Description

OPHTHALMIC COMPOSITIONBACKGROUND OF THE INVENTION

[0001] 1. FIELD OF THE INVENTION

[0002] The present disclosure relates to an ophthalmic composition with a poorly water-soluble active pharmaceutical ingredient (API) . More particularly, the disclosed invention relates to an ophthalmic composition characterized by being a clear solution with a pH value of less than 5.5.

[0003] 2. DESCRIPTION OF RELATED ART

[0004] Axitinib is a tyrosine kinase inhibitor and an active pharmaceutical ingredient (API) in ophthalmological compositions for treating disorders such as age-related macular degeneration (AMD) , diabetic retinopathy, and retinal vein occlusion. Ideally, API should remain completely soluble and stable in ophthalmic compositions; further, the pH value of the ophthalmic composition should be at or near physiological pH to reduce the discomfort felt by the user. Nevertheless, the poor water solubility of axitinib poses a considerable challenge in its use in ophthalmologic applications (e.g., eye drops, injectable bolus, etc. ) , as the bioavailability and efficacy of axitinib are limited by its hydrophobicity.

[0005] In view of the foregoing, there exists in the related art a need of an improved ophthalmic composition, in which high concentration of a poorly-soluble API (e.g., axitinib) remain soluble in the improved ophthalmic composition.SUMMARY

[0026] 1. Definition

[0027] For convenience, certain terms employed in the specification, examples and appended claims are collected here. Unless defined otherwise, all technical and scientific terms used herein have the same meaning as commonly understood by one of the ordinary skilled in the art to which this invention belongs.

[0028] The singular forms “a” , “and” , and “the” are used herein to include plural referents unless the context clearly dictates otherwise.

[0029] Values expressed in a range format should be interpreted in a flexible manner to include only the numerical values explicitly recited as the limits of the range but also to include all the individual numerical values or sub-ranges encompassed within that ranges as if each numerical value and sub-range is explicitly recited. Thus, the term “about” as used herein allows for a degree of variability in a value or range, for example, within 10%, within 5%, within 1%, within 0.5%, within 0.1%, or within 0.01%of a stated value or of a stated limit of a range. Alternatively, the term “about” means within an acceptable standard error of the mean when considered by one of ordinary skill in the art. Accordingly, unless indicated to the contrary, the numerical parameters set forth in the present disclosure and attached claims are approximations that can vary as desired.

[0030] The terms “poor water-solubility” and “poorly soluble in water” are used interchangeably herein and independently refers to a pharmaceutical compound having a solubility of less than 1 mg / mL at a pH value between 6 to 8 at 20 ± 5℃. For example, axitinib, has a solubility of about 0.2 μg / mL in aqueous media over a pH range of 6.0 to 7.8, which is considered poorly soluble in water.

[0031] The term “clear solution” as used herein refers to a homogeneous mixture of a solute (for instance, axitinib) and a solvent with no visible particles or turbidity. The homogeneous mixture should appear clear and transparent without clouding, and light may pass through without scattering. In the clear solution, the solute molecules or ions are evenly dispersed and do not precipitate or separate from the solvent. According to the present disclosure, the present ophthalmic composition is a clear solution and does not comprise insoluble precipitates.

[0032] The term “pharmaceutically acceptable excipient” as used herein refers to an inert substance acting as a carrier for a therapeutic agent for the manufacture of a pharmaceutical dosage form. The pharmaceutically acceptable excipient is suitable for administration to a patient without causing adverse reactions or significantly interfering with the therapeutic activity  of the therapeutic agent carried thereby. According to embodiments of the present disclosure, examples of pharmaceutically acceptable excipients suitable for use in the present liquid formulation include preservatives, stabilizing agents, and thickening agents, but are not limited thereto.

[0033] 2. Description of the invention

[0034] The present disclosure is based, at least in part, on the discovery that the solubility of poorly water-soluble active pharmaceutical ingredients (APIs) (e.g., axitinib) can be enhanced via acid dissolution (e.g., in acidic conditions) . Thus, the present disclosure provides an ophthalmic composition of axitinib in the form of a clear solution with a pH value less than 5.5 (e.g., 3 to 5.5) . Compared to ophthalmic compositions comprising axitinib produced by known procedures, the present ophthalmic composition possesses a higher concentration and stability of APIs.

[0058] 2.2 Preparation of the present ophthalmic composition

[0059] The present ophthalmic composition is produced by acidic solution, in which a weak acid solution is employed as a preliminary solvent to increase the solubility of poorly soluble API, i.e., axitinib. Note that no organic solvent is used in the present procedures and the resulted final product. Typically, the present ophthalmic composition is produced by following procedures described below. First, cyclodextrin, preferably β-cyclodextrin, is dissolved in an acidic solution with a pH value ranging from 0 to 2 to form an acidified cyclodextrin solution. Next, axitinib, which has a water-solubility of about 0.2 μg / mL, is added to the acidified cyclodextrin solution and thoroughly mixed until a clear solution is formed. One or more of a pharmaceutically acceptable excipient such as a thickening agent, a stabilizer, and a preservative is / are then optionally added to the clear solution, followed by adjusting the pH of the clear solution to about 3.0 to 5.5, thereby producing the ophthalmic composition of the present disclosure. Note that in the thus-produced ophthalmic composition, axitinib is completely dissolved, in other words, the present ophthalmic composition does not comprise any insoluble precipitates.

[0060] It should be noted that the aforementioned processes can be combined according to practical conditions or needs when necessary. For example, axitinib and β-cyclodextrin can be added and mixed in the acidic solution simultaneously and mixed until the solution becomes clear.

[0061] According to embodiments of the present disclosure, the acidified β-cyclodextrin solution may be prepared by adding an acid into a β-cyclodextrin solution until a desired pH value (e.g., 0 to 2) is reached. Examples of the acid suitable for use in the present disclosure include, but are not limited to, hydrochloric acid (HCl) , sulfuric acid (H2SO4) , nitric acid (HNO3) , phosphoric acid (H3PO4) , citric acid (C6H8O7) , and oxalic acid (H2C2O4) solutions. According to some embodiments of the present disclosure, the acid is added to the β-cyclodextrin solution utile the solution has an acid concentration above 0.01 M, preferably about 0.02 to 1 M. In one working example, 0.1 M of HCl is added to the β-cyclodextrin solution until the solution has a pH value of 1.0 (i.e., the solution has a concentration of 0.1 M) .

[0062] Examples of the β-cyclodextrin suitable for use in the present method include, but are not limited to hydroxypropyl-β-cyclodextrin (HPβCD) , methyl-β-cyclodextrin (MβCD) , sulfobutylether-β-cyclodextrin (SBE-β-CD) , randomly methylated β-cyclodextrin (RM-β-CD) ,  carboxymethyl-β-cyclodextrin, and a combination thereof. In one working example, an HPβCD solution is adjusted to a pH value of about 1.0 with 0.1M HCl, then, axitinib is added to the acidified HPβCD solution, and the mixture is thoroughly mixed to give a clear solution.

[0063] According to embodiments of the present disclosure, the pH value of the clear solution is re-adjusted to about 3 to 5.5, such as 3.0, 3.1, 3.2, 3.3, 3.4, 3.5, 3.6, 3.7, 3.8, 3.9, 4.0, 4.1, 4.2, 4.3, 4.4, 4.5, 4.6, 4.7, 4.8, 4.9, 5.0, 5.1, 5.2, 5.3, 5.4, and 5.5, thereby forming the ophthalmic composition of axitinib. Since the clear solution is derived from the acidified cyclodextrin solution, an alkali solution is added to readjust the pH of the clear solution. Examples of the alkali solution suitable for use in the present procedure include, but are not limited to, sodium hydroxide (NaOH) , potassium hydroxide (KOH) , ammonium hydroxide (NH4OH) solutions, and a combination thereof. In one working example, the alkali solution used in the present method is NaOH.

[0064] As described section 2.1, in the preferred embodiments of the present disclosure, the thickening agent, the stabilizer, and the preservative are individually and optionally added to the clear solution. According to embodiments of the present disclosure, the clear solution further includes the thickening agent, the stabilizer, and the preservative. According to alternative embodiments of the present disclosure, the clear solution further includes the thickening agent and the stabilizer, but not the preservative.

[0065] Examples of the thickening agent suitable for use in the present disclosure include, but are not limited to, hydroxypropyl methylcellulose (HPMC) , sodium carboxymethylcellulose, carbomer, polycarbophil, polyethylene glycol (PEG) , hyaluronic acid (HA) , and a combination thereof. Examples of the stabilizer suitable for use in the present disclosure include, but are not limited to, purine, caffeine, theobromine, isoguanine, xanthine, hypoxanthine, uric acid, and a combination thereof. Examples of the preservative suitable for use in the present disclosure include but are not limited to, benzalkonium chloride (BAK) , alkyl parabens, and chlorobutanol. According to working examples, the thickening agent is HPMC, the stabilizer is caffeine, and the preservative, if needed, is BAK.

[0066] The amount of β-cyclodextrin and axitinib added to the acidic solution, as well as the ratio between the two, can be adjusted according to the need, where the concentration of β-cyclodextrin is about 10%to 60% (w / v) and the concentration of axitinib is 0.1%to 1.2% (w / v) , provided that the pH value of the final product is maintained at 3.0–5.5 without any insoluble precipitates. According to embodiments of the present disclosure, the ratio between β-cyclodextrin and axitinib ranges from 25: 1 to 100: 1, such as 25: 1, 31.25: 1, 33.3: 1, 37.5: 1, 50: 1 or 100: 1.

[0067] Optionally or in addition, the present ophthalmic composition of axitinib can be further subjected to a sterilization process well-known in the art, which includes but is not limited to, autoclaving, dry-heat, filtration, ethylene oxide (ETO) , radiation, and chemical sterilization. Appropriate sterilization can be chosen based on practical needs and formulation types. In some working examples, filtration sterilization is applied to the present clear solution by passing the present ophthalmic composition sequentially through filters having a pore size of 0.2 μm.

[0068] By the virtue of the above features set forth in sections 2.1 and 2.2, the present disclosure provides an improved ophthalmic composition and its preparation method for enhancing the solubility of poorly water-soluble axitinib, thereby increasing its stability in the aqueous ophthalmic composition.

[0069] EXAMPLE

[0070] Example 1 Preparation and characterization of the present ophthalmic composition of axitinib

[0071] 1.1 Preparation of the present ophthalmic composition

[0072] In this example, various formulations (Formulations 1 to 11) as listed in Table 1 were prepared by following procedures set forth below.

[0073] To produce an ophthalmic composition with a batch size of 5 mL, Hydroxypropyl-β-cyclodextrin (HPβCD) (0.5 g, 1 g, 1.5 g, 2 g, or 3 g ) and axitinib (5 mg, 10 mg, 20 mg, 30 mg, 40 mg, or 60 mg) were mixed in 3 mL of HCl solution (0.1 M, or 0.1 N) with continuous stirring (at a stirring speed of 200 rpm) until a clear transparent or light-yellow solution was formed. Caffeine (25 mg, 50mg, or 100 mg) , HPMC (12.5 mg or 25 mg) , and BAK (1 mg) were subsequently added to the clear or light-yellow solution and mixed until they were completely dissolved. The pH value of the clear solution was then re-adjusted to pH 3.5, 4.0, 5.0, or 5.5 by adding NaOH (1.0 N) . The solution was diluted to about 5 mL with distilled water and then filtered through 0.2 μm filters, to produce the liquid form of ophthalmic composition of axitinib.

[0074] 1.2 Characterization of the ophthalmic composition of Example 1

[0075] The stabilities of the ophthalmic compositions in Example 1 were investigated by storing them individually at 25℃ for over one month. Observations for precipitates were made regularly, and the results are also summarized in Table 1.

[0076] Table 1 Composition and stability of formulations 1 to 11

[0077] *This column is recorded as days in which a clear solution of the composition is sustained without precipitates.

[0078] The results provided in Table 1confirmed that the ophthalmic compositions of the present application were highly stable, no precipitates were observed after being stored for at least one month. Notably, formulations 2, 3, 5, 6, and 9 exhibited much better stability, no precipitates were observed after being stored for more than 100 days of storage.

[0079] The present ophthalmic compositions have osmolarities similar to that of human body fluids, such as tears, which can improve medication adherence.

[0080] Example 2 The stabilities of the present axitinib compositions

[0081] To investigate the stability of compositions prepared under different conditions, comparative compositions A to C were produced by following similar procedures described in Example 1 except the pH value and / or the concentration of HPβCD listed in Table 2 were used. Specifically, the pH values of comparative compositions A and B were independently greater than 5.5, and comparative composition C had a HPβCD concentration lower than 10% (w / v) . The stabilities of the comparative compositions A to C were investigated by storing them individually at 25℃. Observations for precipitates were made regularly, and the results are summarized in Table 2.

[0082] Table 2 Stability of comparative compositions A to C

[0083] *This column is recorded as days in which a clear solution of the composition is sustained without precipitates.

[0084] The results in Table 2 confirmed that precipitates appeared within one month in all three comparative compositions.

[0085] Specifically, the concentrations of axitinib and HPβCD in comparative composition A were same as those in formulations 4-6 of the present application. However, once the pH value of the composition exceeded 5.5 (i.e., pH 5.6) , precipitates were observed after being stored for two weeks, indicating a decrease in stability. Similarly, in comparative composition B, although the concentrations of axitinib and HPβCD were same as those in formulation 2, yet an increase in pH value (i.e., 5.9) resulted in decreased stability.

[0086] In addition, in comparative composition C, when the HPβCD concentration was lower than 10%, precipitates were observed after being stored for 20 days, indicating reduced stability.

[0087] It will be understood that the above description of embodiments is given by way of example only and that various modifications may be made by those with ordinary skill in the art. The above specification, examples, and data provide a complete description of the structure and use of exemplary embodiments of the invention. Although various embodiments of the invention have been described above with a certain degree of particularity, or with reference to one or more individual embodiments, those with ordinary skill in the art could make numerous alterations to the disclosed embodiments without departing from the spirit or scope of this invention.
