Abstract
Description
[Technical field]
[0001] The present invention relates to a fixation device for use in radiotherapy. [Background technology] Radiation therapy is a treatment in which a patient's tumor is exposed to radiation.
[0002] Radiation therapy generally involves the following process: (1) The patient's irradiation position is determined, a fixation device is prepared to fix the patient in that irradiation position, and then a simulation is performed using a simulation device such as a CT simulator with the fixation device in place. Next, (2) a treatment plan is created on a computer called a treatment planning device, and verification work is performed. Finally, (3) the patient is placed in the desired irradiation position using the fixation device prepared in (1), and radiation is irradiated after accurate setup and position matching using surface markers, a laser sight, a light irradiation field, an image matching device, etc.
[0003] In this way, the main role in maintaining the irradiation position during radiation therapy is the immobilization device, and the immobilization device plays an important role in increasing the reproducibility of the irradiation position. Since a series of radiation therapy requires multiple irradiations, it is important to reproducibly change the irradiation position so that the same area can be irradiated in the same way every time.
[0004] In radiation therapy, there are conventionally used irradiation positions and fixation devices depending on the type of cancer and the site of irradiation. For example, in head and neck cancer, a planar thermoplastic shell is used to cover the body surface from the head to the neck. In addition, when performing stereotactic body irradiation for lung cancer, liver cancer, etc., a fixation device is used to hold the patient in a supine position with both arms elevated. In breast cancer, radiation therapy is primarily performed in the supine position with both arms elevated, or the prone position with both arms elevated and the breast hanging down, and there are immobilization devices suitable for each position.
[0005] Patent Document 1 discloses a fixation device for radiation therapy in a supine position for fixing both upper limbs in a raised state. To facilitate fixing the body position, a left arm rest and a right arm rest on which the raised arms of both upper limbs are placed, and a left hand grip bar and a right hand grip bar are arranged symmetrically between these arm rests. The patient holds the grip bars while raising the upper limbs, and maintains the raised state of both upper limbs. [Prior art documents] [Patent documents]
[0006] [Patent Document 1] Patent No. 5770592 Summary of the Invention [Problem to be solved by the invention]
[0007] However, when using the fixation device for radiotherapy as described in the above Patent Document 1, depending on the comorbidities and conditions of each individual patient, it may be difficult to use the conventional irradiation position and fixation device, and the accuracy of fixation of the patient may be reduced. For example, there are cases where a patient cannot fully raise his / her upper limbs due to various comorbidities such as shoulder joint disease and fracture. In this case, if a conventional fixation device for radiotherapy is used to perform radiotherapy, the upper limbs may interfere with the irradiation field, making it difficult to perform radiotherapy.
[0008] An object of the present invention is to provide a technique for solving the above-mentioned problems. [Means for solving the problem]
[0009] In order to achieve the above-mentioned object, the radiotherapy fixation device of the present invention is a radiotherapy fixation device used with a patient in a supine irradiation position, and is equipped with a back support part that can be positioned between the patient's back and a tabletop placed on a bed provided in a radiotherapy system, and legs that can be fixed to the tabletop.
[0010] In order to achieve the above-mentioned object, the radiotherapy fixation device of the present invention is a radiotherapy fixation device used with a patient in a supine irradiation position, and is equipped with a back support part that can be positioned between a top plate mounted on a bed provided in a radiotherapy system and the back of the patient, and a storage part that is positioned between the back support part and the top plate and can store the upper limbs of the patient. Effect of the Invention
[0011] According to the present invention, the shoulder joint can be easily extended in the dorsal direction in the supine position, and even if the upper limbs cannot be raised, radiation can be irradiated to the irradiation site in an appropriate irradiation position. [0013] <Radiation therapy system> The radiotherapy system S includes a radiotherapy device L for irradiating an irradiation site of a patient P with therapeutic radiation, and a bed D on which a tabletop T on which the patient takes an irradiation position is placed. The radiotherapy system S including a medical linac includes an aiming device (not shown) such as a laser sight used for patient aiming, an image matching device (not shown) for capturing an image of the patient P on the tabletop T and evaluating the positional deviation between the patient P in the treatment plan and the patient P in actual irradiation, and a moving device (not shown) for moving the tabletop T provided on the bed D in the horizontal and rotational directions, and can correct the patient's position based on the data obtained by the image matching device. A respiratory synchronization system, a moving body tracking system, a body surface monitoring system, and the like (not shown) may also be included.
[0014] <Radiation therapy fixture> The radiotherapy fixation device 1 of the present invention is used by being placed between a patient P and a tabletop T in order to support the patient P in a supine irradiation position within a radiotherapy system S, and is fixed at a predetermined position on the tabletop T by a holding mechanism (not shown). The radiotherapy fixation device 1 of the present invention includes a back support section 10 that supports the back of patient P, a neck support section 20 that supports the neck of patient P, a lumbar support section 30 that supports the lumbar region of patient P, a lower limb support section 40 that supports the lower limbs of patient P, a head support section 50 that supports the head of patient P, a leg section 60 that can be fixed to a tabletop T, and a storage section 70 in which patient P can place his or her upper limbs on the back side of patient P (Figure 1). The radiotherapy fixing device 1 of the present invention may be used by being placed on a support base that can be fixed onto a tabletop T. The radiotherapy fixing device 1 is configured to have a size that can be accommodated in a simulation device such as a CT simulator and a radiotherapy system S. The radiotherapy fixing device 1 can be configured to be adjustable in height H from the tabletop T and angle θ of the upper body of the patient P so as not to interfere with the gantry and image matching device of the radiotherapy device L.
[0015] <Back support part> The back support section 10 includes a back support member 11 that supports the back of the patient P. The back support section 10 can be placed between a tabletop T placed on a bed D included in the radiation therapy system S and the back of the patient P. The back support member 11 includes a portion that supports the midline of the patient P at the shoulders of the patient P. Therefore, when the patient P takes a supine position on the back support section 10, the shoulders of the patient P can be positioned outside the back support member 11. As a result, the back support member 11 (protruding portion 12) allows for adduction of the scapula, enabling the patient P to take an appropriate irradiation position. The back support part 10 is provided between the top board T of the bed D and the back of the patient P. Therefore, the patient P is positioned higher than the top board T, and the upper limbs can be moved toward the back of the patient P compared to a case without the back support part 10. As a result, the degree of freedom of the irradiation position is increased, and an appropriate irradiation position can be taken.
[0016] The back support section 10 may support not only the area around the shoulders of the patient P but also the back of the upper and lower body of the patient P. That is, the back support section 10 may include some or all of the areas where the neck support section 20, the lumbar support section 30, the lower limb support section 40, and the head support section 50, which will be described later, contact and support the back of the patient P. Therefore, the back support section 10 (back support member 11) may be integrated with the neck support section 20 (neck support member 21), the lumbar support section 30 (lumbar support member 31), the lower limb support section 40 (lower limb support member 41), and the head support section 50 (head support member 51), which will be described later, or each of them may be a separate body.
[0017] The back support material 11 may also have a protuberance 12 or a recess 13 to be placed on the back of the patient P. The protuberance 12 and the recess 13 are preferably symmetrical, but do not have to be symmetrical. When the back support material 11 has both the protuberance 12 and the recess 13, the protuberance 12 can be two or more symmetrical protuberances centered on the center of the back.
[0018] Additionally, the rear support member 11 is preferably rectangular, but the shape is not particularly limited.
[0019] <Cervical support> The neck support part 20 includes a neck support member 21 that supports the neck of the patient P. The neck support part 20 (neck support member 21) may be integral with the back support part 10 (back support member 11) and a head support part 50 (head support member 51) described below, or may be separate.
[0020] <Lumbar support part> The lumbar support part 30 includes a lumbar support member 31 that supports the lumbar region of the patient P. The lumbar support part 30 may include a cover 32 on the lumbar support member 31. The cover 32 prevents the patient P from falling off the radiotherapy fixation device 1. The cover 32 may include a cushion that is placed at the lumbar level of the patient P.
[0021] The lumbar support member 31 is preferably rectangular, but the shape is not particularly limited. The lumbar support member 30 (lumbar support member 31) may be integrated with the back support member 10 and the lower leg support member 40 (lower leg support member 41) described later, or may be separate from them.
[0022] <Lower limb support section> The lower limb support section 40 includes a lower limb support member 41 that supports the lower limbs of the patient P. The lower limb support member 41 can support or fix the lower limbs of the patient P in an extended or bent state. The lower limb support member 41 may include a cushion that contacts the area behind the knees of the patient P. The lower limb support section 40 may be integrated with the lumbar support section 30 (lumbar support member 31) or may be separate.
[0023] <Head support> The head support part 50 includes a head support member 51 that supports the head of the patient P. The head of the patient P is supported or fixed on the surface of the head support member 51 of the head support part 50. As shown in Fig. 1, the head support member 51 may include a fitting part 52 into which the head of the patient P fits. The head support part 50 may be integrated with the neck support part 20 (neck support member 21) or may be separate from it.
[0024] <Legs> The radiotherapy fixation device 1 includes legs 60 that can be fixed to the tabletop T by a holding mechanism (not shown). The legs 60 are appropriately provided on the back support section 10, the neck support section 20, the lumbar support section 30, the lower limb support section 40, and the head support section 50 in order to stably support and fix the radiotherapy fixation device 1 on the tabletop T. The legs 60 can be provided between the back support section 10 and the tabletop T. With this configuration, a space (accommodation section 70) capable of accommodating the upper limbs can be provided between the back of the patient P and the tabletop T. As a result, the patient P can take an appropriate irradiation position.
[0025] The legs 60 preferably include a front leg 61 that supports the head side of the radiotherapy fixation tool 1 and a rear leg 62 that supports the tail side of the radiotherapy fixation tool 1, and more preferably, the lower limb support part 40 and the head support part 50 are each provided with the front leg 61 and the rear leg 62 in order to stably support the patient P on the radiotherapy fixation tool 1 on the tabletop T. With this configuration, a space (accommodation part 70) capable of accommodating the upper limbs can be provided between the back of the patient P and the tabletop T. As a result, the patient P can take an appropriate irradiation position.
[0026] The legs 60 may be formed integrally with the back support section 10, the neck support section 20, the lumbar support section 30, the lower limb support section 40, and the head support section 50. In addition, when the back support section 10, the neck support section 20, the lumbar support section 30, the lower limb support section 40, and the head support section 50 are separate from the legs 60, the legs 60 and these members may be provided so as to be movable relative to each other. In this case, the radiotherapy fixture 1 has a fixing section for fixing these members and the legs 60. With this configuration, the legs 60 can be moved to a position on the tabletop T where the patient P can be stably supported, and the legs 60 and these members can be fixed at a position where the patient P can be stably supported, which is preferable because it increases the variation of the irradiation position of the patient P. Note that the number of the legs 60 is not limited to two, and the number of legs may be one or three or more.
[0027] <Containment Unit> The radiotherapy fixation device 1 of the present invention includes a storage section 70 in which the patient P can place his / her upper limbs. The storage section 70 is provided on the back or side of the patient P when the patient P is in a supine position in the radiotherapy fixation device 1. That is, the storage section 70 is provided to be located between the back of the patient P (back support section 10) and the tabletop T and capable of storing the upper limbs. With this configuration, it is possible to irradiate the irradiation site in an appropriate irradiation position without the upper limbs interfering with the irradiation field.
[0028] The storage section 70 may be provided so that the patient P can fix his / her upper limbs. In this case, the storage section 70 includes a gripping portion or the like that the patient P can hold with the palm of his / her hand. Examples of the gripping portion include a handle, a lever, and the like. In addition, the storage section 70 may include an upper limb cover for fixing the upper limbs.
[0029] Furthermore, when the radiotherapy fixation device 1 is placed on the tabletop T, the storage section 70 may be formed by the legs 60 between the back support section 10, the lumbar support section 30 or the lower limb support section 40 and the tabletop T. In this case, the storage section 70 becomes a space between the back support section 10, the lumbar support section 30 or the lower limb support section 40 and the tabletop T.
[0030] The height and width of the accommodation section 70 are not particularly limited, so long as an appropriate irradiation position can be taken and the upper limbs can be positioned therein. The material of the fixing tool for radiotherapy 1 of the present invention is not particularly limited, but it is preferable to use a radiotransparent material.
[0031] In the radiotherapy fixing device 1 of the present invention, the back support section 10 supporting the back of the patient P, the neck support section 20 supporting the neck of the patient P, the waist support section 30 supporting the waist of the patient P, the lower limb support section 40 supporting the lower limbs of the patient P, the head support section 50 supporting the head of the patient P, and the legs 60 fixable to the tabletop T may be integrated or may be separate. Also, some of the components may be integrated while the remaining components may be separate.
[0032] <First embodiment> Next, embodiments of the present invention will be described with reference to the drawings. First, a first embodiment of the present invention will be described.
[0033] 2 and 3, the back support part 10A of the radiotherapy fixing device 1A includes a back support member 11A that supports the back of the patient P, and a neck support part 20A. That is, the back support part 10 includes the neck support part 20. The back support member 11 includes a raised part 12A and a recessed part 13A.
[0034] In this embodiment, the back support part 10A is generally rectangular with a length direction along the midline of the patient P and a width direction intersecting the length direction. The back support part 10A can be placed on the tabletop T or a support base that can be placed on the tabletop T. Although the back support part 10A and the neck support part 20A are integral with each other in FIG. 3, these members may be configured as separate members.
[0035] The back support member 11 (raised portion 12A) has a recess 13A extending in the longitudinal direction at the center. When the patient P is placed in a supine position, the recess 13A is positioned so that the midline (spine) of the patient P fits along it. As a result, the midline of the patient P can be appropriately fixed. This is particularly preferable as it enables the position of a patient with kyphosis, whose spine is tilted forward, to be fixed.
[0036] The neck support part 20A includes neck covers 22A on both sides of the neck support material 21A. The neck covers 22A have a shape that fits the neck of the patient P. This allows the neck of the patient P to be securely fixed by the neck support part 20A.
[0037] A narrow portion 14A is provided between the back support member 11A and the neck support member 20A (neck support member 21A). The narrow portion 14A is narrower in the width direction than the protruding portion 12A and the neck support member 20A, and has a constricted shape. The narrow portion 14A has a width that supports at least the midline (around the spine) of the patient P. Therefore, when the patient P takes a supine position with the radiotherapy fixation device 1A, the midline (around the spine) of the patient P is supported and fixed, while at least a part of the scapula of the patient P is not fixed, so that the range of motion of the shoulder joint of the patient P is increased, and the shoulder joint can be easily extended in the dorsal direction, allowing the patient P to take an appropriate irradiation position.
[0038] When the patient P lies supine on the back support part 10A, the back of the patient P is supported, the neck is supported and fixed, and the upper limbs of the patient P can be positioned outside the back support material 11A. The back support part 10 is provided between the top board T of the bed D and the back of the patient P. Therefore, the patient P is positioned higher than the top board T, and the upper limbs can be moved toward the back side compared to a case without the back support part 10. As a result, the degree of freedom of the irradiation position is increased, and an appropriate irradiation position can be taken.
[0039] The raised portion 12A is provided in the center of the back support portion 10A. The raised portion 12A has a slope from the apex toward the back support member 11A. When the back of the patient P is placed on the raised portion 12, the patient P opens his / her chest on the back support portion 10A. As a result, the patient P can take an appropriate irradiation position.
[0040] As another example of this embodiment, the rear support part 10a may include a rear support material 11a, and the rear support material 11a may include a plurality of raised portions 12a (FIG. 4). The rear support part 10a may be placed on a top plate T or a support base that can be placed on the top plate T. The rear support material 11a behind the plurality of raised portions 12a is a flat portion 15a, and a recess 13a may be provided therein.
[0041] The plurality of protuberances 12a come into contact with the periphery of the spine and shoulder blades when the patient P is in a supine position. The head and neck of the patient P are supported by the head support portion 50a and the neck support portion 20a, respectively.
[0042] In this state, when patient P lies supine on back support part 10a, back support part 10a has multiple protrusions 12a, which allow patient P's own weight to push up around the spine, opening the chest further and allowing patient P to assume an appropriate irradiation position (Figure 5).
[0043] In other embodiments, the back support portion 10a and the neck support portion 20a may be separate bodies.
[0044] The apexes of the raised portions 12A and 12a come into contact with the back of the patient P, and therefore may be made of a soft material such as urethane to reduce the burden on the patient.
[0045] As an example of the lower limb support part 40, the lower limb support part 40A is provided with a triangular prism-shaped lower limb support member 41A so as to support and fix the lower limb in a bent state (FIG. 6). This makes it easier to support the lower limb of the patient P and fix the lower limb in a bent state.
[0046] Another embodiment of the lower limb support part 40 is shown in Fig. 7. The lower limb support part 40A has a lower limb support member 41a, and the lower limb support member 41a has a mold 42a whose slope contacts the lower limb of the patient P so that the lower limb fits into the mold 42a. The mold 42a is configured so that the thigh, knee, and calf fit into the mold 42a. This makes it easier for the lower limb of the patient P to fit into the mold 42a and to fix the lower limb in a bent state.
[0047] The head support portion 50A may be anything that supports the head of the patient P protruding from the neck support portion 20, and may be, for example, a pillow.
[0048] The legs 60A include front legs 61A extending from the neck support section 20A to the head support section 50A, and rear legs 62A extending from the lumbar support section 30A to the lower limb support section 40A. As a result, an accommodation section 70A capable of accommodating the upper limbs of the patient P is formed between the table top T and the back support section 10A, the lumbar support section 30A, or the lower limb support section 40A.
[0049] The lower back support section 30A, the lower leg support section 40A, and the head support section 50A may be integrated or may be separate.
[0050] Furthermore, the location of the legs 60A is not particularly limited as long as the accommodation section 70A is provided and the patient P can be stably supported in the supine position. The accommodation section 70A is not limited to being provided between the table top T and the back support section 10A, the lumbar support section 30A or the lower limb support section 40A, and may be configured by providing a hole in the legs 60A, for example.
[0051] In this embodiment, the back support part 10A may extend toward the head side beyond the neck cover 22A and include a head support part 50A. The back support part 10A may extend toward the lower leg side beyond the raised part 12A or the flat part 15a and include a lower leg support part 40A.
[0052] <Second embodiment> Next, a second embodiment of the present invention will be described, focusing on the configuration different from the first embodiment.
[0053] The back support section 10B of the radiotherapy fixation tool 1B according to the second embodiment includes a neck support section 20B, a lumbar support section 30B, and a lower limb support section 40B. That is, the back support section 10B includes the neck support section 20B, the lumbar support section 30B, and the lower limb support section 40B. The lower limb support section 40B includes a triangular prism-shaped lower limb support member 41B so that the lower limbs can be supported and fixed in a bent state. As shown in FIG. 8, the back support section 10B is inclined from the neck support section 20B toward the lumbar support section 30B, and the patient P can be fixed in a semi-seated state. The angle θ and height H of the back support section 10B may be adjusted by an adjustment mechanism that can adjust the angle and height of the radiotherapy fixation tool 1B. Alternatively, the angle θ and height H of the back support section 10B may be predetermined.
[0054] In this embodiment, the fixing tool 1 for radiotherapy includes a stopper 43B for fixing the buttocks on the tabletop T. This makes it easy to fix the patient P on the tabletop T in a semi-sitting position.
[0055] The head support part 50B may be integrated with the back support part 10B or may be separate, so long as it supports the head of the patient P protruding from the neck support part 20B. It is necessary to be able to adjust the height H and angle θ so that the head of the patient P and the head support part 50B do not interfere with the gantry and image matching device of the radiation therapy device L.
[0056] The legs 60B include a front leg 61B provided on the neck support 20B and a rear leg 62B extending from the lumbar support 30B to the lower leg support 40B. The legs 60B form an incline from the neck support 20B of the back support 10B toward the lumbar support 30B. A storage section 70B is formed between the top board T and the back support 10B.
[0057] In FIG. 8, the front legs 61B are not in contact with the top board T and are not supported thereon, but the front legs 61B may be in contact with the top board T and supported thereon.
[0058] In addition, the position where the legs 60B are provided is not limited as long as the accommodation section 70B is provided and the patient P can be stably supported and fixed in a semi-sitting position. The accommodation section 70B is not limited to being provided between the table top T and the back support section 10A or the lumbar support section 30A, and may be configured by providing holes in the legs 60B, for example.
[0059] In this embodiment, the rear legs 62B extending from the lumbar support section 30B to the lower limb support section 40B are not an essential component. In this case, the front legs 61B are supported and fixed on the tabletop T to support the upper body of the patient P. Furthermore, by providing the rear legs 62B on the lumbar support section 30B, the radiotherapy fixing device 1B can be stably fixed on the tabletop T. The buttocks of the patient P may be placed on the tabletop T of the bed D, and in this case, the lower body of the patient P is supported and fixed on the tabletop T by the stopper 43B and the lower limb support section 40B (lower limb support member 41B).
[0060] <Third embodiment> A third embodiment of the present invention will be described below, focusing on configurations different from the first and second embodiments.
[0061] A lower limb support part 40C of a radiotherapy fixing device 1C according to the third embodiment includes a lower limb support member 41C that supports the lower limbs of a patient P from the dorsal side, and a lower limb cover 42C that fixes the lower limbs from the ventral side (FIG. 9). This ensures that the feet of the patient P are fixed.
[0062] Another embodiment of the lower limb support part 40C includes a foot cover 43C for fixing the feet of the patient P (FIG. 10). The foot cover 43C includes a sole support part 44C that contacts the sole of the foot, and a side cover 45C that covers the side of the foot. This ensures that the soles of the feet of the patient P are fixed in place. In addition, the patient P can be prevented from falling off the tabletop T.
[0063] The lower limb support portion 40C is a lower limb support material 41C that supports the lower limbs from the dorsal side once the patient P's feet are fixed, and the lower limb cover 42C that fixes the lower limbs from the ventral side is not required, and the lower limb support material 41C that fixes the patient P's lower limbs from the side is also acceptable.
[0064] <Other embodiments> In the first embodiment, the back support part 10A may be disposed on a support base that can be disposed on the top board T. In this case, the support base may include a head support part, a lumbar support part, and a lower limb support part, and the back support part may be disposed between the head support part and the lumbar support part.
