Abstract
Description
[0001] MEDICAL INSTRUMENT TO REMEDY CHOKING
[0002] DESCRIPTION
[0003] TECHNICAL SECTOR
[0004] The present invention belongs to the field of health, and more specifically to the field of first aid instruments.
[0005] The objective of the self-management health tool developed is to provide society with a safe and effective alternative remedy for choking.
[0006] BACKGROUND OF THE INVENTION
[0007] According to data from the National Institute of Statistics (INE), choking is the third leading cause of unnatural death in Spain. Emergency medical assistance and the Heimlich maneuver cannot prevent the high number of deaths that occur in Spain each year. In other words, society lacks the means to remedy choking, and when a certain level of deprivation is reached, that is, when it becomes very intense, it transforms into necessity.
[0008] The Heimlich maneuver must be performed following a protocol of a series of established guidelines that, without sufficient experience, are difficult to perform at least successfully. Since choking causes a generalized state of stress, it can generally cause nervousness and consequently unintentional bad practices that lead to accidental injuries to those affected by choking. It should be noted that in specific situations or cases, such as when the victim cannot speak, cough forcefully, or breathe adequately, and with obese people or women at the end of pregnancy, the procedure must be different depending on the case. Therefore, if you are not a healthcare professional or do not have sufficient experience, it is truly complicated to perform the Heimlich maneuver or to establish the appropriate, adequate, or pertinent guidelines for the specific situation on-site.
[0009] The health instrument is based on the high and alarming mortality rates that occur annually due to asphyxiation. Consequently, it was created to support the Heimlich maneuver. Currently, this maneuver is the
[0010] 1
[0011] REPLACEMENT SHEET (RULE 26) the only remedy for choking. On the other hand, there are devices that are supposedly used to clear the airway. One of these devices is registered as a suction device in the United States; however, the Food and Drug Administration (FDA) declares that it has not approved medical instruments that specifically open the airway or facilitate breathing in emergency situations due to asphyxiation. Furthermore, both the Spanish Cardiopulmonary Resuscitation Council (CERCP) and the European Research Council (ERC) do not include the use of these devices to treat foreign body airway obstruction (FBO).
[0012] Considering the appropriate standard established by health professionals of not introducing hands or improvised tools through the oral cavity, and noting the medical instrument's status as an invasive medical device, which will be used by people without first aid training, its optimal inclusive design has been planned to be harmless, making it impossible to aggravate the problem to be solved.
[0013] Furthermore, by providing the appropriate measures and configuring the arrangement of the crucial attributes that provide optimal operation to the sanitary instrument, it has been developed by combining the decisive effectiveness, which is shown and demonstrated, of the secondary function found in the figure and attributes that a well-known food such as sausage has, and considering the practices and revealing events detailed below.
[0014] On the one hand, there are people who, for particular reasons, insert and extract sausages measuring 20 centimetres in length by 3 centimetres in diameter into their oral cavity, other people swallow sausages measuring 15 centimetres in length by 2 centimetres in diameter, for other reasons there are people who insert more than 100 narcotic capsules measuring 2 centimetres in diameter into their stomach, and for obvious reasons we have verified the suitability of the sausage, checking its insertion and extraction, confirming the effectiveness provided by its flexibility and adaptability necessary for the finishing and manufacturing of the medical instrument in this argument, likewise, a person in a choking situation would have sufficient motivation to proceed with the intervention or be assisted with the medical instrument.
[0015] On the other hand, a person affected by choking performed the questionable
[0016] 2
[0017] REPLACEMENT SHEET (RULE 26) imprudence resulting in the happy outcome of inserting a flexible tube of approximately 1.5 centimetres in diameter, relieving himself instantly, without causing any harm, in another situation due to choking, a person used a toothbrush as a remedy, accidentally swallowing it, managing to relieve himself instantly without consequences, obviously they had to remove the toothbrush in the hospital without further significance with respect to his health.
[0018] Furthermore, in relation to the introduction of potentially traumatic objects without any safety measures, a medical team found 20 spoons, 4 forks, toothbrushes and jewelry in the stomach of a person who arrived at the hospital after months of discomfort. The unusual episode was resolved through surgery, with the only complication for the patient being having to remain in hospital due to the injuries caused by the metals in his stomach.
[0019] In short, it is evident, and has also been confirmed by assessments carried out by healthcare professionals, that the insertion of a traumatic or atraumatic object into the oral cavity involves a pharyngeal phase with the well-known descent of the epiglottis.
[0020] In another order of things, in the field of first aid instruments, the medical instrument is equated as a health safety measure to a defibrillator. It is equally evident that it is positioned as an optimal tool for catering establishments and spaces where meals are served in general. It is also suitable for the home. It is noteworthy in this context that the medical instrument replaces any unsuitable object that a person affected by choking might improvise for use, whether a harmful toothbrush or a rigid or flexible device. It is noted that performing the Heimlich maneuver in a single- or two-person situation is presumed to be counterproductive and complicated without adequate training.
[0021] EXPLANATION OF THE INVENTION
[0022] On the one hand, the medical instrument in this argument, belonging to the field of first aid instruments, is characterized by being an invasive medical device. Regarding its purpose, it is designed to clear the airway, with the main objective of addressing an urgent situation or choking symptoms in order to avoid asphyxiation, its mission being to direct the
[0023] 3
[0024] REPLACEMENT SHEET (RULE 26) food stuck in the esophagus.
[0025] Regarding its purpose, to protect the sensitive pathway involved in the intervention and facilitate the mobility of stuck food, we benefit from the biocompatibility of extra virgin olive oil, which is primarily used as a natural lubricant due to its density and nutritional status. It is also a great ally against throat inflammation and favorably influences the development of sensory desensitization techniques. This technique is determined by the choking situation and is performed to promote a suitable modification of the threshold for the onset of the heightened, normal, or attenuated pharyngeal reflex, applying with great caution and attention, progressive bilateral stimulation to the tongue root, posterior pharyngeal wall, and adjacent structures.
[0026] Regarding its use, the medical instrument is indicated for adults and allows for intervention in self-management mode and assistance mode in conscious patients with an absent or attenuated and controlled pharyngeal reflex, where the choking was caused by a non-traumatic food. Self-management mode is excluded, and its size is reduced, suitable for children. The presentation of this instrument is postponed pending validation of the medical instrument for this reasoning.
[0027] Regarding the characteristics of the medical instrument, it is made entirely of medical-grade silicone. First, it has a circular oral stop measuring 3 to 8 centimeters in diameter and 0.5 to 3 centimeters wide. This oral stop primarily prevents the entire medical instrument from accidentally being inserted into the stomach. Furthermore, as a distinctive and equally important and convenient feature, it features pictograms on the front and back sides to help you position the medical instrument properly in the oral cavity and keep it securely in place during the procedure, both in assisted and self-management mode.
[0028] Adjacent to the back of the oral stop is a truncated cylinder measuring 19 centimeters, made up of three distinct sections. First, there is a cylindrical section or straight cylinder measuring 9 centimeters in length and a diameter of between 1.5 and 2.5 centimeters. This cylindrical section, adjacent to the back of the oral stop, has a series of closed curved lines. These closed curved lines are modeled in relief comparable to the Braille writing system, for sensorial localization.
[0029] 4
[0030] REPLACEMENT SHEET (RULE 26) the intervention has started in self-management mode, after visual location, and its function is to indicate the insertion measurements, being established for the elderly and large adults over 1.80 meters tall and over 80 kilograms in weight, medium from 1.60 to 1.75 meters tall and 60 to 75 kilograms in weight and small from 1.50 to 1.60 meters tall and 45 to 55 kilograms in weight, and being indicative sizes, no calculations or fixed insertion lengths are specified, consequently, the insertion measurements have been established for large adults between 17 and 18 centimeters, medium between 16 and 17 centimeters and small between 15 and 16 centimeters. It should be added that, considering that an older person has an age range starting from 59 years onwards, adult from 27 to 59 years, young person from 15 to 27 years and adolescent from 12 to 15 years, and considering corpulence as a conditioning factor, an older person or an adolescent can have the weight, height and robustness of a small adult, and likewise, the anatomical structure, in relation to the length of the neck, determines the insertion measurements.
[0031] Next to the first cylindrical section, there is a second ovoid section or nexus that progressively increases its diameter from 1.5 to 2.5 centimeters in an interval or length of 1 centimeter, then there is a third cylindrical section or truncated cylinder of 9 centimeters in length and a diameter of between 1.5 to 2.5 centimeters.
[0032] In this cylindrical section, at a distance of between 5.5 and 8.5 centimeters from its truncated elliptical base, a numbering (1 2 3 4) is located, modeled in relief comparable to the Braille writing system for sensorial location in self-management mode, which indicates a 4-second pause, an instant or phase where the sanitary instrument is positioned and oriented to lubricate its path and the stuck food in advance. Next, at a distance of between 5 and 8 centimeters from the truncated elliptical base, a closed curved line is also located, also modeled in relief comparable to the Braille writing system for sensorial location in self-management mode, which indicates a pause to evaluate the category of the pharyngeal reflex. Both indications are indicative, these processes being conditioned by the approximation of the truncated elliptical base to the sensitive pharyngeal area.
[0033] Finally, at the opposite end to the oral stop is the fundamental truncated elliptical base of 2 by 1.8 centimeters that serves as an oropharyngoesophageal guide blade and dynamic epiglottis, and stands out as the main element, due to its decisive and vital role. In its elliptical base is located a main concavity (5) or "concavity (1) seen in the figures with the modifications", deposit and custodian of the
[0034] 5
[0035] REPLACEMENT SHEET (RULE 26) food whose dimensions are 1.5 centimeters in diameter by 1 centimeter deep. This concavity is outlined by a toroid, which, when placed on the truncated elliptical base, appears as an atraumatic semi-toroid.
[0036] Inside the main concavity (5) or “concavity (1) seen in the figures with the modifications”, there is a second concavity (6) or “concavity (2) seen in the figures with the modifications”, 0.8 centimetres in diameter by 0.4 centimetres deep, whose function is to keep an adequate amount of oil deposited to subsequently lubricate in advance the oropharyngeal path made by the sanitary instrument, in addition to facilitating the movement of the stuck food.
[0037] The truncated elliptical base manages the displacement, while directing the food towards the esophagus, which simultaneously welcomes and synergistically safeguards the participating main concavity (5) or “concavity (1) seen in the figures with the modifications”.
[0038] The configuration of the medical instrument then performs an action comparable to the resultant force caused by the displacement of a food bolus, applying it to the epiglottis, contributing to its descent. This action is mitigated and supported by the soft texture and elastic structure of the extension, formed at the convergence of the major generatrix of the truncated cylinder with the major axis of the truncated elliptical base.
[0039] The process can also be synchronized depending on the location of the food, directing it and flexing the epiglottis simultaneously.
[0040] For the finishing and manufacturing of the sanitary instrument with the purpose of improving its practical function and optimizing its resolution capacity, small modifications are valued such as changing the lubrication method, diametral variation, forming transverse holes on a truncated elliptical base, more superficial main concavity (1), and different curved line finish to check pharyngeal reflex and 4-second pause indication.
[0041] In relation to the recommendations for use, first of all, the extreme delicacy and firm determination that must be applied in the handling and use of the health instrument is highlighted, particularly in the development of the desensitization technique, this process being the most determining setback of the
[0042] 6
[0043] REPLACEMENT SHEET (RULE 26) intervention, due to the mitigation and possible reappearance of the pharyngeal reflex between phases. Secondly, especially in assistance mode, close attention must be paid, always remaining alert, conditioning the process to the sensory perception of the affected person. Likewise, it is effective to suggest, as a specific mitigation method, clenching the fists tightly enough to overlap or alleviate the unique sensation implied by the intervention, combined with the prevailing symptoms of choking. Furthermore, both in self-management mode and in assistance mode, the elevation and anterior displacement of the larynx should be avoided, which could cause the food to oscillate if it is located in the ventricular folds. Likewise, in both assistance modes, try not to speak or make sounds to request help, in order to avoid movement or opening of the vocal cords.
[0044] Furthermore, in the event of a lack of adequate training, and in order to speed up the process, it is advisable to establish a distribution of tasks, through multi-person collaboration in assistance or self-management mode, mainly by facilitating the interpretation of warning symbols and instructions for use to the person who will perform the intervention, while they prepare the first phase of the procedure, and also cooperating until the conclusion of the intervention.
[0045] On another note, to ensure optimal use of the healthcare instrument, ensuring the reliability and speed necessary to carry out and complete a successful intervention, educational dissemination is crucial and advisable, as is the provision of appropriate training by authorized entities, primarily for emergency medical technicians and, equally, for professionals in the catering sector.
[0046] The medical instruments included in the first aid kit are complemented by instructions for use, contraindications, warning pictograms or symbols, nitrile gloves, anesthetic spray, single-dose olive oil, sanitary wipes, a syringe, and a tray necessary for hygienic lubrication, with separate compartments for the medical instrument and syringe.
[0047] The preparation for the execution and conclusion of the intervention described below is presumed to be too extensive, but in practice is brief and decisive.
[0048] The procedure for performing the intervention is established in 5 phases. This is
[0049] 7
[0050] REPLACEMENT SHEET (RULE 26) makes prior notice to the health emergency services.
[0051] The first phase of preparation, which is preferably completed in a time interval of 15 to 20 seconds, consists of putting on gloves or, failing that, sanitizing the hands with wipes. Next, a local anesthetic spray is applied to the back of the tongue or sensitive pharyngeal area by pressing the valve for half a second. The tray is placed near the person being assisted. Next, the sanitary instrument is placed in its compartment, and both the sanitary wipes and olive oil are ready for use. Next, the syringe is loaded and, by rotating the sanitary instrument, the contour of the second cylindrical section is oiled, from the curved line indicative of the evaluation of the degree of pharyngeal reflex, to its truncated elliptical base.Lubrication should be carried out several times if necessary. It is advisable not to spread or manipulate the oil with your hands. For subsequent uses after the first lubrication, place the filled syringe in the tray.
[0052] In the second phase of evaluation, which is completed within a time interval of 5 to 10 seconds, the pharyngeal reflex category is checked. This is done by holding the oral stop of the medical instrument with the right hand and the ovoid nexus with the left. The instrument is then positioned in the oral cavity and preferably inserted, noting contact between the truncated base and the sensitive oropharyngeal area, or locating the curved line indicative of a pharyngeal reflex pause at the level of the dental arch. If we detect an absent pharyngeal reflex category, the sensory desensitization technique is not performed. Otherwise, if the pharyngeal reflex category is increased, normal, or attenuated, the process is paused. We withdraw or accompany the extraction of the medical instrument, slightly and appropriately reducing the expulsion reflex effect. We place it in its compartment. Once contact with the oil is established, we use the wipes.
[0053] The third phase of desensitization is established within a maximum time interval of 120 seconds, conditioned by the mitigation and the different categories of the pharyngeal reflex. To facilitate and expedite its development, the medical instrument is oiled, and the sensory desensitization technique is performed, with great delicacy and determination, through cycles of 10 to 15 seconds with 5-second pauses, at approximate intervals and conditioned by the mitigation of the pharyngeal reflex, from 30 to a maximum of 120 seconds, if it is categorized as increased, from 25 to a maximum of 90 seconds normal, and from 15 to a maximum of 60 seconds attenuated. As a factor
[0054] 8
[0055] REPLACEMENT SHEET (RULE 26) determining, depending on the skill applied to perform the desensitization, the sensory response or reaction of the affected person may be affected, consequently, both the cycles and the intervals may be shortened or prolonged. Likewise, the different categories of the pharyngeal reflex largely condition the time and development of the stimulation, and the mitigation of the increased or normal pharyngeal reflex must be approached with special caution and skillful determination. Finally, once the category of absent or attenuated pharyngeal reflex is achieved and controlled, we extract the medical instrument, holding the oral stopper in the right hand and the ovoid nexus in the left, we will place it in its compartment, and once contact with the oil is established, we will use the wipes.
[0056] The fourth phase of lubrication is established in a time interval of 20 to 50 seconds, conditioned by the possible reappearance of the pharyngeal reflex. If this reappears, the process must be paused to perform a sensory desensitization established in a maximum interval of 30 seconds, and once the absent or attenuated pharyngeal reflex is achieved and controlled, lubrication is performed, preferably using a wipe to hold the ovoid nexus with the left hand, and placing the medical instrument in a vertical position, we deposit 2 or 3 drops of oil in the concavity (5) or "concavity (1) seen in the figures with the modifications", and place the syringe in its compartment.Next, we place the sanitary instrument in a horizontal position, raising the truncated base to keep the oil in the concavity (6) or “concavity (2) seen in the figures with the modifications”, we place the truncated base in the oral cavity, remove the wipe, and keeping the head of the affected person straight, we slide the sanitary instrument, preferably noting the contact of the truncated base with the sensitive oropharyngeal area, or confirming the positioning of the numerical signal (1234) at the height of the dental arch, and confirming the orientation of the sanitary instrument, we pause the intervention for 4 seconds so that the oil lubricates the oropharyngeal path to the stuck food.
[0057] The fifth phase of completion is established within a time interval of 20 to 50 seconds, conditioned by the possible reappearance of the pharyngeal reflex. If it reappears, the process must be paused to perform sensory desensitization, established within a maximum interval of 30 seconds. This phase is performed immediately after the 4-second pause, if the pharyngeal reflex is absent, or once the absent or attenuated pharyngeal reflex has been achieved and controlled. The medical instrument is then positioned in the oral cavity and slid until the signal is reached.
[0058] 9
[0059] REPLACEMENT SHEET (RULE 26) numerical pause of 4 seconds, and bending down or flexing the head slightly downwards, the curved lines of the insertion measurements are located, depending on whether the assisted person is a large, medium or small adult, to adequately establish the introduction of the sanitary instrument, we continue the insertion process and at the time of positioning the curved line of insertion measurements of 15 centimeters, at the height of the dental arch, if choking persists, the process must be predisposed with culminating attention conditioned by the expected sensation of relief. Finally, successfully concluding the intervention, the sanitary instrument is slowly withdrawn, or its extraction is accompanied, slightly and appropriately reducing the expulsion reflex effect, and otherwise, concluding the intervention without success, because the food is located in the ventricle folds, the sanitary instrument is also withdrawn, and the Heimlich maneuver is performed.
[0060] Considering the measurements or dimensions of the oropharynx up to the pharyngoesophageal junction of an adult person, to set the diameter of the second cylindrical section at 1.8 centimeters and adapt an optimal insertion of the sanitary instrument, in addition, achieved its appropriate texture in medical grade silicone that in synergy with olive oil manages to undertake an adequate sensory desensitization and motivates a delicate atraumatic insertion or oral phase, as well as its fundamental truncated elliptical base that performs an optimal pharyngeal phase firmly and safely directing the stuck food towards the esophagus, it is confirmed that with absent or attenuated and controlled pharyngeal reflex, and excluding the first preparation phase, the interventions are completed in a few seconds, experiencing a singular sensation overlapped by the prevailing anguish that choking implies, highlighting that the intervention does not imply aggravation, even if the food causing the obstruction is fragmented and positioned in the upper and lower part of the epiglottis, or is located in the ventricular folds.
[0061] Regarding its use, on the one hand, it is prohibited to perform the intervention if the cause of choking is a traumatic food, with sharp or piercing elements, a rasp, a thorn, a bone, or excessively hard food. It is also prohibited to perform the intervention without having successfully completed the sensory desensitization technique. On the other hand, conditions that can influence the evaluation and moderation process of the pharyngeal reflex have been described, such as uncontrolled hypersensitivity caused by physical and psychological factors, systematic conditions that can alter the pharyngeal reflex, stomach infections or gastrointestinal diseases,
[0062] 10
[0063] REPLACEMENT SHEET (RULE 26) metabolic, central nervous system, injuries such as stroke, tumors, and diseases affecting the glossopharyngeal and vagus cranial nerves. These conditions or pathological processes are factors that may make it impossible to mitigate the pharyngeal reflex and, consequently, make it impossible to perform the procedure.
[0064] In another order of things, the decision to carry out the intervention corresponds solely to the affected person, who must be informed of the instructions for use, and due to ignorance or possible incapacitation, he or she should never be assisted, without consent with him or her or in his or her place, with the person who represents him or her, the approval to carry out the intervention, noting the advantages of using the sanitary instrument in self-management mode, and in assistance mode, demonstrating that the procedure to follow is known, being absolutely sure that the intervention can be carried out properly, and most importantly, verifying that the food is atraumatic. On the other hand, if the cause of choking is traumatic food, it is strongly emphasized that performing the intervention is strictly prohibited, and after notifying the emergency health services, it is advisable to exhaust all possible alternative means, and obeying a critical life-or-death situation, if the decision is finally made to perform the intervention, all responsibility will fall on the affected person, their representative, or the person who performs the intervention without being authorized.
[0065] On the other hand, it is possible that a foreign body could be the cause of choking, and there are no impediments to performing the intervention if it is atraumatic, since, according to the Spanish Association of Pediatrics (AEP), of the total number of foreign bodies ingested by children, it is estimated that around 80% are spontaneously eliminated in the feces since they pass without difficulty through the digestive tract. However, this is not common in an adult; children of a certain age are more likely to accidentally swallow or ingest a foreign body: the use of the medical instrument in this argument is strictly prohibited if the victim is a child, an adolescent under 12 years of age, or of a build and measurements smaller than those established for a small adult.Similarly, for various reasons or motivations, psychiatric patients and older adults may ingest non-food objects or items. In these cases, the presence of a trained person to perform the intervention is crucial due to the victim's possible incapacity. On the other hand, if the choking is caused by a traumatic object,
[0066] 11
[0067] REPLACEMENT SHEET (RULE 26) strongly emphasizing the strict prohibition of performing the intervention, it is advisable, after having notified the emergency health services, to exhaust all possible alternative means, and obeying a critical life or death situation, if the decision is finally made to perform the intervention, all responsibility will fall on the affected person, representative or the person who performs the intervention without being authorized.
[0068] In addition to this, considering age and some systematic conditions as motivating factors that alter the pharyngeal reflex, diminishing it or even making it non-existent, and noting that there is a considerable percentage of the population with an absent pharyngeal reflex, the health instrument not causing aggravation by performing interventions with symptoms of choking, and noting that the intervention is harmless in cases of tracheal obstruction, it is resolved not to detect possible injuries or setbacks as a consequence of its adequate and established use.
