Antibody-drug conjugates and pharmaceutical compositions thereof and methods of treating cancer therewith. The antibody-drug conjugates are conjugates of belotecan derivatives and anti-CD44v9 antibodies or antigen binding fragments thereof. The antibody-drug conjugates exhibit superior anti-tumor activity in various solid tumors.

Description

ANTIBODY-DRUG CONJUGATES, PHARMACEUTICAL COMPOSITIONS AND USES THEREOFField of the Invention

[0001] The present disclosure relates to antibody-drug conjugates and, in particular, to anti-CD44v9 antibody-drug conjugates. The present disclosure also relates to pharmaceutical compositions, preparation methods, uses, and methods of treatment of the conjugates.Background of the Invention

[0002] CD44 is a family of single transmembrane glycoproteins involved in homotypic cell-cell, cell-matrix, and cell-cytoskeletal interaction. There are several well-known ligands of CD44 including hyaluronic acid (HA) , osteopontin (OPN) , collagens, and matrix metalloproteinases (MMPs) . In humans, CD44 is encoded by 19 exons, among which the first five exons (exons 1-5) and the last five exons (exons 16-20) constantly encode CD44s which is the most common and the smallest CD44 protein. CD44 variant isoforms (CD44v) are generated by alternative splicing (v2-v10) and possess the ten CD44s exons plus any combination of the remaining nine variant exons. CD44v isoforms play various roles in cancer and could have impact on key features of carcinogenesis such as tumor initiating and metastasis. These key features often originate from altered signal transduction, resistance to oxidative stress, chemo- / radio resistance, adhesion, extracellular matrix interaction, tumor matrix formation or modulation of CSCs niche. In addition, CD44 variants have been shown to be important for Ras activation through formation of a co-receptor complex with the hepatocyte growth factor HGF and its receptor tyrosine kinase Met, whose phosphorylation activates Ras signaling. Some isoforms also exhibit an antiapoptotic effect and can block Fas-mediated apoptosis.

[0003] One isoform among those, CD44v9, so named for the spliced variants containing the v9 exon, is overexpressed in a variety of solid tumors including (but not limited to) head and neck, non-small cell lung, esophageal, pancreatic, colorectal, breast, bladder, and gastric cancers, with restricted presence on normal healthy tissues. Positive CD44v9 expression was also associated with larger tumor size, deeper tumor invasion, earlier lymph node metastasis, and more advanced clinical stage when compared with CD44v9-negative patient data in a meta-analysis study. Based on the above characteristics, CD44v9 is a promising target for potential antibody-derived cancer therapeutics development.

[0004] The major categories of CD44-targeted therapies include peptide mimetics, aptamers, pharmacological inhibitors, CD44 decoys, HA oligomers, HyACT (Hyaluronic Acid Chemo Transport) , neutralizing antibodies, and antibody drug conjugates. Monoclonal CD44-specific antibodies, which interfere with the binding of HA to CD44, led to the inhibition of anchorage-independent growth of murine mammary carcinoma cells and human colon carcinoma cells and can also induce apoptosis. HA oligosaccharides had similar effects and inhibited tumor growth in vivo most likely by interfering with the binding of hHA  (high molecular weight HA, ~2000 kDa) to CD44. Additionally, CD44 antibodies were employed as targeting agents for tumor-specific delivery of radiotherapy or cytotoxic payloads. Several therapeutic compounds mentioned above have been tested in clinical trials.

[0005] For example, a randomized phase II study of Hyaluronan-irinotecan (HA-IR) showed improvement in progression free survival compared to irinotecan in metastatic colorectal cancer patients, but the results were unfortunately not confirmed in a subsequent Phase III trial. Another Phase IIa trial of HA-irinotecan was conducted in combination with Carboplatin in extensive-stage small cell lung cancer (SCLC) compared to irinotecan with Carboplatin. The study concluded that while the treatments were well tolerated, there was no statistically significant difference between treatment and control arm, although there was a trend that CD44s positive tumors may gain survival benefit. For direct CD44 and specific variant targeting, a humanized antibody binding to the HA-binding proximal domain of all CD44 isoforms, RG7356, was tested  in clinical trials for AML (acute myelocytic leukemia) and advanced solid tumors with limited clinical efficacy but generally well tolerated. A phase I study of 186Re-Labeled chimeric monoclonal antibody specific for CD44v6, U36, was conducted in Head &Neck squamous cell cancer (HNSCC) patients. The study showed that U36 radio-conjugate can be safely administered with dose-limiting myelotoxicity and yielded transient therapeutic effect. While the above CD44-targeting drug modalities only produced modest clinical efficacy results, they were generally well tolerated, with some accompanying imaging studies suggesting tumor-specific targeting was achieved. However, a different clinical outcome was observed with a humanized monoclonal antibody (mAb) , Bivatuzumab (BIWA-4) , combined with potent microtubule inhibitor mertansine to specifically target CD44v6 in HNSCC (Squamous Cell Carcinoma of Head and Neck) and metastatic breast cancer patients in phase I trials. Although partial responses were observed in 3 patients at high dose levels, these clinical trials were discontinued due to significant cutaneous toxicities with one fatal outcome attributed to the bivatuzumab-mertansine conjugate.

[0006] Taken together, while there are quite a few CD44 targeting anti-cancer agents have been investigated in clinical trials, no active compound has been successfully developed to be both satisfactorily therapeutic effective and safe sufficiently, and none of these investigated compounds in clinical trials are CD44v9 targeting. To date, no antibody drug conjugates (ADCs) targeting CD44v9 are approved or investigated in clinical trials. The unique information on druggability such as efficacy, safety, pharmacokinetic properties and therapeutic window of CD44v9 anti-cancer agents, especially ADCs, are unknown and need to be studied and established.Summary

[0083] The antibody-drug conjugate in the present disclosure specifically targets CD44v9 and induces cytotoxicity in cells with ectopic CD44v9 expression while no obvious toxic side effects on CD44v9-normally expressed cells.

[0084] The antibody-drug conjugate in the present disclosure shows encouraging preclinical in vivo efficacy in multiple solid tumor models (including but not limited to NSCLC (Non-Small Cell Lung Carcinoma) , HNSCC (Squamous Cell Carcinoma of Head and Neck) , ESCC (esophageal squamous cell carcinoma) , GC (Gastric Carcinoma) , BC (Bladder Cancer) , TNBC (Triple Negative Breast Cancer) and liver cancer) .

[0085] The antibody-drug conjugate hHTS033-KL in the present disclosure was well tolerated in non-human primate toxicology studies (HNSTD=30mg / kg) and no severe dermal toxicity were observed. hHTS033-KL could potentially have a wide  therapeutic window in clinical testing and can provide benefits for cancer patients with high unmet medical needs.Brief Description of the Drawings

[0390] To evaluate the binding affinity of ADCs, tumor cells of NCI-H292 (Non-small cell lung cancer, BMCR, Cat#SCSP-582) , A253 (submaxillary salivary gland cancer, Meisen CTCC, Cat#CTCC-007-0437 ) , A549 (Non-small cell lung cancer, iCell, Cat#iCell-h011) , PC-9 (Non-small cell lung cancer, CELLCOOK, Cat#CC0204) and Detroit 562 (Pharyngeal cancer, BMCR, Cat#TCHu231) were detached by 1mM Trypsin-EDTA and centrifuged at 1200rpm for 5 min. Resuspended cell pellet in 5%BSA in PBS. 5×105 / 50 μL / well cells were added into the 96-well assay plate, incubated at 2-8℃ for 30min. ADCs were diluted to the final concentration as indicated and incubated with cells at 4 ℃ for 1h Wash cells and anti-Human IgG (Fc specific) -FITC (Fluorescein Isothiocyanate) antibody (Sigma, Cat#F9512-2ml) was used as a  secondary Ab at the 4℃ incubator for 45 minutes. The data were analyzed using a FACSAriaTM Fusion flow cytometer (MACSQuant X) and Graphpad Prism 8 software.

[0393] Example 7. Internalization of hHTS033-KL on tumor cell lines

[0394] PC-9, A253, NCI-H292 and Detroit 562 cell culture were maintained in vitro as independent monolayer cultures at 37℃ in an atmosphere of 5%CO2. The cells were detached by 1mM EDTA in PBS, washed with PBS for 2 times, then were harvested by centrifugation at 1000 rpm for 5 minutes. Cell pellet were resuspended and incubated with 50 μg / mL hHTS033-KL at 4℃ for 1h. After washing, cells were incubated in 2%BSA in PBS respectively at 37℃ for 0, 0.5, 1, 2, 4, and 8 hours. Then cells were fixed with 4%PFA  (paraformaldehyde) at room temperature for 15 min. Wash and resuspend cells in PBS, and put it at 4 ℃ overnight. Cells were then stained with FITC conjugated anti-Human antibody for 45 min and analyzed with FACs by calculating surface MFI (Mean Fluorescent Intensity) .

[0422] Tumors (shown in Table 15) were sliced into 3 mm× 3 mm× 3 mm (about 45~60 mg) fragments and implanted subcutaneously in the right flank of mice (female Nu / Nu mice, ages 35-42 days) which were purchased from Beijing Vital River Laboratory Animal Technology co. LTD. When the average tumor size reached around 150 mm3 on day 34 post inoculation, the mice were randomized into 3 groups based on the tumor volume. Mice were given hHTS033-KL, isotype control ADC (Sichuan Kelun-Biotech Biopharmacuetical Co., Ltd) at 10mg / kg and Vehicle control intravenously on Day 0 (Day of the first administration of the drug) , Day 7 and Day 14 respectively. Tumor size and body weight were measured twice weekly. The tumor volume was expressed in mm3 using the formula: V = 0.5 a × b2 where a and b are the long and short diameters of the tumor, respectively. One-way ANOVA was performed to compare the tumor volume of each group with Vehicle group. t-test was performed to compare the differences between 2 groups. All data were analyzed with GraphPad  Prism 8, while p<0.05 meant the difference between groups reached statistical significance.

[0428] Example 13.4-Week GLP Repeat-Dose Toxicity Study of hHTS033-KL Following a 6-Week Recovery Phase in Cynomolgus Monkeys

[0429] Experiment of repeated administration of hHTS033-KL to cynomolgus monkeys: 40 cynomolgus monkeys (Zhanjiang Prima Biotech Inc. ) were randomly divided into 4 groups, and there were 5 male and 5 female in each group. Vehicle, 10 mg / kg of hHTS033-KL, 20 mg / kg of hHTS033-KL and 30 mg / kg of hHTS033-KL were infused intravenously on day 1, the day 15 and day 29, respectively. During the experiment, animals were observed for any abnormalities, and blood samples thereof were collected for hematology and blood biochemical index analysis. Animals for main phase necropsy (day 36) and animals for recovery phase necropsy (day 71) were euthanized respectively and sampled for pathological analysis.

[0430] As shown in Table 17, under the conditions of this study, repeated dosing of cynomolgus monkeys with hHTS033-KL at 10, 20 and 30 mg / kg via intravenous infusion for 4 weeks (once every 2 weeks for 3 doses in total) , was clinically well tolerated. Transient slight irritation non-test article related histopathological changes were noted at the administration site in all doses groups, and no significant abnormalities were noted in the neurologic, cardiovascular and respiratory system. The highest non-severely toxic dose (HNSTD) of hHTS033-KL was 30 mg / kg, and the main toxicity findings were decreased reticulocytes and pigmentation in skin and / or oral mucosa. After a 6-week recovery phase, these above changes resolved without notable delayed toxicity.
