AI Technical Summary
During mitral valve replacement, existing biological prosthesis causes abnormal displacement of the natural anterior mitral valve leaflet, causing the risk of left ventricular outflow tract obstruction, and complete removal or alteration of the anterior lobe can impair left ventricular function.
A cardilateral valve device is designed, including the bracket frame assembly, peripheral ring and multiple lobes. This device is set up by the peripheral ring in the atrium part and the ventricular part, limiting the protrusion of the bracket frame assembly to the ventricle, reducing the impact on the ventricular outflow, and allowing the natural first -two valve leaf to completely reserve.
This device reduces the protrusion of the heart of the heart of the cardiac fake device to the ventricle, avoids the risk of the left ventricular outflow debut, and at the same time retains the function of the natural first valve leaf, and enhances the pumping capacity of the left ventricle.

[0002] This application claims the benefit of Australian Provisional Application No. 2022900505 filed on March 3, 2022 and Australian Provisional Application No. 2022902706 filed on September 19, 2022, each of which is incorporated herein by reference in its entirety. Background Art
[0003] The mitral valve is the entrance valve of the main pumping chamber of the heart, the left ventricle. The mitral valve has two leaflets, the anterior leaflet and the posterior leaflet, both of which contribute to left ventricular function through a force feedback loop system. The anterior mitral leaflet is the most important and supports left ventricular function by resisting the force of intraventricular pressure and transferring that force back through the left ventricular muscle, thereby assisting the pumping function of the heart. Any changes to the anterior mitral leaflet can significantly impair left ventricular function and the heart's pumping ability.
[0004] With currently available bioprostheses, complete retention of the anterior mitral leaflet during mitral valve replacement carries the risk of left ventricular outflow tract (LVOT) obstruction due to the abnormal displacement of the native anterior mitral leaflet by the bioprosthesis below the aortic valve. LVOT obstruction is associated with a high mortality rate, so the current standard of care is to completely remove or substantially modify the anterior leaflet when performing valve replacement. However, as previously described, removal or modification of the anterior leaflet can substantially impair left ventricular function and cardiac pumping capacity. Therefore, an improved mitral valve bioprosthesis is desired. Summary of the invention

[0023] Unless otherwise defined, all art terms, symbols, and other technical and scientific terms or terminology used herein are intended to have the same meaning as those commonly understood by those of ordinary skill in the art to which the claimed subject matter belongs. In some embodiments, for clarity and / or for ease of reference, terms with commonly understood meanings are defined herein, and the inclusion of such definitions herein should not necessarily be interpreted as representing a substantial difference from what is generally understood in the art.
[0024] Throughout this application, various embodiments may be presented in a range format. It should be understood that the description in a range format is only for convenience and brevity, and should not be interpreted as a hard limit to the scope of the present disclosure. Therefore, the description of the range should be regarded as having specifically disclosed all possible sub-ranges and individual numerical values within the range. For example, a range description such as 1 to 6 should be regarded as having specifically disclosed sub-ranges such as 1 to 3, 1 to 4, 1 to 5, 2 to 4, 2 to 6, 3 to 6, and individual numbers within the range, such as 1, 2, 3, 4, 5 and 6. Regardless of the breadth of the range, this description applies.
[0025] As used herein, the term 'about' when referring to a number means plus or minus 20% of that number. The term 'about' when referring to a range means the range minus 20% of its lower value and plus 20% of its upper value.
[0026] As used herein, the singular forms "a", "an", and "the" include plural references unless the context clearly dictates otherwise. For example, the term "sample" includes a plurality of samples, including mixtures thereof.
[0027] As used herein, the term "substantially D-shaped" means that the substantially straight side of the "D" shape is an arc having a radius of curvature substantially greater than the radius of curvature of the arc of the "D" (e.g., at least 1.1 times the radius of curvature of the arc of the "D"). In some embodiments, the "D" shape is symmetrical about the anterior-posterior midline axis. In some embodiments, the "D" shape is asymmetrical about the lateral midline axis.
[0028] As used herein, the term "atrialisation" or "achieving atrialisation" refers to reducing the protrusion of a mitral valve prosthesis device into the ventricular chamber of the heart, thereby reducing the effects on ventricular outflow or natural leaflet deviation caused by such protrusion (including displacement of the natural anterior mitral valve leaflet to block the left ventricular outflow tract). In some embodiments, the mitral valve prosthesis device has a stent frame assembly including a plurality of ventricular posts configured to protrude into the ventricular chamber of the heart. As used herein, the term "atrialisation" or "achieving atrialisation" also refers to reducing the protrusion of a plurality of ventricular posts into the ventricular chamber, thereby reducing the effects on ventricular outflow or natural leaflet deviation caused by such protrusion. In some embodiments, "atrialisation" of the prosthetic device also enables the natural leaflets (e.g., the natural anterior mitral valve leaflets) to fully deflect, thereby retaining the function of the natural leaflets (e.g., anterior mitral valve leaflet movement and associated valve-ventricular interaction).
[0029] With currently available bioprostheses for mitral valve replacement, complete retention of the native anterior mitral leaflet (AML) during mitral valve replacement carries the risk of left ventricular outflow tract (LVOT) obstruction due to abnormal displacement of the native AML beneath the aortic valve. Displacement of the native AML occurs when the implanted bioprosthesis protrudes too far through the native mitral valve and protrudes into the ventricular chamber and limits the complete displacement of the native AML. However, aspects of the present disclosure provide a mitral valve prosthesis device that exhibits reduced protrusion into the ventricular chamber and does not cause the native AML to obstruct the LVOT, even if the native AML is not resected when the prosthetic device is implanted in the native mitral valve. In some embodiments, the mitral valve prosthesis device of the present disclosure is configured to augment rather than replace the native mitral valve.
[0030] refer to Figure 1-Figure 4 , an exemplary mitral valve prosthesis device 100 of the present disclosure is shown in various views. [0120] Bracket frame covering
[0121] Figure 8 It is along Figure 200 . 0 ...
[0122] In some embodiments, the stent frame assembly 120 includes a covering 126 disposed on the stent frame 530. In some embodiments, the covering 126 includes an inner skirt 825 and an outer skirt 827. In some embodiments, at least one of the covering, the inner skirt, or the outer skirt comprises fabric or pericardial tissue.
[0123] In some embodiments, the peripheral ring 150 includes an inner fabric 852 and an outer fabric 854. In some embodiments, the peripheral ring includes at least one of a braided polyester fabric (e.g., polyethylene terephthalate or polyester), silk, polypropylene, other fiber or fabric materials, silicone, PET, or expanded PTFE. In some embodiments, the peripheral ring includes a wire 858, wherein the wire enables visualization of the prosthetic device 100 under imaging. In some embodiments, the fabric covering encapsulates the wire.
[0124] In some embodiments, the fixed edge 217a of the leaflet 110a is attached to the stent frame 530 by passing sutures through the plurality of suture holes 531. In some embodiments, the covering 126 (e.g., the inner skirt 825 and / or the outer skirt 827) is attached to the stent frame 530 by passing sutures through the plurality of suture holes 531.
[0125] Alternative attachment locations for peripheral rings
[0126] Figure 9A-9B is an elevation view of a stent frame depicting changes in the attachment position of the peripheral ring to the stent frame. Fig. 9B Compared to either of the attachment locations 959b and 959c depicted in FIG. Fig. 9A Attachment location 959a depicted in achieves a higher degree of atrialization. In some embodiments, the lowermost surface 957a of the peripheral ring facing the ventricle is positioned further away from the uppermost atrial end 582 of the barrel 580 than from the ventricular end 581 of the barrel. In some embodiments, the uppermost surface 956a of the peripheral ring facing the atrium is generally aligned horizontally with the surface of the scalloped tip 583.
[0127] Attachment location 959c achieves minimal atrialization compared to attachment locations 959a and 959b.In some embodiments, the ventricle-facing inferiormost surface 957c of the peripheral ring is positioned closer to the superiormost atrial end 582 of the barrel 580 than to the ventricular end 581 of the barrel.
[0128] Attachment location 959b has a degree of atrialization between attachment locations 959a and 959c. Attachment location 959b represents a "horizontal" or "flat" position of the peripheral ring, wherein the lowermost surface 957b of the peripheral ring facing the ventricle is aligned with the horizontal surface of the ventricular end 581 of the cylinder.
[0129] Complete or partial saddle-shaped peripheral ring
[0130] In some embodiments, the peripheral ring has a flat (i.e., horizontal) configuration. In some embodiments, atrialization can be achieved or improved by giving the peripheral ring a partial saddle or a full saddle configuration. In some embodiments, the peripheral ring 1050 can be saddle-shaped so that the peripheral ring is closer to the scalloped portion of the atrial end of the cylinder 580 on the anterior side along the AP diameter and closer to the scalloped portion of the atrial end of the cylinder 580 along the transverse diameter (e.g., a full saddle configuration), such as Fig.10 In some embodiments, the various suture holes on the cylinder 580 can be shifted to better align with or follow the fully saddled peripheral ring 1050 on the cylinder.
[0131] In some embodiments, the peripheral ring 1050 can be saddle-shaped, such that the peripheral ring is closer to the scalloped portion of the atrial end of the cylinder 580 on the anterior side along the AP diameter, and the peripheral ring is horizontal or flat on the posterior side of the AP diameter (e.g., a first partially saddle-shaped configuration). In some embodiments, the peripheral ring 1050 can be saddle-shaped, such that the peripheral ring is closer to the scalloped portion of the atrial end of the cylinder 580 on the anterior side along the AP diameter, the peripheral ring is horizontal or flat on the posterior side of the AP diameter, and the peripheral ring is also horizontal or flat along the transverse diameter (e.g., a second partially saddle-shaped configuration), such as Fig.10 In some embodiments, the various suture holes on the cylinder 580 can be shifted to better align with or follow the partially saddled peripheral ring 1050 on the cylinder.
[0132] Complete or partial saddle-forming can achieve or improve atrialization because the ventricular end 581 of the cylinder can be reverse scalloped toward the lowest surface of the peripheral ring 1050 adjacent to the ventricular end 581, thereby aligning the shape of the ventricular end 581 of the cylinder with or following the shape of the saddle-shaped peripheral ring 1050 and thereby reducing the total area of the stent frame 530 protruding into the ventricular chamber.
[0133] Atrialization of mitral valve prosthesis by implantation method
[0134] The native mitral valve separates the atrial chamber (e.g., left atrium LA) and the ventricular chamber (e.g., left ventricle LV) of the heart. The native mitral valve annulus has a side facing the atrium and a side facing the ventricle. In some embodiments, atrialization can be achieved or improved by implanting a mitral valve prosthesis device on the atrial-facing side of the native mitral valve annulus. Fig.11 An exemplary mitral valve prosthesis device 100 is depicted being placed in the native mitral valve annulus MV by a method of achieving atrialization. In some embodiments, a higher degree of atrialization of the prosthetic device 100 is achieved by suturing the peripheral ring to the atrial-facing side of the native mitral valve annulus, such as Fig.11 , thereby reducing the protrusion of the prosthetic device 100 (e.g., the multiple ventricular posts 140a, 140b, and 140c) into the ventricular chamber. Achieving a higher degree of atrialization can mitigate the effects on ventricular outflow (e.g., LOVT obstruction) or native leaflet excursion caused by such protrusion because the native anterior leaflet of the native mitral valve is not displaced by the prosthetic device 100 or is minimally displaced below the aortic valve. In addition, in the case of a higher degree of atrialization, the native anterior mitral leaflet does not need to be resected or altered, and thus the structure of the left ventricle as well as the chordae CH and papillary muscles PM can continue to be maintained, thereby maintaining their natural support for left ventricular function. Further, in the case of a higher degree of atrialization, the effects on native leaflet excursion will be mitigated (e.g., the protrusion of the stent frame assembly into the ventricular chamber causes limited or less than maximal excursion).
[0135] In some embodiments, when implanting the prosthetic device 100, a standard suture implantation technique can be applied, including suturing the peripheral ring 150 to the atrial side of the mitral valve annulus with a first suture, and then suturing the peripheral ring to the mitral valve wall with a second suture. In some embodiments, a continuous flat stitch (e.g., a whip stitch) can be used for the second suture. In some embodiments, the addition of a second suture helps to cover any cracks that occur at the junction of the sewing ring and the atrial wall or annulus, thereby further preventing the risks associated with stasis and thrombosis.
[0136] In some embodiments, first anterior post 140a and second anterior post 140b are positioned adjacent to the anterior side of the native mitral valve annulus when prosthetic device 100 is implanted. In some embodiments, posterior post 140c is positioned adjacent to the posterior side of the native mitral valve annulus.
[0137] Atrialization of mitral valve prosthesis by scalloping of the peripheral annulus
[0138] Figures 12A-12Dare various views depicting an alternative version of atrialization, wherein an exemplary mitral valve prosthesis device 1200 of the present disclosure includes a peripheral ring 1205 that is scalloped toward a ventricular-facing end 1210 of a plurality of ventricular posts. Figure 13A-13B Depicted separately Figures 12A-12D Elevation and cross-sectional views of a stent frame 1215 of an exemplary mitral valve prosthesis device 1200 are depicted in FIG. Fig.13A The overall height 1220 , column height 1225 , barrel height 1230 , and scallop height 1235 of the stent frame are shown. Fig. 13B Shown along Fig.13A A cross-sectional view of the stent frame 1215 taken along line 13B in FIG.
[0139] In some embodiments, atrialization is achieved by changing the ratio of the cylinder height to the total height compared to the previously shown embodiments. In some embodiments, the sewing ring is scalloped in the ventricular direction along each ventricular column of the stent frame assembly. Such scalloping significantly reduces the ventricular length of the stent frame assembly, thereby reducing the protrusion of the stent frame assembly into the ventricular chamber. In some embodiments, the lowermost surface of the peripheral ring can be measured from the scalloped portion of the peripheral ring.
[0140] The devices described herein may be applicable to other heart valves, such as the tricuspid valve. In some embodiments, a heart valve prosthetic device is configured to enhance, support, or improve the function of a native tricuspid valve when the prosthetic device is implanted in the native tricuspid valve.
