Circular RNA-based Therapeutics for Methylmalonic AcidemiaTECHNICAL FIELD

[0001] The present invention relates to the field of the circular RNA and the use thereof for the treatment of Methylmalonic Acidemia.BACKGROUND OF THE INVENTION

[0002] Methylmalonic Acidemia (MMA) is an autosomal recessive metabolic disorder with significant mortality, which were 0.79, 1.12, 1.22 and 6.04 per 100,000 newborns in Asia-Pacific, Europe, North America, the Middle East and North Africa regions as illuminated by meta-analysis. The disease is mainly due to methylmalonyl-CoA mutase (MUT) defect or metabolism abnormalities of its coenzyme cobalamin, resulting in accumulation of methylmalonate and methylcitrate, leading to multi-organs dysfunction in nerves, liver, kidneys, and bone marrow. Specifically, methylmalonyl-CoA mutase is the enzyme most frequently mutated in MMA, it catalyses the conversion of methylmalonyl-CoA to succinyl-CoA in mitochondria, which is divided into complete enzyme defect called mut0 subtype and enzyme partial defect called mut-subtype. Cobalamin metabolism is associate with several genes such as MMAA (cblA) , MMAB (cblB type) , MMACHC (cblC type) , MMADHC (cblD type) , LMBD1 (cblF type) , in which, cblC type is most common in Chinese patients, and c. 609G>A is the highest frequency pathogenic mutation as reported in previous literature (He R, et al., Orphanet J Rare Dis. 15: 200 (2020) ) .

[0003] Despite the high unmet need for methylmalonic acidemia, however, there is no approved therapy for it. Current treatments include dietary restriction, cofactor supplement, liver and / or kidney transplant, which is the only effective treatment even in infants, but transplant is not curative and is associated with mortality.

[0005] The invention relates to a novel circular RNA encoding MUT or CblC and the use thereof for treating methylmalonic acidemia (MMA) .

[0067] Production of circRNA Encoding human MUT, human CblC and mouse CblC by Group1 intron self-splicing reaction

[0068] The codon optimized nucleotide sequences of human Methylmalonyl-CoA mutase (SEQ ID NO: 1-5) , human Cyanocobalamin reductase (SEQ ID NO: 6-10) and mouse Cyanocobalamin reductase (SEQ ID NOs: 11-15) were synthesized by Beijing Ruibo Biotech Co., Ltd. The CVB3 IRES sequence was provided as SEQ ID NO: 19 and the Anabaena Group 1 intron permuted-intron-exon sequence were provided as SEQ ID NO: 20 and SEQ ID NO: 21. In the case of circRNA produced by Group 1 intron self-splicing reaction, the linear RNA undergoes two sequential trans-esterification reactions to obtain covalently 5′ to 3′ linked circular RNA with the help of Mg2+ and exogenous guanosine or guanosine nucleotide.

[0069] Example 1A: RNA transcription and circularization.

[0070] Briefly, the linearized plasmid DNA template was obtained by digesting with Pme1 restriction endonuclease (New England Biolabs) , and column purified using DNA Clean &Concentrator-25 Kit (ZYMO RESEARCH) . The precursor RNA was synthesized by in vitro transcription using a T7 High Yield RNA Synthesis Kit (New England Biolabs) . In a traditional IVT reaction system, reaction components were assembled with 400 ng linearized DNA template, 100 U T7 RNA Polymerase, 10 mM NTP solution mix, 10×reaction buffer, 10 U RNase Inhibitor and 0.008 U pyrophosphatase, then program the thermocycler for the IVT reaction by incubation at 37℃ for 2 hours. After in vitro transcription, the IVT reaction was treated with 10 U DNase I (New England Biolabs) , and incubation at 37℃ for 30 minutes to remove the linearized DNA template. After DNase I treatment, additional GTP nucleotide was added to the IVT reaction system, and the reactions were heated at 55℃for 15 minutes to get as many transesterification reactions as possible, RNA was then purified by RNA Cleanup Kit (New England Biolabs) .

[0071] Example 1B: CircRNA purification by HPLC.

[0072] To remove the small, spliced intron fragments accompanied by group 1 intron splicing reaction, high-performance liquid chromatography-based purification was analyzed. The stock RNA solution was diluted to 500 ng / μL first, then run through a 4.6 × 300 mm size exclusion column with particle size of 5 μm and pore size of  (Sepax Technologies) on an Agilent 1100 Series HPLC (Agilent) . In detail, the mobile phase was RNase-free TE buffer (10 mM Tris, 1 mM EDTA, pH: 6) , the flow rate was 1.0 mL / minute, the separated circRNA fractions were collected and column purified using RNA Cleanup Kit (ZYMO RESEARCH) . To reduce the immunity induced by triphosphates at the 5′ends of the nicked or the remaining precursor RNA, a phosphatase treatment step after HPLC purification was performed as follows, reaction was assembled with 1 μg circRNA, 0.5 U calf intestinal alkaline phosphatase (CIP, New England Biolabs) , and 10×reaction buffer, then incubated at 37℃ for 30 minutes. The final products were column purified using DNA Clean &Concentrator-25 Kit (ZYMO RESEARCH) . The purified circular RNA was analyzed, and the results are shown in Figure 1A, 1B and 1C.

[0074] Production of circRNA Encoding human CblC and human MUT by T4 RNA ligase II (Rnl2)

[0075] The codon optimized nucleotide sequences of human Methylmalonyl-CoA mutase (SEQ ID NOs: . 16-18) were synthesis by Beijing Ruibo Biotech Co., Ltd. The construct sequence of the human CblC was provided as SEQ ID NO: 7. The spliced CBV3, EV71 and HRV-A1 IRES sequences were provided in SEQ ID NOs: 22-29. In the case of circRNA produced by enzyme ligation method, the T4 RNA ligase II catalysis the formation of a phosphodiester bond between adjacent 5′ monophosphate and 3′ hydroxyl of linear RNA.

[0076] Example 2A: RNA transcription and circularization by T4 Rnl2

[0077] Briefly, the linearized plasmid DNA template was obtained by digesting with restriction endonuclease (New England Biolabs) and purified using DNA Clean &Concentrator-25 Kit (ZYMO RESEARCH) . RNA was synthesized by in vitro transcription using a T7 High Yield RNA Synthesis Kit (New England Biolabs) . In a traditional IVT reaction system, reaction components were assembled with 400 ng linearized DNA template, 100 U T7 RNA Polymerase, a NTP solution mix including 2 mM GTP, 8 mM GMP solution, 10 mM ATP, 10 mM CTP, 10 mM UTP, 10×reaction buffer, 10 U RNase Inhibitor and 0.008 U Pyrophosphatase, then program the thermocycler for the IVT reaction by incubation at 37℃ for 2 hours. After in vitro transcription, the reaction was treated with 10 U DNase I (New England Biolabs) , and incubation at 37℃ for 30 minutes to remove the linearized DNA template. After DNase I treatment, the RNA transcripts were purified by RNA Cleanup Kit (New England Biolabs) . Then, the RNA ligation reactions were assembled with 2 U T4 Rnl2 per microgram linear RNA, and the reactions were incubated at 25℃for 8 hours. The RNA transcripts contained circRNA were then purified by  RNA Cleanup Kit (New England Biolabs) .

[0081] Lipid Nanoparticle Formulations of circRNA.

[0082] CircRNA-LNPs were prepared by mixing the ethanolic solution of lipids with an aqueous solution of circRNA at 50 mM citrate buffer (pH 4.0) . Upon mixing, the ionizable lipids become cationic in acidic buffer, and interact with anionic RNA through electrostatic complexation, rapid mixing leading to circRNA being encapsulated in homogeneous LNPs. In detail, the lipid mixture consists of four main components, ionizable lipid SM102, DSPC, cholesterol and PEG-DMG, which were dissolved in ethanol at molar ratios of 50: 10: 38.5: 1.5. The ethanolic and aqueous solutions were mixed at a volume ratio of 3: 1 with a flow rate of 12 mL / min using the microfluidic mixer NanoAssemblr system (Precision NanoSystems, Vancouver, BC) . After mixing, the circRNA-LNPs product were buffer-exchanged into phosphate-buffered saline (pH 7.4) with 4%sucrose and concentrated by ultrafiltration using Millipore Ultra-15 centrifugal filters (100 kDa MWCO) . Finally, the circRNA-LNPs were filtrated through a 0.22 μm filter and stored in presterilized vials at -80℃ until use. For quality control, the encapsulation efficiency, particle size and PDI were determined by a QUANT-ITTM RNA assay (Invitrogen Corporation Carlsbad, CA) and A Zetasizer Nano ZS (Malvern Instruments Ltd, Malvern, Worcestershire, UK) equipment respectively.

[0084] In vitro expression efficacy of circRNA in 293T cell.

[0085] The expression efficiency of various circRNA constructs were evaluated in 293T cells. Cells were seeded in 24-well plates and transfected at 70-80%confluency with 1 μg circRNAs using Lipofectamine MessengerMAX Transfection Reagent (Thermo Fisher Scientific) . After 24 hours, cells were lysed in RIPA buffer (Beyotime) containing proteinase inhibitor cocktail (Biosharp) . Protein expression was assessed by western blotting using antibodies against methylmalonyl-CoA mutase (Origene, TA506883) , MMACHC (Abcam, ab177956) , or MMACHC (Proteintech, 21462-1-AP) . As shown in Figure 3, all circRNA constructs were efficiently expressed, albeit with slight variations. The efficiency of different IRES elements driving human MUT protein translation was also evaluated (Figure 4) . All tested IRES elements facilitated efficient protein translation in 293T cells.

[0088] The mut0 fibroblasts (GM50) which exhibit no detectable enzyme activity and expresses decreased amounts of MUT mRNA were purchased from Coriell Institute, those cells were plated on glass coverslips in 6-well plates and transfected with 2 μg hMUT circRNA-LNP. 24 hours after transfection, 200 nM MitoTracker Red CMXRos (M7512, ThermoFisher Scientific) were added to the culture medium and incubated at 37℃ for 30 minutes to label mitochondria, after that cells were washed with prewarmed 1×PBS three times, then fixed, permeabilized, and incubated with an anti-MUT monoclonal antibody (Origene, TA506873; 1: 300 dilution) overnight at 4℃, followed by an Alexa Fluor 488-conjugated secondary antibody (Beyotime, P0188; 1: 1000 dilution) . Nuclei were counterstained with DAPI. Images were acquired using a Nikon A1RSi+ confocal microscope with a 100× / 1.45 oil objective and NIS-Elements software. As shown in Figure 5A and Figure 5B, upon hMUT circRNA-LNP transfection, the expression of human MUT protein was effectively restored and co-localized with mitochondria correctly in mut0 fibroblasts.

[0090] In vivo expression efficacy of circRNA in mouse hepatic tissue.

[0091] This study was designed to characterize hMUT and hCblC protein expression efficiency in vivo following intravenous administration of circRNA-loaded lipid nanoparticles (LNPs) in C57BL / 6 mice. Two dose levels (0.2 mg / kg and 1 mg / kg) of circRNA-LNPs were administered, as well as a combined administration of hMUT and hCblC circRNAs. 24 hours after injection, mice were euthanized by cervical dislocation and hepatic tissue were collected, then hepatic tissues were homogenized in RIPA buffer (Beyotime) supplied with 100× diluted proteinase inhibitor (Biosharp) using bead-based Homogenizer (Jingxin, JXFSTPRP-L) , the sonication process was programmed with 60 Hz for 1.5 min, the supernatant was collected by centrifuge at 13000 g for 10 min and protein expression level was measured by western blot using anti-Flag tag antibody (Proteintech, 66008-4-Ig) and anti-MMACHC antibody (Abcam, ab177956) respectively. The results revealed a dose-dependent increase in protein expression, correlating with increased circRNA levels (Figures 6 and 8) . Moreover, the expression levels of both proteins were not comprised when the hMUT circRNA and hCblC circRNA administrated simutaniously (Figure 8) , indicating the potential of combo treatment in clinic.

[0092] For the metabolic and pharmacokinetic studies, C57BL / 6 mice were administered a single intravenous bolus of 0.5 mg / kg hMUT or hCblC circRNA LNPs and sacrificed at various time points (1, 2, 6, 16 hours, 1, 2, 3, 5, 7 days; n = 3 mice / time point) , the Hepatic hMUT and hCblC protein levels were quantified by western blot as mentioned above and the results were shown in Figure 7, both human MUT and CblC are expressed shortly after injection and sustained for at least one week in mouse liver.

[0094] hMUT circRNA improved cell viability in MUT-deficient fibroblasts.

[0095] Cells deficient in the MUT gene exhibit metabolic imbalances when cultured in medium enriched with methylmalonyl-CoA precursors such as propionate. To investigate whether hMUT circRNA could ameliorate these imbalances and improve cell viability, we utilized a propionate-enriched culture medium on MUT0 fibroblasts (GM01673) . These cells lack detectable MUT activity and exhibit reduced MUT mRNA expression. Cell viability, proliferation, and cytotoxicity were assessed using a CCK-8 assay after transfecting MUT0 cells with hMUT circRNA and incubating them in a 10 mM propionate-enriched medium for 24 and 48 hours. The CCK-8 assay quantifies viable cells by measuring their metabolic activity. Live cells convert the WST-8 reagent into a formazan dye, and the resulting absorbance at 450 nm correlates directly with cell viability. The results demonstrated that hMUT circRNA significantly improved the viability of MUT0 cells compared to vehicle controls, suggesting a functional replenishment effect. This was likely through the compensation of the missing or diminished MUT enzyme activity (Figure 9) .

[0097] hCblC circRNA reduced Methylmalonic Acid in a skeletal muscle-specific Mmachc knock-in mouse model

[0098] A muscle-specific Mmachc knock-in (cki) mouse model was generated by inserting a "loxP-En2 SA-β-geo-6×SV40 pA-loxP" cassette into intron 1 of the Mmachc gene (NM_025962.3) on chromosome 4 via homologous recombination. Subsequent breeding with a Myf5-Cre strain enabled skeletal muscle-specific expression of Mmachc upon Cre-mediated excision of the floxed Stop cassette, rescuing the neonatal lethality observed in Mmachc null mice. These cki mice lacked hepatic methylmalonyl-CoA mutase activity, exhibited reduced weight, and accumulated methylmalonic acid, mimicking severe methylmalonic acidemia in humans. A single intravenous injection of hCblC circRNA encapsulated in lipid nanoparticles in Mmachc cki mice resulted in a substantial and sustained reduction of plasma methylmalonic acid. At doses of both 0.2 mg / kg and 1 mg / kg, a rapid reduction of 70-76%was observed, sustained for at least 5 days post-injection (Figure 10, n = 4-5 mice) . These results demonstrated the therapeutic potential of hCblC circRNA for treating methylmalonic acidemia.
