Abstract
Description
[Technical field]
[0001] CROSS-REFERENCE TO RELATED APPLICATIONS This application claims priority to U.S. Provisional Application No. 62 / 725,653, filed Aug. 31, 2018, which is incorporated herein by reference in its entirety. [Background technology]
[0002] Acetylcholine is a typical neurotransmitter that is increasingly recognized to be present in a wide variety of cells outside the central nervous system (CNS). Acetylcholine has been shown to be produced in fibroblasts, melanocytes, endothelial cells, and cells of the immune system. Acetylcholine acts on cells through two classes of receptors, namely nicotinic acetylcholine receptors and muscarinic receptors, where it can alter a variety of cellular functions of the cells. Nicotinic acetylcholine receptors are ligand-gated ion channels formed by five subunits: α3, α5, β2, β4 subunits, and α7 subunits, which can form their own functional nicotinic receptors. The presence of these structures, i.e. in keratinocytes, can be demonstrated by histochemical methods, i.e. antibodies against α3 or α7 subunits.
[0003] Donepezil hydrochloride, chemically known as (±)-2,3-dihydro-5,6-dimethoxy-2-[[1-(phenylmethyl)-4-piperidinyl]methyl]-1H-inden-1-one hydrochloride, is a reversible inhibitor of the acetylcholinesterase enzyme. Donepezil hydrochloride is usually referred to in the pharmacological literature as E2020. It has the theoretical formula of C24H29NO3HCl and a molecular weight of 415.96. Donepezil hydrochloride is a white crystalline powder, readily soluble in chloroform, soluble in water and glacial acetic acid, sparingly soluble in ethanol and acetonitrile, and practically insoluble in ethyl acetate and n-hexane.
[0004] Donepezil hydrochloride has been approved for human treatment as Aricept® for the treatment of Alzheimer's disease. It is an orally administered drug. Attempts have been made to formulate donepezil for transdermal delivery. The goal of such formulations is to achieve rapid transdermal delivery for systemic administration to treat Alzheimer's disease, dementia, and related psychiatric disorders. This route is advantageous because of concerns about patient compliance when using oral dosage forms. Some transdermal embodiments include patch formats in which the transdermal formulation is attached or embedded. U.S. Patent No. 9,155,711 to Choi et al. and U.S. Patent No. 8,840,922 to Kawakami et al. discuss treatment of Alzheimer's disease and transdermal drug delivery systems that not only exhibit high skin penetration rates but also maintain therapeutically effective blood levels continuously for at least 24 hours. Furthermore, the Kawakami art teaches donepezil for the treatment of Alzheimer's via transdermal delivery using an absorption enhancer selected from lauryl alcohol, triethyl citrate, isopropyl myristate, cetyl lactate, oleyl alcohol, sorbitan monooleate, polyethylene glycol monostearate, lauromacrogol, N-methyl-2-pyrrolidone and triacetin. However, there is no teaching that donepezil can and should be delivered intradermally.
[0005] Further complicating the use of donepezil for topical application is the known negative side effect of eczema and pruritus, one common side effect observed during human clinical trials of Aricept (donepezil hydrochloride).
[0006] More recent teachings have shown that donepezil hydrochloride and donepezil can be applied directly onto the skin to treat a number of skin disorders, see, for example, U.S. Patent Nos. 9,186,345 and 9,730,919 to Snorrason et al.
[0007] Although there are some teachings in the art that donepezil or its salts can be used to treat or alleviate skin diseases, no topical formulations are known to achieve intradermal delivery, which is associated with complications. The skin has evolved to impede the flux of exogenous molecules, especially molecules such as drugs, by providing an excellent barrier to molecular delivery. Only a small number of drug molecules are available in transdermal dosage forms, for example, less than 30 drug molecules are available for transdermal patches. The challenge of penetrating drug molecules through the main barrier, delivering the drug to the outermost layer of the epidermis, called the stratum corneum, and maintaining the intradermal concentration of the drug still presents challenges.
[0008] Several so-called physical methods have been described to deliver active agents through the stratum corneum. Such methods remove the stratum corneum by ablation or damage or puncture it. Physical methods include iontophoresis or electroporation, sonication, and electrically assisted techniques such as hypodermic needles or microneedle arrays. A preferred alternative to physical methods are the so-called passive methods. Rather than causing damage with physical devices, passive methods use chemical or molecular means to increase the permeability of the stratum corneum. The most attractive passive methods involve the use of molecular penetration enhancers, or MPEs, which are molecules that interact with the structure of the stratum corneum at a molecular level to facilitate the entry of molecules by one or more of several mechanisms, for example by disrupting the lipid bilayer of the stratum corneum.
[0009] More than 300 substances have been identified as MPEs, but only a small proportion have been successfully incorporated into commercial formulations. There are several reasons for this. First, many of the potent MPEs are skin irritants, thus making them impractical and counter to the goal of treating skin diseases. Second, there are substantial regulatory obstacles to incorporating new excipients into topical or transdermal formulations, as the burden of establishing the safety of such excipients can be doubled with that placed on new drug molecules. Third, the best performing MPE systems are rarely individual MPEs, but rather cocktails of multiple MPEs. Fourth, high-performance MPEs are usually more or less molecule specific. An MPE system that works well for one drug molecule will usually perform less well for a different drug molecule. Fifth, it is generally impossible to predict which MPE system will work best for a particular drug molecule in a particular base formulation chassis and in a particular formulation form. Thus, for each molecule that is attractive for topical or transdermal delivery, there is a huge need for an effective MPE system. Moreover, the need is even greater if there is an additional requirement to maintain a therapeutically effective skin concentration of the molecule. Summary of the Invention [Problem to be solved by the invention]
[0010] Thus, even with knowledge of the dermatological uses for donepezil and donepezil hydrochloride, there remains a need to develop formulations that allow maximum penetration and retention of the compound into the dermal layers of the skin. [Means for solving the problem]

DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0018] Detailed Description Before particular embodiments of the invention are disclosed and described, it is to be understood that the invention is not limited to the particular methods and materials disclosed herein, as such may vary to some extent. Also, the terminology used herein is used only to describe particular embodiments and is not intended to be limiting, and the scope of the invention will be defined only by the appended claims and their equivalents.
[0019] In describing and claiming the present invention, the following terminology is used.
[0020] The singular forms "a," "an," and "the" include plural referents unless the context clearly dictates otherwise. Thus, for example, reference to a "penetration enhancer" includes reference to one or more of such penetration enhancers.
[0021] As used herein, the term "about" is used to provide flexibility to the endpoints of a numerical range by indicating that a particular numerical value may be "slightly above" or "slightly below" its endpoint. The degree of flexibility of the term can be described by the particular variable, which would be within the knowledge of one of ordinary skill in the art to determine based on experience and the relevant descriptions herein. For example, in one embodiment, the degree of flexibility may be within about ±10% of the numerical value. In another embodiment, the degree of flexibility may be within about ±5% of the numerical value. In another embodiment, the degree of flexibility may be within about ±2%, ±1%, or ±0.05% of the numerical value.
[0022] As used herein, the term "active agent" refers to a compound or mixture of compounds that has the property of providing a particular therapeutic effect when added to a composition, and herein is donepezil or a salt thereof.
[0023] As used herein, the term "comparison formulation" is a formulation that is compositionally identical except that a first compound and a second compound are each replaced with an equal amount (wt %) of water. As used herein, the term "intradermal" means in the dermal compartment of the skin.
[0024] The term "intradermal administration" refers to administration from the outside of the skin into the dermal compartment of the skin such that the concentration of the administered agent is substantially greater than that of a comparative formulation with respect to the concentration of the agent in another skin compartment or applied transdermally.Intradermal administration of an active agent is highly desirable when its mechanism of action is to interact with a target in the dermal tissue.Because the active agent reaches the dermal compartment by diffusion through the stratum corneum and epidermis, intradermal administration entails establishing a concentration of the active agent in the epidermal tissue.Similarly, intradermal administration does not exclude the small portion of the active agent that penetrates all the way through the skin.
[0025] The term "molecular penetration enhancer or MPE" herein refers to an agent that enhances the migration of molecules, such as active agents (e.g., drugs), into or through the skin. Various conditions can occur at various body locations, either in or under the skin, creating a need for targeted delivery of compounds. For example, psoriasis treatment would benefit from delivery of therapeutic drug levels in deeper tissues. A "penetration enhancer" delivers an active agent directly to the skin or underlying tissues, or indirectly to the affected area through systemic distribution. A penetration enhancer may be a pure substance or may include a mixture of different chemical entities. In this specification, "penetration enhancer", "chemical penetration enhancer", "molecular penetration enhancer" and "MPE" are used interchangeably.
[0026] As used herein, the term "skin contact area" refers to the area where a topical formulation comes into contact with the skin.
[0027] The term "subject" as used herein refers to all members of the animal kingdom, including mammals, and most typically refers to humans.
[0028] The term "topical administration" is used in its ordinary sense to mean the delivery of a substance, such as a therapeutically active substance, to the skin or to a localized external area of the body, including skin (intact, diseased, ulcerated or broken skin) and mucosal surfaces that are normally at least partially exposed to air, such as the lips, genital and anal mucosa, nasal mucosa and oral mucosa. Topical administration of drugs can often be advantageously applied, for example, in the treatment of various skin diseases or conditions.
[0029] As used herein, the term "topical formulation" refers to a formulation that can be applied to external areas of the body, and can be applied to mucosal surfaces, including genital, anal, nasal and oral mucosa, eyes, or lips, in addition to the skin.Topical formulations can be used, for example, to provide therapeutic benefits to patients or cosmetic benefits to consumers.Topical formulations can be used for both topical and transdermal administration of substances.
[0030] As used herein and as is well understood in the art, the term "treat" or "treatment" refers to an approach to obtain beneficial or desired results, including clinical results. Beneficial or desired clinical results can include, but are not limited to, alleviation or amelioration of one or more symptoms or conditions, whether detectable or undetectable, reduction in the extent of disease, stabilization of the disease state (i.e., not worsening), slowing or delaying the progression of disease, improvement or palliation of the disease state, reduction in recurrence of disease, and remission (partial or total).
[0031] "Treating" or "treatment" can also mean prolonging survival as compared to expected survival if not receiving treatment. In addition to being useful as a therapeutic method, the methods described herein can be useful in the prophylaxis or prevention of disease.
[0032] The term "water" as an ingredient in the compositions of the present disclosure refers to pharma- ceutically acceptable water.
[0033] Concentrations, amounts, and other numerical data may be expressed or presented in a range format herein. It should be understood that such range formats are used merely for convenience and brevity, and thus should be flexibly interpreted to include not only the numerical values explicitly recited as limits of the range, but also all individual numerical values or subranges subsumed within the range, as if each numerical value and subrange were explicitly recited. By way of example, a numerical range of "about 0.01 mm to 2.0 mm" is interpreted to include not only the explicitly recited values of about 0.01 mm to about 2.0 mm, but also the individual values and subranges within the indicated range. This numerical range includes individual values such as 0.5 mm, 0.7 mm, 1.5 mm, and subranges such as 0.5 mm to 1.7 mm, 0.7 mm to 1.5 mm, and 1.0 mm to 1.5 mm. Moreover, such interpretation should be applied regardless of the width or characteristics of the range described. Furthermore, it should be noted that all percentages are by weight unless otherwise indicated.

[0093] Thus, the present invention comprises a method of treating a skin disease or problem in a mammal, which may be a human, by topically administering to the affected or problem area at least one of the formulations of donepezil or donepezil HCl described herein. EXAMPLES
[0094] The present invention will be described in more detail with reference to the following experimental examples. These examples are provided for illustrative purposes only and are not intended to be limiting unless otherwise specified. Therefore, the present invention should not be construed as being limited to the following examples, but rather as embracing any and all variations that become evident as a result of the teachings provided herein.
[0095] Without further description, it is believed that one of ordinary skill in the art can, using the preceding description and the following illustrative examples, make and utilize the present invention and practice the claimed methods. Thus, the following examples specifically point out preferred embodiments of the present invention, and are not to be construed as limiting the remainder of the disclosure. Example 1: Analytical Method
[0096] A high performance liquid chromatography ("HPLC") method was used to assay the concentrations of donepezil and donepezil hydrochloride. The mobile phase, column and chromatographic conditions were similar for both donepezil hydrochloride and donepezil base. A summary of the method is provided in Table 1 below.
[0097] [Table 1] Example 2: Preparation of formulations A number of test article formulations were prepared as listed in Tables 2A-2G (formulations were grouped according to the dispersion test they were tested in). Formulations Arctic F1-Arctic F133 are all free-flowing solutions (except for formulations with a "G" at the end which has the addition of hydroxypropylcellulose). Simple solution formulations were prepared by adding all ingredients together, including the active, and sonicating until all ingredients were dissolved and completely dispersed. Formulations Arctic F134-F165 consisted of cream or gel formulations.
[0098] The gel formulation was prepared by first mixing all ingredients except the thickener and sonicating / vortexing until the ingredients were completely dispersed / dissolved. The thickener was added to this solution. The resulting mixture was then left to rotate on a rotisserie until the gallant was completely swollen and the formulation was completely mixed (typically about 24 hours).
[0099] Cream formulations were prepared by separately preparing an oily phase (consisting of oil-soluble components) and an aqueous phase (consisting of water-soluble components). The two phases were mixed in an overhead mixer while heating the formulation at 60° C., and then additional co-solvents (e.g., ethanol, Transcutol®, and propylene glycol) were added and the resulting mixture was further mixed until homogenous. Table 2A: All formulations were flowable solutions. The corresponding flux tests are listed according to the test code (e.g. May0117ArcTor Q95) with the date they were performed.

[0129] Example 3: General procedure for pig skin permeability screening assays in an array format
[0130] As taught in U.S. Patent No. 8,277,762, blocks containing 24 or 48 small diffusion cells arranged in a matrix format are used for initial screening of skin permeability and retention. One serial section of porcine skin trimmed to a uniform thickness of 1.2 mm was introduced between two 48-well plates (porcine skin is more readily available in the required size and is less expensive than human skin. Pig skin is usually slightly less permeable than human skin, but permeability results with porcine skin are predictive of human skin performance).
[0131] Each formulation composition is typically delivered in a pseudo-fixed dose of 20 μL, with a volume of 0.30 cm 2 100 mm2 in six-fold replicates (sex replicates) across the appropriate set of donor wells in a 24-well or 48-well diffusion cell plate. 2 applied to the addressed skin area.
[0132] Phosphate buffered saline ("PBS") at pH 7.4 containing 0.01% sodium azide (preservative) was used as the receptor well fluid. This fluid was validated to provide sink conditions for donepezil throughout the study. The receptor well plate was maintained at 32 (±0.5)°C throughout the experiment, and each receptor well was stirred and agitated using a magnetic stir bar. At the 24 hour mark, each addressed skin area was washed and then dried with a Q-tip. The 24 or 48 well plate was disassembled, each receptor well was sampled, and the retention of donepezil in the skin was assessed by extraction into DMSO followed by analysis using a validated high performance liquid chromatography ("HPLC") method with ultraviolet ("UV") detection at 270 nm.
[0133] Using this screening method using pig skin, the permeability of the formulations shown in the Aug2417ArctFlx table in Tables 2A-2E was tested.
[0134] Example 4: General procedure for measuring human and porcine skin permeability using vertical diffusion cells
[0135] The Franz diffusion cell test method was used to analyze the flux rate of donepezil from the compositions taught under the present invention through human skin. The Franz diffusion cell is a common and well-known method for measuring transdermal flux rate. The general Franz cell procedure is described by Franz {Franz, 1975 #108}.
[0136] In the examples described herein, Franz diffusion cells ("FDCs") with a receptor well volume of 3.3 mL were used with either pig skin or human cadaver skin.
[0137] For porcine skin, by-product dorsal skin from approximately 10-week-old female Yorkshire pigs (sacrificed for purposes unrelated to this permeability study) was provided by Thomas D Morris (Reistertown, MD). The porcine skin was placed on dry ice and maintained at -20°C until the morning of the test. On the day of the test, the porcine skin was removed from the freezer and allowed to thaw at room temperature on the benchtop. The porcine skin was then harvested to a set thickness of approximately 1 mm using a skin skiving system.
[0138] For human cadaveric skin, split thickness human cadaveric skin (0.015"-0.018") was obtained from AlloSource (Centennial, CO) or skin bank New York Firefighters (New York, NY). Skin tissue was harvested at a thickness of approximately 250 μm and shipped frozen on dry ice by the tissue bank. All information available from the cadaveric skin supplier pertaining to source of tissue, donor information, body part, condition of tissue, and storage time prior to receipt was kept in the study file. Immediately upon receipt of donor skin, skin pieces were stored at -20°C until use. Skin pieces were removed from the freezer and allowed to thaw completely at ambient temperature prior to use.
[0139] The donor well is approximately 0.55 cm 2 The skin area of the donor is addressed. The receptor well is filled with PBS containing 0.01% sodium azide (preservative) ("receptor fluid"), and this fluid was identified as providing the immersion conditions for donepezil throughout the study. The receptor well of the FDC is maintained at 37°C in a stirring dry block with continuous stirring of the receptor fluid in the receptor well using a magnetic stir bar (skin surface temperature is 32 (±0.5)°C). The donor and receptor chambers are clamped around the skin piece under uniform pressure using pinch clamps (SS #18 VWR 80073-350).
[0140] After assembly of the FDCs, the skin was allowed to hydrate in contact with the receptor fluid for 20 minutes. Any FDCs that demonstrated any leakage during this period were discarded.
[0141] The integrity and quality of each skin strip was tested prior to application of the test formulation through measurements of the transdermal flux of tritiated water or transepidermal electrical resistance ("TEER") (skin integrity was not routinely tested for porcine skin strips). TEER measurements were performed as follows: A 150 μL aliquot of PBS was introduced into each FDC donor well. After 10 minutes, a flat electrode probe was placed into the donor well and allowed to rest lightly on the surface of the skin under its own weight. A second electrode was then inserted into the receptor fluid via a sample port on the receptor chamber of the FDC. A waveform generator was used to apply a 100 Hz, 100 mV root mean square ("RMS") alternating current ("AC") signal to the skin, and then impedance was measured with a digital multimeter and the results were recorded in kΩ (kiloohms). Any FDCs exhibiting abnormally low impedance (apparently <2 kΩ) were discarded, and the FDCs were ranked according to the magnitude of the measured impedance readings. Test articles were then assigned to batches of FDCs such that replicates for each test article were each applied to skin strips with approximately equivalent mean transdermal electrical resistance values.
[0142] After the skin membrane health test was completed and the cells were properly screened, a sample of the test article was applied to the stratum corneum of the skin. A single dose regimen was used for testing. Six replicates of each test formulation are typically tested in a batch of 36 FDCs in total.
[0143] Doses were applied using a Nichiro positive displacement pipettor. The dose was dispensed from the pipettor onto the skin and spread across the surface using the blunt end of a glass rod. Typical aspirated doses were 10 μL of formulation per cell for most experiments. The formulations themselves were typically manufactured at 1 weight percent (wt%). The 10 μL dose applied to the skin was achieved with no loss of formulation adherence to the glass rod when spreading, a specific gravity of 1.0 in the formulation, and 0.55 cm per cell. 2 Assuming a surface area of 181.8 μg / cm, each FDC contains approximately 181.8 μg / cm 2 This means that the patient was administered donepezil.
[0144] Samples were removed from each receptor well at preset times, typically 24 hours. A graduated Hamilton syringe was used to extract a 300 μL aliquot from the sampling port of each FDC at 24 hours. Each extracted aliquot was introduced into an individual well of a 96-well microtiter plate. Samples were stored in a refrigerator at 4-8°C prior to HPLC analysis. Samples were analyzed within 5 days of collection.
[0145] At 24 hours, the skin was tape stripped three times with Cellophane tape. For each tape strip, a strip of Cellophane tape was applied to the skin with light pressure and then the tape was removed to systematically remove the top layer of the stratum corneum. The tape strips were discarded.
[0146] After tape stripping was completed, the remaining skin was divided into epidermal and dermal compartments using a pair of spatulas. If necessary, the skin was placed on a hot plate set at 60°C for 1 minute to aid in the separation of the skin. The epidermal and dermal compartments were then placed separately into glass vials to which 3 mL of DMSO was added. The skin pieces were then incubated at 40°C for 24 hours with gentle agitation. After the 24-hour incubation period, the samples were collected.
[0147] The samples extracted from the receptor wells and the skin extracts were then analyzed by a validated HPLC method using Chemstation® software. The AUC of donepezil was recorded and converted to μg / mL values using a calibration curve constructed from the AUC values of the calibration standards and the known concentration values. These μg / mL values were imported into the Test Results Excel workbook. These concentration values were then multiplied by the receptor volume (3.3 mL) or skin extract volume (3 mL) and the surface area of the skin exposed to the receptor fluid (0.55 cm 2 ) to obtain the final cumulative amount in μg / cm 2 The concentration of active ingredient (Active) was assayed and reported for each example.
[0148] Using this screening method using human cadaver skin, the formulations shown in Table 2E, Aug3017ArctFlx Q95 through Table 2H were tested.
[0149] Materials and Reagents The following materials and reagents were used in this study:
[0150] [Table 3-1]
[0151] [Table 3-2]
[0152] result Several conclusions were made from the first round of screening, shown in Figures 1-6: Pure DMSO performed best as an enhancer when used alone (Arctic F3 - Figure 1).
[0153] Mixtures of DMSO with other solvents (Figures 3 & 4) did not perform significantly better than non-DMSO solvent mixtures (based on F20 vs. F14).
[0154] An ethanol / propylene glycol mixture (Arctic F20: formulation containing donepezil hydrochloride, posterior Arctic F34: formulation containing donepezil base) was selected as the base control chassis for improvement.
[0155] Donepezil base tended to penetrate the skin more readily than did donepezil hydrochloride, as shown by F34 vs. F20 (Figures 4-6).
[0156] Based on additional rounds of screening in Phase I trials (shown in Figures 7-15), several further modifications were made to the base formulation: The chassis was changed to an ethanol / propylene glycol / transcutol mixture (based on F61 vs. F34) and isopropyl palmitate was additionally added (based on F88 vs. F61). A lead formulation was identified as Arctic F88 for further optimization.
[0157] Based on the results of studies using human skin cadavers and the Franz-type cell procedure (Figures 16-24), the following findings were obtained: The ethanol / PEG / Transcutol / isopropyl palmitate formulation was able to deliver approximately five times more donepezil to the skin (retained in the dermis) than the control formulation. The lead F160 gel formulation was identified.
[0158] At 24 hours, approximately 10 times more active remained in the epidermis or dermis for the F160 formulation rather than penetrating all the way through the skin. Donepezil HCl cream was also administered and delivered approximately three times more to the skin than the control.
[0159] [Table 4]
[0160] Example 5: Treatment of Psoriasis Vulgaris with Preferred Gel and Cream Formulations The formulation is tested against a reference formulation: Control HPMC (hydroxypropyl methylcellulose) 3% PVP-40 (Polyvinylpyrrolidone) 1% Donepezil hydrochloride 0.1% or 1.0% H20 Balance.
[0161] Patients for psoriasis treatment were selected based on a diagnosis of plaque psoriasis with silvery scales covering the surface of the lesions. None of these patients had arthritis.
[0162] Psoriasis is a hyperproliferative disease that alters the differentiation of keratinocytes. Keratinocytes in psoriatic skin lesions have an increased rate of maturation, with psoriatic basal cells taking 3-4 days to reach the stratum corneum, compared with the normal 3-4 weeks.
[0163] Topical treatment, which may vary from 2 weeks to 1 month, can improve the scaling, size, thickness, and erythema appearance of the lesions. All of the lesions in these patients are symmetrical, and each patient can be treated with the formulation of the present invention on one side and a control on the other side. The treatment consists of topically applying either the gel formulations F160 or F161, or the cream formulations F162, F165, F164A, F164B or F164C. Gel formulation F160 and cream formulations F162 and F165 are preferred.
[0164] Example 6: Wrinkles A similar test to Example 5a was conducted to treat wrinkles. Subjects between the ages of 50 and 60 can be treated once a day, three times a week for 8 weeks. The patients can then be observed for improved skin texture and reduction in fine lines, and the images are shared with the dermatologist in charge.
[0165] Example 7: Wound healing For wound healing, a similar test to Example 5a was carried out: patients with chronic ulcers or wounds that had not healed well could be treated for up to 4 weeks. The treatment consisted of topically applying gel formulations F160 or F161, or cream formulations F162, F165, F164A, F164B or F164C. Gel formulation F160 and cream formulations F162 and F165 are preferred.
[0166] Example 8: Pruritus (itch) A similar study to Example 5a was carried out in a group of people with severe sunburn to examine the effectiveness of reducing pain, erythema and itch. The treatment consisted of topically applying either gel formulations F160 or F161, or cream formulations F162, F165, F164A, F164B or F164C. Gel formulation F160 and cream formulations F162 and F165 are preferred.
[0167] Example 9: Edema A similar study to Example 5a was carried out in subjects with traumatic edema. Topical treatment is expected to reduce traumatic edema, especially during the acute phase of inflammation.
[0168] The treatment consisted of topically applying either gel formulations F160 or F161, or cream formulations F162, F165, F164A, F164B or F164C. Gel formulation F160 and cream formulations F162 and F165 are preferred.
[0169] Example 10: Treatment of Acne As shown in Panel A of FIG. 25 (FIG. 25A), a 26 year old male with grade 3 pustular-papular acne on the left shoulder and back refractory to other treatments (including Accutane) experienced a significant improvement in the distribution and size of acne (top panel before treatment with F162, bottom panel after 10 days of treatment with F162). Panels B) and C) demonstrate a similar improvement in facial pustular-papular acne in a 26 year old and a 28 year old female, respectively, both refractory to other treatments (including Accutane). Both females improved to grade 1 acne after 10 days of treatment with F162 and also showed a significant improvement in scars from previous acne.
[0170] The F162 cream formulation was applied topically twice daily onto acne-affected areas for 10 days to each of three patients.

[0179] Day 7: Retest appointment for full evaluation, blood draw for plasma for PK. Owners will be requested to postpone application of spray and gel until the appointment is completed. Study staff will apply the spray and gel at the time of visit. Owners can take this study spray and gel to accountability to receive more if needed.
[0180] Day 14: Final appointment for all evaluations, blood and urine collection. Plasma samples will be banked for pharmacokinetics (PK). Owners are requested to postpone application of sprays and gels until the appointment is completed. Study staff will apply sprays and gels at the visit. Owners will return all study sprays and gels at this visit.
[0181] Test evaluation: Baseline data (demographics, physical examination, pruritus and dermatitis assessment) will be collected at enrollment on Day 0. CBC, serum chemistry, and urinalysis will be collected on Days 0 and 14.
[0182] Owner Rating: 1. Visual Analog Scale (PVAS) to assess pruritus The PVAS, consisting of a 10 cm long line with word descriptors spaced 2 cm apart, is used by dog owners to rate the severity of "pruritus" (itch). Owners are instructed to make a mark on the PVAS line at the point where their dog best manifests pruritus. At completion, the distance (in centimeters) from the base of the line ("normal dog") to the owner's mark on the line is measured and recorded. Owners perform the PVAS assessment on days 0, 7, and 14.
[0183] 2. Owner-reported treatment outcomes (OGATE) Owners will be asked at day 28 (end of study) how they would rate their overall response to treatment. 0 - No response 1-Slight response 2- Legitimate Response 3- Good response 4- Excellent response
[0184] Veterinary Evaluation: The CADESI-04 score will be used by clinicians to assess dermatitis on days 0, 7 and 14. Sample size: 15 dogs
[0185] Outcome measures: CADESI-04 For the current treatment group and standard of care for our treatment group, the percentage of dogs with veterinarily assessed skin lesion scores within the normal range (i.e., CADESI 0-9) or with mild AD (i.e., CADESI 10-35) will be recorded. ·PVAS The proportion of dogs with owner-assessed pruritus within the normal (0.0–1.9) range and with mild AD (i.e. PVAS 2.0–3.5) at the end of the study was assessed. ·OGATE Percentage of dogs whose owners rated their overall response to treatment as "good" or "excellent" (i.e., a score of 3 or 4, respectively).

[0216] Example 13: Safety and efficacy of piperidine acetylcholinesterase reversible inhibitors in client's dogs with atopic dermatitis The study described in Example 13 is a principal of concept, open-label clinical trial whose primary objective is to evaluate the efficacy of piperidine acetylcholinesterase inhibitors in improving clinical signs associated with atopic dermatitis (i.e., dermatitis and pruritus) in dogs. Eleven client-owned dogs with atopic dermatitis were enrolled in the study. Investigators assessed the severity and extent of skin lesions on days 0, 7, and 14 using a validated scoring method (Canine Atopic Dermatitis Extent and Severity Index-four replicates; CADESI-4). At the same study visits, owners assessed the dogs' pruritus levels using a validated pruritus visual analog scale (PVAS) method. In addition, on day 14 (end of study), owners completed the Global Assessment of Treatment Response (OGATE) using a 5-point scale (i.e., 0=no response; 1=poor response; 2=moderate response; 3=good response; 4=best response).
[0217] The mean CADESI-4 scores on days 0, 7 and 14 were 252, 119 and 139, respectively. The PVAS scores on days 0, 7 and 14 were 71.6, 59.2 and 55.4. The confidence intervals of the mean differences for CADESI-4 and PVAS scores are shown in Table 9. There was a significant decrease in skin inflammation (CADESI: P<0.00048, PVAS: P<0.0108) and pruritus levels (PVAS: P<0.009, PVAS: P<0.012) between days 0 and 14. Three dog owners rated their dog's clinical improvement as excellent, five as good, one as fair, one as poor and one as no response.
[0218] [Table 9]
[0219] Despite the fact that a significant reduction in PVAS scores was sought during the study, most owners commented that itching (pruritus) levels did not decrease.
[0220] No adverse effects were observed during 14 days of application of the ointment.
[0221] In summary, the results of this study indicate that the piperidine-based reversible inhibitors of acetylcholinesterase tested in this open-label clinical trial may be beneficial in suppressing clinical signs associated with atopic dermatitis in dogs. A double-blind, placebo-controlled study will also be conducted to further confirm these results.
