COMPOSITIONS AND METHODS FOR TREATING INFECTIONSBACKGROUND

[0001] Antimicrobial resistance is a major global health crisis. In particular, hospital-acquired pneumonia and ventilator-associated pneumonia caused by multidrug-resistant (MDR) Pseudomonas aeruginosa, Acinetobacter baumannii, and Enterobacterales are major causes of morbidity and mortality. The US Centers for Disease Control (CDC) listed carbapenem-resistant Acinetobacter as an urgent threat and multidrug-resistant Pseudomonas aeruginosa as a serious threat. Consistent with the global nature of these resistant bacteria, the World Health Organization (WHO) has designated carbapenem-resistant Acinetobacter baumannii (A. baumannii) , and carbapenem-resistant Pseudomonas aeruginosa (P. aeruginosa) as organisms with a critical need for new drugs.

[0002] Polymyxins were first isolated in 1947 from the Gram-positive soil bacterium Bacillus polymyxa of which polymyxin B and colistin (polymyxin E) became commonly used in clinical treatment until the 1980s, when the associated nephrotoxicity caused a decline in usage. However, with the increasing prevalence of multidrug-resistant gram-negative bacteria, in particular carbapenemase producing Enterobacterales, A. baumannii, and P. aeruginosa, the use of polymyxin antibiotics as salvage therapy began to again increase despite the known nephrotoxicity and neurotoxicity associated with polymyxins.

[0003] Polymyxins act by binding to lipopolysaccharides in the outer cell membrane of Gram-negative bacteria leading to disruption of the outer cell membrane, leakage of intracellular contents, and bacterial cell death.

[0004] The effective use of polymyxin B and colistin in the clinic is limited due to several major safety issues. The therapeutic window of polymyxin B and colistin is very narrow due to significant dose-limiting nephrotoxicity, which can occur in up to 60%of patients. Acute toxicity severely limits the maximal dose of intravenous administration. Also importantly, their limited exposure in the lungs following intravenous administration and binding to lung surfactant render them often inefficacious for pulmonary infections. Therefore, there is an urgent need to develop new therapies with a much-improved therapeutic window and efficacy.SUMMARY

[0005] The instant disclosure provides compositions and methods are provided for treating bacterial infections. The composition comprises BRII-693, a cyclic peptide, or the salts thereof.

[0006] In accordance with one embodiment of the present disclosure, provided is a method for treating a bacterial infection in a subject in need thereof, comprising administering to the subject a daily amount of greater than 300 mg up to 800 mg of a compound of the structure or a pharmaceutically acceptable salt thereof for at least 5 days.

[0007] In some embodiments, the infection is a lung infection, a urinary tract infection, an intra-abdominal infection, or a blood infection. In some embodiments, the bacterial infection is bacteremia.

Example 3. Clinical Therapeutic Dose Prediction

[0108] This example describes the determination of the clinical therapeutic doses based on modeling and simulation with the Phase I pharmacokinetic information, non-clinical pharmacokinetics (PK)  / pharmacodynamic (PD) data, preclinical toxicity study results, and Phase I safety and tolerability data. To predict the human therapeutic doses of BRII-693, a series of simulations were carried out using the software program.

[0129] A total of 112 carbapenem-resistant A. baumannii (CRAB) and 21 carbapenem-resistant P. aeruginosa (CRPA) clinical isolates were collected through the CHINET program from 26 provinces between 2018 and 2019 in China; 100 CRAB and 100 CRPA clinical isolates were collected through PUMCH from 28 provinces between 2018 and 2022. Carbapenem resistance was defined by a minimum inhibitory concentration (MIC) value of meropenem or imipenem ≥8mg / L. No overlapping isolates were collected from the same hospital in the same year. Susceptibility testing was blinded to drug identity and conducted using the Clinical and Laboratory Standards Institute (CLSI) reference of broth microdilution method. Commercially available antibiotics, including colistin and newly marketed β-lactam / β-lactamase inhibitors (BL / BLIs) , were used as comparators.
