[0004] Hypertension (high blood pressure), also known as hypertension (hypertension), is the leading cause of preventable morbidity and mortality, and it has been determined that treatment to reduce blood pressure (BP) is beneficial. However, despite the availability of a large number of antihypertensive drugs, as demonstrated by multiple large-scale population studies, blood pressure control in many patients remains poor. Factors influencing poor blood pressure control include poor compliance, complex guidelines that suggest multiple upward titration steps, and treatment inertia. In addition, most patients receiving treatment only receive monotherapy, which is still limited in effectiveness even at high doses with increased side effects and reduced tolerance. [0225] Example
[0226] Example 1: Overview of Cardiovascular Measurements in Spontaneously Hypertensive Rats Received Combinations of Antihypertensive Drugs
[0227] The aim of this study was to assess the comparative effects of three different combinations of angiotensin II receptor blockers, calcium channel blockers, and diuretics (thiazide diuretics or thiazide-like diuretics) on blood pressure. The primary aims were to assess whether there were differences in effect between combinations using different drugs from the same class, and between combinations using different doses of the same drug, including very low doses (i.e. doses below the lowest approved and manufactured dose, such as 50% of the lowest hypertension therapeutic dose (LHTD)).
[0228] The specific combinations studied were:
[0229] Combination 1: telmisartan, amlodipine besylate, and indapamide, each at 50% of the lowest hypertension therapeutic dose (LHTD) or one-quarter of the FDA-recommended usual maintenance dose (corresponding to 10 mg telmisartan, 1.25 mg amlodipine besylate, and 0.625 mg indapamide);
[0230] Combination 2: telmisartan, amlodipine, and indapamide, each at 100% of the lowest hypertension therapeutic dose (LHTD) or half of the FDA-recommended usual maintenance dose (corresponding to telmisartan 20 mg, amlodipine besylate 2.5 mg, and indapamide 1.25 mg); and
[0231] Combination 3: losartan, amlodipine besylate, and hydrochlorothiazide, all at 100% of the lowest hypertension therapeutic dose (LHTD) or half of the FDA-recommended usual maintenance dose (corresponding to losartan 25 mg, amlodipine besylate 2.5 mg, and hydrochlorothiazide 12.5 mg).
[0232] The study was conducted in spontaneously hypertensive rats (SHR), the most commonly used animal model for studying hypertension (see: Pinto YM, Paul M, Ganten D. "Lessons from rat models of hypertension: from Goldblatt to genetic engineering". Cardiovascular Research. 39(1): 77–88). Standard allometric scaling methods and C from each of the six antihypertensive drugs were used. max Drug doses were calculated based on published literature data on RT-PCR and AUC. Each animal was exposed to a single dose of each drug combination in a Latin square design.
[0233] method
[0234] The following studies used the following as vehicles: 0.5% methylcellulose (w / v) and 0.25% polysorbate 80 (v / v) in 25 mM phosphate buffer (pH 8 + / - 0.2).
[0235] The following animals were used in the study: Spontaneously hypertensive rats (strain: SHR / NCrl). These rats were obtained from Charles River Laboratories, Inc., Kingston, New York. The age at the start of dosing was approximately 12 weeks. Thirteen male rats were used for acclimation. Eight male rats were used for the study. Animals were identified by cage cards and tattoos.
[0236] Telemetry Implantation: Animals were implanted with a Data Science International transmitter (HD-S10) for the collection of blood pressure and heart rate data. Animals were not administered dosing formulations until at least 10 days after surgery.
[0237] Housing: Animals were housed individually in fixed-bottom cages equipped with water bottles.
[0238] Diet: Unless otherwise specified, Teklad Global Diet-Rodent 2014 (EnvigoRMS, Inc.) was provided ad libitum. In some cases, animals may be fed a dietary form of this diet if health conditions dictate.
[0239] Water: Greenfield city water is available ad libitum.
[0240] Contaminants: There were no known contaminants in the diet, water, or bedding (if any) at levels that would interfere with this study.
[0241] Environment: The environmental controls of the animal room were set to maintain the following room conditions: temperature range of 20 to 26 °C, relative humidity of 30% to 70%, and a 12 h light / 12 h dark cycle.
[0242] Acclimation (pre-dose phase): The acclimation phase is a maximum of 1 week.
[0243] Environmental and dietary enrichment: Animals were provided with various cage enrichment devices and dietary enrichment (not required for analysis).
[0244] Randomization: Animals were randomly selected based on mean arterial pressure values during the pre-dose period.
[0245] The following table shows the grouping of rats used in this study:
[0246] Table 3.
[0247]
[0248]
[0249] The following table shows the dose levels administered in this study:
[0250] Table 4.
[0251]
[0252] Dosing procedure: For combinations 1, 2 and 3, each test combination formulation was freshly prepared on each day of dosing. A portion of the vehicle (about 80%) was added to the test combination formulation and mixed until the formulation was uniform. If a uniform suspension or solution was not obtained, 1N NaOH and / or 1N HCl was added to adjust to pH 9±0.2. The remaining vehicle was added and mixed with a stirring rod. The test combination formulation was stirred continuously at room temperature and protected from light for about 30 minutes before dosing and throughout the dosing process. The test combination formulation was stored in the dark and stirred in a refrigerator set to maintain 2 to 8°C.
[0253] Dosing Procedure: For pre-dose treatment, the test combination formulation was equilibrated to approximately room temperature for at least 30 minutes prior to dosing. Animals were dosed at a volume of 10 mL / kg, and the actual dose volume was based on the most recent body weight. Doses were administered by oral gavage. Dosing intervals were once daily on days 1, 8, 15, and 22. After dosing, the remaining test combination formulation was disposed of according to standard operating procedures.
[0254] Telemetry Collection: Animals will not be disturbed or manipulated shortly before and during telemetry data collection without prior approval. Such disturbances include, but are not limited to, cage changes, bedding changes, mopping, cleaning, or any manipulation that would disrupt the natural quiet environment that is essential for the collection of cardiovascular telemetry data.
[0255] Animal Observations: Each rat was observed once daily in the morning. Any abnormal findings were recorded. Rats were observed for mortality, abnormalities, and signs of pain or distress. Any abnormal findings observed during unscheduled observation periods were also noted.
[0256] Body Weights: Obtain body weights at least once during the pre-dose period and before each scheduled dose. Record additional body weights, if appropriate, to monitor animal health. Equip Animals with Transmitters: Prior to collecting body weights, tarre using a representative transmitter and wire for dose calculations.
[0257] Telemetry Data Collection: Arterial pressure raw signals were digitized at a sampling rate of 500 Hz. The derivation parameters were the same for the pre-dose and dosing phases. For pre-dose data collection, all implanted telemetry devices were checked for signal consistency and the telemetry signal was verified to be acceptable for analysis. Signal checks included collecting at least one telemetry recording from each rat considered for this study. Telemetry data was recorded continuously for approximately 24 hours. Telemetry data was reviewed to determine whether the rat met the study criteria. Data were retained in the study records and used to calculate the nominal 24-hour mean arterial pressure average to support randomized animal selection. For dosing phase data collection, continuous telemetry data was collected during the dosing phase, starting at least 90 minutes before dosing until approximately 48 hours after dosing.
[0258] Nominal dosing time: Telemetry acquisition time points are based on a single nominal dosing time for all animals. The nominal dosing time for each day of the dosing phase is the end of dosing time recorded on each computer for the first half of the animals dosed that day for all animals on the computer.
[0259] Telemetry Data Evaluation: Telemetry parameters including heart rate (beats / minute), systolic blood pressure (mmHg), diastolic blood pressure (mmHg), mean arterial pressure (mmHg), and arterial pulse pressure (mmHg) were analyzed and reported. Telemetry data generated by Ponemah during the dosing phase were analyzed in 1-minute samples. Data were processed as 15-minute averages and provided to data review. The 15-minute average data was further averaged by dividing into the following analysis periods:
[0260] Phase 1: 0.5 to 2 hours after administration;
[0261] Phase 2: 2 to 4 hours after dosing;
[0262] Phase 3: 4-8 hours after administration;
[0263] Phase 4: 8-12 hours after dosing;
[0264] Phase 5: 12-20 hours after administration;
[0265] Phase 6: 20-32 hours after dosing (second light cycle); and
[0266] Phase 7: 32 to 44 hours after dosing (second dark cycle).
[0267] analyze
[0268] Blood pressure was measured over 44 hours using an implanted telemetry device. The primary measure was systolic blood pressure.
[0269] Statistical analyses were performed using all available data points and were weighted to reflect the uneven timing of the measurements. Estimates of treatment effects were calculated using the estimated differences between the two treatments using mixed models with a direct product autoregressive correlation structure (SAS 9.4, SAS Institute, Cary, NC) to account for repeated measures within individuals over time.
[0270] result
[0271] Eight animals were started in the study. However, one animal's telemetry transmitter malfunctioned. As a result, complete data were available from seven animals.
[0272] The following table shows the difference in systolic blood pressure (mmHg) between treatments:
[0273] Table 5.
[0274]
[0275]
[0276] Figure 1 Mean systolic blood pressure (mmHg) over the period is shown by treatment. Figure 2 The mean diastolic blood pressure (mmHg) during the period is shown by treatment. Figure 3 The average heart rate during the session is shown by treatment.
[0277] In this non-limiting example, the results show that Combination 1 produced significantly greater reductions in systolic blood pressure than Combination 3, and Combination 2 produced significantly greater reductions in systolic blood pressure than Combination 3 or 1. These differences persisted throughout the 44-hour observation period. The results show that Combination 1 and Combination 2 produced significantly greater reductions in systolic blood pressure than Combination 3. These differences persisted throughout the 44-hour observation period. There were similar differences between the three combinations in terms of DPB reduction, while there were no differences between the combinations in terms of heart rate.
[0278] In this non-limiting example, these results demonstrate the unexpected difference between the combination of telmisartan, amlodipine besylate, indapamide and the combination of losartan, amlodipine besylate, hydrochlorothiazide. Specifically, compared with the combination of losartan, amlodipine besylate and hydrochlorothiazide, at equal or lower doses, the combination of telmisartan, amlodipine besylate and indapamide produces a significantly greater reduction in blood pressure. Since the dose of amlodipine besylate in combination 2 and 3 is the same, the results demonstrate the previously unknown effectiveness differences of specific angiotensin II receptor blockers and specific diuretics (such as thiazide diuretics and thiazide-like diuretics) when provided in parallel with amlodipine besylate.
[0279] Example 2: Triple combination therapy for treating hypertension
[0280] method
[0281] This study is a randomized, placebo-controlled, double-blind crossover trial. The study is divided into three phases. In the first phase (4 weeks), participants were randomized (1:1) to receive the triple combination therapy or placebo. This was followed by a two-week washout (placebo), after which participants were crossed to the opposite group to receive the other treatment for four weeks. Participants were recruited from the community primarily through community general practices in Western Sydney, Australia.
[0282] Participants
[0283] Participants were eligible if they met the following inclusion criteria: 1) adults 18 years and older; 2) office SBP>140mmHg and / or DBP>90mmHg in two readings on different days; plus baseline ambulatory SBP>135 and / or DBP>85; 3) not taking medication for hypertension. Exclusion criteria included: no clear contraindications to one or more of the components of the triple combination; the responsible physician believed that a change in current therapy would put the patient at risk; severe or accelerated hypertension; pregnancy; inability to provide informed consent; and medical illness with a life expectancy of less than 3 months.
[0284] Intervention
[0285] For these studies, triple combinations with each component at 50% of the lowest hypertension therapeutic dose (LHTD) or triple combinations with each component at 100% of the lowest hypertension therapeutic dose (LHTD) were tested.
[0286] If this study tests a triple combination where each component is 50% of the lowest hypertension treatment dose (LHTD), the test composition is as follows. The triple combination composition is a single encapsulated pill containing the following three components in the specified amounts: 10 mg of telmisartan, 1.25 mg of amlodipine besylate, and 0.625 mg of indapamide. The placebo capsule looks the same and contains a placebo tablet of similar weight to the pills in the triple combination composition.
[0287] If this study tests a triple combination where each component is 100% of the lowest hypertension treatment dose (LHTD), the test composition is as follows. The triple combination composition is a single encapsulated pill containing the following three components in the specified amounts: telmisartan 20 mg, amlodipine besylate 2.5 mg, and indapamide 1.25 mg. The placebo capsule looks the same and contains a placebo tablet of similar weight to the pills in the triple combination composition.
[0288] Participants were administered a single pill, ie, the triple combination composition or placebo, throughout the trial. Patients were instructed to take the tablets at the same time each day and encouraged to take them in the morning, but the time of day (morning or evening) was based on patient preference.
[0289] All trial drugs were prepared by TGA-cGMP (Therapeutic Goods Australia – Certification of Good Manufacturing Practice) licensed manufacturing facilities. Where appropriate, low-strength doses were obtained by bisecting half-strength doses using a pill splitting device without crushing, and weighing them to ensure the accuracy of the bisected doses. Subsequently, gelatin capsules (DBCaps-Capsugel) were used to encapsulate the low-strength doses. The capsules were stored in a cool, dry place and monitored using a temperature recorder until they were distributed.
[0290] Neither the researchers nor the participants were aware of the treatment assignments. In addition to the study medication, all participants received education about healthier lifestyle choices recommended by guidelines for the management of hypertension.
[0291] Randomization
[0292] The computer-assisted randomization sequence was generated by a statistician and provided to the drug packaging company. The research assistants, recruitment team, and investigators were all blinded to this sequence. For each patient, i.e., the assigned randomization number, the pills were packaged into three child-resistant packages corresponding to the three phases of the study. All packages had the same appearance to ensure that the patients and researchers were blinded. Subsequently, the drug packages were issued in an organized order.
[0293] Results and data collection
[0294] The primary outcome measure was the reduction in mean 24-hour systolic blood pressure at 4 weeks using ambulatory blood pressure monitoring (ABP). Secondary outcomes included:
[0295] a. Reduction in mean 24-hour diastolic BP, as well as daytime and nighttime SBP and DBP at 4 weeks
[0296] b. Reduction in office SBP and DBP as measured by a standardized automated blood pressure cuff
[0297] c. Proportion with controlled blood pressure at 4 weeks, defined as 24-hour blood pressure <135 / 85 mmHg and office blood pressure <140 / 90 mmHg
[0298] d. Adverse events by laboratory parameters and pre-specified adverse events: Transaminase (ALT / AST) elevation greater than 3x the upper limit of normal or doubling in the setting of a known elevated baseline level; Estimated glomerular filtration rate decrease >20% as estimated from serum creatinine, sodium, potassium, and uric acid levels
[0299] e. Acceptability and tolerability assessment
[0300] Patients undergo the following 4 24-hour ABP monitoring: baseline (no study drug), 4 weeks (Phase 1 drug), 6 weeks (placebo) and 10 weeks (Phase 3 drug). In order to minimize inconvenience, patients are transferred to the laboratory for ABP monitoring. The ABP unit is calibrated regularly by the laboratory according to the manufacturer's instructions. In order to minimize variability, subsequent readings are repeated from the same collection center using the same brand of device. Participants are reimbursed for face costs to cover their transportation and parking costs. Study drugs and research are provided free of charge to participants. OMRON T9P (HEM-759-C1) is used to record office BP three times at each visit. The second and third readings are averaged for study analysis. In addition, at weeks 4 and 10, patients undergo blood tests to evaluate biochemical side effects, questionnaires on clinical side effects are issued to patients, and compliance is assessed by self-report and pill counting. When completing this questionnaire, patients are still blinded to their treatment allocation.
[0301] Medication acceptability and tolerability were also assessed at the end of the study. All adverse events were recorded. In addition, specific inquiries were made regarding the following clinical adverse events that may be related to the antihypertensive medications: dizziness, blurred vision, syncope / fainting, chest pain / angina, shortness of breath, cough, wheezing, foot edema, rash, and pruritus.
[0302] The trial had a simplified data safety and management committee with two core members with expertise in clinical medicine, trial and statistics. A meeting was held when 10 patients were randomized to the trial to review safety and recommend that the study continue.
[0303] Statistical considerations
[0304] A sample size of 50 patients was planned to provide 90% power at p = 0.05 to detect a SBP difference of 12 mmHg between intervention and control, assuming a SD of 12 mmHg for the within-patient difference, accounting for a 10% probability of loss to follow-up.
[0305] Statistical methods
[0306] Analyses were performed on an intention-to-treat basis. All tests were two-sided and the nominal level of alpha was 5%. All statistical analyses were not adjusted for prognostic covariates. We reported compliance with study medication using data on pills (doses) taken during this period and doses missed.
[0307] According to the method of Kenward and Roger (Kenward MG, Roger JH. The use of baseline covariates in crossover studies. Biostatistics 2010; 11 (1): 1-17), a linear mixed model was used to estimate the effect of treatment on the change in blood pressure relative to baseline for each treatment period. In order to make appropriate adjustments for the baseline levels collected at the beginning of each treatment period (week 0, week 6), this method uses all measurements (baseline and follow-up in these two periods) as the outcome, but takes into account the covariance between measurements within individuals (Liu GF, Lu K, Mogg R, Mallick M, Mehrotra DV. Should baseline be a covariate or dependent variable in analyses of change from baseline in clinical trials? Stat Med 2009; 28 (20): 2509-30). Linear contrasts between variables representing time period (first / second), measurement type (baseline / final), and treatment received (placebo / triple combination composition) produced unbiased estimates of the effect of the triple combination composition on changes in blood pressure compared with placebo. All available data were included in the model, and no missing data were imputed. If a patient had missing data in one time period, the data for the available time period were used. A sensitivity analysis was performed that included only patients with available data in both time periods to determine whether the treatment effect changed. The denominator degrees of freedom were also adjusted according to Kenward and Roger (2009), which is optimal for smaller sample sizes (Kenward MG, Roger JH. An improved approximation to the precision of fixed effects from restricted maximum likelihood. Computational Statistics & Data Analysis 2009; 53(7): 2583-95).
[0308] Carry over tests used unpaired t-tests for the primary measures with order as an effect. The period effect was tested using paired t-tests comparing the primary measures of period 1 with the primary measures of period 2 from the same patient. Sensitivity analyses were also performed using normal paired t-tests to compare the primary measures between different periods (different treatments) from the same patient ignoring the baseline level of each period.
[0309] Continuous secondary endpoints (e.g., daytime / nighttime dynamic SBP / DBP) with baseline values were analyzed similarly to the primary endpoint. Paired t-tests were used to analyze other continuous variables without baseline values in each period. Counts and percentages of all adverse events were reported. As a sensitivity analysis, the analysis was repeated for complete cases (i.e., complete data for each measurement period).
[0310] The treatment effect was related to age (<=60 years and >60 years), gender and BMI (<=30 kg / m 2 and <30kg / m 2 ) were tested for interaction effects. Subgroup analyses of each variable were also performed. All analyses were performed using SAS 9.4 (Cary, NC, USA) software.
[0311] Example 3: Comparative study of triple combination therapy with standard dose monotherapy for the treatment of hypertension Objective
[0312] The primary objective of this study was to investigate in a double-blind, randomized, controlled trial whether initial treatment with a triple combination therapy would be more effective in lowering blood pressure and have fewer side effects than initial standard-dose monotherapy according to current guidelines in patients with hypertension. Secondary objectives were to assess whether this approach was safe and had fewer side effects than standard care.
[0313] Study Design
[0314] This was a 12-week, double-blind, randomized, controlled trial (1:1) in 650 patients with grade 1 and 2 essential hypertension. Subjects were randomized by a central computer-based randomization service to receive initial therapy with a triple combination or angiotensin receptor blocker (ARB) according to current Australian hypertension guidelines (with the option to add a calcium channel blocker (CCB) as needed). The primary outcome measure was the reduction in mean systolic blood pressure measured using a standardized automated BP cuff at 12 weeks. Secondary outcomes included: proportion with controlled blood pressure at 6 weeks, 12 weeks, ambulatory blood pressure (ABP) measurements, and tolerability / occurrence of adverse events.

[0361] Statistical methods
[0362] All analyses of study measures were performed according to the intention-to-treat principle. The primary analysis of change in systolic blood pressure (SBP) at 12 weeks was performed using analysis of covariance (ANCOVA) including treatment group and baseline SBP as covariates. Continuous secondary measures were analyzed similarly. Additional analyses included 6-week and 12-week measurements in longitudinal models, including treatment group, visit, and treatment-by-visit interactions as well as baseline measures. Intra-patient correlations were modeled using generalized estimating equations. Similar approaches were applied to binary endpoints (e.g., hypertension control), with log-binomial regression instead of linear regression. There were also predefined subgroup analyses, including those defined according to baseline blood pressure, sex, age, and history of hypertension treatment. A detailed analysis plan was developed before unblinding.
