Description

[0001] COMPOSITION FOR TOPICAL TREATMENT OF EPITHELIAL TISSUE

[0002] The present invention relates to a composition for the treatment of human epithelial tissue comprising at least one organic acid with at least two functional groups, wherein at least one functional group is a carboxylic acid, and a water-soluble inorganic polyvalent salt, and its use.

[0003] The term "epithelial tissue" refers to layers of so-called epithelial cells that are closely connected. It is one of the four basic tissue types – the other three being muscle, nerve, and connective tissue. Epithelial tissue has several characteristic properties: for example, epithelia do not contain blood vessels. The cells of the epithelial tissue are separated from the connective tissue by the basement membrane, through which they are supplied with nutrients.

[0004] Epithelial tissue covers internal and external body surfaces - for example, the skin, the largest organ of the human body, consists of epithelial tissue.

[0005] Among its many functions (for example, thermoregulation and as a sensory organ), the barrier and protective function, which prevents dehydration (and thus ultimately the entire organism), is arguably the most important task of the skin. At the same time, the skin acts as a barrier against the penetration and absorption of external substances. Particularly noteworthy here are harmful UV rays, microbiological attacks, and the penetration of foreign pollutants. This barrier function is achieved by the epidermis (outer layer), which, as the outermost layer, forms the actual protective shield against the environment.

[0006] Special attention is paid to the outer stratum corneum (stratum corneum), a resistant cell layer whose individual corneocytes are connected via corneodesmosomes involving cadherins and calcium ions (Ca). 2+ The corneal cells are connected to each other by )-dependent transmembrane glycoproteins from the group of adhesion proteins. A lipid layer is located between and on the corneal cells, which provides physical and chemical protection and supports the cohesion of the corneal stratum corneum.

[0007] The natural pH of the skin surface is typically between 4 and 7. At about one-tenth of the total skin thickness, the epidermis is also the thinnest layer. The melanocytes (pigment cells) in the basal cell layer produce the pigments that give the skin its color. This layer includes not only the epidermis but also the underlying mesodermal connective tissue of the dermis (corium). Both layers are intimately interlocked by a system of papilla- or ridge-shaped dermal papillae and the epidermal papillae that project deeply between them. The richly vascularized dermis also supplies the epidermis with nutrients. The dermal papillae contain the sensory receptors for touch, pain, and temperature sensation.

[0008] The superficial cell layers of the epidermis are subject to constant wear and tear. In addition, the epidermis can be very sensitive to external influences, reacting with irritation, redness, itching, flaking, and cracking. Furthermore, it is constantly exposed to the risk of inflammation and injury (wounds). Therefore, to ensure optimal protection, the epidermis completely renews itself within approximately 30 days. This epidermal renewal occurs from a basal, lifelong, actively dividing germ layer through continuous replacement.

[0009] Cosmetic and dermatological preparations for skin care are well-known. Such compositions are based on O / W or W / O emulsions, which, for example, are based on a mixture of long-chain fatty acids, mono- and / or diglycerides of fatty acids, ethoxylated fatty acid stems, nonpolar lipids, fatty alcohols, lipophilic consistency enhancers, hydrogenated polyisobutenes, and polar lipids.

[0010] To improve the skin barrier of the epidermis, numerous cosmetic formulations have been offered for decades, typically delivering lipids to the skin from the outside. However, such formulations have several disadvantages. Penetration into the skin is often rather poor, which can leave the skin feeling unpleasantly greasy. Due to this extremely limited penetration, the lipids only reach the outer layers of the stratum corneum, which comprises 12-20 layers of dead skin cells. Once the applied lipids are washed away by contact with water and surfactants, their effect largely disappears. The lipids are also removed through constant friction from textiles. Furthermore, with increasing age, the activity of keratinocytes decreases, meaning that cell division and skin regeneration, and thus the formation of the skin's own barrier, decline.

[0011] This creates a need for improved pharmaceutical and cosmetic treatment methods, particularly a need for formulations that strengthen the skin's protective and barrier function while simultaneously improving skin feel and appearance, with no or only negligible side effects. There is a particular need for formulations that penetrate deeper into the stratum corneum and strengthen the cohesion of the corneocytes within it, thereby supporting the protective function of the epidermis and compensating for skin impairments caused by pre-existing conditions such as poorly healing wounds, irritations, inflammations, and others.

[0012] Based on this, the object of the present invention was to provide a well-tolerated composition for the treatment of the skin, in particular for strengthening the protective and barrier function of the skin. Furthermore, this composition should be suitable for topical application and overcome the disadvantages of compositions known from the prior art.

[0013] In particular, the object of the present invention was to provide an effective and well-tolerated composition that supports the regeneration of the epidermis and positively influences wound healing. At the same time, the composition should be easy to process.

[0014] This problem was surprisingly solved by the composition according to the invention and its use.

[0167]

[0168] Study 1

[0169] In a clinical study, the improvement of the skin's barrier properties through the use of formulations according to the invention was tested in comparison to comparator formulations. The following skin creams were used, wherein formulations 1 and 2 correspond to the examples 6J and 6E mentioned above, respectively:

[0170] In 23 study participants, the formulations were randomly applied to the AD areas of the forearms shown in Fig. 1 and applied twice daily for 2 weeks.

[0171] The barrier function of the skin was characterized by the penetration of a model substance. The concentration of the substance at a penetration depth of 40–60 pm was determined using confocal Raman spectroscopy. Caffeine, a 5% solution in water / propylene glycol at pH 6–7, was used as the model substance. A defined amount of the solution was applied to the test areas by a technician using a standardized method. Before measurement with the Raman spectroscope (highly sensitive confocal Raman system gen2-SCA®, RiverD International BV, Rotterdam, The Netherlands), the solution was left on the test areas for two hours to dry. At D14, the formulations were applied at least one hour before the model substance was applied.

[0172] The improvement in the skin's barrier properties is shown by comparing the penetration profiles at DO vs. D14, i.e., before treatment (DO) vs. after 2 weeks of treatment (D14). Figure 2 shows the percentage change in caffeine penetration between DO and D14.

[0173] Result:

[0174] By using the formulations according to the invention, the skin penetration of the model substance can be significantly reduced (see Fig. 2).

[0175] Study 2

[0176] In a further clinical study, the skin compatibility and cosmetic efficacy of a formulation according to the invention, which was applied to the face, were evaluated.

[0177] 49 subjects with dry to very dry skin were randomly assigned to two groups. One group applied formulation 2 from study 1 at least once daily for 28 days, while the other group applied comparator formulation 1 from study 1 for the same period (see table in study 1 for formulations).

[0178] Facial skin was examined and assessed by a dermatologist before (DO) application of the product and after (D29) application. In addition, the subjects rated itching and tightness as subjective parameters. Figure 3 shows the average change in itching and tightness from DO to D29.

[0179] Result:

[0180] After 28 days of using the formulation according to the invention, both itching and the feeling of tightness improved more than after using the comparison formulation (see Fig. 3).
