Abstract
Description
Technical Field
[0001] The present invention relates to the technical field of traditional Chinese medicines, and in particular to a traditional Chinese medicine composition for treating closed soft tissue injuries, and a preparation method and application thereof. Background Art
[0002] Soft tissue injury refers to the injury of human skin, subcutaneous tissue, fascia, muscle, blood vessel, nerve, ligament, joint capsule, synovial capsule and other parts, without fracture or dislocation, with local skin ecchymosis, swelling, pain and limb movement disorder as the main clinical manifestations, and is divided into acute and chronic injuries. Acute soft tissue injury is the most common disease in orthopedics, mostly caused by improper exercise, falls, sprains, and beatings, and often with joint or local soft tissue swelling and pain, and limited movement as the main symptoms. With people's activities becoming more frequent and the pace of life constantly accelerating, the incidence of acute soft tissue injury has gradually increased in recent years, seriously affecting the quality of life of more than 20% of adults and more than 50% of the elderly in my country. Chronic soft tissue injury is mostly caused by the failure to treat, misdiagnose, improper treatment of acute soft tissue injury, or chronic strain, and is often caused by long-term and repeated labor, and recovery is relatively difficult. It includes many diseases such as chronic neck, shoulder, waist and leg pain, stenosing tenosynovitis, tendinitis, and medial and lateral epicondylitis of the humerus. After the injury, the patient will experience varying degrees of local pain, swelling, or pain radiating to distant areas, or dysfunction of nearby joints, which seriously affects the patient's work and life. Compared with acute injuries, chronic injuries are more serious and more difficult to cure, such as neck and shoulder pain, back pain, etc., which do not heal for a long time, and some can last for decades or even a lifetime, causing unbearable pain. Clinically, this kind of chronic pain is very common, difficult to treat, and has great harm.
[0003] One of the methods commonly used by Western medicine to treat soft tissue injuries is drug blockade. Although local injection of anesthesia and anti-inflammatory drugs in patients has short-term efficacy, it is only symptomatic treatment. Moreover, the treatment itself is somewhat damaging and needs to be carried out in hospitals or clinics with medical care conditions, and patient compliance is poor. Traditional Chinese medicine believes that soft tissue injuries belong to the category of "tendon injuries". The syndrome is qi stagnation and blood stasis, and the treatment is suitable for promoting blood circulation and removing blood stasis, reducing swelling and relieving pain. Treatment based on syndrome differentiation has achieved good clinical results. From the current status of clinical research, Chinese medicine has not only good efficacy in treating soft tissue injuries, but also a variety of treatment methods. In terms of drug treatment, traditional therapeutic drugs are mainly external tinctures, ointments, liniments, powders and various oral dosage forms, with massage or Tuina, application, fumigation, wet compress, rubbing, hot ironing, etc., to achieve good clinical results. The high incidence of traumatic injuries in orthopedics and the characteristics of patients taking medicines on their own determine the huge potential market for Chinese herbal medicines for external use. In addition, some existing commonly used Chinese medicines have definite efficacy and a broad clinical application basis, which also provides a rare opportunity for the development of such drugs.
[0004] Therefore, finding new Chinese herbal preparations for external use in the treatment of soft tissue injuries is a current research focus. Summary of the invention
[0005] In order to solve the above technical problems existing in the prior art, the present invention provides a Chinese medicine composition for treating closed soft tissue injury and its preparation method and application. Specifically, it is achieved through the following technical solutions:
[0006] A traditional Chinese medicine composition for treating closed soft tissue injury comprises the following raw materials: angelica sinensis, chuanxiong rhizome, notopterygium wilfordii, safflower, zanthoxylum bungeanum, dried ginger and moxa slices.
[0007] Further, the weight proportions of the raw materials are: 5-30 parts of angelica, 2-20 parts of chuanxiong, 1-20 parts of notopterygium wilfordii, 1-20 parts of safflower, 2-20 parts of pepper, 2-25 parts of dried ginger, and 1-15 parts of mugwort slices. Preferably, 8 parts of angelica, 7 parts of chuanxiong, 5 parts of notopterygium wilfordii, 4 parts of safflower, 4 parts of pepper, 3 parts of dried ginger, and 2 parts of mugwort slices.
[0008] The preparation method of the Chinese medicine composition comprises the following steps:
[0009] Grind angelica, chuanxiong, notopterygium wilfordii, safflower, prickly ash, and dried ginger into coarse powder in proportion, perform reflux extraction, add moxa tablets to dissolve, filter, and keep the filtrate for later use.
[0010] Furthermore, the reflux extraction is performed by reflux extraction with ethanol.
[0011] Furthermore, the ethanol is 60-80% ethanol.
[0012] Furthermore, the reflux extraction is specifically performed by heating and reflux extraction with 70% ethanol twice, each time for 1 hour, and combining the extracts.
[0013] The Chinese medicine composition is used in preparing medicines for treating acute and chronic closed soft tissue injuries and medicines for promoting blood circulation and removing blood stasis, reducing swelling and relieving pain, and relaxing muscles and tendons.
[0014] An external preparation is prepared using the traditional Chinese medicine composition.
[0015] Soft tissue injury belongs to the category of "tendon injury" in traditional Chinese medicine. Its pathogenesis is caused by various injury factors acting on the body, causing muscle injury, and the veins are damaged. The blood does not flow along the normal path, overflows outside the veins, and stagnates locally. Blood stasis will inevitably affect the operation of the qi, thus leading to qi stagnation. Therefore, the treatment is mainly to promote blood circulation and remove blood stasis, and promote qi and relieve pain. This prescription uses Angelica sinensis and Chuanxiong as the main ingredients. Angelica sinensis replenishes blood and promotes blood circulation, dispels wind and dredges the collaterals, and Chuanxiong promotes blood circulation and qi, removes dampness and removes wind and relieves pain. It is a blood-qi medicine, pungent and warm, and moves without staying as the main medicine. It is supplemented by Qianghuo, which dispels wind and dampness, promotes blood circulation and dredges the meridians, and disperses blood stasis and relieves pain. Safflower strengthens the dispelling of wind and pain, promoting blood circulation and dredges the collaterals as the minister medicine, and pepper and dried ginger promote qi and relieve pain, and promote blood circulation and remove blood stasis as the adjuvant medicine, which helps the monarch and ministers to achieve the effect of dispelling wind and dredges, promoting qi and relieving pain. The wormwood tablets are pungent and slightly cold, with aromatic and moving properties, can open the obstruction of the meridians, lead all the medicines directly to the diseased area, and are used as a messenger. All the medicines are used together to play the functions of promoting blood circulation and removing blood stasis, reducing swelling and relieving pain, and relaxing muscles and unblocking meridians. The Chinese medicine complex of the present invention is prepared into an external preparation by a conventional preparation method, and an animal model of acute closed soft tissue injury is established by using an external force direct impact method for therapeutic effect research. It is found that the Chinese medicine composition can significantly reduce tissue edema and congestion after acute soft tissue injury in rats, and relieve tissue dysfunction after injury, and can be used to prepare drugs for treating acute and chronic closed soft tissue injuries.
[0016] Compared with the prior art, the technical effects created by the present invention are embodied in:
[0017] This prescription uses Chinese angelica and Chuanxiong as the main medicine. Chinese angelica nourishes blood and promotes blood circulation, dispels wind and dredges the collaterals. Chuanxiong promotes blood circulation and qi, removes dampness and removes wind and relieves pain. It is a blood-qi medicine. It is pungent and warm, and it is a main medicine that moves but does not stay. It is supplemented with Qianghuo, which dispels wind and dampness, promotes blood circulation and dredges the meridians, and relieves blood stasis and relieves pain. Safflower strengthens the dispelling of wind and pain, and promotes blood circulation and dredges the collaterals as a ministerial medicine. Zanthoxylum bungeanum and dried ginger promote qi and relieve pain, and promote blood circulation and remove blood stasis as adjuvants, which help the main and ministerial medicines to achieve the effects of dispelling wind and dredges the collaterals, and promoting qi and relieving pain. Artemisia argyi tablets are pungent and slightly cold in nature, with a fragrant and roving nature. They can open the obstruction of the meridians and lead all the medicines to the diseased area directly, and are used as messengers. All the medicines are used together to play the functions of promoting blood circulation and removing blood stasis, reducing swelling and relieving pain, and relaxing muscles and dredges. It can reduce tissue edema and blood stasis after acute soft tissue injury, and relieve tissue dysfunction after injury. Reduce muscle cell swelling and damage. Its mechanism may be related to effectively inhibiting the increase of IL-1β in rats after closed soft tissue injury, reducing the content of TNF-α and MDA in serum and tissue, and alleviating the oxidative stress response caused by oxygen free radicals in soft tissue after injury. This formula uses local Chinese herbal medicines in our province as raw materials, and has a wide source of crude drugs, which is conducive to the development and utilization of Chinese herbal medicine resources. It will promote and promote the utilization of the advantages of natural drug resources in our province, develop high-tech and high-value-added products, and have good social and economic benefits for the in-depth development of Chinese herbal medicines in our province. [0049] 2. Experimental Methods
[0050] 2.1 Establishment of animal model
[0051] 24h before the experiment, the rats were depilated with depilatory cream on both sides, and the depilatory cream was washed with saline. On the day of modeling, 3% sodium pentobarbital was injected intraperitoneally for anesthesia, with a dose of 1mL / Kg body weight, and then the rats were fixed on a homemade striker, and a hard plastic tube with a cross-sectional diameter of 45mm and a length of 400mm was positioned between the knee joint and hip joint of the left hind limb of the rat, and the position of the thick outer thigh muscle was kept vertical. Taking the upper edge of the plastic tube as the starting point (vertical distance 400mm), let the balance weight of 200g fall vertically along the lumen of the plastic tube to hit the same part, and hit 4 times in a row, with a striking area of about 1.0 square centimeters, to establish a non-open soft tissue injury model, and exclude animals with skin damage or fractures. The control group was also depilated, but no modeling was performed.
[0052] 2.2 Grouping and Dosing
[0053] 150 SD rats, half male and half female, were randomly selected to establish the model after modeling, and the remaining 144 were randomly divided into 6 groups according to the number table method: control group, model group, positive drug group (Yunnan Baiyao aerosol), high-dose (0.6g / kg), medium-dose (0.3g / kg) and low-dose (0.15g / kg) groups of Chinese medicine combination, with 24 rats in each group. Except for the control group, non-open soft tissue injuries were caused by weight impact in all groups, and corresponding drug treatment was given 30 minutes after injury. The control group was given 70% ethanol, and the model group was not given drug treatment after modeling.
[0054] 2.3 Specimen collection and testing
[0055] (1) Injury symptom index. The degree of congestion, swelling, and activity of the modeling limbs of the rats were observed and graded on the 1st, 3rd, and 7th days after administration. (1) Congestion degree: dark purple was scored as 2 points, dark red was scored as 1 point, and normal color was scored as 0 points; (2) Swelling degree: obvious swelling was scored as 2 points, slight swelling was scored as 1 point, and no swelling was scored as 0 points; (3) Activity degree: inability to move the modeling limb was scored as 2 points, poor mobility of the modeling limb was scored as 1 point, and normal mobility of the modeling limb was scored as 0 points.
[0056] (2) Collection of rat serum. Eight rats were randomly selected from each group 1 day (4 doses), 3 days, and 7 days after modeling. They were first anesthetized with 3% sodium pentobarbital injected intraperitoneally at a dose of 1 mL / kg body weight. After scoring the rat injury symptom index, blood was collected from the abdominal aorta (7-8 mL) and placed in a centrifuge tube at an ambient temperature of 18-20°C. After standing for 1.5 hours, the blood was centrifuged (3000 rpm, 20 min) to collect serum and store it at -80°C. The levels of IL-β and TNF-α were measured within 1 week. The animals were fasted but not watered for 12 hours before sampling.
[0057] (3) Sampling of muscle tissue at the injured site. After the animal dies, 300 mg of muscle tissue at the injured site is immediately removed by dissection and divided into three portions. Each portion, about 100 mg, is immediately frozen at -80°C for detection of superoxide dismutase (SOD) activity and malondialdehyde (MDA) content in the tissue.
[0058] 3. Statistical analysis
[0059] SPSS24.0 was used for data statistics. Statistical methods: For the experimental indicators of dynamic detection, the normality test of each group of data was first performed. If they were in accordance with normal distribution, repeated measures variance analysis (2wayANOVA) was used for statistical analysis. Bonferroni's multiple comparisons test was used for pairwise comparisons between groups. Graphpad Prism software was used for drawing.
[0060] 4. Experimental Results
[0061] 4.1 Comparison of injury symptom index among rat groups
[0062] After modeling, the injured limbs of rats rapidly developed redness, swelling, subcutaneous ecchymosis, and functional impairment, and the injury continued to worsen. The swelling reached the peak 24 hours after injury, and then gradually subsided. By the 7th day, the swelling had basically subsided, and only purple skin ecchymosis remained locally or distal to the injury. Compared with the normal control group, the injured part of the model group rats 1 day, 3 days and 7 days after modeling was dark and purple-red, the muscles were swollen, there was subcutaneous congestion, and the injury symptom index was significantly increased (p < 0.01). After 1 day of treatment, the muscles of the injured part of the rats in the medium-dose group of the Chinese medicine composition were dark and light red, the muscles were slightly swollen, there was a small amount of subcutaneous congestion, and the soft tissue injury symptom index was lower than that of the model group, with statistical differences (p < 0.01), and the injury symptom index of the medium-dose group was lower than that of the positive group (p < 0.05). After 3 days of treatment, subcutaneous congestion and swelling in each treatment group were alleviated. The injury symptom index of the high and medium doses of the Chinese medicine combination was significantly lower than that of the model group (p < 0.01), and was lower than that of the positive drug group (p < 0.01). After 7 days of treatment, the injury symptom index of the high-dose group was significantly lower than that of the model group and the positive drug group (p < 0.01), and the low-dose group was significantly lower than the model group (p < 0.05), as shown in Table 1. Figure 1 , Figure 2 .

[0066] 4.2 Histopathological observation of injury site
[0067] The muscle tissue of the rat hind thigh was taken, about 1cm×2cm×0.5cm, embedded in paraffin, sliced and stained with HE to observe pathological changes such as tissue congestion, muscle cell edema, and necrosis. The results showed that there was no congestion around the muscle tissue of the control group, no inflammatory cell aggregation, the muscle fibers were arranged neatly, there was no obvious inflammatory cell infiltration in the fiber gap, the muscle cell structure was intact, and there was no swelling. Figure 3 A. In the model group, there were large areas of congestion and a large number of inflammatory cells around the injured tissue, a large number of inflammatory cells infiltrated in the gaps between muscle fibers, muscle cells swelled and degenerated, and cracks appeared in the cells. Figure 3B. In the positive drug group, a large number of inflammatory cells gathered around the damaged tissue, and inflammatory cells infiltrated significantly in the gaps between muscle fibers. Some muscle cells were swollen. Figure 3 C. In the high-dose group, no obvious congestion and inflammatory cells were observed around the muscle tissue, inflammatory cells were seen in the gaps between muscle fibers, some muscle cells were swollen, and no obvious cell necrosis was observed. Figure 3 D. In the medium-dose group, no large areas of congestion and inflammatory cells were observed around the injured tissue, but inflammatory cells infiltrated the gaps between muscle fibers, and some muscle cells showed swelling and damage. [0068] 4.3 Dynamic detection of IL-1β in serum
[0069] The results showed that the IL-1β content in the serum of the model group had a gradual increase trend after soft tissue injury, while there was no increase trend in the control group, positive drug group and each dose group of the Chinese medicine composition. On the 7th day after injury, the serum IL-1β content of the model group was significantly increased compared with the control group, p < 0.05. Compared with the model group, the serum IL-1β content of the Chinese medicine composition medium and low dose groups was significantly reduced, p < 0.05, see Table 2, Figure 4 .
[0070] The results showed that the inflammatory factor IL-1β in the serum of rats showed a gradual increasing trend after non-open soft tissue injury, while the medium and low doses of the Chinese medicine composition could inhibit the increase of IL-1β.

[0089] 5. Results
[0090] The experimental results showed that after the closed soft tissue injury model was established by heavy blow, the rats' injured limbs quickly developed redness, swelling, subcutaneous ecchymosis, and functional impairment, and the injury continued to worsen, reaching the peak of swelling 24 hours after injury, and then gradually subsided. The results of dynamic observation of the injury symptom index score showed that compared with the normal control group, the injured part of the model group rats was dark and purple, the muscles were swollen, there was subcutaneous congestion, and the activity of the affected limb was significantly limited, and the score was significantly higher than that of the control group 1 day, 3 days, and 7 days after modeling. One day after injury, compared with the model, the soft tissue injury symptom index of the medium-dose group of the Chinese medicine composition was significantly lower than that of the model group and the positive group (all p < 0.01); after 3 days of treatment, subcutaneous congestion and swelling of each treatment group were alleviated, and the injury symptom index of the high- and medium-dose Chinese medicine composition was lower than that of the model group and the positive drug group, with significant differences (all p < 0.01); after 7 days of treatment, the high-dose group was significantly lower than the model group and the positive drug group (all p < 0.01), and the low-dose group was significantly lower than the model group (p < 0.05). This suggests that the Chinese medicine composition can reduce tissue edema and congestion after acute soft tissue injury and relieve tissue dysfunction after injury.
[0091] Local swelling and bruising are the main manifestations of acute soft tissue injury. The main causes of injury are edema, deformation and necrosis caused by rupture of small blood vessels, leakage of red blood cells, infiltration of inflammatory cells and local muscle fiber damage at the site of injury. The results of pathological histological observation showed typical pathological changes such as local tissue congestion, inflammatory cell aggregation, inflammatory cell infiltration in the intermuscular space, muscle fiber rupture and muscle cell swelling in the model group. After 3 days of treatment, the aggregation and infiltration of inflammatory cells at the site of injury in each treatment group decreased, and the swelling and injury of muscle cells in the high, medium and low dose groups of the Chinese medicine composition were significantly alleviated.
[0092] The pathological basis of acute soft tissue injury is the damage of muscles and micro-vessels at the site of injury, the damage of local capillaries, the increase of vascular wall permeability, and the extravasation of a large amount of intravascular fluid, which eventually leads to the swelling of the tissue at the site of injury. In addition, with the congestion and necrosis of the tissue, many inflammatory factors in the human body are activated, such as IL-1β, TNF-α, IL-6, etc., which trigger an inflammatory response at the site of injury and produce a large number of inflammatory factors, further aggravating the degree of tissue damage, causing inflammatory reactions such as fever, swelling and pain. The results of this experiment showed that the inflammatory factors IL-1β and TNF-α in the serum of rats showed a gradual increase trend within 7 days after closed soft tissue injury, especially on the 7th day after injury, the levels of IL-1β and TNF-α in the serum of the model group were significantly higher than those of the control group, indicating that IL-1β and TNF-α are important inflammatory mediators in the inflammatory process after soft tissue injury. At the same time, on the 7th day after injury, the IL-1β content in the serum of the medium and low-dose groups of the Chinese medicine composition was significantly lower than that of the model group, suggesting that the drug can effectively inhibit the increase of IL-1β in rats after non-open soft tissue injury, which may be one of its mechanisms for inhibiting post-injury inflammatory response.
[0093] Under the pathological state of acute soft tissue injury, the generation of free radicals in the injured tissue increases, which can cause oxidative stress reaction, trigger lipid peroxidation of cell membrane, and further aggravate the injury. MDA is one of the products of oxygen free radicals attacking cell membranes and causing lipid peroxidation. The amount of MDA reflects the level of free radical generation in tissues and the degree of membrane lipid peroxidation. SOD is an important antioxidant enzyme in the body and the primary substance for the body to scavenge free radicals. SOD activity reflects the body's ability to resist oxidative damage. SOD in muscle tissue is one of the important antioxidant enzymes of myogenic cells. The experimental results showed that the MDA content in the injured tissue of the closed soft tissue injury rat model group increased while the SOD activity decreased, indicating that there was significant oxidative stress damage in the muscles and soft tissues after injury. The results of the MDA detection experiment showed that after 1 day of treatment, the MDA content in the injured tissue of the medium-dose group was significantly lower than that of the model group. After 7 days of treatment, the MDA content in the medium-dose group and the positive drug group was significantly lower than that of the model group, indicating that the composition may alleviate tissue damage by reducing the oxidative stress reaction caused by oxygen free radicals in the soft tissue after injury.
[0094] 6 Conclusion: The Chinese medicine composition composed of Angelica sinensis, Chuanxiong, Notopterygium wilfordii, Carthamus tinctorius, Zanthoxylum bungeanum, Dry ginger, and Artemisia selengensis can reduce tissue edema and congestion after acute soft tissue injury, and relieve tissue dysfunction after injury. It can also reduce muscle cell swelling and damage. Its mechanism may be related to effectively inhibiting the increase of IL-1β in rats after closed soft tissue injury, reducing the levels of TNF-α and MDA in serum and tissues, and reducing the oxidative stress response caused by oxygen free radicals in soft tissue after injury.
