[0001] The present disclosure relates to anti-4-1BB single domain antibodies, heavy chain antibodies, bispecific antibodies, immunoconjugates, pharmaceutical compositions and uses thereof in treating, preventing or diagnosing cancers.Background

[0002] 4‐1BB (CD137, tumor necrosis factor receptor superfamily 9) , a member of the TNF receptor superfamily (TNFRSF) , is a costimulatory molecule. 4-1BB is widely expressed on activated T cells, activated NK cells, NKT cells, B cells, eosinophils, mast cells, neutrophils, mature Tregs, activated monocytes, and dendritic cells. In different cells, 4-1BB signaling shows different mechanisms of action after activation, and 4‐1BB plays an important role in regulating the activity of a variety of immune cells. 4-1BB agonists can enhance immune cell proliferation, survival, cytokine secretion and cytolytic activity of CD8 T cells, and 4-1BB target drugs have a wide range of application prospects.

[0003] Trastuzumab (Herceptin) , a monoclonal antibody targeting HER2 / neu, is used to treat HER2 breast cancer. Its sales exceeded $6 billion in 2013 and has been growing steadily for many years. Compared with chemotherapy alone, the combination therapy with Herceptin can reduce the mortality by 11%, and the response rate can be increased from 32%to 50%. Therefore, better HER2-targeted drugs and treatment strategies are urgently needed to improve the response rate.

[0368] The phage selection method was optimized by reducing the antigen dosage and increasing the frequency of magnetic bead washes. Briefly, a 1 mL incubation system was prepared by amalgamating the fully human single-domain antibody phage library with MPBS and biotinylated antigen. The antigen quantity was gradually diminished in each round of incubation at room temperature using a shaker for 30 minutes. Subsequently, 20 μL of magnetic beads were added, and the mixture was further incubated at room temperature for 1.5 hours. Solid-liquid separation was achieved using a magnetic rack, and the number of magnetic bead washes was augmented with each round of selection. Following infection of TG1 cells, 10 μL of M13 helper phage was added, and the phage was precipitated using PEG8000 the following day for the subsequent round of selection. Monoclonal antibodies demonstrating specific binding to the 4-1BB antigen were chosen for further analysis of their distinctive sequences and sequence diversity. These anti-4-1BB fully human single-domain antibodies were numbered BM157. Binding activity was measured by ELISA after prokaryotic expression, as shown in Figure 1, with an EC50 range of 7 -585 nM (Table 1) .

[0376] A 96-half well microtiter plate was coated with 50 μL of 2 μg / mL antigen protein diluted in PBS at 4 ℃ overnight, including: 4-1BB-Fc, HER2-Fc. Fc-isotype was used as a control antigen to detect the nonspecificity of BM231. The microtiter plates were then blocked with 100 μL / well of 3%BSA and incubated in a constant temperature incubator at 37 ℃ for 1 hour. Threefold dilutions of antibody from 500 nM were added to each well for a total of three repeat Wells per concentration and incubated for 1.5 h at 37 ℃. Plates were washed with 0.05%PBST and then incubated with Anti-Fab-HRP (SIGMA) antibody for 45 min at 37 ℃. After washing, the plates were developed with TMB substrate and detected with a microplate reader at OD405nm, and the data were analyzed with Graph Pad Prism 8 software to calculate EC50.

[0379] Example 4: Binding of bispecific antibodies to cell surface 4‐1BB or HER2

[0380] To evaluate the cell-surface antigen binding properties of BM231, the binding of anti-HER2 ‐4‐1BB antibody to FCHO expressing 4‐1BB and SKOV3 overexpressing HER2 were analyzed by FACS. The total number of 1×105 FCHO‐4‐1BB cells in each well was incubated for 30 min at 4 ℃ with antibody serially diluted in FACS buffer (containing 2%FBS in PBS) at a maximum antibody concentration of 500 nM in 10 gradients of one-third dilution. After washing with FACS buffer, FITC anti-human Fc antibody was added to each well and incubated for 30 min at 4 ℃. After washing, the MFI of FITC was determined by FACS with IgG4-isotype as a negative control antibody (Baiying Biological) . Statistics of 50 values are presented in Table 4. As shown in Figure 4, Some of the anti-HER2 / 4‐1BB antibodies tested (BM231-60 / 61 / 66 / 74 / 77) showed a concentration-dependent binding ability to 4‐1BB, and BM180-96 (Urelumab) had an EC50 of 0.72 nM. The EC50 of BM231 bispecific antibodies ranged from 3.85 nM to 42.21nM, and the EC50 of BM130-97 (Herceptin) was 20.76 nM.

[0383] To further demonstrate that anti-HER2 / 4‐1BB antibodies can bind both HER2 and 4‐1BB, a double antigen capture ELISA assay was performed. Briefly, microtiter plates were coated with human HER2‐Fc protein in PBS at 100 ng / well overnight at 4 ℃ and then blocked with 100 μL / well of 3%BSA for 1 h at 37 ℃. A threefold dilution of BM231 diluted starting at 500 nM was added to each well and incubated for 1.5 hours at 37 ℃. Plates were washed with 0.05%PBST and then incubated for 1 h at 37 ℃ with 100 ng / well of human / cynomolgus / moues 4‐1BB‐His protein diluted in 1%BSA. The plates were washed with 0.05%PBST and then incubated with anti-His-HRP (GENEWIZ) for 45 min at 37 ℃. After washing, the plates were stained with TMB substrate and detected by microplate reader at OD405nm. The data were analyzed by Graph Pad Prism 8 software to calculate EC50.

[0386] Functional characterization of cell line-based anti-HER2 / 4‐1BB bispecific antibodies was performed to test the ability of bispecific antibodies to promote 4‐1BB signaling. In this study, 4-1BB-NF-κB‐Luc2p-Jurkat cell line (Nanjing Cobioer) was used as the effector cells, SKOV3 and MC38 cells with or without HER2 expression were used as the target cells, and huHER2-MC38 cell line with stable expression of human HER2 was also used as the target cells. The 4-1BB-NF-κB ‐Luc2p-Jurkat cell line was genetically modified to stably express 4‐1BB and luciferase. Luciferase expression is induced upon antibody binding to the 4‐1BB receptor. In brief, BM231 was incubated at a maximum concentration of 300 nM, and the antibody was diluted in a gradient of 1640 medium containing 5%FBS, one third diluted for 11 gradients. 4‐1BB-NF-κB‐Luc2p-Jurkat cells were centrifuged at 1600 rpm to collect cell resuspension counts, the ratio of effector cells to target cells was 1: 1, 2.5×104 target cells plus 2.5×104 effector cells were seeded in a 96-well plate in a total volume of 50 μL per well. Then 25 μL of gradient diluted antibody to be tested was added to each well, and the actual final concentration of antibody used was 100 nM. The cells were incubated for 5 h at 37 ℃ in a 5 %CO2 constant temperature incubator. 75 μL assay substrate (promega) was added to each well of the assay plate after 5 hours. After 10 minutes of incubation at room temperature, 100 μL per well was transferred to a 96-well white opaque plate for chemiluminescence measurement using a microplate reader. Four-parameter logarithmic curve analysis was performed with GraphPad software.

[0389] Example 7: Anti-HER2 / 4-1BB bispecific antibody promotes PBMC activation activity

[0390] To test the ability of anti-HER2 / 4‐1BB bispecific antibody (BM231) to activate human peripheral blood mononuclear cells (PBMC) , cytokine production assays were used. Human PBMC cells were co-stimulated with human anti-CD3 antibody and BM231-60, BM231-77, BM231-99, HER2 high expression SKOV3 cells were used as target cells, and FCHO was used as control target cells. 96-well plates were coated with 0.25 μg / mL human anti-CD3 antibody in 200 μL per well. Human peripheral blood mononuclear cells (2×105 / well) were co-cultured with SKOV3 or control FCHO cells (2×104 / well) in the presence of human anti-CD3 antibody. Serial dilutions of bispecific antibodies were added to the mixed cultures, and the highest concentration used for detection of IFN-γ was 2000 nM in 8 gradients of 1 / 5 dilutions. For the detection of IL-2, the highest concentration used was 80 nM in six gradients of 1 / 5 dilution. After 48 hours, the culture medium supernatant was removed to measure the level of IL-2 expression using the Human IL-2 kit of Perkinelmer. After 72 hours, the culture medium supernatant was removed to measure IFN-γ expression using Perkinelmer's Human IFN-γ kit cytokine assay.

[0391] As shown in Figure 7, all antibodies failed to effectively stimulate IFN-γsecretion by PBMC cells when co-incubated with HER2-negative FCHO cells, with BM231-08 and BM231-76 showing A tendency to reduce secretion even at the highest concentrations with high background values (Figure 7A) . However, all bispecific antibodies activated PBMC in a HER2-dependent manner to promote IFN-γproduction after co-incubation with HER2-overexpressing SKOV3 cells, which increased with the concentration of antibodies (Figure 7B) . The EC50 values of BM231-60 were the highest: 57.9 nM, but the highest expression of IFN-γ was not as high as that of the other bispecific antibodies. Compared with the negative control group (Figure 7 C) , the expression level of IL-2 in the experimental group co-incubated with SKOV3 was increased by 4-to 5-fold as a whole (Figure 7 D) , indicating that BM231 significantly activated PBMC to promote IL-2 production.

[0392] Example 8: Anti-HER2 / 4-1BB bispecific antibody promotes the expression of granzyme B by cytotoxic T cells

[0393] CD3 was diluted to 0.1 μg / mL with PBS to coat plates, and PBMC (2×105 / well) were co-activated with BM231 for 24 h. The highest concentration of BM231 bispecific antibody was 2000 nM. Three gradients were diluted in one-tenth dilutions. Cells were first stained with NIR live or dead staining solution reagent, then blocked by Fc-block incubation for 10 min, and washed once with staining Buffer. Cell surface markers, including human CD45, CD3, CD4 and CD8, were stained at room temperature in the dark for 30 min, washed with staining Buffer, and centrifuged at 1500 rpm for 3 min to remove the supernatant. 200 μL fixation / membrane breaking working solution was added to each well and incubated for 30 min at room temperature in the dark. Samples were centrifuged at 2500 rpm for 5 min at room temperature and the supernatant was discarded. 200 μL of 1×membrane breaking solution was added to each well, the samples were centrifuged at 2500 rpm for 5 min at room temperature, and the supernatant was discarded. Finally, the intracellular staining was performed, and the intracellular staining flow antibody Granzyme B (GZMB) was diluted with 1× membrane breaking solution, the volume was adjusted to about 100 μL per well, and the cells were incubated at room temperature in the dark for more than 30 minutes. Experimental samples were examined by flow cytometry, and experimental data were evaluated by FlowJo analysis.

[0396] To evaluate the antigen recognition epitopes of BM231-60, BM231-77 bispecific antibodies, as well as the positive control bispecific antibody BM231-99, we first constructed extracellular domain antigens of 4-1BB, including 4-1BB-CRD1-2-Fc, 4-1BB-CRD2-3-Fc, and 4-1BB-CRD3-4-Fc. ELISA was performed to differentiate the antigen recognition epitopes of BM231 bispecific antibodies, with Fc-isotype serving as the negative control antigen. As shown in Figure 9A, comparative analysis revealed that the 4-1BB antigen recognition epitopes of BM231-60, BM231-77, and the positive control bispecific antibody BM231-99 were all CRD4, while the 4-1BB antigen recognition epitope of the positive control monoclonal antibody BM180-96 (Urelumab) was CRD1.

[0400] We utilized a humanized mouse model expressing human 4-1BB extracellular domain, hu4-1BB-KI mice were subcutaneously engrafted with MC38-huHER2 tumor cells. Each group consisted of 5 mice, and they were intraperitoneally injected with the following antibodies every three days: IgG4-isotype control (10 mg / kg) , BM231-60 bispecific antibody (10 mg / kg) , BM231-77 bispecific antibody (10 mg / kg) , BM231-99 bispecific antibody (10 mg / kg) , and BM180-96 (Urelumab) antibody (10 mg / kg) . During the experiment, tumor volume was measured using calipers 2-3 times per week. As shown in Figure 10A, BM231 bispecific antibodies exhibited varying degrees of tumor inhibition activity in mice. The tumor volume for each mouse in each group is presented in Figure 10B. The tumor growth inhibition rate (TG1) was calculated based on the last three measurements as shown in Figure 10C, using the formula: 100 × (tumor volume of control group -tumor volume of treatment group)  /  (tumor volume of control group -tumor volume of control group on day 0) . Among them, BM231-60 showed the best antitumor activity with a tumor inhibition rate of 76.75%on day 14, which was 2.5 times higher than the positive control bispecific antibody BM231-99.

[0409] Example 14: In vivo dose gradient pharmacodynamic experiment of BM231-60 bispecific antibody

[0410] To evaluate the anti-tumor effectiveness of the BM231-60 bispecific antibody, hu4-1BB-KI mice were subcutaneously implanted with HER2-MC38 cells and treated with different concentrations of BM231-60 bispecific antibody. Each group comprised eight mice, receiving intraperitoneal injections of the following antibodies every three days for a total of six doses: IgG4-isotype control (12 mg / kg) , BM231-99 (12 mg / kg) , BM130-97 (Herceptin: 9.72 mg / kg) , BM231-60 (12 mg / kg) , BM231-60 (4 mg / kg) , and BM231-60 (1 mg / kg) . Tumor volume was measured twice a week throughout the experiment using calipers. As depicted in Figure 14A, BM231-60 exhibited optimal anti-tumor activity at moderate doses. The tumor growth volume data for each mouse in every group are presented in Figure 14B. Analysis of the individual tumor growth curves revealed that the BM231-60 (4 mg / kg) group demonstrated significant and consistent tumor inhibition activity across all mice. In one particular mouse, the measured tumor volume at the end of the experiment was 27 mm3, which was lower than the group average. Notably, one mouse in the BM231-60 (12 mg / kg) group displayed significantly faster tumor growth compared to others in the experiment, indicating that the medium-dose group exhibited superior and more stable anti-tumor activity than the high-dose group, which was influenced by individual differences.

[0411] Simultaneously, we computed the tumor inhibition rate based on the last three measurements, as illustrated in Figure 14C. The middle dose group (4 mg / kg) of BM231-60 demonstrated an impressive tumor inhibition rate of 80.23%against HER2-MC38 tumor growth at the conclusion of the experiment. The TGI for the BM231-60 (12 mg / kg) group was 67.63%. Combined analyses confirmed that the intermediate dose of BM231-60 had optimal antitumor activity. To further elucidate the disparities among the experimental endpoints, a scatter plot depicting the tumor volume for each group on day 21 was generated, with each point representing the tumor volume of an individual mouse, as depicted in Figure 14D. According to t-test data analysis comparing the two groups, the low, medium, and high dose groups of BM231-60 exhibited statistically significant differences from the negative control group (P = 0.0466, 0.002, 0.0116, respectively) . Additionally, a significant difference was observed between the medium dose group of BM231-60 and the high dose group of BM231-99 (P = 0.011) .

[0412] Example 15: BM231 does not mediate 4 1BB signaling activation via FcγRⅡB

[0413] For agonistic antibodies, the interaction between the Fc domain and FcγR is instrumental in modulating their agonistic activity. It has been demonstrated that FcγRⅡB plays a crucial role in the cross-linking and bridging aggregation of 4-1BB, thereby enhancing its agonistic activity. However, in the case of the 4-1BB bispecific antibody, FcγRⅡB-mediated cross-linking raises concerns about off-target activation and potential systemic safety issues. Despite opting for the IgG4 antibody subtype, which exhibits the lowest affinity for Fc receptors, in the design of the BM231 bispecific antibody molecule, it is imperative to ascertain whether FcγRⅡB-dependent activation occurs. To address this, stable FcγRⅡB-CHO-K1 cell lines were generated as target cells for T cell functional assays. The effector cells used were the 4-1BB-NF-κB‐Luc2p-Jurkat cell line (Nanjing Kebai) , and each well contained a mixture of 2.5×104 effector cells and 2.5×104 target cells. The total volume in each well was 50 μL, and three replicate wells were included for each experimental group. Subsequently, 25 μL of the gradient-diluted antibody to be tested was added to each well, achieving a final antibody concentration of 100 nM. The cells were then incubated for 5 hours at 37 ℃ in a 5 %CO2 regulated incubator. Following incubation, 75 μL of assay substrate (Promega) was added to each well of the assay plate. After a 10 minute incubation at room temperature, 100 μL from each well was mixed, transferred to a 96-well white opaque plate, and subjected to chemiluminescence measurement using a microplate reader. Finally, four-parameter logarithmic curve analysis was performed using GraphPad software.

[0419] Soluble 4-1BB (s4-1BB) , lacking the transmembrane domain and secreted by cells, exists in a soluble trimeric form. Although it possesses binding activity, it cannot activate downstream signaling of 4-1BB. Studies have shown that the expression of s4-1BB in lymphocytes requires strong activation. Therefore, we also quantitatively measured the level of s4-1BB in the serum of animals subjected to high-dose toxicity experiments, in order to evaluate the level of activation of BM231 in peripheral blood. A human s4-1BB ELISA kit was used for quantitative detection of s4-1BB in the serum samples. The experimental data, presented in Figure 16C, revealed significant differences between groups. The serum s4-1BB level of BM180-96 (Urelumab) was significantly higher than that of the experimental group. From Day 2 to Day 9, the s4-1BB level increased from 5453.48 pg / mL to 16555.5 pg / mL. On Day 16, a slight decrease in s4-1BB expression was observed, but it remained at a high level. This indicates that BM180-96 (Urelumab) induces strong activation of the immune system in peripheral blood. The s4-1BB level in BM231-60 serum samples is approximately one-eighth of that in BM180-96 (Urelumab) , significantly lower than the activation level of BM180-96 (Urelumab) . The limited expression of s4-1BB in peripheral blood by BM231-60 suggests that it does not induce strong overactivation. This further demonstrates that the systemic safety of BM231-60 is significantly superior to that of Urelumab.

[0446] As shown in Fig. 24, tumor volume of each group was showed as mean±SEM. All BM331 bispecific antibodies significantly inhibited tumor growth at the low dose of 4 mg / kg or the high dose of 12 mg / kg. Specifically, on day 26, the TGI (tumor growth inhibition) for BM331-01 at 12 mg / kg was 101.05%, which was higher than the TGI of 93.69%at 4 mg / kg (Figure 24A) . The anti-tumor activities of BM331-03 and BM331-05 at both high and low doses were similar to those of BM331-01 (Figure 24B and 24D) . Furthermore, the TGIs for BM331-04 and BM331-06 at the high dose of 12 mg / kg were 102.04%and 101.57%, respectively (Figure 24C, 24E and 24F) . These bispecific antibodies exhibited stronger inhibitory effects compared to BM180-96 (anti-4-1BB antibody, Urelumab) and BM110-98 / BM110-96 anti-PD-L1 antibody, Tecentriq / Imfinzi) even at lower dosage (Figure 24F) . The body weight of mice in all groups remained stable and showed a slight increase (Figure 24G) .

[0447] Before the experiment (0h) and 24 hours after the last administration, we collected serum from the mice of each group. The result shows that after 8 dose administrations, BM331-04 exhibited AST levels similar to its molar-equivalent parent drug Tecentriq, while BM331-06 demonstrated lower AST levels compared to Urelumab and its parent drugs Imfinzi (Figure 25) . Additionally, pathology score of the liver indicated that the BM331-04 group had less necrosis and bleeding than the Urelumab group. The BM331-06 group showed lower scores for edema, necrosis, bleeding, and inflammatory infiltration compared to the Urelumab group, indicating that both BM331-04 and BM331-06 have better safety profiles than Urelumab (Figure 25) .
