Abstract
Description
[Technical field]
[0001] In general, the present disclosure relates to devices for disinfecting and sterilizing access ports, for example with male luer and female luer fittings, and in particular to disinfecting and sterilizing devices that can accommodate multiple types of connectors. [Background technology]
[0002] Vascular access devices (VADs) are commonly used therapeutic devices and include intravenous (IV) catheters. There are two general classifications of VADs: peripheral vascular catheters and central venous catheters. Bacteria and other microorganisms may gain entry to the VAD through access hubs and ports / valves into the patient's vascular system to deliver fluids or medications. Each access hub (port / valve or connection) is associated with the risk of contracting a catheter-related bloodstream infection (CRBSI), which can be costly and potentially fatal.
[0003] To reduce the number of catheter-related bloodstream infections (CRBSIs) and to ensure that VADs are used and maintained correctly, standard practices, including disinfection and cleaning methods, have been developed.
[0004] Antiseptic caps have been added to the Society for Healthcare Epidemiology of America (SHEA) guidelines, and early indications are that the caps will also be incorporated into the 2016 Infusion Nurse Standards (INS) guidelines.
[0005] In developed markets, when utilizing IV catheters, needleless connectors are typically used to shut off the system and are then subsequently accessed to administer medications or other necessary fluids to the patient via the catheter. The INS Standards of Practice recommend the use of needleless connectors and state that they should be "consistently and thoroughly disinfected using alcohol, tincture of iodine or a combination of chlorhexidine gluconate / alcohol prior to each access." Disinfection of needleless connectors is ultimately intended to help reduce bacteria that may live on the surface and result in various catheter-related complications, including the previously described CRBSI events. Nurses will typically complete this disinfection task by utilizing a 70% IPA alcohol pad to perform what is known as "scrubbing the hub." However, compliance with this practice is typically very low. In addition to the lack of compliance with "scrubbing the hub," it is also known throughout clinician interviews that there is often variability in scrubbing times, drying times, and the number of times needleless connectors are scrubbed.
[0006] There is a significant risk of exposure or contamination through a range of means associated with transmission of microorganisms that can result in CRBSI. Contamination can occur during drug mixing, during cannula connection, and during insertion into the axle hub. Because the means of connecting to a VAD are so common and simple, the risks associated with insertion into the patient's vascular system are often overlooked. Currently, the risk to hospitals and patients is substantially functioning of the physician's efforts to make the connections, and this effort is largely uncontrollable.
[0007] Currently, male needleless connectors, female needleless connectors, intravenous (IV), and hemodialysis lines use different designs, and therefore the types of connectors for attaching caps are limited. Therefore, there is a need for a disinfection device that can accommodate multiple types of connectors, simplifying the disinfection process. There is a need for a disinfection device that can perform multiple days of continuous disinfection. Summary of the Invention

The second absorbent material 232 is compressible, twisting the cap 202 onto the male luer, causing the male luer surface to come into contact with the absorbent material 232 and the sanitizing or antimicrobial agent. The second open end 228 creates a fluid seal with the male luer, as does the unopened second seal 250 of the cap 202. The device 200 can be left on the male luer as long as necessary to protect the male luer and keep it clean until the next use of the male luer.
[0043] 20A and 20B, to cap and / or sanitize the female luer 602, as on a stopcock, the first seal 250 is removed and the cap 202 is applied to the female luer via the first open end 216. The first absorbent material 230 exits into the female luer 602 to allow a disinfectant or antimicrobial agent to contact and disinfect the female luer tip 602. In one or more embodiments, the female luer 602 is not sealed by the device 200 because the fluid path is already stopped by the stopcock on the female luer 602.
[0044] With reference to FIG. 21, in an alternative embodiment, an elastomeric seal 240 can be placed on the inside of the cap 202 adjacent the first absorbent material 230 to seal the female luer 602 .
[0045] 22A and 22B, to cap and / or sterilize a needleless luer access device (LAD) 604, such as a Q-Syte or MaxPlus, the first seal 250 is removed and the cap 202 is placed on the LAD 604, similar to how a cap 202 is placed on a female luer. The first absorbent material 230 does not enter the LAD 604, as a female luer would, but instead the first absorbent material 230 pushes the device 200 towards the second seal 260. This provides space for the LAD 604, while at the same time allowing a disinfectant or antimicrobial agent to contact the top surface of the LAD 604, thereby disinfecting the LAD 604. The LAD is self-sealing and the device 200 does not provide a fluid seal.
[0046] In one or more embodiments, the porous and / or absorbent materials 230 and 232 may be solid, open cell foam, or polymeric sintered materials that can be molded into a defined shape, such as products manufactured by Porex Corporation (Fairburn, GA).
[0047] Currently, there are only disinfection caps on the market that can disinfect one of a variety of types, such as female or male luer connectors. Thus, to avoid the need to use different types of disinfection caps to clean different types of connectors, the two end caps of the present disclosure engage with male luer connectors and also engage with female luer connectors, thereby allowing a user to clean different types of connectors with a single device. Upon installation of the cap onto the female luer connector, the female luer connector is inserted into the first chamber and contacted with the absorbent material and the disinfectant or antibacterial agent. Upon installation of the cap onto the male luer connector, the male luer connector is inserted into the second chamber and contacted with the absorbent material and the disinfectant or antibacterial agent. Thus, the device of the present disclosure can be installed onto both male and female luer connectors, thus fulfilling a current need in the art.
[0048] 23-26, in one or more embodiments, the cap of the device of the present disclosure can form a fluid-tight seal with a female luer connector 300, a male luer connector 400, or a hemodialysis connector 500. With reference to FIGs. 23-26, in one or more embodiments, the cap of the device of the present disclosure is tapered to form a fluid-tight seal with a male luer connector. In certain embodiments, the cap meets ISO standards (e.g., ISO 594-1:1986 and 594-2:1998) for forming a seal with a male luer.
[0049] In one or more embodiments, the cap of the device of the present disclosure has threads having a size and pitch to engage with a threaded portion of a female connector, such as, for example, a female Luer connector. Such connectors are commonly and commonly used as catheters and other fluid-tight protective connectors in medical applications. In some embodiments, the cap provides a protective cover for the female Luer connector when engaged with the connector, as threads from the female Luer connector engage and form a peelable connection with the threads of the cap.
[0050] In some embodiments, the connector includes a needleless injection site, sometimes referred to as a needleless injection portion, hub, valve or device, or a needleless access portion, portion, hub, valve or device, and these may include brands such as, for example, Clave® (available from ICU Medical, Inc.), SmartSite® (available from Cardinal Health, Inc.), and Q-Syte™ (available from Becton, Dickinson & Co.). In some embodiments, the cap can be coupled to any of a variety of different needleless injection sites, as listed above. In one or more embodiments, after the cap is coupled to the connector, it is not necessary to disinfect the connector (e.g., by treating with an alcohol swab) before each recoupling of the connector to another connector, so that the connector remains uncontaminated while coupled to the cap. The use of the cap replaces the standard swabbing protocol for cleaning the connector.
[0051] In one or more embodiments, the threads of the cap are sized and pitched to mate with the threads of a male Luer lock connector. For example, the connector can comprise the end of an IV tubing set that is not connected to an IV catheter needleless infusion site.
[0052] A second embodiment of the present disclosure relates to a method of disinfecting a medical connector. In one or more embodiments, the method of disinfecting a medical connector includes removing the first seal 150, connecting a male luer connector 400 to the first open end 116, compressing the absorbent material 130 in the first chamber 114, contacting a disinfectant or antibacterial agent, and disinfecting the male luer connector 400. In an alternative embodiment, the method of disinfecting a medical connector includes removing the second seal 160, connecting a female luer connector 300 to the second open end 128, compressing the absorbent material 130 in the second chamber 126, contacting a disinfectant or antibacterial agent, and disinfecting the female luer connector 300. In another embodiment, a method of disinfecting a medical connector includes removing the first seal 150, connecting a female luer connector to the first open end 116, compressing the absorbent material 130 in the first chamber 114, contacting a disinfectant or antibacterial agent, and disinfecting the female luer connector, removing the second seal 160, connecting a male luer connector to the second open end 128, compressing the absorbent material 130 in the second chamber 126, contacting a disinfectant or antibacterial agent, and disinfecting the male luer connector.
[0053] A third embodiment of the present disclosure relates to an assembly comprising the device 100 of one or more embodiments coupled to a medical connector. In one or more embodiments, the medical connector is selected from a male luer connector, a female luer connector, a needleless connector, a stopcock, and a hemodialysis connector.
