[0001] Peptides for use in the treatment of sarcoma

[0002] Field of the invention

[0003] The present invention relates to peptides for use in treating and / or preventing sarcoma, a pharmaceutical composition for use in treating and / or preventing sarcoma as well as a method for treating sarcoma in a subject in need thereof. The peptides for use of the present invention are CXC chemokine receptor 4 (CXCR4) antagonistic peptides (proteins). The peptides for use of the present invention include the naturally occurring form of human circulating antiviral albumin fragment (ALB408-423), peptides obtainable by multiple synthesis which have the biological activity of ALB408-423, and biologically active fragments and / or variants and / or derivatives of the aforementioned.

[0004] Background of the invention

[0005] Sarcomas are malignant neoplasms of mesenchymal origin, i.e., arising from cells of the mesenchymal supporting tissue. Sarcomas form a subgroup of malignant tumors along with neoplasms of epithelial tissue (carcinomas), neoplasms of the blood (lymphomas and leukemias), and neuroendocrine and dysontogenetic tumors.

[0006] Sarcomas are rare, making up only 1% of all adult cancer diagnoses and about 15% of childhood cancer diagnoses. Approximately 16,000 sarcomas are diagnosed in the United States each year (around 4,000 bone sarcomas and approximately 13,000 soft tissue sarcomas). In France, approximately 4,000 people are diagnosed with sarcoma each year. In Italy, the numbers are higher, with an estimated 5,890 of new sarcoma cases annually. Sarcoma affects both children and adults. Generally, soft tissue sarcoma occurs more frequently in adults. Bone sarcoma diagnoses occur more often in children, teens and people older than 65 years. Sarcomas form when immature bone or soft tissue cells have changes to their DNA, and they develop into cancer cells that grow in an unregulated way. They may eventually form a mass or tumor that can invade nearby healthy tissues. Left untreated, the cancer can travel via the bloodstream or lymphatic system from the primary location where it formed to other organs (metastasis).

[0007] The current WHO classification of sarcomas divides tumors into more than 100 different entities. This classification is based, among other things, on the morphology, the origin, and the molecular genetic alterations of the tumor. Sarcomas can be roughly classified into bone sarcoma, cartilage sarcoma, and soft tissue sarcoma. A bone sarcoma is a primary malignant bone tumor. The three main types of bone sarcoma are osteosarcoma, Ewing's sarcoma, and chondrosarcoma. Osteosarcoma is the most frequently found bone sarcoma. It is a malignant bone tumor that consists of cells which produce the bone matrix and is most prevalent in teenagers and young adults. Ewing's sarcoma is the next most found sarcoma in childhood and adolescence and chondrosarcoma is the most common type found in adults.

[0008] Surgery is the most common form of the treatment for most sarcomas that have not spread to other parts of the body. Limb-sparing surgery, as opposed to amputation, can now be used to save the limbs of patients in at least 90% of extremity (arm or leg) sarcoma cases. Additional treatments, including chemotherapy, radiation therapy (also called "radiotherapy") and proton therapy, may be administered before surgery (called "neoadjuvant" chemotherapy or radiotherapy) or after surgery (called "adjuvant" chemotherapy or radiotherapy). The use of neoadjuvant or adjuvant chemotherapy and radiotherapy significantly improves the prognosis for many sarcoma patients. Treatment can be a long and arduous process, lasting about a year for many patients. In childhood sarcomas, the cytotoxic agent cyclophosphamide is widely used and has shown good anti-tumor efficacy.

[0009] A complete radical, surgical, en bloc resection of the cancer, is the treatment of choice in osteosarcoma. Although about 90% of patients are able to have limbsalvage surgery, complications, particularly infection, prosthetic loosening and non-union, or local tumor recurrence may cause the need for further surgery or amputation. [0011] Chemokine receptors are expressed on the surface of certain cells, which interact with cytokines called chemokines. The CXC chemokine receptor 4 (CXCR4) is a G-protein-coupled receptor that transduces signals of its endogenous ligand, the chemokine CXCL12 (stromal cell-derived factor-1, SDF- 1). Following interaction of CXCR4 / CXCL12 intracellular calcium (Ca2+) ions fluxes are triggered. This causes cellular responses, including chemotaxis allowing cells to travel within the organism. CXCR4 is expressed on myeloid cells, T-lymphocytes, B-lymphocytes, epithelial cells, endothelial cells and dendritic cells. The chemokine CXCL12 is the only known agonistic ligand of CXCR.4. The interaction between CXCL12 and CXCR4 plays a crucial role in the migration of progenitor cells during embryologic development of the cardiovascular, hemopoietic or central nervous systems. This interaction is also known to be involved in several diseases such as HIV infection / AIDS, cancer cell metastasis, leukemia cell progression, pulmonary fibrosis and rheumatoid arthritis. It is assumed that this interaction may be a critical therapeutic target in all of these diseases. Substances interfering with CXCR4 / CXCL12 signaling are assumed to have drug potential, e.g., in HIV / AIDS therapy, or to prevent cell migration processes involved in cancer metastasis, leukemia, and inflammatory diseases such as pulmonary fibrosis, rheumatoid arthritis or asthma (reviewed in Tsutsumi et al., 2007, Peptide Science 88: 279 - 289). In contrast to receptor agonists such as CXCL12 that induce cellular responses, receptor antagonists are ligands or drugs that do not induce a biological response, i.e., cell migration or Ca2+signaling, upon binding to their receptor. Receptor antagonists are useful drugs already in clinical use (e.g., angiotensin antagonists, 13-adrenergic antagonists, serotonergic antagonist or CCR5 antagonists) that can block HIV-1 infection (CCR5 antagonist) or decrease agonist mediated cellular responses. Interaction of receptor antagonists with the receptor inhibits the function of an agonist. Most drug antagonists achieve their potency by competing with endogenous ligands or substrates at structurally defined binding sites on receptors.

[0012] It has already been shown in vitro and in vivo that CXCR4 antagonists block cancer cell migration and hence metastasis. CXCR4 is expressed on the surface of a variety of cells (myeloid cells, T-lymphocytes, B-lymphocytes, epithelial cells, endothelial cells, and dendritic cells) as well as in 23 different types of cancer cells. CXCL12-CXCR4 interaction is involved in metastasis of several types of cancer, including cancer of the breast, kidney, prostate, lung, and pancreas, and melanoma, neuroblastoma, non-Hodgkin's lymphoma, multiple myeloma, ovarian cancer, and malignant brain tumors (reviewed in Tsutsumi et al., 2007, Peptide Science 88: 279 - 289). It has been shown that CXCR4 antagonists such as T140 analogous suppress CXCL12 induced pancreatic cell migration and invasion or breast carcinoma cell migration in vitro and in vivo (reviewed in Tsutsumi et al., 2007, Peptide Science 88: 279 - 289). Human albumin is a soluble monomeric serum protein having a molecular weight of about 65,000 that accounts for more than half the total plasma protein (concentration: 3.5 to 5 g / dL). The function of human albumin is predominantly described as a carrier molecule for all kinds of hydrophobic as well as hydrophilic substances, e.g., steroid and peptide hormones, fatty acids, vitamins, pharmaceuticals and cations. Due to its very high serum concentration, it contributes substantially to the stabilization of the blood pH, the extracellular liquid volume and the maintenance of colloid-osmotic pressure. Albumin has a globular structure stabilized by a high number of disulfide bridges and is usually not glycosylated, but alterations due to acetylation, enzymatic glycosylation and non-enzymatic glycosylation occur frequently in the course of molecular ageing or upon pathophysiological changes. It is synthesized by the liver as pre-pro-albumin having 609 amino acids; the N-terminal signal peptide comprising 18 amino acids is cleaved of intracellularly upon entry in the endoplasmic reticulum; another 6 amino acids are removed in the Golgi apparatus before the mature albumin comprising 585 amino acids is secreted by the liver cells. The clearance of albumin takes place through the kidney, the gastrointestinal tract and in the tissue cells of the liver.

[0092] The peptide sequence of ALB408-423 for use according to the invention starts with amino acid 408 and thus comprises the amino acids 408 to 423 of the circulating form of albumin. It is evidently produced by natural processing of the albumin precursor by corresponding proteases.

[0093] The peptide ALB408-423 for use according to the invention is obtainable by chromatographic purification from human hemofiltrate (HF). HF is obtained in large amounts during ultrafiltration of the blood of kidney patients (Example 1). HF contains all peptides and proteins circulating in human blood with a molecular weight below 30 kDa. Peptides and proteins in HF were extracted using cation exchange chromatography. Column bound peptides and proteins were eluted with buffers systems of various pH values and eluates were subjected to reversed phase chromatography (Example 1). Preferred peptides for use of the invention obtainable by multiple synthesis which have the biological activity of ALB408-423 are 12-mer derivatives of the 16-mer peptide ALB408-423. Starting with the C terminal truncation of ALB408- 423 (LVRYTKKVPQVS; SEQ ID NO: 12), subsequent peptides were generated through amino acid substitutions. Firstly, leucine was replaced with isoleucine resulting in IVRYTKKVPQVS (SEQ ID NO: 21). Then, tyrosine in SEQ ID NO: 21 was replaced with tryptophan resulting in IVRWTKKVPQVS (SEQ ID NO: 32). Replacement of lysine with cysteine in SEQ ID NO: 32 resulted in I RWTCKVPQVS (SEQ ID NO: 33). Substitution of threonine, cysteine, and glutamine in SEQ ID NO: 33 by serine, lysine and cysteine, respectively resulted in IVRWSKKVPCVS (SEQ ID NO: 34). Further 12-mer derivatives of ALB408-423 derived from the abovementioned derivatives are shown in Table 1 (Example 4; SEQ ID NOs: 35 to 45).

[0244] Use of IVRYTKKVPQVS (SEQ ID NO: 21) in the treatment of sarcoma

[0245] A patient (male, 21 years old) suffering from osteosarcoma was treated under the terms of compassionate use.

[0246] The peptide IVRYTKKVPQVS (SEQ ID NO: 21) was provided in a citrate / bicarbonate buffer (Pharmacopoeia Europaea, Ph. Eur.), in a concentration of 1 mg / mL (1 mg of the peptide in 1 mL of the buffer). This composition was administered intranasally as a nasal spray. The nasal spray was administered three times a day (morning, noon, evening), two strokes into each nasal cavity, for six weeks. The course of the disease / clinical picture of the patient was monitored via an immune panel measurement, once a week. In blood count measurements, the numbers of erythrocytes, thrombocytes, etc. were determined. After four weeks of treatment, the blood picture was normalized. Before the start of the treatment, the patient suffered from severe pain and was unable to move. During the course of the treatment the mobility was restored and after six weeks of treatment, the patient was pain-free. A regression of the tumor, especially in the area of the spine and the vertebral holes, was observed in MRT measurements. No side effects were observed.

Claims

Claims1. A peptide for use in treating and / or preventing sarcoma, wherein the peptide is selected from the group consisting of the following :(i) a peptide having the following amino acid sequence:Z1-LVRYTKKVPQVSTPTL-Z2 (ALB-408);(ii) peptides obtainable by multiple synthesis which have the biological activity of ALB408-423; and(iii) biologically active fragments and / or variants and / or derivatives, especially PEGylated, HESylated, amidated, acetylated, sulfated, phosphorylated and / or glycosylated derivatives, of (i) and (ii); wherein Z represents a number of from 0 to 10 amino acid residues.
