Abstract
Description
[0001] PHARMACEUTICAL FORMULATION COMPRISING A COATED TABLET PRIORITY This application claims priority to United States Provisional Application Number 63 / 532,164 that was filed on 11 August 2023. The entire contents of this provisional application is hereby inorporated by reference. FIELD OF THE INVENTION The invention relates to a pharmaceutical formulation and in particular to a pharmaceutical formulation comprising GDC-0077 (INAVOLISIB) or a pharmaceutically acceptable salt thereof. BACKGROUND OF THE INVENTION GDC-0077 (INAVOLISIB) ((S)-2-((2-((S)-4-(difluoromethyl)-2-oxooxazolidin-3-yl)- 5,6-dihydrobenzo[f]imidazo[1,2-d][1,4]oxazepin-9-yl)amino)propanamide has previously been described in WO2017 / 001645A1 (PCT / EP2016 / 065455): -0077 (INAVOLISIB) WO2018 / 109204A1 (PCT / EP2017 / 083143) relates to a method for preparing GDC- 0077 (INAVOLISIB). GDC-0077 (INAVOLISIB) is a benzoxazepin oxazolidinone compound with selective activity in modulating mutant forms of the PI3Kα (alpha) isoform. Upregulation of the phosphoinositide-3 kinase (PI3K) / Akt signaling pathway is a common feature in most cancers (Yuan and Cantley (2008) Oncogene 27:5497-510). Genetic deviations in the pathway have been detected in many human cancers (Osaka et al (2004) Apoptosis 9:667-76) and act primarily to stimulate cell proliferation, migration, and survival. Activation of the pathway occurs following activating point mutations or amplifications of the PIK3CA gene encoding the p110α (alpha) PI3K isoforms (Hennessy et al (2005) Nat. Rev. Drug Discov.4:988-1004). Genetic deletion or loss of function mutations within the tumor suppressor PTEN, a phosphatase with opposing function to PI3K, also increases PI3K pathway signaling (Zhang and Yu (2010) Clin. Cancer Res.16:4325-30. These aberrations lead to increased downstream signaling through kinases such as Akt and mTOR and increased activity of the PI3K pathway has been proposed as a hallmark of resistance to cancer treatment (Opel et al (2007) Cancer Res.67:735-45; Razis et al (2011) Breast Cancer Res. Treat.128:447-56). Phosphatidylinositol 3-Kinase (PI3K) is a major signaling node for key survival and growth signals for lymphomas and is opposed by the activity of the phosphatase PTEN. The phosphoinositide 3-dependent kinase (PI3K) signaling pathway is the most dysregulated pathway in hormone receptor positive breast cancer (HR+ BC). The PI3K pathway is also dysregulated in aggressive forms of lymphoma (Abubaker (2007) Leukemia 21:2368-2370). Eight percent of DLBCL (diffuse large B-cell lymphoma) cancers have PI3CA (phosphatidylinositol-3 kinase catalytic subunit alpha) missense mutations and 37% are PTEN negative by immunohistochemistry test. Phosphatidylinositol is one of a number of phospholipids found in cell membranes, and which participate in intracellular signal transduction. Cell signaling via 3'-phosphorylated phosphoinositides has been implicated in a variety of cellular processes, e.g., malignant transformation, growth factor signaling, inflammation, and immunity (Rameh et al (1999) J. Biol Chem.274:8347-8350). The enzyme responsible for generating these phosphorylated signaling products, phosphatidylinositol 3-kinase (also referred to as PI 3-kinase or PI3K), was originally identified as an activity associated with viral oncoproteins and growth factor receptor tyrosine kinases that phosphorylate phosphatidylinositol (PI) and its phosphorylated derivatives at the 3'-hydroxyl of the inositol ring (Panayotou et al (1992) Trends Cell Biol 2:358-60). Phosphoinositide 3-kinases (PI3K) are lipid kinases that phosphorylate lipids at the 3- hydroxyl residue of an inositol ring (Whitman et al (1988) Nature, 332:664). The 3- phosphorylated phospholipids (PIP3s) generated by PI3-kinases act as second messengers recruiting kinases with lipid binding domains (including plekstrin homology (PH) regions), such as Akt and PDK1, phosphoinositide-dependent kinase-1 (Vivanco et al (2002) Nature Rev. Cancer 2:489; Phillips et al (1998) Cancer 83:41). The PI3 kinase family comprises at least 15 different enzymes sub-classified by structural homology and are divided into 3 classes based on sequence homology and the product formed by enzyme catalysis. The class I PI3 kinases are composed of 2 subunits: a 110 kd catalytic subunit and an 85 kd regulatory subunit. The regulatory subunits contain SH2 domains and bind to tyrosine residues phosphorylated by growth factor receptors with a tyrosine kinase activity or oncogene products, thereby inducing the PI3K activity of the p110 catalytic subunit which phosphorylates its lipid substrate. Class I PI3 kinases are involved in important signal transduction events downstream of cytokines, integrins, growth factors and immunoreceptors, which suggests that control of this pathway may lead to important therapeutic effects such as modulating cell proliferation and carcinogenesis. Class I PI3Ks can phosphorylate phosphatidylinositol (PI), phosphatidylinositol-4-phosphate, and phosphatidylinositol-4,5- biphosphate (PIP2) to produce phosphatidylinositol-3-phosphate (PIP), phosphatidylinositol-3,4- biphosphate, and phosphatidylinositol-3,4,5-triphosphate, respectively. Class II PI3Ks phosphorylate PI and phosphatidylinositol-4-phosphate. Class III PI3Ks can only phosphorylate PI. A key PI3-kinase isoform in cancer is the Class I PI3-kinase, p110α as indicated by recurrent oncogenic mutations in p110α (Samuels et al (2004) Science 304:554; US 5824492; US 5846824; US 6274327). Other isoforms may be important in cancer and are also implicated in cardiovascular and immune-inflammatory disease (Workman P (2004) Biochem Soc Trans 32:393-396; Patel et al (2004) Proc. Am. Assoc. of Cancer Res. (Abstract LB-247) 95th Annual Meeting, March 27-31, Orlando, Florida, USA; Ahmadi K and Waterfield MD (2004) “Phosphoinositide 3-Kinase: Function and Mechanisms” Encyclopedia of Biological Chemistry (Lennarz W J, Lane M D eds) Elsevier / Academic Press), Oncogenic mutations of p110α (alpha) have been found at a significant frequency in colon, breast, brain, liver, ovarian, gastric, lung, and head and neck solid tumors. About 35-40% of hormone receptor positive (HR+) breast cancer tumors harbor a PIK3CA mutation. PTEN abnormalities are found in glioblastoma, melanoma, prostate, endometrial, ovarian, breast, lung, head and neck, hepatocellular, and thyroid cancers. PI3 kinase (PI3K) is a heterodimer consisting of p85 and p110 subunits (Otsu et al (1991) Cell 65:91-104; Hiles et al (1992) Cell 70:419-29). Four distinct Class I PI3Ks have been identified, designated PI3K α (alpha), β (beta), δ (delta), and γ (gamma), each consisting of a distinct 110 kDha catalytic subunit and a regulatory subunit. Three of the catalytic subunits, i.e., p110 alpha, p110 beta and p110 delta, each interact with the same regulatory subunit, p85; whereas p110 gamma interacts with a distinct regulatory subunit, p101. The patterns of expression of each of these PI3Ks in human cells and tissues are distinct. In each of the PI3K alpha, beta, and delta subtypes, the p85 subunit acts to localize PI3 kinase to the plasma membrane by the interaction of its SH2 domain with phosphorylated tyrosine residues (present in an appropriate sequence context) in target proteins (Rameh et al (1995) Cell, 83:821-30; Volinia et al (1992) Oncogene, 7:789-93). The PI3 kinase / Akt / PTEN pathway is an attractive target for cancer drug development since such agents would be expected to inhibit cellular proliferation, to repress signals from stromal cells that provide for survival and chemoresistance of cancer cells, to reverse the repression of apoptosis and surmount intrinsic resistance of cancer cells to cytotoxic agents. PI3K is activated through receptor tyrosine kinase signaling as well as activating mutations in the p110 catalytic subunit of PI3K, loss of the tumor suppressor PTEN, or through rare activating mutations in AKT. GDC-0077 (INAVOLISIB) is a modulator of PI3Kα which is useful for treating cancers, particularly an inhibitor of PI3Kα that is selective for mutant PI3Kα expressing tumors relative to non-mutant PI3Kα expressing cells. GDC-0077 (INAVOLISIB) selectively inhibits the PI3Kα isoform relative to the PI3Kβ, PI3Kδ, and PI3Kγ isoforms, which may be expected to result in an enhanced therapeutic window and less side-effects. GDC-0077 (INAVOLISIB) is light sensitive. A coloured coating prevents transmission of certain wavelengths of light, the wavelengths depending on the colour of the coating, and therefore protects any underlying formulation from such light. This may provide an advantage in that the components of the formulation, particularly GDC-0077 (INAVOLISIB), are protected from such wavelengths of light and any associated degradation that such exposure may cause. In preferred embodiments the colored coating on the present pharmaceutical formulations provides a coating that has low light transmissibility. In particular, low transmissibility to one or more of visible or ultraviolet light. Example 7. Stability Data GDC-0077 (INAVOLISIB) is sensitive to light, and a solid dosage pharmaceutical formulation comprising GDC-0077 (INAVOLISIB) is also sensitive to light. In this study, a solid dosage pharmaceutical formulation comprising GDC-0077 (INAVOLISIB) with different colored film coatings were evaluated to compare the effects of film coating colors on light protection and guide the selection the appropriate film coating for formulation. The formulation used is shown in Table 3. The experimental conditions are as follows. A. ICH light exposure A Suntest CPS+ light chamber was used with the following settings: Power: 765 W / m2; Temperature: 35°C; Time: 7hours (1x ICH, International Conference for Harmonization standard), 14 hours (2x ICH), 21 hours (3x ICH). Per ICH Q1B, 1x ICH light exposure is an overall illumination of not less than 1.2 million lux hours and an integrated near ultraviolet energy of not less than 200 watt hours / square meter. 1x ICH (8 hours of exposure was actually used in this experiment) For each of the three colored film coatings,10 tablets were placed in separate glass scintillation vials respectively. The film-coated tablets were arranged in a single layer at the bottom of each scintillation vial.10 uncoated tablets were also placed in a separate scintillation vial as a positive control. All scintillation vials were placed upright inside the light chamber. In addition, 10 tablets for each of the three colored film coatings were placed in separate white HDPE containers with the lids on and containers upright. 2x ICH and 3x ICH 16 brown film-coated tablets were placed in two scintillation vials and exposed up to 3x ICH light conditions. The tablets were arranged in a single layer at the bottom of the scintillation vials.16 uncoated tablets were also placed in separate scintillation vials as a positive control. All four scintillation vials were placed upright inside the light chamber. B. Direct sunlight exposure (during daytime) For each of the three colored film coatings, 30 coated tablets were placed in two glass scintillation vials laid on their side to create a single layer of tablets for 24 hours.10 uncoated tablets were also placed in a separate scintillation vial to be used as a positive control. All scintillation vials were placed on their side on a windowsill where they were directly exposed to sunlight during the day, with the caps pointed away from the window. The windows of the labs have VEP 35 EnerLogic film on them. In addition, 24 brown film-coated tablets were placed in three glass scintillation vials laid on their side to create a single layer of tablets up to 7 days.16 uncoated tablets were also placed in two separate scintillation vials to be used as a positive control. All scintillation vials were placed on their side on a windowsill where they were directly exposed to sunlight during the day, with the caps pointed away from the window. C. Indoor light For each of the three colored film coatings, 30 coated tablets were placed in two glass scintillation vials laid on their side to create a single layer of tablets for 48 hours.10 uncoated tablets were also placed in a separate scintillation vial to be used as a positive control. All scintillation vials were placed on their side on a bin in a fume hood, with the light turned on. D. Negative control A negative control was used for each condition by duplicate preps of unexposed coated tablets for each color of coating. No negative control was used for the uncoated tablets. Analytical Method A validated HPLC method was used for the photostability study. The method conditions are described in Table 5 below. Table 5. HPLC method The degradation products data by HPLC analysis for the tablets with three different coatings and uncoated tablets are summarized in Table 6. Table 6. Degradation products in GDC-0077 (INAVOLISIB) coated and uncoated tablets under various light conditions. ND: none detected. The LOQ of the method is 0.05%. Values lower than 0.05% are reported here for comparative purposes. The uncoated tablets showed the largest increase in degradation products under all light conditions. The pink and yellow film-coated tablets showed moderate increase in degradation products after 1x ICH light exposure and direct sunlight exposure. The brown film-coated tablets showed the least increase in degradation under all light conditions. The white HDPE bottles did not appear to provide light protection, as the levels of degradation were the same whether the tablets were exposed to light directly or they were inside the HDPE bottles. The chromatograms overlay for the tablets before and after 1x ICH light exposure is shown in FIG.8. The key for each chromatogram depicted in FIG.8 is as follows: Uncoated tablets (light exposed): 800 Yellow Tablets (light exposed): 810 Pink Tablets (light exposed): 820 Brown Tablets (light exposed): 830 Yellow Tablets (not exposed): 840 Pink Tablets (not exposed): 850 Brown Tablets (not exposed): 860 Blank: 870 The degradation products from HPLC analysis of the brown film-coated tablets and uncoated tablets subject to additional light exposure are summarized in Table 7.The brown film coating continued to provide light protection that resulted in much lower degradation even at 2x and 3x ICH light exposure and up to 7 days of direct sunlight exposure, compared with uncoated tablets. Table 7. Degradation products in GDC-0077 (INAVOLISIB) brown film-coated tablets and uncoated tablets under extended light exposure conditions color coated tablets were unchanged after exposure to all the light conditions tested (ICH, sunlight, indoor light). Visual observations Exterior color The color of the exterior of the three color coated tablets were unchanged after exposure to all the light conditions tested (ICH, sunlight, indoor light). Interior color On examination of the interior colour, the presence of yellow color in the white tablet core is indicative of decomposition or change on exposure to light of the active GDC-0077 (INAVOLISIB) and correlates with lower assay of the active. A white tablet core is indicative of low or no decomposition or change on exposure to light of the active GDC-0077 (INAVOLISIB) and correlates with a relateively higher assay of the active. 1x ICH light exposure After 1xICH light exposure, the tablets with three different color coatings were cut open to inspect any color change inside the tablets. The core of the yellow and pink tablets under the coating turned yellow, whether the tablets were directly exposed or inside white HDPE bottles. Under the same conditions, the core of the brown tablets under the coating remained white after ICH exposure. The pictures of tablets with different color coatings before and after 1x ICH light exposure are shown in FIGS.3-5. 2x and 3x ICH light exposure After 2x and 3x ICH light exposure, the brown film-coated tablets were cut open to inspect any color change inside the tablets. Slight yellow hues were observed under the film- coating, as shown in FIG.6. Direct sunlight exposure After 3 and 7 days of direct sunlight exposure on the windowsill, the brown film- coated tablets were cut open to inspect any color change inside the tablets. Slight yellow hues were observed under the film-coating, as shown in FIG.7. ICH and other light conditions Table 7 from Anderson, N. H., and Stephen J. Byard. "Photostability testing: Design and interpretation of tests on new drug substances and dosage forms.” Photostability of drugs and drug formulations. CRC Press, Boca Raton, Fl, 1996, 137-172 shows the light levels under various light conditions. The ICH light exposure of not less than 1.2 million lux hours and an integrated near ultraviolet energy of not less than 200 watt hours / square meter is similar to two days of light exposure on a sunny windowsill. The data in Table 6 and Table 7 show that degradation levels after 1x ICH light exposure are similar to those after 1-2 days on a sunny windowsill. The photostability data for GDC-0077 (INAVOLISIB) tablets on sunny windowsill are consistent with and supplemental to the data under ICH light exposure. Table 8 Light exposure under various condition Among the three film coatings tested, brown coating provided the best light protection for GDC-0077 (INAVOLISIB) tablets. There is no color change on the exterior or in the tablet core under the film coatings after 1x ICH light exposure. The brown tablets also showed the least increase in degradation products among the tablets coated with three different colors. No degradation product >0.05% was observed in the brown tablets after various light exposure conditions. Together with the intragranular and extragranular excipients provides the desired stability and processability. The yellow and pink film coated tablets maintained their exterior color, but the tablet core under the coating turned yellow after light exposure at 1x ICH conditions. There were also higher levels of degradation products after 1x ICH light exposure, compared with brown film- coated tablets. Hunterlab measurements The tablets and API were evaluated for appearance at each time point. This was performed visually with the Munsell Book of color and by measuring reflected color with the help of a spectrophotometer (UltraScan PRO – Hunterlab) The tablets were evaluated from the exterior but also in the interior (only with the Munsell Book for the interior of the tablets). This allowed tracking of any slight color change that starts to happen but that was still masked by the coating. Principle of Hunterlab system: The UltraScan Pro is a spectrophotometer with mathematical modeling of all the colors perceived by the human being (3-dimensional color space L*a*b*) . The evaluation of color change is performed based on the ΔΕ2000value. ΔΕ2000is the distance between points within the 3-dimensional L*a*b* color space where : L* = lightness / darkness value + = lighter – = darker a* = red / greenaxis + = redder – = greener b* = yellow / blueaxis + = yellower – = bluer This distance correlates to human perception. As used herein and based on normal use of this system, no color change can be concluded if the ΔΕ2000 value ≤ 2. The tablets were also analyzed regarding the content and degradation products. The specification criteria was used to evaluate the samples. Regarding the appearance due to the broad range of colors included in the specifications, the actual appearance of the tablet was required to be properly described and recorded. This allows a better understanding of how the changes are happening. There is a high chance that the formed degradation products associated with the color change do affect the quality of the product. Therefore, it is important to record accurately the difference between internal and external of appearance. In this view, an exterior unchanged appearance might be acceptable even if the interior shows evident color change. The pink and red colored tablets formed in Example 5 were tested and showed a Hunterlab measurement ΔΕ2000≤1 , meaning that no color change was observed. This is indicative of an acceptably low degradation of the exterior appearance.
