Abstract
Description
[Technical field]
[0001] This application claims priority from Korean Patent Application No. 10-2021-0042250, filed on March 31, 2020, and all contents disclosed in the specification and drawings of the corresponding application are incorporated by reference into this application.
[0002] The present invention relates to a sleeve gastrectomy assist device, and more particularly to a sleeve gastrectomy assist device that is inserted inside the stomach and assists in a resection operation. [Background technology]
[0003] Generally, sleeve gastrectomy is a representative surgical method for severe obesity. Sleeve gastrectomy is a laparoscopic surgery that uses special tools such as an observation camera, a stapler, and a traction device to cut the stomach, leaving only the shape of a thin stomach with a certain diameter and a volume of 100-150mL. Unlike other surgeries, sleeve gastrectomy directly accesses internal organs without external incisions, so recovery after surgery is quick, and it is a surgery that simply reduces the size of the stomach, so it has the advantage of being less stressful for the user and having the lowest incidence of complications compared to other surgical methods. Sleeve gastrectomy is performed by inserting a bougie into the stomach cavity through the mouth and cutting the stomach to fit the shape of the bougie using a stapler and a traction device.
[0004] However, conventional surgical tools for sleeve gastrectomy do not have a means to quantitatively provide necessary information during resection, and therefore the process of cutting and pulling the stomach is dependent on the technique of the user without a set standard, which causes a large difference between the shape of the remaining stomach after surgery and the target shape of the stomach. If the volume of the remaining stomach is too small, the side effect of gastric esophageal reflux occurs, and if the volume of the remaining stomach is too large, the patient's weight control fails or weight regain occurs.
[0005] The background art of the present invention is disclosed in Korean Patent Publication No. 10-2020-0103489 (published on September 2, 2020, title of invention: Tube for guiding gastric resection). [Prior art documents] [Patent documents]

[0041] FIG. 1 is an installation state diagram illustrating, in outline, the installation state of a sleeve gastrectomy auxiliary device according to one embodiment of the present invention; FIG. 2 is an oblique view illustrating in outline the configuration of a sleeve gastrectomy auxiliary device according to one embodiment of the present invention; and FIG. 3 is a side view illustrating in outline the configuration of a sleeve gastrectomy auxiliary device according to one embodiment of the present invention.
[0042] 1 to 3, a sleeve gastrectomy auxiliary device 1 according to one embodiment of the present invention includes a guide unit 100, an expansion unit 200, a first measurement unit 300, and a monitor unit 400. As shown in FIG.
[0043] The guide unit 100 is inserted into the stomach 2 through the patient's oral or nasal cavity and the esophagus during sleeve gastrectomy. The guide unit 100 supports the expansion unit 200 described below and guides the expansion unit 200 to be inserted into the stomach 2. The guide unit 100 may include a flexible material such as silicone or synthetic resin so that its shape can be changed. As a result, the guide unit 100 can be more smoothly inserted into the stomach 2, and can flex and curve in response to the curvature and shape of the stomach, thereby dynamically determining the shape of the stomach 2 remaining after the resection surgery.
[0044] The guide part 100 according to one embodiment of the present invention is formed in the form of a tube containing a silicone material. The diameter of the guide part 100 may be formed to be smaller than the diameter of the patient's esophagus. The length of the guide part 100 may be designed to have various values within a range of a length that can be inserted into the stomach 2. The end of the guide part 100 is formed to be bulged with a predetermined curvature, so that the insertion into the stomach 2 can be performed more smoothly.
[0045] The expansion part 200 is installed outside the guide part 100, and its volume is variable by expanding or contracting according to the amount of air inside. The expansion part 200 determines the volume of the stomach 2 remaining after resection by adjusting its volume to have a predetermined volume inside the stomach 2. The expansion part 200 may include a flexible natural resin such as rubber or latex, or a synthetic resin material such as chloroprene. The expansion part 200 according to an embodiment of the present invention is formed in the form of a cylindrical rubber membrane and is disposed so as to surround the outer circumferential surface of the guide part 100. Both ends of the expansion part 200 are fixed to the outer circumferential surface of the guide part 100 by a fixing means having a ring shape. The expansion part 200 is connected to the injection part 10 and receives injection of air therein. As air is injected therein, the expansion part 200 expands in volume outward from the guide part 100. In this case, the expansion part 200 expands in a form in which the distance between two points arbitrarily set on the surface increases between both ends that are in close contact with the outer circumferential surface of the guide part 100, almost like a balloon.
[0046] The injection unit 10 is connected to the expansion part 200 and is provided to supply air to the inside of the expansion part 200 or to exhaust air from the inside of the expansion part 200. The injection unit 10 according to an embodiment of the present invention is formed to have the shape of a hollow can with an open interior, and includes a hose member having one side connected to the inside of the expansion part 200, and an injection member formed in the form of a syringe, air pump, etc., connected to the other side of the hose member to inject air into the inside of the expansion part 200 or to exhaust air from the inside of the expansion part 200.
[0047] The first measuring unit 300 is disposed inside the expansion unit 200 and measures a pressure value applied to the expansion unit 200. More specifically, the first measuring unit 300 measures a pressure applied to the expansion unit 200 from the outside of the expansion unit 200 by the resection tool 3 pressing the stomach 2 during the resection operation. The first measuring unit 300 is connected to the monitor unit 400 described below and transmits the measured pressure value to the monitor unit 400. The first measuring unit 300 is provided so that its shape can be changed in conjunction with a volume change of the expansion unit 200. Accordingly, the first measuring unit 300 can absorb an external force such as tension applied when the volume of the expansion unit 200 changes by changing its shape. The first measuring unit 300 may be formed in a form extending along the length direction of the expansion unit 200. A plurality of first measuring units 300 may be provided, and the plurality of first measuring units 300 may be disposed at predetermined intervals along the circumferential direction of the expansion unit 200. In this case, each of the first measuring portions 300 may extend to a different length.
[0048] The first measuring unit 300 according to the embodiment of the present invention includes a first measuring member 310 and a connecting unit 320 .
[0049] The first measuring member 310 is attached to the inner surface of the expansion part 200 to measure the pressure applied to the expansion part 200. A plurality of first measuring members 310 are provided to measure the pressure applied to the expansion part 200 individually. Accordingly, the first measuring member 310 can more accurately measure the pressure value applied to each position of the expansion part 200. The first measuring member 310 according to an embodiment of the present invention may be formed in the form of a contact type pressure sensor such as a load cell that outputs an electric signal according to the pressure value applied to the outside of the expansion part 200. The first measuring member 310 may be formed in the form of a flexible thin film so that it can be closely attached to the inner surface of the expansion part 200 regardless of bending of the expansion part 200.
[0050] The first measuring members 310 provided in each first measuring unit 300 may be spaced apart at a predetermined interval along the length of the expansion unit 200. In this case, the first measuring members 310 provided in each first measuring unit 300 may be spaced apart at the same interval, and the first measuring members 310 provided in different first measuring units 300 may be arranged along the same circumference of the expansion unit 200. That is, as shown in the drawing, the first measuring members 300 may be arranged to form a ring shape on different circumferences of the expansion unit 200 spaced apart along the length of the expansion unit 200. Accordingly, the first measuring members 310 may enable the monitor unit 400 described later to determine the state of contact of the expansion unit 200 with the inner wall of the stomach 2.
[0051] The connecting portion 320 is connected to the adjacent first measuring members 310. The connecting portion 320 is made of a conductive material such as copper, electrically connects the adjacent first measuring members 310, and transmits an electrical signal output from the first measuring members 310 to the monitor unit 400 described below. The connecting portion 320 is provided in a plurality of pieces and sequentially connects the first measuring members 310 arranged at a predetermined interval along the length direction of the extension portion 200. More specifically, the remaining connecting portions 320 except for the rearmost connecting portion 320 are connected at both ends to a pair of first measuring members 310, respectively, and the rearmost connecting portion 320 has one end connected to the rearmost first measuring member 310 and the other end connected to the monitor unit 400. The plurality of connecting portions 320 may be arranged in a direction parallel to the length direction of the extension portion 200.
[0052] The connecting part 320 is configured to expand and contract according to the distance between the adjacent first measuring members 310. More specifically, the connecting part 320 is configured to absorb the change in the distance between the adjacent first measuring members 310 caused by the volume change of the expansion part 200 by the expansion and contraction action. Accordingly, the connecting part 320 can be prevented from breaking due to the tension applied when the distance between the adjacent first measuring members 310 becomes larger.
[0053] 4a and 4b are enlarged views showing a schematic configuration of a connecting portion according to an embodiment of the present invention.
[0054] 4a and 4b, the connecting part 320 may be formed in the form of a wire extending in a zigzag shape along the length direction. More specifically, the connecting part 320 is compressed in a zigzag shape prior to the expansion of the volume of the expansion part 200 to pre-accumulate an extendable length, and extends in a straight line to extend its length as the volume of the expansion part 200 is expanded. As a result, the connecting part 320 can smoothly perform an expansion and contraction operation without taking up an excessive volume.
[0055] 5a and 5b are enlarged views that diagrammatically show the configuration of a connecting portion according to another embodiment of the present invention.
[0056] 5a and 5b, a connecting part 320 according to another embodiment of the present invention may include a connecting member 321.
[0057] The connecting member 321 is formed to expand in a direction parallel to the direction in which an external force is applied. A plurality of connecting members 321 are provided and connected to each other in a lattice shape. The connecting member 321 according to an embodiment of the present invention is formed in a ring-shaped wire shape. When an external force is applied in one direction (up and down direction in FIG. 5) as the distance between adjacent first measuring members 310 increases, the diameter of the connecting member 321 expands in the corresponding direction and the diameter in a direction perpendicular to the corresponding direction decreases. The plurality of connecting members 321 are connected in a shape in which four different connecting members 321 are entangled and repeated on the top, bottom, left and right of any one connecting member 321. The plurality of connecting members 321 are electrically connected to each other so as to transmit an electrical signal output from the first measuring member 310.
[0058] The monitor unit 400 is connected to the first measuring unit 300, determines resection position information based on the pressure value measured by the first measuring unit 300, and provides the determined resection position information to a user. Here, the resection position information may be exemplified by information regarding the distance from the expansion unit 200 to the resection tool 3.
[0059] FIG. 6 is a flow chart showing a schematic configuration of a monitor unit according to an embodiment of the present invention.
[0060] Referring to FIG. 6, a monitor unit 400 according to an embodiment of the present invention includes a control unit 410 and a display unit 420.
[0061] The control unit 410 determines the resection position information based on the pressure value measured by the first measuring unit 300. More specifically, the control unit 410 converts the pressure value measured by the first measuring unit 300 into information on the distance between the expansion unit 200 and the resection tool 3 to determine the resection position information of the resection tool 3. That is, the control unit 410 determines the resection position information by determining the resection position section D according to the magnitude of the pressure value measured by the first measuring unit 300. The control unit 410 according to an embodiment of the present invention may be exemplified by a microprocessor, a computer, etc. connected to the first measuring member 310 and receiving the pressure value measured from the first measuring member 310. The control unit 410 may receive the pressure value measured from the first measuring member 310 by wire through the connection unit 320. In this case, the control unit 410 may determine the resection position information of the resection tool 3 using the average value of the pressure measured by the plurality of first measuring members 310.
[0062] The resection position section D refers to a section obtained by dividing the pressure values according to their magnitude, based on the initial pressure P0 value inside the stomach 2 measured by the first measuring unit 300 immediately after the insertion of the guide unit 100. More specifically, the resection position section D refers to a section between a pair of different pressure values among the pressure sections measured by the first measuring unit 300 by changing the pressurization or resection position of the stomach 2 with the resection tool 3 after the insertion of the guide unit 100.
[0063] FIG. 7 is a diagram illustrating an ablation position section determined by a control unit according to an embodiment of the present invention.
[0064] Referring to FIG. 7, the resection position section D according to an embodiment of the present invention includes an approach required section D1, an advanceable section D2, and a separation required section D3.
[0065] The approach necessary section D1 indicates a case where the pressure measured by the first measuring unit 300 is equal to or greater than the initial pressure P0 and equal to or less than the first set pressure P1. Here, the first set pressure P1 is a pressure value measured at the maximum distance from the expansion unit 200 to the resection tool 3 that can form the size of the remaining stomach 2 targeted by the user, and can be variously modified in design depending on the shape and size of the stomach 2. In other words, the approach necessary section D1 corresponds to a section where the control unit 410 determines that the resection tool 3 is too far away from the expansion unit 200 based on the pressure value measured by the first measuring unit 300.
[0066] The separation-required section D3 indicates a case where the pressure measured by the first measuring unit 300 is equal to or greater than the second set pressure P2. Here, the second set pressure P2 is a pressure value measured at the minimum distance between the expansion unit 200 and the resection tool 3 that can form the size of the remaining stomach 2 targeted by the user, and can be variously modified in design depending on the shape and size of the stomach 2. More specifically, the second set pressure P2 indicates a case where the pressure is equal to or less than the maximum pressure that the first measuring unit 300 can measure or the pressure (Pmax) at which the mucosa of the stomach 2 ruptures. In other words, the separation-required section D3 corresponds to a section where the control unit 410 determines that the resection tool 3 is too close to the expansion unit 200 based on the pressure value measured by the first measuring unit 300.
[0067] The advanceable section D2 indicates that the magnitude of the pressure measured by the first measuring unit 300 is between the first set pressure P1 and the second set pressure P2. That is, the advanceable section D2 corresponds to a section in which the control unit 410 determines that the resection tool 3 is located at an appropriate distance from the expansion unit 200 based on the pressure value measured by the first measuring unit 300.
[0068] When a portion of the expansion unit 200 is not inserted into the stomach 2 due to the short length of the patient's stomach, the control unit 410 excludes the first measuring member 310 of the first measuring unit 300 that measures the pressure value of the corresponding expansion unit 200 from the judgment target. More specifically, the control unit 410 distinguishes between the first measuring member 310 that is disposed inside the stomach 2 and measures a measurement value that is the same as the initial pressure P0 inside the stomach 2, and the first measuring member 310 that is disposed outside the stomach 2 and measures a measurement value different from the initial pressure P0. The control unit 410 determines that the first measuring unit 300 that measures a measurement value that is the same as the initial pressure P0 inside the stomach 2 is in the approach required section D1, and excludes the first measuring unit 300 that is not in the approach required section D1 from the judgment target.
[0069] The control unit 410 can determine whether the expansion unit 200 inserted into the stomach 2 has properly adhered to the inner wall of the stomach 2 through the measurement values of the multiple first measuring members 310. More specifically, when the expansion unit 200 is inserted into the stomach, the control unit 410 can determine whether the expansion unit 200 inserted into the stomach 2 has properly adhered to the inner wall of the stomach 2 by comparing the magnitudes of pressure values individually measured by the multiple first measuring members 310 arranged along the same circumference of the expansion unit 200. That is, the control unit 410 determines that the expansion unit 200 has properly adhered to the inner wall of the stomach 2 when the difference between the measurement values of the first measuring members 310 that contact the inner wall of the stomach 2 among the multiple first measuring members 310 arranged along the same circumference of the expansion unit 200 is within a set size, and determines that the expansion unit 200 has not properly adhered to the inner wall of the stomach 2 when the measurement value of any pair of the first measuring members 310 that contact the inner wall of the stomach 2 is equal to or greater than the set size. Here, the set pressure value can be variously changed according to the user's settings.
[0070] The display unit 420 visually displays the information determined by the control unit 410 to the user. Accordingly, the display unit 420 may be exemplified by a display module that outputs image information. The display unit 420 displays different colors according to the type of the resection position section D determined by the control unit 410. As the first measuring unit 300 is provided with a plurality of first measuring members 310, the display unit 420 may display information for each first measuring member 310 individually.
[0071] The display unit 420 according to an embodiment of the present invention displays a first color C1 for the first measuring member 310 excluded from the judgment target of the control unit 410, displays a second color C2 for the first measuring member 310 judged by the control unit 410 to be in the approach required section D1, displays a third color C3 for the first measuring member 310 judged by the control unit 410 to be in the separation required section D3, and displays a fourth color C4 for the first measuring member 310 judged by the control unit 410 to be in the progress possible section D2. Various design changes are possible for the first to fourth colors C1 to C4 within the types of colors that are distinguishable from one another.
[0072] As described below, the display unit 420 can generate a warning signal when the control unit 410 determines that the expansion unit 200 is not in close contact with the inner wall of the stomach 2. In this case, the warning signal can be exemplified by an auditory signal such as an alarm sound, or a visual signal of a hue different from the first hue C1 to the fourth hue C4.
[0073] The operation of the sleeve gastrectomy assist device 1 according to one embodiment of the present invention will be described in detail below.
[0074] 8 and 9 are operation diagrams illustrating the operation process of the sleeve gastrectomy assist device according to one embodiment of the present invention.
[0075] 8 and 9, the guide unit 100 inserted into the patient's oral or nasal cavity is inserted into the stomach 2 through the esophagus. The guide unit 100 inserted into the stomach 2 is disposed so that its end faces the pylorus of the stomach 2 and is disposed close to the inner surface of the stomach 2. In this case, the shape of the guide unit 100 can be changed in response to the curvature of the stomach 2, etc.
[0076] Thereafter, air is injected into the expansion part 200 through the injection part 10 to expand the volume of the expansion part 200. In this case, both ends of the expansion part 200 are tightly attached to the outer surface of the guide part 100 to prevent air from escaping, so that the volume can be maintained in an expanded state. The expansion part 200 expands in a form in which the distance between two points arbitrarily set on the surface increases between both ends that are in close contact with the outer circumferential surface of the guide part 100, similar to a balloon.
[0077] In this process, the shape of the first measurement part 300 changes in conjunction with the change in volume of the expansion part 200.
[0078] More specifically, as the volume of the expansion portion 200 expands, the distance between the adjacent first measuring members 310 increases.
[0079] As the distance between the adjacent first measuring members 310 increases, the pair of first measuring members 310 generates tension in the connecting portion 320 .
[0080] The connecting portion 320 expands in a direction parallel to the tension by an amount corresponding to a change in the distance between the adjacent first measuring members 310, thereby canceling out the applied tension. Accordingly, the connecting portion 320 can be prevented from breaking due to the tension.
[0081] Thereafter, when the volume of the expansion portion 200 is reduced again and the distance between the first measuring members 310 becomes closer, the connection portion 320 is restored to its original shape by the elastic restoring force of the expansion portion 200 .
[0082] The monitor unit 400 provides the user with information on the ablation position through the pressure value measured by the first measuring unit 300 .
[0083] 10a and 10b are schematic operational views illustrating an operation state during an insertion process of a guide part of a control part according to an embodiment of the present invention.
[0084] 10a and 10b, first, with the guide unit 100 inserted into the stomach 2, the control unit 410 calculates the number of first measuring members 310 that are placed inside the stomach 2 and measure a measurement value that is the same as the initial pressure P0 inside the stomach 2, and the number of first measuring members 310 that are placed outside the stomach 2 and measure a measurement value different from the initial pressure P0. Thereafter, the control unit 410 determines that the first measuring members 310 that measure a measurement value that is the same as the initial pressure P0 inside the stomach 2 are in the approach required section D1, and excludes the first measuring members 310 that are not from the determination targets.
[0085] The display unit 420 externally displays information determined by the control unit 410. That is, as shown in Fig. 10, the display unit 420 displays a first color C1 for the first measuring member 310 excluded from the determination target by the control unit 410, and displays a second color C2 for the first measuring member 310 determined by the control unit 410 to be in the approach required zone D1.
[0086] 11a and 11b are operation diagrams illustrating an operation state of a control unit in an approach required section according to an embodiment of the present invention.
[0087] 11a and 11b, the control unit 410 determines the resection position information of the resection tool 3 through the changing pressure value of the first measuring unit 300, and the display unit 420 displays the determined information to the outside.
[0088] More specifically, as shown in FIG. 11a, when the resection tool 3 pressurizes the stomach 2 at a distance farther from the expansion portion 200 than the distance from the resection tool 3 to the expansion portion 200 that can form the size of the remaining stomach 2 that the user is targeting, the first measuring member 310 measures a pressure lower than the first set pressure P1.
[0089] The control unit 410 determines that the pressure value measured by the first measuring member 310 is in the approach necessary zone D1, and transmits the corresponding information to the display unit 420. In this case, the control unit 410 may use the average value of the pressures measured by the remaining first measuring members 310 that have not been excluded from the determination targets among the plurality of first measuring members 310, i.e., the first measuring members 310 determined to be in the approach necessary zone D1 in Figs. 10a and 10b.
[0090] The display unit 420 displays the second color C2 externally, thereby visually providing information that the cutting tool 3 is too far away from the extension portion 200 and therefore needs to be brought closer.
[0091] 12a and 12b are operation diagrams illustrating an operation state of a control unit according to an embodiment of the present invention in a section requiring separation.
[0092] 12a and 12b, when the resection tool 3 pressurizes the stomach 2 in a state where the resection tool 3 is closer than the distance from the expansion portion 200 to the resection tool 3 that can form the size of the remaining stomach 2 that the user is targeting, the first measuring member 310 measures a pressure higher than the second set pressure P2.
[0093] The control unit 410 determines that the pressure value measured by the first measuring member 310 is in the separation necessary section D3, and transmits corresponding information to the display unit 420.
[0094] The display unit 420 externally displays the third color C3, thereby visually providing information that the cutting tool 3 is too far away from the extension portion 200 and therefore needs to be moved away.
[0095] 13a and 13b are operation diagrams illustrating an operation state of a control unit in a travelable section according to an embodiment of the present invention.
[0096] Referring to Figures 13a and 13b, when the resection tool 3 pressurizes the stomach 2 within a range of the distance from the expansion portion 200 to the resection tool 3 that can form the size of the remaining stomach 2 that the user is targeting, the first measuring member 310 measures a pressure value between the first set pressure P1 and the second set pressure P2.
[0097] The control unit 410 determines that the pressure value measured by the first measuring member 310 is within the advanceable section D2, and transmits corresponding information to the display unit 420.
[0098] The display unit 420 externally displays the fourth color C4, thereby visually providing information that the cutting tool 3 is positioned appropriately from the extension unit 200 and cutting with the cutting tool 3 can proceed.
[0099] Meanwhile, after the expansion part 200 is inserted into the stomach 2, the control unit 410 can determine whether the expansion part 200 inserted into the stomach 2 has properly adhered to the inner wall of the stomach 2 based on the measurement values of multiple first measuring members 310 arranged along the same circumference of the expansion part 200.
[0100] FIG. 14 is a diagram showing a state in which the expansion part according to one embodiment of the present invention is in close contact with the inner wall of the stomach, and FIG. 15 is a diagram showing pressure values measured by multiple first measuring members arranged along the same circumference of the expansion part in the state shown in FIG.
[0101] 14 and 15, when the expansion part 200 inserted inside the stomach 2 completely adheres to the inner wall of the stomach 2, of the multiple first measuring members 310 arranged along the same circumference of the expansion part 200, the first measuring member 310 that comes into contact with the inner wall of the stomach 2 outputs a measurement value within a set size due to a uniform pressure force, and the control unit 410 determines that the expansion part 200 is properly adhered to the inner wall of the stomach 2.
[0102] FIG. 16 is a diagram showing a state in which the expansion portion according to one embodiment of the present invention is not in close contact with the inner wall of the stomach, and FIG. 17 is a diagram showing pressure values measured by multiple first measuring members arranged along the same circumference of the expansion portion in the state shown in FIG.
[0103] 16 and 17, when the expansion part 200 inserted inside the stomach 2 is not in complete contact with the inner wall of the stomach 2, among the multiple first measuring members 310 arranged along the same circumference of the expansion part 200, the first measuring members 310 in contact with the inner wall of the stomach 2 output different measurement values due to uneven pressure.
[0104] When the measured value of any one pair of first measuring members 310 in contact with the inner wall of the stomach 2 is equal to or greater than a set size, the control unit 410 determines that the expansion unit 200 is not properly in contact with the inner wall of the stomach 2. When the control unit 410 determines that the expansion unit 200 is not properly in contact with the inner wall of the stomach 2, the display unit 420 generates a warning signal to inform the user that the expansion unit 200 is not properly in contact with the inner wall of the stomach 2.
[0105] The configuration of a sleeve gastrectomy assist device 1' according to another embodiment of the present invention will be described below.
[0106] In this process, the description overlapping with that of the sleeve gastrectomy assisting device 1 according to one embodiment of the present invention will be omitted.
[0107] FIG. 18 is a perspective view showing a schematic configuration of an auxiliary device for sleeve gastrectomy according to another embodiment of the present invention.
[0108] Referring to FIG. 18, a sleeve gastrectomy assist device 1 ′ according to another embodiment of the present invention includes a guide unit 100 , an expansion unit 200 , a second measurement unit 500 , and a monitor unit 600 .
[0109] The second measuring unit 500 measures the air pressure inside the expansion unit 200. More specifically, the second measuring unit 500 is provided to measure the air pressure value inside the expansion unit 200, which changes according to the pressure applied to the expansion unit 200 by the resection tool 3 pressurizing the stomach 2 during the resection operation. The second measuring unit 500 according to an embodiment of the present invention may be formed in the form of a pressure sensor connected to the injection unit 10 communicated with the expansion unit 200 and measuring the pressure of air injected into the expansion unit 200 through the injection unit 10. Accordingly, the second measuring unit 500 can measure the pressure applied to the expansion unit 200 from outside the expansion unit 200, and therefore can be easily installed and managed. The second measuring unit 500 may be provided with a pressure gauge capable of providing the measured pressure value to the user in visual form.
[0110] The monitor unit 600 is connected to the second measuring unit 500, determines resection position information based on the pressure value measured by the second measuring unit 500, and provides the determined resection position information to a user. Here, the resection position information may be exemplified by information regarding the distance from the expansion unit 200 to the resection tool 3.
[0111] FIG. 19 is a flow chart illustrating a schematic configuration of a monitor unit according to another embodiment of the present invention.
[0112] Referring to FIG. 19, a monitor unit 600 according to another embodiment of the present invention includes a control unit 610 and a display unit 620.
[0113] The control unit 610 determines the resection position information based on the pressure value measured by the second measurement unit 500. More specifically, the control unit 610 converts the pressure value measured by the second measurement unit 500 into information on the distance between the expansion unit 200 and the resection tool 3 to determine the resection position information of the resection tool 3. That is, the control unit 610 determines the resection position information by determining the resection position section D according to the magnitude of the pressure value measured by the second measurement unit 500. The control unit 610 according to another embodiment of the present invention may be exemplified by a microprocessor, a computer, etc. connected to the second measurement unit 500 and receiving the pressure value measured from the second measurement unit 500. The control unit 610 may receive the pressure value measured from the second measurement unit 500 in a wired manner through a cable, etc., or may receive the pressure value measured from the second measurement unit 500 through a separate wireless communication module.
[0114] The resection position section D refers to a section obtained by dividing the pressure values according to their magnitude, based on the initial pressure P0 value inside the stomach 2 measured by the second measuring unit 500 immediately after the insertion of the guide unit 100. More specifically, the resection position section D refers to a section between a pair of different pressure values among the pressure sections measured by the second measuring unit 500 by changing the pressurization or resection position of the stomach 2 with the resection tool 3 after the insertion of the guide unit 100.
[0115] The resection position section D includes an approach required section D1, an advanceable section D2, and a separation required section D3.
[0116] The approach necessary section D1 indicates a case where the pressure measured by the second measuring unit 500 is equal to or greater than the initial pressure P0 and equal to or less than the first set pressure P1. Here, the first set pressure P1 is a pressure value measured at the maximum distance from the expansion unit 200 to the resection tool 3 that can form the size of the remaining stomach 2 targeted by the user, and can be variously modified in design depending on the shape and size of the stomach 2. In other words, the approach necessary section D1 corresponds to a section where the control unit 410 determines that the resection tool 3 is too far away from the expansion unit 200 based on the pressure value measured by the second measuring unit 500.
[0117] The separation-required section D3 indicates a case where the pressure measured by the second measuring unit 500 is equal to or greater than the second set pressure P2. Here, the second set pressure P2 is a pressure value measured at the minimum distance between the expansion unit 200 and the resection tool 3 that can form the size of the remaining stomach 2 targeted by the user, and can be variously modified in design depending on the shape and size of the stomach 2. More specifically, the second set pressure P2 indicates a case where the second measuring unit 500 can measure a maximum pressure or a pressure (Pmax) at which the mucosa of the stomach 2 ruptures. In other words, the separation-required section D3 corresponds to a section where the control unit 610 determines that the resection tool 3 is too close to the expansion unit 200 based on the pressure value measured by the second measuring unit 500.
[0118] The advanceable section D2 indicates that the magnitude of the pressure measured by the second measuring unit 500 is between the first set pressure P1 and the second set pressure P2. That is, the advanceable section D2 corresponds to a section in which the control unit 610 determines that the resection tool 3 is located at an appropriate distance from the expansion unit 200 based on the pressure value measured by the second measuring unit 500.
[0119] The display unit 620 visually displays to the user the information determined by the control unit 610. Accordingly, the display unit 620 may be exemplified by a display module that outputs image information. The display unit 620 displays different colors according to the types of the ablation position sections D determined by the control unit 610.
[0120] According to another embodiment of the present invention, the display unit 620 displays a second color C2 when the control unit 610 determines that the pressure value measured by the second measuring unit 500 is in the approach required section D1, displays a third color C3 when the control unit 610 determines that the pressure value measured by the second measuring unit 500 is in the separation required section D3, and displays a fourth color C4 when the control unit 610 determines that the pressure value measured by the second measuring unit 500 is in the progress possible section D2. The second to fourth colors C2 to C4 can be variously modified within the types of colors that are distinguishable from one another.
[0121] Hereinafter, the operation of the sleeve gastrectomy assisting device 4 according to another embodiment of the present invention will be described in detail.

[0126] The control unit 610 determines that the pressure value measured by the second measuring unit 500 is in the approach necessary zone D1, and transmits corresponding information to the display unit 620.
[0127] The display unit 620 displays the second color C2 externally, thereby visually providing information that the cutting tool 3 is too far away from the extension portion 200 and therefore needs to be brought closer.
[0128] 22a and 22b are operation diagrams illustrating an operation state of a control unit according to another embodiment of the present invention in a separation required section.
[0129] 22a and 22b, when the resection tool 3 pressurizes the stomach 2 in a state where the resection tool 3 is closer than the distance from the expansion portion 200 to the resection tool 3 that can form the size of the remaining stomach 2 that the user is targeting, the second measuring portion 500 measures a pressure higher than the second set pressure P2.
[0130] The control unit 610 determines that the pressure value measured by the second measuring unit 500 is in the separation necessary section D3, and transmits corresponding information to the display unit 620.
[0131] The display unit 620 displays the third color C3 externally, thereby visually providing information that the cutting tool 3 is too far away from the extension portion 200 and therefore needs to be moved away.
[0132] 23a and 23b are operation diagrams illustrating an operation state of a control unit in a travelable section according to another embodiment of the present invention.
[0133] 23a and 23b, when the resection tool 3 pressurizes the stomach 2 within a range of the distance from the expansion portion 200 to the resection tool 3 that can form the size of the remaining stomach 2 that the user is targeting, the second measuring portion 500 measures a pressure value between the first set pressure P1 and the second set pressure P2.
[0134] The control unit 610 determines that the pressure value measured by the second measuring unit 500 is within the progress possible section D2, and transmits corresponding information to the display unit 620.
[0135] The display unit 620 externally displays the fourth color C4, thereby visually providing information that the cutting tool 3 is positioned appropriately from the extension unit 200 and cutting with the cutting tool 3 can proceed.
