Abstract
Description
Technical Field
[0001] The invention relates to a minimally invasive surgical instrument, and in particular to an easily detachable inner core component and a surgical instrument using the same. Background Art
[0002] Laparoscopic minimally invasive surgery uses extended laparoscopic handheld instruments to enter the patient's body through natural cavities or constructed puncture channels to complete operations such as tissue grabbing, shearing, separation, coagulation, and suturing. Compared with traditional open surgery, its main advantages are reducing trauma, alleviating pain, and accelerating recovery. So far, reusable laparoscopic handheld instruments (referred to as reusable instruments) have dominated the market, and disposable laparoscopic handheld instruments (referred to as disposable instruments) are relatively rarely used clinically. Studies have shown that reusable instruments have unreliable and uncontrollable problems in cleaning, distribution, and use, and there is no reliable solution so far. Summary of the invention

Working state: when the drive head is in the working displacement Lw1, the pin head of the kingpin keeps in contact with the first and second cylindrical surfaces, that is, the first and second bite pairs are in the bite state; moving the drive rod can make the first and second jaws close or open within the angle range of 0~A1. Critical state: when the drive head is in the critical displacement Le1, the drive rod continues to move from the proximal end to the distal end. At this time, the opening angle of the first and second jaws reaches the maximum limit angle A1, and Le1 no longer decreases, so that the drive head pushes the first and second jaw tails toward the distal end, thereby pushing the pin head of the kingpin toward the distal end and pushing the first bite and the second bite to open. Separation state: In the critical state, the driving rod continues to move from the proximal end to the distal end, pushing the pin head of the main pin toward the distal end, completely expanding the first and second bites and sliding into the first and second guide openings, that is, the first and second bite pairs are in a failed separation state, thereby realizing the separation of the inner rod assembly and the outer tube assembly.

The driving head 471 comprises a first driving shaft 473 and a second driving shaft 475 extending laterally outward. The driving tail 479 comprises a driving rod annular groove 478.
[0026] Figure 3 , Figure 8-Figure 11 The structural composition and assembly relationship of the inner rod assembly 4 are described in more detail. The first jaw tail 413 and the second jaw tail 423 are stacked on each other; the second pin head 455 passes through the first base hole 415 and the second base hole 425; the retaining ring 46 is fixed to the adjacent area of the end of the second pin head 455, fixing the first and second jaw tails together to form a first rotation pair that can rotate around the main pin 45. The first connecting rod shaft 435 penetrates the first tail hole 414, fixing the first connecting rod head 431 and the first jaw tail 413 together to form a second rotation pair that can rotate around the first connecting rod shaft 435. The second connecting rod shaft 445 penetrates the second tail hole 424, fixing the second connecting rod head 441 and the second jaw tail 423 together to form a third rotation pair that can rotate around the second connecting rod shaft 445. The first drive shaft 473 penetrates the first connecting rod hole 437, fixing the first connecting rod tail 433 and the drive head 471 together to form a fourth rotation pair that can rotate around the first drive shaft 473. The second driving shaft 475 penetrates the second connecting rod hole 447, fixes the second connecting rod tail 443 and the driving head 471 together and forms a fifth rotation pair that can rotate around the second driving shaft 475. Pulling the driving rod 47, the first, second, third, fourth and fifth rotation pairs can rotate simultaneously.
[0027] Figure 2 , Figure 12-13 The assembly relationship of the inner core assembly 3 is described in more detail. The driving rod 47 is installed in the axial through hole 551 of the outer tube assembly; the first and second jaw tails and the first and second connecting rods are installed in the U-shaped groove 506; the first pin head 451 is embedded in the second cylindrical surface 522 to form a first bite pair; the second pin head 455 is embedded in the first cylindrical surface 512 to form a second bite pair. The first and second bite pairs support and fix the two ends of the main pin shaft. When the driving head 471 is moved, the first and second connecting rods are driven to move and rotate, driving the first and second jaw tails to rotate, thereby realizing the closing or opening of the first and second jaws.
[0028] Now combined Figure 1 , Figure 2 , Figure 4 and Fig.14The handle assembly 2 includes a first handle 92 and a second handle 93 connected to a handle shaft 91, and the first handle and the second handle can rotate around the handle shaft. The rotating wheel assembly 95 includes a rotating wheel 951, an outer sleeve 953 and an inner sleeve 955, the outer sleeve 953 is connected to the rotating wheel 951 as a whole, the inner sleeve 955 is connected to the first handle 92 as a whole, and the outer sleeve 953 and the inner sleeve 955 constitute a rotating mechanism so that the rotating wheel 951 can rotate relative to the first handle. The button assembly 97 is installed in the button installation chamber 98 of the first handle 92. The button assembly 97 includes a locking member 972, one end of the locking member 972 is connected to the elastic element 971 and the other end is connected to the button 974, the fastener 973 restricts the locking member 972 in the installation chamber 98, and the elastic element 971 drives the locking member 972 to move laterally in the installation chamber 98 to a locked position. The handheld instrument 1 includes a handle assembly 2 and an inner core assembly 3. The driving tail 479 matches the second handle 93, the elastic element 971 drives the lock 972 to move laterally in the installation bin 98 to the locking position and matches the limiting groove 48, and the connecting head 575 matches the rotating wheel 951 and transmits the rotation torque to the inner core assembly by the rotating wheel. The first handle and the second handle rotate around the handle shaft, thereby pushing the driving tail 479 to move axially, and then pushing the driving head to move axially, and then driving the first and second connecting rods to move and rotate, driving the first and second jaw tails to rotate, and realizing the closing or opening of the first and second jaws. Pressing the button 97 compresses the elastic element 971 to make the lock 972 move laterally away from the locking position, so that the limiting groove 48 is separated from the lock 972, thereby separating the inner core assembly 3 and the handle assembly 2.
[0029] Combination Figure 1-Figure 3 and Figure 9-Figure 14 It is understood that in a preferred design scheme, the inner core component 3 includes three states: working state, critical state and separation state, and the corresponding drive head 70 includes working displacement Lw1, critical displacement Le1 (displacement measurement method: the shortest distance between the first drive shaft axis and the main pin axis), where Lw1>Le1.
[0030] Working state: When the drive head is in the working displacement Lw1, the first pin head keeps in contact with the second cylindrical surface, that is, the first occlusal pair is in the occlusal state; the second pin head keeps in contact with the first cylindrical surface, that is, the second occlusal pair is in the occlusal state. In the working state, the first handle and the second handle can be operated to drive the first and second jaws to close or open within the angle range of 0~A1. Usually A1 is designed to be 65~85°.
[0031] Critical state: when the driving head is in the critical displacement Le1, continue to operate the first handle and the second handle to move away from each other, so that the driving head moves in the direction of reducing the distance between the axis of the first driving shaft and the axis of the main pin. At this time, the opening angle of the first and second jaws reaches the maximum limit angle A1, and Le1 no longer decreases; continue to operate the first handle and the second handle to move away from each other. At this time, the driving head pushes the first and second jaw tails toward the distal end, and then pushes the first and second pin heads toward the distal end and pushes the first bite and the second bite to open.
[0032] Separation state: In the critical state, continue to operate the first handle and the second handle to move away from each other; push the first and second pin heads toward the distal end; fully expand the first and second bites and slide into the first and second guide openings; that is, the first and second bite pairs are in a failed separation state; thereby achieving the separation of the inner rod assembly and the outer tube assembly.
[0033] Those skilled in the art should be able to imagine that, according to the actual needs of the surgical instrument, the size and shape of the first and second connecting rods, the first and second jaw tails and the drive head can be adjusted to adjust the specific value of Le1. An additional limiting mechanism can be added so that the displacement of the drive head is limited to the working displacement Lw1>Le1 during use of the instrument 1 to prevent it from falling out during use.
[0034] In another embodiment, the inner core assembly 3 has no obvious critical state, that is, the operating force causes the first handle and the second handle to move away from each other, and cannot push the first and second pin heads to fully expand and form the first and second bites. Those skilled in the art should be able to understand that auxiliary tools can be used to push or pull the inner rod assembly and the outer tube assembly to separate from each other, thereby pushing the first and second pin heads to move toward the distal end; fully expand and form the first and second bites and slide into the first and second guide openings; that is, the first and second bite pairs are in a failed separation state; thereby achieving the separation of the inner rod assembly and the outer tube assembly.
[0035] The reassembly of the inner rod assembly and the outer tube assembly is the reverse operation of the disassembly action: operate the first handle and the second handle to close each other, pull the first and second pin heads to move toward the proximal end through the first and second guide openings; fully expand the first and second bites and insert them into the first and second cylindrical surfaces; that is, the first pin head keeps in contact with the second cylindrical surface, that is, the first bite pair is in a bite state; the second pin head keeps in contact with the first cylindrical surface, that is, the second bite pair is in a bite state; thereby realizing the reassembly of the inner rod assembly and the outer tube assembly. Similarly, auxiliary tools can also be used for reassembly.
[0036] Reusable laparoscopic handheld instruments can usually be disassembled into two parts: a handle and an inner core assembly, which can be disassembled, cleaned, reassembled and sterilized before use. The sterilization method is usually high-temperature moist heat sterilization or low-temperature plasma sterilization. However, no matter which sterilization method is used, the used instrument must first be thoroughly cleaned before it can be sterilized according to the corresponding sterilization procedure before it can be used again for surgery. During surgery, the head of the instrument needs to operate the patient's internal tissue, and the moving joints of the head instrument are prone to residual tissue residues and are difficult to clean. In order to solve this problem, the prior art discloses a solution for disassembling the inner core assembly into 2 parts or 3 parts. However, so far, the existing technology that has been disclosed and the reusable laparoscopic handheld instruments that have been commercialized are usually not removable between the first jaw, the second jaw and the base of the head. This causes the rotating joints between the first jaw, the second jaw and the base to easily retain pathological tissue or bacteria and viruses, which are extremely difficult to clean, making the sterilization process unreliable and posing a risk of cross infection. When the inner rod assembly and the outer tube assembly of the inner core assembly 3 are disassembled, the base and the first and second jaws can be separated, which effectively solves the problem that the residual tissue in the base is extremely difficult to clean. This detachable mechanism makes the head rotation pairs of the inner rod assembly more robust, and effectively increases its strength and precision while meeting the requirement of detachability.
[0037] There are many ways to fix the retaining ring 46 and the second pin head 455, including but not limited to interference fit, welding, bonding, etc. Laser welding is preferred. Considering that the main working load borne by the main pin 45 is shear load, while the working load borne by the retaining ring 46 is relatively small, an improved main pin 45a (such as Figure 15-16 ), comprising a first pin head 451, a second pin head 455a and a pin flange 453 disposed therebetween; the second pin head 455a comprises a pin ring groove 457. The retaining ring 46a comprises a retaining ring hole 465a defined by an annular sheet 461a, the shape and size of which match the pin ring groove 457; in an optional solution, the retaining ring 46a further comprises a retaining ring opening 463a to facilitate assembly.
[0038] refer to Figure 15-16 , the first jaw tail 413 and the second jaw tail 423 are stacked on each other; the second pin head 455a passes through the first base hole 415 and the second base hole 425; the retaining ring 46a is fixed in the pin ring groove 457, fixing the first and second jaw tails together and forming a first rotating pair that can rotate around the main pin 45a. In one solution, the retaining ring opening 463a can be welded to enhance the firmness of the fixation. In another solution, the annular sheet 461a can be welded to the first jaw tail 413. When the welding solution is adopted, the retaining ring 46a can be divided into 2 or more pieces for splicing to facilitate assembly. Welding the retaining ring itself or welding the retaining ring and the jaw tail without welding the main pin can reduce the impact of welding on the main pin and prevent shear fracture.
[0039] Reference Figure 9-11 , among which the main reference Fig.11 In a preferred embodiment, the end face of the first connecting rod shaft 435 includes a first blind hole 439 defined by a first shaft wall 436; during assembly, after the first connecting rod shaft 435 penetrates the first tail hole 414, a tool is used to force the first shaft wall 436 to deform outward to form a first curling edge (not shown in the figure); thereby, the first connecting rod head 431 and the first jaw tail 413 are fixed together to form a second rotation pair that can rotate around the first connecting rod shaft 435. The end face of the second connecting rod shaft 445 includes a second blind hole 449 defined by a second shaft wall 446; during assembly, after the second connecting rod shaft 445 penetrates the second tail hole 424, a tool is used to force the second shaft wall 446 to deform outward to form a second curling edge (not shown in the figure); the second connecting rod head 441 and the second jaw tail 423 are fixed together to form a third rotation pair that can rotate around the second connecting rod shaft 445.
[0040] refer to Fig.10 Similarly, the end face of the first drive shaft 473 includes a third shaft wall 472; during assembly, after the first drive shaft 473 is passed through the first connecting rod hole 437, a tool is used to force the third shaft wall 472 to deform outward to form a third curling edge (not shown in the figure); thereby, the first connecting rod tail 433 and the driving head 471 are fixed together and form a fourth rotation pair that can rotate around the first drive shaft 473. The end face of the second drive shaft 475 includes a fourth shaft wall 474; during assembly, the second drive shaft 475 is passed through the second connecting rod hole 447, and a tool is used to force the fourth shaft wall 474 to deform outward to form a fourth curling edge (not shown in the figure); thereby, the second connecting rod tail 443 and the driving head 471 are fixed together and form a fifth rotation pair that can rotate around the second drive shaft 475.
[0041] Those skilled in the art should be able to understand that the first, second, third, and fourth curling structures disclosed above can also form a tiny curling by tapping the end of the shaft without the first (second, third, fourth) shaft wall, and can still prevent the second (third, fourth, fifth) rotation pair from disengaging. The drive head 471, the first drive shaft 473, and the second drive shaft 475 disclosed above are connected as a whole, but a drive head through hole can also be set in the drive head, and an additional shaft can be riveted in the drive head through hole to form the first drive shaft and the second drive shaft. The drive head can also be clamped between the tails of the first and second connecting rods, and then an additional shaft can be passed through the first and second connecting rod holes and the drive head through hole, and then riveted together to form the fourth and fifth rotation pairs. There are more approximate alternatives that are easy to think of, which will not be repeated here.
