Description

[Technical field]

[0001] The present invention relates to a transfer device and a transfer method. [Background technology]

[0002] In recent years, a treatment has been performed in which cultured cells are formed into a sheet-like graft and then placed on a damaged organ (see, for example, Patent Document 1). When a sheet-like graft is grafted onto the surface of an organ, it can survive for a long time by using the blood vessels on the surface of the organ as a vascular bed.

[0003] In addition to transplants having cultured cells, sheet-shaped members may be placed in the body. Hereinafter, sheet-shaped implants and other members are referred to as sheet-shaped placement items. [Prior art documents] [Patent documents]

[0004] [Patent Document 1] JP 2013-198442 A Summary of the Invention [Problem to be solved by the invention]

[0005] Since sheet-shaped implants and other sheet-shaped implants have a certain size, their placement on the surface of an internal organ is usually performed by surgically cutting the living body open to expose the surface of the organ, but surgically cutting the living body open is a heavy burden on the patient.

[0006] The present invention has been made to solve the above-mentioned problems, and has an object to provide a transportation device and transportation method that can easily transport and place a sheet-shaped implant at a desired location in the body in a minimally invasive manner. [Means for solving the problem]

[0007] The above object can be achieved by the invention described in (1) below. (1) The transport device of the present invention is a transport device for transporting a sheet-like indwelling object into the body, and is characterized in having a support device having a long shaft and a flexible support body that is fixed to the tip of the shaft and can be reduced in diameter along a radial direction centered on the long axis of the shaft, and a holding device having a groove or through hole formed therein that can accommodate at least a portion of the support device. Effect of the Invention

[0008] The transport device described in (1) above can transport the sheet-like indwelling object into the body via a cylinder inserted into the body by storing the support device, which has the sheet-like indwelling object wrapped around the support, in a groove or through-hole of the holding device. The support device can protrude from the groove or through-hole of the holding device inside the body, and can easily release the wrapped sheet-like indwelling object while spreading it out at the desired position by bending while reducing its diameter. Therefore, this transport device can easily transport and place the sheet-like indwelling object at the desired position inside the body in a minimally invasive manner.

[0009] (2) In the transport device described in (1) above, the support has a plurality of brush bristles protruding radially outward from the shaft, and the brush bristles may be inclined toward the tip side. This allows the support to be contracted in the radial direction and to be flexible. The frictional force between the brush bristles and the sheet-like object increases when the brush bristles move toward the tip and decreases when the brush bristles move toward the base. Therefore, by moving the support device toward the tip, the sheet-like object supported by the brush bristles can be transported well toward the tip, and by moving the support device toward the base, it is easy to move only the brush bristles toward the base while leaving the sheet-like object behind.

[0010] (3) In the delivery device described in (1) above, the support may have a foam protruding radially outward from the shaft, thereby making the support contractible along the radial direction and flexible.

[0011] (4) In the transport device according to any one of (1) to (3) above, the support device may have a flange fixed to the shaft on the proximal side of the support and protruding radially outward, whereby when the support device is moved in the distal direction to transport the sheet-like retaining object supported by the support in the distal direction, the flange can hold the proximal end of the sheet-like retaining object and suppressing the sheet-like retaining object from coming off the support and remaining unable to move in the distal direction.

[0012] (5) In the transfer device according to any one of (1) to (4) above, the shaft may be curved at the position where the support is fixed. With this, the support is straightened and stored in the holding device, and when released from the groove or through-hole of the holding device, it returns to its original curved shape, and the wrapped sheet-like retaining object can be spread and released. In addition, the curved support can support the spread sheet-like retaining object at multiple support points that spread two-dimensionally, so that the support can be well supported to the target position.

[0013] (6) The method of the present invention is a method of transferring a sheet-like indwelling object into a body, characterized in that the sheet-like indwelling object is wound around a support device having a long shaft and a flexible support device fixed to the tip of the shaft, capable of shrinking in a radial direction centered on the long axis of the shaft, and held by wrapping the sheet-like indwelling object around the support device, and the support device and the support device are inserted into the body through a cylindrical body inserted into the body in a state in which at least a part of the support device is accommodated in a groove or a through hole of the support device, and the support device is protruded from the support device inside the body, and the sheet-like indwelling object wound around the support device is spread and placed at a target position. This transfer method allows the sheet-like indwelling object to be transferred into the body through a cylindrical body inserted into the body. Therefore, this transfer method allows the sheet-like indwelling object to be easily transferred and placed at a target position in the body in a minimally invasive manner. [0015] Hereinafter, an embodiment of the present invention will be described with reference to the drawings. Note that the dimensions in the drawings may be exaggerated and may differ from the actual dimensions for the sake of explanation. In addition, in this specification and the drawings, components having substantially the same functions are given the same reference numerals to avoid repeated explanation. In this specification, the side of the device that is inserted into the living body is referred to as the "distal side" and the side that is operated is referred to as the "base side".

[0016] As shown in Figs. 1 and 2, the transfer device 10 according to this embodiment is a device that is inserted into the abdominal cavity or thoracic cavity via a trocar 100 (cylinder) that is percutaneously inserted into a body cavity such as the abdominal cavity or thoracic cavity, and transfers a sheet-like indwelling object 110 to the surface of an organ that can be reached from the abdominal cavity or thoracic cavity. Organs that can be reached from the abdominal cavity include, for example, the liver, pancreas, small intestine, large intestine, duodenum, and diaphragm (peritoneal cavity side). Organs that can be reached from the thoracic cavity include, for example, the surface of blood vessels such as the heart, lungs, and aorta. The transfer device 10 may be used to transfer the sheet-like indwelling object 110 to a place other than the abdominal cavity or thoracic cavity (for example, inside a large blood vessel).

[0017] The trocar 100 is a known device that is percutaneously inserted into the abdominal or thoracic cavity to provide an access passage while keeping the abdominal or thoracic cavity inflated with a gas such as carbon dioxide. The trocar 100 has a trocar tube 101 with an inner diameter of about 5 mm to 12 mm. The trocar tube 101 provides an access passage to the abdominal or thoracic cavity for other devices.

[0018] The sheet-shaped implant 110 is not particularly limited as long as it is a sheet-shaped, flexible implant, and may be, for example, a sheet-shaped implant formed of cultured cells in a sheet shape. The cells constituting the sheet-shaped implant include, for example, somatic stem cells, adult stem cells, mesenchymal stem cells, ES cells (embryonic stem cells), or differentiated cells derived from iPS cells (induced pluripotent stem cells). The somatic stem cells are, for example, skeletal myoblasts (myoblast cells). The differentiated cells are, for example, cardiomyocytes, fibroblast cells, etc.

[0019] The transfer device 10 has a support device 20 that supports the sheet-like indwelling object 110 and a holding device 60 that can hold at least a part of the transfer device 10.

[0020] The support device 20 has a long shaft 30 , a support 40 fixed to the tip end of the shaft 30 , and a flange 50 fixed to the shaft 30 on the base end side of the support 40 .

[0021] The shaft 30 is a long, flexible member. The shaft 30 has a shaft tip portion 31 to which the support 40 is fixed at the tip side, and a grip portion 32 that can be gripped and operated by an operator at the base end side. The shaft tip portion 31 has a curved portion 33 that is bent at an angle θ in a natural state in which no external force is applied. The angle θ is not particularly limited, but is greater than 0 degrees and less than 180 degrees, and is preferably 30 degrees to 60 degrees. The shaft tip portion 31 does not necessarily have to have the curved portion 33.

[0022] The cross-sectional shape of the shaft 30 is circular, but does not have to be circular. The material from which the shaft 30 is made is not particularly limited as long as it is flexible and has a certain degree of rigidity, and may be, for example, a metal such as a Ni-Ti alloy, stainless steel, a Cu alloy, or an Al alloy, or a resin. It is particularly preferable that the shaft 30 is made of a superelastic metal such as a Ni-Ti alloy. The length L1 of the shaft 30 in the longitudinal axis direction X is, for example, 100 mm to 400 mm.

[0023] The support 40 has a plurality of brush bristles 41 connected from near the tip to near the base end of the shaft tip 31. The plurality of brush bristles 41 are flexible and elastically deformable, and are formed with approximately the same length. The plurality of brush bristles 41 are arranged approximately evenly in the circumferential direction and the long axis direction X of the shaft tip 31. Each brush bristles 41 protrudes radially outward from a fixed position with respect to the shaft tip 31 and inclines toward the tip side. Each brush bristles 41 can support the sheet-like retaining object 110 at a point at its top. The angle α at which each brush bristles 41 inclines toward the tip side with respect to a plane perpendicular to the long axis of the shaft 30 is not particularly limited, but is more than 0 degrees and less than 90 degrees, and is preferably 5 degrees to 20 degrees. Note that each brush bristles 41 may not incline toward the tip side, and the angle α may be 0 degrees. The material of the multiple brush bristles 41 is not particularly limited as long as it can support the sheet-like retention object 110 at the top of the brush bristles 41 without damaging it. For example, resins such as polyamide, polypropylene, polyethylene, polyethylene terephthalate, PEEK (polyether ether ketone), polycarbonate, polyimide, polyetherimide, etc., or materials other than resins can be used.

[0024] The outer diameter D1 of the support 40 formed by the multiple brush bristles 41 is a dimension that can be accommodated in a holding device 60 that can be inserted into a trocar 100, and is, for example, 5 mm to 10 mm. The length L2 of the support 40 in the long axis direction X is, for example, 40 mm to 100 mm. The support 40 may be a foam (for example, a sponge) made of a soft resin.

[0025] The flange 50 is a member that suppresses the sheet-like retaining object 110 supported by the support 40 from shifting toward the base end side from the support 40. The flange 50 is fixed to the base end side of the shaft tip 31 to which the support 40 of the shaft 30 is fixed, close to the shaft tip 31. The flange 50 is formed in a disk shape arranged approximately perpendicular to the long axis direction X, but the shape is not limited. The outer diameter D2 of the flange 50 is preferably larger than the outer diameter D1 of the support 40 so that the sheet-like retaining object 110 supported by the support 40 can come into contact with the sheet-like retaining object 110 when the sheet-like retaining object 110 moves toward the base end. Furthermore, the outer diameter D2 of the flange 50 is preferably less than the inner diameter D3 of the holding device 60 described later so that the flange 50 can be accommodated in the holding device 60. The material of the flange 50 is not particularly limited as long as it can restrict the movement of the sheet-like retaining object 110, and metals such as Ni-Ti alloys, stainless steel, Cu alloys, and Al alloys, and resins can be used.

[0026] The holding device 60 is a device that holds the support device 20 that supports the sheet-like indwelling object 110 in order to transfer it into the body. The holding device 60 has a shape obtained by cutting a cylinder with a plane parallel to the central axis near the central axis. Therefore, the holding device 60 has a groove 61 that extends in the long axis direction X to hold the support device 20, and the cross section perpendicular to the long axis direction X is formed in an arc shape. Note that the shape of the cross section perpendicular to the long axis direction X of the holding device 60 does not have to be an arc shape.

[0027] Since the holding device 60 needs to be insertable into the trocar 100, when the inner diameter of the trocar 100 is 12 mm, the inner diameter D3 of the holding device 60 is preferably, for example, 10 mm or less. The length L3 of the long axis direction X of the holding device 60 is preferably, for example, 250 mm to 300 mm. The material of the holding device 60 is not particularly limited, and may be, for example, a metal such as a Ni-Ti based alloy, stainless steel, a Cu based alloy, or an Al based alloy, or a resin.

[0028] Next, a method of using the delivery device 10 according to this embodiment will be described using an example in which the sheet-like implant 110 is transplanted onto the surface of an organ in the abdominal cavity.

[0029] First, as shown in Figs. 3-4, the operator places the sheet-like retaining object 110 around the support 40 of the support device 20 so as to wrap it around the support 40. The sheet-like retaining object 110 supported by the support 40 is supported by the tops of a plurality of brush bristles 41. The brush bristles 41 are flexible, so that damage to the sheet-like retaining object 110 can be suppressed. The base end of the sheet-like retaining object 110 supported by the support 40 is disposed on the tip side of the flange 50. It is preferable that the ends of the sheet-like retaining object 110 wrapped around the support 40 do not overlap each other so that the sheet-like retaining object 110 can easily return to its original expanded shape. For this reason, the length L4 (see Fig. 2) of the sheet-like retaining object 110 wrapped around the support 40 in the circumferential direction is preferably equal to or less than the circumferential length of the support 40 accommodated in the holding device 60 having an inner diameter D3 of 10 mm or less. If the outer diameter D1 of the support 40 is 10 mm, the perimeter of the support 40 is 10πmm, and therefore the length L4 of the sheet-like retaining object 110 is preferably 10πmm or less. For example, if the length L4 of the sheet-like retaining object 110 is about 30 mm, which is 10πmm or less, the sheet-like retaining object 110 can be wound around the support 40 so that the ends do not overlap each other.

[0030] Next, the surgeon places the support device 20 with the sheet-like indwelling object 110 wrapped around it in the holding device 60. At this time, the surgeon corrects the curved portion 33 to a straight state by pushing the support device 20 or applying a rotational force to the support device 20, and the curved portion 33 can be placed in the groove 61 of the straight holding device 60. The support device 20 and the holding device 60 can be easily inserted into the trocar 100 because they are straight.

[0031] Next, as shown in Fig. 7(A), the surgeon inflates the abdominal cavity with gas by pneumoperitoneum and percutaneously inserts multiple trocars 100 into the abdominal cavity. Next, the surgeon inserts a laparoscope 120 into the abdominal cavity from one of the trocars 100, and while observing the abdominal cavity with the laparoscope 120, inserts a holding device 60 containing a support device 20 wrapped with a sheet-like indwelling object 110 into the abdominal cavity from another trocar 100. The support device 20 is held by the holding device 60 having a semicircular cross section, so that friction of the transport device 10 within the trocar 100 can be reduced, and damage and deformation of the sheet-like indwelling object 110 can be suppressed.

[0032] Since the side of the holding device 60 where the groove 61 is located is open, the surgeon can easily grasp the direction in which the holding device 60 is open using the laparoscope 120. Therefore, even with a narrow field of view during laparoscopic surgery, it is easy to position the holding device 60, the support device 20, and the sheet-like placement object 110 at the desired position (implantation site).

[0033] As shown in Figs. 5 and 7(B), the surgeon moves the support device 20 from the tip of the holding device 60 toward the tip. As a result, the curved portion 33, which had been forced to be straight, protrudes toward the tip side from the support device 20, and the curved portion 33 returns to its original shape. As a result, the support 40 returns to a curved shape together with the shaft tip portion 31, and a force is applied to the sheet-like retaining object 110 so that a gap is formed between the support 40 and the sheet-like retaining object 110. As a result, the sheet-like retaining object 110, which was wrapped around and in close contact with the support 40, tends to spread apart from the support 40 as if floating. At this time, since the support 40 is curved rather than straight, the spread sheet-like retaining object 110 can be well supported at multiple support points that spread two-dimensionally. Therefore, the support 40 can support the sheet-like retaining object 110 in a well-balanced manner without dropping it.

[0034] The surgeon places the sheet-like indwelling object 110 on the surface of the target organ while checking with the laparoscope 120. At this time, the surgeon may operate the sheet-like indwelling object 110 with forceps or the like inserted into the abdominal cavity via the trocar 100 to guide the sheet-like indwelling object 110 to the surface of the target organ. This allows the surgeon to accurately position the sheet-like indwelling object 110 at the target position. Next, the surgeon removes the support device 20 and the holding device 60 from the trocar 100 as shown in FIG. 7(C). Next, the surgeon places the second and subsequent sheet-like indwelling objects 110 on the surface of the target organ and transplants them, if necessary, in the same manner as the placement of the first sheet-like indwelling object 110 described above. At this time, the surgeon may arrange two or more sheet-like indwelling objects 110 side by side. Therefore, as shown in Fig. 6, for example, by arranging three sheet-like placement objects 110 each having a length L4 of 30 mm, a sheet-like placement object 110 with a total side length of 90 mm can be placed. In this manner, by using the transport device 10 according to this embodiment, a large sheet-like placement object 110 with a side length of about several tens of mm can be placed in a target position in the abdominal cavity in a minimally invasive manner through a narrow passage of the trocar 100. The sheet-like placement object 110 placed in a target position may be fixed to an organ or the like with fibrin glue or the like using the trocar 100 from which the support device 20 and the holding device 60 have been removed or another trocar 100.

[0035] As described above, the transport device 10 according to this embodiment is a transport device 10 for transporting a sheet-like indwelling object 110 into a body, and includes a support device 20 having a long shaft 30, a support 40 fixed to the tip of the shaft 30, capable of contracting in a radial direction centered on the long axis of the shaft 30, and having flexibility, and a holding device 60 in which a groove 61 capable of accommodating at least a part of the support device 20 is formed. As a result, the transport device 10 can accommodate the support device 20, in which the sheet-like indwelling object 110 is wrapped around the support 40, in the groove 61 of the holding device 60, and transport the sheet-like indwelling object 110 into the body via a cylindrical body (trocar 100) inserted into the body. The support 40 of the support device 20 can protrude from the groove 61 of the holding device 60 inside the body, and can easily release the wrapped sheet-like indwelling object 110 while spreading it at a target position by bending while contracting in diameter. Therefore, the delivery device 10 can easily deliver and leave the sheet-like indwelling object 110 at a desired position in the body in a minimally invasive manner.

[0036] The support 40 has a plurality of brush bristles 41 protruding radially outward from the shaft 30, and the brush bristles 41 are inclined toward the tip side. This allows the support 40 to be contracted in the radial direction and to be flexible. The frictional force between the brush bristles 41 and the sheet-like retaining object 110 increases when the brush bristles 41 move toward the tip end and decreases when the brush bristles 41 move toward the base end. Therefore, by moving the support device 20 toward the tip end, the sheet-like retaining object 110 supported by the brush bristles 41 can be transferred favorably toward the tip end, and by moving the support device 20 toward the base end, it is easy to move only the brush bristles 41 toward the base end while leaving the sheet-like retaining object 110 behind.

[0037] The support 40 may have a foam (for example, a sponge) protruding radially outward from the shaft 30. This allows the support 40 to be contractible along the radial direction and to be flexible.

[0038] The support device 20 has a flange 50 fixed to the shaft 30 on the base end side of the support 40 and protruding radially outward. With this, when the support device 20 is moved in the distal direction to transfer the sheet-like retaining object 110 supported by the support 40 in the distal direction, the flange 50 holds the base end of the sheet-like retaining object 110 and prevents the sheet-like retaining object 110 from coming off the support 40 and remaining unable to move in the distal direction.

[0039] The shaft 30 is curved at the position where the support 40 is fixed. As a result, the support 40 is straightened and stored in the holding device 60, and is released from the groove 61 of the holding device 60 to restore the original curved shape, and the wrapped sheet-like retaining object 110 can be spread and released. In addition, the curved support 40 can support the spread sheet-like retaining object 110 at multiple support points that spread two-dimensionally, so that the support 40 can be well supported to the target position.

[0040] The transfer method in this embodiment is a transfer method for transferring the sheet-like indwelling object 110 into the body, and includes a long shaft 30 and a flexible support 40 fixed to the tip of the shaft 30, capable of contracting in the radial direction centered on the long axis of the shaft 30, and the sheet-like indwelling object 110 is wound around the support 40 of the support device 20, and is held in the support device 20. The support device 20 and the support device 20 are inserted into the body through a cylindrical body (trocar 100) inserted into the body, with the support 40 protruding from the support device 60 inside the body, and the sheet-like indwelling object 110 wound around the support 40 is placed in a target position while being expanded. In this way, the transfer method can transfer the sheet-like indwelling object 110 into the body through the cylindrical body inserted into the body. Therefore, this delivery method allows the sheet-like indwelling object 110 to be delivered to and placed at a desired position in the body easily and minimally invasively.

[0041] The present invention is not limited to the above-described embodiment, and various modifications can be made by those skilled in the art within the technical concept of the present invention. For example, as in a first modified example shown in Fig. 8, the holding device 60 may be tubular (cylindrical), and a through hole 62 extending in the long axis direction X may be formed instead of the groove 61 to hold the support device 20. In this case, the support device 20 is inserted into the through hole 62 from the opening on the base end side of the through hole 62 of the holding device 60.

[0042] 9, the flange 50 may have a cylindrical cover 51 that covers the base end of the support 40. The cover 51 can cover and protect the radial outside of the sheet-like retaining object 110 supported around the support 40. The flange 50 having the cover 51 is, for example, cup-shaped with a recess formed on the tip side. The cover 51 can prevent the sheet-like retaining object 110 from falling off the support 40 and damage or deformation of the sheet-like retaining object 110 due to friction within the trocar 100.

[0043] Also, as in the third modified example shown in FIG. 10, the support device 20 may have a plurality of shafts 30 and supports 40. The support device 20 has a connecting portion 34 to which the plurality of shafts 30 are connected. This allows the surgeon to integrally operate the plurality of shafts 30 and supports 40. The sheet-like indwelling object 110 may be wound around the plurality of supports 40 so as to surround the plurality of supports 40 collectively. Also, a part of the sheet-like indwelling object 110 may be sandwiched and held between the adjacent supports 40. This strengthens the force with which the support device 20 holds the sheet-like indwelling object 110, and prevents the sheet-like indwelling object 110 from falling off. [Explanation of symbols]
