AI Technical Summary
Current anesthetic recovery methods are prolonged due to opioid-induced sedation and respiratory depression, with opioid antagonists reducing both beneficial analgesia and side effects, necessitating a need for agents that accelerate recovery while preserving analgesic effects.
Utilization of methyl 3-((methylsulfonyl)amino)-2-(((4-phenylcyclohexyl)oxy)methyl)piperidine-1-carboxylate (Compound (I)) or its salts as orexin type 2 receptor agonists to promote postoperative recovery by reducing opioid-induced sedation and respiratory depression without affecting analgesia.
Compound (I) shortens anesthesia recovery time, reduces opioid-induced sedation and respiratory depression, and maintains pain relief, providing a safer and more efficient postoperative recovery process.
Abstract
Description
[Technical field]
[0001] Certain embodiments of the present invention relate to the use of orexin type 2 receptor agonists for post-operative recovery (amelioration of delayed consciousness after maintenance of anesthesia, and amelioration of opioid-induced sedation and respiratory depression). [Background technology]
[0002] Currently, there is no drug that shortens the time required for recovery from anesthesia. To suppress pain during the postoperative period, an appropriate amount of analgesics, especially opioids, is usually used. However, the side effects associated with opioids, such as sedation and respiratory depression, may limit the use of opioids after surgery (Non-Patent Document 1). Opioid antagonists such as naloxone are widely used when opioid-induced severe sedation and respiratory depression are observed. However, opioid antagonists reduce not only these side effects but also the analgesic effect of opioids. Therefore, there is a need for a drug that accelerates the recovery time from anesthesia and suppresses opioid-induced side effects while maintaining the analgesic effect of opioids.
[0003] The orexin system plays an important role in controlling sleep / wake states (Non-Patent Document 2). There are two postsynaptic G protein-coupled receptors for orexin peptides: orexin 1 receptor (OX1R) and orexin 2 receptor (OX2R) (Non-Patent Document 3). Of these two receptors, OX2R is thought to play a crucial role in sleep / wake regulation, since abnormal sleep cycles are observed in OX2R KO mice but not in OX1R KO mice (Non-Patent Document 2). As an orexin 2 receptor agonist, methyl 3-((methylsulfonyl)amino)-2-(((4-phenylcyclohexyl)oxy)methyl)piperidine-1-carboxylate (compound (I)) has been disclosed (Patent Document 1). [0112] The following examples are presented to provide those of ordinary skill in the art with a complete disclosure and description of how to make and use the compounds and therapeutic methods of the present invention, and are not intended to limit the scope of what the inventors regard as their invention.
[0113] Compound (I) (methyl 3-((methylsulfonyl)amino)-2-(((4-phenylcyclohexyl)oxy)methyl)piperidine-1-carboxylate) was prepared according to the method described in WO2017 / 135306 A1 and used in the following experimental examples.
[0114] Recovery time from anesthesia (isoflurane and propofol) Experimental Example 1: Effect of Compound (I) on recovery time from isoflurane-induced anesthesia as assessed by righting reflex in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. To induce and maintain isoflurane anesthesia, rats were placed in a pre-filled anesthesia induction chamber (insoflurane 4%). Anesthesia induction was defined as the inability to move left or right in the prone position (loss of righting reflex: LORR) after the rat was placed in the dorsal position in the cage. After confirming LORR, rats were exposed to 3% isoflurane using a face mask and maintained for 30 minutes. 15 minutes after LORR, Compound (I) was administered subcutaneously (SC) to rats at a volume of 5 mL / kg body weight (BW). As a vehicle-treated control, rats were administered Captisol® (CyDex Pharmaceuticals, KS, USA) 10.5% (w / v) / 1.5 mM Na2HPO4 in distilled water. Thirty minutes after confirming LORR, isoflurane inhalation was terminated, and the time until recovery from LORR was recorded as the anesthesia time. Body temperature during and after anesthesia was maintained at 37.0-37.5 °C using a heating pad. The results are shown in Table 1.
[0115] [Table 1]
[0116] Experimental Example 2: Effect of Compound (I) on recovery time from propofol-induced anesthesia as assessed by righting reflex in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. A bolus dose of propofol (10 mg / kg) was administered intravenously to the rats to induce and maintain propofol anesthesia. After confirming LORR, the rats were gently placed in a supine position on a heating pad to maintain body temperature at 37.0-37.5°C. Propofol was then administered continuously at 60 mg / kg / hr for 30 min. 15 min after LORR, Compound (I) was administered SC to the rats at 5 mL / kg body weight. As a vehicle-treated control, rats were administered 10.5% (w / v) Captisol® / 1.5 mM Na2HPO4 in distilled water. Thirty min after confirming LORR, the propofol infusion was terminated and the time to recovery from LORR was recorded as the anesthesia time. The results are shown in Table 2.
[0117] [Table 2]
[0118] As can be seen from Tables 1 and 2, Compound (I) shortened the recovery time from isoflurane- and propofol-induced anesthesia as assessed by righting reflex in rats.
[0119] Suppression of opioid (fentanyl)-induced side effects without affecting pain Experimental Example 3: Effect of Compound (I) on fetanyl (0.1 mg / kg)-induced sedation as assessed by righting reflex in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. In this study, LORR was used as an index of sedation. To induce sedation, 0.1 mg / kg fentanyl was administered SC to rats at 2 mL / kg body weight. Ten minutes after fentanyl administration, all rats were observed for LORR, and then Compound (I) was administered SC at 5 mL / kg body weight. Vehicle-treated controls were administered 10.5% (w / v) Captisol® / 1.5 mM Na2HPO4 in distilled water. The recovery time from LORR was recorded as the duration of sedation.
[0120] The results are shown in Table 3. Compound (I) reduced the duration of fentanyl-induced sedation as assessed by righting reflex in rats.
[0121] [Table 3]
[0122] Experimental Example 4: Effect of Compound (I) on fentanyl (0.1 mg / kg)-induced respiratory depression assessed by whole-body plethysmography (WBP) in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. Respiratory function was assessed by WBP, which was continuously supplied with fresh room air and equipped with inflow and outflow ports to remove exhaled carbon dioxide. Under these experimental conditions, rats were placed in the WBP chamber immediately after administration of fentanyl (0.1 mg / kg, SC). Respiratory parameters, such as respiratory rate (RR), tidal volume (TV), and minute ventilation (MV), were detected from a pressure transducer and recorded with data acquisition software (FinePoint software, Data Sciences International, Inc.). All respiratory parameters were analyzed as individual integrated values of 1-minute measurements from 0 to 30 minutes after fentanyl administration, integrated over a 30-minute period. Compound (I) was administered SC to rats 15 minutes before fentanyl administration.
[0123] The results are shown in Table 4. As a result, fentanyl (0.1 mg / kg as vehicle group) decreased RR and MV compared to the basal group. Under these experimental conditions, compound (I) increased RR, TV and MV compared to the vehicle group, suggesting that compound (I) suppressed fentanyl-induced respiratory depression in rats.
[0124] [Table 4]
[0125] Experimental Example 5: The effect of Compound (I) on fentanyl-induced analgesia assessed by using the formalin test in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. 2.5% by volume formalin solution was SC injected into the left hind leg 30 min after administration of compound (I), and fentanyl (0.045 mg / kg) was administered 15 min after administration of compound (I). For the measurement of pain threshold, the number of pain behaviors (flinching) was counted at 13 time points: 1, 6, 11, 16, 21, 26, 31, 36, 41, 46, 51, 56, and 61 min after formalin injection. Phase I (pain caused by direct stimulation of the peripheral nervous system from formalin) was 1 to 11 min after formalin injection, and Phase II (pain related to increased sensitivity of spinal dorsal horn neurons induced by stimulation in Phase I) was 16 to 61 min after formalin injection, and the number of flinchings in each phase was summed.
[0126] The results are shown in Table 5. Fentanyl (0.045 mg / kg) exhibited analgesic effects in the formalin pine model, as assessed by a reduction in the total number of flinches in rats. Under these experimental conditions, Compound (I) did not interfere with the analgesic effects of fentanyl in rats.
[0127] [Table 5]
[0128] Experimental Example 6: Effect of Compound (I) on fentanyl-induced analgesia assessed by using a postoperative skin incision pain model in rats. Eight-week-old male Sprague-Dawley rats were purchased from Charles River, Japan. Under isoflurane anesthesia, a 1 cm longitudinal incision was made through the skin of the plantar aspect of the right foot, starting 0.5 cm from the edge of the heel and extending toward the toes. The exposed plantar muscle was lifted with tweezers and incised longitudinally. After the plantar muscle was returned to its original position, the skin incision was mattress sutured in two places with sterile needle thread (nylon, 5-0). After suturing, an ointment containing an antibiotic was applied to the surgical site. The surgical foot was disinfected with iodine and alcohol, and sterile gloves and sterile surgical instruments were used. Measurement of the pain threshold (g) of the right hind footpad was performed using a dynamic plantar anesthesia meter set to apply pressure from 0 g to 50 g in 10 seconds. The pain threshold was the average of three measurements. Fentanyl was administered SC immediately after skin incision, and compound (I) was administered SC 15 min before skin incision.
[0129] The results are shown in Table 6. Fentanyl (0.03 mg / kg) showed analgesic effects in a postoperative skin incision model, as measured by pain threshold (g) in rats. Under these experimental conditions, Compound (I) did not interfere with the analgesic effects of fentanyl in rats.
[0130] [Table 6]
[0131] The results suggest that compound (I), an OX2R selective agonist, may accelerate recovery time from anesthesia and suppress opioid-induced side effects without affecting pain.
