Description

[Technical field]

[0001] This application claims priority to U.S. Provisional Patent Application No. 63 / 177,616, filed April 21, 2021, which is incorporated by reference in its entirety.

[0002] Provided herein are methods of using 2-(2,6-dioxopiperidin-3-yl)-4-((2-fluoro-4-((3-morpholinoazetidin-1-yl)methyl)benzyl)amino)isoindoline-1,3-dione, or an enantiomer, mixture of enantiomers, tautomer, isotopologue, or pharmaceutically acceptable salt thereof in combination with tafasitamab, obinutuzumab, or tazemetostat to treat, prevent, or manage non-Hodgkin's lymphoma. [Background technology]

[0003] Cancer is primarily characterized by an increase in the number of abnormal cells originating from a given normal tissue, the invasion of adjacent tissues by these abnormal cells, or the lymphatic or hematogenous spread and metastasis of malignant cells to regional lymph nodes. Clinical data and molecular biology studies indicate that cancer is a multi-step process that may begin with minor precancerous changes and progress to neoplasia under certain conditions. Neoplastic lesions may develop clonally and develop increased capacities for invasion, growth, metastasis, and heterogeneity, especially under conditions where neoplastic cells evade the host's immune surveillance. Current cancer therapies may require surgery, chemotherapy, hormonal therapy, and / or radiation therapy to eradicate neoplastic cells in patients. Recent advances in cancer therapy are described by Non-Patent Document 1.

[0004] All current cancer therapies have significant drawbacks for patients. Surgery may be contraindicated, for example, due to the patient's health, or may not be tolerated by the patient. Furthermore, surgery may not be able to completely remove the neoplastic tissue. Radiotherapy is only effective if the neoplastic tissue exhibits a higher sensitivity to radiation than normal tissue. Radiotherapy can also often induce serious side effects. Hormonal therapy is rarely administered as a single agent. Although hormonal therapy can be effective, it is often used to prevent or delay the recurrence of cancer after other treatments have removed the majority of the cancer cells.

[0005] Regarding chemotherapy, there are various chemotherapeutic agents available for the treatment of cancer. The majority of cancer chemotherapeutic agents act by inhibiting DNA synthesis by directly or indirectly inhibiting the biosynthesis of deoxyribonucleotide triphosphate precursors, thereby preventing DNA replication and the associated cell division. Non-Patent Document 2.

[0006] Despite the availability of a variety of chemotherapy agents, chemotherapy has many drawbacks. Non-Patent Document 3. Nearly all chemotherapy agents are toxic, and chemotherapy causes significant and often dangerous side effects, including severe nausea, bone marrow suppression, and immunosuppression. Furthermore, even when a combination of chemotherapy agents is administered, many tumor cells are resistant to or develop resistance to the chemotherapy agents. In fact, cells that are resistant to a particular chemotherapy agent used in a treatment protocol often prove to be resistant to other drugs, even if those agents act by a mechanism different from that of the drug used in the particular treatment. This phenomenon is called pleiotropic drug or multidrug resistance. Due to drug resistance, many cancers prove or become refractory to standard chemotherapy treatment protocols.

[0007] Non-Hodgkin lymphoma (NHL), also known as non-Hodgkin's lymphoma, is the fifth most common cancer in both men and women in the United States. An estimated 385,700 patients worldwide were diagnosed with NHL in 2012, and approximately 199,700 patients died as a result of the disease. Non-Patent Document 4. NHL is a heterogeneous disease that includes a variety of B-cell and T-cell lymphoma subtypes, which together account for approximately 4% of all new cancer cases and 3% of cancer-related deaths in the United States (US). Most NHLs (80%-90%) are of B-cell origin, and the majority of the remaining are T-cell lymphomas. Common subtypes of NHL include diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), mantle cell lymphoma (MCL), and primary central nervous system lymphoma (PCNSL).

[0008] Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of NHL, accounting for up to 30% of newly diagnosed cases, and is clinically classified as an aggressive lymphoma. With the introduction of a combination regimen of rituximab and chemotherapy, more than 50% of patients with DLBCL are cured. However, more than 30% of patients in remission will eventually relapse. For patients who relapse, treatment approaches for secondary DLBCL are less clearly defined and are often ineffective in achieving long-term disease control. In patients who have undergone second-line or higher treatment, have relapsed and / or are refractory, and are not candidates for potentially curative treatment due to advanced age or poor performance status, DLBCL remains an incurable disease for which clinical trials are indicated. New treatment approaches are still needed.

[0009] For follicular lymphoma (FL), the age-adjusted incidence rate in the United States in 2011-2012 was 3.4 per 100,000. There is no standard of care for relapsed or refractory (R / R) FL patients. Despite efforts and advances in first-line treatment, patients with FL continue to experience recurrent relapses and require further treatment. First-line systemic anticancer treatment is also considered in second-line treatment; more recently, second-line and beyond treatment options may include "chemotherapy-free" regimens that are under development and may become standard of care in the near future. In third-line treatment, patients who do not respond to rituximab-containing regimens and who relapse or are refractory to further treatment have limited treatment options and poor prognosis. There is a high unmet medical need to develop novel treatments for FL patients who do not respond to standard treatments and have exhausted treatment options for disease remission.

[0010] Approximately 6% of all new lymphoma cases each year are mantle cell lymphoma (MCL). The age-adjusted incidence rate of mantle cell lymphoma (MCL) in the United States in 2011-2012 was 0.8 per 100,000. Despite several available first-line treatments for MCL with long-term responses, MCL remains an incurable B-cell malignancy. Patients with MCL are often treated with a rituximab-chemotherapy combination with or without consolidation therapy with stem cell transplantation. Relapse is common, and MCL becomes increasingly resistant to treatment over time.

[0011] The age-adjusted incidence rate of primary central nervous system lymphoma (PCNSL) in the United States in 2011-2012 was 0.3 per 100,000. Despite high response rates with initial high-dose methotrexate (HD-MTX)-based regimens, more than half of responders relapse. Once PCNSL relapses, the prognosis remains poor. Novel therapeutic agents with CNS penetration, better efficacy, and acceptable toxicity profiles are urgently needed.

[0012] There remains a great need for safe and effective methods of treating, preventing and managing NHL, particularly NHL that is refractory to standard therapies such as surgery, radiation therapy, chemotherapy and hormonal therapy, while reducing or avoiding the toxicity and / or side effects associated with conventional therapies.

[0020] definition Unless otherwise defined, all technical and scientific terms used herein have the same meaning as commonly understood by those skilled in the art. All patents, applications, published applications and other publications are incorporated by reference in their entirety. In the event that there are multiple definitions for terms herein, the definitions in this section prevail unless otherwise stated.

[0021] As used herein, and in this specification and the appended claims, the indefinite articles "a" and "an" and the definite article "the" include plural as well as singular referents unless the context clearly dictates otherwise.

[0022] As used herein, the terms "comprising" and "including" may be used synonymously. The terms "comprising" and "including" should be interpreted as specifying the presence of the features or components described as referred to, but do not exclude the presence or addition of one or more features, or components thereof, or groups. Furthermore, the terms "comprising" and "including" are intended to include examples encompassed by the term "consisting of". As a result, the term "consisting of" may be used in place of the terms "comprising" and "including" to provide more specific embodiments of the present invention.

[0023] The term "consisting of" means that the object has at least 90%, 95%, 97%, 98% or 99% of the recited features or components of which it is composed. In another embodiment, the term "consisting of" excludes from the scope of any succeeding recitation any other features or components, excepting those that are not essential to the technical effect to be achieved.

[0024] As used herein, the term "or" should be interpreted as an inclusive "or" meaning any one or any combination. [0026] As used herein and unless otherwise indicated, the term "stereoisomer" or "stereoisomerically pure" refers to one stereoisomer of a compound that is substantially free of other stereoisomers of that compound. For example, a stereoisomerically pure compound having one chiral center will be substantially free of the opposite enantiomer of that compound. A stereoisomerically pure compound having two chiral centers will be substantially free of other diastereomers of that compound. A typical stereoisomerically pure compound comprises more than about 80% by weight of one stereoisomer of the compound and less than about 20% by weight of the other stereoisomers of the compound, more than about 90% by weight of one stereoisomer of the compound and less than about 10% by weight of the other stereoisomers of the compound, more than about 95% by weight of one stereoisomer of the compound and less than about 5% by weight of the other stereoisomers of the compound, or more than about 97% by weight of one stereoisomer of the compound and less than about 3% by weight of the other stereoisomers of the compound. The compounds may have chiral centers and may exist as racemates, individual enantiomers or diastereomers, and mixtures thereof, and all such isomers are included within the embodiments provided herein, including mixtures thereof.

[0027] The use of stereoisomerically pure forms of such compounds, as well as mixtures of those forms, are encompassed by the embodiments provided herein. It should be understood that the chiral centers of the compounds provided herein may undergo epimerization in vivo. Thus, those skilled in the art will recognize that administration of a compound in its (R) form is equivalent to administration of a compound in its (S) form in the case of compounds that undergo epimerization in vivo.

[0029] Optically active (+)- and (−), (R)- and (S)-, or (D)- and (L)-isomers may be prepared using chiral synthons or chiral reagents, or resolved using conventional techniques, such as chromatography on chiral stationary phases.

[0030] "Tautomers" refer to isomers of a compound that are in equilibrium with each other. The concentration of isomers depends on the environment in which the compound is found, and may vary depending on, for example, whether the compound is a solid or in an organic or aqueous solution. For example, in an aqueous solution, pyrazole may exhibit the following isomers, which are called tautomers of each other: [ka]

[0031] As will be readily appreciated by one of ordinary skill in the art, a wide variety of functional groups and other structures may exhibit tautomerism, and all tautomers of the compounds are within the scope of the compounds provided herein.

[0032] It should also be noted that the compounds provided herein may contain unnatural proportions of atomic isotopes at one or more of the atoms. For example, the compounds may contain, for example, tritium ( 3 H), iodine-125( 125 I), Sulfur-35( 35 S), or carbon-14 ( 14 It may be radiolabeled with a radioisotope such as C, or with deuterium ( 2H), Carbon-13( 13 C), or nitrogen-15 ( 15 As used herein, an "isotopologue" is an isotope-enriched compound. The term "isotopically enriched" refers to an atom having an isotopic composition other than the natural isotopic composition of that atom. Approval in R / R FL was based on data from the GADOLIN study (Sehn LH, et al., Lancet Onocol. 2016;17(8)1081-1093), an open-label, randomized, phase 3 study for adult patients with CD20-positive indolent non-Hodgkin's lymphoma refractory to rituximab. Patients were randomized to receive obinutuzumab in combination with bendamustine or bendamustine monotherapy in six 28-day cycles of obinutuzumab 1000 mg (days 1, 8, and 15, cycle 1; day 1, cycles 2-6) and bendamustine 90 mg / m 2 / day (days 1 and 2, cycles 1 to 6), and bendamustine monotherapy at 120 mg / m 2 / day (days 1 and 2, all cycles). Non-progressing patients in the obinutuzumab and bendamustine group received obinutuzumab maintenance therapy (1000 mg every 2 months) for up to 2 years. In total, 396 patients were randomized, of whom 194 were assigned to the obinutuzumab and bendamustine arm. After a median follow-up of 21.9 months (obinutuzumab and bendamustine) and 20.3 months (bendamustine monotherapy), the primary endpoint, PFS, was significantly longer in the obinutuzumab and bendamustine arm (NR, not reached, [95% CI 22.5 months-not estimable]) compared with 14.9 months (95% CI: 12.8 to 16.6 months) for the bendamustine monotherapy arm (hazard ratio 0.55 [95% CI: 0.40 to 0.74]). Serious AEs occurred in 38% of patients in the obinutuzumab and bendamustine group and 33% of patients in the bendamustine monotherapy group, with deaths due to adverse events occurring in 6% of patients in each group. The most common AEs (>20%) of all grades were nausea, fatigue, neutropenia, cough, pyrexia, diarrhea, and vomiting in both arms, plus thrombocytopenia in the monotherapy arm and constipation in the combination arm. Grade 3 / 4 neutropenia was reported in 26% and 33% of patients in the monotherapy and combination arms, respectively; grade 3 / 4 thrombocytopenia in 15% and 17%, respectively; and anemia in 8% and 10%, respectively. Febrile neutropenia was rare in both arms. Tazemetostat (also known as EPZ-6438) has the chemical name N-[(1,2-dihydro-4,6-dimethyl-2-oxo-3-pyridinyl)methyl]-5-[ethyl(tetrahydro-2H-pyran-4-yl)amino]-4-methyl-4'-(4-morpholinylmethyl)-[1,1'-biphenyl]-3-carboxamide, and the structure: [ka] has.

[0076] Tazemetostat is a selective inhibitor of histone methyltransferase enhancer of zest homolog 2 (EZH2), which blocks the methylation of lysine 27 in histone H3, altering gene expression patterns related to cancer pathways and reducing tumor cell proliferation (Vire E, et al., Nature. 2006; 439 (7078) 871-874). Somatic gain-of-function mutations in EZH2 have been identified in a variety of tumors, particularly in FL, and studies have found that up to 27% of patients have EZH2 mutation status (Bodor C, et al., Blood. 2013; 122 (18) 3165-3168). Tazemetostat recently received accelerated approval from the FDA for the treatment of R / R FL in adult patients whose tumors are positive for EZH2 mutations as detected by an FDA-approved test, have received at least two prior systemic therapies, and have R / R FL with no satisfactory alternative treatment options. Approval was based on data from two single-arm cohorts of a multicenter study that enrolled patients with FL who had received two or more prior systemic therapies. Treatment consisted of tazemetostat 800 mg orally twice daily until disease progression or discontinuation due to toxicity. The ORR in 42 patients with EZH2 mutation status was 69%, with a CR rate of 12%; the mDOR for these patients was 10.9 months. Among 53 patients with EZH2 wild-type status, the ORR was 34%, with a CR rate of 4%; the mDOR for this cohort was 13 months. Serious AEs occurred in 30% of patients, with the most common being general physical health deterioration, abdominal pain, pneumonia, sepsis, and anemia in ≥2% of patients. The most common AEs of any grade (≥20%) were fatigue, upper respiratory tract infection, nausea, abdominal pain, and musculoskeletal pain. Grade 3 / 4 neutropenia was reported in 7% of patients, thrombocytopenia in 7%, and anemia in 8%. Important safety information includes warnings for secondary malignancies and embryo-fetal toxicity.

The cycles may include, in certain cases, varying the administration period and / or rest period described herein. EXAMPLES

[0201] The following examples are offered by way of illustration and not by way of limitation.

[0202] Example 1: Phase I Clinical Trial A Phase 1, multicenter, open-label study is being conducted to evaluate the safety, pharmacokinetics, and preliminary efficacy of an orally available small molecule, Compound 1, alone and in combination with anti-lymphoma agents in subjects with relapsed or refractory non-Hodgkin's lymphoma (R / R NHL).

[0203] Indications: relapsed or refractory diffuse large B-cell lymphoma (R / R DLBCL); relapsed or refractory follicular lymphoma (R / R FL); relapsed or refractory primary central nervous system lymphoma (R / R PCNSL); relapsed or refractory mantle cell lymphoma (R / R MCL).

[0204] Objectives: The primary objective of the study is to determine the safety and tolerability of Compound 1, alone and in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat, in subjects with R / R NHL. Another primary objective is to define the maximum tolerated dose (MTD) and / or recommended phase 2 dose (RP2D) of Compound 1 in subjects with R / R NHL.

[0205] Secondary objectives are to characterize the pharmacokinetics (PK) of Compound 1 and provide preliminary efficacy information of Compound 1 alone and in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat in R / R NHL.

[0206] Study Design: This is an open-label, Phase 1, dose-escalation (Part A) and dose-expansion (Part B), first-in-human (FIH) clinical trial of Compound 1, administered orally, alone and in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat. Compound 1 will be administered in the form of the hydrochloride salt. Compound 1 will be administered as monotherapy in subjects with R / R NHL (including DLBCL (de novo or transformed), FL, MCL, or PCNSL) who have failed at least two prior lines of therapy (or have received at least one prior line of standard therapy and are not eligible for any other therapy). The combination of Compound 1 and rituximab will be studied in subjects with R / R DLBCL or R / R FL. Compound 1 will also be tested in combination with obinutuzumab in R / R FL subjects, in combination with tafasitamab in R / R DLBCL subjects, and in combination with tazemetostat in R / R DLBCL and R / R FL subjects.

[0207] Dose escalation (Part A) evaluates the safety and tolerability of escalating doses of Compound 1 in R / R DLBCL and R / R FL to determine the MTD of Compound 1 as monotherapy. An accelerated titration design is used at the initial dose level; then, a two-parameter Bayesian logistic regression model (BLRM) with escalation with overdose control (EWOC) (Babb J, Rogatko A, Zacks S. Cancer phase I clinical trials: efficient dose escalation with overdose control. Stat Med 1998;17(10):1103-20; Neuenschwander B, Branson M, Gsponer T. Critical aspects of the Bayesian approach to phase I cancer trials. Stat Med 2008;27(13);2420-39) helps guide the dose escalation / de-escalation decision of Compound 1.

[0208] Part B will evaluate the safety and efficacy of Compound 1, administered alone or in combination, at or below the MTD in selected expansion cohorts of up to about 20 evaluable subjects each in R / R DLBCL and R / R FL, MCL, or PCNSL to determine the RP2D. In addition to testing Compound 1 monotherapy, combinations tested in Part B may include Compound 1 in combination with rituximab in R / R DLBCL and R / R FL subjects, Compound 1 in combination with obinutuzumab in R / R FL subjects, Compound 1 in combination with tafasitamab in R / R DLBCL subjects, and Compound 1 in combination with tazemetostat in R / R DLBCL and R / R FL subjects.

[0209] Part B Cohort B evaluates the effect of food on the PK of Compound 1 in a subset of approximately 10-12 evaluable subjects with R / R FL, MCL, and / or PCNSL. Subjects are randomized into two groups and receive an oral dose of Compound 1 after consumption of a standard high-fat, high-calorie breakfast on day 1 of cycle 1 in group 1 or the last dosing day in the first dosing period (e.g., day 7 of cycle 1 in a 7 / 14 dosing schedule) in group 2. For the remainder of the treatment period, subjects receive an oral dose of Compound 1 in the fasted state. Data from this food effect cohort provides an exploratory evaluation of the effect of food on the PK profile of Compound 1 in subjects.

[0210] Compound 1 will be administered orally once daily (QD) on the scheduled dosing days. Part B expansion cohorts may test different doses and / or schedules of Compound 1 based on safety and tolerability determined in Part A. All treatments will be administered in 28-day cycles until clinically significant disease progression, unacceptable toxicity, or subject / physician decision to discontinue.

[0211] In Part A, each subject will receive an assigned dose of Compound 1 on day 1 of cycle 1 and once daily thereafter on the planned dosing days. The starting dose / schedule of Compound 1 is 0.4 mg / day starting on day 1 for 5 consecutive days followed by 2 days of study drug rest (5 / 7 day schedule) every 7 days of each 28 day cycle. If the starting dose / schedule is not tolerated, a lower dose or a lower intensity schedule may be explored. The planned dosing cohorts for Compound 1 in Part A include 0.1 mg (dose level-2), 0.2 mg (dose level-1), 0.4 mg (dose level 1), 0.6 mg (dose level 2), 0.8 mg (dose level 3), 1.2 mg (dose level 4), and 1.6 mg (dose level 5). Alternative dosing schedules based on consideration of available clinical safety, PK and PD data may be explored, including, for example, starting on day 1 for 7 consecutive days followed by a 7-day study drug holiday every 14 days in each 28 day cycle (7 / 14 day schedule); starting on day 1 for 5 consecutive days followed by a 9-day study drug holiday every 14 days in each 28 day cycle (5 / 14 day schedule); starting on day 1 for 14 consecutive days followed by a 14-day study drug holiday every 28 days in each 28 day cycle (14 / 28 day schedule); and starting on day 1 for 21 consecutive days followed by a 7-day study drug holiday every 28 days in each 28 day cycle (21 / 28 day schedule).

[0212] In Part A, after the first dose is administered at any dose level, subjects at each dose level are observed for at least 28 days (Cycle 1, Days 1-28, Dose-Limiting Toxicity [DLT] Evaluation Period) before the next higher dose level may begin. The first two subjects in any escalating dose level will enter the DLT period, separated by at least 1 day.

[0213] After completion of dose escalation (Part A), selected expansion cohorts of approximately 20 efficacy-evaluable subjects per cohort will receive Compound 1 alone or in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat. Doses, schedules, and treatment regimens may be selected for cohort expansion. Expansion may occur at the MTD established in Part A and / or at lower doses, or alternative acceptable dosing schedules based on review of available safety, PK, and PD data. Compound 1 doses may be selected to be tested in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat in Part B, which may be initially evaluated in 6 subjects (safety run-in) before opening up each of Cohorts C-H to approximately 20 total subjects. If the dose of Compound 1 is not tolerated in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat, a lower dose level of Compound 1 may be explored. After review of available safety, PK, and / or PD observed in at least 6 subjects (safety run-in phase), doses / schedules for cohorts C-H may be selected. Combination expansion cohorts may open after review of data for 6 subjects treated for a given combination. Recommended doses may vary for each combination. One or more dosing regimens may be selected for cohort expansion.

[0214] In addition, further dose exploration with prophylactic use of granulocyte colony-stimulating factor (G-CSF) may be performed in Part A. Two findings may trigger such an investigation: if neutropenia (i.e., prolonged severe neutropenia or febrile neutropenia) proves to be the major toxicity determining the MTD, and if analysis of PD response indicates that the maximum effect has been achieved, it suggests exploring a higher Compound 1 dose. After reviewing the entirety of PK, PD, safety and efficacy data, the SRC may choose to open a cohort in Part A after establishing the MTD without the use of prophylactic G-CSF in Cycle 1 to test whether optimal management of neutropenia would benefit from prophylactic G-CSF use in Cycle 1. One or more cohorts in Part B may be opened based on the MTD as determined without the use of prophylactic G-CSF, while dose exploration with the use of prophylactic G-CSF in Part A continues based on review of the data.

[0215] Subjects enrolled in cohorts selected to receive prophylactic growth factor support with G-CSF (i.e., filgrastim) in cycle 1 will receive filgrastim by subcutaneous or IV injection twice weekly during non-dosing weeks at 5 mcg / kg / day (e.g., 300 mcg for a 60 kg subject administered twice weekly on weeks 2 and 4 on a 7 / 14 day schedule and weeks 3 and 4 on a 14 / 28 day schedule). Different schedules for G-CSF may be used in cycle 1. After cycle 1, the dose and schedule of prophylactic G-CSF (e.g., filgrastim or pegfilgrastim) will be determined and adjusted as needed by the investigator depending on the response observed for each subject. Therapeutic use of G-CSF in cycles 1 and beyond is at the discretion of the investigator. Filgrastim may be administered in either an inpatient or outpatient setting according to locally approved filgrastim prescribing information or local institutional practice.

[0216] The study will be conducted in accordance with the International Conference on Harmonisation (ICH) Good Clinical Practices (GCP).

[0217] Study Population: Subjects (male or female) aged 18 years or older with R / R NHL that has relapsed after standard anti-cancer therapy, progressed during standard anti-cancer therapy (or become intolerable due to medical comorbidities or unacceptable toxicity), or for which no other approved conventional treatment exists, will be enrolled in the study.

[0218] This multicenter, open-label study will enroll approximately 205 subjects with R / R NHL. Approximately 45 subjects with R / R DLBCL or R / R FL will be enrolled in Part A to determine the MTD of Compound 1 monotherapy. Part B will further evaluate the safety and efficacy of Compound 1 administered at or below the MTD (alone or in combination with rituximab, obinutuzumab, tafasitamab, or tazemetostat) in selected expansion cohorts in subjects with R / R DLBCL, FL, MCL, or PCNSL of up to approximately 20 evaluable subjects in each cohort (C-H) to determine the RP2D. Potential food effects on the PK of Compound 1 will also be investigated in a subset of 10-12 subjects in Part B Cohort B. Approximately 160 total subjects will be studied in Part B to evaluate the RP2D. Enrollment will occur in the United States (US), Europe, and / or Canada for the trial. Additional locations may be added for Part B.
