[0034] Despite the benefits derived from the current single drug pain relief regimens, these regimens have disadvantages. One area of concern relates to the incidence of unwanted side effects caused by many of the pain treatment regimens available today. Narcotic analgesics, such as morphine, are sparingly prescribed for chronic pain because of the well-known addictive effects and central nervous system (CNS) side effects and gastrointestinal side effects resulting from their single administration.

[0035] Another concern of the current pain treatment regimens relates to their effectiveness. Many single active ingredients such as antidepressant agents or GABA analogs employed in current pain relief regimens cannot achieve adequate pain alleviation even at their maximum approved therapeutic doses in certain severe pain states. In addition to not achieving adequate pain alleviation, increasing the drug dose may produce an increase in unwanted side effects such as cognitive impairment, dizziness, somnolence, nausea, and constipation.

[0036] Furthermore, other concerns of GABA analogs and many narcotic analgesics relate to their less favorable pharmacokinetic and physiological properties. Many orally administrated opioid molecules are extensively metabolized by digestive organs before reaching systemic circulation. Rapid systemic clearance and saturable absorption of some of the GABA analogs have limited these drugs to reach their full potential in treatment of pain and other CNS disorders. These sub-optimal properties often lead to less than adequate efficacy and unwanted side effects in patients.

[0037] Sustained released formulations are a conventional method to address the issue of rapid systemic clearance, as it is well known to those skilled in the art (e.g., “Remington’s Pharmaceutical Sciences, ” Philadelphia College of Pharmacy and Science, 17th Edition, 1985) . GABA analogs, such as baclofen, gabapentin and pregabalin are not absorbed through the large intestine. Rather, these compounds are typically absorbed in the small intestine by the neutral amino transporter systems (Jezyk et al., Pharm. Res., 1999, 16, 519-526) . The rapid passage of conventional tract has prevented the successful application of sustained release approach to these GABA analogs.

[0038] Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for the management of pain often as first line therapy. Naproxen, approved by the FDA in 1976 and approved as OTC drug in 1994 is one of the most commonly used NSAIDs for the management of both acute and chronic pain. Though generally well tolerated, the oral use of NSAIDs such as naproxen with systemic exposure is often associated with serious gastrointestinal (GI) side effects such as ulcers or bleeding (Aleve USPI ref; Kyremateng 2019; Solomon 2017) .

[0039] In view of these concerns, it is evident that there is a need for an improved pain regimen that provides an improved therapeutic benefit (i.e., reduced severity and / or frequency of pain) and / or reduces  the incidence of unwanted side effects caused by many of the current regimens. In addition, improving pharmacokinetic profile of GABA analogs will also lead to more customized dosing regimens according to patients’ need. Furthermore, topical administration of NSAIDs via naproxen will eliminate gastrointestinal (GI) and systemic exposure and improve overall safety.

[0040] Naproxen is marketed products for the treatment of various types of pain (due to rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, tendinitis, bursitis, and acute gout) and pregabalin is marketed for the treatment of neuropathic pain, neuralgia, and fibromyalgia. Naproxen was initially approved by the United States (US) Food and Drug Administration (FDA) in 1976 and approved as over-the-counter over-the-counter (OTC) drug in 1994. Pregabalin was initially approved by the US FDA in 2004, and generic versions are available since 2018. The topical formulation of naproxen that aimed to delivery naproxen topically has been developed to reduce the side effects and improving patient compliance (Patwardhan et al. 2017) . Topical formulation of naproxen gel 10%w / v is approved in several European countries (Momendol Gel) . Similarly, the various product formulation development studies for pregabalin were completed to develop pregabalin controlled transdermal alternative to the conventional oral route (Arafa and Ayoub 2017 Arafa and Ayoub 2017; Bhatia et al. 2012) to minimize its CNS side effects. It could be an useful treatment option to avoid or minimize the central nervous system (CNS) -mediated side effects of orally administered pregabalin (Fukasawa et al. 2014) . However, there is no topical pregabalin product on market thus far likely due to its hydrophilic chemical nature making topical delivery of pregabalin very challenging.

[0041] Multimodal therapeutic approaches for pain are recommended to minimize the adverse effects of individual drugs as well as to target different mechanisms of pain, including nociception, inflammation, and neuropathic (Finnerup 2019; American Pain Society monograph on pain; Hsu et al. 2019) . Using such an approach may lead to better efficacy and lower opioid use for pain management in both acute and chronic pain syndromes. Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for the management of pain often as first line therapy. Naproxen, approved by the FDA in 1976 and approved as OTC drug in 1994 is one of the most commonly used NSAIDs for the management of both acute and chronic pain. Addition of pregabalin, an agent known to target neuropathic pain to a NSAID may result in improved pain control in both acute (Wang et al. 2010, bunionectomy model) and chronic pain (Sofat et al. 2017 -hand osteoarthritis, Derry et al 2017-acute and chronic neuropathic pain, Lambrechts et al. 2013 -chronic post thoracotomy pain) .

[0042] (S) -3- ( ( ( ( (S) -1- ( ( (S) -2- (6-methoxynaphthalen-2-yl) propanoyl) oxy) ethoxy) carbonyl) amino) methyl) -5-methylhexanoic acid is a drug conjugate of pregabalin and naproxen. This drug conjugate is being developed for the management of both acute and chronic pain syndromes. Pre-clinical animal models have demonstrated improved efficacy with drug conjugate for pain and inflammation compared with naproxen alone. Topical (S) -3- ( ( ( ( (S) -1- ( ( (S) -2- (6-methoxynaphthalen-2-yl) propanoyl) oxy) ethoxy) carbonyl) amino) methyl) -5-methylhexanoic acid is expected to fulfill an unmet clinical need for improved management of both acute and chronic pain and  reduce the need for opioids. There is a need for pure (S) -3- ( ( ( ( (S) -1- ( ( (S) -2- (6-methoxynaphthalen-2-yl) propanoyl) oxy) ethoxy) carbonyl) amino) methyl) -5-methylhexanoic acid.

[0122] A reactor was vacuumed to ≤-0.08 MPa and then inflated with nitrogen to atmosphere for three times. NaHCO3 (2.0 eq. ) was dissolved in purified water (10 V) . 2-Fluorophenol (1.0 eq. ) was charged to the system. The temperature was cooled to 2±3 ℃ and 1-chloroethyl chloroformate (1.3 eq. ) was added dropwise to the system. The temperature was raised to 15±5 ℃ after dropping. n-Heptane (10 V) was charged into the system and the temperature was kept at 25±5℃. The resulting mixture was stirred for at least 20 mins. The mixture was then allowed to rest for at least 30 mins, separated, and the organic phase was separated. The organic phase was washed with purified water (5 V) twice and NaCl (10wt%) aqueous solution (5 V) once. The Organic phase was concentrated to 4~5 V (in vacuum and system temperature ≤ 50℃ and the jacket temperature ≤ 60℃) . Toluene (10 V) was added to the system and the mixture was concentrated to 4~5 V (in vacuum and system temperature ≤ 50℃ and the jacket temperature ≤ 60℃) . This step was repeated a second time. Intermediate A was placed in a high density polyethylene keg or 304 stainless steel storage tanks, weighed, and stored at room temperature.

[0124] A reactor was vacuumed to ≤-0.08 MPa and then inflated with nitrogen to atmosphere for three times. Naproxen (2.0 eq. ) and Intermediate A (1.0 eq. ) were dispersed in toluene (5.0 V) . Cuprous oxide (0.8 eq. ) was added to the system. The temperature rose to 115±5℃. The reaction mixture was cooled to 60±5℃ after reacting for at least 8.0 hours at 115±5℃. A sample was taken for HPLC monitoring the area%of Intermediate A ≤ 2.5%. If area%of Intermediate A >2.5%, the temperature was increased to 115±5℃ for reacting at least 1.0 hour, then cooled to 60±5℃, sampled for HPLC monitoring until the area%of Intermediate A ≤ 2.5%. If area%of Intermediate A ≤ 2.5%, the temperature was cooled to 50±5℃ and MTBE (10.0 V) was added into the reactor A at 50±5℃, then continued cool to 25±5℃. The reaction mixture was filtered. The filter cake was washed with MTBE (0.5 V) twice. The filtrate was transferred to a new reactor. Purified water (10.0 V) was added into the reactor, and the mixture was cooled to 0±5℃.

[0125] Ammonium hydroxide (15wt%) (1.0~2.0 V) was slowly added to the reactor , the pH was adjusted to 9~11. Th organic phase was separated and collected. The organic phase was washed with  ammonium hydroxide (0.5wt%) aq. (10.0 V) once. The organic phase was washed with NaCl (10wt%) aq. (10.0 V) once. The organic phase was concentrated under vacuum below 50℃ until the system is 2.5~3.0 volumes. MeCN (10.0 V) was added to the reactor. The mixture was concentrated under vacuum below 50℃ until the mixture is 2.5~3.0 volumes. This step was done until the peak area proportion of toluene in the system was ≤ 10%. A sample for HPLC analysis was taken to measure the purity of Intermediate B, Specification: the area%of Intermediate B is ≥85.0%. Intermediate B in MeCN was packaged into high density polyethylene keg or 304 stainless steel storage tanks, and stored at room temperature under nitrogen protected.

[0127] The reactor was vacuumed to ≤-0.08 MPa and then filled with nitrogen to atmosphere three times. Intermediate B (1.0 eq. ) in ACN (1.5 V) and MTBE (2.0 V) was added. Purified water (2.0 V) was added to the system followed by pregabalin (2.0 eq. ) . The temperature was cooled to 15±5 ℃. The temperature rose to 25±3 ℃ after charging TEA (2.0 eq. ) dropwise at 15±5 ℃. The reaction was allowed to proceed for at least 4 hours at 25±3 ℃ until the area%of Intermediate B was ≤0.5%. The temperature was cooled to 5±5℃. KHSO4 (10wt%) aq. (about 10~11 V) was added dropwise to the reactor at 5±5℃, the pH was adjusted to 3~5. MTBE (5.0 V) was charged into the system. The organic phase was separated and collected. The organic phase was washed with purified water (5.0 V) once. The organic phase was washed with NaCl (20wt%) aq. (5.0 V) once. The organic phase was filtered through Micro porous filter, and then concentrated under vacuum not more than 45℃ until 2.5~3.0 V. i-PrOH (5.0 V) was added into the reactor through micro porous filter and the reaction mixture was concentrated under vacuum not more than 45℃ until 2.5~3.0 V. This step was repeated until the proportion of the sum area%between MTBE and MeCN in system was not more than 5%. The reaction mixture was heated to 40~45℃ and stirred until dissolved. Heptane (6.8 V) was added to the reactor at 40~45℃.

[0128] The mixture was cooled to 25±3℃ after stirring for 1.0~1.5 hours at 40~45℃, then cooled to 10~15℃ after stirring for 15~16 hours at 25±3℃. The temperature was kept at 10~15℃ stirring for 1.0~1.5 hours. The reaction mixture was filtered. The resulting filter cake was washed with heptane (0.5  V) twice. The filter cake is compound 2. The filtrate comprising some remaining compound 2 and compound 1 was collected and concentrated under vacuum below 45℃ until the residual volume is 3~4 V. This step was done three times. The mixture comprising some remaining compound 2 and compound 1 was transferred to high density polyethylene drum and stored at 10~30 ℃.

[0130] The reactor was vacuumed to ≤-0.08 MPa and then filled with nitrogen to atmosphere three times. The mixture comprising some remaining compound 2 and compound 1 from Step 4 (1.0 eq. ) and ethyl acetate (10 V) were added to the reactor and stirred. N-methylbenzylamine (1.0 eq. ) was added to the reactor at 25±5℃. The resulting mixture was stirred at least 4 hours at 25±5℃. The mixture was filtered and the filter cake was washed twice using ethyl acetate (2 V) . The filter cake was dried under vacuum at 45℃ at least 12 hours. Compound 3 was collected and packaged in double low density polyethylene bags sealed with cable ties, placed into a fiber drum, and stored at room temperature.

[0132] The reactor was vacuumed to ≤-0.08 MPa and then inflated with nitrogen to atmosphere for three times. Compound 3 (1.0 eq. ) and ethyl acetate (10 V) were added to the reactor. The temperature was increased 45±5℃ and the reaction mixture was stirred for at least 4 hours at 45±5℃. The mixture was then cooled to 25±5℃ and filtered. The filter cake was washed twice with ethyl acetate (2 V) . The product was sampled for HPLC analysis. The criteria was: the area%of compound 1 ≥ 97.0%in the salt of compound 1, 2-fluorophenol ≤ 0.2%, naproxen ≤ 0.5%, compound 2 ≤ 2.0%and unspecified single Impurity ≤ 0.10%. If the standard is not met then the hot slurry operation was repeated. If the standard was met, the filter cake was dried in vacuum under 45℃ at least 12 hours. Pure compound 3 was packaged in double low density polyethylene bags sealed with cable ties and stored at room temperature.

[0134] The reactor was vacuumed to ≤-0.08 MPa and then inflated with nitrogen to atmosphere for three times. Purified water (5 V) and KHSO4 (1.2 eq. ) were added into the reactor and stirred until all dissolved. Ethyl acetate (5 V) and pure compound 3 (1.0 eq. ) were added and the temperature was adjusted to 25±5℃. The mixture was stirred for at least 2 hours at 25±5℃. The mixture was then allowed to stand for at least 30 minutes. The organic phase was separated and washed with purified water (5 V) once, then washed with 15%NaCl aq. (5 V) once. Repeat washing steps until N-methylbenzylamine ≤ 0.1%. The organic phase was concentrated under vacuum below 40℃ until the system is 3~4 volumes. Ethyl acetate (5 V) was added into the reactor and the mixture was concentrated under vacuum below 40℃ until the system is 3~4 volumes. This step was repeated until KF ≤ 0.5%. The reaction mixture was filtered through an activated carbon filter cartridge to remove any color, then through a microporous filter, and then transferred to the reactor. The temperature was kept at 25±5℃. n-Heptane (5 V) was added to the reaction mixture. A seed was added (0.5%, w / w) to the reaction mixture and it was stirred 0.5~1.0 hour. n-Heptane (5 V) was added dropwise to the system at 25±5℃ and then stirred for at least 4 hours. The rection mixture was cooled to 5±5℃ and kept at that temperature for at least 1 hour. The mixture was centrifuged and the resulting filter cake was washed with n-heptane (2 V) twice. A sample was taken for HPLC analysis. The purity criteria is: the area%of compound 1 ≥ 98.0%, N-methylbenzylamine ≤ 0.1%, naproxen ≤ 0.5%, compound 2 ≤ 1.0%, and unspecified Impurities ≤ 0.10%, with total impurities ≤ 2.0%. The cake was dried under vacuum at 40℃ for at least 15 hours. The residual solvent criteria is: 2-Propanol ≤ 5000ppm, acetonitrile ≤ 410ppm, tert-butyl methyl ether ≤ 5000ppm, n-Heptane ≤ 5000ppm, toluene ≤ 890ppm, and ethyl acetate≤ 5000ppm. Compound 1 was packaged in double low density polyethylene bags sealed with cable ties with nitrogen protection and stored in a well-closed container at room temperature.
