Description

[Technical field]

[0001] The claims of this application claim priority under 35 U.S.C. §119(e) to U.S. Provisional Application No. 62 / 711,449, filed July 27, 2018, which is incorporated herein by reference in its entirety.

[0002] The present invention relates generally to general surgical, orthopedic implants used to replace the articular surfaces of joints, e.g., shoulder prostheses. More specifically, but not exclusively, the present invention relates to glenoid implants for anatomical shoulder arthritis, as well as methods for their use. [Background technology]

[0003] Shoulder replacement is a surgical procedure in which all or part of the glenohumeral joint is replaced with a prosthetic implant. Such joint replacements are typically performed to relieve arthritis pain or repair severe joint damage.

[0004] Shoulder replacement surgery is an option for the treatment of severe arthritis of the shoulder joint. Arthritis is a condition that affects the cartilage of the joint. As the cartilage layer wears away, the protective layer between the bones is lost. When this happens, painful bone-to-bone arthritis develops. Severe shoulder arthritis can be very painful and cause limited movement. This can be tolerated with medication and lifestyle adjustments, but there may come a time when surgical treatment is necessary. Summary of the Invention [Problem to be solved by the invention]

[0005] There are several primary approaches to access the shoulder joint. The first is the deltopectoral approach, which preserves the deltoid but requires cutting the subscapularis. The second is the transdeltoid approach, which provides a straight approach at the glenoid fossa. However, during this approach, the deltoid is at risk of potential injury. [Means for solving the problem]

[0006] Shoulder arthroplasty, also known as shoulder arthroplasty or glenoid arthroplasty, was first developed in 1893 by French surgeon Jules-Emile Pine. His procedure consisted of physically lubricating the shoulder joint and implanting platinum and rubber materials. The next notable case in the advancement of shoulder arthroplasty was in 1995 when Charles Neal performed the first hemiarthroplasty, essentially replacing only the humeral head and leaving the natural shoulder socket or glenoid intact. Although this procedure gained exponential popularity over time, patients often developed cartilage loss even on the glenoid surface, causing pain and glenoid erosion. This prompted the development of procedures that replaced not only the humeral component but also the glenoid component.

[0007] Throughout the development of the procedure, it has become widely accepted that the rotator cuff muscles are essential to producing the best results in terms of strength, range of motion, and pain reduction. In addition to this discovery, the physical constraints of the shoulder's normal ball-and-socket anatomy have limited most developments in one way or another. For example, the severely restrictive system limited the range of motion, and the unique anatomy of the glenoid cavity made it difficult to anchor prostheses and fix components without fracturing them. These challenges and high failure rates led to the development of reverse total shoulder arthroplasty to overcome the limitations imposed by the natural shoulder anatomy.

[0008] During the 1970s there was an exponential increase in surgical approaches using this methodology, and the number and variations of surgical techniques are numerous. However, in 1985 Paul Grammont came along with a superior technique that is the basis for most reverse shoulder arthroplasties today.

[0009] In a conventional total shoulder arthroplasty, the approach begins with separating the deltoid muscle from the pectoralis muscle to facilitate access to the glenohumeral joint via a relatively nerve-free passageway. The shoulder joint is initially covered by the rotator cuff muscles (subscapularis, supraspinatus, infraspinatus, and teres minor) and the joint capsule (suprascapularis ligament). Typically, a single rotator cuff muscle is identified and cut to allow direct access to the shoulder joint. At this point, the surgeon can remove the arthritic portion of the joint and then fix a ball and socket prosthesis into the joint.

[0010] The development of safer and more effective techniques has led to the increased use of reverse total shoulder arthroplasty, which is usually indicated when the rotator cuff muscles are severely injured.

[0011] Many existing reverse shoulder systems require a base plate and a glenosphere. These systems generally differ in how the base plate is secured to the glenoid cavity and how the glenosphere engages with the base plate. In some systems, the base plate may be secured to the glenoid cavity of the scapula by multiple screws, and the glenosphere, which has a convex articular surface, may be screwed into the base plate using an axial thread feature and / or taper that is part of the base plate. In other systems, the glenosphere may engage with the base plate only via a taper connection.

[0012] If the glenosphere engages the base plate via either a threaded or tapered connection, the glenosphere and base plate may separate after a period of time, causing the glenosphere to tilt relative to and possibly move away from the base plate, either of which will result in a misalignment of the base plate and glenosphere.

[0013] Some systems involve first fixing a central screw to the glenosphere and then guiding the connection between the baseplate and the glenosphere via the central screw. The process of guiding the connection between the baseplate and the glenosphere is usually a major consideration due to the minimal access and visibility the surgeon may have during the reverse shoulder procedure. Access to the baseplate is generally narrow, making it relatively difficult for the surgeon to obtain the visibility required to properly adjust the engagement between the baseplate and the glenosphere.

[0014] What is needed in the art is a shoulder implant that improves on prior art devices by providing design advantages that result in less bone loss, improved bone graft retention, and greater initial and long-term implant fixation.

The central screw 330 also includes a proximal head portion 334. The head portion 334 has a surface area that increases in a direction opposite the threaded portion 342. The head portion 334 of the central screw 330 includes a recess 336 including a proximal female section 338 for engaging a torque device (such as a screwdriver). The recess 336 includes a central threaded portion 340 and a distal non-threaded portion 342. The recess 336 is disposed through the proximal head 334. As shown in FIGS. 21 and 22 , the central screw 330 is slidably disposed through the central bore 316 of the base plate 310. The diameter of the head 334 is larger than the diameter of the non-threaded portion 332 of the central bore 316 of the base plate 310 such that the central screw 330 is captured by the central bore 316 of the base plate. The central screw 330 is suitable for screwing into, for example, a bone.

[0036] 32-34 show peripheral screws 350. Screws 350 may be, for example, standard compression screws of a type known in the orthopedic implant art. Peripheral screws 350 may include a head portion 352, a drive aperture 354 inserted into head portion 352 at a first end of screw 350, and a threaded portion 356 extending away from head portion 352. Each screw 350 is insertable through at least one of peripheral bores 322 of base plate 310 into a desired bone to compress implant 300 into the bone.

[0037] 35-37, a coupling member or modular taper 360 is shown. The modular taper 360 includes an outer tapered cylindrical shape. The modular taper 360 further includes a distal outer threaded portion 362 and a proximal recess 364 for engagement with a torque device, such as a screwdriver. The modular taper 360 also includes a central bore 366 disposed therethrough. As shown in FIGS. 21 and 22, the modular taper 360 is threadedly connected within the central bore 316 of the base plate 310.

[0038] 38-42, there is shown a glenosphere 380. The glenosphere 380 is hemispherical and includes a central bore 382 having a distal tapered portion 384, a central threaded portion 386, and a proximal cylindrical portion 388. In FIGS. 21 and 22, the glenosphere 380 is positioned over the modular taper 360 such that the exterior of the modular taper 360 and the distal tapered portion 384 of the glenosphere bore 382 form a mechanical taper connection.

[0039] 28-30 collectively, a post 390 is shown. The post 390 includes a cylindrical proximal head 392 having a diameter and a proximal recess 394 for engaging a torque device. The post 390 further includes a central portion 396 having a diameter smaller than the diameter of the post proximal head 392 and a distal portion 398 having a diameter smaller than the diameter of the post proximal head 392. The post 390 also includes an externally threaded portion 300 disposed along a portion of the post 390 between the central post portion 396 and the distal post portion 398. When assembled, the post 390 is disposed through the central bore 382 of the glenosphere 380, through the central bore 366 of the modular taper 360, and threadably connected to the recess 336 of the central screw 330, as shown in FIGS. 21 and 22.

[0040] The implant 300 according to the present disclosure offers several advantages over other known reverse glenoid devices. The base plate 310 can accept a central locking screw 330 to provide compression, which is then locked into place to stiffen the construct via a tapered cap 360 that accepts a glenosphere 380. The construct can be pre-assembled to allow the surgeon to insert the entire locked construct 300 as one piece to streamline the process or simplify revision surgery. Tapers can be added later and / or have built-in offsets to allow for decentering, or have longer taper options for laterally incorporating.

[0041] The screw 330 has a post portion that essentially replaces the distal post of the baseplate, transferring its shape to the screw, allowing the post diameter to be much smaller since the screw does not have to pass through it, ultimately preserving bone. Additionally, the screw 330 passes through the baseplate, providing compression through the threaded portion as well as long term fixation through the proximal portion.

[0042] An exemplary method of using the orthopaedic implant assembly 300 includes placing the base plate 310 on the bone, inserting the central screw 330 into the bone through the base plate 310, inserting the peripheral compression screws 350 through the peripheral bores 322 of the base plate 310, and threading the modular taper 360 into the central bore 316 of the base plate 310. These steps can be performed during an open surgical procedure or can be pre-assembled prior to surgery.

[0043] Next, an exemplary method of using the implant 300 of the present disclosure includes placing the glenosphere 380 onto the modular taper 360, inserting the post 390 into the glenosphere 380 and taper 360, and threading the same into the central screw 330.
