Description

[Technical field]

[0001] The present invention relates to an injector for inserting an intraocular lens into the capsular bag of the eye. [Background technology]

[0002] In eye cataract treatment, typically only a small incision is made in the cornea of the eye, said incision being large enough to allow the tip of an injector to be inserted through the incision into the eye. Once the incision is made in the cornea, the eye's lens is crushed, for example by phacoemulsification, and then aspirated from the eye's lens capsule. An intraocular lens is then inserted into the eye. In this process, the intraocular lens is folded to fit into the tip of the injector. The tip is inserted into the lens capsule through the incision, and the folded intraocular lens is pushed by the piston of the injector through the tip into the lens capsule, where it unfolds and thus replaces the natural lens. The injector is conventionally inserted into the lens capsule manually, and the piston is often manually driven to move it. It is therefore desirable for the injector to be as easy to operate as possible.

[0003] US Patent No. 9700407B2 discloses an intraocular lens injector.EP Patent Application Publication No. 2340786A1 discloses a device for inserting an intraocular lens. Summary of the Invention [Problem to be solved by the invention]

[0004] SUMMARY OF THE PRESENT EMBODIMENT It is therefore an object of the present invention to provide an injector for inserting an intraocular lens into the capsular bag of the eye, which is easy to operate. [Means for solving the problem]

[0005] The injector according to the invention for inserting an intraocular lens into the lens capsule of the eye comprises an injector body having an injector tip with an opening and a proximal end facing away from the opening, a first piston directed towards the opening and designed to contact and displace the intraocular lens, and a second piston arranged in the region of the proximal end and designed to be able to be operated from outside the injector, the first piston and the second piston being in each case arranged so as to be longitudinally displaceable along an injector axis of the injector within the injector body, either in an uncoupled state in which the pistons can be longitudinally displaced relative to each other, or in a coupled state in which the pistons are rigidly coupled together in the direction of the injector axis and the total longitudinal extent of the pistons is greater than in the uncoupled state. Further, the injector according to the present invention has a handle disposed within the distal region of the injector body, which is designed to be operated from outside the injector and to be coupled to the first piston, whereby, when the pistons are in an uncoupled state, the first piston can be displaced towards the opening and relative to the second piston by moving the handle, whereby the pistons can be brought into a coupled state and by contact between the first piston and the intraocular lens, the intraocular lens can be moved towards the opening within the injector tip.

[0006] Once the first piston and the second piston are in a coupled state, the second piston is manipulated by displacing the second piston longitudinally toward the opening. Due to the rigid coupling of the two pistons in the coupled state, the first piston is displaced accordingly in unison, thereby moving the intraocular lens further toward the opening within the injector tip. When both pistons reach their end positions, the intraocular lens can exit the opening and be inserted into the capsular bag of the eye.

[0007] In that the movement of the intraocular lens is divided by two pistons, it is advantageously possible to embody the second piston shorter in terms of its longitudinal extent. The stroke of the second piston is therefore shorter than that of, for example, a conventional injector provided with only a single piston. The injector according to the invention can therefore have a shorter longitudinal extent, whereby the injector according to the invention is easier to operate than a conventional injector.

[0008] Both the handle and the second piston are preferably manually operated from outside the injector. The handle is located in a distal region of the injector body and the second piston is located in a region of the proximal end of the injector body, so that the handle and the second piston are operated from opposite sides. This reduces the possibility of accidentally confusing the handle and the second piston during operation, as would be the case if the handle and the second piston were operated from approximately the same side of the injector. This makes the injector safe and error-free.

[0009] The distal region of the injector body here can be formed by a first portion of the injector body, the first portion extending from the opening in the direction of the injector axis by up to 50% of the total longitudinal extent of the injector body, and the proximal region of the injector body can be formed by a second portion of the injector body, the second portion extending from the proximal end in the direction of the injector axis by up to 50% of the total longitudinal extent of the injector body.

[0010] The injector body has an injector body clearance, and the handle extends through the injector body clearance into the injector body and is preferably directly coupled to the first piston. It is particularly preferred that the handle is removably coupled to the first piston. This allows the handle to be removed from the rest of the injector before the tip of the injector is inserted into the capsular bag of the eye. The injector therefore advantageously has no components on the outside of the injector body that get in the way when the injector tip is inserted into the capsular bag. It is envisaged that the handle is removed when the first piston and the second piston are brought into a coupled state by the handle. It is particularly preferred that the first piston has a piston groove in which the handle engages.

[0011] Alternatively, the injector has a slide longitudinally displaceably arranged on the injector body and directly coupled to the first piston, and when the handle is moved away from the proximal end, the handle is preferably designated to entrain the slide such that the handle is indirectly coupled to the first piston by the slide. It is particularly preferred here that the slide has a lens holder designated to hold an intraocular lens, and when the handle is displaced away from the proximal end and before the handle entrains the slide, the handle is designated to displace the intraocular lens out of the lens holder and to fold said intraocular lens. As a result, the handle is not only designated to displace the first piston towards the opening, but also to fold the intraocular lens.

[0012] It is particularly preferred that the slide is releasably coupled to the first piston. This allows the slide, in particular together with the handle, to be removed from the rest of the injector before the tip of the injector is inserted into the capsular bag of the eye. As a result, the injector has no components outside the injector body that get in the way when the injector tip is inserted into the capsular bag. The slide and optionally the handle are intended to be removed when the handle brings the first piston and the second piston into a coupled state. The first piston preferably has a piston groove in which the slide engages.

[0013] The handle preferably has a cap which defines an internal space in which the injector body is arranged. The cap makes it particularly easy to grip the handle with one hand and to move the first piston in the direction of the opening. It is particularly preferred that the cap projects from the injector body in the direction of the injector axis beyond the opening. It is particularly preferred that the cap has a casing which defines the internal space in the radial direction relative to the injector axis and an end wall which is arranged next to the injector body in the direction of the injector axis and which defines the internal space in the direction of the injector axis. As a result, the injector body is advantageously protected in the distal region.

[0014] The second piston preferably has a piston opening on its end side facing the opening, by means of which the first piston extends into the interior of the second piston, and the first piston is mounted on the piston opening such that in the uncoupled state it is longitudinally displaceable relative to the second piston. Alternatively, it is also conceivable that the first piston has a piston opening on its end side facing away from the opening, and the second piston extends through the piston opening into the interior of the first piston, and the second piston is mounted on the piston opening such that in the uncoupled state it is longitudinally displaceable relative to the first piston. The piston is preferably designed to be telescopic. Furthermore, the piston is preferably arranged coaxially with respect to the injector axis.

[0015] The second piston is preferably designed to be displaced longitudinally in the direction of the opening by a pushing or screwing movement. Particularly preferably, the second piston is designed such that the pushing or screwing movement is performed manually, i.e. the injector does not have, for example, a motor.

[0016] The injector preferably includes an intraocular lens. The intraocular lens may be disposed within the injector body. If a lens holder is provided, the intraocular lens may alternatively be disposed within the lens holder.

[0017] The first piston preferably has a piston projection and the second piston preferably has a piston clearance, the piston projection being designed to be located outside the piston clearance when the first and second pistons are in an uncoupled state and designed to engage within the piston clearance to bring the first and second pistons into a coupled state when the first piston moves towards the opening, thereby providing a particularly simple structure that allows the first and second pistons to be brought into a coupled state.

[0018] In the following the invention will be explained in more detail with reference to the attached schematic drawings. [Brief description of the drawings]

[Figure 12] FIG. 3 shows a view of detail II of FIG. 2, in which the first piston and the second piston are in a coupled state. DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS

[0020] As can be seen from Figures 1 to 12, an injector 1 for inserting an intraocular lens 5 into the lens capsule of the eye comprises an injector tip 6 with an opening 7, an injector body 2 with a proximal end 23 facing away from the opening 7, a first piston 8 directed towards the opening 7 and designated for contacting and displacing the intraocular lens 5, and a second piston 9 arranged in the region of the proximal end 23 and designated for being able to be operated from outside the injector 1. The first piston 8 and the second piston 9 are in each case arranged in the injector body 2 so as to be longitudinally displaceable along an injector axis 27 of the injector 1, either in an uncoupled state in which the pistons 8, 9 can be longitudinally displaced relative to one another, or in a coupled state in which the pistons 8, 9 are rigidly coupled together in the direction of the injector axis 27 and the total longitudinal extent of the pistons 8, 9 is greater than in the uncoupled state. Further, the injector 1 has a handle 29 arranged in the distal region 21 of the injector body 2, which is specified to be operable from outside the injector 1 and to be coupled to the first piston 8, so that when the pistons 8, 9 are in an uncoupled state, the first piston 8 can be displaced towards the opening 7 and relative to the second piston 9 by moving the handle 29, so that the pistons 8, 9 can be brought into a coupled state, and by contact between the first piston 8 and the intraocular lens 5, the intraocular lens 5 can be moved towards the opening 7 within the injector tip 6.

[0021] The distal region 21 of the injector body 2 herein may be formed by a first portion of the injector body 2, the first portion extending from the opening 7 in the direction of the injector axis 27 by up to 50% of the total longitudinal extent of the injector body 2. The proximal region 22 of the injector body 2 herein may be formed by a second portion of the injector body 2, the second portion extending from the proximal end 23 in the direction of the injector axis 27 by up to 50% of the total longitudinal extent of the injector body 2.

[0022] Figures 1-6 and 8-10 further show that the injector 1 may have an injector handle 17 mounted on the injector body 2, protruding outwardly from the injector body 2, and positioned in the direction of the injector axis 27 between the proximal end 23 and the handle 29.

[0023] 1, 2, 4 and 6 to 9 show that the handle 29 may have a cap 11 that defines an internal space in which the injector body 2 is arranged. The cap 11 may protrude from the injector body 2 beyond the opening 7 in the direction of the injector axis 27. The cap 11 may have a casing 31 that defines the internal space in a radial direction 33 relative to the injector axis 27 and in particular completely in a circumferential direction 28, i.e. the casing 31 is embodied without interruption in the circumferential direction 28. The circumferential direction 28 and the radial direction 33 are shown by way of example in FIG. 1. Furthermore, the cap 11 may have an end wall 32 that is arranged next to the injector body 2 in the direction of the injector axis 27 and that defines the internal space in the direction of the injector axis 27. The cap 11 may only have a single cap opening arranged facing away from the end wall 32.

[0024] As can be seen from Figures 1 to 12, the second piston 9 can have a piston opening 26 on its end surface facing the opening 7, the first piston 8 extends into the interior of the second piston 9 through the piston opening 26, and the first piston 8 is mounted in an unbonded manner on the piston opening 26 so as to be longitudinally displaceable relative to the second piston 9.

[0025] In a first embodiment of the injector 1 according to the invention shown in Figures 1 to 3 and in a second embodiment of the injector 1 shown in Figures 4 and 5, the injector body 2 has an injector body clearance 10, and the handle 29 extends through the injector body clearance 10 into the interior of the injector body 2 and is directly coupled to the first piston 8. If the handle 29 has a cap 11 with a casing 31, the cap 11 may have a cap protrusion 16 (see Figures 1, 2 and 4), which is part of the handle 29 and protrudes radially inwards from the casing 31 and engages with the first piston 8. The handle 29 may be releasably coupled to the first piston 8. For this purpose, the first piston 8 may have a piston groove 12 in which the handle 29 engages.

[0026] In Fig. 1 and Fig. 4 the injector 1 is shown in the uncoupled state of the first piston 8 and the second piston 9 at a first time point for the first embodiment and at a first time point for the second embodiment. The first piston 8 and the second piston 9 can be brought into a coupled state at the point where the handle 29 is displaced away from the proximal end 23, for example by gripping the injector handle 17 with one hand and the handle 29 with the other hand and moving the hands away from each other. This is shown at a second time point for the first embodiment according to Fig. 2. In this case the intraocular lens 5 can also be displaced towards the opening 7. Figs. 1 to 5 show that the injector body clearance 10 can have a distal end formed by the injector body 2, against which the handle 29 can impinge when displaced away from the proximal end 23 (see Fig. 2). When the handle 29 is subsequently moved further away from the proximal end 23, the portion of the handle 29 disposed within the piston groove 12 can bend towards the proximal end 23 and thus be ejected from the piston groove 12. As a result, the handle 29 is no longer coupled to the first piston 8 and can therefore be removed from the injector body 2. This is shown at a third point in time in the first embodiment (see FIG. 3) and at a second point in time in the second embodiment (see FIG. 5).

[0027] At a third time point in the first embodiment (see FIG. 3 ) and at a second time point in the second embodiment (see FIG. 5 ), in the coupled state, the second piston 9 is displaced in the direction of the opening 7, so that the first piston 8 is also displaced in the direction of the opening 7. In this process, the first piston 8 displaces the intraocular lens 5 out of the injector through the opening 7. As shown as an example of the first embodiment, the second piston 9 can be specified to be displaced longitudinally in the direction towards the opening 7, in particular by a pushing movement, manually and without a motor. Alternatively, as shown as an example of the second embodiment, the second piston 9 can be specified to be displaced longitudinally in the direction towards the opening 7, in particular by a screw movement, manually and without a motor. For this purpose, the injector body 2 can have an internal thread 24 and the second piston 9 can have an external thread 25, with which the internal thread 24 engages.

[0028] 6 to 10 show that the injector 1 according to the third embodiment is arranged so as to be longitudinally displaceable on the injector body 2 and may have a slide 30 which is directly coupled to the first piston 8, and which is designated to entrain the slide 30 when the handle 29 is displaced away from the proximal end 23, such that the handle 29 is indirectly coupled to the first piston 8 via the slide 30. The slide 30 may have a lens holder 18 which is designated to hold an intraocular lens 5, and which is designated to displace the intraocular lens 5 out of the lens holder 18 and fold said intraocular lens 5 when the handle 29 is displaced away from the proximal end 23 and before the handle 29 entrains the slide 30. For this purpose, the handle 29 may have a folding element 20 which protrudes from the remaining handle 29 and is designated to protrude into the injector body 2 and to displace the intraocular lens 5 out of the lens holder 18 and fold said intraocular lens 5. The slide 30 and the folding element 20 are shown in a view from above in Figures 6 to 10, all other components of the injector 1 being shown in longitudinal section. In Figures 6 and 7 the injector 1 is shown at a first point in time when the intraocular lens 5 is placed in the lens holder 18 and the first piston 8 and the second piston 9 are in an uncoupled state. At a second point in time when the handle 29 is displaced away from the proximal end 23, for example here by gripping the injector handle 17 with one hand and the handle 29 with the other hand and moving the hands away from each other, the handle 29 is displaced until it hits the slide 30, as shown in Figure 8. As can be seen from Figure 8, the intraocular lens 5 is removed from the lens holder 18 and positioned below the folding element 20.

[0029] Now, when the handle 29 is displaced further away from the proximal end 23, the handle 29 will collide with the slide 30, thereby causing the handle 29 to entrain the slide 30. In this process, the first piston 8 is also displaced towards the opening 7, which causes the piston 8 to contact the intraocular lens 5 and displace the intraocular lens 5 towards the opening 7. Figures 6-10 show that the injector body clearance 10 may have a distal end formed by the injector body 2, and when the handle 29 is displaced away from the proximal end 23, the slide 30 may collide with the distal end (see Figure 9 showing the injector 1 at a third time point).

[0030] 5 to 9 show that the slide 30 can be releasably coupled to the first piston 8. For this purpose, the first piston 8 can have a piston groove 12 in which the slide 30 engages. In particular, the slide 30 can have a slide pin 19 which engages in the piston groove 12. When the handle 29 moves further away from the proximal end 23 after a third time point, the part of the slide 30 arranged on and in the piston groove 12 can be deformed in the direction of the proximal end 23 and thus ejected from the piston groove 12. As a result, the slide 30 is no longer coupled to the first piston 8 and can therefore be removed from the injector body 2 together with the handle 29. This is shown at a fourth time point (see FIG. 10).

[0031] At a fourth time point (see FIG. 10 ), in the coupled state, the second piston 9 is displaced in the direction of the opening 7, so that the first piston 8 is also displaced in the direction of the opening 7. In this process, the first piston 8 displaces the intraocular lens 5 out of the injector through the opening 7. The second piston 9 can be designated to be displaced longitudinally in the direction of the opening 7 by a pushing or screwing movement as also shown in the third embodiment.

[0032] 11 and 12 show that the first piston 8 may have a piston protrusion 13a and the second piston 9 may have a piston clearance 14a, where the piston protrusion 13a is designated to be located outside the piston clearance 14a when the first piston 8 and the second piston 9 are in a non-coupled state (see FIG. 11) and is designated to engage within the piston clearance 14a when the first piston 8 is displaced toward the opening 7 to bring the first piston 8 and the second piston 9 into a coupled state (see FIG. 12). The piston clearance 14a may be incorporated into the side of the second piston 9, radially inwardly 33. When the piston protrusion 13a is located outside the piston clearance 14a, the piston protrusion 13a abuts against that side of the second piston 9, radially inwardly 33, and may therefore be pushed inwardly in the radial direction 33 and therefore be under tension. When the first piston 8 is displaced towards the opening 7, the piston projection 13a enters the piston clearance 14a, so that the tension on the piston projection 13a is at least reduced. Figures 11 and 12 further show that the first piston 8 can have a further piston projection 13b and the second piston 9 can have a further piston clearance 14b, the further piston projection 13b being designated to be located outside the further piston clearance 14b when the first piston 8 and the second piston 9 are in a non-coupled state (see Figure 11) and to be engaged within the further piston clearance 14b when the first piston 8 is displaced towards the opening 7 in order to bring the first piston 8 and the second piston 9 into a coupled state (see Figure 12).

[0033] 1 to 10 show that the injector 1 may include an intraocular lens 5. The intraocular lens 5 may be placed in the injector body 2 and / or the lens holder 18 at each first time point.

[0034] As can be seen from Figures 1 to 10, the second piston 9 may have a piston head 15 at its distal end. The piston head 15 may be designed to impinge on the injector body 2 to limit the movement of the second piston towards the opening 7.

[0035] 6 to 10 show that the injector body 2 may have a cylinder 3 in which a second piston 9 is mounted so as to be longitudinally displaceable, and a cartridge 4 in which the first piston 9 is disposed for longitudinal displacement of the intraocular lens 5. The cylinder 3 and the cartridge 4 may be releasably coupled to each other. [Explanation of symbols]
