The invention is directed to an apparatus for use in a transluminal procedure. The apparatus, comprising, for example, a housing having a guide lumen and a seal proximal to a distal end of the housing that extends across and completely seals the guide lumen; a fixation element in the housing and adapted to secure the distal end of the housing to tissue; and a channel extending through the side wall of the housing having an outlet in communication with the lumen distal of the seal. Methods are also provided. For example, a method includes, performing a transluminal procedure by: securing a datum and position indicator to a wall of a target lumen; forming an opening in the wall; advancing an instrument through the opening; and tracking the advancement of the instrument using the datum and position indicator.

A surgical instrument including a housing, an endoscopic portion, a shaft portion and an end effector is disclosed. The endoscopic portion extends distally from the housing and defines a longitudinal axis. The shaft portion is selectively connectable to a distal end of the endoscopic portion. A heart valve can be replaced using minimally invasive methods which include a sutureless sewing cuff that and a fastener delivery tool that holds the cuff against the patient's tissue while delivering fasteners, two at a time to attach the cuff to the tissue from the inside out. These structures may be modified by suturing, stapling, snaring, or shortening, using interventional tools which are introduced to a heart chamber. Preferably, the tissue structures will be temporarily modified prior to permanent modification. The present invention provides devices and methods for attachment of an implanted device, such as an artificial stoma device, a gastrointestinal sleeve device or an attachment cuff, within a patient's digestive tract for treatment of obesity. Special surgical fasteners provide a lasting and durable attachment to the gastrointestinal tissue without causing excessive pressure that could result in tissue erosion and detachment of the implanted device. Fastener delivery devices that facilitate peroral placement and deployment of fasteners and secondary devices are also provided. Also described are implantable devices and attachment means that avoid causing excessive pressure within the tissue by having compliance that is compatible with the gastrointestinal tissues where it is attached.

A surgical instrument may include an end effector, an input drive shaft, a closure drive, and/or a firing drive. The end effector may comprise a jaw assembly including a first jaw member and a second jaw member. The end effector may comprise an I-beam member axially translated between a proximal position and a distal position within the end effector. In one embodiment, the input drive shaft may selectively engage the firing drive and/or the closure drive.
