[0004] Currently, there are needs in the development of effective drugs and methods in treating hypertrophic cardiomyopathy (HCM) . The compositions, recombinant adeno-associated virus (rAAV) particles, systems, methods, and kits of the present disclosure solve the needs thereof.

[0005] In one aspect, disclosed herein is a recombinant adeno-associated virus (rAAV) particle, comprising a polynucleotide sequence comprising a coding sequence encoding human cardiac isoform of myosin binding protein C (cMyBP-C) , wherein the coding sequence is a MYBPC3 gene sequence, wherein the MYBPC3 gene sequence is a codon-optimized gene sequence or a wild type gene sequence.

[0169] AAV vectors reduce disease biomarkers at mRNA level in the heart at 6 weeks post injection. Nppa and Nppb encodes ANP (atrial natriuretic peptide) and BNP (B-type natriuretic peptide) , respectively. Both hormones are secreted by the heart and are involved in cardiac development, cardiorenal homeostasis, and implicated in response to cardiac injury and stress. Both Nppa and Nppb genes were elevated in vehicle treated Mybpc3 KO mice compared to WT (wild-type) and significantly decreased with treatment from most of AAV vectors (PA456 to PA459) . Results are summarized in FIG. 4.

[0170] Example 6 -Evaluation of the effect of various constructs in the juvenile Mybpc3 KO mice.

[0171] Juvenile Mybpc3 KO mice (~4 weeks old) are administered with various doses (1×1013 to 3×1014 vg / kg) of AAV vectors or vehicles as a control. Echocardiography is used to assess cardiac function changes 4 weeks after injection, 8 weeks after injection and 18 weeks after injection. AAV vectors significantly improve left ventricular ejection fraction (LVEF) and fractional shortening of the mice. Heart stress and damage biomarkers (cTnI and NT-proBNP) in the serum are significantly decreased 8 weeks after injection. Masson’s trichrome staining shows that heart fibrosis is improved with AAV treatment. RT-qPCR shows mRNA of human MYBPC3 gene is highly expressed in the heart tissue, but not in the liver or the spleen. Immunofluorescence staining shows that human cMyBP-C protein is highly expressed in the cardiomyocytes, and colocalized to the α-actinin in the sarcomere, indicating its proper function for cardiomyocyte contraction. Collectively, the data suggest that AAV vector constructs significantly improve the cardiac function and reduce heart damage in juvenile Mybpc3 KO mice.

[0173] Adult Mybpc3 KO mice (~3.5 months old) are injected with 5×1013 and 1.5×1014 vg / kg of AAV vectors or vehicles as a control. Echocardiography is used to assess cardiac function changes before injection, 6 weeks after injection and 12 weeks after injection. Wild type mice serve as wild-type control for cardiac function. AAV vectors significantly improve left ventricular ejection fraction (LVEF) and cardiac output of the mice. Western blotting shows that cMyBP-C protein is only expressed in the heart, but not expressed in the liver. Immunochemistry staining shows broad human cMyBP-C protein expression in the heart. The data demonstrates that the AAV vector constructs can specifically transduce the heart and improve the cardiac function in adult Mybpc3 KO mice, suggesting that these AAV vector constructs are promising AAV gene therapy to treat adult HCM patients.
