[0001] SYSTEM AND METHOD FOR CONTROLLING A PLURALITY OF COAXIAL ENDOVASCULAR DEVICES

[0003] The present invention relates to a system and method for controlling a plurality of coaxial endovascular devices.
[0004] BACKGROUND OF THE INVENTION
[0005] In a robotics endovascular procedure, the user needs to select which device(s), among different coaxial endovascular devices (e.g. coaxial guidewires, catheter, diverter / stent, sheath etc.), to use and the robot to actuate accordingly.
[0006] Many companies are working on endovascular robots that allow users to control and switch between three or more endovascular devices. As part of the endovascular procedure, the user needs to be able to select which device or combination of devices they want to control. Current solutions usually consist of a controller such as a joystick to manipulate the devices' movement and a set of buttons on a control panel surface next to the joystick for selecting which device to control. While this interaction works, it is slow and cumbersome because the user has to let go of the joystick and look down at the control panel to select which device they want to use. Other solutions have multiple joysticks, each mapped to a single device. While this simplifies the selection, it takes up more space and the user has to switch between joysticks. This solution becomes particularly cumbersome if there are four or more devices to control. Furthermore, there is a safety risk associated with the use of multiple joysticks. Specifically, when switching hands between joysticks, unintended joysticks may be accidentally bumped.
[0007] It is very important that the user interface (UI) of the robot shows some flexibility in its use to allow a greater ease of use, such as an easy selection / de-selection of the devices during procedure which prevents the user to leave his / her eyes from the screen. Physicians have indeed stated that they do not like to pause and look down to select different devices as it disrupts their flow and focus. Furthermore, the user interaction should be optimized in terms of speed and efficiency US 2015 / 351864 Al discloses a surgical robot which enables an operator to judge which medical instrument is selected. This surgical robot has a robot body which selectively operates a plurality of medical instruments, an input unit configured to input control information of the robot body, the input unit being common among the plurality of medical instruments, and a display unit configured to display an image of a surgical site of a patient, the display unit having a selecting situation display function which displays a selecting situation of the plurality of medical instruments.The input unit of the surgical robot may comprise a joystick with an operation button provided to an end portion of the joystick, which may be pressed so as to select a desired medical instrument. In particular, US 2015 / 351862 Al discloses that once a pointer is moved to the position of a desired selection switch of a slave screen by operating the joystick, the operation button may be pressed so as to select a desired medical instrument.
[0008] Hence, the surgical robot disclosed in US 2015 / 351864 Al requires a use of the joystick to move a virtual pointer and to push said operation button to select the desired device, i.e. two steps. A simpler selection for using a desired medical device would be desirable for the user.
[0009] SUMMARY OF THE INVENTION
[0010] It is an object of the present invention to provide a system for controlling a plurality of (coaxial) endovascular devices, which is easy and quick to use and which does not require the user to take their eyes off the screen or their hands off the joystick.
[0011] In a first aspect of the present invention a system for controlling a plurality of elongated endovascular devices, comprising:
[0012] a body to be mounted to a base and arranged to be manually manipulated with respect to the base to provide command signal of movement of at least one of said plurality of elongated endovascular devices;
[0013] a plurality of controls configured to be manually actuated while the body is being manually actuated with the same hand, and
[0014] a controller configured to associate the plurality of controls with a plurality of elongated endovascular devices and to select and / or deselect one or more of the elongated endovascular devices in accordance with the actuation of the controls, for controlling movement of the one or more elongated endovascular devices.
[0015] Optionally, the system further comprises the base.
[0016] While this system is arranged to allow a user to physically manipulate the body in directions associated with the directions of the elongated device in a particularly intuitive manner, it further allows an easy selection of elongated device(s) by using the same hand as the one used to manipulate the body. This system allows therefore an intuitive (and potentially remote) manipulation of the elongated devices with an easy selection of the elongated devices to be moved, while keeping a free hand to do (or not) another action (e.g. manipulating another similar body whose directions are associated with another elongated device, or interacting with an interactive screen, a mouse etc.).
[0017] “Control(s)” can be any user interfacing element allowing a user to actuate using his or her hand(s) or fmger(s) such as without any limitation whatsoever a hardware button, a virtual button (i.e. interactive element provided on a screen or projected onto a surface or in a volume), a mouse, ball, or any other type of user interfacing element.“Direction” means an action, or an indication of an action, pointing to a location, which may be along a straight line (which may then be symbolized by a vector) or an orientation along a circular or curvilinear trajectory.
[0018] “Body” is a volumetric object, protruding from the base, and manipulable by a user. It may be for instance a joystick, a knob, and can take any shape as long as it is usable and manipulable by finger(s) or hand of a user.
[0019] In a particular embodiment of this first aspect of the invention, the body may be connected to an endovascular robot or a driving system (which may comprise arm(s) and / or cassette(s)) arranged with driving elements (comprising typically motors and gearing mechanism) to drive the plurality of elongated endovascular devices in accordance with the command signal received from the body and the controls. This driving is usually operated at a proximal end of the devices, using appropriate mechanical interfaces engaged with the devices as known in the art.
[0020] As an option of the first aspect of the present invention, at least one of the controls is arranged or positioned through and / or on the body. In this way the control of movement and the selection of the elongated device(s) are provided on the same body : this user interfacing elements are therefore less cumbersome on the base, lead to easier manipulation with only a few fingers (e.g. only two) and the maintenance and replacement of the overall user interfacing necessitates only to repair of or replace this body - more simple. Moreover this arrangement which gathers different functionalities on one body (the “body”) of a user interface, adapted to be manipulated with one hand, facilitates the manipulations and selections needed for controlling elongated endovascular devices within vascular structures by involving a minimum movement of the user’s hand, and only a few fingers, while being further sure that the eyes of the user do not leave the screen or the surgical scene of such highly precise surgical procedures (endovascular field may include neurovascular). Such use of one hand, and only a few fingers of it with minimum of movements of these fingers, results indeed in a very sensitive way of manipulating the body which suits very well with the control of elongated endovascular devices (which are often flexible and very thin devices or wires) in vascular structures (which are also very thin and may be peripheral vessels or neurovascular structures) which is usually very touchy and requires high precision within narrow lumen with soft and fragile tissues : this new user interface does definitely improve user experience and the quality of the procedure.
[0021] As another option of the first aspect of the present invention, the body is arranged to be manually manipulated in at least two different directions with respect to the base to provide command signal of movement of at least one of said plurality of endovascular devices in at least two directions associated with said at least two directions of the body.
[0022] “At least two directions of an elongated device are associated with at least two directions of the body” means that these two possible directions of the body when the user manipulates it are functionally linked to the two directions of the elongated device, e.g. via controlling element(s) (e.g. via mechanical elements, circuitry, controller, processor, algorithmicand / or software components). It does not necessarily mean that a “push” of the body means a “push” of the elongated device, but a “push” of the body may result for example in a rotation of the elongated device.
[0023] As another option of the first aspect of the present invention, two directions of said at least two directions of the body are non-parallel or non-opposite to each other and / or two directions of said at least two directions of said at least one elongated device are non-parallel or non-opposite to each other. By not having only straightforward parallel or opposite directions for the body manipulation - such as e.g. up-down or left-right or clock-wise - anti -clockwise manipulation of the body - the associated directions of movements of the elongated device may be more intuitively and more clearly distinguished one to the other - particularly useful in case those device directions are also non-parallel one to the other. By not having only straightforward parallel or opposite directions for the elongated device control - such as e.g. push - pull translation or clock-wise - anti-clockwise rotation of the elongated device - allows therefore different movements from a single body which means a more intuitive and easier use by the user. In addition a controller may be added to the system and configured to link the directions of manipulation of the body to respective directions of movement of the elongated device(s), and optionally magnitude and / or speediness, of movement with a plurality of elongated devices. The controller may further be configured to operate this association based on the selection and / or deselection actuated by controls. The system may further comprise an endovascular robot whose arm(s) or cassette(s) are arranged with driving systems (comprising typically motors and gearing mechanism) to drive the plurality of endovascular devices in accordance with the control signal received from the controller or the command signal received from the body. This driving is usually operated at a proximal end of the devices, using appropriate mechanical interfaces engaged with the devices as known in the art.
[0024] As another option of the first aspect of the present invention, the base extends over a surface and two directions of said at least two directions of the body are substantially not perpendicular to this surface. By having some directions of manipulated body being not perpendicular to the surface of the base, makes the associated directions of movements of the elongated device being more intuitively driven by the user and more clearly distinguished one to the other -particularly useful in case those device directions are also non-parallel one to the other.
[0025] As another option of the first aspect of the present invention, the actuation of the controls is to select / deselect elongated device(s) to be controlled by said body. Then the selection / deselection relates to the elongated device(s) whose movement by the body controlled by the same hand of the user, which leads an even more intuitive control of the elongated devices.

[0089] DETAILED DESCRIPTION OF EMBODIMENTS
[0090] Fig. 1 shows a schematic diagram of a first embodiment of a system 100 according to the present invention. The system 100 comprises a joystick 12 and a controller 14.
[0091] The joystick 12 has a shape which is designed similarly to a car's gearstick. It is hardware that is arranged on a desktop. However, the joystick may likewise be clipped to the side of an iGT (interventional Guided Therapy) table. The joystick 12 comprises two buttons 32 and 34 on its top surface 122 which are configured to be actuated, particularly pressed, by a user. The controller 14 of the system 100 is connected to the j oystick via a cable connection 13. However, the j oystick 12 and the controller may be connected wirelessly as well.

System for controlling a plurality of elongated endovascular devices, comprising:a body to be mounted to a base and arranged to be manually manipulated with respect to the base to provide command signal of movement of at least one of said plurality of elongated endovascular devices;a plurality of controls with at least one of the controls being arranged through and / or on the body and configured to be manually actuated while the body is being manually actuated with the same hand, anda controller configured to associate the plurality of controls with a plurality of elongated endovascular devices and to select and / or deselect one or more of the elongated endovascular devices in accordance with the actuation of the controls, for controlling movement of the one or more elongated endovascular devices.
2. System according to claim 1, wherein the body is arranged to be manually manipulated in at least two different directions with respect to the base to provide command signal of movement of at least one of said plurality of endovascular devices in at least two directions associated with said at least two directions of the body.
3. System according to claim 2, wherein two directions of said at least two directions of the body are non-parallel or non-opposite to each other and / or two directions of said at least two directions of said at least one elongated device are non-parallel or non-opposite to each other.

System according to any one of the preceding claims, wherein the controller is arranged to provide a configuration allowing at least one control to select / deselect two or more elongated elements, such that they can be moved together by a single user interfacing element or by the body, synchronously at the same speed or at determinate respective speed rate(s) or speed profile.13..
