Abstract
Description
Use of AKT inhibitors in the preparation of drugs for preventing or treating ovarian cancer
[0001] This application claims priority to Chinese patent application No. 202310707215.6, filed on June 14, 2023, and Chinese patent application No. 202311223529.5, filed on September 21, 2023. This application cites the full text of the above-mentioned Chinese patent applications. Technical Field
[0002] The present invention belongs to the field of biomedicine. Specifically, the present application relates to the use of an AKT inhibitor in the preparation of a drug for preventing or treating ovarian cancer. Background Art
[0003] Cancer is second only to cardiovascular disease in terms of mortality, with both the number of cancer patients and deaths increasing globally. Ovarian cancer is a common malignancy in women, with the highest mortality rate among all cancers in women. Due to the lack of obvious symptoms in its early stages, the disease is insidious, making it prone to misdiagnosis. Most ovarian cancer patients have already developed metastases by the time they are diagnosed, making surgery alone ineffective in completely eliminating the tumor and leading to a high risk of recurrence. Over 70% of patients are diagnosed in the advanced stage, and approximately 70% experience recurrence within two years of surgery.
[0004] Drug therapy is an adjunct to surgical treatment of ovarian cancer, and commonly used therapeutic drugs are platinum compounds. Platinum compounds (such as cisplatin and carboplatin) can activate the DNA damage response, causing DNA damage, thereby inducing cell apoptosis and exerting significant anti-cancer activity. Although tumor cells are initially sensitive to platinum treatment, ovarian cancer patients are prone to drug resistance. Resistance to platinum may be attributed to upregulation of the ABCB1 gene, reversal of BRCA1 and BRCA2 germline alleles, or impaired apoptosis pathways. Therefore, there is an urgent need to develop a reliable drug to treat drug-resistant ovarian cancer.
[0005] Currently, the anti-ovarian cancer drugs under development are primarily targeted drugs. Targeted therapies specifically target altered genes or proteins in tumor cells. They offer advantages such as high specificity, significant efficacy, and minimal toxicity and side effects, making them a novel treatment option alongside the three traditional treatments of surgery, radiotherapy, and chemotherapy.
[0006] Poly(ADP-ribose) polymerase-1 (PARP-1) inhibitors are new molecular targeted anti-tumor drugs developed and marketed in recent years. They are clinically indicated for patients with advanced ovarian cancer who have or do not have mutations in the tumor suppressor gene BRCA. PARP-1 inhibitors work through a "synergistic lethal effect," meaning that in the presence of DNA homologous recombination defects caused by BRCA mutations, the simultaneous action of PARP-1 inhibitors and BRCA gene mutations can further aggravate DNA damage, preventing DNA repair in ovarian cancer cells with BRCA mutations and leading to their death. Currently, several PARP-1 inhibitors, including olaparib, rucaparib, niraparib, talazoparib, and fluzoparib, have been approved for marketing or are in clinical trials.
[0007] AKT is a type of serine / threonine kinase that affects cell survival, growth, metabolism, proliferation, migration and differentiation through numerous downstream effectors. More than 50% of human tumors have overactivation of AKT. Overactivation of AKT can lead to tumorigenesis, metastasis and drug resistance. AKT has three subtypes: AKT1, AKT2 and AKT3. As a typical protein kinase, each subtype consists of an amino-terminal PH domain (Pleckstrin homology domain), a central ATP-binding kinase domain and a carboxyl-terminal regulatory domain. The three subtypes share approximately 80% amino acid sequence homology, with only significant differences in the linker region between the PH domain and the kinase domain. Therefore, molecular targeted therapy targeting AKT is also an important direction in the current development of tumor targeted drugs.
[0008] WO2020156437A1 discloses a compound represented by formula (A), whose chemical name is (R)-4-((1S,6R)-5-((S)-2-(4-chlorophenyl)-3-(isopropylamino)propionyl)-2,5-diazabicyclo[4.1.0]hept-2-yl)-5-methyl-5,8-dihydropyrido[2,3-d]pyrimidin-7(6H)-one, and its chemical molecular formula is shown in the following formula (A):
[0009] The compound represented by formula (A) is an AKT protein kinase inhibitor that can effectively inhibit AKT kinase activity and is used to treat locally advanced or metastatic solid tumors. However, there are currently no reports of the compound represented by formula (A) being used in combination with other ovarian cancer therapeutics to prepare drugs for the prevention or treatment of ovarian cancer.

[0371] 2.4 Data Analysis
[0372] GraphPad Prism 5 software was used to fit the dose-effect curve, and the IC value of the compound on cell proliferation inhibition was obtained using the following nonlinear fitting formula. 50 (50% inhibitory concentration):
[0373] Y=minimum value+(maximum value-minimum value) / (1+10^((LogIC 50 -X)*slope))
[0374] X: log value of compound concentration, Y: inhibition rate (%inhibition)
[0375] Inhibition rate (% inhibition) = 100-(compound well reading-low reading control reading) / (high reading control reading-low reading control reading)*100
[0376] The Q value is the combination index, and the Q value is calculated using the following formula:
[0377] Q value = EA + B / (EA + EB - EA × EB), where EA + B is the inhibition rate of combined drug use, and EA and EB are the inhibition rates of drug A and drug B, respectively.
[0378] Evaluation criteria for the combination index: Q value > 1.15, the interaction between the two compounds is synergistic; 0.85 ≤ Q value ≤ 1.15, the interaction between the two compounds is additive; Q value < 0.85, the interaction between the two compounds is antagonistic.
