Abstract
Description
[Technical field]
[0001] The present invention relates to contraceptive methods with reduced cardiovascular effects, such as reduced risk of thromboembolism, including reduced risk of venous thromboembolism (VTE) and reduced risk of aortic thromboembolism (ATE).
[0002] The method of the present invention comprises administering to the female mammal an effective amount of an estetrol component in combination with a progestogen component.
[0003] As described in further detail herein, the methods of the present invention enjoy a favorable thromboembolic profile compared to currently available methods using so-called second, third or fourth generation contraceptives. [Background technology]
[0004] The first combined hormonal contraceptives (CHCs) contained estrogen doses above 50 μg. However, studies soon showed that these preparations were associated with an unacceptable increase in the risk of cardiovascular effects, which depended on the dose of the estrogen component. Subsequent studies showed that the risk was much reduced by lowering the estrogen dose, which led to the introduction of newer preparations containing less than 50 μg of estrogen. However, it has become recognized that at low doses of estrogen, progestogenic properties can also affect the risk of thromboembolism.
[0005] All early progestogens were derived from testosterone, and concerns about possible cardiovascular risks led to the evolution of a new set of progestogens designed to reduce adverse cardiovascular effects.
[0006] As a result of their gradual introduction onto the market, different types of CHCs have been categorized into "generations."
[0007] First generation (estrone): Refers to the first generation of CHCs, including estrogens other than ethinylestradiol (EE) and progestogens such as norethisterone and norethindrone.
[0008] Second generation: (Gonanes): Represents products developed in the 1970s that combined ethinyl estradiol with testosterone-derived progestogens, most commonly levonorgestrel (LNG) and norgestrel. These had higher affinity for the progesterone and androgen receptors compared to first generation CHCs.
[0009] Third generation: Represents products developed in the 1980s that contain progestogens derived from levonorgestrel (gestodene, desogestrel, and norgestimate). These were predicted to have fewer metabolic side effects.
[0010] 4th generation: Antiandrogenic progestogens, generally referring to products containing cyproterone acetate, drospirenone (DRSP), dienogest, and chlormadinone.
[0011] It is important to note that fourth generation COCs have been developed to offer a better quality of life, and COCs using DRSP, for example, have indeed shown a reduction in many of the side effects compared to previous generations of COCs.
[0012] Although VTE is a rare side effect of all combined oral contraceptives (COCs), Women using third-generation COCs containing 30 μg ethinyl estradiol have a slightly increased risk of VTE compared with women using second-generation COCs. There are no epidemiological data to suggest that third-generation COCs containing 20 μg ethinyl estradiol pose a lower risk of VTE than third-generation COCs containing 30 μg ethinyl estradiol. There is an excess risk of VTE in the first year that a woman uses any COC. The impact of the relative risk of VTE with third-generation compared with second-generation COCs on the number of additional cases is therefore greatest in the first year that a woman uses a COC for the first time. This should be taken into consideration when a woman is first prescribed and uses a COC. This shows that:
[0013] The estimated risk of developing a blood clot in users of CHCs based on LNG, norethisterone, or norgestimate is 5 to 7 cases per 10,000 women per year. For users of CHCs containing drospirenone, the estimated risk of developing a blood clot is 9 to 12 cases per 10,000 women per year.
[0014] In comparison, the estimated risk of developing a blood clot in non-pregnant, non-CHC users is about 2 per 10,000 women per year.
[0015] Thus, the use of combined contraceptives is associated with an increased risk of venous thromboembolic events (VTE). It is generally accepted that, compared with non-users, the use of second-generation COCs doubles the risk of VTE, and the use of third- and fourth-generation COCs quadruples the risk. The absolute risk of VTE associated with the use of a specific combined contraceptive can only be assessed in very large epidemiological studies. However, at the request of the European Medicines Agency, it is possible to measure some surrogate markers of VTE risk in small clinical settings to estimate the risk.
[0016] Another way to present the change in venous thrombosis risk as a function of the generation of COC used is to look at the relative risk when compared with no oral contraceptive use (relative risk of 1.0). Numerous publications (WHO (1995) Lancet 346, 1575-1588; Jick et al. (1995) Lancet 346, 1589-1593; Spitzer et al. (1996) BMJ 312, 127-132; Vlieg et al. (2009) BMJ 339:b2921; Lidegaard et al. (2009) BMJ 339:b2890; Lidegaard et al. (2011) BMJ 343:d6423) have addressed this issue and their findings are summarized in the table below.
[0017] [Table 1] However, all the above approaches relied on COCs utilizing synthetic estrogens such as ethinylestradiol (EE).
[0018] Of particular importance is the fact that estrogens are involved in the regulation of the synthesis of various proteins in the liver, such as angiotensinogen, sex hormone-binding globulin (SHBG), ceruloplasmin, corticosteroid-binding globulin (CBG), some coagulation factors, coagulation inhibitors, and fibrinolytic markers. Changes in these hemostatic markers under the influence of potent estrogens such as EE may collectively create an imbalance between procoagulant and anticoagulant factors, increasing the risk of venous thromboembolic (VTE) events.
[0019] SHBG plasma levels are a reliable marker of the effect of estrogen on the synthesis of these proteins by hepatocytes, which means that a correlation may exist between the levels of SHBG induced by a particular COC and the risk of VTE associated with that COC (Odlind V et al., Acta Obstet Gynecol Scand, 2002, 81:482).
[0020] Although cohort studies with sufficient numbers of subjects are needed to assess the risk of VTE due to specific COCs, different hemostatic markers and carrier proteins (e.g., SHBG) can be measured to estimate this risk in a limited number of subjects.
[0021] Regarding the effect of the progestogen component, a significant number of studies and two excellent meta-analyses have evaluated the risk of thrombosis from CHCs containing drospirenone. These studies used a number of different data sources from different countries and, with the exception of the study by Dinger (Dinger J, Assmann A, Mohner S, Minh TD. Risk of venous thromboembolism and the use of dienogest- and drospirenone-containing oral contraceptives: results from a German case-control study., J Fam Plann Reprod Health Care., 2010, 36(3):123-9), all consistently showed an increased risk of VTE in drospirenone users compared with levonorgestrel users, which was in most cases statistically significant. Risk estimates most commonly range from about 1.5 to 2 times that of levonorgestrel. Observational studies always have limitations, but bias and residual confounding factors may not explain all of the observed increased risk. The Sidney study (Sidney S, Cheetham TC, Connell FA, Ouellet-Hellstrom R, Graham DJ, Davis D, Sorel M, Quesenberry CP Jr, Cooper WO. Recent combined hormonal contraceptives (CHCs) and the risk of thromboembolism and other cardiovascular events in new users. Contraception 2013, 87(1): 93-100) is considered to provide strong evidence, especially since the analysis was restricted to new users (there were almost 140,000 new users in this cohort study).
[0022] Overall, consistent findings support an excess VTE risk from LNG-associated DRSP.
[0023] In a recently published study, Kluft et al. reported that reduced hemostatic effects were observed in COCs using estetrol as the estrogen compared to COCs using EE as the estrogen (Kluft et al., Reduced hemostatic effects with drospirenone-based oral contraceptives containing estetrol vs. ethinyl estradiol, Contraception, 95 (2017), pp. 140-147).
[0024] In this publication, both the new estetrol-based COC and the comparator (commercially available YAZ®) used drospirenone as the progestogen. The conclusion drawn from this study is that the estrogen estetrol offers a safer hemostatic profile than the estrogen ethinyl estradiol when both are associated with drospirenone.
[0025] The effect of progestogens on the safety profile is discussed above and reported by Kluft et al. themselves (page 141, left column, beginning of the fourth paragraph): "markers and variables are notably less modified with LNG compared to [...] DRSP."
[0026] For this reason, and even though the Kluft et al. publication teaches a better safety profile when estetrol replaces EE in COCs using DRSP as a progestogen, it was unexpected before the present invention that COCs using estetrol and DRSP would compare favorably with second generation COCs, such as COCs using LNG as a progestogen. In particular, the Kluft et al. publication does not disclose or suggest that a composition comprising an estetrol component and DRSP as a progestogen component reduces the thrombotic risk to a level below the lower limit of the thrombotic risk associated with DRSP-based COCs. Considering the general knowledge that fourth generation COCs (DRSP-based), despite their many advantages in terms of reduced side effects, present a much higher thrombotic risk than previous generations of COCs, it was unthinkable before the present invention that DRSP-based COCs could present a thrombotic risk outside the range of risks associated with DRSP-based COCs of the prior art.
[0027] WO 2018 / 024912 and WO 2018 / 065076 disclose the use of estetrol to relieve the symptoms of dysmenorrhea. These patent applications are silent about the reduction of the risk of thrombosis. Table 7 in these patent applications shows the same clinical data as Table 1 in the Kluft et al. publication, but in a slightly different way, with the Kluft et al. publication showing the median values (expressed as a percentage of the baseline value, taken as 100%), whereas the patent applications show the mean values (percentage change).
[0028] Thus, there remains a need for contraceptive approaches that offer a better safety profile than currently available COCs, particularly with a lower risk of thromboembolic events. This is particularly important in high-risk populations, such as first-time users, transitioners or re-initiations with a discontinuation of more than 4 weeks, and women at increased baseline risk of VTE due to one or more major risk factors (including, but not limited to, BMI >30, older age, and positive personal and / or family history).
[0029] Summary of the Invention The present invention relates to a method of contraception having reduced cardiovascular effects which comprises administering to a female mammal an effective amount of an estetrol component in combination with a progestogen component.
[0030] In one embodiment of the present invention, the number, frequency and / or severity of VTE events is reduced as compared to other contraceptive methods.
[0031] In certain embodiments, the risk of VTE during use of the contraceptive methods of the present invention is similar to the risk of VTE during use of CHCs based on levonorgestrel, norgestimate, or norethisterone.
[0032] In a preferred embodiment, the risk of VTE during use of the contraceptive methods of the present invention is lower than the risk of VTE during use of CHCs based on levonorgestrel, norgestimate, or norethisterone.
[0033] In one embodiment of the present invention, the number, frequency and / or severity of pulmonary embolism events is reduced as compared to other contraceptive methods.
[0034] In another embodiment of the present invention, the number, frequency and / or severity of deep vein thrombosis (DVT) events is reduced as compared to other contraceptive methods.
[0035] In another embodiment of the present invention, no hemostatic changes beyond the boundaries of the normal range, as further defined herein, occur upon administration of the compositions of the present invention.
[0036] In yet another embodiment, the methods of the invention exhibit less "hemostatic change" (as further defined herein) than contraceptive methods employing second, third or fourth generation CHCs. In particularly preferred embodiments, the present invention exhibits less "hemostatic change" than contraceptive methods employing second generation CHCs.
[0037] In further embodiments, the markers of coagulation and / or fibrinolysis markers exhibit smaller changes during the contraceptive methods of the present invention (compared to the levels of the markers of coagulation and / or fibrinolysis markers prior to administration of any contraceptive composition) than the changes observed with other contraceptive methods.
[0038] In certain embodiments, the markers of coagulation and / or fibrinolysis show less change during a contraceptive method of the present invention than during a contraceptive method using a second, third or fourth generation CHC. In preferred embodiments, the markers of coagulation and / or fibrinolysis show less change during a contraceptive method of the present invention than during a contraceptive method using a second generation CHC.
[0039] In one embodiment of the present invention, an effective amount of an estetrol component is used in a combined oral contraceptive composition using drospirenone as the progestogen component to reduce the risk of thrombosis associated with the use of drospirenone.

[0148] The present method utilizes the ingredient estetrol, which is a natural estrogen (ie, an estrogen that occurs in nature) and a biogenic estrogen (ie, an estrogen that occurs naturally in the human body).
[0149] Because biogenic estrogens occur naturally in the fetal and female bodies, a good tolerability and safety profile is observed, particularly when exogenous administration of such estrogens results in serum levels that do not significantly exceed naturally occurring concentrations. [0151] definition The term "estetrol component" as used throughout this specification includes substances selected from the group consisting of estetrol, esters of estetrol in which at least one hydrogen atom of the hydroxyl group is replaced by an acyl radical of a hydrocarbon carboxylic acid having 1 to 25 carbon atoms, a sulfonic acid or a sulfamic acid, and combinations thereof. Even more preferably, the estetrol component is estetrol (including estetrol hydrate). Most preferably, the estetrol component contained in the dosage unit is estetrol monohydrate.
[0152] The term "progestogen moiety" is defined as a substance capable of inducing a progestogenic response in vivo, or a precursor capable of liberating such a substance in vivo. Typically, a progestogen moiety is capable of binding to a progestogen receptor.
[0153] "About" as used herein with reference to a measurable value, such as a parameter, amount, duration, and the like, is intended to encompass a variation of no more than + / - 10%, more preferably no more than + / - 5%, and even more preferably no more than + / - 1% of the specified value, insofar as such variations are appropriate for the practice of the disclosed invention. However, it will be understood that the value to which the modifier "about" refers is itself also specifically disclosed.
[0154] The term "effective amount" refers to the amount necessary to obtain a physiological effect, which may be obtained by a single administration or by multiple administrations.
[0155] As used herein, the term "thrombotic risk" is equated with "thromboembolic risk" and "venous thromboembolic risk."
[0156] As used herein, the term "second generation COCs" refers to COCs that combine ethinyl estradiol as the estrogen and levonorgestrel (LNG) or norgestrel as the progestogen component.
[0157] As used herein, the term "third generation COCs" refers to COCs that use gestodene, desogestrel, or norgestimate as the progestogen component.
[0158] As used herein, the term "fourth generation COCs" refers to COCs that use cyproterone acetate, drospirenone, dienogest, nomegestrol or chlormadinone as the progestogen component.
[0159] As used herein, "positive family history," in the context of risk factors, refers to a family history of venous or arterial thromboembolism or pulmonary embolism, known thrombotic mutations (exemplary but not limited to, Factor V Leiden; prothrombin mutations; protein S, protein C, and antithrombin deficiencies) that are considered risk factors for venous thromboembolism in combined oral contraceptive users.
[0160] As used herein, "positive personal history," in the context of risk factors, refers to a personal history of venous or arterial thromboembolism or pulmonary embolism that is considered a risk factor for venous thromboembolism in combined oral contraceptive users.
[0161] As used herein, "elderly" in the context of risk factors refers to age greater than 35 years.
[0162] As used herein, women are "first-time users" and are in their first year of CHC use, which is generally understood to be a period of increased VTE risk.
[0163] As used herein, a woman is a "resumee" if she discontinued CHC use for 4 weeks or more. There is some evidence that resuming CHC use after a 4-week or longer cessation of use increases the risk of VTE.
[0164] As used herein, a woman is a "conversion" if she stops using one type of CHC and starts using another type of CHC. Optionally, the transition includes a cessation of use for 4 weeks or more.
[0165] As used herein, "BMI" is an abbreviation for Body Mass Index, and is defined as weight divided by height squared, where mass is in kilograms and height is in meters, so kg / m 2 It is expressed in units of.
[0166] As used herein, "COC" stands for Combined Oral Contraceptive and "CHC" stands for Combined Hormonal Contraceptive. Throughout this patent application, these two terms are used interchangeably and an embodiment describing a COC(s) should be read in the same manner as an embodiment describing a CHC(s) and vice versa. In particular, second, third and fourth generation COCs correspond to second, third and fourth generation CHCs, respectively.
[0167] As shown in the Examples, the contraceptive method of the present invention has been demonstrated to have a surprising safety profile, especially in direct comparison with contraceptive methods using either fourth or second generation COCs.
[0168] Another important advantage of the estetrol component of the present invention is that it is relatively insensitive to interactions with other drugs (drug-drug interactions). It is well known that some drugs can reduce the effect of estrogens, such as ethinylestradiol, and other drugs can enhance the activity of estrogens, resulting in increased side effects. Similarly, estrogen can inhibit the metabolism of other drugs. In general, the effect of other drugs on estrogens is due to the interference with the absorption, metabolism or excretion of these estrogens, while the effect of estrogen on other drugs is due to the competition of metabolic pathways.
[0169] The clinically most important group of estrogen-drug interactions occurs with drugs that can induce hepatic microsomal enzymes that can reduce plasma levels of estrogens to subtherapeutic levels (e.g., anticonvulsants; phenytoin, primidone, barbiturates, carbamazepine, ethosuximide, and methsuximide; antituberculous drugs such as rifampin; antifungal drugs such as griseofulvin). The estrogenic substances of the present invention are not dependent on up- and down-regulation of microsomal hepatic enzymes (e.g., P450), nor are they sensitive to competition with other P450 substrates. Similarly, the estrogenic substances do not significantly interfere with the metabolism of other drugs.
[0170] In particular, estetrol at a high concentration of 10 μmol / l, unlike estradiol, does not inhibit (less than 10%) the major cytochrome P450 enzymes (CYP1A2, CYP2C9, CYP2C19, CYP2D6 and CYP3A4). In fact, estradiol exerts a substantial inhibitory effect of 63% and 19% on CYP2C19 and CYP1A2, respectively. Similarly, ethinylestradiol, the estrogen used in many COCs, exerts a substantial inhibitory effect of 82% and 45% on CYP2C19 and CYP3A4, respectively.
[0171] The above observations help explain why the estetrol component of the present invention is subject to little drug-drug interaction and therefore provides very consistent, i.e., predictable, effects. Therefore, the effectiveness of the estetrol component of the present invention is very reliable.
[0172] Moreover, estetrol is characterized by a terminal half-life of 31.7 hours, whereas naturally occurring estrogens have a terminal half-life ranging from 2 to 14 hours. As a result, the use of estetrol in the method of the invention allows the receptors to be covered by the treatment for more than 24 hours. This pharmacokinetic property increases the effectiveness of the product even in cases where the user's compliance with the treatment is low.
[0173] It should be noted that estetrol (E4) in combination with 3 mg of drospirenone (DRSP) improves bleeding profile and cycle control compared to other combined oral contraceptives that use physiological estrogens, i.e. estradiol valerate (E2V) or estradiol (E2).
[0174] In a study evaluating bleeding patterns and cycle control with different E4 / DRSP combinations compared to commercially available quadruphasic combined oral contraceptives containing E2V and desogestrel (DSG), the 15 mg E4 / DRSP combination was associated with a lower incidence of unscheduled bleeding / spotting compared to the control. Furthermore, absence of withdrawal bleeding (also called amenorrhea) was much lower with E4-containing preparations, especially E4 combined with DRSP, compared to the control. Finally, the mean number of days of unscheduled bleeding / spotting per cycle was also lower with the 15 mg E4 / DRSP combination compared to the E2V / DNG preparation. This was also the case when compared to published data for commercially available combined oral contraceptives containing E2 combined with nomegestrol acetate (NOMAC) as the estrogen.
[0175] Additionally, daily use of currently available estrogens (ethinyl estradiol (EE), E2, E2V, conjugated equine estrogens) is associated with a dose-proportional increase in triglyceride levels. In humans, high levels of triglycerides in the bloodstream have been linked to atherosclerosis and, by extension, the risk of heart disease and stroke. Contrary to currently available estrogens, E4 produces minimal increases in triglyceride levels, even at high doses.
[0176] As shown in the Examples, the changes in surrogate markers of VTE were minimal compared to those observed with Yaz® (a combination of 20 μg EE and 3 mg DRSP). DRSP is a fourth generation progestin that, when combined with the synthetic estrogen EE, confers the highest risk of VTE. As a result, the changes in surrogate markers of VTE seen with the combination of EE and DRSP are substantial. On the other hand, the changes observed with the combination of the estetrol component are minimal, even though DRSP is associated with the estetrol component.
[0177] More importantly, it can be seen in the examples that the combination of estetrol and DRSP of the present invention compares favorably with a second generation COC comparator consisting of 30 mcg EE and 150 mcg LNG (Melleva®). This result is quite surprising, given that it was previously recognized that substituting DRSP for LNG doubled the risk of VTE.
[0178] In particular, parameters such as APC resistance (e.g., ETP-based APC resistance) are almost an order of magnitude lower with estetrol-based regimens compared to ethinyl estradiol-based regimens (mean percent changes in Tables 2 and 3 and Table 5 below).
[0179] Acquired activated protein C resistance (versus mutation-associated) has been shown to be an independent risk factor for increased venous thrombosis in the COC user population (Rosing et al., 1997, Br. J. Haematol. 97, 233-8). APC resistance determined using an ETP-based assay (as in the present application) is an excellent marker for the thrombogenicity of COCs.
[0180] Additionally, as seen in Tables 1 and 4 below, statistical significance (for change from baseline to cycle 6) was observed between the COC of the present invention and both the second generation (Melleva®) and fourth generation (Yaz®) COCs for the following parameters: plasminogen (%), t-PA (ng / mL), ETP-based APC-r, prothrombin fragment 1+2 (nmol / L), and soluble E-selectin (ng / mL).
[0181] Statistical significance (change from baseline to cycle 6) was observed between the COC of the present invention and the fourth generation (Yaz®) COC for the following parameters: Factor VII activity (%), Protein S (%), Protein S free (%), Protein C activity (%), aPTT-APC (sec) and SHBG (nmol / L).
[0182] Treatment method The method of the present invention typically employs uninterrupted oral administration of the estetrol component and the progestogen component for a period of at least 10 days, preferably at least 20 days.
[0183] The term "without interruption" as used herein means that the components are administered at relatively regular intervals, without significant (therapeutic) interruptions. Of course, minor interruptions that do not affect the overall effectiveness of the method of the present invention may occur, and indeed such deviations are encompassed by the present invention. In a preferred embodiment, and more arithmetically, a dosing regimen is considered to be continuous if the longest interval between two subsequent administrations does not exceed 3.5 times the average interval. Even more preferably, said longest interval is not more than 2.5 times the average interval, and most preferably not more than 1.5 times.
[0184] In the method of the invention, the estetrol component and the progestogen component may be administered in separate dosage units, although it is also possible, and indeed very convenient, to combine these two components in a single dosage unit.
[0185] In the methods according to the present invention, the combination of the progestogen component and the estetrol component is suitably administered without interruption for a period of at least 10 days.
[0186] The present invention may be appropriately reduced to practice in the form of various administration methods known to those skilled in the art. Among these methods are the so-called "combined" methods. Combination methods use monophasic preparations with dosage units containing a fixed amount of estrogen and progestogen, or biphasic or triphasic preparations with different levels of estrogen and progestogen, and most often consist of a relatively constant level of estrogen, with a gradual increase in progestogen throughout the cycle. What combination methods have in common is that they are based on a regimen that includes an approximately 7-day administration-free interval during which withdrawal bleeding occurs, mimicking natural menstruation. Thus, there are 21-day hormone administration intervals alternating with 7-day hormone-free intervals.
[0187] In a preferred embodiment of the method of the present invention, a dose-free interval of about 4 days is used, in this embodiment, a hormone dose interval of 24 days is alternated with 4 days without hormone administration.
[0188] In yet another preferred embodiment of the method of the present invention, there are 24 day hormone administration intervals during which the estetrol component and the progestogen component are administered, alternating with 4 days (days 25 to 28) during which only the estetrol component is administered.
[0189] As an alternative to the combination method described above, so-called "sequential" methods have been proposed. A typical sequential method includes two consecutive stages, namely, a stage in which estrogen but not progestogen is administered, and another stage in which a combination of estrogen and progestogen is administered. The first sequential method used a dosing-free interval of about 7 days, similar to the combination method described above. Recently, sequential methods that do not include a dosing-free (or placebo) period have been proposed, meaning that estrogen is administered throughout the entire cycle and progestogen is co-administered during only a portion of the cycle. WO 95 / 17895 (Ehrlich et al.) describes such an uninterrupted sequential method.
[0190] Yet another example of a method encompassed by the present invention is the so-called "sequential combination method," which is a specific version of the combination method in which the progestogen and estrogen components are administered in combination without interruption for an extended period of time, e.g., more than 50 days. In contrast to the regular combination method or the sequential method, in the sequential combination method, amenorrhea is induced by the continuous administration of the indicated amount of progestogen, so that regular menstruation does not occur.
[0191] In one embodiment of the present invention, relating to a continuous combination method, the method of the present invention comprises the oral administration of a combination of an estetrol component and a progestogen component without interruption for a period of at least 28 days, preferably at least 60 days.
[0192] In one particular embodiment of the continuous combination method described in the present invention, one tablet containing the combination of estetrol component and progestogen component is first taken every day for at least 24 consecutive days. Then, between the 25th day and the 120th day, the patient may decide to take a 4-day tablet-free break. The tablet-free break should not be longer than 4 days.In any case, the 4-day tablet-free break must be performed after 120 days of continuous tablet administration. After each 4-day tablet-free break, a new cycle is started with a minimum of 24 days and a maximum of 120 days of continuous administration.
[0193] In another embodiment of the invention, in relation to sequential and combination methods using significant administration-free intervals, the methods of the invention include an interval of at least 2 days, preferably 3-9 days, and most preferably 5-8 days, during which the progestogen and estetrol components are not administered, resulting in a decrease in serum concentrations of the progestogen and estetrol components, and thus induction of menstruation.
[0194] Yet another embodiment of the invention is a sequential method without significant pauses, comprising uninterrupted oral administration of the estetrol component for at least 28 days, preferably at least 60 days, and characterized in that the estetrol component and the progestogen component are administered in combination, followed by consecutive administration of the estetrol component but not the progestogen component for 3 to 18 days, preferably 5 to 16 days, and the resulting decrease in serum concentration of the progestogen component should normally be sufficient to induce menstruation.
[0195] According to the present invention, the contraceptive composition can reduce the number, frequency and / or severity of adverse side effects, including VTE, ATE.
[0196] In certain embodiments of the present invention, the method does not cause hemostatic changes beyond the borders of the normal range. As used herein, "hemostatic changes" is defined as a change in the plasma levels of one or more markers selected from the following upon administration of the composition according to the present invention: sex hormone binding globulin (SHBG), free tissue factor pathway inhibitor (free TFPI), free and total protein S, protein S activity, corticosteroid binding globulin (CBG), ceruloplasmin, antithrombin III, activated protein C (APC) resistance (e.g. APTT-based APCr or ETP-based APCr), protein C activity, D-dimer, prothrombin, prothrombin activity, prothrombin fragment 1+2, factor VII, factor VIII, von Willebrand factor, factor II, PAI-1, tissue-type plasminogen (t-PA), plasminogen, E-selectin, and fibrinogen.
[0197] The above markers are well known to those of skill in the art and methods for determining the levels of the markers are within the general knowledge of those of skill in the art.
[0198] As used herein, when referring to levels of a hemostatic marker, "normal range" refers to a prediction interval that falls within which 95% of a population falls.
[0199] In one embodiment of the invention, the method does not cause hemostatic changes beyond the boundaries of the normal range after one cycle of treatment, preferably the method does not cause hemostatic changes beyond the boundaries of the normal range after two cycles of treatment, and even more preferably the method does not cause hemostatic changes beyond the boundaries of the normal range after three cycles of treatment.
[0200] In another particular embodiment of the invention, the method does not cause a change in Protein S levels beyond the borders of the normal range.
[0201] In another specific embodiment of the invention, the method does not cause a change in free TFPI levels beyond the boundaries of the normal range.
[0202] In a preferred embodiment, the contraceptive methods of the present invention, which provide a superior safety profile over currently available COCs and in particular exhibit a lower risk of thromboembolic events, are used in high-risk populations, such as first-time users; transitioners or resumers with a discontinuation of more than 4 weeks; and women at increased baseline risk of VTE due to one or more major risk factors (e.g., but not limited to, BMI>30, older age, positive personal and / or family history, and risk factors such as those identified in Anderson and Spencer, 2003, Circulation, 107:I-9 to I-16, "Risk factors for venous thromboembolism", which illustratively include, but are not limited to, major surgery, prolonged immobility, childbirth, postpartum).
[0203] In relation to the elderly high-risk population, as reported by Lidegaard et al. (2011) BMJ 343:d6423 (Risk of venous thromboembolism from use of oral contraceptives containing different progestogens and oestrogen doses: Danish cohort study, 2001-9), specifically in table 1 on page 9, the risk of VTE in non-users increases from 0.7 per 10,000 women-years in the 15-19 age group to 5.8 per 10,000 women-years in the 45-49 age group. In this latter population, COC use increases the VTE risk by 3.6-fold (table 1 in Lidegaard et al. 2011). The advantageous use of COCs according to the present invention is beneficial across all age groups, but more significantly for older age groups (e.g., over 35 years old) who have the highest incidence in the absence of COC use and whose incidence is most affected by the use of prior art COCs.

[0224] result The following hemostatic parameters were measured at baseline (without COC) and after 3 and 6 cycles of COC use:
[0225] clotting factors These are: fibrinogen, prothrombin, factor VII, factor VIII, and von Willebrand factor (Note: although grouped together for ease of reference, von Willebrand factor is not actually a clotting factor in its own right).
[0226] Anticoagulant Proteins : Antithrombin, Protein S (ELISA and activity), Factor XIV = Protein C, and TFPI.
[0227] Proteins involved in fibrinolysis : Plasminogen, tissue plasminogen activator (tPA), and plasminogen activator inhibitor type 1 (PAI-1).
[0228] Functional coagulation tests : aPTT-based activated protein C resistance (APCr), ETP-based activated protein C resistance, activated clotting time-APC, activated clotting time+APC.
[0229] Markers of ongoing clotting : Prothrombin fragment 1+2, and D-dimer.
[0230] others : soluble E-selectin and sex hormone binding globulin (SHBG). Regarding thrombus formation of COCs, hemostatic parameters that are associated with an increased risk of venous thrombosis or are so-called (surrogate) risk markers of venous thrombosis:
[0231] clotting factors: Fibrinogen, prothrombin, and factor VIII
[0232] Anticoagulant Proteins : Antithrombin, Protein S (ELISA and activity), Factor XIV = Protein C, and TFPI.
[0233] Proteins involved in fibrinolysis : Not generally considered a risk factor for VTE.
[0234] Functional coagulation tests aPTT-based activated protein C resistance (APCr), ETP-based activated protein C resistance
[0235] Markers of ongoing clotting : Prothrombin fragment 1+2, and D-dimer.
[0236] others : Sex hormone-binding globulin (SHBG) is a surrogate marker of VTE risk during hormone use.
[0237] Of particular interest in relation to prothrombin is the publication by Poort et al. (A common genetic variation in the 3'-untranslated region of the prothrombin gene is associated with elevated plasma prothrombin levels and an increase in venous thrombosis, Blood, Vol. 88, No. 10, 1996, pp. 3698-3703). Table 2 on page 3701 of this publication establishes the relationship between the level of prothrombin activity and the odds ratio of the risk of thrombosis. From this table it can be seen, for example, that if the prothrombin activity is increased at a level between 5% and 15% above normal (which is taken as 100%), the odds ratio of the risk of thrombosis is 1.4. If the prothrombin activity exceeds 115%, the odds ratio changes to 2.1.
[0238] Since real populations are in fact spread around the mean of the Gaussian distribution, an apparently small change in the location of the center of the Gaussian (mean prothrombin level) implies a significant change in the number of subjects in the high-risk category (the category displayed at the end of Table 2 of Poort et al., i.e. subjects with a prothrombin level of 115% or more and therefore with an odds ratio of 2.1). For example, in a group of subjects not taking COCs (and therefore displaying an average prothrombin level of 100%), the "tail" of the Gaussian curve corresponding to high-risk subjects (with an odds ratio of 2.1) would be 14% of the entire population, whereas in a group of subjects taking COCs that displace the average prothrombin level to 111% (as for EE / LNG treatment in the data of the examples, see the item "Prothrombin activity" in the 6th cycle of Table 3), the high-risk patient population (with an odds ratio of 2.1) would be very significantly increased to 38% of the entire population.
[0239] From this exemplary calculation, it can be easily seen that small variations in the mean levels of thrombosis parameters can actually have a very large impact in terms of the number of subjects that fall into the high risk category.
[0240] To summarize the effect of different COCs on hemostatic parameters, a number of different approaches were used for each parameter. In the first method, the change in the mean measured level between subjects in a given group at cycles 3 and 6 was calculated by comparing with the mean level at baseline. The corresponding values are shown in Table 1 below.
[0241] In the second approach, the changes in cycles 3 and 6 compared to baseline were first calculated for each patient, and then the average of these changes was calculated. The corresponding results are shown as percentages in Table 2 below.
[0242] Third, the mean measured levels at baseline and at cycles 3 and 6 were used to calculate the percentage change from baseline to cycles 3 and 6, which are shown in Table 3 below.
[0243] In the present study, the effects of EE / DRSP on hemostatic parameters were more pronounced than those of EE / LNG. However, quite surprisingly, the effects of E4 / DRSP were comparable to those of EE / LNG (second-generation COC). In fact, the effects of E4 / DRSP were even smaller than those of EE / LNG in most parameters, suggesting that E4 / DRSP may be less thrombogenic than EE / LNG.
[0244] In relation to the odds ratios from the abovementioned paper by Poort et al., what is important in Table 3 is that when the method of the present invention is used, prothrombin activity is found to increase by only 5.9% after the sixth cycle, whereas when EE / LNG and EE / DRSP are used, it is found to increase by 11.2% and 10.4%, respectively. In this respect, the method of the present invention is shown to place fewer subjects in the "at risk" category (defined as having an odds ratio of 2.1).
[0245] Similarly, it is known from Odlind et al. (Can changes in sex hormone binding globulin predict the risk of venous thromboembolism with combined oral contraceptive pills? Acta Obstet Gynecol Scand, 2002, 81, pp. 482-490) that the incidence of VTE is directly linked to the amount of increase in SHBG levels observed after a COC is administered. Based on the levels observed in Table 3, it can be seen that the method of the present invention is estimated to induce a slightly lower risk than the EE / LNG COC and a much lower risk than the EE / DRSP COC. In other words, the contraceptive method of the present invention would appear in Figure 1 of Odlind et al., in the lower left corner of the graph, close to the "LNG" data point and very far from the "DSG / GST" and "CPA" data points.

[0248] [Table 7]
[0249] Calculating VTE risk A similar clinical trial program evaluated the efficacy, cycle control, general safety and tolerability of E4 / DRSP in healthy women aged 16-50 years, enrolling subjects for 12 months (13 cycles, 1 cycle = 28 days). 2Up to 10,000 women were included in the study. In this clinical trial involving 3417 subjects, one case of VTE occurred. Based on the number of subjects in the study and the duration of COC administration to each subject, this occurrence can be translated into an estimated VTE risk for the COC of the present invention of 3.7 VTE / 10,000 women / year.
[0250] As reported in the Background section above, the estimated risk of developing a thrombosis in users of CHCs based on LNG, norethisterone or norgestimate is 5-7 per 10,000 women per year. For users of CHCs containing drospirenone, the estimated risk of developing a thrombosis is 9-12 per 10,000 women per year.
[0251] Thus, the COC of the present invention proves to be advantageous compared to second, third and fourth generation products or earlier. For reference, the estimated risk of thrombosis in non-pregnant non-CHC users is about 2 per 10,000 women per year (more specifically, the article by de Bastos et al., "Combined oral contraceptives: venous thrombosis.", Cochrane Database Syst Rev. 2014, March, 3; (3), reports incidence rates of 1.9 and 3.7 per 10,000 per year in cohorts of non-CHC users, in conjunction with a previous finding of 1.6 per 10,000 per year).
[0252] From the above it can be concluded that the COCs of the present invention, with an incidence of VTE of 3.7 per 10,000 women, is at the upper end of the range for non-CHC users (3.7 per 10,000 women-years reported in Lidegaard et al. (2011) BMJ 343:d6423 - see Table 2 on page 10 of the same), and far from the lower end of the safest generation COCs of the prior art (a risk of at least 5 per 10,000 women per year reported for second generation COCs). A second measure of ETP-based APC resistance To confirm the data presented above, the inventors decided to perform a second analysis of plasma samples collected during the clinical trial described above, specifically the measurement of APC resistance based on ETP.
[0253] The experimental protocol for analyzing the plasma samples was improved and a new calibration routine was used. After performing these new measurements, the following results were obtained, and Tables 4 and 5 show the results in the same format as Tables 1 and 2 above, respectively.
[0254] [Table 8]
[0255] [Table 9]
[0256] Despite protocol modifications and a complete rerun of all plasma samples, the trends reported above in Tables 1-3 regarding ETP-based APC resistance are fully confirmed above in Tables 4 and 5. In particular, the advantages of the COC of the present invention are directly evident by comparison with a fourth generation COC (Yaz®), but more importantly, also by comparison with a second generation COC (Melleva®).
[0257] Statistical analysis further revealed highly significant differences from baseline, especially for the Yaz® and Meleva® control COCs. More importantly, as shown by the p-values displayed in Tables 1 and 4, there was a highly significant difference between the COC of the present invention and both control drugs, while the control drugs were not significantly different from each other.
[0258] From the above, it can be seen that the COCs of the present invention are significantly different from the second and fourth generation COCs of the prior art. Taken together, the measured parameters demonstrate that the hemostatic profile of the COCs of the present invention is at least comparable to the safest second generation LNG-containing COCs, and shows a more favorable effect on hemostatic parameters than the fourth generation DRSP-containing COCs.
