Abstract
Description
Collection kit and collection method
[0001] The present invention relates to a collection kit and collection method for collecting blood products for testing.
[0002] Blood products include red blood cell products, plasma products, platelet products, and whole blood products. Blood products containing the components needed by patients are used for transfusions. Blood products are stored and transported in medical bags. To ensure safety, small samples of blood products may be taken and cultured. Culture tests detect the presence or absence of pathogens by collecting the sample in a culture bottle and placing the bottle in an environment favorable for bacterial growth.
[0003] Among blood products, platelet products are inspected using the following procedure: A small-capacity collection bag is connected to a medical bag containing the platelet product (hereafter referred to as the platelet bag). A portion of the platelet product in the platelet bag is then transferred to the collection bag. The collection bag is then disconnected from the platelet bag.
[0004] The collection bag is then transferred to a clean bench. In the clean bench, the platelet product in the collection bag is injected into a blood culture bottle. Prior to injection, the tube of the collection bag is connected to a tube extending from a sample collection tube. Using the scale on the sample collection tube as a guide, a predetermined amount of the platelet product is transferred from the collection bag to the sample collection tube.
[0005] The operator then operates the valve of the sample collection tube while visually checking the scale on the sample collection tube to transfer a specified amount of platelet product from the sample collection tube to a blood culture bottle. The culture test involves anaerobic and aerobic culture. Therefore, the platelet product is dispensed into a culture bottle for anaerobic culture and another for aerobic culture. The culture bottles are then placed in a culture device and subjected to culture testing for a specified period.
[0006] For example, US Pat. No. 8,777,921 discloses a collection device for collecting samples from medical bags.
[0007] To further improve the safety of platelet preparations, culture testing of all platelet preparations is being considered. Therefore, there is a need to improve the work efficiency of culture testing of platelet preparations. Furthermore, when culture testing is performed on all platelets, it is desirable that the equipment used to separate platelet preparations into culture bottles has a simple structure and is low-cost.
[0008] An object of the present invention is to solve the above-mentioned problems.
[0009] One aspect of the following disclosure is a collection kit comprising: an inflow tube to which a medical bag containing a collection target is connected; a collection bag connected downstream of the inflow tube and having a plurality of storage chambers formed by overlapping and welding two flexible sheets, each of the storage chambers containing a predetermined amount of the collection target; a plurality of outlet ports connected to each of the storage chambers and allowing the collection target to flow out of the storage chamber; and an adapter connected downstream of each of the outlet ports and to which a culture bottle is attached, thereby connecting the culture bottle and the outlet port; the collection bag having a connecting flow path connecting adjacent storage chambers, the connecting flow path being formed integrally with the storage chambers by the two flexible sheets, and having a tubular portion formed between adjacent storage chambers by cutting out the surrounding flexible sheets.
[0010] Another aspect is a collection method using the collection kit of the above aspect, comprising the steps of connecting the medical bag to the inflow tube, introducing the material to be collected into the storage chamber of the collection bag through the inflow tube, separating the medical bag from the inflow tube, blocking the tubular portion to prevent movement of the material to be collected between the multiple storage chambers, and connecting the culture bottle to the adapter and injecting the material to be collected into the culture bottle.
[0011] The collection kit and collection method of the above aspects have a simple configuration, and therefore can be manufactured inexpensively and has excellent work efficiency.
[0012] Fig. 1 is a plan view of a collection kit according to a first embodiment. Fig. 2 is an explanatory diagram of a process for introducing a platelet preparation into the collection kit of Fig. 1. Fig. 3 is an explanatory diagram of a process for introducing the platelet preparation introduced into the first and second storage chambers of Fig. 1 into first and second culture bottles. Fig. 4 is a schematic explanatory diagram of a collection kit according to a second embodiment. Fig. 5A is a perspective view of the collection kit of Fig. 4, and Fig. 5B is a cross-sectional view taken along line VB-VB in Fig. 4.
[0013] (First Embodiment) A collection kit 10 according to this embodiment shown in FIG. 1 is used, for example, in a culture test to confirm the safety of blood products at a business facility such as a blood center that produces blood products. The culture test involves culturing anaerobic bacteria and aerobic bacteria. Therefore, two bottles are used in the culture test: a first culture bottle 90 (see FIG. 3) for anaerobic culture and a second culture bottle 92 (see FIG. 3) for aerobic culture. The collection kit 10 is used to collect a liquid sample (platelet product) and dispense predetermined amounts into the two culture bottles 90, 92.
[0014] Although not particularly limited, this embodiment will describe an example in which a platelet product is used as the blood product. As shown in Fig. 1 , a collection kit 10 includes an inlet tube 12, a collection bag 14, a first sealing member 16, a second sealing member 18, an exhaust valve 20, and first and second adapters 21a and 21b. Furthermore, in the collection kit 10, the direction in which the inlet tube 12 and the exhaust valve 20 are located is referred to as the upper side or upward, and the direction in which the first and second adapters 21a and 21b are located is referred to as the lower side or downward, depending on the arrangement of the collection kit 10 during use. Furthermore, the direction perpendicular to the up-down direction and along the plane of the collection bag 14 is referred to as the width direction, and the direction perpendicular to the plane of the collection bag 14 is referred to as the thickness direction.
[0015] The inflow tube 12 is a translucent medical tube made of a thermoplastic resin such as polyvinyl chloride. The inflow tube 12 can be connected to or disconnected from other medical tubes using, for example, a sterile connection device or a tube sealer without exposing the interior to the outside air. The inflow tube 12 has a first end 12a on the upstream side and a second end 12b on the downstream side. The first end 12a is initially welded and sealed. The second end 12b is connected to the upper end of the collection bag 14.
[0016] The collection bag 14 includes two flexible sheets 22 stacked in the thickness direction. The two flexible sheets 22 are welded and joined to each other at multiple welds 24. The flexible sheet 22 includes a first region 22a for forming one storage chamber (first storage chamber 26), a second region 22b for forming the other storage chamber (second storage chamber 28), and a connecting region 22c connecting the first region 22a and the second region 22b. The first region 22a has a rectangular shape with the same dimensions as the second region 22b in a plan view. The connecting region 22c extends in the width direction and integrally connects the first region 22a and the second region 22b. The flexible sheet 22 includes a pair of rectangular notches 22d above and below the connecting region 22c. The connecting region 22c is formed between the upper and lower cutouts 22d, and is formed so that its vertical dimension is narrower than its width dimension. Although not particularly limited, the connecting region 22c is located at the vertical center of the first region 22a and the second region 22b.
[0017] The collection bag 14 includes a first storage chamber 26, a second storage chamber 28, and a connecting flow path 30, which are defined by the welded portion 24. The first storage chamber 26 is formed in the first region 22a of the flexible sheet 22. The first storage chamber 26 stores the platelet preparation to be introduced into the first culture bottle 90. In the initial state (the state at the time of product provision), the first storage chamber 26 bulges in the thickness direction of the flexible sheet 22. In the first storage chamber 26, one flexible sheet 22 and the other flexible sheet 22 are spaced apart in the thickness direction. The first storage chamber 26 has a capacity for storing a predetermined amount (e.g., approximately 8 ml or approximately 10 ml) of the platelet preparation.
[0018] The collection bag 14 has an inlet port 32 connected to the first storage chamber 26, and a first outlet port 34. The inlet port 32 is connected to the upper end of the first storage chamber 26 of the collection bag 14. The inlet port 32 is connected to the second end 12b of the inlet tube 12, connecting the inlet tube 12 to the first storage chamber 26. The first outlet port 34 (outlet port) is connected to the lower end of the first storage chamber 26. The first outlet port 34 is connected to the first sealing member 16. The first adapter 21a is connected to the first outlet port 34 via the first sealing member 16.
[0019] The second storage chamber 28 is formed in the second region 22b of the flexible sheet 22. The second storage chamber 28 stores the platelet preparation to be introduced into the second culture bottle 92. The second storage chamber 28 has the same shape as the first storage chamber 26 and stores the same volume of platelet preparation.
[0020] The collection bag 14 has a second outlet port 36 connected to the second storage chamber 28 and an exhaust port 38. The second outlet port 36 (outlet port) is connected to the lower end of the second storage chamber 28 of the collection bag 14. The second outlet port 36 is connected to the second sealing member 18. The second adapter 21b is connected to the second outlet port 36 via the second sealing member 18. The exhaust port 38 is connected to the upper end of the second storage chamber 28. The exhaust port 38 is connected to the exhaust valve 20.
[0021] The connecting flow path 30 is a flow path connecting the first storage chamber 26 and the second storage chamber 28. An upstream end 30a of the connecting flow path 30 is connected to the upper part of the first storage chamber 26. The connecting flow path 30 extends obliquely upward from the end 30a, curves at an apex 30b, and extends downward. The connecting flow path 30 extends downward downstream of the apex 30b to the center of the collection bag 14 in the vertical direction and then bends in the width direction at a bent portion 30c. The connecting flow path 30 forms an intermediate portion 30d downstream of the bent portion 30c, extending in the width direction away from the first storage chamber 26. The intermediate portion of the connecting flow path 30 in the flow path direction is located at the center of the width direction of the intermediate portion 30d. The intermediate portion 30d extends along the connecting region 22c of the flexible sheet 22 and extends to the second region 22b. The connecting flow path 30 has a bent portion 30e downstream of the intermediate portion 30d. The connecting flow path 30 bends downward at a bending portion 30e. The connecting flow path 30 downstream of the bending portion 30e turns obliquely upward in a V-shape at a turning portion 30f, and a downstream end portion 30g located downstream of the turning portion 30f connects to the lower portion of the second storage chamber 28.
[0022] In its initial state, the connecting flow path 30 bulges in the thickness direction. The intermediate portion 30d of the connecting flow path 30 forms a tubular portion 31, which is elongated in the width direction at the portion passing through the connecting region 22c. The upper and lower ends of the tubular portion 31 are sealed by narrow welding portions 24, and the intermediate portion 30d of the connecting flow path 30 can be sealed by attaching a clamp 42 through the notch 22d. The tubular portion 31 can be cut while sealing using a tube sealer, which cuts the flow path while heating the tube with high frequency or ultrasound.
[0023] In the initial state, the first sealing member 16 seals the first outflow port 34, preventing fluid from passing between the first storage chamber 26 and the first adapter 21a. The first sealing member 16 is a cylindrical member with a breakable plug inside, and in the initial state, when the plug is bent, the plug breaks, opening the internal flow path. The second sealing member 18 is a member similar to the first sealing member 16, and in the initial state, seals the second outflow port 36, preventing fluid from passing between the second storage chamber 28 and the second adapter 21b.
[0024] The exhaust valve 20 communicates with the second storage chamber 28 and the first storage chamber 26 through an exhaust port 38. The exhaust valve 20 has a filter therein that allows gas to pass through but prevents liquid from passing through. Although not particularly limited, the exhaust valve 20 may have a hydrophilic filter. When the exhaust valve 20 has a hydrophilic filter, after the first storage chamber 26 and the second storage chamber 28 are filled with a platelet preparation, the pores of the filter are blocked by the liquid when the exhaust valve 20 comes into contact with the platelet preparation, and thereafter the exhaust valve 20 prevents gas and liquid from passing through. Such an exhaust valve 20 can prevent air from flowing back into the second storage chamber 28.
[0025] 3, the first adapter 21a accommodates the neck of the first culture bottle 90. The needle tube inside the first adapter 21a penetrates the stopper of the first culture bottle 90, thereby connecting the first culture bottle 90 to the first storage chamber 26 after the first sealing member 16 is opened. The second adapter 21b is a member similar to the first adapter 21a, and connects the second culture bottle 92 to the second storage chamber 28 after the second sealing member 18 is opened.
[0026] The collection kit 10 of this embodiment is configured as described above. This collection kit 10 is used as follows.
[0027] In this embodiment, a collection method will be described in which a platelet preparation is dispensed into two culture bottles 90, 92 using the collection kit 10. First, a platelet bag 94 is attached to the collection kit 10. As shown in FIG. 2, the platelet bag 94 is a medical bag containing a platelet preparation. The platelet bag 94 has a connecting tube 96. The tube 96 is attached to the inlet tube 12. The tube 96 is attached to the inlet tube 12 using a sterile attachment device without coming into contact with the outside air.
[0028] Next, the platelet product is introduced into the collection bag 14. In this step, the collection kit 10 and the platelet bag 94 are positioned such that the platelet bag 94 is above the collection bag 14. The first sealing member 16 and the second sealing member 18 of the collection kit 10 are maintained in a closed state.
[0029] The platelet preparation flows out of the platelet bag 94 by gravity and flows into the collection bag 14 via the inflow tube 12. The platelet preparation that has flowed into the collection bag 14 flows into the first storage chamber 26 via the inflow port 32. The platelet preparation is gradually stored in the first storage chamber 26, and the liquid level of the platelet preparation in the first storage chamber 26 rises. The air inside the first storage chamber 26 is pushed out by the platelet preparation and is discharged from the exhaust valve 20 via the connecting flow path 30 and the second storage chamber 28.
[0030] When the first storage chamber 26 is filled with the platelet preparation, the platelet preparation flows into the upstream end 30a of the connecting flow channel 30. The platelet preparation then flows from upstream to downstream through the connecting flow channel 30 and flows into the second storage chamber 28 from the lower part of the second storage chamber 28. As the inflow of the platelet preparation progresses, the liquid level of the platelet preparation inside the second storage chamber 28 rises. Air in the second storage chamber 28 is exhausted through the exhaust valve 20. The second storage chamber 28 is filled with the platelet preparation. Furthermore, when the exhaust valve 20 comes into contact with the platelet preparation, the exhaust valve 20 closes, preventing the inflow of the platelet preparation. This completes the introduction of the platelet preparation into the collection bag 14. A predetermined amount (e.g., approximately 8 ml) of the platelet preparation is contained in the first storage chamber 26 and the second storage chamber 28.
[0031] The platelet bag 94 is then separated from the inflow tube 12 using a tube sealer (high frequency or ultrasonic sealer) or the like. The tube sealer separates the inflow tube 12 from the tube 96 of the platelet bag 94 and simultaneously seals the first end 12a of the inflow tube 12 by welding. The separation of the inflow tube 12 and the tube 96 is performed without exposing the internal flow path to the outside air.
[0032] The removed platelet bag 94 is stored until the culture test is completed, and then the collection kit 10 filled with the platelet preparation is carried into a clean bench.
[0033] Then, as shown in FIG. 3 , a first culture bottle 90 is connected to the first adapter 21a of the collection kit 10, and a second culture bottle 92 is connected to the second adapter 21b. The tubular portion 31 of the connecting flow path 30 is then closed with a clamp 42. Closing the tubular portion 31 prevents fluid movement between the first storage chamber 26 and the second storage chamber 28. The tubular portion 31 may be closed while being cut with a tube sealer. Alternatively, the tubular portion 31 may be closed by welding two flexible sheets 22 together using a seal (also called an ultrasonic seal or a high-frequency seal) that melts and seals resin by heating using ultrasound or high-frequency (electromagnetic) waves. In this case, the clamp 42 does not need to be installed in the circuit beforehand.
[0034] Thereafter, the first sealing member 16 is opened, the platelet preparation in the first storage chamber 26 is sucked out by the negative pressure of the first culture bottle 90, and a predetermined amount of the platelet preparation is introduced into the first culture bottle 90. In addition, the second sealing member 18 is opened, and the platelet preparation in the second storage chamber 28 is sucked out by the negative pressure of the second culture bottle 92, and a predetermined amount of the platelet preparation is introduced into the second culture bottle 92.
[0035] The first culture bottle 90 is removed from the first adaptor 21a, and the second culture bottle 92 is removed from the second adaptor 21b. Through the above steps, the collection of the platelet preparation using the collection kit 10 is completed.
[0036] As described above, the collection kit 10 of this embodiment has a structure in which the collection bag 14 is integrally formed using two flexible sheets 22, allowing for low-cost manufacturing. Furthermore, the number of times the tubes need to be joined and separated can be minimized, resulting in excellent work efficiency. Furthermore, the need to measure the platelet product by eye by aligning it with the scale line is eliminated, simplifying the work and reducing measurement errors. Furthermore, the collection kit 10 reliably separates the first storage chamber 26 and the second storage chamber 28 by closing the tubular portion 31 of the collection bag 14 with a clamp or the like, preventing the movement of the platelet product between the first storage chamber 26 and the second storage chamber 28. Therefore, the collection kit 10 reduces variation in the amount of platelet product introduced into the first culture bottle 90 and the second culture bottle 92.
[0037] Second Embodiment As shown in Fig. 4, a collection kit 10A of this embodiment differs from the collection kit 10 shown in Fig. 1 in that it further includes a frame member 50 that holds the collection bag 14. In the collection kit 10A of Fig. 4, the inflow tube 12, the collection bag 14, the exhaust valve 20, the first adapter 21a, and the second adapter 21b are the same as those in the collection kit 10, and therefore detailed description thereof will be omitted.
[0038] As shown in Fig. 5A, the collection kit 10A has a frame member 50 that holds the collection bag 14. The frame member 50 has a first frame member 50a and a second frame member 50b. The first frame member 50a is disposed adjacent to one side of the collection bag 14 in the thickness direction. The second frame member 50b is disposed adjacent to the other side of the collection bag 14 in the thickness direction. The frame member 50 has a structure in which the collection bag 14 is sandwiched between the first frame member 50a and the second frame member 50b in the thickness direction.
[0039] As shown in FIGS. 5A and 5B , the first frame member 50a includes a contact plate 52, a frame body 54, and a connecting portion 56. The contact plate 52 includes a rectangular first flat plate portion 52a formed in a portion corresponding to the first region 22a and a second flat plate portion 52b formed in a portion corresponding to the second region 22b. The frame body 54 is formed on three sides of the first flat plate portion 52a, excluding the side toward the center in the width direction. The frame body 54 is also formed on three sides of the second flat plate portion 52b, excluding the side toward the center in the width direction. The frame body 54 bulges in the thickness direction relative to the first flat plate portion 52a and the second flat plate portion 52b. The frame body 54 has grooves in predetermined locations for passing piping connecting to the inlet tube 12, the first sealing member 16, the second sealing member 18, and the exhaust valve 20. The first flat plate portion 52a and the second flat plate portion 52b are connected to each other via a connecting portion 56. The connecting portion 56 is located, for example, at the lower end of the first frame member 50a. The connecting portion 56 is spaced apart from the tubular portion 31 of the collection bag 14 and is positioned so as not to interfere with the attachment of the clamp 42 or the like to the tubular portion 31.
[0040] The first frame member 50a has a recess 58 formed in a portion surrounded by the frame body 54. The recess 58 accommodates a portion of the first storage chamber 26 and the second storage chamber 28 in the thickness direction.
[0041] The second frame member 50b is formed symmetrically to the first frame member 50a in the thickness direction, and has the same abutment plate 52, frame body 54, and connecting portion 56 as the first frame member 50a. The second frame member 50b has a recess 58 in the portion surrounded by the frame body 54, and accommodates the other portion of the first storage chamber 26 and the second storage chamber 28 in the thickness direction.
[0042] The contact plates 52 of the first frame member 50a and the contact plates 52 of the second frame member 50b contact the first storage chamber 26 and the second storage chamber 28 so as to sandwich them in the thickness direction, thereby preventing expansion (deformation) in the thickness direction of the first storage chamber 26 and the second storage chamber 28. The contact plates 52 prevent changes in volume due to expansion in the thickness direction of the first storage chamber 26 and the second storage chamber 28, thereby suppressing variations in the amount of platelet preparation introduced.
[0043] The frame body 54 of the first frame member 50a and the frame body 54 of the second frame member 50b sandwich and hold the peripheral edge of the collection bag 14 in the thickness direction. The frame body 54 improves the handleability of the collection kit 10A by maintaining the planar shape of the collection bag 14. Furthermore, the frame member 50 having the frame body 54 prevents volumetric changes due to deformation of the first storage chamber 26 and the second storage chamber 28, thereby suppressing variations in the amount of platelet product introduced.
[0044] In the collection kit 10A of this embodiment, a clamp structure for closing the tubular portion 31 may be integrally attached to the frame member 50. In this case, the work of attaching a separately prepared clamp 42 to the tubular portion 31 is not necessary, further improving operability and handling.
[0045] The above disclosure can be summarized as follows:
[0046] One aspect is a collection kit (10) comprising: an inflow tube (12) to which a medical bag (94) containing a collection target is connected; a collection bag (14) connected downstream of the inflow tube and having a plurality of storage chambers (26, 28) formed by overlapping and welding two flexible sheets (22), each of which stores a predetermined amount of the collection target; a plurality of outlet ports (34, 36) connected to each of the storage chambers and allowing the collection target to flow out of the storage chamber; and adapters (21 a, 21 b) connected downstream of each of the outlet ports and to which culture bottles (90, 92) are attached, thereby connecting the culture bottles to the outlet ports, wherein the collection bag has a connecting flow path (30) connecting adjacent storage chambers, and the connecting flow path is formed integrally with the storage chambers by the two flexible sheets, and has a tubular portion (31) formed into a tubular shape by cutting out the surrounding flexible sheets between adjacent storage chambers.
[0047] The collection kit described above can be manufactured at low cost because the collection bag is integrally formed using two flexible sheets. Furthermore, the collection kit can reliably separate adjacent storage chambers by closing the tubular portion of the collection bag with a clamp or the like, thereby reducing variations in the amount of platelet product introduced.
[0048] In the collection kit, the plurality of storage chambers may include a first storage chamber (26) connected to an upper portion of the inlet tube and a second storage chamber (28) disposed in the first storage chamber via the connecting flow path, and an exhaust valve (20) for exhausting air from the first storage chamber and the second storage chamber may be connected to an upper portion of the second storage chamber. This collection kit allows the platelet preparation to be smoothly introduced by exhausting the air from the first storage chamber and the second storage chamber through the exhaust valve.
[0049] In the collection kit, the upstream end (30a) of the connecting flow path may be connected to an upper portion of the first storage chamber, and the downstream end (30g) of the connecting flow path may be connected to a lower portion of the second storage chamber. This collection kit allows the platelet preparation to be smoothly introduced into the first storage chamber and the second storage chamber without leaving any air in them.
[0050] In the collection kit, the plurality of outlet ports may include a first outlet port (34) connected to a lower portion of the first storage chamber and a second outlet port (36) connected to a lower portion of the second storage chamber. This collection kit allows the platelet product in the first storage chamber and the platelet product in the second storage chamber to be introduced into the respective culture bottles through different outlet ports, eliminating the need to transfer the platelet product between different storage chambers and providing excellent operability.
[0051] The collection kit may further include a frame member (50) that holds the peripheral edge of the flexible sheet and maintains the flexible sheet in a flat state. This collection kit can maintain the collection bag in a flat shape, eliminating the need to maintain the collection bag in a flat state during use, making it easier to handle.
[0052] In the collection kit, the frame member may include a frame body (54) that holds the peripheral edge of the flexible sheet, and an abutment plate (52) that abuts against the storage chamber in the thickness direction of the flexible sheet to prevent expansion of the storage chamber in the thickness direction. This collection kit can reduce variation in the amount of platelet product introduced by suppressing expansion of the storage chamber in the thickness direction.
[0053] Another aspect is a collection method using the collection kit, comprising the steps of connecting the medical bag to the inflow tube, introducing the material to be collected into the storage chamber of the collection bag through the inflow tube, separating the medical bag from the inflow tube, closing the tubular portion to prevent movement of the material to be collected among the storage chambers, and connecting the culture bottle to the adapter and injecting the material to be collected into the culture bottle. This collection method can improve the efficiency of the culture test of a platelet preparation.
[0054] In the above-described collection method, in the step of blocking the tubular portion to prevent movement of the collection target between the plurality of storage chambers, the tubular portion may be blocked by a clamp, ultrasonic sealing, or high-frequency sealing.
[0055] The present invention is not limited to the above disclosure, and various configurations can be adopted without departing from the gist of the present invention.
