MULTI-SPECIFIC MOLECULES AND USES THEREOFTechnical Filed

[0001] The present disclosure relates to a multi-specific molecule, and in particular to a multi-specific antibody and uses thereof. The present disclosure is more specifically directed to a trispecific killer engager (TriKE) targeting both 5T4 and CD16A and crosslinking an IL-15 / IL-15Rα complex. The present disclosure also relates to polypeptides, pharmaceutical compositions, uses and methods in relation to the trispecific killer engager.Background

[0002] Natural killer (NK) cells are defined by the expression of the cell adhesion marker CD56 and lack of the T-cell receptor CD3 (CD56+CD3-) . NK cells can be divided into two functionally distinct subsets, CD56bright and CD56dim, based on the cell surface density of CD56. Comprising approximately 10%of circulating NK cells, CD56brightNK cells are generally thought to be more proliferative, to have a higher capacity for cytokine production after stimulation with IL-12 and IL-18, and to have poor cytotoxic effector activity at rest. CD56dimNK cells, however, are potently cytotoxic without stimulation, mediate antibody dependent cellular cytotoxicity (ADCC) , and produce cytokines after stimulation with target cells.

[0003] CD16 (FcγRIII) binds to the Fc portion of IgG antibodies; one type, CD16A, is a transmembrane protein that co-localizes with CD3ζ and Fc-εRI-γ on NK cells. Upon ligation, it induces a potent series of signals resulting in cytokine production and cytotoxic effector activity via ADCC. The second type, CD16B, is found on neutrophils and is not involved in tumor cell killing. Although the extracellular domains are highly homologous, glycosylphosphatidylinositol linkage differentiates CD16B from CD16A. Most CD56brightNK cells in the peripheral blood express little to no CD16A. In contrast, the majority of CD56dim cells uniformly express high levels of CD16A. Down-regulation of CD16A occurs after mitogen stimulation and co-culture with malignant targets, an effect that is presumably mediated by a metalloprotease. This process may be important for rapid modulation of the surface density of CD16A, and in turn the activation status and effector function of NK cells.

[0004] The ADCC function of NK cells is highly pursued in antibody intervening immunotherapy. Bispecific antibodies with two targeting modules, one for recruiting ADCC receptors CD16A and the other for recognizing antigen, have drawn extensive attention due to the high efficacy for engaging NK cells to kill the target cells both in vitro and in vivo.

[0005] Interleukine-15 (IL-15) is a 15KD cytokine that enhances the proliferation and effector functions of IL-2Rβ / γ-expressing lymphocytes, such as NK cells and T cell subtypes. IL-15 is expressed in many cell types and tissues including monocytes, macrophages, DCs, keratinocytes fibroblasts, muscle cells, and neural cells. It shows 19%sequence homology but high similarity of many biological functions to IL-2. Unlike IL-2 receptor, IL-15 receptor alpha is trans expressed on IL-15 producing cells and forms IL-15α / IL-15 complex to deliver signals to NK, NKT, and memory CD8 T cells expressing IL-15Rβ (IL-2Rβ) and common γchain. This signaling induces the recruitment and activation of JAK1 and JAK3 then escalates to STAT3 and STAT5. The phosphorylated STAT3 and STAT5 form the homodimer which translocate to the nucleus and promote the transcription of target genes which can strongly promote the proliferation and differentiation of NK cells, enhance the proliferation and cytotoxicity of T lymphocytes. Although IL-15 activates the lymphocytes like IL-2, but IL-15 has no obvious effect on Treg cells and does not cause capillary leak syndrome, so IL-15 is a superior choice for tumor immunotherapy compared to IL-2.

[0006] Recombinant human IL-15 (rhIL-15) was evaluated in patients with metastatic melanoma and was shown to induce regression of lung metastases in two patients, but the narrow therapeutic window limited its continued development (NCT01021059) . Since the heterodimer IL-15 / IL-15Rα is a natural form of the IL-15 that is cleaved from cells and can also stimulate the downstream signaling independently, many studies are designed to explore the clinical efficacy of the IL-15 / IL-15Rα sushi complex. ImmunityBio’s nogapendekin alfa inbakicept (N803, IL-15N72D: IL-15RαSu / Fc) is approved by FDA in combination with the Bacillus Calmette–Guérin (BCG) vaccine for BCG-unresponsive non-muscle invasive bladder cancer in 2024. IL-15N72D mutation increases the affinity to bind IL-15Rβγc and enhanced biological activity; IL-15RαSu, N-terminal IL-15Rα fragments, the so-called sushi domain (Su) , can act as the IL-15Rβ chain agonist independent of cell membrane IL-15Rα; IgG1 Fc fusion is used to improve the half-life and the biodistribution. Although the nogapendekin alfa inbakicept achieved proof of concept in bladder cancer treatment by intravesical administration, it is still very challenging to broaden the cancer indications due to the non-targeted exposure and systematic immune activation (NCT03228667) .

[0007] 5T4, also known as trophoblast glycoprotein (TPBG) , is a 72 kDa N-glycosylated transmembrane glycoprotein, containing eight leucine-rich repeats for protein–protein interaction. 5T4 protein is expressed in many solid tumors including colorectal, breast, ovarian, lung, gastric, liver, and renal carcinomas but is minimal in normal adult tissues. High 5T4 expression is associated with advanced disease and worse prognosis in colorectal, ovarian, non-small-cell lung (NSCLC) and gastric cancer. Beyond oncofetal tumor associated antigen, 5T4 is also identified as a cancer stem cell marker. Functionally, 5T4 expression is associated with epithelial to mesenchymal transition (EMT) , C-X-C motif chemokine receptor 4 (CXCR4)  / CXCL12 chemotaxis, and noncanonical Wnt signaling which involved tumor metastasis and survival.

[0008] Multiple agents targeting 5T4 are evaluated in clinical trials. Naptumomab estafenatox, an anti-5T4-Fab staphylococcal-enterotoxin-A conjugate, is studied in phase2 / 3 (NCT00420888) and efficacy is shown in the anti-SEA normal / IL-6 low renal carcinoma population; APV-527, a bispecific antibody both targeting 5T4 and CD137, is in phase 1 exploration in solid tumor including breast cancer; GEN1044, a T cell engager, is terminated at phase1 / 2 due to safety issues inducing by anti-CD3 arm to over activate endogenous T cells. A lot of preclinical studies are on-going to discover the anti-tumoral effect of 5T4 targeting agents, such as antibody drug conjugates (ADC) and bispecific antibodies.

[0009] Currently, several formats of these bispecific antibodies are undergoing basic and clinical research, such as Bispecific Killer Cell Engager (BiKE) , bispecific diabody (BidAb) and tetra-valent, bispecific TandAb. Recently, the more promising format TandAb has been developed by Affimed Therapeutics AG (Heidelberg, Germany) . TandAb is a tetravalent, bispecific, tandem diabody composed of the tail-to-head homodimer of the two tandem scFv connected by three (GGS) 3 linkers. One TandAb, CD16 / CD30 AFM13, has completed phase II clinical trials for patients with relapsed or refractory Hodgkin's lymphoma (clinical trial no. NCT02321592) . However, TandAb likely will encounter more production problems than BidAb since it contains more sub-domains, all of which should be paired elegantly to promise a correct TandAb. Furthermore, due to its tetra valence, two for CD16A and two for CD30, TandAb has risks for non-specifically activating CD16A in the absence of CD30 and inducing side effects. TriKEs, designed to target NK cells and tumor cells are actively evaluated in pre-clinical stage. TriKEs form an immune synapse between the NK cell and the tumor cell, mediate ADCC through the highly potent CD16 activating receptor, further enhances NK cell proliferation and persistence by IL-15 fusion.Summary

[0234] The binding of trispecific antibodies to CD122 / CD132 (IL-2Rβγ or IL-15Rβγ) heterodimeric receptor protein from different species including human, cynomolgus, mouse and rat were characterized by ELISA. The heterodimer protein from human (ACROBiosystems, ILG-H5283) , cynomolgus (ACROBiosystems, ILG-C5283) , mouse (ACROBiosystems, ILG-M5283) and rat (ACROBiosystems, ILG-R5283) were coated on the ELISA plate 4℃ overnight, then washed with PBS containing 0.05%Tween20 (PBST) . Serially diluted trispecific antibody BMP05-02 and the positive control N-803 (MCE, HY-P99661) were added to coated ELISA plate and incubated at 37℃ for 1 hour, and the binding ability of trispecific antibody to the heteromeric receptor was detected with an HRP-conjugated Goat anti-human Fc antibody (Jackson Immuno Research, 109-035-098) . The ELISA results showed the binding of BMP05-02 trispecific antibody and control antibody N-803 to human, cynomolgus, mouse and rat CD122 / CD132 heterodimers. Detailed results were shown in Figure 4.

[0243] IL-2 and IL-15 share the heterodimeric CD122 (IL-2Rβ)  / CD132 (IL-2Rγ) receptor to deliver their signals within target cells. Their specificity of action is conferred by their αreceptor chains, CD25 (IL-2Rα) and CD215 (IL-15Rα) . The binding of IL-15 or IL-15 / IL-15RαSu complex to their heterodimeric or heterotrimeric receptors leads to the activation of a signaling cascade through tyrosine kinases of the Janus family (JAK1 and JAK3) and signal transducer and transcription activator 5 (STAT5) . Thus, IL-15 activity can be measured using hIL2 / IL15 dual effector (hCD122 / hCD132) reporter cell (Cobier, CBP74170) that expresses an inducible produced firefly luciferase after IL-15 receptor activation.

[0244] The day before experiment, hIL2 / IL15 dual effector (hCD122 / hCD132) reporter cells were inoculated onto 96 plates at a density of 5×105 per well, 100 μL / well, and cultured overnight at 37℃ in a 5%CO2 incubator. On the day of the experiment, the antibody to be tested was diluted 3 times in RPMI-1640 medium containing 10%heat-inactivated FBS. Then 11.1 μL diluted antibody was added to each well containing effector cells and cultured further in a 5%CO2 incubator at 37℃ for 6 hours. Recombinant human IL-15 (Sino Biological, GMP-10360-HNAE) was used as assay controls. At the end of incubation, 100 μL of Ultra Luciferase Detection Kit (Cobier) reaction substrate was added to each well, pipette repeatedly to lyse the cells, leave at room temperature for 3-10 min, then transfer the lysate to a 96-well white plate to avoid the generation of air bubbles, and use a microplate reader to detect the fluorescence value. The relative light unit (RLU) value of the test well was shown and putted into GraphPad Prism software for statistical analysis and graphing.

[0248] CD3+ T cells were isolated from PBMCs (Oricells) using human CD3 MicroBeads (Miltenyi Biotec, 130-097-043) following manufacturer's instructions. 1×105  / 100 μL / well T-cells were seeded in 2 96-well plates, and stimulated with 50 μL 3-times serial-diluted antibodies with KBM 581 media. IL-2 was used as positive control. Cells were incubated for 72 hours at 37℃ in a 5%CO2 incubator, then phase contrast images were collected. A96-well plate of T-cells were seeded and stimulated for 96 hours at 37℃ in a 5%CO2 incubator. Afterward, cells were subjected to immunostaining and fluorescent analysis. Cells were collected and stained with CD3-Alexa Flour 700 (Invitrogen, 56-0038-42) , CD4-APC (BioLegend, 300514) , CD8-eFluor 450 (Invitrogen, 48-0088-42) , CD25-R-PE (BioLegend, 356105) and CD69-Alexa Fluor 488 (BioLegend, 310916) under corresponding conditions, after which flowcytometry and fluorescent-pattern analysis were conducted using the AttuneTM NxT flow cytometer (Invitrogen, A24860) and FlowJo analysis software.

[0267] One day before ADCC killing experiment, target cells were digested and well-stained with Celltracker CM-DiI (YEASEN, 40718ES72) , then 5×103 / 100μL target cells were seeded in a 96 plate and incubated overnight at 37℃ in a 5%CO2 incubator. On the day of the experiment, the antibody to be tested was serially diluted 5 times with RPMI-1640 medium. The target cell culture medium was discarded, and 50 μL diluted antibody were added to each well containing target cells. Then 1×105 human PBMC cells were added to each well at 50 μL / well. Finally, diluted   Caspase -3 / 7 Dye for Apoptosis (Sartorius, 4440) were added to each well at 50 μL / well. Set up the following three groups of controls at the same time. (1) Target cell maximum growth group: same amount of target cells and 100 μL test medium without test molecules; and (2) Antibody-independent cytotoxicity group: same amount of target cells and effector NK cells with 50 μL medium without test molecules; and (3) Effector only group: same amount of effector cells and 100 μL test medium without test molecules. Plate was placed in   S3 Live-Cell Analysis Instrument and incubate further for more than 3 days inside a 5%CO2 incubator at 37℃ with a scan every 2 hours.

[0272] Bone marrow cells from CB-17 / SCID-hCD16A mice (Beijing Biocytogen Co., Ltd) were transplanted into the tail vein of irradiated 4-week old CB17 / SCID mice (Beijing Vital River Laboratory Animal Technology Co., Ltd. ) to construct hCD16A knock-in mice. MDA-MB-231 cells were cultured in DMEM medium containing 10%heat-inactivated FBS, and the tumor cells were collected when the required cell number was reached. One month later, 0.2 ml 5×106 MDA-MB-231 human breast cancer cells + 50%Matrigel (Corning, 354234) (PBS : Matrigel at 1: 1 volume ratio) were injected subcutaneously into the right forelimb of each mouse for tumor engraftment. When the average tumor volume reached 68.05±0.74 mm3, the tumor-bearing mice were randomly divided into 5 groups (8 mice per group) according to the tumor volume and body weight, and the tumor volume CV within group is <1 / 3. The day of grouping was defined as PG-D0, and the test antibody was given on the day of random grouping. The doses were 0.8 mg / kg, 2.4 mg / kg and 7.2 mg / kg BMP05-02, respectively. The related parental bispecific antibody BMP02-10 was dosed at 6.4 mg / kg, which was an equal molar to the highest dose of BMP05-02. Antibodies were intraperitoneally administered twice a week in the following 4 weeks. Body weight and tumor volume were measured twice a week. Tumor growth inhibition (TGI) was calculated as follows: TGI%= (1-T / C) x100%, T and C were the average TVs of the treatment group and the control group at the end of the experiment. Data analysis was performed using GraphPad Prism software, and the results were shown as the mean + S. E. M of tumor volume.

[0277] A375 cells were cultured in DMEM medium containing 10%heat-inactivated FBS, and the tumor cells were collected when the required cell number was reached. One month later, 0.2 ml 5×106 A375 human melanoma cancer cells + 50%Matrigel (Corning, 354234) (PBS: Matrigel at 1:1 volume ratio) were injected subcutaneously into the right forelimb of each mouse for tumor engraftment. When the average tumor volume reached 76.03 mm3, 1×107 human PBMC (Donor:XW0801084W) were injected intravenously through the tail. On the next day, the tumor-bearing mice were randomly divided into 6 groups (8 mice per group) according to the tumor volume and body weight, and the tumor volume CV within group is <1 / 3. The day of grouping was defined as PG-D0, and the test antibody was given on the day of random grouping. The doses were 0.03 mg / kg, 0.1 mg / kg, 0.3 mg / kg and 1 mg / kg BMP05-02, respectively. BMP05-02 antibodies were intraperitoneally administered once a week in the following 3 weeks. Besides, 5 mg / kg Keytruda was included as a positive control antibody since its efficacy on A375 model was well studied. Keytruda was intraperitoneally administered twice a week in the following 3 weeks. Body weight and tumor volume were measured twice a week. Tumor growth inhibition (TGI) was calculated as follows: TGI%=(1-T / C) x100%, T and C were the average TVs of the treatment group and the control group at the end of the experiment. Data analysis was performed using GraphPad Prism software, and the results were shown as the mean 士 S.E.M of tumor volume.

[0283] DU 145 cells were cultured in RPMI-1640 medium containing 10%heat-inactivated FBS, and the tumor cells were collected when the required cell number was reached. On day 0, 0.1 ml 8×10^6 DU 145 cells + 0.53×10^6 human PBMCs (Hycells, WEZ4375) (DU 145 : huPBMC at 15: 1 density ratio) or 0.1 ml 8×10^6 DU 145 cells + 0.3×10^6 human PBMCs (Hycells, WEZ4375) (DU 145 : huPBMC at 30: 1 density ratio) were mixed and injected subcutaneously into the right forelimb of 6~8-week-old female NOG mice (Beijing Vital River Laboratory Animal Technology Co., Ltd. ) . The engrafted mice were randomly divided into 2 groups with 3 mice in each group according to the tumor volume and body weight. The day of engrafting was defined as D0, and the test antibody was given on the day 27 (PG-D0) . Mice were treated with either PBS or 3 mg / kg BMP05-02 intraperitoneally twice a week for 1 week. Tumor volumes were recorded twice a week from the day post administration. Data analysis and graphing were performed using GraphPad Prism software.

1.A multi-specific molecule comprising(a) an anti-5T4 antibody comprising two heavy chains and two light chains;(b) an IL-15 polypeptide, covalently linked to C-terminus of one of the two heavy chains of the anti-5T4 antibody;(c) an IL-15Rα Sushi polypeptide, covalently linked to C-terminus of the other of the two heavy chains of the anti-5T4 antibody; and(d) an anti-CD16A VHH polypeptide, covalently linked to C-termini of both the IL-15 polypeptide and the IL-15Rα Sushi polypeptide.2.The multi-specific molecule of claim 1, wherein the anti-5T4 antibody is selected from a group consisting of:(a) an anti-5T4 antibody specifically binding to a human, monkey, chicken or mouse 5T4 mature polypeptide, preferably, having an amino acid sequence shown in SEQ ID NOs: 8, 9, 10, and 11, respectively;(b) an anti-5T4 antibody comprising HCDR1, HCDR2 and HCDR3 of heavy chain, and LCDR1, LCDR2 and LCDR3 of light chain, wherein the HCDR1, HCDR2, HCDR3, LCDR1, LCDR2, and LCDR3 have amino acid sequences as shown in SEQ ID NOs: 12, 13, 14, 15, 16, and 17, respectively, according to Kabat definition;(c) an anti-5T4 antibody comprising HCDR1, HCDR2 and HCDR3 of heavy chain, and LCDR1, LCDR2 and LCDR3 of light chain, wherein the HCDR1, HCDR2, HCDR3, LCDR1, LCDR2, and LCDR3 have amino acid sequences as shown in SEQ ID NOs: 18, 19, 20, 21, GAS, and 23, respectively, according to IMGT definition;(d) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 24 and 25, respectively;(e) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 39 and 40, respectively;(f) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 41 and 42, respectively;(g) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 43 and 44, respectively;(h) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 45 and 46, respectively;(i) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 47 and 48, respectively; and(j) an anti-5T4 antibody comprising a heavy chain variable region and a light chain variable region as shown in SEQ ID NOs: 49 and 50, respectively.3.The multi-specific molecule of claim 2, wherein the anti-5T4 antibody comprises:(a) a human IgG1 heavy chain constant region and a human IgG1 light chain constant region;(b) a first human IgG1 heavy chain constant region having substitutions of T366S+L368A+Y407V, and a second human IgG1 heavy chain constant region having a T366W substitution, according to Eu Numbering, and a human IgG1 light chain constant region;(c) a heavy chain constant region and a light chain constant region having at least 80%, preferably 100%, sequence identity to those as shown in SEQ ID NOs: 33 and 36, respectively; or(d) a first heavy chain constant region as shown in SEQ ID NO: 34, a second heavy chain constant region as shown in SEQ ID NO: 35, and a light chain constant region as shown in SEQ ID NO: 36.4.The multi-specific molecule of any one of claims 1-3, wherein the IL-15 polypeptide has one or more features selected from a group consisting of:(a) is a human IL-15 polypeptide;(b) has an amino acid sequence having at least 80%, preferably 100%, sequence identity to that as shown in SEQ ID NO: 4;(c) is linked to a heavy chain via a linker, preferably a peptide linker, more preferably, a linker having an amino acid sequence as shown in SEQ ID NO: 37 or 38; and(d) is linked to a heavy chain comprising a human IgG1 heavy chain constant region having substitutions of T366S+L368A+Y407V, according to Eu Numbering.5.The multi-specific molecule of any one of claims 1-4, wherein the IL-15Rα sushi polypeptide has one or more features selected from a group consisting of:(a) is a human IL-15Rα sushi polypeptide;(b) has an amino acid sequence having at least 80%, preferably 100%, sequence identity to that as shown in SEQ ID NO: 3;(c) is linked to a heavy chain via a linker, preferably a peptide linker, more preferably, a linker having an amino acid sequence as shown in SEQ ID NO: 37 or 38; and(d) is linked to a heavy chain comprising a human IgG1 heavy chain constant region having a T366W substitution, according to Eu Numbering.6.The multi-specific molecule of any one of claims 1-5, wherein the anti-CD16A VHH polypeptide has one or more features selected from a group consisting of:(a) binds specifically to a monkey, mouse, rat or human CD16A polypeptide, preferably having an amino acid sequence shown in SEQ ID NOs: 58, 59, 60, and 61, respectively;(b) has CDR1, CDR2, and CDR3 having amino acid sequences as shown in SEQ ID NOs: 27, 28, and 29, respectively, according to Kabat definition;(c) has CDR1, CDR2, and CDR3 having amino acid sequences as shown i
