound X and compound 5 in serum (2 and 6 hours after dosing) and urine (up to 6 hours after dosing) on day 8 are shown in Figures 24 and 25, respectively.
