Abstract
Description
[Technical field]
[0001] The present disclosure relates to systems and methods for improved navigation through body passageways and enhanced visualization and sampling of target tissue. [Background technology]
[0002] Navigation and visualization within body passageways with ultrasound probes is difficult to prevent damage to the probe and to optimize visualization. For example, radial endobronchial ultrasound (R-EBUS) offers a minimally invasive option when clinical symptoms indicate the need for tissue biopsy within the lung system. The ultrasound element of such lung tissue biopsy systems is important for visualization of target nodules within the lung passageways. Ultrasound energy can only propagate through materials such as polymers with specific densities and refractive indices, which requires that the ultrasound probe be extendable beyond the distal end of the delivery catheter. Due to dimensional constraints around the lung, the exposed portion of the ultrasound probe tends to be small and fragile, and therefore vulnerable to damage. Without a tapered / atraumatic tip on the exposed portion, even a gentle bump along the wall of the lung passageway can cause the ultrasound probe to kink and / or break. Additionally, visualization of target nodules in larger lung passageways tends to be compromised by the presence of open spaces (eg, air) between the ultrasound probe and tissue walls that block / attenuate the propagation of ultrasound signals. [0004] Thus, various advantages may be realized by a system that allows for improved navigation through narrow body passageways and enhanced visualization of target tissue within larger body passageways. [Means for solving the problem]
[0005] The present disclosure, in its various aspects, provides the advantage of a sampling system that may enable improved navigation through body passageways and enhanced visualization and sampling of target tissue in medical fields, such as the field of pulmonary endoscopy. In various embodiments, the sampling system includes an end cap that protects the exposed portion of the ultrasound probe and enhances contact and propagation of ultrasound signals with the walls of the body passageway for improved visualization of the target tissue.
[0006] In one aspect, the disclosure relates to a system comprising an elongate member including first and second working channels defining respective first and second openings. An ultrasonic probe is disposed within the first working channel such that a distal portion of the ultrasonic probe is extendable distally beyond the distal end of the elongate member. An end cap is attached to the distal end of the elongate member such that the end cap surrounds the distal portion of the ultrasonic probe when the ultrasonic probe extends outside the first opening. A tissue sampling element is slidably disposed within the second working channel. The first working channel extends between the proximal and distal ends of the elongate member to define a first opening. The second working channel extends between the proximal end of the elongate member and an outer surface of the elongate member to define a second opening. The second working channel defines an angled surface at the second opening within the distal portion of the elongate member. The end cap comprises a hard plastic material. The end cap is filled with a conductive material. The end cap comprises an expandable member configured to transition between a first configuration and a second configuration. The expandable member is transitionable from the first configuration to the second configuration by flowing inflation fluid into an interior region of the expandable member. The expandable member is transitionable from the second configuration to the first configuration by flowing inflation fluid out of an interior region of the expandable member. The ultrasonic probe may be fixed within the first working channel. The ultrasonic probe may be slidable within the first working channel. The end cap may be attached around a distal end of the elongate member. The end cap may be attached around (e.g., around) a first opening at the distal end of the elongate member.
[0007] In another aspect, the disclosure relates to a system comprising an elongate member including first and second working channels defining respective first and second openings. An ultrasonic probe is disposed within the first working channel such that a distal portion of the ultrasonic probe is extendable distally beyond the distal end of the elongate member. An end cap is attached to the distal portion of the ultrasonic probe. A tissue sampling element is slidably disposed within the second working channel. The first working channel extends between a proximal end and a distal end of the elongate member to define a first opening. The second working channel extends between a proximal end of the elongate member and an outer surface of the elongate member to define a second opening. The second working channel defines an inclined surface at the second opening within the distal portion of the elongate member. The end cap comprises an expandable member configured to transition between a first configuration and a second configuration. The expandable member is transitionable from the first configuration to the second configuration by flowing an inflation fluid into the interior of the expandable member. The expandable member is transitionable from the second configuration to the first configuration by exiting an inflation fluid from an interior region of the expandable member. The ultrasonic probe may be fixed within the first working channel. The ultrasonic probe may be slidable within the first working channel.
[0008] In another aspect, the disclosure relates to a method including advancing a system into a body passageway. The system includes an elongate member, an ultrasonic probe disposed within a first working channel of the elongate member, and an expandable member attached to a distal end of the elongate member. A distal portion of the ultrasonic probe extends distally beyond the distal end of the elongate member. The expandable member is attached to the distal end of the elongate member so as to surround the distal portion of the ultrasonic probe. The expandable member transitions from a first configuration to a second configuration to position the expandable member in contact with a wall of the body passageway adjacent the target tissue. An ultrasonic image of the body passageway and / or the target tissue is obtained using the ultrasonic probe. The expandable member is advanced through the body passageway in the second configuration. The method further includes advancing the tissue sampling element distally into the target tissue beyond the second opening such that a portion of the target tissue is captured within the lumen of the tissue sampling element. The system is withdrawn from the body passageway, and the tissue sample captured within the lumen of the tissue sampling element is analyzed.

[0015] The present disclosure generally provides a tissue biopsy system for improved navigation and visualization of large and small body passageways. Specifically, the disclosed tissue biopsy system generally comprises an elongate member having a working channel for accommodating respective tissue sampling elements and an ultrasonic probe, and an end cap configured to protect / support an exposed portion of the ultrasonic probe and maintain direct tissue wall contact with body passageways of various sizes and / or shapes. In various embodiments, the ultrasonic probe may be fixedly or slidably disposed within a first working channel of the elongate member. Additionally, a distal portion of the ultrasonic probe including an ultrasonic transducer extends distally through a first opening of the elongate member and beyond a distal end of the elongate member.
[0016] 1A, in one embodiment, the tissue biopsy system of the present disclosure comprises an elongate member 110 (e.g., a catheter, a delivery device, etc.) including first and second working channels 114, 116 that define respective first and second openings 115, 117. The first and second openings 115, 117 are not limited to being generally circular and oval, respectively, and may have a variety of other shapes and / or configurations. The first working channel 114 extends between a proximal end (not shown) and a distal end 112 of the elongate member 110 to define the first opening 115. The second working channel 116 extends between a proximal end (not shown) and an outer surface 113 of the elongate member 110 to define the second opening 117. The second working channel 116 defines an angled surface 118 at the second opening 117 within a distal portion of the elongate member 110. The angled surface 118 is angled from about 5 degrees to about 10 degrees, or any angle therebetween, relative to the longitudinal axis of the elongate member 110. The second working channel 116 is configured to slidably receive a tissue sampling element 150, such as a biopsy needle.
[0017] The first working channel 114 is configured to receive an ultrasonic probe 120 having an ultrasonic transducer 124 rotatably disposed on its distal end. In one embodiment, the ultrasonic probe 120 may be fixedly disposed within the first working channel 114. In another embodiment, the ultrasonic probe 120 may be slidably disposed within the first working channel. A distal portion 122 of the ultrasonic probe 120, including the ultrasonic transducer 124, extends distally beyond the distal end 112 through the first opening 115 of the elongate member 110.
[0018] The end cap 130 is attached to the distal end 112 of the elongate member 110 to enclose and protect the ultrasound transducer 124. In one embodiment, the end cap 130 comprises a polymeric material, including but not limited to high density polyethylene (HDPE), having a suitable thickness and refractive index to allow unattenuated propagation of ultrasound energy therethrough. For example, the end cap 130 comprises a hard plastic material with an atraumatic end (e.g., cone-shaped, bullet-shaped, etc.) to navigate through tortuous and narrow lung passages. The end cap 130 is permanently attached to the distal end 112 of the elongate member 110 by suitable welding, soldering, brazing, adhesives, epoxies, glues, and / or resins. Additionally or alternatively, the end cap 130 may be reversibly attached to the distal end 112 of the elongate member 110 by at least one of a snap fit, a press fit, an interference fit, and / or a compressive fit. The end cap 130 defines an open (e.g., hollow) interior space that is filled with a conductive material 140 (e.g., polyethylene glycol (PEG), petroleum gel, carbowax, saline, etc.) that eliminates the need to flush the ultrasound probe with saline throughout the course of the medical procedure. In embodiments in which the end cap 130 is reversibly attached to the elongate member 110, the end cap is filled or refilled with the conductive material immediately prior to use. The end cap 130 has an outer diameter approximately equal to the outer diameter of the elongate member 110 to provide radial (e.g., 360 degree) contact with the tissue wall of the lung passageway. The ability of the end cap 130 to directly contact the entire inner circumference of larger lung passageways provides improved ultrasound imaging by eliminating spaces (e.g., air) that would block / attenuate ultrasound energy from contacting / transmitting through tissue walls. Additionally, the atraumatic contour and low friction outer surface allows the end cap 130 to maintain full circumferential contact with tissue as the tissue biopsy system is advanced into the narrower lung periphery.Referring to FIG. 1B, the tissue sampling element 150 is advanced distally along the inclined surface 118 of the second working channel 116 such that the tissue sampling element 150 deflects (e.g., bends) away from the longitudinal axis of the elongate member 110 without contacting the end cap 130 as it exits the second opening 117.
[0019] In one embodiment, referring to FIG. 2A, the distal end 112 of the elongate member 110 includes an end cap 230 (e.g., expandable member, balloon, etc.) that includes one or more elastomeric and / or compliant materials having a suitable thickness and refractive index to allow unattenuated propagation of ultrasonic energy therethrough. Semi-compliant materials include, by way of non-limiting example, polyester elastomers (e.g., Arnitel®, Hytrel®, etc.). Compliant materials include, by way of non-limiting example, latex and silicone. As will be appreciated by those skilled in the art, end caps formed from compliant materials can be infinitely expanded (e.g., do not have a fixed final diameter). For example, these materials preferably have a compliance in the range of 10%-800%, more preferably in the range of 50%-200%. The end cap 230 is permanently attached to the distal end 112 of the elongate member 110 by a suitable adhesive, epoxy, glue and / or resin. Alternatively, the end cap 230 may be reversibly attached to the distal end 112 of the elongate member 110 by at least one of a snap fit, press fit, interference fit, and / or compression fit. The end cap 230 is configured to transition between a first configuration (e.g., a contracted configuration, a pre-inflated configuration, etc.) and one or more second configurations (e.g., an expanded configuration, an inflated configuration, etc.) by flowing an inflation fluid (e.g., saline, gel, etc.) between an external fluid source (not shown) and an interior region 236 of the end cap. In one embodiment, the elongate member 110 includes a dedicated inflation fluid delivery lumen 128 extending between the proximal end (not shown) and the distal end 112 in fluid communication with the external fluid source and the interior region 236. Alternatively, the inflation fluid may flow through the first working channel 114 of the elongate member 110 and in the annular space around the ultrasonic probe 120 between an external fluid source and the interior region 236 of the end cap 230 .
[0020] In the first configuration, the end cap 230 folds or collapses around the distal portion 122 of the ultrasonic probe 120 to provide a reduced profile (e.g., smaller diameter) for the tissue biopsy system to pass through a working channel of a medical device (e.g., a bronchoscope, etc.) In addition, the reduced profile of the end cap 230 provides structural support to the distal portion 122 of the ultrasonic probe 120 to prevent kinking and / or bending as the tissue biopsy system is advanced through the pulmonary passageways.
[0021] An end cap having a compliant or semi-compliant material allows the end cap 230 to transition from a first configuration (FIG. 2A) to various second configurations (FIGS. 2B-2E) depending on the particular dimensions of the lung passageway. Once the tissue biopsy system is positioned within the portion of the lung passageway to be visualized, inflation fluid is introduced into the interior region 236 of the end cap 230 until the outer surface of the end cap 230 is placed in direct contact with the inner circumference of the lung passageway. Although FIGS. 2B-2E show the end cap 230 expanding in a generally uniform or symmetrical fashion, the compliant or semi-compliant material may allow the end cap 230 to expand asymmetrically to establish full contact with irregularly or non-uniformly shaped body passageways (e.g., bifurcations, etc.). The atraumatic contour and low-friction outer surface of the end cap 230 maintain full circumferential contact with the tissue as the tissue biopsy system is advanced into the narrower lung periphery. Inflation fluid may be introduced into or removed from interior region 236 of end cap 230 as necessary to maintain contact with the tissue walls of the lung passageway throughout the medical procedure. Regardless of the degree of inflation of end cap 230, tissue sampling element 150 is advanced distally along angled surface 118 of second working channel 116 such that tissue sampling element 150 deflects (e.g., bends) away from the longitudinal axis of elongate member 110 without contacting end cap 230 upon exiting second opening 117.
[0022] In another embodiment, the end cap 230 includes one or more non-compliant materials configured to transition from a first configuration to a second configuration of single / constant diameter. Non-compliant materials include, by way of non-limiting example, polyethylene terephthalate (PET). Compared to compliant or semi-compliant materials, the non-compliant materials provide increased structural support for the distal portion 122 of the ultrasonic probe 120 when the end cap 230 is in the first configuration. The non-compliant materials also provide increased structural support when in the second configuration, for example, to better control the forces exerted on the inner circumference of the lung passageway and / or to prevent penetration due to unintended contact with the tissue sampling element 150.
[0023] 3A-3B, in another embodiment, the tissue biopsy system of the present disclosure includes an end cap 330 comprising a compliant, semi-compliant, or non-compliant material attached to a distal portion 122 of an ultrasonic probe 120. The end cap 330 transitions between a first configuration (FIG. 3A) and a second configuration (FIG. 3B) by flowing an inflation fluid (such as saline) between an external fluid source (not shown) and an interior region 336 of the end cap 330. For example, the inflation fluid flows between an external fluid source and the interior region 336 of the end cap 330 through a dedicated fluid delivery lumen (not shown) that extends along or through the ultrasonic probe 120. Alternatively, the end cap 330 may be attached to the distal end 112 of the elongated tubular member 110 about the first opening 115 such that inflation fluid may flow between an external fluid source and an interior region 336 of the end cap 330 through the first working channel 114 of the elongated member 110 and within the associated space around the ultrasonic probe 120. In the first configuration, the end cap 330 folds or collapses around the distal portion 122 of the ultrasonic probe 120 to provide a reduced profile (e.g., smaller diameter) for the tissue biopsy system to pass through the working channel of a medical device (e.g., a bronchoscope, etc.). Additionally, the end cap 330 provides structural support to the distal portion 122 of the ultrasonic probe 120 to prevent kinking and / or bending as the tissue biopsy system is advanced through the pulmonary passageways.
[0024] Once the tissue biopsy system is positioned within the portion of the lung passageway that the medical professional wishes to visualize, inflation fluid is introduced into the interior region 336 of the end cap 330 until the outer surface of the end cap is placed in direct contact with the inner periphery of the lung passageway. As described above, end caps 330 comprising compliant or semi-compliant materials may expand asymmetrically to establish full contact with irregular or non-uniformly shaped body passageways. The atraumatic contour and low friction outer surface of the end cap 330 maintain full circumferential contact with the tissue as the tissue biopsy system is advanced into the narrower lung periphery. Inflation fluid may be introduced into or removed from the interior region 336 of the end cap 330 as needed to maintain contact with the tissue walls of the lung passageway throughout the medical procedure. Regardless of the degree of expansion of the end cap, the tissue sampling element 150 is advanced distally along the inclined surface 118 of the second working channel 116 such that the tissue sampling element 150 deflects (e.g., bends) away from the longitudinal axis of the elongate member 110 without contacting the end cap 330 as it exits the second opening 117.
[0025] 4A-4B, in one embodiment, the tissue biopsy system of the present disclosure includes two or more (compliant, semi-compliant, or non-compliant) end caps 430 attached to the distal portion 122 of the ultrasonic probe 120. The end caps 430, when in the first configuration (FIG. 4A), provide additional structural support to the distal portion 122 of the ultrasonic probe 120 compared to a single end cap, such as the configuration of FIG. 3A. In addition, each of the end caps 430 includes a separate / dedicated fluid delivery lumen (not shown) such that each end cap can independently transition between the first and second configurations as needed to maintain full circumferential contact with an irregular or non-uniformly shaped body passageway. For example, inflation fluid flows between an external fluid source and an interior region of the end caps 430 through separate dedicated fluid delivery lumens (not shown) that extend along or through the ultrasonic probe 120.
[0026] In use, by way of example, the biopsy system disclosed herein is advanced into a body passageway (e.g., peripheral lung) such that the end cap 130, 230, 330, 430 is positioned in radial (e.g., 360 degree) contact with a tissue wall of the body passageway. Upon identification of target tissue within or adjacent to the tissue wall, the tissue sampling element 150 is advanced distally along the angled surface 118 of the second working channel 116 such that the tissue sampling element 150 is deflected (e.g., bent) away from the longitudinal axis of the elongate member 110 and into the target tissue without contacting the end cap 130, 230, 330, 430. The biopsy system is then withdrawn from the body passageway and the target tissue captured within the lumen of the target tissue sampling element 150 is removed for analysis. Alternatively, the tissue sampling element 150 may be retracted proximally along the angled surface 118 of the second working channel 116, the biopsy system advanced through the body passage to one or more additional target tissue sites, and the tissue sampling element 150 advanced distally to capture additional target tissue for analysis.
[0027] Although the disclosed end caps have been described with respect to the specific embodiment shown in Figures 1A-4B, in various embodiments, the disclosed tissue biopsy system may include any number of compliant, semi-compliant, or non-compliant end caps symmetrically or asymmetrically attached to the distal end 112 of the elongate member 110 or the distal portion 122 of the ultrasound probe 120. The inner or outer surface of an end cap, such as any of the disclosed end caps 230, 330, 430, may include one or more pressure sensors to ensure that the end cap does not overstretch the lung passageway causing distension of the lung passageway. Similarly, the end cap 130 of Figures 1A-1B may include one or more pressure sensors to ensure that excessive force is not exerted against the tissue wall as the tissue biopsy system is advanced into the peripheral lung. In addition to providing improved visualization of the lung passageway, any of the disclosed end cap configurations 130, 230, 330, 430 provide a further benefit by reducing the likelihood that the tissue sampling element will miss the target tissue by securing and / or immobilizing the tissue biopsy system within the body passageway to minimize longitudinal and / or rotational movement when the tissue sampling element is actuated. A further advantage of any of the disclosed end cap configurations is that a portion of the lung passageway adjacent the target tissue can be placed under tension such that the target tissue is at least partially immobilized for more precise / accurate biopsy by the tissue sampling element.
[0028] The medical devices of the present disclosure are not limited to bronchoscopes, but may include a variety of medical devices for accessing body passageways including, for example, catheters, ureteroscopes, duodenoscopes, colonoscopes, arthroscopes, cystoscopes, hysteroscopes, etc. Finally, although embodiments of the present disclosure are described for use with a bronchoscope, the tissue biopsy system of the present disclosure may be placed within a patient in the absence of an associated medical device.
[0029] All of the devices and / or methods disclosed and claimed herein can be made and executed without undue experimentation in light of the present disclosure. While the devices and methods of this disclosure have been described in terms of preferred embodiments, it will be apparent to those skilled in the art that variations can be made to the devices and / or methods described herein, and in the steps or sequence of steps of those methods, without departing from the concept, spirit and scope of the present disclosure. All such similar substitutes and modifications apparent to those skilled in the art are deemed to be within the spirit, scope and concept of the present disclosure as defined by the appended claims.
