Description

[0001] Bispecific Antibodies for use in lowering the risk of cardiovascular disease events in subjects known to be a carrier of clonal expansion of hematopoietic cell lines with somatic mutations.

[0002] TECHNICAL FIELD

[0003] The invention relates to bispecific antibodies for the use in lowering the risk of cardiovascular disease events in subjects known to be a carrier of clonal expansion of hematopoietic cell lines with somatic mutations (CHIP).

[0004] BACKGROUND OF THE DISCLOSURE

[0005] Cardiovascular diseases (CVD) remain the leading cause of disease burden in the world. The estimated global prevalence of CVD doubled from 271 million in 1990 to 523 million in 2019 (Roth GA, Mensah GA, Johnson CO, et al (2020) Global Burden of Cardiovascular Diseases and Risk Factors, 1990-2019: Update from the GBD 2019 Study. J Am Coll Cardiol; 76(25):2982-3021). A substantial residual risk for CVD events related to chronic inflammation remains despite standard of care (SoC) management of classic risk factors (diabetes, hypertension, lipids, etc.)).

[0006] Atherosclerotic CVD is a condition commonly characterized by an elevated inflammatory state. Arterial inflammation and endothelial dysfunction play key roles at all stages of the atherothrombotic process. Inflammatory mediators are intimately implicated with the cascade of events leading to atherosclerotic plaque initiation, progression, and rupture. Vascular endothelial cells express a variety of adhesion molecules that recruit monocytes when chronically exposed to noxious stimuli or pathological conditions. Adverse conditions such as hyperlipidemia are associated with enrichment of a pro- inflammatory subset of monocytes. These monocytes apparently enter the intima under the influence of chemotactic stimuli and engulf modified low density lipoprotein (LDL) and cholesterol crystals (Duewell P, Kono H, Rayner KJ, et al (2010) NLRP3 inflammasomes are required for atherogenesis and activated by cholesterol crystals. Nature; 464(7293):1357-61). The material internalized by phagocytes induces phagolysosomal damage and subsequent leakage of contents into cytosol to activate inflammasomes and caspase 1 , and consequently the generation of interleukin-1 p (IL-1 ) from pro-interleukin- 1 P.

[0007] Interleukins are key mediators in the chronic vascular inflammatory response in CVD and have been demonstrated in animal models and in humans to be potent modulators of pro-inflammatory processes. The fact that these cytokines and their receptors are highly expressed and are functional in almost all cell types implicated in the pathogenesis of atherosclerosis including smooth muscle cells, certain subset of macrophages and T cells, as well as endothelium, support the role of interleukins in vascular disease. For example, I L-1 p is a potent smooth muscle cell mitogen, an activator of endothelial cells and increases extra cellular matrix and collagen deposition, which plays a role in plaque burden and arterial thickening. Furthermore, lack of I L-1 p or ablation of IL- 1 receptor has been shown to decrease severity of atherosclerosis in apoE deficient mice.

[0008] Clinical evidence from the CANTOS (Canakinumab Anti-Inflammatory Thrombosis Outcome Study) clinical trial demonstrated that I L-1 neutralization with canakinumab can reduce cardiovascular risk by approximately 15% in patients who have had a prior myocardial infarction (Ml) and elevated high-sensitivity C-reactive protein (hsCRP). Patients in the CANTOS trial that had lower on-treatment levels of IL-6 and hsCRP had even greater CVD benefit (approximately 25-35%). I L-1 p signaling promotes the release of IL-6 and hsCRP, and lower on-treatment levels may identify post-MI patients with the greatest potential CVD benefit. In post-hoc subgroup analyses in CANTOS, two populations demonstrated substantially increased residual risk of CVD that was not completely addressed by I L-1 p neutralization. First, patients with increased IL-18 levels at baseline in CANTOS had an increased risk of major adverse cardiovascular event (MACE) (15% increase in risk [95% confidence interval (Cl) 3-29%, p=0.02] for each increase in baseline IL-18), corroborating other preclinical findings implicating IL-18 in CVD pathogenesis (Ridker PM, MacFadyen JG, Thuren T, et al (2020) Residual inflammatory risk associated with interleukin-18 and interleukin-6 after successful interleukin-1 p inhibition with canakinumab: further rationale for the development of targeted anti-cytokine therapies for the treatment of atherothrombosis. Eur Heart J; 41 (23) :2153-63). Second, patients in CANTOS with evidence at baseline of CHIP were found to have higher risk of MACE during the trial follow-up as compared to patients without CHIP (Svensson EC, Madar A, Campbell CD, et al (2022) TET2-driven clonal hematopoiesis and response to canakinumab: an exploratory analysis of the CANTOS randomized clinical trial. JAMA Cardiol; 7(5):521-28).

[0009] CHIP refers to the presence of clonal populations of hematopoietic stem cells that occur in absence of diagnostic criteria for hematologic malignancy, in absence of morphological variation in blood cells, and with candidate driver gene mutations at variant allele frequency (VAF) of at least 2% in peripheral blood (Steensma DP, Bejar R, Jaiswal S et al (2015) Clonal hematopoiesis of indeterminate potential and its distinction from myelodysplastic syndromes. Blood; 126(1):9-16). CHIP is a disorder of aging with about 15% of people affected by 75 years of age. The risk of coronary heart disease for individuals with CHIP is approximately 2-times greater than in non-carriers matched for CVD factors including age, sex, type 2 diabetes and smoking history (Jaiswal S, Natarajan P, Silver A et al (2017) Clonal Hematopoiesis and Risk of Atherosclerotic Cardiovascular Disease. N Engl J Med; 377:111-121). In murine models, there is data suggesting a causative role of CHIP in development of both atherosclerosis and cardiac dysfunction (Fuster J J , Maclauchlan S, Zuriaga MA et al (2017) Clonal hematopoiesis associated with TET2 deficiency accelerates atherosclerosis development in mice. Science; 355:6327; Sano S, Oshima K, Wang Y et al (2018) CRISPR-Mediated Gene Editing to Assess the Roles of Tet2 and Dnmt3a in Clonal Hematopoiesis and Cardiovascular Disease. Circulation Research; 123:335-341). CHIP likely contributes to CVD risk through enhanced inflammation, including increased NOD-, LRR- and pyrin domain-containing protein 3 (NLRP3) activation. Analysis of the CANTOS data revealed that patients with CHIP, particularly CHIP due to somatic mutations in the TET2 gene, were found to have over 60% higher risk of MACE during the trial follow-up as compared to patients without CHIP (Svensson EC, Madar A, Campbell CD, et al (2022) TET2-driven clonal hematopoiesis and response to canakinumab: an exploratory analysis of the CANTOS randomized clinical trial. JAMA Cardiol; 7(5):521-28). Mutations in epigenetic regulators (DNMT3A and TET2) account for approximately 80% of CHIP and are linked to a pro- inflammatory state (Marnell C, Bick A, Natarajan P (2021) Clonal hematopoiesis of indeterminate potential (CHIP): Linking somatic mutations, hematopoiesis, chronic inflammation and cardiovascular disease. J Mol Cell Cardiol; 161 :98-105).

[0010] CVD is a heterogenous disorder, and the presence of CHIP may mark individuals with a greater inflammatory driven contribution to CVD pathogenesis and potentially greater response to targeted anti-inflammatory therapies. Therapeutic approaches that target additional inflammatory pathways and selecting populations with the potential for greater response to anti-inflammatory therapy (i.e., CHIP) may further attenuate the residual CVD risk seen in CANTOS.

[0011] Inhibition of I L-1 p and IL-18 has the potential to significantly reduce cardiovascular risk in patients.

[0037] DETAILED DESCRIPTION OF THE DISCLOSURE

[0038] Without wishing to be bound by theory, inflammation is thought to play an important role in CVDs and the occurrence of CVEs, particularly in subjects being carrier of clonal expansion of hematopoietic cell lines / clonal hematopoiesis of indeterminate potential (CHIP) with somatic mutations in either of the genes TET2, DNMT3A, ASXL1 , PPM1 D, TP53, SF3B1 , SRSF2 or JAK2 (CHIP population). The present disclosure is inter alia based on the unexpected finding that certain antibodies that simultaneously neutralize IL- 1 P and IL-18 more potently attenuate pro-inflammatory cytokine production compared to single IL-1 p or IL-18 neutralization alone, which is considered by the inventors to be an efficacious treatment for reducing the risk of or preventing cardiovascular events and recurring cardiovascular events in a CHIP population.

[0039] Clonal hematopoiesis of indeterminate potential (CHIP) is the results of clonally expanded hematopoietic stem cell caused by a mutation. CHIP populations have a twofold higher cardiovascular risk compared to non-CHIP individuals, because CHIP results in a pro-inflammatory state that seems to be linked to CVEs like thromboembolic events, myocardial infarction, and coronary artery disease (Marnell C, Bick A, Natarajan P (2021) Clonal hematopoiesis of indeterminate potential (CHIP): Linking somatic mutations, hematopoiesis, chronic inflammation and cardiovascular disease. J Mol Cell Cardiol; 161 :98-105. The most prominent mutations resulting in CHIP have been identified in DNA damage repair genes (e.g., PPM1 D, TP53), the tyrosine kinase JAK2, mRNA spliceosome components (e.g., SF3B1 , and SRSF2) and epigenetic regulators (e.g., DNMT3A, TET2, ASXL1).

[0040] As used herein, the umbrella terms cardiovascular disease (CVD) or cardiovascular event (CVE) will be used as they best reflect the underlying etiology of the disease and are used to describe coronary heart diseases, also simply known as heard disease, or known and called as myocardial ischemia, ischemic heart disease or coronary artery disease. All these indications involve a reduction of blood flow to the heart due to e.g., atherosclerotic plaques in the arteries of the heart. Such a cardiovascular event can be recurrent and one embodiment of the herein disclosed uses or methods relates to reducing the risk of recurrent cardiovascular events.

[0041] A cardiovascular event can be a Myocardial Infarction (Ml). Different types of Ml are known, like: i) spontaneous Ml related to ischemia due to a primary coronary event such as plaque erosion and / or rupture, fissuring or dissection, ii) Ml secondary to ischemia due to either increased oxygen demand or decreased supply, e.g., coronary artery spasm, anemia, hypotension, coronary embolism, arrhythmias, hypertension or hypotension, iii) Sudden unexpected cardiac death including cardiac arrest, often with symptoms suggestive of myocardial ischemia, iv) Ml associated with PCI (Percutaneous Coronary Intervention), v) Ml associated with stent thrombosis as documented by autopsy or angiography, vi) Ml associated with CABG (Coronary artery bypass grafting), vii) stable angina and unstable angina. In one embodiment, the CVEs or CVDs treated according to the methods and uses disclosed herein is a recurrent CVE or CVD

[0042] Herein are described methods or uses for reducing the risk of or preventing CVEs or CVDs, which can be a recurrent CVE or CVD, in a CHIP population by administering to a subject in need thereof an effective amount of a bispecific antibody targeting both I L-1 p and IL-18 simultaneously (e.g., bbmAb) or functional fragments thereof. Accordingly, in one aspect provided is a method for reducing the risk of or preventing CVEs or CVDs, which can be a recurrent CVE or CVD, in a CHIP population, comprising administering to a subject in need thereof an effective amount of a bispecific antibodies targeting both IL- 1 and IL-18 simultaneously (e.g., bbmAb).

[0043] In one embodiment, the disclosure relates to uses or methods of the bispecific antibody targeting both IL-1 and IL-18, e.g., bbmAb, for reducing the risk of or preventing CVEs or CVDs, wherein said events or diseases are selected from non-fatal myocardial infarction, non-fatal stroke, cardiovascular death, and hospitalization for unstable angina requiring unplanned revascularization.

[0044] In one embodiment, the disclosure relates to uses or methods of the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, for reducing the risk of or preventing a cardiovascular event, wherein said event is non-fatal myocardial infarction.

[0045] In one embodiment, the disclosure relates to uses or methods of the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, for reducing the risk of or preventing a cardiovascular event, wherein said event non-fatal stroke.

[0046] In one embodiment, the disclosure relates to uses or methods of the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, for reducing the risk of or preventing a cardiovascular event, wherein said event cardiovascular death.

[0047] In one embodiment, the disclosure relates to uses or methods of the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, for reducing the risk of or preventing a cardiovascular event, wherein said event is unstable angina requiring unplanned revascularization.

[0048] The disclosure relates to bispecific monoclonal antibodies or variants thereof, e.g., bispecific antibodies targeting both IL-i p and IL-18 simultaneously (e.g., bbmAb) for the use in reducing the risk of or preventing CVEs or CVDs, which can be a recurrent CVE or CVD, in disease patients having hyper elevated I L-1 p and / or I L-18. The disclosure also relates to methods, treatment regimens, uses, kits and therapies for reducing the risk of or preventing CVEs or CVDs, which can be a recurrent CVE or CVD, by employing bispecific antibodies targeting both IL-1 p and IL-18 simultaneously, e.g., bbmAb.

[0049] Moreover, the present inventors hypothesize that treatment with an antibody that simultaneously neutralizes IL-1 p and IL-18 (e.g., bbmAb), can allow for a significantly reduced dose frequency, with single administration compared to the more complex investigational combination requiring anti-IL-1 p (every two weeks for e.g., canakinumab or every day for anakinra) potentially with glucocorticoids, cyclosporin and IL-18BP (every two days).

[0050] In one aspect of the invention, the hsCRP levels of >2 mg / L assessed at least 28 days after Ml is reduced by 20% or 21% or 22% or 23% or 24% or 25% or 26% or 27% or 28% or 29% or 30% or more after a single subcutaneous (s.c.) administration of about 600mg bbmAb, measured at 3 weeks after the administration of the bbmAb.

[0051] Another biomarker that is useful in assessing residual inflammatory risk include downstream mediators of IL-1 such as interleukin-6 (IL-6). IL-6 is a known marker of cardiovascular disease associated with obesity, type 2 diabetes, and myocardial infarction. It is known that administration of an IL-1 antibody (e.g., canakinumab) to stabilize post-MI patients resulted in lowering of levels of IL-6, a marker for inflammation. Accordingly, in another embodiment of any use or method according to the invention, IL-6 is used as a biomarker for assessing the response of the stable Ml patient to administration of about 600mg bbmAb administered at the earliest 30 days after Ml.

[0078] 1. Definitions

[0079] For purposes of interpreting this specification, the following definitions will apply and whenever appropriate, terms used in the singular will also include the plural and vice versa. Additional definitions are set forth throughout the detailed description.

[0080] The term “about” in relation to a numerical value x means, for example, + / -10%. When used in front of a numerical range or list of numbers, the term “about” applies to each number in the series, e.g., the phrase “about 1-5” should be interpreted as “about 1 - about 5”, or, e.g., the phrase “about 1 , 2, 3, 4” should be interpreted as “about 1 , about

[0081] 2, about 3, about 4, etc.”

[0082] The term “approximately” when used in the context of a time period refers to a given time point (e.g. 90 days or 3 month) including a time period before or after that given time point. The term approximately 3 months includes a time period of 90 days + / - 10 days.

[0083] As used herein, the term approximately “6 months” refers to a time period of 180 days that includes + / - 15 days.

[0084] As used herein, the term approximately “9 months” includes a time period of 270 days that includes + / - 20 days.

[0085] The term "IL- 18" is synonym to IL- 18 polypeptide, Interleukin- 18 polypeptide, IFN- gamma inducing factor or Interferon-gamma-inducing-factor or INF-y inducing factor. The term "IL-18" refers to human IL-18, unless another species is indicated. IL-18 is well known to a person skilled in the art, and for example obtainable from MBL® International Corporation under product reference #B001-5. Throughout this specification, the term IL- 18 encompasses both pro-IL-18 (precursor of mature IL-18 prior protease cleavage) and mature IL-18 (post protease cleavage) interchangeably unless it is specified that the pro- or mature form is meant.

[0086] The term “IL-1p” or “IL-1 b” is synonym to IL-1 p polypeptide and lnterleukin-1 polypeptide. The term “IL-1 p” refers to human I L-1 p unless another species is indicated. IL-1 p is well known to a person skilled in the art, and commercially available (e.g., Sino Biological under product reference #10139-HNAE-5). Total IL-18 in serum can be measured by conjugating anti-human IL-18 antibody (e.g., clone 125-2H, MBL International) to Bio-plex Magnetic COOH beads (Bio-Rad, Inc.), detected using biotinylated anti-human IL-18 (clone 159-12B, MBL), and concentrations calculated using the IL-18 contained in the Group II cytokine standard curve (Bio-Rad, Inc.). Free IL-18 can be measured as described in Girard et al. Rheumatology (Oxford). 2016 Dec;55(12):2237- 2247. Serum IL-1 p can be measured using a commercially available ELISA kit (88-7261- 88, eBioscience) used in accordance with manufacturer’s instructions.

[0087] The term "antibody" refers to an intact immunoglobulin or a functional fragment thereof. Naturally occurring antibodies typically comprise a tetramer which is usually composed of at least two heavy (H) chains and at least two light (L) chains. Each heavy chain is comprised of a heavy chain variable region (abbreviated herein as VH) and a heavy chain constant region, usually comprised of three domains (CH1 , CH2 ad CH3). Heavy chains can be of any isotype, including IgG (lgG1 , lgG2, lgG3 and lgG4 subtypes), IgA (lgA1 and lgA2 subtypes), IgM and I g E. Each light chain is comprised of a light chain variable region (abbreviated herein as VL) and a light chain constant region (CL). Light chain includes kappa (K) chains and lambda (A) chains. The heavy and light chain variable region is typically responsible for antigen recognition, whilst the heavy and light chain constant region may mediate the binding of the immunoglobulin to host tissues or factors, including various cells of the immune system (e.g., effector cells) and the first component (Clq) of the classical complement system. The VH and VL regions can be further subdivided into regions of hypervariability, termed complementarity determining regions (CDR), interspersed with regions that are more conserved, termed framework regions (FR). Each VH and VL is composed of three CDRs and four FRs arranged from aminoterminus to carboxy-terminus in the following order: FR1 , CDR1 , FR2, CDR2, FR3, CDR3, FR4. The variable regions of the heavy and light chains contain a binding domain that interacts with an antigen.

[0088] Furthermore, although the two domains of the Fv fragment, VL and VH, are coded for by separate genes, they can be joined, using recombinant methods, by a flexible linker that enables them to be made as a single protein chain in which the VL and VH regions pair to form monovalent molecules (known as single chain Fv (scFv); see e.g., Bird et al., (1988) Science 242:423-426; and Huston et al., (1988) Proc Natl Acad Sc;. 85:5879-5883). Such single chain antibodies are also intended to be encompassed within the term "antigen-binding portion" of an antibody. These antibody fragments are obtained using conventional techniques known to those of skill in the art, and the fragments are screened for utility in the same manner as are intact antibodies.

[0089] Throughout this specification, complementarity determining regions (“CDR”) are defined according to the Kabat definition unless specified that the CDR are defined according to another definition. The precise amino acid sequence boundaries of a given CDR can be determined using any of a number of well-known schemes, including those described by Kabat et al. (1991), “Sequences of Proteins of Immunological Interest,” 5th Ed. Public Health Service, National Institutes of Health, Bethesda, MD (“Kabat” numbering scheme), Al-Lazikani et al., (1997) JMB 273, 927-948 (“Chothia” numbering scheme) and ImMunoGenTics (IMGT) numbering (Lefranc, M.-P., The Immunologist, 7, 132-136 (1999); Lefranc, M.-P. et al., Dev. Comp. Immunol., 27, 55-77 (2003) (“IMGT” numbering scheme). For example, for classic formats, under Kabat, the CDR amino acid residues in the heavy chain variable domain (VH) are numbered 31-35 (HCDR1), 50-65 (HCDR2), and 95-102 (HCDR3); and the CDR amino acid residues in the light chain variable domain (VL) are numbered 24-34 (LCDR1), 50-56 (LCDR2), and 89-97 (LCDR3). Under Chothia the CDR amino acids in the VH are numbered 26-32 (HCDR1), 52-56 (HCDR2), and 95-102 (HCDR3); and the amino acid residues in VL are numbered 26-32 (LCDR1), 50-52 (LCDR2), and 91-96 (LCDR3). By combining the CDR definitions of both Kabat and Chothia, the CDRs consist of amino acid residues 26-35 (HCDR1), 50-65 (HCDR2), and 95-102 (HCDR3) in human VH and amino acid residues 24-34 (LCDR1), 50-56 (LCDR2), and 89-97 (LCDR3) in human VL. Under IMGT the CDR amino acid residues in the VH are numbered approximately 26-35 (CDR1), 51-57 (CDR2) and 93-102 (CDR3), and the CDR amino acid residues in the VL are numbered approximately 27-32 (CDR1), 50-52 (CDR2), and 89-97 (CDR3) (numbering according to “Kabat”). Under IMGT, the CDR regions of an antibody can be determined using the program IMGT / DomainGap Align.

[0090] By convention, the CDR regions in the heavy chain are typically referred to as H- CDR1 , H-CDR2 and H-CDR3 and in the light chain as L-CDR1 , LCDR2 and L-CDR3. They are numbered sequentially in the direction from the amino terminus to the carboxy terminus.

[0091] As used herein, the term “antagonist” is intended to refer to a binding molecule that inhibits the signalling activity in the presence of activating compound. For example, in the case of IL-18, an IL-18 antagonist would be a binding molecule inhibiting the signalling activity in the presence of IL-18 in a human cell assay such as IL-18 dependent Interferongamma (IFN-y) production assay in human blood cells. Examples of an IL-18 dependent IFN-y production assay in human blood cells are described in more details in the examples below.

[0092] The term bispecific antibody or bispecific antibodies refer to antibodies binding to two different targets, such as IL-18 and IL-1 p. Typically, the bispecific antibody binds each target monovalently.

[0093] The bispecific antibodies are “hetero-dimers”, which means that one part comes from first antibody, specific for a first target, and another part comes from a second antibody, specific for a second target. A “hetero-dimerization modification” is a modification to one or both parts of the antibodies forming the hetero-dimeric bispecific antibody, intended to facilitate such formation. An example of hetero-dimerization modifications of the Fc domains of two I gG 1 parts of antibodies intended to form a bispecific is a “knob” with a bulky amino acid (aa) side chain (S354C, T366W) in the first heavy chain and a “hole” with small aa side chains (Y349C, T366S, L368A, Y407V) were introduced in the second heavy chain as well as an additional disulfide bridge in the CH3 region connecting both heavy chains (Merchant et al., Nat. Biotechnol., 16:677-681 (1998), page 678, table 1).

[0094] As used herein, the term "subject" includes any human subjects receiving the bispecific antibody as presently described. As used herein, the term “patient” and “subject” are used interchangeably. The term subject can additionally or alternatively include any human subject that is known to be a carrier of clonal expansion of hematopoietic cell lines with somatic mutations in either of the genes TET2, DNMT3A, ASXL1 , PPM1 D, TP53, SF3B1 , SRSF2 or JAK2. In one aspect a subject is a human being a carrier of clonal expansion of hematopoietic cell lines with somatic mutations in either of the genes TET2, DNMT3A, ASXL1 , PPM1 D, TP53, SF3B1 , SRSF2 or JAK2 wherein at least one unique mutation must be present at a variant allele frequency >2%. In another aspect a subject is a human being a carrier of clonal expansion of hematopoietic cell lines with somatic mutations in either of the genes TET2 or DNMT3A (TET2 or DNMT3A CHIP) wherein at least one unique mutation must be present at a variant allele frequency >2%. In another aspect of the disclosure a subject is a patient with coronary heart disease and TET2 or DNMT3A CHIP, wherein at least one unique mutation must be present at a variant allele frequency >2%.

[0095] As used herein, the terms “C-reactive protein” and “CRP” refers to serum C- reactive protein, which is used as an indicator of the acute phase response to inflammation. In certain embodiments of the uses and methods described herein, hsCRP levels are assessed in a biological sample, e.g., blood, obtained from the patient. A biological sample from the patient is assayed for the level of hsCRP. As used herein, the term “hsCRP” refers to the level of CRP in the blood as measured by high sensitivity CRP testing. The level of CRP or hsCRP in plasma may be given in any concentration, e.g., mg / dl, mg / L, nmol / L. Levels of CRP or hsCRP may be measured by a variety of well-known methods, e.g., radial immunodiffusion, electroimmunoassay, immunoturbidimetry, ELISA, turbidimetric methods, fluorescence polarization immunoassay, and laser nephelometry. Testing for CRP may employ a standard CRP test or a high sensitivity CRP (hsCRP) test (i.e., a high sensitivity test that is capable of measuring low levels of CRP in a sample, e.g., using laser nephelometry). Kits for detecting levels of CRP or hsCRP may be purchased from various companies, e.g., Calbiotech, Inc, Cayman Chemical, Roche Diagnostics Corporation, Abazyme, DADE Behring, Abnova Corporation, Aniara Corporation, Bio-Quant Inc., Siemens Healthcare Diagnostics, etc.

[0096] The term “high sensitivity C-reactive protein (hsCRP) level” in the context of a method for reducing the risk of or preventing cardiovascular events in a subject, refers to: • a 60 % increase from normalized levels in treated patients of C-reactive protein (CRP), wherein normalized levels indicate minimal or absent inflammation or disease activity (e.g., CRP < 2 mg / L); or

[0097] • an elevation of CRP of > 2 mg / L.

[0098] As used herein, the term “cardiovascular death” includes sudden cardiac death, death due to acute myocardial infarction (AMI), death due to heart failure, death due to stroke, and death due to other cardiovascular causes.

[0099] As used herein, “sudden cardiac death” is a sudden death that occurs in a previously stable patient who does not have a prior terminal condition, such as malignancy not in remission or end-stage chronic lung disease.

[0100] Death due to acute myocardial infarction (AMI): refers to a death within 30 days after a myocardial infarction (Ml) related to consequences seen immediately after the myocardial infarction, such as progressive congestive heart failure (CHF), inadequate cardiac output, or recalcitrant arrhythmia.

[0101] Death due to heart failure or cardiogenic shock refers to death occurring in the context of clinically worsening symptoms and / or signs of heart without evidence of another cause of death and includes sudden death occurring during an admission for worsening heart failure as well as death from progressive heart failure or cardiogenic shock following implantation of a mechanical assist device. Cardiovascular death due to heart failure is selected from a heart failure with reduced ejection fraction (HFrEF), heart failure with midrange ejection fraction or heart failure with mildly reduced ejection fraction (HFmrEF), heart failure with preserved ejection fraction (HFpEF), heart failure after acute myocardial infarct, or acute decompensated heart failure.

[0102] Death due to stroke (intracranial hemorrhage or non-hemorrhagic stroke) refers to death occurring up to 30 days after a suspected stroke based on clinical signs and symptoms as well as neuroimaging and / or autopsy, and where there is no conclusive evidence of another cause of death.

[0103] As used herein, “death due to other cardiovascular causes” refers to death due to a cardiovascular cause not included in the above categories (e.g. dysrhythmia, pulmonary embolism, cardiovascular intervention, aortic aneurysm rupture, or peripheral arterial disease). Mortal complications of cardiac surgery or non-surgical revascularization, even if “non-cardiovascular” in nature, should be classified as cardiovascular deaths.

[0104] As used herein, the term “myocardial infarction (Ml)” refers to “acute myocardial infarction”: the term myocardial infarction (Ml) is used when there is evidence of myocardial necrosis in a clinical setting consistent with myocardial ischemia. The term Ml includes an ST-elevated Ml (STEMI) or a non-ST-elevated Ml (NSTEMI). Under these conditions any one of the following criteria meets the diagnosis for Ml: The term “spontaneous Ml” refers to the detection of rise and / or fall of cardiac biomarkers with at least one value above the 99th percentile of the upper reference limit (URL) together with evidence of myocardial ischemia with at least one of the following: symptoms of ischemia, ECG changes indicative of new ischemia (ST Elevation - New ST elevation at the J-point in two contiguous leads with the cut-off points:> 0.2 mV in men or > 0.15 mV in women in leads V2-V3 and / or > 0.1 mV in other leads, ST depression and T- wave changes - New horizontal or down-sloping ST depression > 0.05 mV in two contiguous leads; and / or T inversion > 0.1 mV in two contiguous leads with prominent R waves or R / S ratio >1., development of pathological Q waves in the ECG (Any Q-wave in leads V2-V3 > 0.02 seconds or QS complex in leads V2 and V3, Q-wave > 0.03 seconds and > 0.1 mV deep or QS complex in leads I, II, aVL, aVF, or V4-V6 an any two leads of a contiguous lead grouping (I, aVL, V6, V4-V6, II, III, aVF), imaging evidence of new loss of viable myocardium or new regional wall motion abnormality.

[0105] The term “percutaneous coronary intervention (PCI) related myocardial infarct” refers to PCI in patients with normal baseline troponin values elevations of cardiac biomarkers above the 99th percentile URL within 24 hours of the procedure are indicative of peri-procedural myocardial necrosis. By convention increases of biomarkers greater than 3 x 99th percentile URL are consistent with PCI related myocardial infarction. If the cardiac biomarker is elevated prior to PCI a > 20% increase of the value in that second cardiac biomarker within 24 hours of the PCI and documentation that cardiac biomarkers were decreasing (two samples at least 6 hours apart) prior to the suspected recurrent Ml is also consistent with PCI related Ml. Symptoms of cardiac ischemia are not required.

[0106] Criteria for Prior Myocardial Infarction: Any of the following criteria meets the diagnosis for prior myocardial infarction: development of new pathological Q waves with or without symptoms, imaging evidence of a region of loss of viable myocardium that is thinned and fails to contract in the absence of a non-ischemic cause, pathological findings of a healed or healing myocardial infarction:

[0107] ECG changes associated with prior Myocardial Infarction:

[0108] • Any Q wave in leads V2-V3 > 0.02 seconds or QS complex in leads V2 and V3

[0109] • Q-wave > 0.03 seconds and > 0.1 mV deep or QS complex in leads I, II, aVL, aVF, or V4-V6 in any two leads of a contiguous lead grouping (I, aVL, V6, V4-V6, II, III, and aVF)

[0110] • R-wave > 0.04 seconds in V1-V2 and R / S > 1 with a concordant positive T- wave in the absence of a conduction defect

[0111] Criterion for Reinfarction: In patients where recurrent Ml is suspected from clinical signs or symptoms following the initial infarction, an immediate measurement of the employed cardiac biomarker is recommended. A second sample should be obtained 3-6 hours later. Recurrent infarction is diagnosed if there is a > 20% increase of the value in the second sample. This value should exceed the 99th percentile URL. However, if cardiac biomarkers are elevated prior to the suspected new Ml, there must also be documentation of decreasing values (two samples at least 6 hours apart) prior to the suspected new Ml. If the values are falling criteria for reinfarction by further measurement of biomarkers together with features of the ECG or imaging can be applied.

[0112] ECG diagnosis of reinfarction following the initial infarction: may be confounded by the initial evolutionary ECG changes. Reinfarction should be considered when the ST elevation > 0.1 mV reoccurs in an inpatient having a lesser degree of ST elevation or new pathognomonic Q-waves, in at least two contiguous leads, particularly when associated with ischemic symptoms for 10 minutes or longer. The re-evaluation of the ST segment can, however, also be seen in threatening myocardial rupture and should lead to additional diagnostic work-up. ST depression or LBBB on their own should not be considered valid criteria for Myocardial Infarction.

[0113] If biomarkers are increasing or peak is not reached, then there is insufficient data to diagnose recurrent Ml.

[0114] Clinical Classification of different types of Myocardial Infarction:

[0115] • Type 1 - Spontaneous Ml related to ischemia due to a primary coronary event such as plaque erosion and / or rupture, fissuring or dissection.

[0116] • Type 2 - Ml secondary to ischemia due to either increased oxygen demand or decreased supply, e.g., coronary artery spasm, anemia, hypotension, coronary embolism, arrhythmias, hypertension, or hypotension.

[0117] • Type 3 -Sudden unexpected cardiac death including cardiac arrest, often with symptoms suggestive of myocardial ischemia accompanied by presumably new ST elevation, or new LBBB, or evidence of fresh thrombus in a coronary artery by angiography and / or at autopsy, but death occurring before blood samples could be obtained or at a time before the appearance of cardiac biomarkers in the blood.

[0118] • Type 4a -Ml associated with PCI (Percutaneous Coronary Intervention).

[0119] • Type 4b -Ml associated with stent thrombosis as documented by autopsy or angiography.

[0120] • Type 5 -Ml associated with CABG (Coronary artery bypass grafting)

[0121] The term “silent Ml”: the following criteria will be used by the central ECG reading vendor to define interval “silent” (no clinical symptoms or signs) Ml between baseline and yearly ECGs (Surawicz B et al, Chou's electrocardiography in clinical practice: adult and pediatric. Philadelphia: Saunders; 2001): Myocardial infarctions are reported only on the basis of pathologic Q waves. Pathologic Q waves are defined as Q wave duration > 40ms and Q / R ratio = 1 / 3.

[0122] Any Q wave in V1 or V2 that is followed by an R wave should be considered abnormal.

[0123] When pathologic Q waves (i.e., myocardial infarction) are present, ST elevation or T wave inversion may be used to classify the infraction as New or Acute. However, ST elevation or T wave inversion in the absence of pathologic Q waves are not sufficient criteria for diagnosis of myocardial infarction.

[0124] • Anterolateral Ml - Pathologic Q waves in leads V3-V6.

[0125] • Anterior Ml - Pathologic Q waves in V3 and V4.

[0126] • Anteroseptal Ml - Pathologic Q waves or QS in leads V1-V4.

[0127] • Extensive Anterior Ml - Pathologic Q waves in leads I, aVL, and V1-V6.

[0128] • High lateral Ml - Pathologic Q waves in leads I and aVL.

[0129] • Inferior Ml - Pathologic Q waves or QS in at least two of the inferior leads: aVF, III, II.

[0130] • Lateral Ml - Pathologic Q waves in leads I, aVL, and V5-V6.

[0131] • Septal Ml - Pathologic Q waves or QS in leads V1-V2, (V3). In the presence of LAHB or LVH a Q or QS in V3 is required.

[0132] • Posterior Ml - Initial R wave duration 40 ms in V1 or V2, and R > S and upright T wave; Inferior or Lateral Ml are usually also present.

[0133] The term “new Ml” as used herein is based on criteria for Ml more stringent than the Expert Consensus Document criteria, requiring Q waves to be > 0.04 sec in duration and an R / S ratio > 1 / 3. These criteria (drawn from the cardiology literature) are designed to minimize the false positive detection of Mis due to very small physiologic Q waves in the inferior and anterolateral leads.

[0134] As used herein, the term “stroke” is defined as the rapid onset of a new persistent neurological deficit attributed to an obstruction in cerebral blood flow and / or cerebral hemorrhage with no apparent non-vascular cause (e.g. tumor, trauma, infection). Available neuroimaging studies will be considered to support the clinical impression and to determine if there is a demonstrable lesion compatible with an acute stroke. Non-fatal strokes will be classified as ischemic, hemorrhagic, or unknown.

[0135] As used herein, the term “unstable angina requiring unplanned revascularization” is defined as no elevation in cardiac biomarkers and clinical presentation (one of the following) with cardiac symptoms lasting >10 minutes and considered to be myocardial ischemia on final diagnosis (rest angina or new onset (<2 months) severe angina (CCS classification severity > III; Grading of Angina Pectoris According to Canadian Cardiovascular Society Classification) or increasing angina (in intensity, duration and / or frequency) and severe recurrent ischemia requiring urgent revascularization: as defined by an episode of angina prompting the performance of coronary revascularization on the index hospitalization or an episode of recurrent angina after discharge that resulted in rehospitalization during which coronary revascularization was performed; and at least one of the following: new or worsening ST or T segment changes on ECG, ST Elevation (new ST elevation at the J point in two anatomically contiguous leads with the cut-off points: > 0.2 mV in men (> 0.25 mV in men < 40 years) or > 0.15 mV in women in leads V2-V3 and / or > 0.1 mV in other leads), ST depression and T-wave Evidence of ischemia on stress testing with cardiac imaging, evidence of ischemia on stress testing without cardiac imaging but with angiographic evidence of > 70% lesion, and / or thrombus in the epicardial coronary artery or initiation / increased dosing of anti-anginal therapy, angiographic evidence of > 70% lesion and / or thrombus in an epicardial coronary artery.

[0136] As used herein, “coronary revascularization" is defined as an invasive procedure, which usually follows coronary angiography, wherein either percutaneous transluminal intervention, followed by Stent Placement, Balloon Angioplasty, or CABG is performed to relieve obstructed coronary arteries. A team of medical professionals lead by either an invasive cardiologist (percutaneous transluminal intervention, followed by stent placement, balloon angioplasty) or a thoracic surgeon (CABG), who performs the described procedures.

[0137] As used herein, the term ”non-coronary revascularization" is defined as vascular surgery or percutaneous intervention. Vascular surgery is defined as the placement of a conduit with or without proximal and / or distal anastamoses. Percutaneous intervention is defined as balloon inflation with or without stenting.

[0138] As used herein, the term “atherosclerosis” occurs when fatty material and a substance called plaque builds up on the walls of the arteries. This causes their lumen to get narrow.

[0139] The term “treat”, “treating”, “treatment”, “prevent”, “preventing” or “prevention” includes therapeutic treatments, prophylactic treatments and applications in which one reduces the risk that a subject will develop a disorder or other risk factor. Treatment does not require the complete curing of a disorder and encompasses the reduction of the symptoms or underlying risk factors.

[0140] The term “treating or preventing” includes the administration of a compound, e.g., the bispecific antibody targeting both IL-ip and IL-18, e.g., bbmAb, optionally in combination with at least one further therapeutic agent, to prevent or delay the onset of the symptoms, complications, or biochemical indicia of a disease, condition, disorder, or syndrome (e.g., for reducing the risk of or preventing cardiovascular events in a subject known to be a carrier of clonal expansion of hematopoietic cell lines with somatic mutations. Treatment may be prophylactic (to prevent or delay the onset of the disease, condition, disorder, or syndrome, or to prevent the manifestation of clinical or subclinical symptoms thereof) or therapeutic suppression or alleviation of symptoms after the manifestation of the disease, condition, disorder, or syndrome.

[0141] As used herein, the term “prevent”, “preventing”, or “prevention” in connection to a disease, condition, disorder, or syndrome also refers to prophylactic treatment of a subject who is at risk of developing a condition {e.g., risk of a cardiovascular events in a subject) resulting in a decrease in the probability that the subject will develop the condition.

[0142] Also “treating” may refer to slow the progress of, halt, or reverse disease, condition, disorder, or syndrome progression and improve clinical outcomes.

[0143] As used herein, the term “therapeutically effective amount” of the the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, described herein refers to an amount of said antibody that will elicit the biological or medical response of a subject, for example, reducing the risk of or preventing cardiovascular events, ameliorate symptoms, alleviate conditions, slow or delay disease progression, or prevent a disease, condition, disorder, or syndrome, etc. In one non-limiting embodiment, the term “a therapeutically effective amount” refers to the amount of the bispecific antibody targeting both IL-1 p and IL-18, e.g., bbmAb, that, when administered to a subject, is effectively reducing the risk of or preventing cardiovascular events in a CHIP population.

[0144] As used herein, the term “recurrent CV event” is a repeated CV event taking place after the myocardial infarction and comprises non-fatal Ml, non-fatal stroke, cardiovascular (CV) death and hospitalization for unstable angina requiring unplanned revascularization.

[0145] Accordingly, in one embodiment of any method or use of the invention, said recurrent CV event is selected from non-fatal Ml, non-fatal stroke, cardiovascular (CV) death and hospitalization for unstable angina requiring unplanned revascularization.

[0146] In another embodiment of any method or use of the invention, said recurrent CV event is selected from non-fatal Ml, non-fatal stroke and cardiovascular (CV) death.

[0147] In yet another embodiment of any method or use of the invention, said recurrent CV event is non-fatal Ml or cardiovascular (CV) death.

[0148] In another embodiment of any method or use of the invention, said recurrent CV event is non-fatal Ml.

[0178] 4. Fc modifications

[0179] In addition, or alternative to modifications made within the framework or CDR regions, antibodies used in the methods of the invention may be engineered to include modifications within the Fc region, typically to alter one or more functional properties of the antibody, such as serum half-life, complement fixation, Fc receptor binding, and / or antigendependent cellular cytotoxicity. Furthermore, an antibody used in the methods of the invention may be chemically modified (e.g., one or more chemical moieties can be attached to the antibody) or be modified to alter its glycosylation, again to alter one or more functional properties of the antibody. Each of these embodiments is described in further detail below. The numbering of residues in the Fc region is that of the Ell numbering scheme of Edelman et al., PNAS, 1969 May, 63(1):78-85.

[0180] In one embodiment, the hinge region of CH1 is modified such that the number of cysteine residues in the hinge region is altered, e.g., increased or decreased. This approach is described further in U.S. Patent No. 5,677,425 by Bodmer et al. The number of cysteine residues in the hinge region of CH1 is altered to, for example, facilitate assembly of the light and heavy chains or to increase or decrease the stability of the antibody.

[0181] In another embodiment, the Fc hinge region of an antibody used in the methods of the invention is mutated to decrease the biological half-life of the antibody. More specifically, one or more amino acid mutations are introduced into the CH2-CH3 domain interface region of the Fc-hinge fragment such that the antibody has impaired Staphylococcal protein A (SpA) binding relative to native Fc-hinge domain SpA binding. This approach is described in further detail in U.S. Patent No. 6,165,745 by Ward et al.

[0182] In another embodiment, the antibody used in the methods of the invention is modified to increase its biological half-life. Various approaches are possible. For example, one or more of the following mutations can be introduced: T252L, T254S, T256F, as described in U.S. Patent No. 6,277,375 to Ward. Alternatively, to increase the biological half life, the antibody used in the methods of the invention can be altered within the CH1 or CL region to contain a salvage receptor binding epitope taken from two loops of a CH2 domain of an Fc region of an IgG, as described in U.S. Patent Nos. 5,869,046 and 6,121 ,022 by Presta et al.

[0183] In yet other embodiments, the Fc region of the antibody used in the methods of the invention is altered by replacing at least one amino acid residue with a different amino acid residue to alter the effector functions of the antibody. For example, one or more amino acids can be replaced with a different amino acid residue such that the antibody has an altered affinity for an effector ligand but retains the antigen-binding ability of the parent antibody. The effector ligand to which affinity is altered can be, for example, an Fc receptor or the C1 component of complement. This approach is described in further detail in U.S. Patent Nos. 5,624,821 and 5,648,260, both by Winter et al.

[0184] In another embodiment, one or more amino acids selected from amino acid residues can be replaced with a different amino acid residue such that the antibody used in the methods of the invention has altered C1q binding and / or reduced or abolished complement dependent cytotoxicity (CDC). This approach is described in further detail in U.S. Patent Nos. 6,194,551 by Idusogie et al.

[0185] In another embodiment, one or more amino acid residues are altered to thereby alter the ability of the antibody to fix complement. This approach is described further in PCT Publication WO 94 / 29351 by Bodmer et al.

[0186] In yet another embodiment, the Fc region is modified to increase the ability of the antibody to mediate antibody dependent cellular cytotoxicity (ADCC) and / or to increase the affinity of the antibody for an Fey receptor by modifying one or more amino acids. This approach is described further in PCT Publication WO 00 / 42072 by Presta. Moreover, the binding sites on human lgG1 for FcyRI, FcyRI I, FcyRI 11 and FcRn have been mapped and variants with improved binding have been described (see Shields, R.L. et al, (2001) J Biol Chem 276:6591-6604).

[0187] In certain embodiments, the Fc domain of lgG1 isotype is used. In some specific embodiments, a mutant variant of lgG1 Fc fragment is used, e.g., a silent lgG1 Fc which reduces or eliminates the ability of the fusion polypeptide to mediate antibody dependent cellular cytotoxicity (ADCC) and / or to bind to an Fey receptor. An example of an lgG1 isotype silent mutant wherein Leucine residue is replaced by Alanine residue at amino acid positions 234 and 235 as described by Hezareh et al, J. Virol (2001); 75(24):12161-8.

[0188] In certain embodiments, the Fc domain of the antibody used in the methods of the invention comprises a mutant preventing glycosylation at position 297 of Fc domain. For example, the Fc domain contains an amino acid substitution of asparagine residue at position 297. Example of such amino acid substitution is the replacement of N297 by a glycine or an alanine.

[0189] Silenced effector functions can be obtained by mutation in the Fc region of the antibodies used in the methods of the invention and have been described in the art: LALA and N297A (Strohl, W., 2009, Curr. Opin. Biotechnol. vol. 20(6):685-691); and D265A (Baudino et al., 2008, J. Immunol. 181 :6664-69; Strohl, W., supra); and DAPA (D265A and P329A) (Shields RL., J Biol Chem. 2001 ;276(9):6591 -604; U.S. Patent Publication US2015 / 0320880). Examples of silent Fc lgG1 antibodies comprise the so-called LALA mutant comprising L234A and L235A mutation in the lgG1 Fc amino acid sequence. Another example of a silent lgG1 antibody comprises the D265A mutation. Another example of a silent lgG1 antibody is the so-called DAPA mutant, comprising D265A and P329A mutations to the lgG1 Fc amino acid sequence. Another silent lgG1 antibody comprises the N297A mutation, which results in aglycosylated / non-glycosylated antibodies. Additional Fc mutations for providing silenced effector function are described in PCT publication no. WO2014 / 145806 (e.g., in Figure 7 of WO2014 / 145806), herein incorporated by reference in its entirety. One example from WO2014 / 145806 of a silent I gG 1 antibody comprises a E233P, L234V, L235A, and S267K mutation, and a deletion of G236 (G236del). Another example from WO2014 / 145806 of a silent lgG1 antibody comprises a E233P, L234V, and L235A mutation, and a deletion of G236 (G236del). Another example from WO2014 / 145806 of a silent lgG1 antibody comprises a S267K mutation.

[0190] In still another embodiment, the glycosylation of an antibody used in the methods of the invention is modified. For example, an aglycosylated antibody can be made (i.e., the antibody lacks glycosylation). Glycosylation can be altered to, for example, increase the affinity of the antibody for the antigen. Such carbohydrate modifications can be accomplished by; for example, altering one or more sites of glycosylation within the antibody sequence. For example, one or more amino acid substitutions can be made that result in elimination of one or more variable region framework glycosylation sites to thereby eliminate glycosylation at that site. Such aglycosylation may increase the affinity of the antibody for antigen. Such an approach is described in further detail in U.S. Patent Nos. 5,714,350 and 6,350,861 by Co et al.

[0191] Additionally or alternatively, an antibody to be used in the methods of the invention can be made that has an altered type of glycosylation, such as a hypofucosylated antibody having reduced amounts of fucosyl residues or an antibody having increased bisecting GIcNac structures. Such altered glycosylation patterns have been demonstrated to increase the ADCC ability of antibodies. Such carbohydrate modifications can be accomplished by, for example, expressing the antibody in a host cell with altered glycosylation machinery. Cells with altered glycosylation machinery have been described in the art and can be used as host cells in which to express recombinant antibodies of the invention to thereby produce an antibody with altered glycosylation. For example, EP 1 ,176,195 by Hang et al. describes a cell line with a functionally disrupted FUT8 gene, which encodes a fucosyl transferase, such that antibodies expressed in such a cell line exhibit hypofucosylation. Therefore, in one embodiment, the antibodies used in the methods of the invention are produced by recombinant expression in a cell line which exhibit hypofucosylation pattern, for example, a mammalian cell line with deficient expression of the FUT8 gene encoding fucosyltransferase. PCT Publication WO 03 / 035835 by Presta describes a variant CHO cell line, Lecl3 cells, with reduced ability to attach fucose to Asn(297)-linked carbohydrates, also resulting in hypofucosylation of antibodies expressed in that host cell (see also Shields, R.L. et al., 2002 J. Biol. Chem. 277:26733-26740). PCT Publication WO 99 / 54342 by Umana et al. describes cell lines engineered to express glycoprotein-modifying glycosyl transferases (e.g., beta(1 ,4)-N acetylglucosaminyltransferase III (GnTIII)) such that antibodies expressed in the engineered cell lines exhibit increased bisecting GIcNac structures which results in increased ADCC activity of the antibodies (see also Umana et al., 1999 Nat. Biotech. 17:176-180). Alternatively, the antibodies used in the methods of the invention can be produced in a yeast or a filamentous fungi engineered for mammalian-like glycosylation pattern, and capable of producing antibodies lacking fucose as glycosylation pattern (see for example EP1297172B1).

[0192] Another modification of the antibodies used in the methods of the invention that is contemplated by the invention is one or more modifications to increase formation of a heterodimeric bispecific antibody. A variety of approaches available in the art can be used in for enhancing dimerization of the two heavy chain domains of bispecific antibodies, e.g., bbmAb, as disclosed in, for example, EP 1870459A1 ; U.S. Pat. No. 5,582,996; U.S. Pat. No. 5,731 ,168; U.S. Pat. No. 5,910,573; U.S. Pat. No. 5,932,448; U.S. Pat. No. 6,833,441 ; U.S. Pat. No. 7,183,076; U.S. Patent Application Publication No. 2006204493A1 ; and PCT Publication No. W02009 / 089004A1 , the contents of which are incorporated herein in their entireties.

[0193] Generation of bispecific antibodies using knobs-into-holes is disclosed e.g., in PCT Publication No. W01996 / 027011 , Ridgway et al., (1996), and Merchant et al. (1998).

[0194] In practicing some of the methods of treatment or uses of the present disclosure, a therapeutically effective amount of a bispecific antibodies targeting both IL-1 p and IL-18 simultaneously, e.g., bbmAb, has to be administered to a subject in need thereof.

[0195] Example 1 :

[0196] The generation of bbmAb has been described in detail in the examples 1 to 5 of the patent application WO / 2018 / 229612. The examples 1 of WO / 2018 / 229612, comprising (1) vector construction, (2) Host cell line and transfection, (3) Cell selection and sorting, (4) Cell expansion, (5) Clone stability, (6) Manufacturing, (7) Analytical characterization and purity assessment, (8) Analytical Results are herewith incorporated by reference in their entirety.

[0197] The bbmAb, is a bispecific lgG1 , with LALA silencing mutations, simultaneously binding to two distinct targets, IL-1 p and IL-18. The antibody combines two distinct antigen binding arms (Fab fragments), whereas the Fab directed against IL-1 p is based on mAb2 and contains a kappa light chain (Vk6). The Fab directed against IL-18 is based on mAb1 and is composed of a lambda light chain (VA1). In order to drive hetero-dimerization of the Fc domain during expression a “knob” with a bulky amino acid (aa) side chain (S354C and T366W) in the mAb1 heavy chain and a “hole” with small aa side chains (Y349C, T366S, L368A, Y407V) were introduced in the mAb2 heavy chain. For ease of reference, the amino acid sequences of the hypervariable regions of bbmAb, based on the Kabat definition and the Chothia definition, as well as the VL and VH domains and full heavy and light chains are provided in Table 3, below.

[0198] Table 3. Amino acid sequences of the hypervariable regions (CDRs), variable domains (VH and VL) and full chains of bbmAb. The DNA encoding the first VL of is set forth in SEQ ID NO: 102 and the DNA encoding the second VL is set forth in SEQ ID NO: 70. The DNA encoding the first VH is set forth in SEQ ID NO:86 and the DNA encoding the second VH is set forth in SEQ ID NO: 54.

[0326] Example 2: Therapeutic use

[0327] A randomized, placebo-controlled, parallel-group, investigator- and participant-blinded clinical study to investigate efficacy, safety and tolerability of bbmAb in an adult population with coronary heart disease and Clonal Hematopoiesis of Indeterminate Potential (CHIP).

[0328] 2.1 Protocol summary

[0329] Cardiovascular diseases (CVD) remain the leading cause of disease burden in the world. Persistent inflammation is common in populations at increased risk of CVD events and medications that reduce inflammation have been linked to improved CVD outcomes. Inhibition of I L-1 p and IL-18 by direct neutralization with a bispecific monoclonal antibody (bbmAb), may safely and effectively lower the risk of CVD among people with known heart disease and TET2 or DNMT3A CHIP (VAF >2%). This clinical trial will evaluate the effectiveness and safety of a single s.c. dose of bbmAb, to reduce key markers of inflammation related to CVD risk, such as IL-6 and IL-18, in approximately 28 people with known heart disease and TET2 or DNMT3A CHIP (VAF >2%). 2.2 T reatment of interest related Objectives and endpoints

[0330] The treatment of interest is a single s.c. dose of bbmAb (600 mg), or placebo.

[0331] Table 4 Objectives and related endpoints

[0332] Objective(s) Endpoint(s)

[0333] Primary objective(s) Endpoint(s) for primary objective(s)

[0334] • To evaluate the effect of bbmAb versus • Serum level of IL-6 at 3 weeks placebo to reduce circulating levels of after a single s.c. dose of bbmAb inflammatory markers in participants with coronary heart disease and CHIP

[0335] Secondary objective(s) Endpoint(s) for secondary objective(s)

[0336] • To evaluate the safety and tolerability of • Adverse events, and parameters bbmAb in participants with coronary heart from safety assessments, disease and CHIP including vital signs, electrocardiograms, and laboratory assessments (urine and blood)

[0337] • To assess the pharmacokinetics of • Serum concentrations of bbmAb bbmAb in participants with coronary heart after a single s.c. dose of bbmAb disease and CHIP

[0338] 2.2 Primary estimands The primary analysis for bbmAb will assess the effect on the change in IL-6 compared to placebo after 3 weeks of treatment.

[0339] The primary clinical question of interest is: What is the effect of bbmAb in addition to SoC CVD prevention medication in patients with known coronary heart disease and presence of CHIP on the inflammatory markers IL-6 and IL-18. The estimand will capture the effect of the investigational treatments versus placebo under research-like conditions.

[0340] The estimand is defined by the following attributes:

[0341] 1. Population: participants with known coronary heart disease and TET2 or DNMT3A CHIP (VAF >2%) and potential SoC CVD prevention medication

[0342] 2. Endpoints: Serum levels of IL-6 at week 3 after the start of a dosing period for bbmAb.

[0343] 3. Treatment of interest: bbmAb or placebo single dose.

[0344] 2.3 Study design

[0345] This is a multi-center, randomized, placebo-controlled, participant- and investigator-blinded study to evaluate the efficacy, safety, and tolerability a single s.c. dose of bbmAb for inflammatory marker reduction in participants with coronary heart disease and TET2 or DNMT3A CHIP (VAF >2%).

[0346] Participants will be randomized into the trial, of which a minimum of approximately 40% of randomized participants will be CHIP with TET2 somatic mutations. For subgroup allocation, patients with TET2 or DNMT3A CHIP will be determined based on their most common mutation (e.g., patients with mutations in both DNMT3A + TET2 will be allocated to the subgroup based on the mutation with the highest variant allele frequency (VAF).

[0347] Participants meeting all eligibility criteria will be randomized (1 bbmAb+placebo sequence).

[0348] On Day 1, participants will receive the single s.c. dose of either bbmAb or placebo. After initial dosing, assessments will be conducted at site, as specified in Section 1.3, the Schedule of Activities (SoA).

[0349] Table 5 Rationale for study design

[0350] Study Design Aspect Rationale

[0351] Overall In this study, participants will be randomized to a single s.c. dose of bbmAb, and / or placebo. Primary Endpoints For bbmAb, the only primary endpoint is IL-6 levels measured 3 weeks after a single s.c. dose.

[0352] Analysis of primary The IL-6 response to bbmAb will be measured at approximately endpoints 3 week intervals during the approximately 12 week treatment period after only a single dose level. The level of IL-6 obtained 3 weeks after the single dose of 600mg bbmAb s.c. will be used for the primary endpoint, as the peak observed reduction of IL-6 (when measured q3 weeks over the 12 week treatment period) is expected to occur at that point.

[0353] Treatment Sequences Treatment Sequence 1 contributes data for a single dose of active bbmAb.

[0354] Duration of study bbmAb will result in the simultaneous reduction of both IL-1 p and periods IL-18 levels in serum and effective neutralization of I L-1 as well as IL-18 with reductions captured at each 3 week assessment in the 12 week treatment period. The follow-up period up to Day 114 allows for adequate safety monitoring over a period of approximately 5 half-lives.

[0355] Placebo comparator The use of placebo provides a comparison group for an unbiased collection and assessment of safety, tolerability, efficacy, and PD parameters. The study design includes both inter- and intra-individual placebo comparators.

[0356] Rationale for choice of control drugs (comparator / placebo) or combination drugs

[0357] Placebo treatment will be used as a comparator to provide objective control for the evaluation of efficacy, safety, and tolerability during the treatment with bbmAb. For bbmAb, matching placebo in individual 6 mL glass vials each containing 0 mg / 1 mL as a liquid solution will be provided, and will be indistinguishable in appearance.
