Abstract

ENGINEERED EXOSOMES FOR TREATMENT OF METABOLIC DISORDERSField of the Invention

[0001] The present disclosure relates to engineered exosomes, in particular to an exosome carrying a plurality of polypeptides anchored external to the membrane of the exosome. The present disclosure also relates to a composition comprising the engineered exosome, a nucleic acid construct comprising the polynucleotide encoding the fusion proteins and uses of the engineered exosome in preventing or treating metabolic disorders including obesity and overweight.Background

[0002] Treatment of metabolic disorders such as obesity remains elusive as the regulation of metabolism and fat storage involves complex biological feedback systems. To date, drugs effective in regulating metabolic dysfunction often exhibit several side effects, which limit their usefulness in patient populations, and in particular, in obese patient populations, who may require long term treatment. Overweight and obesity are defined as abnormal or excessive fat accumulation that presents a risk to health. Body mass index (BMI) is calculated based on body weight and height. A BMI over 25 is considered as overweight, and over 30 as obese. Both overweight and obesity are systemic diseases which are associated with the leading causes of morbidity and death in the United States, including type 2 diabetes, heart disease, stroke, and some types of cancer. Obesity is thus a serious condition because it is also associated with poorer mental health outcomes, reduced quality of life, and contributes to a decline in life expectancy.

[0003] Several medicaments have been approved for the treatment of obesity. One example is Semaglutide sold under the trade name Wegovy. Semaglutide is a glucagon-like peptide-1 (GLP-1) agonist which was originally developed and approved for the treatment of type 2 diabetes. The most noteworthy effects of GLP-1 agonists are their ability to promote insulin secretion in a glucose-dependent manner by binding to GLP-1 receptors expressed on the pancreatic beta cells. Almost as importantly, GLP-1 agonists have been shown to inhibit glucagon secretion at glucose levels above fasting levels. Critically, this does not affect the glucagon response to hypoglycemia, making GLP-1 agonists a safe anti-diabetic drug with very low incidence of hypoglycemia compared to insulin.

[0004] In June 2021, Semaglutide was approved by the FDA for chronic weight management in adults with obesity or overweight with at least one weight-related condition (such as high blood pressure, type 2 diabetes, or high cholesterol) . As an anti-obesity drug, GLP-1 agonists work by binding to GLP-1 receptors in hypothalamus thereby suppressing appetite. Furthermore, GLP-1 agonists bind to GLP-1 receptors in the stomach, inhibiting gastric emptying, acid secretion, and motility, which collectively promote satiety. Consequently, diabetic subjects treated with GLP-1 receptor agonists often also experience a beneficial weight loss in addition to controlling their blood sugar levels. However, there is a need for additional compositions and methods to treat metabolic disorders including obesity.Summary of the Invention

[0005] In one aspect of the invention, provided is an engineered exosome, comprising (a) an FGF21 polypeptide, fused to a first anchoring polypeptide, (b) an IL-27 polypeptide, fused to a second anchoring polypeptide, and (c) an activin receptor type II polypeptide, fused to a third anchoring polypeptide, wherein (a) , (b) and (c) are anchored on a membrane of the exosome via the first, second and third anchoring polypeptide, respectively, and wherein the FGF21 polypeptide, the IL-27 polypeptide and the activin receptor type II polypeptide are exposed on an outer surface of the membrane of the exosome.

[0014] Figure 1. Scheme of construction of engineered exosomes derived from stable cell lines. Mammalian cells, e.g. HEK293 or CHO-K1 cells were cultured and infected with lentiviruses, which contain functional genes FGF21, IL-27, and ActRIIB. Then antibiotics were used for stable cell line selection. After three passages selection, the expression of the functional genes was identified in both cell pellet and exosomes. After confirmation of the expression, an adaptation of stable cell line to a serum-free condition was performed. Finally, the cultured supernatant was collected from the serum-free stable cell line and the exosomes were purified via ultracentrifuging.

[0150] The stable cell line was seeded in T150 flasks for 24h, rinsed extensively with PBS and incubated in SFM for another 48h. The cultured cell-free extracellular medium containing exosomes was harvested by centrifugation at 300×g for 10 min to remove the cells. Then centrifuge at 10,000×g for 30 min to remove dead cells and cell debris. Finally, the clear supernatant was centrifuged for 70 min at 100,000×g to pellet the exosomes for twice. And the exosome pellet was resuspended. All centrifugation steps were carried out at 4 ℃. Exosomes were named Exo-No. 1. Mock exosomes were derived from unengineered HEK293 cells with the same preparation method as for Exo-NO. 1.

[0151] Analysis of the particle concentration and size distribution of exosomes

[0152] The particle concentration and size distribution of exosomes from stable cell line NO. 1 were analyzed by the NanoFCM (NanoFCM Inc., Xiamen, China) . The NanoFCM analysis used two single photon counting avalanche photodiodes (APDs) to detect individual particle side scatter (SSC) and fluorescence simultaneously. Firstly, the exosomes pellet was prepared in PBS. Then, 200nm PE and AF488 fluorophore-conjugated polystyrene beads were used for particle concentration and Silica Nanosphere Cocktail (NanoFCM Inc., Xiamen, China) for particle size distribution. The detector recorded particles passing by during a 1-min interval in each test. Each sample was diluted to reach a particle count within the optimal range of 3000-9,000 particles per minute. NanoFCM software (NanoFCM Profession V2.0) was used to convert flow rate and side scattering intensity to vesicle concentration and size.

[0166] All animal experiments were performed under protocols approved by the Institutional Animal Care and Use Committee of Novopathway Biotechnology (Shanghai, China) . The experiment was designed following animal welfare guidelines and the three Rs principles. The diet-induced obesity (DIO) mice were purchased from BioMice (Nantong, China) and housed under specific pathogen-free (SPF) conditions as animal models for fat metabolism research. All the mice had free access to water and standard laboratory chow.

[0167] For the subcutaneous fat metabolism, the mice were randomly divided into two groups: (1) Mock, treated with PBS; (2) Exo-NO. 1, treated with exosomes Exo-NO. 1. The abdomen of DIO mice was topically applied with PBS or 1E9 particles / dose in 155 μl PBS for twice a week, eight times in total. The body weight, abdominal circumference and food Intake were measured every 5 days for a month. At the end of the study, the fat content and muscle content in mice were analyzed via a small animal body composition analyzer. The blood glucose was measured via Glucometer Strip Method and the LDL-C content was detected with automatic biochemical analyzer. In addition, the mice from each group were sacrificed to obtain abdominal fat tissue, which was then fixed, dehydrated, embedded, sectioned, deparaffinized, and rehydrated for H&E staining. Afterwards, the samples were examined under a microscope, and the captured images were analyzed.

[0171] The main evaluation criterion in this human efficacy evaluation was to test the localized fat reduction effect of Exo-NO. 1 by Inger Testing Technology Service (Shanghai) Co., Ltd. In this study, the tested items included waistline, weight, BMI index, abdominal volume and body contour value. A total of 36 subjects, 18 males and 18 females were included and 36 were finalized. The age ranged from 23 to 54 years, with an average age of 40.75±7.89 years. The Exo-NO. 1 was topically applied every three days for 4 weeks. The tested items were recorded and analysis via VECTRA H2 3D Stereo image camera on Day 7 and Day 28, which can show a 360-degree presentation of the contours of various parts of the body and do a visual analysis of the condition of the bottom of the skin as well as the decreased / increased volume.

or(iii) reduction of abdominal volume or waist circumference.
