AI Technical Summary
Conventional endoscopic forceps with monopolar electrodes face difficulties in cutting wide areas and fibrotic tissues efficiently, requiring frequent replacements and laborious procedures, as they struggle to balance cutting and hemostasis tasks effectively.
The endoscope scissors integrate a scissors mechanism with a monopolar electrode at the tip of one scissor member, allowing for simultaneous cutting and hemostasis without replacement, featuring a support shaft for opening and closing, and a monopolar electrode aligned with the scissors' centerline to maintain directional accuracy.
This design enables quick and efficient cutting of wide areas and fibrotic tissues, reduces treatment time and effort, and stabilizes the treatment process by preventing electrode misalignment, allowing for various treatments without replacing the scissors.
Abstract
Description
Endoscopic Scissors
[0001] The present invention relates to endoscopic scissors that are extended from the distal end of an insertion portion of an endoscope for treatment.
[0002] A variety of devices (treatment tools) are available for endoscopy. For example, forceps are used to grasp, compress, or pinch tissue, and scissors are used to cut tissue. Other devices that use high-frequency current to cut tissue or fuse tissue for hemostasis include monopolar electrode electrosurgical scalpels and bipolar electrode forceps.
[0003] For example, Patent Document 1 describes a multifunctional device for endoscopic surgery with forceps or clamps, in which the jaw portion that allows grasping or holding of tissue is also formed as a bipolar electrode and a circular disk-shaped monopolar electrode is incorporated into the solid jaw portion.
[0004] Furthermore, Patent Document 2 describes a bipolar forceps having a pair of opposing jaw members, one of which is provided with a monopolar extension portion that houses a selectively extendable monopolar element.
[0005] JP-T-2009-533109 A JP-A-2011-212449 A
[0006] As described above, the conventional endoscopic forceps described in Patent Documents 1 and 2 are equipped with a monopolar electrode, allowing for grasping of tissue while cutting or hemostasis using the monopolar electrode. However, when cutting a wide area of tissue quickly, it is necessary to remove the conventional endoscopic forceps equipped with the monopolar electrode from the endoscope port and replace it with endoscopic scissors. Furthermore, if the area to be cut has become fibrotic tissue with little moisture, it is difficult to cut it with the high-frequency current from the monopolar electrode, and in this case, it is necessary to replace it with endoscopic scissors, which makes the procedure time-consuming and laborious.
[0007] SUMMARY OF THE INVENTION It is therefore an object of the present invention to provide endoscopic scissors that can reduce the labor required during treatment and shorten the treatment time.
[0008] The endoscopic scissors of the present invention are characterized by comprising a scissors mechanism section in which a pair of scissors members are connected by a support shaft and cut a target area by opening and closing the pair of scissors members using the support shaft as a fulcrum, and an electrode section in which a monopolar electrode is formed at the tip of one of the pair of scissors members.
[0009] With the endoscopic scissors of the present invention, the operator can use the scissors mechanism consisting of a pair of scissors members to quickly cut a target area over a wide area, or use the monopolar electrode to stop bleeding and cut the target area, so that a variety of procedures can be performed without having to replace the endoscopic scissors of the present invention.
[0010] The electrode section can be formed at a position where, when the pair of scissors members are closed, a center line of the scissors mechanism section passing through the support shaft coincides with a center line of the monopolar electrode along the protruding direction. Even when the insertion section of the endoscope is rotated around its axis, causing the endoscopic scissors of the present invention to rotate around its axis, the monopolar electrode is formed at a position where the center line of the scissors mechanism section coincides with a center line of the monopolar electrode along the protruding direction, so that the direction of the monopolar electrode directed toward the target site can be prevented from shifting from the target site, allowing treatment to be performed in a stable state.
[0011] The electrode portion may have a base having the same thickness as the one of the scissors members and provided on the thickness surface of the one of the scissors members. The one of the scissors members and the monopolar electrode may be formed by punching from a single metal plate, and the one of the scissors member and the monopolar electrode may be cut out integrally. Therefore, manufacturing is easy.
[0012] The electrode portion may be formed so that the tip portion is shortened in the width direction (thickness direction) perpendicular to the thickness direction while maintaining the thickness. Since the tip portion of the monopolar electrode can be formed in a pointed shape, an arc discharge can be reliably emitted from the tip portion toward the target site, and treatment can be performed on the target site.
[0013] The electrode portion may include a base portion formed of a rod-like body and a block portion formed at the tip of the base portion. By providing the block portion on the base portion formed of a rod-like body, the block portion may be formed into a hemispherical body, a cylindrical body, a polygonal pillar body, a body with an ellipsoidal cross section, a conical body, or the like.
[0014] The block portion may be formed in a polygonal prism shape. By forming the block portion in a polygonal prism shape, the high-frequency current is more likely to concentrate at a pointed tip, causing an arc discharge. Therefore, by forming the block portion in a polygonal shape, the arc discharge can be generated from the corner closest to the tissue.
[0015] The block portion may be formed in an octagonal prism shape. If the block portion is shaped like an octagonal prism, it is easier to aim and to direct high frequency current to a desired location.
[0016] The other of the pair of scissors members may be a cutter having a blade portion, and one of the pair of scissors members may be a die having a first surface that receives the target area and a second surface that faces the relief surface of the cutter when the cutter moves in the cutting direction.
[0017] The endoscopic scissors of the present invention can be used for various treatments without replacement, which reduces the time and effort required for treatment and shortens the treatment time.

[0019] Endoscopic scissors according to an embodiment of the present invention will be described with reference to the drawings. In this specification, the scissors side will be referred to as the distal end or front side, and the wire side will be referred to as the proximal end or rear side. The direction in which the wire is pushed to open the scissors will be referred to as the advancing direction, and the direction in which the wire is pulled to close the scissors will be referred to as the retreating direction.
[0020] The endoscopic scissors 10 shown in Figures 1A and 1B are inserted through the forceps port of an endoscope with the scissors mechanism unit closed, and are ejected from a suction port at the tip of the insertion section to cut tissue at a target site. The endoscopic scissors 10 include a scissors mechanism unit 20, a link mechanism unit 30, and an electrode unit 40. For example, the scissors mechanism unit 20 according to this embodiment is formed to have a length from the base end to the tip end of approximately 6 mm. Furthermore, each part of the scissors mechanism unit 20 is formed to have a thickness of approximately 0.5 mm. The electrode unit 40 is formed to have a thickness of approximately 1.5 mm.
[0021] The scissors mechanism 20 is formed in an X-shape and includes a pair of scissors members (one scissors member 22 and the other scissors member 21) and a first support shaft S1 (support shaft) that connects the pair of scissors members 21, 22 so that they can be opened and closed. The scissors member 21 has a cutting side 21p formed in a substantially V-shape from the distal end 21x to a central portion 21y where a through-hole 21t for inserting the first support shaft S1 is formed, and an operating side 21q formed in a shape that gradually narrows from the central portion 21y to a proximal end 21z. A through-hole 21u is formed in the proximal end 21z of the scissors member 21 for connection to the link mechanism 30.
[0022] A blade surface with a gradually decreasing thickness is formed on the inner surface of the cutting side 21p of the scissors member 21, so that the scissors member 21 functions as a cutter having a blade portion 21b. In this embodiment, as shown in Fig. 4, the angle θ (blade angle) formed by the rake surface 21c inclined with respect to the cutting direction F1 (closing direction) of the scissors member 21 and the relief surface 21d parallel to the cutting direction F1 is set to 20° to 40°.
[0023] 1A and 1B and 2B, the scissors member 22 has a cutting side 22p extending from the distal end 22x to the central portion 22y, where a through-hole 22t for inserting the first support shaft S1 is formed, that is formed in a substantially V-shape, and an operating side 22q extending from the central portion 22y to the proximal end 22z is formed in a shape that gradually narrows in width, similar to the scissors member 21. A through-hole 22u for connecting to the link mechanism 30 is formed in the proximal end 22z of the scissors member 22.
[0024] The inner surface of the cutting side 22p of the scissors member 22 functions as a die having a first surface 22m that receives the target part and a second surface 22n that faces the clearance surface 21d (see FIG. 4) of the scissors member 21 when the scissors member 21, which functions as a cutter, moves in the cutting direction F1 (closing direction). In this embodiment, the second surface 22n is formed in a plane parallel to the cutting direction F1. The scissors members 21 and 22 are formed to have the same length from the through holes 21t and 22t, into which the first support shafts S1 of the scissors members 21 and 22 are inserted, to the through holes 21u and 22u, into which second support shafts, described below, are inserted.
[0025] 1A and 1B, the link mechanism 30 includes a pair of links 31, 32 of the same length, each connected to a base end 21z, 22z of the scissors mechanism 20 by a pair of second support shafts S21, S22. One link 31 and the scissors member 21 are connected by the second support shaft S21 integrally formed with one end 31e of the link 31 being rotatably inserted and fixed in a through-hole 21u of the scissors member 21 (see FIG. 2A). The other link 32 and the scissors member 22 are connected by the second support shaft S22 integrally formed with one end 32e of the link 32 being rotatably inserted and fixed in a through-hole 22u of the scissors member 22 (see FIG. 2B).
[0026] A third support shaft S3 is formed integrally with the link 31 at the other end 31o of the link 31. A through-hole 32t is formed at the other end 32o of the link 32, into which the third support shaft S3 is inserted in order to connect a wire (not shown) together when the other ends 31o, 32o are overlapped. Furthermore, thin-walled portions 31m, 32m are formed at the other ends 31o, 32o of the links 31, 32 so that the thickness of the overlapped links 31, 32 does not change even when a connecting portion for connecting a wire is disposed therebetween. The third support shaft S3 is formed to a length that protrudes from the link 32.
[0027] 3, the electrode unit 40 has the same thickness as the thickness T of the scissors member 22, and is a monopolar electrode formed by a rod-shaped portion protruding from the thickness surface 22s of the tip 22x of the scissors member 22. As shown in FIG. 5(B), the electrode unit 40 is formed at a position where a center line L1 of the scissors mechanism unit 20 passing through the first support shaft S1 and a center line L2 along the protruding direction of the electrode unit 40 coincide with each other when the scissors members 21, 22 (scissors mechanism unit 20) are closed.
[0028] 3, the electrode portion 40 includes a square-bar-shaped base portion 41 and a semicircular tip portion 42 extending from the tip of the base portion 41. The tip portion 42 of the electrode portion 40 is formed in a semicircular shape while maintaining its thickness. Therefore, the electrode portion 40 is formed such that the tip portion 42 maintains its thickness while shortening in the width direction Fw, which is perpendicular to the thickness direction Ft.
[0029] The electrode unit 40 has a discharge surface 40s formed by a portion of the thickness surface 41s of the base 41 on the tip end 42 side and a thickness surface 42s of the tip end 42 exposed from the insulating coating. Therefore, the surface of the endoscopic scissors 10 other than the discharge surface 40s of the electrode unit 40 is formed as an insulating surface 20s covered with the insulating coating.
[0030] The use of the endoscopic scissors according to the embodiment of the present invention configured as described above will now be described with reference to the drawings. As shown in FIG. 5A, the target area C is positioned between the scissors members 21, 22 of the scissors mechanism unit 20 in the open position. The operator pulls the wire (not shown) connected to the support shaft S3 in the rearward direction F21. As the wire is pulled, the other ends 31o, 32o of the links 31, 32 connected to the third support shaft S3 move rearward (in the rearward direction F21). As the other ends 31o, 32o of the links 31, 32 move rearward, the one ends 31e, 32e are pulled rearward.
[0031] As a result, the operating sides 21q, 22q of the scissors members 21, 22, which are connected to the ends 31e, 32e of the links 31, 32 by the second support shafts S21, S22, move in the closing direction with the first support shaft S1 as a fulcrum. As the operating sides 21q, 22q move in the closing direction, the cutting sides 21p, 22p of the scissors members 21, 22 also move in the closing direction.
[0032] At this time, as shown in FIG. 4 , the blade portion 21b of the scissors member 21 and the first surface 22m (opposing surface) of the scissors member 22 come into contact with the target area located within the area surrounded by the scissors members 21 and 22. The target area may begin to cut when it comes into contact with the scissors member 21 having the blade portion 21b. The target area may also begin to cut when it comes into contact with the right-angle shoulder 22o formed by the first surface 22m and the second surface 22n of the scissors member 22. However, in many cases, the narrow spacing between the tips of the cutting sides 21p and 22p of the scissors members 21 and 22 causes the target area, such as viscoelastic flesh or fibrotic, hard flesh, to slide between the blade portion 21b and the first surface 22m and slip away from the scissors members 21 and 22. The narrow spacing between the base ends of the cutting sides 21p and 22p may also cause the target area to slip away.
[0033] 5(A), the blade portion 21b of the scissors member 21 and the first surface 22m, which is the receiving surface of the scissors member 22, are formed in a concave V-shape. Therefore, when the target area C is pinched at the tip side of the cutting sides 21p, 22p of the scissors members 21, 22, the target area C can be cut at the tip side, and even if the target area C shifts and escapes at the tip side, the target area C can be moved toward the center of the blade portion 21b and the first surface 22m as the scissors members 21, 22 close.
[0034] Furthermore, when the target area C is clamped at the base end of the cutting sides 21p, 22p of the scissors members 21, 22, the target area C can be cut at the base end, and even if the target area C slides away at the base end, the target area C can be moved toward the center of the blade portion 21b and the first surface 22m as the scissors members 21, 22 close.
[0035] 5(B), when the wire is further pulled, the scissors members 21, 22 overlap each other and are closed, so that the target area C is cut from the overlapping positions on both the distal and proximal sides of the scissors members 21, 22. Therefore, because the scissors members 21, 22 can cut from both the distal and proximal sides of the cutting sides 21p, 22p, the target area C can be cut without missing even elastic flesh that is slippery due to mucous membranes or the like.
[0036] Furthermore, because the blade portion 21b is formed up to the tip 21x (cutting edge) of the scissors member 21 and is sharp, even if the meat is located at the tip 21x, the tip 21x can bite into the meat that it comes into contact with and press the meat against the first surface 22m of the scissors member 22. Therefore, the meat can be positioned between the scissors members 21 and 22 and cut without letting it escape.
[0037] In this way, the scissors member 21 functions as a cutter by forming the blade portion 21b with the rake face 21c and the flank face 21d, and the scissors member 22 functions as a die having the first face 22m and the second face 22n. For example, scissors with cutters formed on both scissors members have good cutting ability and bite into the target area, but the thickness of the cutting edge (blade width) is thin and linear, making it difficult to ensure high strength.
[0038] However, in the endoscopic scissors 10 according to this embodiment, the scissors members of the scissors mechanism 20 firmly receive and hold the flesh of the target area on the first surface 22m of the scissors member 22 shown in Fig. 4, while the blade portions 21b of the scissors member 21 pinch and cut the flesh, then pass by the second surface 22n of the scissors member 22. Therefore, the thickness of the cutting edge (blade width) is thick and planar, so that the strength is high and the blade portions 21b of the scissors member 21 will not chip or crack.
[0039] Therefore, the endoscopic scissors 10 according to this embodiment can be made sharp and resistant to chipping or cracking of the blades. Furthermore, if both scissors members were cutters, sharp blades would have to be formed on both, which would require time for processing. However, in the endoscopic scissors 10 according to this embodiment, the scissors members 22 are formed as dies, so the processing time can be shorter than if they were cutters.
[0040] When opening the scissors members 21 and 22 again, as shown in Figure 5 (B), the wire (not shown) is pushed in the advancing direction F22, and the tip portions 21x and 22x (cutting sides 21p and 22p) of the scissors members 21 and 22 shown in Figure 5 (A) open by performing the opposite operation to that described above.
[0041] When bleeding occurs due to an incision in the target tissue, it becomes necessary to stop the bleeding. With the scissors mechanism 20 shown in FIG. 5B in a closed position, the operator points the electrode unit 40 toward the area where the bleeding is to be stopped. Then, by operating a control device (not shown), a high-frequency current adjusted for hemostasis is output to the electrode unit 40. The high-frequency current flows as an arc discharge from the discharge surface 40s (see FIG. 3) of the electrode unit 40, which is not covered with an insulating coating, to the tissue at the treatment area, thereby coagulating the target area and stopping the bleeding.
[0042] Furthermore, when cutting tissue, the operator operates the control device to output a high-frequency current adjusted for cutting to the electrode unit 40. As in the case of hemostasis, the high-frequency current flows as an arc discharge from the discharge surface 40s of the electrode unit 40 to the tissue at the treatment site, thereby burning off and separating the target area.
[0043] In this way, the operator can use the scissors mechanism 20 of the endoscopic scissors 10 to quickly incise a wide area of the target site, or use the monopolar electrode of the electrode unit 40 to stop bleeding or incise the target site. Furthermore, even if the target site has become fibrotic tissue and cannot be incised by applying arc discharge from the electrode unit 40, the scissors mechanism 20 can cut the fibrotic tissue without replacing the endoscopic scissors 10. Therefore, because various treatments can be performed without replacing the endoscopic scissors 10, the endoscopic scissors 10 can reduce the effort required during treatment and shorten the treatment time.
[0044] At this time, even if the insertion section of the endoscope is rotated about its axis, causing the endoscopic scissors 10 to rotate about its axis, the electrode section 40 is formed at a position (thickness surface 22s of tip 22x) where the center line L1 of the scissors mechanism section 20 and the center line L2 of the electrode section 40 coincide when the pair of scissors members 21, 22 are closed, as shown in Fig. 5(B). Therefore, it is possible to prevent the direction of the electrode section 40 toward the target area from shifting, allowing treatment to be performed in a stable state.
[0045] 3, the electrode portion 40 has the same thickness as the thickness T of the scissors member 22 and protrudes from the thickness surface 22s of the tip 22x of the scissors member 22. This allows the scissors member 22 formed by the die and the electrode portion 40 to be punched out of a single metal plate, and the scissors member 22 and the electrode portion 40 can be cut out integrally. This makes it easy to manufacture the scissors member 22 and the electrode portion 40.
[0046] The electrode unit 40 is formed so that the tip portion 42 maintains its thickness while shortening its width direction Fw, which is perpendicular to the thickness direction Ft. This makes it easier to generate an arc discharge from the discharge surface 40s (thickness surface 42s) of the tip portion 42 toward the target area, rather than from other locations on the electrode unit 40, and allows treatment to be performed on the target area.
[0047] As shown in Figure 1(A), when the tips 21x, 22x of the scissors members 21, 22 in the scissors mechanism unit 20 are facing up and the scissors members 21, 22 are opened to the left and right, and then, as shown in Figure 1(B), when the scissors members are closed and the scissors members 21, 22 are stacked on top of each other and viewed from the front, the tip 22x of the scissors member 22 extends to the right from the base of the electrode unit 40, and the tip 21x of the scissors member 21 extends to the left from the base of the electrode unit 40, so that the tips 21x, 22x extend in both left and right directions with the electrode unit 40 as the center.
[0048] Therefore, when the electrode unit 40 is pushed into the target area to cut it, the tip portions 21x, 22x of the scissors members 21, 22 abut against the periphery of the target area, functioning as a stopper. In this way, the tip portion 22x of the scissors member 22 extends beyond the position of the electrode unit 40, and the tip portion 21x of the scissors member 21 extends beyond the position of the electrode unit 40, so that the tip portions 21x, 22x extend in both directions around the electrode unit 40, thereby preventing the electrode unit 40 from penetrating too far. Since the electrode unit 40 is prevented from penetrating too deep into the target area, perforation is less likely to occur, thereby improving safety.
[0049] Furthermore, by configuring the scissors mechanism 20 so that the scissors member 21 is a cutter and the scissors member 22 is a die, when the scissors member 21 and the scissors member 22 overlap, the target tissue can be cut off by cutting rather than shearing, resulting in a sharp cutting edge.
[0050] In this embodiment, the electrode portion 40 has a semicircular tip portion 42 on a square rod-shaped base portion 41 having a thickness T of the scissors member 22, but the electrode portion can also have a conical portion, a spherical portion, a triangular pyramid portion, a disk portion or a polygonal prism-shaped block portion at the tip of the rod-shaped portion.
[0051] For example, the electrode unit 40A shown in Figures 6(A) and 6(B) has a cylindrical rod portion 43a and a conical block portion 43b. The electrode unit 40B shown in Figures 6(C) and 6(D) has a cylindrical rod portion 44a and a hemispherical block portion 44b. The electrode unit 40C shown in Figures 6(E) and 6(F) has a rectangular rod portion 45a and a square plate-like block portion 45b. The electrode unit 40D shown in Figures 6(G) and 6(H) has a rod portion 46a with an elliptical cross section and a spherical block portion 46b.
[0052] As shown in FIG. 7 , the electrode unit 40 can have an octagonal prism-shaped block portion 40X at the tip of the rod-shaped portion 40Y. High-frequency current is more concentrated at a pointed tip, facilitating arc discharge. For example, a square prism-shaped (rectangular or cubic) block portion has four corners on its top surface, while an octagonal prism-shaped block portion 40X has eight corners. Therefore, the eight corners of an octagonal prism-shaped block portion can be positioned closest to the tissue, allowing arc discharge to be emitted from any of the eight corners. Therefore, an octagonal prism shape is easier to aim than a square prism, making it easier to direct high-frequency current to the desired site. However, if a polygonal shape has more corners than an octagonal prism, the interior angles will be greater than the 135° of an octagon, making the corners blunter and making arc discharge more difficult. Therefore, it is desirable for the block portion 40X at the tip of the electrode unit 40 to be octagonal prism-shaped.
[0053] In this way, the block portions 43b to 46b shown in Figures 6(A) to 6(H) and the block portion 40X shown in Figure 7 are formed so as to protrude from the upper end surfaces of the rod-shaped portions 43a to 46a, 40Y around the axis line, so that the peripheral edges of the protruding block portions 43b to 46b, 40X can be hooked onto the target area while cutting.
[0054] The scissors mechanism unit 20 of this embodiment is configured by a combination of a scissors member 21 that functions as a cutter and a scissors member 22 that functions as a die, but the scissors member may be combined with a cutter as shown in Figure 7, or may be combined with a die as shown in the figure.
[0055] The present invention is suitable for endoscopic scissors that are inserted from the tip of an insertion portion of an endoscope for treatment.
