AI Technical Summary
Current treatments for neurodevelopmental disorders and developmental and epileptic encephalopathies are inadequate, particularly in pediatric subjects, as they often fail to address the underlying neuronal excitability issues caused by mutations in potassium channels like KCNQ2 and KCNQ3, leading to severe developmental and epileptic symptoms.
Administration of Kv7 channel activators, specifically compounds such as N-(1-cyclobutyl-4-fluoro-6-(1-hydroxycyclobutyl)-1H-benzo[d]imidazol-2-yl)-4,4,4-trifluoro-3,3-dimethylbutanamide, to pediatric subjects, including fetuses, infants, and children, to modulate KCNQ2 and KCNQ3 channels, thereby restoring cellular electrical balance and reducing hyperexcitability.
The Kv7 channel activators effectively treat a range of neurodevelopmental disorders and epileptic encephalopathies by normalizing neuronal excitability, alleviating symptoms, and improving developmental outcomes in pediatric subjects.

DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0014] Potassium (K +) channels are present on the plasma membrane of most cell types and are the most diverse class of all ion channels, being associated with a wide range of physiological functions including the control of the electrical properties of excitable cells. The primary pore-forming (α) subunits of these highly selective cation channels are divided into three main structural classes based on the number of transmembrane (TM) and pore (P) regions. Currently, there are 6TM / 1P, 2TM / 1P and 4TM / 2PK channels. + The Kv7 gene (originally called KCNQ and named by the HUGO Gene Nomenclature Committee (HGNC)) is a voltage-gated K + The Kv7 subfamily consists of five homologous pore-forming α-subunits, Kv7.1-7.5, which are voltage-gated K channels consisting of a 6TM-spanning region (S1-S6) between the intracellular N- and C-terminal domains, a typical voltage sensor domain in S4 with alternating positively charged residues, and a single P domain between S5 and S6 of each subunit. + It has a typical structure of a channel. The channel is formed as a tetramer of the primary α subunit, and can be formed as a homotetramer or heterotetramer. It is known that Kv7 channels consisting of Kv7.2-7.5 α subunits are expressed in neurons. Some of these gene products are present only in neurons, while others, such as Kv7.4 and Kv7.5, are present in other tissues, such as smooth and skeletal muscles.
[0015] The native M current was first characterized in amphibian sympathetic neurons. It is notable because it activates and deactivates slowly, is activated at or near the resting membrane potential of the neuron, and is reduced by muscarinic cholinergic agonists, and is associated with G protein-coupled receptors (GPCRs) and physiological K +A direct and inhibitory link between the currents was demonstrated. Not until the genes of this subfamily were cloned was the pharmacological and biophysical identity established between Kv7.2 / 7.3 (and presumably Kv7.3 / 7.5) heteromultimers and "M" currents, providing important new evidence for their importance in neuronal regulation.
[0016] The distribution of these channels, both regionally and developmentally, as well as their biophysical properties, support their role in providing durable resistance to depolarizing excitatory effects. Under physiological conditions, they are highly effective in controlling the subthreshold excitability of certain neuronal populations, as demonstrated by native M currents, and play a key role in controlling the frequency, and therefore the pattern, of action potential discharge in many types of neurons. Reducing the influence of Kv7.2 and Kv7.3 channels dramatically altered neuronal excitability, and the importance of Kv7 in neuronal control was highlighted by the discovery that neuronal mutations lead to benign familial neonatal convulsions (BFNC). Mutational analysis of Kv7.2 and Kv7.3 channels also revealed their involvement in BFNC, suggesting their usefulness as targets for antiepileptic drugs (AEDs).
[0017] Mutations in Kv7.2, and less well known mutations in Kv7.3, have been reported as causes of autosomal dominant benign neonatal seizures (BFNS), mainly due to haploinsufficiency. BFNS is an idiopathic epilepsy in which seizures occur within a few days of birth, and is known to be caused by mutations in both Kv7.2 and Kv7.3. Kv7.2 and Kv7.3 interact to give larger currents than the individual currents obtained as homomers. In situ hybridization showed that Kv7.2 is mainly present in the cerebellar cortex, neocortex, and hippocampal formation, including the dentate gyrus. These three structures have different susceptibility to epileptic seizures. Kv7.3 is also localized in the same area in the mouse brain. However, Kv7.2 expression appears earlier than Kv7.3 expression and increases rapidly within the first week of life. At birth, Kv7.2 is already expressed at significant levels, whereas Kv7.3 expression is negligible. Thus, it appears that Kv7.2 and Kv7.3 bind in distinct profiles during development.
[0018] BFNS usually has a good long-term prognosis, but recurrent seizures in adulthood have also been reported. Recently, de novo dominant-negative mutations in the KCNQ2 gene have been found to result in a rare pediatric epileptic encephalopathy, KCNQ2 epileptic encephalopathy (KCNQ2-EE), which accounts for 5–23% of early infantile epileptic encephalopathy (EIEE). KCNQ2-EE is a well-defined clinical disorder with neonatal-specific symptoms including hypotonia, treatment-resistant tonic seizures, interictal EEG abnormalities with prominent burst suppression, and moderate to severe global developmental delay, which has only recently been reported to result from a subset of missense variants in the KCNQ2 gene. Neuroimaging may reveal frontal lobe hypoplasia and transient MRI signal abnormalities in the basal ganglia and thalamus. Even if seizures are controlled by AED use in the first few months of life, all children suffer from adverse neurodevelopmental effects. It is proposed that the clinical features of KCNQ2-EE are associated with the functional impact of the mutations on M-currents and therefore can predict neurological prognosis. The impact of KCNQ2 mutations on Kv7.2 channel function is known to vary in relation to the Kv7.3 subunit, with significant disparities. Homomeric channel density may be the most reliable characteristic associated with Kv7.2 function and encephalopathy. It is hypothesized that Kv7.2 channel homomers play an essential role in fine-tuning neuronal connections during early brain development and correlate with the severity of the dominant-negative effect of encephalopathy-causing KCNQ2 mutations.

Otahara syndrome (OS) or early infantile epileptic encephalopathy (EIEE) is a rare type of epilepsy that usually becomes evident between 1 and 3 months of age. OS can be caused by brain malformations, metabolic abnormalities, or mutations in genes such as KCNQ2. Infants have primarily tonic seizures, but may also have partial seizures and, rarely, myoclonic seizures. Many with the disease have severely underdeveloped parts or all of the cerebral hemispheres. The electroencephalogram of infants with Otahara syndrome shows a characteristic pattern of high-voltage spike-wave discharges followed by little activity. This pattern is called "burst suppression." The course of Otahara syndrome is severely progressive. Seizures become more frequent and are associated with delayed physical and cognitive development. Some children die in infancy, while others survive with significant disabilities. As they grow older, some children transition to other epileptic disorders, such as West syndrome or Lennox-Gastaut syndrome. Due to the early onset of the disease, Kv7 channel activators are an effective treatment option for this disease. In an embodiment, the subject is a pediatric subject;

Several voltage-gated potassium channels, such as mutations or truncations in the KCNQ3 gene, have been implicated in autistic patients. Dysfunction of Kv7 may facilitate seizures and impair normal development, resulting in the autistic phenotype. Autism spectrum disorder (ASD) is a developmental disorder that affects communication and behavior. Autism can be diagnosed at any age, but symptoms generally appear within the first two years of life, hence the term "developmental disorder." According to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), a guide used by the American Psychiatric Association to diagnose mental disorders, individuals with ASD are characterized by: 1) difficulty communicating and interacting with others; 2) restricted interests and repetitive behaviors; and 3) symptoms that impair the ability to function properly in school, work, and other areas of life. Mutations in the SCN1A gene, which encodes a voltage-gated sodium channel, are common in Dravet syndrome. The mutations cause a reduction in sodium current, leading to hyperexcitation of GABAergic inhibitory neurons and resulting in epilepsy. Recently, it has been found that mutations in KCNQ2 contribute to the clinical presentation of Dravet syndrome. Dravet syndrome, formerly known as Severe Myoclonic Epilepsy of Infancy (SMEI), is a rare, treatment-resistant developmental epileptic encephalopathy with significant neurodevelopmental, motor, cognitive, and behavioral impacts that begin in infancy and continue into adulthood. Dravet syndrome is characterized by prolonged febrile and non-febrile seizures within the first year of life. The disease progresses to other seizure types, such as myoclonic and partial seizures, psychomotor delay, and ataxia. It is characterized by cognitive, behavioral, and motor disorders. Behavioral disorders often include hyperactivity, impulsivity, and in rare cases, autistic-like behavior. Seizures experienced by individuals with Dravet syndrome worsen as patients age. The compounds described herein are effective in Dravet syndrome by restoring the balance between excitation and inhibition in these neurons. In an embodiment, the subject is a pediatric subject;

Epilepsy of Infancy with Migratory Focal Seizures (EIMFS) is also called migratory partial epilepsy of infancy, migratory partial seizures of infancy, or malignant migratory partial seizures of infancy. EIMFS is a rare epilepsy syndrome in which epileptic seizures begin within the first few weeks of life. Seizures are focal but migratory to various parts of the brain. Autonomic nervous system changes (respiratory arrest, turning blue, sweating, hiccups) are frequently observed during seizures. EIMFS is associated with mutations in several genes, including the KCNT1 potassium channel, which accounts for 40% of EIMFS cases. The KCNT1 channel contributes to resting membrane potential and neuronal firing. The compounds described herein are effective in treating EIMFS. In an embodiment, the subject is a pediatric subject.

EXAMPLES
[0096] Example 1: Activation of KCNQ2 M-current has been described as a non-activated potassium current resulting from activation of Kv7 channels. M-current is generated by the association of two different Kv7 subunits to form heteromers. Kv7.3 can associate with either Kv7.2 or Kv7.5. Furthermore, Kv7 subunits can form functional homomers. The subunits combine into homomers and heteromers due to developmental and specific expression regulation. Importantly, heteromers have distinct permeability and conductance properties as well as larger currents compared to homomers. The data herein explain the preferential activation of Kv7.2 over Kv7.2 / 7.3 or Kv7.3 by compound A.
[0097] Biological assay methods - Plasmids containing wild-type (wt) KCNQ2 alone, wt KCNQ3 alone, or both wt KCNQ2 and wt KCNQ3 subunits were expressed in Chinese hamster ovary (CHO) cells using lipid-mediated transfection. A whole-cell voltage clamp protocol was used to measure gating and conductance density of wt channels before, during, and after test article administration by focal pipette perfusion. Half-activation voltage (V 1 / 2 ) was determined by Boltzmann fitting of the conductance-voltage (G / V) curves.
[0098] Figure 1 shows that compound A (a compound listed in Table 1) shifts the activation voltage dependence in cells. Compound A significantly negatively shifts the activation voltage dependence for KCNQ2 and KCNQ3 homomers and KCNQ2 / Q3 heteromers. The magnitude of the shift depends on the KCNQ subunit composition, KCNQ2>KCNQ2 / Q3>KCNQ3.
[0099] Figure 2 demonstrates that Compound A has the ability to activate Kv7.2 homomers. The current density from 3 μM Compound A is significantly increased from the control. The current density activation from 3 μM Compound A is similar to that produced by 10 μM ezogabine or ICA-069673. EXAMPLES
[0100] Example 2: Comparison of Compound A and Ezogabine in Homomeric and Heteromeric KCNQ2 Channels KCNQ2 de novo variants (DNVs) are heterozygous mutations that cause KCNQ2 epileptic encephalopathy (KCNQ2-EE). Many KCNQ2-EE variants are missense and cluster in hotspots, domains of key functions of the protein, such as voltage sensing, transmembrane ion flux, and sites controlling PIP2 and calmodulin binding. Many KCNQ2-EE DNVs have been shown to exert dominant-negative effects under voltage clamp when co-expressed with wild-type subunits in heterozygous mimics. Ezogabine (EZO) increases the current of channels containing mutant subunits in vitro and is used in patients with KCNQ2-EE. Potential precision medicines for KCNQ2-EE include novel small molecules that act as potent neuronal KCNQ channel openers.
[0101] Four highly repetitive KCNQ2-EE missense mutations (Figure 3), R210H (located at S4 of the voltage sensor), T274M (located at the pore helix), A294V (located at the boundary between the pore loop and S6), and R581Q / L (located at intracellular helix C) were introduced into KCNQ2 cDNA by QuikChange site-directed mutagenesis. R210H is located at S4 of the voltage sensor, T274M at the pore helix, A294V at the boundary between the pore loop and S6, and R581Q at intracellular helix C. Plasmids containing the pathogenic mutants were coexpressed with wild-type KCNQ2 or KCNQ2 and KCNQ3 subunits by lipid-mediated transfection into Chinese hamster ovary (CHO) cells. The S5 helix mutation W236L of KCNQ2, which abolishes the regulatory function of EZO, was also used to characterize the binding of compound A. Gating and conductance of wild-type and mutant subunit-containing channels were measured using a whole-cell voltage clamp protocol before, during, and after compound application by focal pipette perfusion.
[0102] Compound A increased currents and shifted the G / V curves of wild-type homomeric (Figures 4-6) and heteromeric (Figures 7-9) channels. (Figure 4) Compound A (0.3 μM and 1.0 μM) increased homomeric channel density at -40 mV (Figure 5), (Figure 6) Compound A left-shifted the homomeric KCNQ2 G / V curve (Figure 7) Representative currents recorded from cells expressing heteromeric wild-type KCNQ2 / KCNQ3 channels before and during application of 1.0 μM Compound A (Figure 8) and (Figure 9) Compound A left-shifted these heteromeric wild-type channels (N=3-10 cells; * <0.05, ** <0.01, and *** <0.001). EXAMPLES
[0103] Example 3: Comparison of Compound A with Ezogabine and ICA-069673 in Activating KCNQ2 Homomeric Proteins Activation of KCNQ2 homomers by compound A requires W236, similar to EZO and unlike ICA-069673. (Figure 10, Figure 13) Representative current traces recorded at -40 mV from cells expressing wild-type KCNQ2 (Figure 10) and KCNQ2 W236L (Figure 13) before and after drug application. EZO and compound A increased the current density of wild-type KCNQ2 channels (Figure 11) but had no effect on the current density of W236L channels (Figure 14). Hyperpolarized G / V curves after drug application (N=3-12 cells; * <0.05, ** <0.01, and *** <0.001). EXAMPLES
[0104] Example 4: Activation by Compound A of channels incorporating the recurrent KCNQ2 voltage sensor domain pathogenic mutation, R210H Compound A activates channels incorporating the recurrent KCNQ2 voltage sensor domain pathogenic mutation R210H. (Figure 18) Current family recorded from homomeric R210H channels before and after application of 1.0 μM Compound A. R210H activation is very strongly depolarizing, and this effect is reversed by Compound A (Figure 19) Mean currents at +20 mV before and during application of Compound A. (Figure 20) Comparison of the effect of Compound A on wild-type KCNQ2 and 1:1 co-expressed wild-type and R210H at -40 mV. (Figures 21, 22) Summary of V1 / 2 shifts of channels with different subunit combinations after application of 1.0 μM Compound A (N=6-19; * <0.05, ** <0.01, and *** <0.001). EXAMPLES
[0105] Example 5: Activation of homomeric channels incorporating human KCNQ2 mutants by Compound A Compound A activates homomeric channels incorporating human pathogenic KCNQ2 mutants (pore domain and C-terminus). (Figure 23) Overlaid representative traces recorded at -40 mV from cells expressing wild-type KCNQ2, and 1:1 KCNQ2 and KCNQ2 A294V before and during application of 0.3 μM Compound A are shown. (Figure 24) Effect of 0.3 μM Compound A on the current density of wild-type channels and of channels formed by 1:1 co-expression of wild-type with the indicated pore domain and C-terminus mutants (N=4-8; * <0.05, ** <0.01, and *** <0.001). EXAMPLES
[0106] Example 6: Activation by Compound A of heteromeric channels incorporating the recurrent pathogenic KCNQ2 mutations T274M and A294V Compound A activated heterochannels incorporating recurrent pathogenic KCNQ2 mutants T274M and A294V (wild-type KCNQ2:mutant:wild-type KCNQ3 cDNA ratio = 1:1:2 mimicking heterozygotes). Average current density, conductance / voltage curves, and V1 / 2 shifts at -40 mV for channels with A294V (Figures 25-27) and T274M (Figures 28-30) are shown before and during application of 1.0 μM Compound A (N = 9-11; * <0.05, ** <0.01 and *** <0.001).
[0107] Compound A activates KCNQ2-containing homomers and KCNQ2 / 3 heteromers, shifting activation and increasing maximal current. Such modulation of KCNQ2 requires pore domain residue W236. Mutants from three different KCNQ2-EE mutation hotspots showed strong dominant-negative current inhibition under conditions mimicking heterozygotes. Compound A (0.3-1 μM) increased currents in cells expressing these mutants, increasing the current at -40 mV to wild-type levels. These results suggest that compound A may be effective against KCNQ2-EE.
