Abstract
Description
[Technical field]
[0001] (CROSS REFERENCE TO RELATED APPLICATIONS) This application claims the benefit of priority under 35 U.S.C. § 119 to U.S. Provisional Application No. 62 / 812,247, filed February 28, 2019, entitled “IMPLANTS, SYSTEMS, AND METHODS OF USE,” the disclosure of which is incorporated herein by reference in its entirety.
[0002] The present invention relates generally to general surgical, podiatric, and orthopedic implants used to correct bone deformities. More specifically, but not by way of limitation, the present invention relates to implants, systems, and methods for correcting bone deformities. [Background technology]
[0003] Fusion of the tibio-talo-calcaneus (TTC) complex is generally required due to pathology, trauma, or failure of previous surgery. Currently available options for fusing the three bones include intramedullary nails (IM), crossing screws, or plating. Currently available implants cross two joints, the tibiotalar or ankle joint, and the talocalcaneal or subtalar joint. During the use of currently available implants, there is a period of resorption that develops a gap between the articular surfaces. This gap can lead to failure of the fusion (nonunion) and ultimately failure of the device. Therefore, new implants, systems, and methods of use are required to avoid or minimize gaps and ensure that bone opposition is maintained during the healing process. Summary of the Invention

[0010] Generally, disclosed herein are implants and systems for correcting bone deformities. Additionally, methods for correcting bone deformities using the implants and systems are described.
[0011] In this detailed description and in the claims that follow, the terms proximal, distal, anterior or plantar, posterior or dorsal, medial, lateral, superior, and inferior are defined by standard usage to designate specific portions or regions of a bone or implant according to the relative location of natural bone, or by directional terms. For example, "proximal" means the portion of the device or implant closest to the body, while "distal" refers to the portion of the device or implant furthest from the body. As directional terms, "anterior" means toward the front of the body, "posterior" means toward the back of the body, "medial" means toward the midline of the body, "lateral" means laterally or away from the midline of the body, and "superior" means above another object or structure and "lower" means below another object or structure. Additionally, with particular reference to the foot, the term "dorsal" refers to the top of the foot and the term "plantar" refers to the bottom of the foot.
[0012] Similarly, a location or orientation may be used with respect to an anatomical structure or surface. For example, since the present devices, systems, instruments, and methods are described herein with respect to use on the ankle bone, the bones of the foot, ankle, and lower leg may be used to describe the surface, location, direction, or orientation of the devices, systems, instruments, and methods. Furthermore, the devices, systems, instruments, and methods, and aspects, components, features, etc., disclosed herein are described with respect to one side of the body for brevity. However, since the human body is relatively symmetrical or mirrored with respect to an axis of symmetry (the midline), it is expressly contemplated that the devices, systems, instruments, and methods, and aspects, components, features, etc., described and / or illustrated herein may be changed, altered, modified, reconfigured, or otherwise modified for use on or in association with another side of the body for the same or similar purposes without departing from the spirit and scope of the present disclosure. For example, the devices, systems, instruments, and methods, and aspects, components, features, etc., described herein with respect to the right leg may be mirrored to function on the left leg as well. Additionally, although the devices, systems, instruments, and methods, and aspects, components, features, etc., disclosed herein are described with respect to the leg for brevity, it should be understood that the devices, systems, instruments, and methods may also be used with other bones of the body having a similar structure.
[0013] 1-14, an implant 100 is illustrated. The implant 100 is an IM nail or cross screw. [0015] As shown in FIGS. 1-8 and 11-14, a deformable member, spring, or proximal external spring member 120 is coupled to a first end of the body portion 112. When inserted into a patient, the proximal external spring 120 resides anatomically on the medial side of the tibia. The spring 120 may be inserted, for example, in a first compressed position. Once implantation is complete, the spring 120 may move to at least one second partially compressed position. As shown, the deformable member 120 is a helically cut and machined spring. The helical spring 120 may be cut in one piece or cut integrally with the outer sheath 110 of the nail 100. The spring 120 may be compressed to provide the stored energy necessary to apply a compressive force to the tibio-talar joint by "pulling" the talus toward the tibia after implantation.
[0016] The distal end portion 140 of the first member 110 includes a shaft or body 142 coupled to the second end of the body portion 112. [0030] The distal end portion 440 of the first member 410 includes a shaft or body 442 coupled to the second end of the body portion 412. The distal end portion 440 may also include a distal or second through hole or fastener hole 444 disposed near the second end 404 of the implant 400. The second through hole 444 may be, for example, an elongated hole or an oval hole. The through hole 444 may extend through the body 442 perpendicular to the longitudinal axis of the first member 410 from one side to the other. As shown in FIG. 44, the distal end portion 440 may also include a threaded portion 446 disposed on an inner surface of the through hole 422 of the first member 410. The distal end portion 440 may also include at least one protrusion or tooth 448 disposed on the second end of the first member 410.
[0031] 33, 34, 43, and 44, the second member or inner rod 450 comprises a body portion or shaft 542 having an inner rod clip 460 coupled to and extending away from a first end of the second member 450 and a distal end portion 470 coupled to and extending away from a second end of the second member 450. The second member 450 may also comprise a first or proximal through hole or fastener hole 454 for receiving a bone screw or bone fastener. The through hole 454 may be located, for example, near the inner rod clip 460. The through hole 454 may be located to align with the through hole 414 of the first member 410 when assembled. The second member 450 may further comprise at least one second through hole or fastener hole 456 located near the distal end portion 470. The at least one second through hole 456 may be, for example, two through holes 456 as shown in the illustrated embodiment. The second through hole 456 may be positioned to align with the second through hole 416 of the first member 410 when assembled. The second member 450 may also include at least one anti-rotation pin opening 458 configured or sized and shaped to receive the anti-rotation pin 406. The pin opening 458 may extend through the body 452 in a first direction, for example, while the holes 454, 456 may extend through the body 452 in a second direction, with the first direction being generally perpendicular to the second direction. The pin opening 458 may be positioned to align with the pin opening 418, for example, such that the pin 406 can be inserted through the pin opening 418 and through the pin opening 458 to secure the first member 410 to the second member 430.
[0032] As shown in FIGS. 33, 34, 43, and 44, the inner rod clip 460 includes a stop 462 directly coupled to the body portion 452 at a first end of the inner rod clip 460. The inner rod clip 460 has, for example, a first diameter. The stop 462 has, for example, a second diameter that is greater than the first diameter. The second diameter of the stop 462 may also be less than the diameter of the body 452, for example. The inner rod clip 460 may also include a coupling portion 464 disposed at a second end of the inner rod clip 460. The coupling portion 464 may have, for example, a third diameter that is greater than the first diameter of the inner rod clip 460. The coupling portion 464 may also include an anti-rotation pin opening 466 extending therethrough and configured or sized and shaped to receive the pin 406. The inner rod clip 460 may also include at least one protrusion 468 extending circumferentially away from the inner rod clip 460. As shown, the at least one protrusion 468 may be, for example, two protrusions spaced apart along the longitudinal axis of the inner rod clip 460. The at least one protrusion 468 may be, for example, configured or sized and shaped to have an outer diameter approximately the same as the inner diameter of the deformable member 420. When assembled, the base portion 462 is disposed within the through hole 422 and surrounded by the deformable member 420, as shown in Figures 33 and 34. In an alternative embodiment, the inner rod clip 460 may include slots (not shown) machined therein to allow for flexible teeth (not shown). When the teeth of the inner rod clip 460 engage the outer sheath 410, the teeth help to prevent disengagement. The teeth also transfer and hold the force of the outer spring 420.
[0033] The distal end portion 470 includes a shaft or body 472 extending away from the body portion 52 at an opposite end of the inner rod clip 460. The distal end portion 470 also includes at least one second or distal through hole or fastener hole 474 for receiving a bone screw or bone fastener. As shown, the at least one second through hole 474 may be two second through holes 474. The distal end portion 470 further includes a coupling portion 476. The coupling portion 476 includes an opening 478 extending from the second end to the distal end portion 470. The opening 478 may include internal threads along at least a portion of the opening 478.
[0034] 33, 34, 43, and 44, the fastening screw 490 may include a body portion 492 having a circumferential flange or protrusion 494 disposed near a second end of the body portion 492. The fastening screw 490 may also include a coupling protrusion 496 coupled to and / or extending away from the second end of the body portion 492. The coupling protrusion 496 may also extend away from the flange 494 to the second end of the fastening screw 490. The coupling protrusion 496 may include, for example, a male threaded portion 498 for engaging the female threads of the through hole 478 to tighten the implant 400. The fastening screw 490 may also include a recessed region 500 extending to the first end of the body portion 492. The recessed region 500 may include a drive opening or tool engagement opening 502 for accepting, for example, an insertion, removal, or adjustment tool. The tool engagement opening 502 may accept a tool for removing preload compression from the implant 400. The fastening screw 490 holds the distal external spring 420 in a compressed state. Thus, the implant 400 can be prepared for insertion into a patient in a "preload" position that includes compression of the deformable body 420 and placement of the anti-rotation pins 406 at the bottom of their respective slots. More specifically, the fastening screw 490 can be releasably coupled to the implant 400 such that, for example, manipulation and / or release of the fastening screw 490 actuates the proximal external spring 420 from a preload (e.g., compressed) position to a loaded (e.g., at least partially compressed) position.
[0035] The IM Nail 100, 300, 400 provides continuous compression across both the tibio-calcaneus or ankle joint and the talo-calcaneus or subtalar joint, for example. The IM Nail 100, 300, 400 provides continuous compression across the joints, allowing bone alignment to be maintained throughout the healing process. In one embodiment, the IM Nail 300 allows for different compression loads at each joint. The internal spring 350 and external spring 120 compress the two joints independently, allowing optimal compression at each joint to facilitate bone healing and prevent gaps in the joint. The IM Nail 300 provides continuous compression between the talus and tibia, and between the talus and calcaneus at the tibio-talo-calcaneus joint of the ankle. Compression is dynamic, allowing for continuous compression of the articular surfaces in the event of bone resorption or remodeling.
[0036] 29 and 30 are methods of use of the implant 100, 300, 400. The method may be performed to assist in TTC fusion of the ankle complex. The method may include obtaining a pre-compressed implant 100, 300, 400. The implant 100, 300, 400 may be inserted in a preloaded compressed position. For example, the method may include inserting the implant 100, 300, 400 into the patient with the spring 120, 420 in a compressed position. The method further includes inserting a cross screw or fastener 308 through the patient's bone and the implant 100, 300, 400. The implant 100, 300, 400 may remain in the compressed position while the fastener 308 is inserted. After the fastener 308 is inserted, the disposable set screw is released and removed to effect compression, and the implant 100, 300, 400 moves to a second loaded position where it may be, for example, at least partially compressed. For example, the fastening screw 190, 490 can be releasably coupled to the implant 100, 300, 400 to actuate the proximal external spring 120 from a preloaded (e.g., compressed) position to a loaded (e.g., at least partially compressed) position, e.g., by manipulation and / or release of the fastening screw 190, 490. Finally, the fastening screw 190 can be releasably coupled to the implant 100 to actuate the proximal external spring 120 from a preloaded (e.g., compressed) position to a loaded (e.g., at least partially compressed) position, e.g., by manipulation and / or release of the fastening screw 190. Finally, the surgical incision is closed.
[0037] 45-46, a dynamic or curved nail system is illustrated. The nail system includes a curved nail or implant 550, a first fastener 570, and at least one second fastener. The curved nail 550 may have a first end 552 and a second end 554. The nail 550 may include a shaft or body portion 556 extending between the first end 552 and the second end 554. The curved nail 550 may also include at least one proximal or first through hole 558 disposed near the first end 552. Additionally, the curved nail 550 may include at least one distal or second through hole 560 disposed near the second end. The second through hole 560 may be, for example, an elongated or oval hole 560 extending obliquely through the nail 550. The first through hole 558 may extend through the nail 550 in a first direction and the second through hole 560 may extend through the nail 550 in a second direction. The first direction of the through hole 560 may be disposed, for example, circumferentially rotated or offset from the second direction of the second through hole 558.
[0038] 46-47, an implant 550 is shown inserted into a patient's ankle 590. The nail 550 may be inserted in a compressed state as shown in FIGS. 46 and 47. Fasteners 570, 580 may then be inserted through the nail 550 into the patient's lower leg 590. Specifically, a first fastener 570 may be inserted through the distal through-hole 560 and across the patient's joint. A second fastener 580 may be inserted through at least one proximal through-hole 558 and across the patient's joint.
