Abstract
Description
[0001] CROSS-REFERENCE TO RELATED APPLICATIONS
[0002] This patent application claims the benefit of priority pursuant to 35 USC §119 to U.S. Provisional Application No. 62 / 727,783, filed on September 6, 2018, and U.S. Provisional Application No. 62 / 848,885, filed on May 16, 2019, the entire contents of which are incorporated herein by reference. Technical Field
[0003] The present disclosure relates generally to medical devices and related methods. More particularly, the present disclosure relates to needles for suturing tissue, related assemblies including sutures, methods of attaching sutures to needles, and methods for engaging tissue, among other aspects. Background Art
[0004] In some medical procedures, it may be necessary or useful to connect a part of a tissue to another part of the tissue fixedly. Attaching some parts of a tissue together with one or more sutures, such as holding a wound or damaged tissue together, may form adhesions between the two tissues, so that the suture remains attached after it is absorbed or removed. Typically, an assembly comprising a needle and a suture attached to the needle is used to sew the tissue together. Tissue attachment methods typically include piercing the tissue with a needle and pulling the needle to pull the suture attached to the needle or pass the suture through a hole formed in the tissue. One way to attach a suture to a needle involves a hole on the needle through which the suture passes, and a knot may be tied at one end of the suture to prevent the end from passing through the hole of the needle. However, the size of the knot may not be consistent, and in some examples, the hole in the needle may be pulled through and the needle and suture may be disengaged. When the suture is disengaged from the needle, the surgical operation time may increase because the operator may need to reassemble the needle and suture assembly. This and other suture attachment devices may allow the suture to extend radially outward from the needle, thereby increasing the resistance of the needle when it is pushed through the tissue. Therefore, there is a need for alternative suture and needle assemblies and methods of suturing tissue, for example, to reduce the chance of suture being released from the needle, to facilitate passage of the needle and suture through tissue and / or to simplify the suturing process. [0025] Reference is now made in detail to embodiments of the present disclosure, examples of which are illustrated in the accompanying drawings. Throughout the drawings, the same reference numerals are used to represent the same or similar parts as much as possible. When used herein, the terms "approximately" and "substantially" may indicate a range of values within + / - 5% of a specified value.
[0026] Figure 1AA perspective view of an exemplary medical device 100 is shown, shown as a double-sided needle. The medical device 100 may include a cylindrical body 102 and two ends or ends 104, 106. Each end 104, 106 may be tapered and reach a sharp point 104', 106'. In other examples, the medical device 100 may include a single tip (104' or 106' at one end 104, 106) and include a blunt opposite end (the other of 104, 106). In some examples, the ends 104, 106 may include recessed portions 108, 110. In other examples, the ends 104, 106 may not include recessed portions 108, 110 and may taper from the end of the cylindrical body 102. The recessed portions 108, 110 may have a curved outer surface, which may be curved radially inward relative to the longitudinal axis of the medical device 100. In some examples, the recessed portions 108, 110 may have a curved outer surface that may meet and be flush with the columnar body 102 at one end of the curved surface. In some examples, the recessed portions 108, 110 may be hourglass-shaped and may have an outer surface that curves radially inward relative to the outer surface of the columnar body 102. In some examples, the recessed portions 108, 110 may be equidistant from the inner cavity 112 of the columnar body 102. In some examples, each recessed portion 108, 110 may be configured to receive a bearing ball of a medical device. The portion of each end 104, 106 on the opposite end of the cusp 104', 106' may be attached to the columnar body 102. In some examples, the ends 104, 106 may be welded to the columnar body 102.
[0027] The medical device 100 may also include a lumen 112 extending through the column 102. The lumen 112 may extend through a central portion of the column 102 (shown in FIG. 1 ) and may extend perpendicular to the longitudinal dimension of the column 102 extending through the cusps 104 ', 106 '. In other examples, the lumen 112 may extend through a non-central portion of the column 102 and / or be at an angle transverse to the longitudinal dimension, but not perpendicular. The lumen 112 may include a first opening 113 on one side of the column 102 and a second opening 114 on the opposite side of the column 102 from the first opening 113. In some examples, the column 102 may include recesses or slots 115, 116 extending longitudinally on the radial outer surface of the column 102. In some examples, the slots 115, 116 may be positioned on opposite sides of the column 102. The slots 115, 116 may create a recess in the column 102. The opening 113 of the lumen 112 may be positioned within or otherwise communicate with the slot 115, and the opening 114 may be positioned within or otherwise communicate with the slot 116. The slots 115, 116 may taper radially inward from a portion proximate each longitudinal end of the columnar body 102 toward the openings 113, 114. The slots 115, 116 may be configured to receive the melted ends of the sutures. In some examples, the medical device 100 may be metal or any other suitable biocompatible material.
[0028] Figure 1B A perspective view of another exemplary medical device 1100 is shown, shown as a double-sided needle. The medical device 1100 may include a cylindrical body 1102; two ends 1104, 1106 with sharp points 1104', 1106'; concave portions 1108, 1110; an inner cavity 1112; and an opening 1113, similar to Figure 1A 1114, except for the following differences. The columnar body 1102 may include a single circular counterbore 1117 surrounding the opening 1113 of the lumen 1112. In other examples, the single counterbore 1117 may surround the opening 1114 (the opening at the other end of the lumen 1112), or the medical device 1100 may include two counterbores 1117 located at each opening 1113, 1114 of the lumen 1112. In other examples, the medical device 1100 may include a single counterbore 1117 at the opening 1113, and the opening 1114 may be flush with the outer surface of the columnar body 1102. The counterbore 1117 may include a wall 1116 extending radially inward from the radially outermost surface of the columnar body 1102 and a ridge 1115 positioned around the opening 1113. The counterbore 1117 at one of the openings 1113, 1114 may be configured to receive an end of a suture, such as a melted end of a suture that conforms to the shape of the counterbore 1117. Counterbore 1117 may help prevent suture from passing through lumen 1112 .
[0029] Figure 1CA side cross-sectional view of another exemplary medical device 2100 similar to devices 100 and 1100 is shown. Medical device 2100 may include a columnar body 2102; two ends 2104, 2106; recessed portions 2108, 2110; a lumen 2112; an opening 2113; and slots 2115, 2116. In some examples, suture 2120 may be melted within lumen 2112. In some examples, suture 2120 may not extend beyond the opening of lumen 2112 on the opposite side of columnar body 2102 from slots 2115, 2116. In some examples, lumen 2112 may include one or more protrusions 2131 (each of protrusions 2131 protrudes into lumen 2112) and / or one or more recesses 2133 adjacent to protrusions 2131. Each protrusion 2131 and / or recess 2133 can help to attach the suture 2120 to the medical device 2100. For example, the suture 2120 can be melted and / or deformed so that some portions of the suture 2110 can be positioned within one or more recesses 2133 of the inner cavity 2112. After some portions of the suture 2120 are melted and / or deformed to position some portions of the suture within the one or more recesses 2133, the suture 2120 can be fixedly attached to the medical device 2100 so that when tension is applied to the suture, such as when the medical device 2100 is used to suture a patient, the suture remains within the inner cavity 2112. The protrusion 2131 resists the tension on the suture 2110 and helps to keep the suture 2110 in the inner cavity 2112. The medical device 2100 can allow the suture 2120 to be attached to the medical device 2100 and extend only from one opening 2113 of the inner cavity 2112. As shown in FIG. Figure 1C As shown, suture 2120 may be bent at opening 2113 of lumen 2112 so that a portion of suture 2120 is positioned within slot 2116, thereby allowing medical device 2100 to have a smaller outer diameter. In other examples, lumen 2112 may include any number of protrusions and / or recesses and / or may include curved protrusions and / or recesses. In some examples, lumen 2112 may not extend completely through medical device 2100.
[0030] Figure 2A and Figure 2B Shown including Figure 1A1 and 120. The suture 120 may be cylindrical and may be polypropylene. In other examples, the suture 120 may be polyester, nylon, polyglycolic acid, polylactic acid, a polymer material, or any other biocompatible material that may be absorbable or non-absorbable. In the medical device assembly 101, the suture 120 may be positioned within the lumen 112. The diameter of the suture 120 may be sized to fit within the lumen 112, and its diameter may be smaller than the diameter of the lumen 112. The end 122 of the suture 120 may be T-shaped and its shape may be configured to fit within the slot 115 or the slot 116. The T-shaped shape of the end 122 may prevent the suture 120 from being pulled out of the lumen 112 during use. In some examples, the end 122 of the suture 120 can be positioned in the slot 115 or 116 so that the end 122 does not extend beyond the radially outermost portion of the outer surface of the column 102. In some examples, the outer surface of the end 122 can be flush with the outer surface of the body 102. The slots 115, 116 that do not accommodate the end 122 can be configured to accommodate the suture 120 when the suture 120 is bent to be generally longitudinally aligned / parallel with the medical device 100, for example, when the user pushes the medical device assembly 101 through the patient's tissue, the suture 120 can be rotated at the opening 113 of the inner cavity and positioned in the slot 115, 116 opposite the slot where the end 122 is located. Therefore, by having little suture protruding radially outward on the outer surface of the body 102 or having no suture protruding radially outward on the outer surface of the body 102, the slots 115, 116 can facilitate the insertion of the medical device 100 into the patient's body.
[0031] In some examples, the method of assembling the medical device assembly 101 includes heating an end of the suture 120 so that the end melts, conforms to the shape of one of the slots 115, 116, and is received in one of the slots 115, 116, for example, to form an end 122. When the end of the suture 120 melts due to the heat, the suture 120 can be bonded to the medical device 100 (particularly the body 102), thereby preventing the suture 120 from being removed from the medical device 100 during use. In some examples, the end 122 of the suture 120 can conform to the shape of one of the slots 115, 116. In other examples, such as when the suture is attached to Figure 1BWhen the medical device 1100 is inserted into the medical device 1100, the end of the suture can conform to the shape of the circular counterbore 1117 in the medical device 1100 and extend through the lumen 1112. In some examples, the end 122 of the suture 120 is fixedly attached to the medical device 100. In operation, the narrow grooves 115, 116 can facilitate the movement of the medical device 100 through tissue by allowing the suture 120 to be accommodated in the narrow grooves 115, 116 and preventing the suture 120 from increasing the circumference, diameter, or cross-sectional diameter of the column 102 of the medical device 100 (which can prevent damage to the tissue). The suture 120 can be used in a medical device assembly including any medical device described herein.
[0032] Figure 3 A perspective view of an exemplary medical device 300 similar to medical device 100 is shown. Medical device 300 may include a column 302 and ends or ends 304, 306 connected to the ends of column 302. Ends 304, 306 may include recessed portions 308, 310. Column 302 may be hollow and may include an opening 317. Opening 317 may be H-shaped and may provide access to an interior portion of column 302. In some examples, opening 317 may include two slots 319, 321 extending longitudinally on column 302, and one slot 323 connecting the two longitudinally extending slots 319, 321 and transverse to each of the two longitudinally extending slots 319, 321. In some examples, slot 323 may be located at the longitudinal midpoint of column 302 and / or slots 319, 321. In some examples, slot 323 may be perpendicular to slots 319, 321. The columnar body 302 may include tabs or flanges 313, 315 that may be formed in the wall of the columnar body 302. In some examples, the tabs or flanges 313, 315 may extend longitudinally on the columnar body 302 and may extend from one end of the body 302 to a transverse slit 323 of the opening 317. In some examples, the tabs or flanges 313, 315 may define the opening 317 and / or be surrounded by the opening 317 on at least three sides. The flanges 315, 317 are cantilevered and flexible at the ends 315', 313' of the flanges 315, 317, respectively. The flanges 315, 317 can thus be bent into and away from the hollow interior portion of the body 302.
[0033] Suture (not shown) can be positioned within the hollow columnar body 302 below the tabs or flanges 313, 315. The user can press the tabs or flanges 313, 315 down onto the suture, and crimp the suture in place by pushing the suture between one or more tabs or flanges 313, 315 and the interior portion of the hollow columnar body 302. In some examples, after the user presses one or more tabs or flanges 313, 315, one or more tabs or flanges 313 315 yield and remain bent downward, thereby holding the suture in place. In some examples, the tabs or flanges 313, 315 can provide a mechanical lock on the suture positioned within the interior portion of the hollow columnar body 302, and / or can fixedly attach the suture to the medical device 300, for example, fixedly attaching the suture within the columnar body 302. In some examples, by allowing the ends of the suture to be crimped within the hollow column 302, such as by bending one or more tabs or flanges 313, 315, the medical device 300 can be more streamlined, and by providing a suture fastening mechanism that is completely within the column 302, the outer diameter or cross-sectional size of the column 302 is limited, which can prevent excessive tissue damage when the user operates with the medical device 300. Once the suture is attached to the medical device 300, the suture can exit the medical device 300 at the opening 317.
[0034] Figure 4 A perspective view of another exemplary medical device 400 similar to medical devices 100, 200, and 300 is shown. Medical device 400 may include a column 402 and ends 404, 406. Ends 404, 406 may include recessed portions 408, 410. The column 402 may be hollow and may include an opening 416 and an inner portion 414. The opening 416 may be oval and may extend longitudinally within the column 402. In other examples, the opening 416 may be any suitable shape and / or may be configured to receive a suture. The column 402 may be hollow (e.g., Figure 7 4 and 406. In some examples, the column 402 may be configured to receive the extensions 417, 419 of each end 404, 406. Figure 7 The extensions 417, 419 are received in openings similar to the openings 665, 666 in the main body 602. The end surfaces 660, 661 of the column can be configured to align with the surface of one of the ends 404, 406 (e.g., Figure 8 The surface 665 of the end 606 is in contact with and / or flush with it.
[0035] Figure 5A perspective view of an exemplary end 404 is shown, which includes a recessed portion 410 and a cylindrical solid extension 419. The cylindrical extension 419 can be configured to be inserted into and positioned within the cylindrical portion 402. In some examples, the cylindrical extension 419 can include an end face 423 having a recess 421. The recess 421 of the end face 423 can be curved and / or wavy and can extend from one side of the extension 419 to the opposite side of 419. The longitudinal axis of the recess 421 can be perpendicular to the longitudinal axis of the device 200 extending from the end 404 to the end 406. The recess 421 can be configured to receive a portion of a suture. The recess 421 can be present on each extension 417, 419 of each end 404, 406, and can form a space between the opposite end faces of each extension 417, 419 when positioned within the cylindrical body 402. The diameter of the space is smaller than the diameter of the suture used in the medical device 400. In other examples, the columnar extension 419 may not include the recess 421. In some examples, when the medical device 400 is assembled, the suture (e.g., suture 120) can be positioned in the inner portion of the columnar body 402 through the opening 416 and between each extension 417, 419 of each end 404, 406. The extensions 417, 419 can pinch the suture in the columnar body 402 to keep the suture in place. The end surface of each end 404, 406 can form an opening 412 to the inner portion of the columnar body 402, and the opening 412 can be configured to receive the suture. In some examples, the suture can be kept in the columnar body 402 and between the extensions 417, 419 by the suture being inserted and / or deformed between the extensions 417, 419. In some examples, portions of the ends 404, 406 may be welded to the column 402 after the suture is pinched and / or fixedly positioned between the extensions 417, 419. In some examples, the end faces 423 and / or recesses 421 of one or more ends 404, 406 may be textured to increase friction between the extensions 417, 419 and the suture. In some examples, the end faces 423 and / or recesses 421 may include sharp protrusions configured to be connected to the suture, such as by piercing the suture, to keep the suture in place. In some examples, the end faces 423 may be angled relative to the opening 416 so that the distance between the end faces 423 of each extension 417, 419 changes when the medical device 400 is assembled. For example, the end surface 423 may be angled relative to the radial outer surface of the extension 419, and the end surface 423 may be inclined so that when the medical device 400 is assembled, the distance between the end surfaces 423 of each extension 417, 419 increases as the distance from the opening 416 increases, and vice versa. In other examples, the ends 404, 406 may be attached to the column using glue, cement, or any other adhesive. In some examples, either end 404 or 406 may be integrally formed with the column, and assembling the medical device 400 with the suture may only require inserting the other end of the end 406 or 404 into the column to pinch the suture. When fully assembled, the medical device 400 may secure the end of the suture within the column 402.
[0036] Figure 6 A perspective view of another exemplary medical device 600 similar to medical devices 100, 200, 300, and 400 is shown. Medical device 600 may include a columnar body 602 and ends 604, 606. Ends 604, 606 may include recessed portions 608, 610 and may have extensions 617, 619 similar to those discussed above with respect to medical device 400, except for the differences described herein. The columnar body 602 may be hollow (e.g., Figure 7 665, 666. In some examples, the components of the medical device 600 may include the column 602 and the ends 604, 606.
[0037] Figure 8 A perspective view of the end 606 of the medical device 600 including the extension 617 is shown. In some examples, the extension 617 can be cylindrical and can have a hollow body that defines a lumen 626 therethrough. The first portion 633 and the second portion 635 of the extension 617 can extend longitudinally from the end 629 of the extension 617 toward the opposite end 631 and can form two opposing gaps 625, 627 in the extension 617. The extension 617 can form a clamp (e.g., a suture) that receives the suture in the lumen 626. Figure 6 The extension 617 can be configured to be inserted into the column 602 of the medical device 600. In some examples, the surface 665 of the end 606 can be transverse to the radially outer surface of the extension 617 and can be configured to prevent the end 606 from being inserted into the body 602. For example, when the surface 665 contacts one of the end surfaces 660, 661 of the body 602, the surface 665 can prevent the end 606 from being further inserted into the inner portion 614 and can help to properly position the end 606.
[0038] The first portion 633 and the second portion 635 of the extension 617 may be curved and may form a partial column. In some examples, the cross-sectional diameter of the extension 617 or the cross-sectional diameter of the partial column formed by the first portion 633 and the second portion 635 of the extension 617 may be approximately equal to the inner diameter of the columnar body 602, or the cross-sectional diameter of the inner portion 614. In other examples, the cross-sectional diameter of the extension 617 may be greater than the inner diameter of the columnar body 602. In other examples, the cross-sectional diameter of the extension 617 may decrease from the end 629 to the end 631, for example, from a cross-sectional diameter greater than the cross-sectional diameter of the inner portion 614 of the body 602 at the end 629 to a cross-sectional diameter approximately equal to the cross-sectional diameter of the inner portion 614 at the end 631.
[0039] The diameter of the lumen 626 formed by the first portion 633 and the second portion 635 of the extension 617 can be large enough to allow the suture 640 to be inserted therein, but as described herein, the diameter of the lumen 626 can be made smaller to squeeze the suture 640 and thereby secure the suture 640 therein. In some examples, to attach the suture to the medical device 600, the user can insert one end of the suture 640 into the lumen 626, which may require the user to bend or move the first portion 633 and the second portion 635 radially outward to position the suture 640 within the lumen 626. When positioned within the lumen 626, the first portion 633 and the second portion 635 can apply force to the suture 640 and / or can clamp down on the suture 640. In some examples, to attach suture 640 to medical device 600, a user may first insert suture 640 through opening 616 and one of the ends of column 602, and then position suture 640 within lumen 626 of extension 617. Once positioned within lumen 626, the user may position extension 617 within interior portion 614 of body 602, which may require the user to push first portion 633 and / or second portion 635 radially inward to fit extension 617 within interior portion 614 of body 602. In some examples, the radially outer cross-sectional diameter of extension 617 may increase from end 629 to end 631, which may cause the force applied by first portion 633 and second portion 635 to suture 640 to increase as the user inserts extension 617 into column 602. This design may facilitate secure attachment of suture 640 to medical device 600. In other examples, the cross-sectional diameter of the extension 617 can remain constant from the end 629 to the end 631 and can be greater than the inner diameter of the body 602. When the cross-sectional diameter of the extension 617 remains constant from the end 629 to the end 631 and is greater than the inner diameter of the body 602, the user can push the first portion 633 and the second portion 635 radially inward to allow the extension 617 to be inserted into the body 602, and once inserted, the inner surface of the body 602 can prevent the first portion 633 and the second portion 635 from moving radially outward and can keep the suture 640 positioned in the inner cavity 626 between the first portion 633 and the second portion 635. Inserting the extension 617 into the body 602 can provide a pinching or clamping mechanism, thereby pinching or clamping the suture 640 between the first portion 633 and the second portion 635 and within the inner cavity 626.
[0040] In some examples, the medical device 600 may also include an end 604 that includes a solid cylindrical extension similar to the end 404. Alternatively, the end 604 and the cylindrical body 602 may be integral with each other and may form a single component of the medical device 600. Fig. 9As shown, needle suturing assembly 900 includes a column 902, end 906, recessed portion 908, opening 916, extension 917, and inner portion 914 similar to the column and end of the previous embodiment. Needle suturing assembly 900 can be integrally formed so that column 902 and end 906 form a single component for a suturing needle device. For example, end 606 (such as Figure 8 606) and a needle suture assembly 900 to assemble the medical device 600, thereby allowing the medical device 600 to include two components, namely the needle suture assembly 900 and the end 606, rather than three components including the column 602 and the two ends 604, 606. In some examples, the needle suture assembly 900 can be metal or any other suitable biocompatible material.
