[0003] Alzheimer’s disease (AD) is a progressive neurodegenerative disorder clinically characterized by cognitive impairment, behavioral disturbances, psychiatric symptoms, and disability in activities of daily living. These clinical manifestations constitute AD dementia.
[0004] Alzheimer’s disease International estimates that the number of people living with dementia worldwide will increase from the current value of 47 million to 131 million by 2050. Being the most common cause of dementia, AD accounts for 60 to 80% of dementia cases. In the United States, it is estimated that 5.2 million Americans suffer from dementia caused by AD, and that by 2050 the prevalence will double or triple unless an effective treatment is found.
[0005] Thus, there exists an ongoing need for methods to treat AD patients.SUMMARY

[0050] The present disclosure provides methods for treating patients having or suspected of having Alzheimer’s disease, abbreviated herein as “AD”. AD is a dementia that is primarily identified by clinical diagnosis and established by certain markers of the disease.
[0051] AD is a continuum having certain operationally defined stages of disease progression. AD pathology begins prior to the onset of clinical symptoms. For example, amyloid plaques, one marker of AD pathology, form 10-20 years prior to the onset of AD dementia. The currently recognized stages of AD include preclinical, prodromal, mild, moderate, and severe. These stages may be further divided into subcategories based on the severity of symptoms and measures of AD progression.
[0052] Because AD does not occur in discrete stages, those skilled in the art will recognize that the differences between patient groups may not be distinct in a particular clinical setting. Nevertheless, the clinical disease stage can be characterized by measures, and changes in these measures over time, such as amyloid-beta (A|3) accumulation (CSF / PET), synaptic dysfunction (FDG-PET / fMRI), tau-mediated neuronal injury (CSF), brain structure (volumetric MRI), cognition, and clinical function. (Jack CR, et al. Hypothetical model of dynamic biomarkers of the Alzheimer’s pathological cascade. Lancet Neurol., 2010; 9(1): 119-28).
[0053] Current core clinical criteria for all dementia, referred to as the NINCDS-ADRDA criteria (McKhann GM, V. diagnosis of dementia due to Alzheimer’s disease: Recommendations from the National Inst, on Aging-Alzheimer’s Association workgroups on diagnostic guidelines for Alzheimer’s disease. Alzheimer’s & Dementia, 7 (2011) 263-269), are known in the art and can be employed in practicing this invention. They include cognitive or behavioral impairment involving impaired ability to acquire and remember new information, impaired reasoning and handling of complex tasks, impaired visuospatial abilities, impaired language functions (speaking, reading, writing), and changes in personality, behavior, or comportment. AD is currently diagnosed using the core criteria and is typically characterized by symptoms which have a gradual onset over months to years, not sudden over hours or days (insidious onset). There is usually a clear-cut history of worsening of cognition by report or observation in AD subjects.
[0054] Other diagnostic classification systems have evolved as new information on AD has become available. These systems include the International Working Group (IWG) new research criteria for diagnosis of AD (Dubois B et al., Lancet Neurol., 2007; 6(8):734-736), IWG research criteria, (Dubois et al., Lancet Neurol., 2010;9(l 1 ): 1118-27), NIA / AA Criteria (Jack CR et al. Alzheimer’s Dement., 201 l;7(3):257-62), and DSM-5 criteria (American Psychiatric Association, DSM-5, 2013). These classification systems can also be employed in diagnosing AD subjects for treatment according to the methods of this disclosure.
[0055] In some embodiments, AD is mild AD, early AD, prodromal AD, mild AD dementia, mild cognitive impairment due to AD, mid-stage AD, or late-stage AD.Patients
[0056] As used herein, the terms “patient”, “subject”, or “human subject”, includes any human subject who may benefit from therapy with an anti -beta-amyloid antibody. In some embodiments, a patient has or is suspected of having a disease associated with the accumulation and deposition of beta amyloid, such as Alzheimer's disease (AD), Down's syndrome, mild cognitive impairment, cerebral amyloid angiopathy, vascular dementia, and / or multi-infarct dementia.
[0057] In some embodiments, a patient is one for whom diagnosis, prognosis, prevention, or therapy for AD is desired, and includes a human subject in need of treatment. Those in need of treatment include those already with AD, as well as those prone to have AD, or those in which the manifestation of AD is to be prevented. Typical patients will be men or women aged 40 to 90 (e.g., 45 to 90, 50 to 90, 55 to 90, 60 to 90). In some embodiments, a patient is aged > 65 years. In some embodiments, a patient is aged 65 to 74 years. In some embodiments, a patient is aged > 75 years.
[0058] In some embodiments, the disclosure provides a method of treating a patient with AD (including, without limitation, patients with preclinical, prodromal, mild, moderate, or severe AD). In certain instances, the disclosure provides a method of treating a patient with prodromal AD. In some instances, the disclosure provides a method of treating a patient with early AD. In some instances, the disclosure provides a method of treating a patient to reduce clinical decline in AD. In some embodiments, a patient with AD also has Down’s syndrome.
[0059] In some instances, the disclosure provides a method of treating a patient with mild cognitive impairment due to AD. In other instances, the disclosure provides a method of treating a patient with mild AD dementia. [0072] In some embodiments, an anti-beta amyloid antibody used in a method of the present disclosure comprises aducanumab (also known as BIIB037). Aducanumab is an anti-Ap antibody that recognizes aggregated forms of A , including plaques. BIIB037 contains a human kappa light chain. BIIB037 consists of 2 heavy and 2 human kappa light chains connected by interchain disulfide bonds. By “BIIB037” or “aducanumab” is meant an anti-Ap antibody comprising the amino acid sequences set forth in SEQ ID NOs: 10 and 11.
[0073] In vitro characterization studies have established that antibody BIIB037 recognizes a conformational epitope present in Ap aggregates, the accumulation of which is believed to underlie the development and progression of AD.
[0074] In vivo pharmacology studies indicate that a murine IgG2a chimeric version of the antibody (ch 12F6A) with similar properties significantly reduces amyloid plaque burden in the brains of aged Tg2576 mice, a mouse model of AD. The reduction in parenchymal amyloid was not accompanied by a change in vascular amyloid, as has been reported for certain anti-Ap antibodies (Wilcock OM, Colton CA. Immunotherapy, vascular pathology, and microhemorrhages in transgenic mice. CNS & Neurological Disorders Drug Targets, 2009 Mar;8(l):50-64).

[0091] The present disclosure provides, among other things, methods and compositions for treatment of a disease associated with the accumulation and deposition of beta amyloid, such as Alzheimer's disease (AD), Down's syndrome, mild cognitive impairment, cerebral amyloid angiopathy, vascular dementia, and / or multi-infarct dementia. In some embodiments, a method of treatment of AD described herein includes: (a) inhibiting AD, e.g. arresting its development; (b) relieving AD, e.g. causing regression of AD; or (c) prolonging survival as compared to expected survival if not receiving treatment. Aducanumab, a human immunoglobulin gamma 1monoclonal antibody, was granted accelerated approval by the FDA for the treatment of patients with early AD and is administered as an intravenous (IV) infusion. Subcutaneous (SC) administration of an anti-beta amyloid antibody, such as aducanumab, may offer an alternative, less burdensome route of administration, reducing the need for health care visits and dependence on medical professionals.
[0092] In some embodiments, the treatment is therapeutic. In some embodiments, treatment has a disease modifying effect. A disease modifying effect, as used herein, means that the treatment slows or delays the underling pathological or pathophysiological disease processes and / or there is an improvement in clinical signs and symptoms of AD relative to placebo.
[0093] In some embodiments, treatment results in symptomatic improvement. This may consist of enhanced cognition, more autonomy, and / or improvement in neuropsychiatric and behavioral dysfunction, even if for only a limited duration.
[0094] In some embodiments, the disclosure relates to methods for delaying clinical decline or progression of disease, or relief of symptoms. Delaying clinical decline or disease progression directly impacts the patient and care-givers. It delays disability, maintains independence, and allows the patient to live a normal life for a longer period of time. Relief of symptoms to the best degree possible can incrementally improve cognition, function, and behavioral symptoms, as well as mood.
[0095] The present disclosure describes, among other things, a titration regimen (sequential administration of increasing doses of an anti-beta amyloid antibody) to treat AD. In some instances, the AD is mild AD, early AD, prodromal AD, mild AD dementia, or mild cognitive impairment due to AD.
[0096] In some provided methods of treatment of AD, the anti-beta amyloid antibody is administered to a human patient in increasing amounts over a period of time. This procedure of sequentially administering the antibody to the patient is referred to herein as “titration” because it involves administering a standardized pharmaceutical of known concentrations in carefully measured amounts until completion of the procedure.
[0097] One of the advantages of the titration regimen of the invention is that it makes it possible to administer higher doses of the anti-Ap antibody to AD patients without incurring the same extent of ARIA observed with a standard dose regimen. In certain embodiments, the higher dose comprises a dose or doses of the anti-Ap antibody of 700 mg. Without intending to belimited to any particular mechanism, it is believed that titration results in lower initial amyloid removal and slower removal during the overall treatment.
[0098] Titration of the anti-Ap antibody (e.g., BIIB037) is carried out in multiple doses.
[0099] For example, two or more doses of the antibody can be administered to the patient in an amount per dose that is less than the minimum therapeutic amount, followed by a dose of the antibody in an amount per dose that is about equal to the minimum therapeutic amount. This regime can then be followed by two or more doses in an amount per dose that is more than the minimum therapeutic amount, but less than the maximum tolerated amount until there is an acceptable change in AD in the patient. Progress can be monitored by periodic assessment (e.g., MRI). In some embodiments, according to the methods disclosed herein, the human subject is evaluated 18 weeks after a last dose. In some embodiments, the evaluation comprises a drug and / or patient safety assessment. In some embodiments, the assessment comprises a MRI.
[0100] In some embodiments, the method of sequentially administering the anti-beta amyloid (AP) antibody or an AP-binding fragment thereof subcutaneously to the human subject to treat AD comprises administering a dose of a first dosage of about 70 mg, a dose of a second dosage of about 210 mg, a dose of a third dosage of about 420 mg, and a dose of a fourth dosage of about 700 mg, of said anti-beta amyloid (AP) antibody or an AP-binding fragment thereof to said human subject. In some embodiments, the subcutaneously, sequentially administered doses are administered about two weeks apart.

[0199] Measurement of the risk, existence, severity, and progression of Alzheimer’s Disease (AD) can be determined by clinical diagnosis over time; assessment of the global functional level of the patient; evaluation of the daily living capacities or behavioral deficits; volumetric analysis of brain structures; in vivo measurement of pathological deposits of abnormal proteins in brain (e g. PET beta-amyloid imaging), or biochemical variables in body fluids (e.g. tau proteins or Abeta peptides); and by comparison to the natural course / hi story of the disease.
[0200] The following clinical assessments can be employed in determining the stage of AD in the patient: CDR, FCSRT, Neuropsychiatric Inventory-Questionnaire (NPI-Q), and a neuropsychological test battery comprising Rey Auditory Verbal Learning Test (RA VLT)Immediate and Delayed Recall, Wechsler Memory Scale (WMS) Verbal Pair Associate Learning Test Immediate and Delayed Recall, Delis-Kaplan Executive Function System Verbal Fluency Conditions 1 and 2, and the Wechsler Adult Intelligence Scale Fourth Edition Symbol Search and Coding Subsets; and the Cognitive Drug Research computerized test battery.
[0201] A preferred diagnostic regime comprises determining the change from baseline on the Clinical Dementia Rating (CDR) Scale, a neuropsychological test battery, Cognitive Drug Research computerized test battery, the Free and Cued Selective Reminding Test (FCSRT), Mini Mental State Examination (MMSE), Columbia Suicide Severity Rating Scale (C-SSRS), and Neuropsychiatric Inventory-Questionnaire (NPI-Q).
[0202] Biomarkers have emerged as essential for defining AD and for staging of the disease along its spectrum. Biomarker phenotypes can bridge the gap between clinical phenotypes and neuropathology phenotypes, such as amyloid plaques, neurofibrillary tangles, inflammation, and neurodegeneration. Biomarkers of AD include ApoE isotype, CSF A 42, amyloid PET, CSF Tau, and hippocampal volumetric (HCV) MRI.
[0203] Amyloid plaque burden in certain areas of the brain can be measured by 18F-AV-45 PET. 18F-AV-45 is an amyloid ligand developed by Avid Radiopharmaceuticals (Philadelphia, Pennsylvania). It binds to fibrillar A with a high affinity (Kd = 3.1 nM). Results with 18F-AV- 45 PET imaging have shown that patients with AD have selective retention of tracer in cortical areas expected to be high in amyloid deposition, whereas healthy controls have shown rapid washout from these areas, with only minimal cortical tracer retention. A significant difference in mean uptake of 18F-AV-45 has been observed between AD and age-matched control subjects. Test-retest variance of 18F-AV-45 PET imaging is low (less than 5%) in both AD patients and cognitively healthy controls. Visual interpretation of the 18F-AV-45 PET images and mean quantitative estimates of cortical uptake correlate with presence and quantity of amyloid pathology at autopsy as measured by immunohistochemistry and silver stain neuritic plaque score [Clark et al. 2011],
[0204] Radiation dosimetry of 18F-AV-45 is in the range of typical PET ligands. The average human whole body effective dose is estimated to be 0.019 mSv / MBq. A dose of 370 MBq per injection has also been shown to yield good imaging results.
[0205] Patients with AD have characteristic reductions in FDG PET measurements of regional glucose metabolism, which are related to progressive impairment of cognitive function[Landau 2011 ; Mielke 1994], The effect of BIIB037 in halting the progression of glucose metabolic deficit can be periodically assessed using FDG PET measurements. Radiation dosimetry of FDG is in the range of typical PET ligands. The average human whole body effective dose is estimated to be 0.019 mSv / MBq. The standard FDG imaging protocol uses a dose of 185 MBq per injection. In this invention, patients can typically receive up to 185 MBq with each scan.
[0206] Measurement of Api-42 and T-Tau or P-Tau levels in CSF are gaining acceptance as predictive biomarkers of AD. Evidence suggests that Tau aggregation pathology is a very early event in pathogenesis. Duyckaerts (2011) Lancet Neurol. 10, 774-775, and Braak et al., (2013), Acta Neuropath., 126:631-41.
[0207] AD-related biomarkers can also be employed. These include, but are not limited, to pyroglutamate- Ap, Ap40, and Ap42 in blood, and total Tau, phospho-Tau, pyroglutamate-Ap AP40, and Ap42 in CSF.
[0208] Morphometric MRI measures can also aid in the assessment of AD. These include whole brain volume, hippocampal volume, ventricle volume, and cortical gray matter volume. Cerebral blood flow as measured by ASL-MRI and functional connectivity as measured by tf- fMRI can be included in the assessment protocols.
[0209] Use of antibody BIIB037 for the treatment of AD patients according to the invention results in an improvement in one or more of these parameters over baseline measurements or at least prevents or slows the progression of AD from one stage to the next stage.Syringe
[0210] Compositions of the present invention may be fdled into a syringe for administration to a subject. Any syringe known in the art for use in delivering compounds (e.g., pharmaceutical compounds) to a subject may be used.
[0211] In some embodiments, the syringe may comprise a needle and a plunger rod. In such a syringe, the composition may be administered by contacting the subject at the injection site with the needle, depressing the plunger until the complete injection of the syringe contents is complete, and then withdrawing the needle from the subject.
[0212] The composition may be administered by the subject or by a third person (e.g. doctor).
[0213] In some embodiments, the syringe may also comprise a finger flange.
[0214] In some embodiments, the syringe may also comprise a passive add-on needle guard.
[0215] Parameters such as needle gauge, needle length, and syringe barrel size can potentially impact injection time and depth.
[0216] In certain embodiments, the syringe is a 2.25 mb Type 1 borosilicate glass PFS with a ' / 2-in ch 27-gauge special thin wall staked stainless steel needle.
[0217] Although anti-A[3 antibodies and fragments thereof are typically delivered in intravenous formulations in doses that depend on body weight, flat (weight-independent) dosing may be justified for subcutaneous administration. This is because monoclonal antibodies typically distribute into compartments such as plasma and extracellular fluids that are not greatly impacted by body weight. Flat dosing may simplify treatment by eliminating the need for dose calculations and associated dosing errors.
[0218] It may be desirable to keep the volume of the injection constant. Thus, in some embodiments, syringes used may contain a constant volume of a composition but the various injections may contain compositions containing different concentrations of the antibody or fragment thereof.
[0219] In certain embodiments, the syringe is contained in an auto-injector. Any autoinjector known in the art for use in delivering compounds (e.g., pharmaceutical compounds) to a subject may be used.

[0228] The present disclosure demonstrates that subcutaneous (SC) administration of aducanumab may exhibit comparable pharmacokinetics (PK) to intravenously administered aducanumab. Methods:
[0229] An exemplary study design is shown in FIG. 1. In short, an open-label, parallel-arm study compared a single weight-based 10 mg / kg IV dose of aducanumab (Arm 1) with two 700 mg SC doses of aducanumab administered 2 weeks apart (Arm 2). 124 healthy volunteers (Arm 1 n=61, Arm 2 n=63) were randomized and dosed with study treatment. 120 participants (60 participants in Arm 1 and 60 participants in Arm 2) were included in the PK Analysis Set. The planned total study duration for participants was approximately 19 weeks, with participants in Arm 1 having 11 visits and participants in Arm 2 having 15 visits concluded by a 14 week outpatient monitoring period. The participants in Arm 1 received a single, weight-based dose of aducanumab as 10 mg / kg IV on Day 1. The participants in Arm 2 received 2 fixed doses of aducanumab as 700 mg SC in the lower abdomen on Days 1 and 15, respectively. As shown in the exemplary schematic of FIG. 2, two participants in the study withdrew from the study due to relocation, one participant withdrew due to study visit burden / scheduling conflicts, and one participant withdrew for other reasons unrelated to AEs. PK sampling was intensive on Day 1 with sparse collection up to day 99 in both arms to ensure adequate calculation of PK parameters. Key inclusion criteria included age 40-70 years, body mass index (BMI) between 18-30 kg / m2, and participants required to be in good health. For Japanese participants, both biological parents and all 4 grandparents were of Japanese descent. Japanese participants were included to better understand any potential PK differences between Japanese and non-Japanese populations. Key exclusion criteria included: MMSE score <27 at screening; brain MRI (at screening or <1 year of Day -1) evidence of acute or subacute hemorrhage, prior microhemorrhage or prior subarachnoid hemorrhage, >4 microhemorrhages, cortical infarct, >1 lacunar infarct, or any focal area of superficial siderosis; history of or positive test for HIV or HCV; current HBV infection; symptoms of bacterial, fungal, or viral infection; or prior aducanumab exposure. Study endpoints included a Primary endpoint (PK comparability of SCvs TV administration as measured by the geometric mean ratio of AUCinf) and Secondary endpoints (incidence of adverse events with treatment route and characterization of additional PK parameters). The baseline participant demographics of the participants, which were similar across treatment groups, is provided in Table 6 below. The mean age of the participants was 52.6 years and a total of 21 (16.9%) participants were Japanese.Table 6: Baseline participant demographics - full analysis set*Participants who reported multiple categories of race were counted in each reported category.Results’.
[0230] Aducanumab was well tolerated in healthy volunteers. The overall incidence (%[n]) of adverse events (AEs) was 50.8%
[0031] in Arm 1 and 30.2%
[0019] in Arm 2. Headache was the most common AE in both arms (Arm 1 : 32.8%
[0020] vs Arm 2: 7.9%[5]). General disorders and administration site conditions included injection site erythema (Arm 1 : 0%[0], Arm 2: 6.3%[4]), injection site reaction (Arm 1: 0%[0], Arm 2: 4.8%[3]), adverse drug reaction (Arm 1 : 0%[0], Arm 2: 1 ,6%[1]), injection site pruritis (Arm 1 : 0%[0], Arm 2: 1 ,6%[1]), and oedema peripheral (Arm 1 : 1 ,6%[1], Arm 2: 0%[0]). All AEs of injection site erythema, injection site reaction, and injection site pruritis were assessed by the investigator as related to study treatment, mild in severity, and resolved. One serious AE (SAE) was reported in this study. The SAE (enterocolitis)occurred in an Arm 2 participant (onset of 1 day after first dose) and led to study treatment discontinuation. The investigator assessed the SAE as severe in severity, related to treatment, and resolved 8 days after onset. All other AEs reported during the study were assessed by the investigator as mild or moderate in severity and resolved (except for two AEs assessed as recovering). No deaths occurred during the study. The incidence of treatment-emergent (TE) anti-aducanumab antibodies (ADA) was low; 3 participants (4.8%) in Arm 2 tested positive for TE ADA during the study, as shown in FIG. 8. A summary of the incidences of adverse events, safety results, and tolerability results are provided in Table 7 below, and in FIG. 6 and FIG. 7.Table 7: Incidence of adverse events - safety analysis set**AEs are MedDRA v25.0 preferred terms within a system organ class (MedDRA v25.0).
[0231] An arithmetic mean (±SE) serum concentration profile for 10 mg / kg IV aducanumab (Arm 1) and 700 mg SC aducanumab injections (Arm 2) is provided in FIG. 3. As shown in FIG.3, in participants treated with 10 mg / kg IV aducanumab (Arm 1), mean serum aducanumab concentrations reached peak exposure at the end of the 1-hour IV infusion and declined rapidly thereafter, reaching exposure levels comparable to those of participants treated with two 700 mg SC aducanumab injections around Day 8 (Arm 2). A summary of the tested pharmacokineticparameters of the participants in Arm 1 and Arm 2 is provided in FIG. 4. Based on geometric means, Cmax was approximately 5-fold lower in the SC arm (Arm 2) compared to the IV arm (Arm 1). A statistical comparative analysis of the pharmacokinetic results of the participants in Arm 1 and Arm 2 is provided in FIG. 5. A series of exemplary PK scatter plots by route (participants in Arm 1 and Arm 2) and baseline weight are provided in FIGS. 9A-9C. FIG. 9A shows an exemplary scatter plot of AUCinf (by route) versus baseline weight. FIG. 9B shows an exemplary scatter plot of Cmax (by route) versus baseline weight. FIG. 9C shows an exemplary scatter plot of Clearance (by route) versus baseline weight. Aducanumab AUCinf in participants treated with 2 aducanumab 700 mg SC injections (Arm 2) and participants treated with aducanumab 10 mg / kg IV (Arm 1) were comparable, with a geometric LS mean ratio of 93.9% (0.939, 90% CI [0.863, 1.022]).
[0232] Thus, the present example demonstrates that Aducanumab administered as two doses of 700 mg SC demonstrates a high degree of PK comparability to aducanumab administered as a single weight-based 10 mg / kg IV dose and is well tolerated in healthy volunteersEquivalents Exemplary Embodiments
