Abstract

Electrosurgical handpiece and proximal, central, and distal sections of a main body of an electrosurgical handpiece. Electrosurgical handpieces are primarily used for endoscopic applications in urology or gynecology. However, the scope of use for these instruments is not limited to these areas of the human body but also includes the treatment of other organs in the human abdomen. The invention creates an electrosurgical handpiece, as well as a handle, a main body, and an adapter, which can be manufactured in a particularly simple and economical manner and simultaneously ensure laminar flow of the rinsing fluid at the distal end of the handpiece. This is achieved by creating a funnel-shaped passage through a handle, in which a cross-section of the passage narrows from a distal to a proximal side of the handle.Figure for the abridged version: Fig.1.

Description

Title of the invention: Electrosurgical handheld apparatus and a proximal, central and distal section of a main body of an electrosurgical handheld apparatus

[0001] The invention relates to a central section, in particular a handle, for an electrosurgical handheld device. The invention further relates to a distal section, in particular a body, for an electrosurgical handheld device. Furthermore, the invention relates to a proximal section, in particular an adapter, for an electrosurgical handheld device. Furthermore, the invention relates to an electrosurgical handheld device and to an electrosurgical handheld device.

[0002] Electrosurgical handheld devices, such as resectoscopes, are mainly used for endoscopic applications in urology or gynecology and preferably for treatment in the bladder, uterus or prostate. The field of use of these instruments is not limited to these areas of the human body, but also includes the treatment of other organs in the human abdomen.

[0003] The generic instruments described herein, such as resectoscopes, have by default a carrier and an elongated tube-shaped shaft. To treat diseased tissue, this shaft is introduced through an orifice into the patient's body. Various medical instruments for the treatment and / or examination of the patient can be arranged in this shaft tube. For example, for high-frequency surgery, an electrode that can be powered by a high-frequency alternating current, which is positioned at a distal end of an electrode instrument, can be guided through the shaft. To manipulate or cut tissue, the electrode instrument with the electrode can slide relative to the shaft tube as well as along a shaft axis, so that by moving the electrode along or through the tissue, the tissue is manipulated.

[0004] The electrode instrument is further coupled with its proximal end to the carrier. This manipulation of the carrier enables the cutting movement of the electrode. The carrier has a movably housed contact body, which is also called a carriage. At this contact body, the electrode instrument can be mechanically detachably coupled with an electrical contact. The electrode instrument or the electrode can also be supplied with electrical energy via this mechanical connection. For this purpose, the contact body has an opening, into which an electrical contact of the electrode holder can be guided for the detachable connection. This opening is designed in such a way that the high voltage frequency can be applied via an adjacent connector socket. To do this, a connector is usually plugged into the socket, which in turn can be connected to a high-frequency generator via a line or cable.

[0005] The actuation or longitudinal sliding of the conveyor, and therefore also of the electrode, is carried out by an operator. For this purpose, a handle unit with a handle and a gripping means is associated with the conveyor. The operator grasps the handle, as well as the gripping means, which is designed as a finger unit or as a thumb ring, to actuate the conveyor. The handle is attached to a fixed main body of the conveyor or to the contact body.

[0006] The movement of the conveyor is in the opposite direction to a spring tension force of a spring, which is usually realized as a leaf spring or as a leg spring for the generic conveyor. This spring is fixed with one end at the contact body or the carriage and with the other end at an end body or an optical system guide plate of a reinforcement tube. The type of springs or the type of actuation of this spring mechanism varies depending on whether the conveyor is an active conveyor or a passive conveyor. While the spring is realized as a compression spring in the case of an active conveyor, it is realized as a tension spring in the case of a passive conveyor. The reinforcement tube is fixed to the handle preferably in a removably manner by means of an adapter.

[0007] The cutting of the electrode is usually done by a retraction movement of the carrier. In the case of the active carrier, the electrode is retracted to do this in the direction opposite to the spring force of the spring (in the proximal direction). In the case of the passive carrier, on the other hand, the electrode is first advanced against the spring force (in the distal direction) and then cuts the tissue during the retraction movement caused by the discharge of the spring (in the proximal direction).

[0008] An optical system can also be guided through the shaft of the instruments described herein. Exemplary embodiments are known in which the optical system is guided as a rod lens system or as a glass fiber from a proximal end to the distal end through the shaft. The distal end of the optical system is directed directly onto the area to be operated on or the site of action of the electrode. At the proximal end of the optical system, the operator can observe the treatment through an eyepiece and a camera.

[0009] The electrode instrument has two parallel electrode arms, which are oriented parallel to the axis of the shaft. While the two distal ends of the arms electrode arms carry the electrode, the two proximal ends of the electron arms can be guided into the contact body for mechanical and electrical contact. To achieve this, the two proximal ends of the electrode arms are guided from the distal direction along the shaft through the main body, the handle as well as the adapter.

[0010] In order to distance the contacting or the proximal ends of the electrode arms from the optical system and to create the installation space for sealing the individual passages, the tube-shaped electrode arms have an offset. This offset is implemented in an S-shape and forms a stepped step from a distal section to a proximal section of the electron arms. Also known are embodiments in which the two parallel electrode arms are combined in the proximal direction to form an electrode body. In doing so, the S-shape is located directly in the transition region of the two electrode arms to the electrode body. For structural reasons, the offset is usually located in a distal region of the electrode instrument. Thus, both offsets or one offset are located in the flow of the rinsing liquid, which is rinsed from the proximal end of the instrument to the treatment site. This leads to a major flow disturbance, whereby no laminar flow of the rinsing liquid can form in the field of view of the optical system. A displacement of the type S in the proximal direction, for example, in the main body is, however, very complicated and associated with high costs. This is due, among other things, to the fact that the space around the optical system and seen in the distal direction in front of the main body is very limited. The known electrode bodies are therefore flexibly designed to be deflected in the main body.

[0011] Based on this, the object of the invention is to provide an electrosurgical handheld device 1 as well as a handle, a main body and an adapter, which can be manufactured in a particularly simple and economical manner and at the same time guarantee a laminar flow of the rinsing liquid at the distal end of the handheld device.

[0012] A solution to this problem is a central section, in particular a handle, of a main body for an electrosurgical hand-held device, in particular for a resectoscope, in which the central section can be connected at a distal side in a detachable or fixed manner to a distal section, in particular a body, of the main body and at a proximal side to a proximal section, in particular an adapter, of the main body, and in which the central section has an axial bore for the passage of a rod-shaped component of the hand-held device and in which the central section has parallel to the axial bore at least one passage for the passage of an electrode arm of an electrode instrument of the hand-held device, characterized in that the passage is designed in a funnel shape, in which a cross-section of the passage narrows from the distal side to the proximal side of the handle. It is thus provided that a central section, in particular a handle, of a main body for an electrosurgical hand-held device, for which it may be, for example, a resectoscope, can be connected at a distal side in a detachable manner to a distal section, in particular a body, of the main body and at a proximal side to a proximal section, in particular an adapter, of the main body. The distal section, the central section as well as the proximal section thereby form individual elements of the main body of the carrier, which can be coupled in such a way that they form the main body. The distal section can be designed in this way as a body, the central section as a handle and the proximal section as an adapter or be considered in the sense described above.

[0013] The central section has an axial bore, through which a rod-shaped component of the hand-held device, such as, for example, the optical system, can be guided. Parallel to this axial bore, the central section has at least one passage for the passage of an electrode arm of an electrode instrument of the hand-held device. This central section or handle is characterized in that the passage is designed in a funnel shape, in which a cross-section of the passage narrows from the distal side to the proximal side of the central section. This funnel-shaped configuration of the passage makes it possible to guide the at least one electrode arm of the electrode instrument particularly easily through the passage of the central section.

[0014] It is provided in particular that a first cross section of the passage is formed oval at the distal side and a second cross section of the passage is formed circular at the proximal side. By this preferred configuration of the passage of the central section, it can be used in a particularly simple manner with other components of the hand-held device.

[0015] The invention preferably provides that the first cross-section continuously becomes the second cross-section of the passage of the central section, in particular that the oval cross-section of the passage continuously becomes the circular cross-section of the passage. By this continuous transition, i.e. this non-stepped transition, the end of the electrode arm can be guided particularly simply and efficiently through the passage of the central section or the handle. The end of the electrode arm is thus led from the side with the larger cross-section through the passage to the side with the smaller cross-section.

[0016] Furthermore, the invention may provide that a main axis of the oval cross-section of the passage is oriented vertically and a secondary axis of the first oval cross-section of the passage is oriented horizontally. In doing so, it is entirely possible for the axes to be oriented differently. However, it has been found that the axis orientation claimed here, particularly in combination with the main body of the hand-held device, is particularly advantageous. Particularly in the case of an exemplary embodiment with two passages, the vertical main axes prove to be particularly advantageous. In a vertical orientation, the main axes of the two passages are parallel. This orientation allows an offset of the two electrode arms in a common spatial direction, which has a positive effect on the costs of the electrode instrument.

[0017] A particularly advantageous embodiment of the invention may provide that the central section has two passages for the passage of each electrode arm of the electrode instrument of the hand-held device. In doing so, the two passages are configured as described above. The two passages are designed in a funnel shape and oriented parallel to each other and relative to the axial bore of the central section.

[0018] The central section claimed here of the main body or the handle of the hand-held device or the resectoscope is produced as an injection-molded part. Thus this section is manufactured from a plastic material by means of the known injection-molding process. This proves to be particularly advantageous, given in particular that the funnel-shaped configuration of the passages can only be produced with great difficulty and expense by other manufacturing processes or with other materials.

[0019] Another solution to the problem stated at the beginning is claimed by a distal section, in particular a body, of a main body such as the distal section, in particular body, of a main body for an electrosurgical hand-held device, in particular for a resectoscope, in which the distal section is connected at a distal side to a tube-shaped rod and can be connected at a proximal side in a detachable or fixed manner to a central section of the main body and in which this section has an axial opening or bore for the passage or for the reception of a rod-shaped component of the hand-held device and in which the section has parallel to the axial opening at least one passage for the passage of an electrode arm of an electrode instrument of the hand-held device, characterized in that the at least one passage has an oval or oblong hole-like cross-section.Thus, the distal section is connected, in particular welded, at a distal side, preferably removably or fixedly, to a tube-shaped rod and can be removably connected at a proximal side to a central section. The distal section has an axial opening or bore for the passage or reception of a rod-shaped component of the hand-held device. Parallel to this axial opening, the distal section has at least one passage for the passage of an electrode arm of an electrode instrument of the hand-held device.

[0020] The distal section or the body is characterized in that the at least one passage has an oval or oblong hole-like cross-section. By this oval or oblong hole-like configuration of the passage, the electrode arm can be received in a particularly advantageous manner. In particular, if the electrode arm diverges from its tubular or circular cross-sectional shape and has, for example, an offset, it can be introduced movably in a particularly advantageous manner into the vertically widened passage.

[0021] It is preferably provided that a major axis of the oval cross-section of the passage is oriented vertically and a minor axis of the oval cross-section is oriented horizontally. The cross-section of the passage of the distal section therefore corresponds substantially to the cross-section of the distal side of the passage of the central section.

[0022] A particularly advantageous embodiment of the distal section provides that two passages for the passage of each one electrode arm of the electrode instrument of the hand-held device extend parallel to each other as well as parallel to the axial opening through the distal section or the body. Both passages have the oval or oblong hole-like cross-section described above. This distal section is designed to receive an electrode instrument with two electrode arms. By the configuration of the passages, the two arms of the electrode instrument can be moved forward and backward in a very advantageous manner along an axis of the instrument.

[0023] Furthermore, it is also conceivable that the opening and the at least one passage through the distal section are implemented as a common opening, so that the respective areas merge without being separated from each other by a wall.

[0024] A proximal section, in particular an adapter, of a main body for solving the problem stated at the beginning is described, a proximal section, in particular an adapter, of a main body for an electrosurgical hand-held device, in particular for a resectoscope, in which this section can be connected at a distal side in a detachable or fixed manner to a central section of the main body and is connected at a proximal side to a tube-shaped rod and in which the proximal section has an axial bore for the passage of a rod-shaped component of the hand-held device and in which the section has parallel to the axial bore at least one passage for the passage of an electrode arm of an electrode instrument of the hand-held device, characterized in that the at least one passage has a circular cross-section.This proximal section for an electrosurgical handheld device or resectoscope can be connected in a . removable at a distal side to a central section. At a proximal side, the proximal section is connected to a tube-shaped rod. This rod can, for example, be designed to accommodate other components of the handheld device, such as an optical system. This rod can also be referred to as a reinforcement tube or optical system guide. This adapter also has an axial bore, which is used to accommodate a rod-shaped component of the handheld device. In addition to this axial bore, the proximal section also has at least one passage for the passage of an electrode arm of an electrode instrument. The characterizing feature of the proximal section is that the at least one passage has a circular cross-section. This circular cross-section corresponds to the cross-section of the proximal side of the passage of the central section or the handle.

[0025] Another feature of the proximal section may be that the at least one passage has a seal, preferably a fluid seal, which is integrated into the proximal section or the adapter. During application of the hand-held device, it is conceivable that rinsing liquid passes through the passage. Since the passage is designed to receive an electrical contact from the electrode instrument, the risk of an electrical short circuit is to be avoided. The seal claimed here prevents an especially electrically conductive fluid from passing through the passage of the proximal section.

[0026] Furthermore, it is conceivable that the proximal section or the adapter is characterized by two passages, in which the two passages are designed in such a way that they serve to receive each one electrode arm of the electrode instrument. Both passages have a circular cross-section and are oriented parallel to each other as well as to the axial bore. Due to this symmetrical structure, the handling of the proximal section as well as other components of the hand-held device proves to be particularly simple and reliable.

[0027] Another solution to the problems formulated above is an electrosurgical hand-held device, in particular a resectoscope, with an electrode instrument and a main body which can consist of three sections, namely a central section, in particular a handle, a distal section, in particular a body, and a proximal section, in particular an adapter, in which a distal side of the proximal section can be removably or fixedly connected to a proximal side of the central section and a distal side of the central section can be removably or fixedly connected to a proximal side of the proximal section and axial bores and an opening or bore of the sections perfectly coincide in the connected state and in which the sections have parallel to the axial bores or the opening respectively at least one passage for the passage of an electrode arm of the electrode instrument, characterized in that a first cross-section of a funnel-shaped passage of the central section at the distal side of the central section is identical to an oval or oblong-hole-like cross-section of a passage of the distal section and a second cross-section of the funnel-shaped passage of the central section at the proximal side of the central section is identical to a circular cross-section of a passage of the proximal section, wherein in the connected state, the passages of the distal section, the central section and the proximal section join into an electrode arm guide. An electrosurgical handheld device according to the invention, which may be, for example, a resectoscope, thus has an electrode instrument and a central section, in particular a handle, of a main body, a distal section, in particular a body, of the main body and a proximal section, in particular an adapter, of the main body. The main body thus consists or can consist of three sections. In doing so, a distal side of the proximal section can be removably coupled to a proximal side of the central section and a distal side of the central section to a proximal side of the distal section. Said sections or components of the handheld device all have an axial bore and / or an opening. In the connected state, these bores or the opening of the sections or components overlap perfectly and serve, for example, to receive a tube-shaped rod.

[0028] In addition, the distal section, the central section and the proximal section have at least one passage parallel to the axial bores or the opening respectively, which also merge and serve to receive an electrode arm of an electrode instrument. This hand-held device is characterized in that a first cross-section of a funnel-shaped passage of the central section, in particular of the handle, at the distal side of the central section is similar, in particular identical, to an oval or oblong-hole-like cross-section of a passage of the distal section or of the body. A second cross-section of the funnel-shaped passage of the central section at the proximal side of the central section is identical to a circular cross-section of a passage of the proximal section or of the adapter. In the connected state of the three sections or components, the passages of the distal section, the central section and the proximal section coincide in an electrode arm guide. Viewed from the distal direction, the oval, oblong hole-like passage of the distal section is brought through the central section continuously into a passage with a circular diameter, in which this diameter is smaller than the diameter of the passage of the distal section.

[0029] An alternative solution to the problems formulated above is a hand-held electrosurgical device, in particular a resectoscope, with an electrode instrument and a main body, which has three sections, namely a distal section, a proximal section and a central section, in which the sections have a bore for receiving an optical system and parallel to this at least one passage for the passage of an electrode arm of the electrode instrument, characterized in that the passage consists of three passage sections, in which a first cross section of the passage section through the distal section is made oval or oblong hole-like and a third cross section of the passage section through the proximal section is made circular and in which a second cross section of the passage section through the central section is funnel-shaped, in which the funnel-shaped cross section continuously transforms from the first cross section to the third cross section. An electrosurgical handheld device according to the invention, in particular a resectoscope, thus has an electrode instrument and a main body. This main body in turn has three sections, namely a distal section, a proximal section and a central section. The sections have a common axial bore for receiving an optical system and parallel to this at least one passage for the passage of an electrode arm of the electrode instrument. The at least one passage consists of three passage sections, wherein a first cross-section of a passage section through the distal section is formed in an oval or oblong hole-like manner and a third cross-section of a passage section through the proximal section is formed in a circular manner and wherein a second cross-section of a passage section through the central section is formed in a funnel-shaped manner, wherein the funnel-shaped cross-section transforms continuously from the first cross-section to the third cross-section. Said three sections together form the main body formed in one piece.

[0030] It is preferably provided according to the invention that the distal section, the central section and the proximal section have two passages, which are oriented parallel to each other and to the axial bore and serve to receive two electrode arms of the electrode instrument. Two parallel electrode arm guides, which are designed for simple reception of the proximal ends of the electrode arms, are thus created through the individual passages of said sections or components. In particular, due to the oblong hole-like configuration of the passages in the distal section and the funnel-shaped passages in the central or handle section, the proximal ends of the electrode arms can be coupled in a particularly advantageous manner both mechanically and electrically to the hand-held device.

[0031] Another advantageous embodiment of the invention may provide that the at least one electrode arm of the electrode instrument has an offset. This The offset can be implemented as a stepped transition between a proximal and a distal area of the electrode arm, in which the two areas run parallel to each other. This offset can be implemented in particular in an S-shape and serves to move the proximal end of the electrode arm implemented as an electrical contact away from the axial bore.

[0032] Furthermore, a preferred feature of the hand-held device is that the at least one passage of the distal section or the passage section with the oval or oblong hole-like cross-section is designed in such a way that the offset of the electrode arm in this passage or in the passage section can be moved forward and backward in the proximal and distal direction. The distal areas of the electrode arms are designed straight, so that the rinsing liquid is not disturbed in its flow behavior. A laminar flow is thus formed in front of the distal end of the hand-held device.

[0033] Preferably, it is further provided that the second cross section of the at least one funnel-shaped passage of the central section or of the passage section at the proximal side of the central section and the circular cross section of the at least one passage of the proximal section is designed to receive the proximal region of at least one electrode arm. In this at least one passage of the proximal section, a seal, preferably a fluid seal, can be integrated, which can be fixed in the connected state of the central section and the proximal section by the proximal side of the central section. This seal prevents rinsing liquid from flowing during treatment through the passages in the proximal direction to the electrical contacts of the electrode arms.

[0034] Furthermore, it is conceivable that the main body is made with the three sections as an integral metal body through which the bore and the passage sections extend.

[0035] A preferred embodiment of the invention is explained in more detail below with the aid of the drawing. This shows:

[0036] [Fig-1] a schematic representation of a resectoscope,

[0037] [Fig.2] a schematic representation of an electrode instrument,

[0038] [Fig.3] an exploded representation of a distal section, a central section and of a proximal section,

[0039] [Fig.4] a view of a distal end of the central section, and

[0040] [Fig.5] a section through the distal section, the central section and the section proximal.

[0041] An example of a possible embodiment of a hand-held electrosurgical device, namely a resectoscope 10, is shown very schematically in [Fig. 1]. The resectoscope 10 has a carrier 11, at which an elongated rod 12, in tube shape can be fixed. This rod 12 is shown hatched in [Fig.l] and is fixed with a proximal end at a main body 52 of the conveyor 11.

[0042] The conveyor 11 has, in addition to the main body 52, a handle unit 14. This handle unit 14 is associated with a central section 15. This section 15 may have a handle, in particular a removable handle. While the central section 15 is arranged fixedly or removably at the main body 52, in the embodiment of the conveyor 11 shown here, a gripping means 16 is associated with a contact body 17. It is conceivable in this case that the gripping means 16 is screwed firmly onto the contact body 17.

[0043] The contact body 17 is guided in a sliding manner on a tube-shaped optical system guide 18. Since the contact body 17 can be moved back and forth on the optical system guide 18 along a longitudinal direction of the resectoscope 10 or a longitudinal axis of the shaft 12, the contact body 17 is also referred to as a carriage. While the optical system guide 18 can be detachably connected with a distal end via a proximal section 19, in particular an adapter, from the main body 52 to the central section 15, an optical system guide plate 20 is fixed to a proximal end of the optical system guide 18. The tube-shaped optical system guide 18 extends through the optical system guide plate 20, so that the optical system guide 18 is accessible from the proximal side.

[0044] The gripping means 16 or the contact body 17 are connected to the optical system guide plate 20 by means of a spring element 21. This spring element 21 can be a tension spring or a compression spring depending on the type of construction of the conveyor 11.

[0045] Starting from the main body 52 or from a distal section 13, in particular from a body, a tube-shaped inner tube 22 extends in the distal direction. An electrode instrument 23 extends parallel to the inner tube 22. This electrode instrument 23 is guided through the distal section 13, the central section 15 and the proximal section 19 or through the three through sections 49, 50 and 51 and is releasably coupled with at least one proximal contact mechanically to the contact body 17. At a distal end, the electrode instrument 23 has an electrode 24. This electrode 24 can be supplied with an electrical high-frequency voltage. The diseased tissue can be manipulated or cut by means of a thermal plasma formed at the electrode 24. To do this, the operator moves the gripping means 16 having a thumb ring 25 relative to the central section 15.For stabilization of the electrode instrument 23, it can be guided at the inner tube 22 through guides 26.

[0046] To carry out the operation, a rod-shaped optical system is guided through the inner tube 22 or through the optical system guide 18. A distal end, not visible here, of this optical system is directed towards the electrode 24 so that the operator always has the manipulation of the tissue in view. This optical system can be a rod lens system or a glass fiber. An eyepiece 27 or a camera is located, as shown in [Fig. 1], at the proximal end of the optical system.

[0047] [Fig. 2] shows an example of a possible embodiment of the electrode instrument 23 for the resectoscope 10. This electrode instrument 23 consists substantially of two electrode arms 28, 29, which are connected to each other at a distal region 30 by means of the electrode 24. The proximal ends 31, 32 of the two electrode arms 28, 29 serve as described above both for mechanical coupling and for electrical coupling with the contact body 17. The two electrode arms 28, 29 are oriented parallel both to each other and relative to an axis 33. For stabilization of the two arms 28, 29 as well as for guiding the electrode instrument 23 on the inner tube 22, the electrode arms 28, 29 are connected to each other by through the two guides 26 in the embodiment shown here.

[0048] The distal regions 30 of the two electrode arms 28, 29 are moved parallel to each other relative to proximal regions 34 by a stepped offset 35. This S-shaped offset 35 serves to bring the two proximal ends 31, 32 of the electrode arms 28, 29 into a preferred position for mechanical and electrical coupling. It has proven to be particularly advantageous to place this offset 35 in the proximal region 34, since a particularly good flow behavior of the rinsing liquid can thus be formed in the distal region 30.

[0049] [Fig. 3] shows an exploded view of the distal section 13, the central section 15 and the proximal section 19. The proximal section 19 is firmly connected to the tube-shaped optical system guide 18 in the embodiment shown here. At the proximal end, this optical system guide 18 is connected to the optical system guide plate 20. As described above, in the assembled state of the resectoscope 10, the contact body 17 is pushed via this optical system guide 18. The other features of the optical system guide plate 20 are not discussed in further detail here, since they are not relevant to the nature of the claimed invention.

[0050] The proximal section 19 or the adapter has an axial bore 36, which extends directly into the optical system guide 18. Similarly, the central section 15 or the handle has an axial bore 37. The distal section 13 or the body also has an axial opening or bore, which is however not visible in [Fig. 3]. In the assembled state of the distal section 13, the central section 15 and the proximal section 19, the axial bores 36, 37 and the opening perfectly overlap, so that they form a common passage for, for example, the optical system.

[0051] The inner tube 22 is associated with the distal section 13 in the distal direction. This inner tube 22 constitutes a direct extension of the axial bores 36, 37 as well as of the optical system guide 18.

[0052] The distal section 13, the central section 15 and the proximal section 19 can be removably connected to each other by three screws 38, as shown in [Fig. 3]. For this purpose, a distal side 41 of the proximal section 19 is assembled to a proximal side 42 of the central section 15 and a distal side 43 of the central section 15 is assembled to a proximal side 44 of the distal section 13. Then, the three screws 38 are guided from the proximal direction through three corresponding bores 39 in the proximal section 19, through three corresponding bores 40 ([Fig. 4]) in the central section 15 and screwed into three corresponding bores in the distal section 13 with an internal thread, which are not shown here. For repair or maintenance purposes, the three sections 13, 15, 19 or the three components can be dismantled again by loosening the screws 38.

[0053] In the assembled state of the section 13, the section 15 and the section 19, the electrode instrument 23 is guided with the two proximal ends 31, 32 through these three sections 13, 15, 19 for mechanical and electrical coupling, in order to then engage in the contact body 17. For this purpose, the proximal section 19 has circular passages 45. These passages 45 are arranged at a distance from each other and parallel to the axial bore 36. So that no rinsing liquid can escape during use of the resectoscope 10, a sealing ring 46 can be assigned to these two passages 45. These two sealing rings 46 are wedged or blocked in the assembled state between the distal side 41 of the proximal section 19 and the proximal side 42 of the central section 15.

[0054] The central section 15 also has two parallel passages 47. These passages 47 are made funnel-shaped, in which a first cross-section of the passages 47 is made oval or oblong-hole-like at the distal side 43 of the central section 15 ([Fig. 4]). A second cross-section of the passages 47 has at the proximal side 42 of the central section 15 the same cross-section or diameter as the passages 45 of the proximal section 19. The cross-section of the passages 47 tapers continuously or funnel-shaped from the distal side 43 to the proximal side 42 of the central section 15 ([Fig. 4]).

[0055] The distal section 13 also has two passages 48. These passages 48, not visible in [Fig. 3], are also oriented parallel to each other, just as parallel to the bores 37, 36 or to the inner tube 22 and have an oval or oblong hole-like cross-section, which coincide with the cross-section of the passages 47 of the central section 15 at the distal side 43 of the central section 15. The passages 48 of the distal section 13 do not change their cross-section.

[0056] In the assembled state, the passages 45, 47 and 48 overlap perfectly, so that they form a continuous guide for the two electrode arms 28, 29. As shown in the sectional representation according to [Fig. 5], the electrode instrument 23 is guided through the passages 45, 47 and 48 in such a way that the offset 35 of the electrode arms 28, 29 lies straight in the oblong hole-like passages 48 of the distal section 13.

[0057] It is also very clear from [Fig. 5] how the funnel-shaped passage 47 of the central section 15 serves as an insertion aid for the proximal end 31 of the electrode arm 28. When inserting the electrode arm 28 into the passage 48, the end 31 is automatically guided into the passage 45 by the funnel-shaped configuration of the passage 47. In doing so, the passage 45 is made fluid-tight by the seal 46 of the passages 47, 48.

[0058] The passages 48 are designed in such a way that during an axial displacement of the electrode instrument 23, the offset 35 is movable in the distal and proximal direction. This is necessary to carry out, as described above, the cutting movement by means of the electrode 24. By the displacement of the offset 35 in the main body 52, the distal area of the inner tube 22 remains free for undisturbed flow of the rinsing liquid.

[0059] While the passages 45, 48 can be manufactured in the metal proximal section 19 or in the metal main body 52 by drilling or milling or wire erosion, the more complicated funnel-shaped passage 47 of the central section 15 can be manufactured by an injection molding process. Thus, by the configuration described herein of the individual components, manufacturing is simplified or costs are saved and the flow behavior of the flushing liquid is improved.

[0060] Alternatively, however, it is also provided that the main body 52 is formed from a single piece, in which the main body 52 can be subdivided into the three sections 13, 15, 19. In this embodiment according to the invention, the axial bores 36, 37 as well as the passage section 49, 50, 51 are made from end to end through the entire main body 52. These integral sections 13, 15, 19 of the main body 52 are all manufactured from a metal and this preferably by drilling, turning, milling and / or erosion, in particular die-sinking erosion.
