[0004] Chimeric Antigen Receptor T (CAR-T) cell therapy has achieved breakthrough efficacy against B-cell malignancies, and at present, a number of related products have been approved for marketing. As CAR-T cell therapy is widely used in clinical practice, the toxic and side effects caused by this therapy have received increasing attention. Cytokine release syndrome (CRS) and immune effector cell-associated neurotoxic syndrome (ICANS) are common and life-threatening toxic side effects in CAR-T cell therapy. Up to 93%of patients will develop varying degrees of CRS after receiving CAR-T cell therapy, and up to 10%-30%will develop severe CRS. The incidence rate of ICANS after patients receive currently marketed CAR-T cell products targeting CD19 and BCMA is approximately 10%to 87%. For example, the incidence of ICANS of any grade after treatment with the marketed CAR-T cell products axicabtagene ciloleucel (axi-cel) and brexucabtagene autoleucel (brexu-cel) vary from study to study, but can range from 55-69%, and the incidence of ICANS of grade ≥3 is 21-31%. On the other hand, CRS rates are highest for axi-cel and brexu-cel (90-93%) but are lower for other CAR-T cell products (e.g., around 40-50%for liso-cel) .

[0005] CRS is a reaction resulting from the activation of endogenous or infused T cells or other immune cells in the body caused by immunotherapy, including CAR-T cell therapy. The pathophysiology of CAR-T cell-related CRS involves production of cytokines, such as IFN-γand GM-CSF, by CAR-T cells, which cross-activate myeloid cells to result in production of other cytokines and the clinical manifestations. According to the site of occurrence, CRS can be divided into local CRS and systemic CRS. Local CRS is mainly manifests as an inflammatory reaction in and around the lesion, while the main symptoms of systemic CRS include fever, hypotension, and hypoxemia. According to the clinical manifestations of CRS, it is generally divided into four grades. For CRS of grades 1-2, symptomatic and supportive treatment is generally adopted, and if necessary, specific treatment using IL-6 receptor antagonists, with or without corticosteroids can be used. Grades 3-4 represent severe CRS, and in addition to symptomatic treatment, which may involve inotropic and respiratory support, IL-6 receptor antagonists and steroids are given, with additional measures for refractory cases. In some cases, CRS can progress rapidly and become life-threatening.

[0006] ICANS is specifically defined as a series of clinical manifestations of neurological abnormalities caused by the activation or involvement of endogenous or exogenous T cells and / or other immune effector cells after patients receive immunotherapy including CAR-T cells. Like CRS, CAR-T cell-related ICANS involves the production of inflammatory cytokines by CAR-T cells, resulting in recruitment and activation of myeloid cells –here resulting in inflammation within the central nervous system. The clinical manifestations of ICANS are diverse. Early symptoms often include weakened attention, language impairment, and reduced writing ability. It can further develop into disorientation, abnormal mood, aphasia, drowsiness, confusion, and tremor. A small number of patients may develop severe clinical symptoms, including epileptic seizures, amentia, and increased intracranial pressure. The most severe  clinical manifestation of ICANS is acute cerebral edema, the patient can progress from mild drowsiness to cloudiness of consciousness within a few hours, and further progress can lead to death. Early onset cytokine release syndrome (CRS) is a risk factor for severe ICANS. In clinical treatment, patients are generally divided into four grades according to their symptoms of ICANS. For patients with ICANS scores reaching grades 3 to 4, it is generally recommended to transfer them to the intensive care unit and provide mechanical ventilation support if necessary. To treat ICANS, supportive care and corticosteroid treatment are often used clinically. The preventive use of IL-1 receptor blockers can reduce the incidence and severity of ICANS after receiving CAR-T cell therapy. However, the occurrence of ICANS cannot be completely avoided.

[0007] Improved CAR-T cell therapies are therefore required.SUMMARY OF THE INVENTION

[0008] The present inventors have surprisingly identified an arrangement of intracellular binding domain components that is safer and / or more effective. In particular, the present inventors have identified that interposing a TLR co-stimulatory domain between a second co-stimulatory domain and a signal transduction domain imparts improved safety and / or efficacy in cell therapy.

[0009] These results are particularly surprising because they demonstrate improvements over intracellular domains comprising the same components in alternative arrangement. Further, the results indicate that the improved arrangement of intracellular binding domain components is sufficiently generic to be utilized to impart one or more advantages in various formats, including both treatment of “solid” and “liquid” cancers. In other words, the present inventors have arrived at a general principle for application in cell therapy and, in particular, CAR-T cell therapy.

[0010] Indeed, in certain embodiments, by changing the CAR molecular intracellular domain, the present disclosure provides for reduced secretion of cytokines such as IFN-γ and GM-CSF that contribute to CRS and ICANS.

[0382] Based on the excellent safety and anti-tumor activity of 1928T2z cells in preclinical studies, a phase I clinical trial of autologous 1928T2z T cells for the treatment of refractory / recurrent non-Hodgkin lymphoma was subsequently carried out. This study was a 3+3 dose escalation clinical trial, with the dose being 5×104-1×106 1928T2z T cells / kg.

[0383] From November 2019 to February 2023, this clinical trial included 25 patients, 21 of whom received autologous 1928T2z T cell therapy. Among the 21 patients treated, the median age was 57 years (23-70 years) , and 10 (48%) were female. The participants had received a median of 4 previous treatments for lymphoma, and none of the participants had received previous CAR-T cell therapy.

[0384] As of June 14, 2023, all treated patients had reached the primary treatment follow-up time point (3 months after 1928T2z T cell therapy) . Among the 21 patients, 13 (62%) developed grade 1 to 2 CRS, and no ≥ grade 3 CRS. However, in keeping with the favourable safety profile  indicated by the in vitro studies, no neurotoxic syndromes of any grade (ICANS) were observed. Lymphoma response was observed with 1928T2z T cells at all dose levels, including fully metabolized levels, with 11 patients (52%) achieving complete response at 3 months after injection of the cells. Pharmacokinetic studies demonstrated that 1928T2z T cells were able to effectively expand in patients at all dose levels.

[0385] In this clinical trial, in addition to exhibiting excellent anti-tumor activity, 1928T2z T cells also demonstrated an excellent safety profile, that is, no ICANS of any grade or severe (≥ Level 3) CRS occurred in any of the 21 patients treated with 1928T2z T cells. Patient outcomes are summarized in Table 5 below. This observed safety profile is remarkable, in particular when compared with currently marketed products directed against CD19 and incorporating a CD28 co-stimulatory domain (axicabtagene ciloleucel, brexucabtagene autoleucel) . Indeed, it is worth noting that previous approaches to manage the side effects associated with a CD28 co-stimulatory domain have included replacing it with an alternative co-stimulatory domain, with these products being favoured in certain instances, irrespective of reduced efficacy, due to an improved safety profile. This further evidences the unexpected and significant nature of the present inventors findings as they underpin a general approach for increasing safety of CARs which incorporate a CD28 co-stimulatory domain.

[0479] Single in vitro killing refers to adding tumor target cells to CAR-T cells only once, and then continuously monitoring the killing of tumor target cells by CAR-T cells. In this experiment, 1928T2z T cells, 1928BBz CAR T cells and WT T cells (T-cells) were co-cultured with CD19+K562-CD19 tumour cells as targets, and the effector-to-target ratio was 1: 1 (E: T=1: 1) . Since K562-CD19 cells carry GFP fluorescent protein, the killing of target cells K562-CD19 can be continuously monitored through the IncuCyte Live-Cell Analysis System (Sartorius) . In this experiment, the killing of target cells K562-CD19 by CAR-T cells was monitored by using the IncuCyte for 7 days. Experimental results show that compared with T cells, both 1928T2z CAR T cells and 1928BBz CAR T cells had sustained killing activity against K562-CD19, and the  killing activities of the two were equivalent (Figure 8) .
