Description

Title of the invention: Medical device comprising a needle and a case

[0001] The invention relates to a medical device comprising a needle extending between a free distal end and a proximal end mounted in a base.

[0002] In a particular application, such a medical device can be integrated into a sampling system, which includes at least one bag in fluidic communication with the needle of said device, the latter being arranged to draw blood to be collected in said bag.

[0003] To ensure the sterility of the needle before its use, and to limit the risks of accidental needlestick injury to the practitioner, such devices generally include a case which has a housing in which said needle is placed, being associated with the base via means arranged to break when said case is removed, and thus form an indicator of first opening of said device.

[0004] To achieve this, devices are known in which the case is held on the base by means of a ring of material forming a first opening indicator, being arranged to be able to be broken by relative rotation between the case and the base, at the moment when the practitioner wishes to use the needle.

[0005] To facilitate its breakage, the ring of material generally has a discontinuous circumferential shape, being formed by angularly equidistributed bridges of material.

[0006] With such an arrangement, there is a risk of outside air entering the case via the angular spaces separating the material bridges, and therefore of loss of sterility of the needle disposed in said case.

[0007] To overcome these drawbacks, document JP-2017 / 012638 describes a device in which the base has a distal bearing surface on which the sheath is associated so as to form between them a sealed radial interference, in order to prevent the penetration into the receiving housing of the needle of outside air passing through the frangible crown.

[0008] This arrangement is not entirely satisfactory, as it requires forming a sealing interface on the distal side of the frangible crown, which can degrade the ease of removal of the case due to the tightening of the latter around the base.

[0009] The invention aims to improve the prior art by proposing in particular a medical device in which the sheath is associated with the base of the needle in a simple way by forming a first opening indicator, while ensuring good sealing without hindering the removal of the sheath at the time of use.

[0010] To this end, according to a first aspect, the invention proposes a medical device comprising a needle extending between a free distal end and a proximal end mounted in a base, said device comprising a sheath having a housing in which said needle is disposed, said sheath being held on the base by means of a ring which is axially immobilized around a proximal bearing surface of said base by forming between said ring and said proximal bearing surface a proximal interface arranged to ensure rotational locking of the base / ring assembly, said ring having a distal edge which is connected to a proximal edge of the sheath by means of a ring of material which is arranged to be breakable by relative rotation between the sheath and the base / ring assembly in order to be able to use the needle,The seal of the case housing is achieved through a combination of factors: the proximal interface and the continuous ring of material.

[0011] According to a second aspect, the invention proposes a sampling system comprising at least one bag in fluidic communication with the needle of such a medical device, said needle being arranged to draw blood to be collected in said bag.

[0012] Other features and advantages of the invention will become apparent from the following description, made with reference to the accompanying figures, in which:

[0013] [Fig.l],

[0014] [Fig.2] and

[0015] [Fig.3] represent a medical device according to an embodiment of the invention, respectively in perspective ([Fig.1]), in lateral view ([Fig.2]) and in axial section ([Fig.3]), and in which the case is associated with the base of the needle before use;

[0016] [Fig.3a] and

[0017] [Fig.3b] represent the medical device of the preceding figures, following respectively an enlargement of zone A of [Fig.3] ([Fig.3a]) and the radial section plane BB of figures 2 and 3 ([Fig.3b]);

[0018] [Fig.4a] and

[0019] [Fig.4b] represent the medical device of the preceding figures during the removal of the case for use of the needle, respectively in perspective ([Fig.4a]) and in axial section ([Fig.4b]);

[0020] [Fig.5] represents a sampling system in which a medical device according The previous figures are integrated.

[0021] In relation to these figures, a medical device 1 comprising a needle 2 extending between a free distal end 2a and a proximal end 2b mounted in a hub 3 is described below, as well as a sampling system integrating such a medical device 1 and at least one bag 4 in fluidic communication with needle 1 of said medical device, in order to be able to draw blood to be collected in said bag.

[0022] In the description, the terms for positioning in space are taken with reference to the medical device 1 as shown in the figures. Thus: - the term "axial" refers to a direction parallel to the maximum dimension of needle 2, represented by the X axis in figures 2, 3 and 4b, and the term "radial" refers to a direction perpendicular to this X axis; - the terms "proximal" and "distal" are taken in reference to the direction of introduction of needle 2 into a part of a donor's body, in particular a vein to collect blood from said donor, and to spatial arrangements represented respectively on the right and left in figures 2, 3 and 4b.

[0023] The base 3 has a proximal portion 3a on which is mounted a means 5 for making fluidic communication of the needle 2 with a sampling bag 4, in particular in the form of a flexible and sterilizable tube, as well as a distal portion 3b from which the free end 2a of said needle extends axially.

[0024] To ensure the sterility of the needle 2 before its use and to limit the risks of accidental puncture for the practitioner, the device 1 includes a case 6 which has a housing 7 in which said needle is placed.

[0025] The case 6 is held on the base 3 by means of a ring 8 which is axially immobilized around a proximal bearing 9a formed on the distal portion 3b of said base, forming between said base and said proximal bearing a proximal interface 10a arranged to ensure rotational locking of the base 3 / ring 8 assembly.

[0026] In relation to [Fig.3a], the ring 8 is axially immobilized around the proximal bearing 9a by means of a mechanism formed at the proximal interface 10a, said mechanism comprising a radial projection lia, 11b which is engaged in a groove 12a, 12b.

[0027] In particular, the ring 8 includes a distal groove 12a which is extended axially by a proximal projection 1a, said distal groove and said proximal projection being arranged to cooperate radially with respectively a distal projection 11b and a proximal groove 12b formed for this purpose in the base 3.

[0028] The base 3 further has a proximal collar 13 which extends radially around the proximal edge 14a of the proximal bearing surface 9a, in particular being formed in the axial extension of the proximal groove 12b, the proximal edge 15a of the ring 8 being disposed in axial support against the collar 13.

[0029] Furthermore, to secure the base 3 / ring 8 assembly in rotation, the proximal interface 9a between said ring and said proximal bearing has a geometry which is not of revolution, being for example parallelepiped, and in particular square, as shown in [Fig.3b].

[0030] The ring 8 has a distal edge 15b which is connected to a proximal edge 16 of the sheath 6 by means of a ring of material 17 forming a first opening indicator, and which is arranged to be able to be broken by relative rotation between the sheath 6 and the base 3 / ring 8 assembly, in order to be able to use the needle 2.

[0031] Thus, before using the medical device 1, the practitioner can verify its reliability, and in particular its watertightness, by visually inspecting the condition of the material ring 17, so that the device can be destroyed if the material ring is damaged. The breakable feature also confirms that the sheath 6 has never been opened, since if the material ring 17 had been broken, the sheath would no longer be in place and the needle 2 would be exposed.

[0032] Advantageously, the case 6 and the ring 8 are formed in one piece with the material crown 17, in particular by molding a polymer material, which makes it easier to manufacture.

[0033] In particular, the case 6 / ring 8 / crown 17 assembly can be made of polypropylene. According to an advantageous embodiment, a particularly suitable material is a homopolymer polypropylene with high rigidity having a deformation at break of less than 25% and a Charpy impact resistance of less than 5 kJ / m2 (Charpy Impact Strength Notched standard for impact resistance ISO179).

[0034] This polymer material can also: - contain a dye, in order to give the practitioner a visual indicator relating to the needle, for example its gauge, based on said dye; and / or - be transparent, so as to allow the practitioner to visually check the appearance of needle 2, and in particular its integrity, through the sheath 6, without having to remove said sheath.

[0035] The sealing of the housing 7 is achieved in combination at the proximal interface 10a and by the continuous material ring 17, which prevents any penetration of outside air into the housing via said ring and upstream of said ring, and thus guarantees the sterility of the needle 2 before the rupture of said ring and the removal of the sheath 6.

[0036] As shown in particular in [Fig.3a], the radial interference of the mounting of the ring 8 on the proximal bearing 9a is arranged to ensure the sealing of their proximal interface 10a, in particular by radial play-free fitting of the ring 8 around said proximal bearing.

[0037] The crown of material 17 has a thickness less than that of the ring 8 and the case 6, and in particular between 30 and 100 pm, in order to facilitate its breakage when the practitioner rotates the case 6 around the axis X relative to the base 3.

[0038] In relation to the figures, the base 3 further presents a distal span 9b which is formed on its distal portion 3b by axially extending the proximal span 9a, and which is disposed in the case 6 by forming between them a distal interface 10b, the external dimension of said distal span being less than that of said proximal span.

[0039] Advantageously, as shown in [Fig.3a], the distal interface 10b forms a radial gap 18 between the distal bearing surface 9b and the sheath 6, which makes it possible to avoid any resistance at the level of said radial interface during the rotation of the sheath 6 to break the crown 17, and then during the removal by axial translation of said sheath after said break.

[0040] In particular, the presence of a continuous ring of material 17 makes it possible to avoid the need for an additional sealing interference between the radial bearing 9b and the case 6, insofar as such a ring 17 prevents any penetration of outside air between the base 3 and said case.

[0041] To further facilitate the rotation of the case 6, and then its removal from the base 3, the aforementioned distal interface 10b has a geometry of revolution.

[0042] Similarly, the ring of material 17 is arranged around the proximal edge 20 of the distal span 9b, forming between them a radial annular gap 19.

[0043] In relation to figures 3 and 3a, the proximal edge 20 of the distal span 9b is connected to the distal edge 14b of the proximal span 9a by a radial step 21, which makes it possible to form an annular gap 19 of sufficiently large dimensions to facilitate rotation and axial withdrawal of the case 6 when using the medical device 1.

[0044] The base 3 has a manual gripping area which, in the figures, includes two concave impressions 22 formed on either side of the X axis and between the proximal 3a and distal 3b portions of said base, in order to allow the practitioner to grasp and manipulate said base by pinching two of his fingers on said impressions.

[0045] In particular, as shown in [Fig.2], each impression 22 includes ribs 23 equidistant axially along said impression, in order to limit the slippage of the practitioner's fingers when grasping the base 3.

[0046] Furthermore, the case 6 is equipped with at least one manual gripping organ 24 which is arranged to facilitate its manipulation in rotation.

[0047] In the figures, the organ 24 is formed at the proximal edge 16 of the sheath and includes at least one fin 24 which extends radially from the periphery of said sheath.

[0048] Advantageously, the case 6 is equipped with two diametrically opposed fins 24, in order to allow the practitioner to rotate said case axially by placing two of his fingers on said fins.

[0049] To open the medical device 1, the practitioner can grasp the base 3 with one of his hands, pinching it between two of his fingers at the level of its impressions 22 and then, while holding said base, grasp the fins 24 with two fingers of his other hand, in order to rotate the case 6 by pressing on said fins in a direction of rotation indicated by arrows F on the [Fig.4a], until the ring of material 17 is broken.

[0050] In particular, the medical device 1 may include a visual indicator of the direction of rotation to be applied to the case 6, for example in the form of arrows 25 printed circumferentially on the outer periphery of the ring 8, as shown in [Fig.2],

[0051] A sampling system incorporating such a medical device 1 is now described, said system comprising at least one bag 4 in fluidic communication with the needle 2 of said medical device, said needle being arranged to draw blood to be collected in said bag.

[0052] In [Fig.5], the collection system includes a primary bag 4 for collecting whole blood from a donor, and with which the needle 2 of the medical device 1 is made in fluidic communication via a flexible and sterilizable tube 5.

[0053] In particular, the tubing 5 is connected to a sampling device 26 which allows a quantity of blood to be taken from the donor for other medical uses, including additional blood tests prior to the transfusion of said blood to a recipient, the primary blood collection bag 4 being connected to said sampling device by a tubing 27 similar to that described previously.

[0054] To collect blood from a donor, the practitioner inserts the needle 2 of the medical device 1 into a vein of said donor, for example a vein in an arm, the blood collected by said needle then being conveyed to the primary bag 4 via the tubing 5, 27.

[0055] The represented collection system further comprises two satellite bags 28, 29 each connected to the primary bag by means of a flexible and sterilizable tube 30, 31 respectively, in particular similar to the tubes 5, 27, said satellite bags each being arranged to collect a blood product from the whole blood contained in said primary bag.

[0056] In particular, the collection system includes an upper satellite bag 28 - respectively lower 29 - for receiving the plasma - respectively the red blood cell concentrate - whole blood collected in the primary bag 4, and connected for this purpose via a flexible tube which is associated in the upper -respectively lower - part of said primary bag.

[0057] The primary bag 4 may contain an anticoagulant, such as, for example, a CPD (Citrate Phosphate Dextrose) type solution, to help preserve the collected whole blood. Similarly, the lower satellite bag 29 may contain an additive solution for preserving red blood cells, for example, a SAGM (Saline Adenine Glucose Mannitol) type solution.

[0058] After blood is drawn from a donor, the system containing the whole blood drawn from said donor, and mixed with the anticoagulant solution in the primary bag 4, is placed in a centrifuge cup to be centrifuged, so as to separate the blood into three layers: a supernatant containing plasma; an intermediate layer known as a "buffy coat" comprising a mixture of platelets, plasma and red blood cells; and a pellet containing the red blood cell concentrate.

[0059] By applying pressure to the primary pouch 4, the plasma is sent into the upper satellite pouch 28 and the red blood cell concentrate is sent into the lower satellite pouch 29, the "buffy coat" layer remaining in the primary pouch 4.
