Abstract
Description
Technical Field
[0001] The present invention discloses methods for treating non-GCB DLBCL in a subject, comprising administering to a subject in need thereof (S)-7-(1-acryloylpiperidin-4-yl)-2-(4-phenoxyphenyl)-4,5,6,7-tetrahydropyrazolo-[1,5-a]pyrimidine-3-carboxamide or a pharmaceutically acceptable salt thereof. The present invention also discloses methods for treating non-GCB DLBCL in a subject, comprising administering to a subject in need thereof (S)-7-(1-acryloylpiperidin-4-yl)-2-(4-phenoxyphenyl)-4,5,6,7-tetrahydropyrazolo-[1,5-a]pyrimidine-3-carboxamide or a pharmaceutically acceptable salt thereof in combination with an anti-CD20 antibody. Background Art
[0002] Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin's lymphoma, which is clinically heterogeneous: 40% of patients respond well to existing therapies and have prolonged survival, while the rest die from the disease (Alizadeh et al., Nature 2000; 403(6769): 503-11). DLBCL can be divided into subgroups with germinal center B cell-like (GCB), activated B cell-like (ABC), expression profiles according to their prognostic significance (Alizadeh see above). The GCB group has a better survival rate than the ABC group, but both indications are associated with poor clinical outcomes. Molecular profiles have previously been used to predict survival in DLBCL (Rosenwald et al., N Engl J Med. [New England Journal of Medicine] 2002; 346: 1937-1947). In 2016, the World Health Organization (WHO) revised its classification of lymphoid neoplasms to account for major advances in lymphoma biology since 2008 (Swerdlow et al., Blood. 2016; 127(20): 2375-2390). The WHO classification emphasizes changes in gene expression and clinical importance, such as changes in the MYC, BCL2, and / or BCL6 oncogenes. More than 80% of patients with so-called "double-hit" lymphomas have MYC and BCL2 translocations (Aukema et al., Blood. 2011; 117(8): 2319-2331). This suggests the need for additional molecular markers in addition to MYC and BCL2 that can be used to classify patients before treatment or indicate response to treatment.
[0003] Bruton's tyrosine kinase (BTK) inhibitors alone or in combination with CD20 antibodies have shown only modest activity in R / R non-GCB DLBCL. However, patients with CD79B mutations responded frequently to ibrutinib (5 / 9; 55%), especially those with concomitant myeloid differentiation primary response 88 (MYD88) mutations (4 / 5; 80%). The Phase 3 PHONIX trial of ibrutinib + R-CHOP failed to reach its primary endpoint for the treatment of first-line non-GCB DLBCL patients, however, biomarker analysis showed improved event-free survival in the BCL2 / MYC co-expressing population. And these benefits were only observed in patients under 65 years of age, which also suggests that the combination of ibrutinib and R-CHOP may lead to relatively higher toxicity. Therefore, these clinical results indicate that patient stratification and patient response based on molecular markers will be key to future treatment of non-GCB DLBCL, and further research should be done on less toxic regimens. Summary of the invention
[0004] WO 2014 / 173289 A discloses a series of BTK inhibitors, in particular (S)-7-(1-acryloylpiperidin-4-yl)-2-(4-phenoxyphenyl)-4,5,6,7-tetrahydropyrazolo[1,5-a]pyrimidine-3-carboxamide (hereinafter referred to as Compound 1). Compound 1 can be used to treat cancers in which there are aberrations in the B cell receptor (BCR) and FcR signaling pathways in which BTK plays an important role, and has been shown to have effective and irreversible inhibitory activity against BTK.

When the tumor reached 100-200mm 3 At 14:00, mice were randomized into groups of 7-10 mice / group based on tumor volume. Compound 1 was formulated with 0.5% methylcellulose as a vehicle and treatment was initiated on the day of randomization. Compound 1 was orally administered at 7.5 mg / (kg body weight), BID. Tumor volume was measured twice a week in two dimensions using a caliper and expressed in mm using the following formula: 3 Indicates volume:
[0117] V=0.5(axb 2 ), where a and b are the long and short diameters of the tumor, respectively. Tumor growth inhibition (TGI) was calculated using the following formula:
[0118] %TGI=100×[1-(treatment vt -treat v0 ) / (vehicle vt -vehicle v0 )]
[0119] treat vt = tumor volume of the treatment group at time t.
[0120] treat v0 = tumor volume of the treatment group at the start of treatment.
[0121] vehicle vt = tumor volume of the vehicle group at time t.
[0122] vehicle v0 = tumor volume of the vehicle group at the start of treatment.
[0123] The mean TGI for each model was used and is represented by dots in the figures. P values were calculated using a two-tailed t-test to compare differences between groups.
[0124] result
[0125] Compound 1 resulted in a higher TGI in the EBV-negative (TGI 78.00±43.41, mean±SD) non-GCB DLBCL model than in the EBV-positive (TGI 33.25±22.41, mean±SD) model, P<0.05 (two-tailed t-test). Figure 3 .
[0126] in conclusion
[0127] EBV-negative non-GCB DLBCLs were more sensitive to compound 1 than EBV-positive non-GCB DLBCLs.
[0128] Example 4 Treatment of non-germinal center B-cell-like diffuse large B-cell lymphoma (non-GCBDLBCL) treated with BTK inhibitors
[0129] A total of 121 R / R non-GCB DLBCL patients were recruited in four compound 1 treatment studies conducted in similar time periods. In the four studies, two were compound 1 monotherapy (n=79) and two were compound 1 combined with anti-CD20 antibodies (n=42). Similar inclusion and exclusion criteria and response evaluation criteria were used in all studies. Fifty-six non-GCB patients were further subdivided using the HTG EdgeSeq DLBCL origin cell assay based on gene expression profiles (GEP). The expression of approximately 90 lymphoma-related genes from the HTG GEP assay was analyzed by the R limma package to analyze its association with the response to compound 1 treatment. Seventy-seven patient samples were tested by next-generation sequencing (NGS) with a set of genes. The correlation between mutations and objective response rate (ORR) was assessed using the chi-square test.
[0130] Example 4A: Treatment of non-GCB DLBCL with BTK inhibitor monotherapy
[0131] method
[0132] Monotherapy regimen 1: Compound 1 was orally administered daily at 160 mg twice daily (BID) to R / R non-GCB DLBCL patients over a 28-day cycle.
[0133] Monotherapy regimen 2: Compound 1 was orally administered daily at 160 mg BID to R / R non-GCBD BCL patients in each cycle of 28 days.
[0134] 79 non-GCB DLBCL patients received compound 1 monotherapy (monotherapy regimen 1 or 2).
[0135] result
[0136] The unadjusted ORRs in non-GCB DLBCL were 31.6% and 29.3% for the two monotherapies, and 53.8% and 36%, respectively, for patients with GEP-confirmed activated B-cell DLBCL.
[0137] For the 44 non-GCB patients with HTG gene expression profiles, PAX5 expression was significantly higher in both monotherapy responders (see Figure 4).
[0138] For the 44 non-GCB DLBCL patients who underwent group sequencing, mutations in genes related to the B cell receptor (BCR) pathway or NOTCH1 pathway have been identified to be associated with better responses. Non-GCB DLBCL patients with CD79B mutations (n=17) showed a higher ORR than other populations (52.9% vs. 33.3%, p=0.2). In both monotherapy, patients with NOTCH1 mutants (n=3) had a higher ORR (100% vs. 36.6%, p<0.05). See Table 1.

[0144] in conclusion
[0145] In monotherapy treatment regimens, patients with higher PAX5 expression or NOTCH1 or CD79B mutations showed better responses after administration of Compound 1. Likewise, screening of non-GCB DLBCL patients for PAX5 expression or NOTCH1 or CD79B mutations prior to treatment is a useful marker to determine a patient's response to Compound 1 and to determine whether Compound 1 treatment should be continued.
[0146] Example 4B: Treatment of non-GCB DLBCL with BTK inhibitors in combination with anti-CD20 antibodies
[0147] method
[0148] Combination therapy regimen 1:Compound 1 was orally administered daily in combination with obinutuzumab to patients with R / R non-GCBDLBCL at 160 mg BID in each cycle of 28-day cycles. Obinutuzumab was administered for up to 6 cycles in accordance with the US labeling regimen (100 mg on day 1 of cycle 1, 1000 mg on days 8 and 15, 900 mg on day 2 of cycle 1, and 1000 mg on day 1 of subsequent cycles 2-6).
[0149] Combination therapy regimen 2: Compound 1 was orally administered daily in combination with rituximab to R / R non-GCBDLBCL patients at 160 mg BID in each cycle of 28-day cycles. Rituximab was administered intravenously at 375 mg / m2 on days 1, 8, 15, 22 of cycle 1 and day 1 of cycles 4, 6, 8, 10. Compound 1 was administered at least 30 minutes before the start of rituximab infusion.
[0150] Forty-two patients with non-GCB DLBCL received Compound 1 + anti-CD20 antibody combination therapy regimen 1 or 2.
[0151] result
[0152] The unadjusted ORRs in non-GCB DLBCL were 22.7% and 35% for the two combinations, and 50% and 40%, respectively, for patients with GEP-confirmed activated B-cell DLBCL.
[0153] The expression of approximately 90 lymphoma-related genes from the HTG GEP assay was analyzed by the R limma package for association with response to combination therapy regimens 1 to 2. For the 12 non-GCB patients with HTG gene expression profiles, PIM1, BCL2, and FOXP1 were expressed more highly in combination therapy responders. (See Figure 4).
[0154] Non-GCB DLBCL patients with CD79B mutation (n=8) showed significantly higher ORR than patients without CD79B mutation (see Table 2).

[0160] in conclusion
[0161] The combination therapy results are very different from the monotherapy results. The enrichment of PAX5 expression after monotherapy is not obvious. On the contrary, increased PIM1, BCL2, and FOXP1 expression were observed in combination therapy patients. Patients with CD79B mutations showed significantly better responses to combination therapy. Therefore, screening for PIM1, BCL2, or FOXP1 expression or CD79B mutations in non-GCB DLBCL patients before treatment is a useful marker to determine the patient's response to compound 1 and anti-CD20 antibodies and to determine whether combination therapy should be continued.
[0162] Example 4C: Treatment of non-GCB DLBCL with BTK inhibitors monotherapy or combination therapy
[0163] method
[0164] Monotherapy regimen 1: Compound 1 was orally administered daily at 160 mg twice daily (BID) to R / R non-GCB DLBCL patients over a 28-day cycle.
[0165] Monotherapy regimen 2: Compound 1 was orally administered daily at 160 mg BID to R / R non-GCBD BCL patients in each cycle of 28 days.
[0166] Combination therapy regimen 1: Compound 1 was orally administered daily in combination with obinutuzumab to patients with R / R non-GCBDLBCL at 160 mg BID in each cycle of 28-day cycles.
[0167] Combination therapy regimen 2: Compound 1 was orally administered daily in combination with rituximab to R / R non-GCBDLBCL patients at 160 mg BID in each cycle of 28 days. Rituximab was administered intravenously at 375 mg / m2 on days 1, 8, 15, 22 of cycle 1 and day 1 of cycles 4, 6, 8, 10. Compound 1 was administered at least 30 minutes before the start of rituximab infusion.
[0168] A total of 121 patients with non-GCB DLBCL were included in the response evaluation. 79 patients received compound 1 monotherapy (monotherapy regimen 1 or 2) and 42 patients received compound 1 + anti-CD20 antibody combination therapy ( Combination therapy regimen 1 or 2 ).
[0169] Fifty-six non-GCB patients were further subdivided based on gene expression profiling (GEP) using the HTG EdgeSeq DLBCL cell-of-origin assay. The expression of approximately 90 lymphoma-related genes from the HTG GEP assay was analyzed by the R limma package for association with response to zanubrutinib treatment.
[0170] result
[0171] The median expression value of BCL2 was used as the threshold to define high BCL2 and low BCL2 groups. A total of 28 high BCL2 (BCL2-H) and 28 low BCL2 (BCL2-L) patients were studied. Patients with high BCL2 expression had significantly better BOR (16 / 28, 57% vs. 6 / 28, 21%, p=0.028) and longer PFS (5.3m vs. 2.8m, p=0.03) / OS (10m vs. 6m, p=0.21). See Figure 5A , 5B and 5C.
[0172] The median expression value of MYC was used as the threshold to define high MYC and low MYC groups. A total of 28 high MYC and 28 low MYC patients were studied. Patients with high MYC expression tended to have better BOR (13 / 28, 46% vs. 9 / 28, 32%, p=0.14) and longer PFS (5.4m vs. 2.9m, p=0.52) / OS (10m vs. 7m, p=0.48). See Fig. 6A , 6B and 6C.
[0173] The median expression value of BCL2 / MYC was used as the threshold to define dual expression (DE) or non-DE. Patients (18 DE and 38 non-DE) received two monotherapies and two combination therapies. Patients with DLBCL that dually expressed MYC and BCL2 tended to have a higher ORR (11 / 18, 61% vs. 11 / 38, 29%; P = 0.12) and longer progression-free survival (5.4 months vs. 3.6 months; P = 0.16) and overall survival (10 months vs. 7 months, P = 0.32). See Fig. 7A , 7B and 7C.
[0174] in conclusion
[0175] Patients with MYC, BCL2, or BCL2 / MYC dual-expressing DLBCL tended to have higher ORR and longer progression-free survival and overall survival, with higher response rates in the four Compound 1 treatment studies. Therefore, expression screening of MYC, BCL2, or BCL2 / MYC dual expression in patients with non-GCB DLBCL before treatment is a useful marker to determine the patient's response to Compound 1 and Compound 1 plus anti-CD20 antibody and to determine whether treatment should continue as monotherapy or combination therapy.
