Abstract
Description
A TIBASE FOR A MULTI UNIT ABUTMENTTECHNICAL FIELD
[0001] The present disclosure relates to the field of dental abutments in general, and to TiBase to be used with a dental abutment, in particular.BACKGROUND
[0002] Modern dentistry methods for mouth rehabilitation comprise the usage of multiunit abutments. Common practices, such as but not limited to the All-on-4® or All-on- X® concepts comprise a plurality of dental implants inserted into to patient’s jaw. A denture designed to carry artificial teeth is then placed over the jaw, and connected to the jaw via the implants using TiBase elements inserted into the denture. Connecting elements such as screws are then screwed from the distal part of the denture, through the denture into the TiBase elements and then screwed into an abutment which is connected on top of the implant, or directly to the implant.
[0003] In order to determine the locations for inserting the implants into the jaw, design the denture and determine the locations of the TiBase elements therein, the patient’s mouth needs to be measured. Known technologies including molding producing a physical model, or digital scans. Molds tend to shrink, and although the shrinking is rather small, for example up to 100 micron, it may lead to inaccurate design and locations. Digital scan is not error free either, especially in mouths where multiple teeth are missing, as is often the case when multi-unit abutments are to be used.
[0004] The inaccuracies cause pressure to be applied on the implants when the denture is installed, which may result in inflammations, breaking of the denture, implant failure, or other problems such as. Some methods have been suggested, such as reinforcing the dentures, which resulted in the pressure being applied to the implants and breakage thereof.BRIEF SUMMARY

[0018] One technical problem handled by the disclosure relates to overcoming the inaccuracies in designing dentures and determining the locations and positions of the implants and TiBase elements, which may be caused by measurement inaccuracies. The inaccuracies occur in any known measurement method, including molds where models may shrink, and digital scans which suffer from artifacts and built-in inaccuracies, especially when teeth are missing, as is often the case.
[0019] The inaccuracies cause elements that have to be connected by a connecting element such as a screw, for example the abutments inserted into implants and the TiBase elements inserted into the denture, to be misaligned. As a result of the misalignment, the screws apply pressure at the dentures and cause distortions or breakage.
[0020] Some methods for correcting the pressure applied at the wrong location or vector have been tried, such as adding reinforcement elements into the denture. However, such elements merely shift the pressure to another location or element, such as the abutment or the implant, and cause even more severe damage.
[0021] Thus, it is required to provide a TiBase or a dental set that will overcome the measurement inaccuracies, and provide for installing a denture without creating undesired pressures at various elements.
[0022] One technical solution of the disclosure is a TiBase that comprises two units: an internal unit and an external unit (collectively referred to as "units"), each having a cavity. The cavity of the internal unit covers the distal part of the abutment, and the external unit is inserted into the denture such that its external face is in contact with the denture and its cavity receives the internal unit.
[0023] The units are designed such that they can move relative to one another, and assume a plurality of relative stable positions. Thus, a screw inserted through the external unit and through the internal unit into the abutment, has a degree of freedom and is not forced to a position in which it applies pressure at the denture or at the abutment in an incorrect direction.
[0024] The relative motion between the units is enabled by their design, which leaves a gap between the external face of the internal unit and the internal face of the external unit, wherein the shape of the gap depends on the relative position of the units.
[0025] However, in some embodiments, in order for the gap not to be vulnerable to accumulation of germs or other undesired objects which may cause infections, the proximal areas of the internal unit and the external unit have corresponding curved contours, which in all relative positions assume gapless contact, thereby sealing the gap. In some exemplary embodiments, the curved proximal parts may be shaped as parts of spheres which fit exactly one inside the other, such that they can move relative to each other, for example by “rolling” while maintaining tight contact.
[0026] Thus, the two-unit TiBase structure provides for a self-adjusting passive fit of the structure when screwing and coupling the denture to the patient’s jaw.
[0027] The TiBase may be made of Titanium or any other bio-compatible material.
[0028] Another technical solution of the disclosure is a self-adjusting washer, e.g. a washer having walls which may be straight or curved, which may be inserted into the distal opening of the external unit, and into which the screw is inserted. The self- adjusting washer enables the screw to assume a tilted position, if necessary due to the relative locations of the external part and the abutment, but reduce the application of uneven pressure to certain areas of the perimeter of the external unit than to others. Rather the self-adjusting washer distributes the pressure more evenly over the perimeter of the external unit.
[0029] Another advantage of the self-adjusting washer is that it eliminates friction between the screw and the washer caused by screwing the screw, rather the screwing is done against the self-adjusting washer.
[0030] The self-adjusting washer also enables to use a screw with a smaller head, thus applying less moment on the external unit than a screw with a bigger head would.
[0031] One technical effect of the disclosure is a two-unit TiBase structure, enabling compensation for inaccuracies in the locations of the abutments inserted into the implants within a jaw, a design of a denture, and the locations of the TiBase elements within the denture. The compensation allows for a degree of freedom for a screw, such that the screw does not apply pressure in an eccentric manner on the abutment, nor pressure in a tilted direction on the denture.
[0032] Another technical effect of the disclosure is the provisioning for an additional degree of freedom wherein if the screw is tilted, the pressure is still applied in a to the external unit in a more concentric manner, due the self-adjusting washer which distributes the pressure more evenly.
[0033] Yet another technical effect of the disclosure is its applicability of the disclosure to any type of abutment, including straight or angulated abutment, conical or non- conical abutment, or the like, and regardless of the material of which the abutment or the TiBase are made.
[0034] Yet another technical effect of the disclosure is its applicability to any type of denture, created in any manner, such as manual, by milling for example using a Computer Numerical Control (CNC) machine, by 3 -dimensional printing, or in any other manner.
[0035] Referring now to Figs. 1A and IB, showing the components of a typical settings, where some exemplary embodiments of the disclosure can be used.
[0036] Fig. 1A shows an isometric view of an upper jaw denture 104, carrying artificial teeth 108, and having inserted thereto four units 112 which may constitute TiBase units or parts thereof in accordance with some embodiments of the disclosure.
[0037] The denture may be produced manually, by milling for example using a CNC machine, by molding, 3D printing, or in any other manner.
[0038] In some embodiments, denture 104 may be partial, for example comprising two or three TiBase elements 112, and not necessarily a full jaw denture as shown. In further embodiments, denture 104 may comprise more than four TiBase elements 112, such as five, six, or the like.
[0039] Fig. IB shows a front view of an upper jaw 116 having inserted thereto four implants (not shown) and four abutments 120 corresponding to TiBase elements 112, in accordance with some embodiments of the disclosure.
[0040] Fig. IB also shows a front view of denture 104 with artificial teeth 108, and TiBase elements 112.
[0041] During insertion, a connecting element such as a screw is inserted through each TiBase 112 and through abutment 120, and screwed therein, using an external thread of the screw and a corresponding internal thread of the abutment.
[0042] Referring now to Figs. 2A, showing a front view of the elements in a denture implant, and to Fig. 2B, showing an A-A cross section of the elements.
[0043] Fig. 2A and Fig. 2B show the following elements, from distal to proximal:
[0044] Screw 204 having a substantially straight part 206 and an external thread 208;
[0045] TiBase element 212 comprising a middle area 213 and a proximal area 215; and
[0046] Abutment 216, having a distal part 218, an internal thread 220 adapted to receive screw 204 , and an external thread 222 for inserting the abutment into the patient’s jaw. Abutment 216 can be of any shape, such as but not limited to a conical shape.
[0047] During insertion, abutment 216 is inserted into an implant that has been implanted in the patient’s jaw. The insertion may be performed by screwing external thread 222 into the implant, optionally into a corresponding internal thread of the implant. TiBase 212 may then be positioned on top of abutment 216, such that middle area 213 and proximal area 215 of TiBase 212 cover distal part 218 of abutment 216. Screw 204 is then inserted from the denture (not shown) through TiBase element 212 into abutment 216, such that external thread 208 is screwed into internal thread 220 of abutment 216, thereby coupling the denture to the user’s gums.
[0048] Referring now to Figs. 3A, showing a front view of the elements of a tooth implant, and to Fig. 3B, showing an A-A cross section of the elements, in accordance with some exemplary embodiments of the disclosure.
[0049] The embodiments comprise screw 204 having straight part 206 and external thread 208, and abutment 216 having distal part 218, also referred to as abutment head, internal thread 220 and external thread 222 substantially as in Figs. 2A and 2B above.
[0050] In some embodiments, TiBase element 212 of Figs. 2A and 2B (corresponding to TiBase 112 of Fig. 1) is replaced by two units, external unit 308 and internal unit 312. Internal unit 312 has a proximal rim 328, and an area 324 immediately adjacent to proximal rim 328, area 324 having a curved external contour. External unit 308 has a proximal rim 310, and immediately adjacent thereto an area 316 having a curved internal contour. Areas 316 and 324 are designed such that area 324 fits neatly within the space formed by area 316. In some exemplary embodiments, area 316 and area 324 are both parts of spheres, wherein the different in the radii of the spheres is exactly the thickness of area 316, such that internal unit 312 can be inserted into external unit 308in practically endless number of positions while maintaining gapless contact. Distally from areas 316 and 324, various relative positions between internal unit 312 and external unit 308 are enabled by a gap formed distally from areas 316 and 324, as detailed below. Area 213 and area 215 of external unit may constitute a receiving pocket (or part thereof) for receiving internal unit 312.
[0051] Internal unit 312 may further comprise protrusion 332 which may fit inside space 313 of external unit 308, thereby ensuring that once internal unit 312 is inserted into external unit 308, the units do not split back. In some embodiments, protrusion 332 may be made with some springiness or flexibility, such that it can slip into space 313.
[0052] Internal unit 312 can be of a shape corresponding to the abutment that is being used, whether the top part of the abutment is conical, straight, or of any other design. In further embodiments, if a tissue level implant is used, internal unit 312 may be positioned directly thereon.
[0053] Referring now to Figs. 4A and 4B, showing respectively a front view of internal unit 312 of a TiBase as inserted into external unit 308 of a TiBase and an A-A cross section of the same, in accordance with some exemplary embodiments of the disclosure. It is appreciated that internal unit 312 can be pre-inserted into external unit 308. Alternatively, the insertion may occur as part of the installation of the denture. In either case, internal unit 312 and external unit 308 can move relatively to each other, thus enabling insertion of screw 204 and connecting the denture to the patient’s jaw.
[0054] Fig. 4B thus shows area 324 fitting inside area 316. Where area 324 and area 316 form parts of spheres, internal unit 312 can roll within external unit 308 while keeping tight contact, within the boundaries set by rims 328 and 310.
[0055] Distally from areas 324 and 316, a gap 404 is formed between external face 408 of internal unit 312 and internal face 412 of external unit 308.
[0056] Thus, when internal unit 312 assumes different positions within external unit 308, the proximal parts are in tight contact, while the distal parts have a gap therebetween, wherein the gap shape changes according to the position. For example, when internal unit 312 and external unit 308 are such that their distal-proximal symmetry axes align, the gap is also symmetric around the axis.
[0057] Referring now to Figs. 5A and 5C, showing a front view of internal unit 312 within external unit 308, and to Figs.5B and 5D, showing an A-A cross section and aB-B cross section, respectively, of the same, in accordance with some exemplary embodiments of the disclosure.
[0058] As seen for example in Fig. 5A, internal unit 312 and external unit 308 are misaligned, as shown by rims 328 of internal unit 312 not being on a parallel plan to rim 310 of external unit 308. As a result, gap 404 is asymmetrical, and is wider where rims 310 and 328 are closer to each other, and narrower where rims 310 and 328 are farther apart.
[0059] As further shown in Figs. 5C and 5D, internal unit 312 and external unit 308 are misaligned, as shown by rims 328 of internal unit 312 being closer to rim 310 of external unit 308 towards the front than towards the back. As a result, gap 404 is again asymmetrical, and is narrower in the front areas where rims 310 and 328 are closer to each other, as seen for example in comparison to gap 404 of Fig. 4B, and is wider in the back areas (not shown), where rims 310 and 328 are farther apart.
[0060] Thus gap 404 together with the radii correspondence of area 316 of internal unit 312 and area 324 of external unit 308, enable relative motion and thus self-adjustment between internal unit 312 and external unit 308. Therefore, in the likely case of measurement inaccuracies in the locations, the two-unit TiBase will self adjust and assume passive fit, such that a connecting element will not apply undesired pressure onto various locations, for example on certain areas of the implant, on the denture, or the like.
[0061] It is appreciated that in once screw 204 is inserted and screwed to the abutment (or to the tissue level implant) it couples internal unit 312 and external unit 308 to each other, regardless of the relative position of internal unit 312 and external unit 308, which may form any shape of gap 404. The corresponding radii of area 316 of internal unit 312 and area 324 of external unit 308 which provide for the gapless fitting of area 316 and area 324, cause the sealing of gap 404, such that germs or other contaminating particles or objects cannot enter gap 404.
[0062] Referring now back to Fig. 4B, further showing protrusion 332 within space 313, which strengthens the coupling between internal unit 312 and external unit 308. Fig. 5B and 5D show space 313 having a different shape.
[0063] Referring now to Fig. 6 A showing a front view of internal unit 312 within external unit 308, and to Fig. 6B, showing an A-A cross section of the same, with afurther embodiment for strengthening the coupling between internal unit 312 and external unit 308, in accordance with some exemplary embodiments of the disclosure.
[0064] Thus, Fig. 6A shows opening 604 in external unit 308. It is appreciated that one, two or more openings 604 may be opened in external unit 308.
[0065] Openings 608 may be opened in internal unit 312, corresponding to openings 604 of external unit 308. During installation, a peg 612 or another object may be inserted through opening 612 in external unit 308 into internal unit 312, for strengthening the coupling therebetween.
[0066] It is appreciated that protrusion 332 and space 313 whichever shape thereof is assumed, or alternatively openings 604 and 608 and peg 312 are exemplary only, and multiple other embodiments for strengthening the coupling between external unit 308 into internal unit 312, may be designed, including parts, protrusions, spaces, or the like. Moreover, such strengthening is not mandatory, and the disclosure can be applied without the strengthening.
[0067] As is also seen in Fig. 7B straight area 206 of screw 204 may be in a taper angle, for example between 0 and 30° relative to the main longitudinal axis of external unit 308. The taper angle enables for Morse taper connection, and thereby to cold welding when straight area 206 is in contact with the corresponding area of internal unit 312, thus avoiding screw loose and further strengthening the contact between screw 204 and parts of internal unit 312 or external unit 308, and securing of the structure.
[0068] Referring now back to Figs. 3 A and 3B showing self-adjusting washer 304, and referring also to Figs. 7A, showing a front view of internal unit 312 within external unit 308, with connecting element 204 inserted, and to Fig. 7B, showing an A-A cross section of the same, in accordance with some exemplary embodiments of the disclosure.
[0069] Self-adjusting washer 304 may be described as a washer with wall, wherein the walls, optionally connecting smoothly to the basis of self-adjusting washer 304, may be curved on their external side, for example having curve 309. In some embodiments, curve 309 may correspond to the wall of internal curved area 314 of external element 308, such that self-adjusting washer 304 can fit into area 314. Self-adjusting washer 304 may be inserted into area314 of external unit 308, and connecting element 204 may be inserted into self-adjusting washer 304 and then into external unit 308 and internal unit 312. Since internal unit 312 receives abutment head 218 of abutment 216, externalthread 208 of connecting element 204 may be screwed into internal thread 220 of abutment 216, thereby coupling denture 104 to jaw 116.
[0070] As shown in Fig. 7B, a gap 704 is formed between screw 204 and self-adjusting washer 304.
[0071] Referring not to Fig. 8 A, showing a front view of an installed set, including screw 204, self-adjustment washer 304, external unit 308, and abutment 216, and to Fig. 8B, showing an A-A cross section of the same, as well as internal unit 312, in accordance with some exemplary embodiments of the disclosure. As detailed above, external surface 309 of washer self-adjustment washer 304 fits into the surface of area 314 of external unit 308.
[0072] Fig. 8A and Fig. 8B show a situation where the measurement misaccuracies or other reasons require internal unit 312 and external unit 308 to be at an angle to each other, such that their longitudinal symmetry axes do not coincide.
[0073] When screw 204 is inserted into external unit 308 and internal unit 312, its longitudinal axis does not coincide with the longitudinal axis of external unit 308, but does coincide with the symmetry axis of internal unit 312, therefore a first angle is formed between external unit 308 and internal unit 312 as shown by space 313 and 313’ which is non-uniform. A second angle is formed between screw 204 and selfadjustment washer 304, as shown by non-uniform space 804 where area 804’ is narrower than area 804.
[0074] Thus, washer 304 which rotates with screw 204, and space 804 between washer 340 and screw 204, enable screw 204 to stay at the tilted position relative to external unit 308, and does not apply force that would cause external unit 308 to centralize around internal unit 312. Absent self-adjusting washer 304, screw 204 would apply uneven pressure on area 311 of external unit 308, and force it to centralize around internal unit 312, thereby applying forces in undesired areas and causing damage to the denture or the implant.
[0075] In some embodiments, during installation, a physician may bend distal part 708 of external unit 308 towards screw 204, to hold self-adjusting washer 304 in place.
[0076] However, self-adjusting washer 304 is not mandatory and the two unit TiBase may be implemented without self-adjusting washer 304.

6. The TiBase according to Claim 1, wherein the first unit comprises a first element and the second unit comprises an opening, such that the first element is inserted into the opening when the first unit is inserted into the second unit.
7. The TiBase according to Claim 1, wherein the first unit comprises a first element and the second unit comprises a second element, such that the first element and the second element connect to each other when the first unit is inserted into the second unit.
8. The TiBase according to Claim 1, wherein the abutment is straight.
9. The TiBase according to Claim 1, wherein the abutment is angulated.
10. The TiBase according to Claim 1, wherein the connecting element has an area shaped in a taper angle, thereby enabling cold welding between the connecting element and the first unit and eliminating screw loose .
11. A dental set comprising the TiBase according to Claim 1, and a self-adjustment washer adapted to correspond to a distal part of the second unit and to receive the connecting element, such that when the connecting element is inserted into the second unit the connecting element and the second unit are not aligned.
12. A dental set, comprising: a TiBase comprising: a first unit having a distal end and a proximal end, the first unit adapted to be positioned distally over a dental implant having an internal thread; and a second unit having a distal end, a proximal end, and an internal space comprising a receiving pocket at the proximal end for receiving the first unit, and an opening at the distal end for receiving a screw having an external thread corresponding to the internal thread of the dental implant; anda self-adjustment washer adapted to correspond to a distal part of the second unit and to receive the screw, wherein the first unit has at its proximal end a circumferential area having an external contour, and the second unit has at its proximal end a circumferential area having an internal contour, wherein the extremal contour and the internal contour are parts of spheres having corresponding radii, such that when the first unit is inserted into the second unit and the screw is connected, the circumferential area of the first unit and the circumferential area of the second unit form gapless contact, thereby sealing an adjustable space formed between the distal part of the first unit and the distal part of the second unit, and wherein the adjustable space and a space formed between the screw and the self-adjustment washer enable misalignment between the first unit and the second unit and between the second unit and the screw, thereby allowing for self-adjustment of a direction of the screw.
