Abstract
Description

[0002] Related Applications
[0003] This application claims the benefit of priority under 35 U.S.C. §119(e) to U.S. Provisional Application No. 62 / 930,054, filed on November 4, 2019, which is incorporated herein by reference in its entirety for all purposes. Background Art
[0004] Arginase is a manganese metalloenzyme that catalyzes the conversion of L-arginine to urea and L-ornithine. There are two isoforms, arginase 1 and arginase 2. Although L-arginine is not an essential amino acid because it can be provided by protein turnover in healthy adults, under various physiological and pathological conditions (e.g., pregnancy, autoimmune diseases, cancer), the expression and secretion of arginase are increased, resulting in reduced L-arginine levels. Immune cells are particularly sensitive to reduced L-arginine levels. Tumors use a variety of immunosuppressive mechanisms to evade the immune system. One of these is to reduce L-arginine by increasing the level of circulating arginase, increasing arginase expression and secretion by tumor cells, and recruiting myeloid-derived suppressor cells that express and secrete arginase. Together, these mechanisms lead to reduced L-arginine in the tumor microenvironment and produce an immunosuppressive phenotype. Pharmacological inhibition of arginase activity has been shown to reverse low L-arginine-induced immunosuppression in animal models. However, many proteins and pathways are involved in cancer, and their research is progressing rapidly. Therefore, there is a need to provide patients with new cancer therapies. [0112] Immunomodulators
[0113] As used herein, "immunomodulator" refers to an agent that enhances an immune response (e.g., an anti-tumor immune response). An immunomodulator can be an antibody or an antigen-binding fragment thereof, a protein, a peptide, a DNA or RNA fragment, a small molecule, or a combination thereof. In some embodiments, an immunomodulator is an immune checkpoint inhibitor. In some embodiments, an immunomodulator is an immunostimulant. As used herein, "immune checkpoint inhibitor" refers to an agent (i.e., immune checkpoint agent) that inhibits blocking the immune system, such as attacking a protein or peptide of a cancer cell. In some embodiments, the immune checkpoint agent that blocks the immune system prevents the generation and / or activation of T cells. In some embodiments, an immune checkpoint inhibitor is a cytotoxic T lymphocyte-associated protein 4 (CTLA-4), a programmed cell death protein 1 (PD1), or a programmed death ligand 1 (PD-L1). PD-L1 and PD1 form a ligand-receptor pair bound to the cell surface, which, in healthy individuals, suppresses the immune response to prevent an overreaction of the immune system. In some embodiments, cancer cells hijack the normal PD-L1 / PD1 immune checkpoint mechanism by overexpressing the ligand PD-L1 (which binds to the PD1 on effector CD8 T cells), thereby preventing T cells from establishing an immune response to cancer cells and / or tumors. PD-L1 is expressed at high frequencies in a wide range of cancers. Tumor PD-L1 overexpression is associated with poor prognosis in a variety of cancers (see, for example, Hamid et al., Expert Opin Biol Ther [Expert Review of Biological Therapy] 13 (6): 847-861, 2013). As used herein, "immunostimulant" means a substance that stimulates the immune system without any antigen-specificity in an immune response by inducing activation of any component of the immune system or increasing its activity. In some embodiments, the immunostimulant is a toll-like receptor 3 (TLR3) agonist, such as polyinosinic acid: polycytidylic acid, which is also referred to as poly I: C or poly (I: C).
[0114] A review describing immune checkpoint pathways and blocking such pathways with immune checkpoint inhibitor compounds is provided by Pardoll in Nature Reviews Cancer (April 2012), pp. 252-264. Immune checkpoint inhibitor compounds show anti-tumor activity by blocking one or more endogenous immune checkpoint pathways that downregulate anti-tumor immune responses. Typically, inhibition or blocking of immune checkpoint pathways involves inhibiting the interaction of checkpoint receptors and ligands with immune checkpoint inhibitor compounds to reduce or eliminate signals and result in a reduction in anti-tumor responses.
[0115] As used herein, "immune checkpoint inhibitors" refer to agents (i.e., immune checkpoint agents) that inhibit proteins or peptides that block the immune system, such as attacking cancer cells. Immune checkpoint inhibitors can be antibodies or antigen-binding fragments thereof, proteins, peptides, small molecules, or combinations thereof. In some embodiments, immune checkpoint agents that block the immune system prevent the generation and / or activation of T cells. In some embodiments, immune checkpoint agents are cytotoxic T lymphocyte-associated protein 4 (CTLA-4), programmed cell death protein 1 (PD1), programmed death ligand 1 (PD-L1), or inhibitory receptors (such as NKG2A) that recognize HLA-E and are expressed by NK cells and T cell subsets. PD-L1 and PD1 form a cell surface-bound ligand-receptor pair that, in healthy individuals, suppresses immune responses to prevent overreaction of the immune system. In some embodiments, cancer cells hijack normal PD-L1 / PD1 immune checkpoint mechanisms by overexpressing the ligand PD-L1 (which binds to PD1 on effector CD8 T cells), thereby preventing T cells from establishing immune responses to cancer cells and / or tumors. PD-L1 is expressed at high frequencies in a wide range of cancers. Tumor PD-L1 overexpression is associated with poor prognosis in a variety of cancers (see, e.g., Hamid et al., Expert Opin Biol Ther 13(6):847-861, 2013).
[0116] In some embodiments of the present disclosure, immune checkpoint inhibitor compounds inhibit the signal transduction interaction between immune checkpoint receptors and the corresponding ligands of immune checkpoint receptors. Immune checkpoint inhibitor compounds can work by blocking the activation of immune checkpoint pathways, which is achieved by inhibiting (antagonizing) immune checkpoint receptors (some examples of receptors include CTLA-4, PD-1, and NKG2A) or by inhibiting the ligands of immune checkpoint receptors (some examples of ligands include PD-L1 and PD-L2). In such embodiments, the effect of immune checkpoint inhibitor compounds is to reduce or eliminate the downregulation of certain aspects of the anti-tumor response of the immune system in the tumor microenvironment.
[0117] In some embodiments, the immune checkpoint inhibitor inhibits the CTLA-4 pathway or the PD-L1 / PD1 pathway. In some embodiments, the immune checkpoint inhibitor is an antibody. In some embodiments, the immune checkpoint inhibitor comprises an antibody that inhibits CTLA-4, PD1, or PD-L1. Immune checkpoint inhibitors, immune checkpoint inhibitors, and examples thereof are provided, for example, in WO 2016 / 062722.
[0118] In some embodiments, the immune checkpoint inhibitor is an anti-CTLA-4 antibody or a derivative or antigen-binding fragment thereof. In an embodiment, the anti-CTLA-4 antibody selectively binds to the CTLA-4 protein or a fragment thereof. Examples of anti-CTLA-4 antibodies and their derivatives and fragments are described in, for example, US 6,682,736; US 7,109,003; US 7,123,281; US 7,411,057; US 7,807,797; US 7,824,679; US 8,143,379; US 8,491,895, and US 2007 / 0243184. In some embodiments, the anti-CTLA-4 antibody is tremelimumab or ipilimumab.
[0119] The immune checkpoint receptor cytotoxic T lymphocyte-associated antigen 4 (CTLA-4) is expressed on T cells and is involved in signaling pathways that reduce the level of T cell activation. CTLA-4 is believed to downregulate T cell activation through competitive binding and sequestration of CD80 and CD86. In addition, CTLA-4 has been shown to be involved in enhancing T Reg Immunosuppressive activity of cells.
[0120] In some embodiments, the immune checkpoint inhibitor is an anti-PD-L1 antibody or a derivative or antigen-binding fragment thereof. In some embodiments, the anti-PD-L1 antibody or a derivative or antigen-binding fragment thereof selectively binds to a PD-L1 protein or a fragment thereof. Examples of anti-PD-L1 antibodies and derivatives and fragments thereof are described in, for example, WO 01 / 14556, WO 2007 / 005874, WO2009 / 089149, WO 2011 / 066389, WO 2012 / 145493; US 8,217,149, US 8,779,108; US2012 / 0039906, US2013 / 0034559, US2014 / 0044738, and US2014 / 0356353. In some embodiments, the anti-PD-L1 antibody is MEDI4736 (durvalumab), MDPL3280A, 2.7A4, AMP-814, MDX-1105, atezolizumab (MPDL3280A), or BMS-936559.
[0121] The immune checkpoint receptor programmed death 1 (PD-1) is expressed by activated T cells upon prolonged exposure to antigens. Engagement of PD-1 with its known binding ligands (PD-L1 and PD-L2) occurs primarily within the tumor microenvironment and results in downregulation of anti-tumor specific T cell responses. Both PD-L1 and PD-L2 are known to be expressed on tumor cells. Expression of PD-L1 and PD-L2 on tumors is associated with reduced survival outcomes.
[0122] In some embodiments, the anti-PD-L1 antibody is MEDI4736, also known as durvalumab. In some embodiments, the anti-PD-L1 antibody comprises an amino acid sequence that is at least 80%, at least 85%, at least 90%, at least 95%, or at least 99% identical to any one of SEQ ID NO: 1-8. MEDI4736 is an anti-PD-L1 antibody that is selective for PD-L1 polypeptides and blocks the binding of PD-L1 to PD-1 and CD80 receptors. MEDI4736 can relieve PD-L1-mediated inhibition of human T cell activation in vitro, and can further inhibit tumor growth in a xenograft model via a T cell-dependent mechanism. MEDI4736 is further described, for example, in US 8,779,108. The fragment crystallizable (Fc) domain of MEDI4736 contains triple mutations in the constant domain of the IgG1 heavy chain that reduce binding to the complement component C1q and Fcγ receptors responsible for mediating antibody-dependent cell-mediated cytotoxicity (ADCC).
[0123] In some embodiments, MEDI4736 or its antigen-binding fragment comprises a heavy chain and a light chain or a heavy chain variable region and a light chain variable region. In some embodiments, MEDI4736 or its antigen-binding fragment for use comprises a light chain variable region and a heavy chain variable region, the light chain variable region comprises an amino acid sequence of SEQ ID NO: 1, and the heavy chain variable region comprises an amino acid sequence of SEQ ID NO: 2. In some embodiments, MEDI4736 or its antigen-binding fragment comprises a heavy chain variable region and a light chain variable region, wherein the heavy chain variable region comprises a CDR1, CDR2, and CDR3 sequence defined by Kabat of SEQ ID NO: 3-5, and wherein the light chain variable region comprises a CDR1, CDR2, and CDR3 sequence defined by Kabat of SEQ ID NO: 6-8. One of ordinary skill in the art will be able to easily identify Chothia's, Abm's, or other CDR definitions known to one of ordinary skill in the art. In some embodiments, MEDI4736 or an antigen-binding fragment thereof comprises the variable heavy chain and variable light chain CDR sequences of the 2.14H90PT antibody as described in WO 2011 / 066389.
[0124] In some embodiments, the immune checkpoint inhibitor is an anti-PD-1 antibody or a derivative or antigen-binding fragment thereof. In some embodiments, the anti-PD-1 antibody selectively binds to the PD-1 protein or a fragment thereof. In some embodiments, the anti-PD1 antibody is nivolumab, pembrolizumab, or pidilizumab.
[0125] NKG2A receptor is an inhibitory receptor that is combined with HLA-E and expressed on tumor-infiltrating cytotoxic NK and CD8 T lymphocytes. By expressing HLA-E, cancer cells can protect themselves from being killed by NKG2A+ immune cells. HLA-E is often upregulated on cancer cells of many solid tumors or malignant hematological diseases. Monalizumab (IPH2201) is a humanized IgG4 that blocks the combination of NKG2A and HLA-E, thereby allowing the activation of NK cells and cytotoxic T cell responses. Examples of anti-NKG2A antibodies and derivatives and fragments thereof are described in WO 2016 / 041947, the contents of which are hereby incorporated by reference in their entirety (including but not limited to sequence listing).

Tumor length and width were measured with a caliper and the formula (length x width) was used. 2 )*π / 6 to calculate the tumor volume, which was then reported as the calculated tumor volume.
[0275] result: like Figures 1A to 1D , Figures 2A to 2D ,and Figures 3A to 3F As shown in , compound 12 monotherapy was moderately effective in all three mouse syngeneic tumor models. Administration of anti-PDL1 alone also showed moderate efficacy in two colorectal cancer tumor models. The combination of agents showed significantly stronger efficacy: 87% tumor growth inhibition in MC38-OVA and 46% total tumor growth inhibition in the CT.26WT model (with one complete remission). Adding anti-NKG2a to compound 12+anti-PDL1 treatment resulted in 53% total tumor growth inhibition and 3 complete remissions. TLR3 agonists (poly I:C) showed moderate monotherapy activity, with a TGI of 76% on day 16. The combination of compound 12+poly I:C resulted in 87% tumor growth inhibition.
[0276] like Figures 4A to 4F As shown in, in vivo PD combination studies of ARG inhibitors with anti-PDL1 in the MC38-OVA model showed increases in multiple tumor immune cell populations (approximately 4-fold CD8+ T cells, 2-fold NK cells, 2-fold CD103+ DCs) and increased CD8 T cell activation. Figure 5A and 5B As shown in the results, ARG inhibition led to an increase in peripheral (draining LN) CD8+ T cells producing IFNg and TNFa. In the CT.26WT model, there was one complete remission.
[0277] Example 11. Compound 12 (arginase 1 inhibitor) combined with radiation therapy (RT) in preclinical models of cancer Benefits
[0278] method:1e6 LL / 2 murine lung cancer cells were implanted subcutaneously into the right flank of six to eight week old BL / 6 mice (Charles River Labs). On day 6 post-implantation, animals were randomized into four groups (n=12 / group) based on the average tumor volume per cage. Group 1 was anesthetized on days 6 and 9 for simulated irradiation and was administered vehicle (water) PO, BID from day 6. Group 2 was anesthetized on days 6 and 9 and given 5 Gy of tumor-targeted radiation. Group 3 was administered AZ8400 at 30 mg / kg PO bid from day 6, and Group 4 was administered AZ8400 at 30 mg / kg from day 6, and anesthetized and irradiated on days 6 and 9. Body weight and tumor volume were measured twice a week until tumors were >1500 mm3, at which time the animals were humanely sacrificed.
[0279] result:
[0280] like Figure 5A and 5B As shown in, in the mouse syngeneic lung tumor model LL / 2, the combination of compound 12 and radiation therapy exhibited significantly stronger efficacy, reaching approximately 60% tumor growth inhibition (p<0.05 compared to vehicle and monotherapy treatments), compared to compound 12 administered twice daily at 30 mg / kg and radiation therapy (5 Gy on days 6 and 9 post-implantation; approximately 30% tumor growth inhibition, p>0.05 compared to vehicle).
